INTRODUCTION Early in the morning of July 16, 1979, there was a breach in the earthen retaining dam of a tailings pond at the United Nuclear Corporation's (UNC's) Church Rock uranium mill. The acidified liquid and tailings slurry spilled through the damaged portion of the retaining wall into an arroyo that is a tributary to the Rio Puerco river system. The Rio Puerco runs through Gallup, New Mexico, and eventually crosses the New Mexico-Arizona border (Fig. 1). On its way to Gallup, the Rio Puerco and its tributaries pass through land with a checkerboard pattern of ownership, with portions owned or leased by the Navajos, individuals, the Bureau of Land Management, and the State. In terms of tailings liquid volume (3.6 x 108L; 94 million gal), the UNC spill ranks as one of the largest. The mass of solids released in the slurry (10.0 x 105 kg; 1 100 tons) appears to be close to the median for accidents of this kind, however [U.S. Nuclear Regulatory Commission (NRC), 1979]. The UNC first opened its Church Rock uranium mill in 1977 on land adjacent to acreage belonging to the Navajo tribe. The mill, which is next to the UNC Church Rock mine, is located approximately 16 km (10 miles) northeast of Gallup, New Mexico (Fig. 1). Gallup, a town of 18 000 people, is the closest population center. The region surrounding the plant site is sparsely populated by Navajos, at a density of approximately 5.8 persons/km2 (15 persons/sq mile). The UNC mill and mines employ approximately 650 persons, and the adjacent Kerr-McGee uranium mine employs about 300. The UNC mill normally processes 3.2 x 106kg/day (3 500 tons/day) of uranium ore, depositing the acidified tailings slurry in a series of three earthen holding ponds. The tailings ponds are located east of the pipeline arroyo that feeds into the Rio Puerco approximately 2.4 km (1.5 miles) from the southernmost tailings dam. The liquid portion of the tailings slurry evaporates in the ponds; hence, under normal conditions, there is no surface flow from the holding ponds to the arroyo. Both runoff from the plant site after heavy rains and possible seepage from the tailings ponds may deliver radionuclides to the arroyo-river system, however. The dam across the southernmost tailings pond was considered to be in keeping with the state of the art. However, the New Mexico Environmental Improvement Division (NMEID) had warned UNC about dangers of locating the pond over a heterogeneous geological formation. The state Engineer's Office approved of the site only after UNC agreed to strict design criteria. Others have pointed to dangers of constructing earthen dams for impoundment of uranium mill tailings (Carter, 1978). Causes of the dam break were multiple: the UNC mill filled the tailings pond to a level that exceeded permit criteria; the tailings pond was lined improperly; the dam was constructed using clay that was compacted excessively, resulting in cracking and subsequent seepage; and the unstable substrate beneath the dam permitted differential settling. The UNC Church Rock mine has continuously released dewatering effluent into the pipeline arroyo at a rate of 88.3 L/sec (1 400 gal/min) since 1968. Before 1975 this effluent was not treated; after 1975 it received precipitation treatment for removal of Ra-226. Radionuclides are also released into the river system through the dewatering of the Kerr-McGee uranium mine 1.6 km (1.0 mile) north of the UNC mill. During usual mining operations, approximately 227 L/sec (3 600 gal/min) are released into the pipeline arroyo and subsequently into the Rio Puerco. The Kerr-McGee mine began continuous release of dewatering effluent in January 1972. In 1974 Kerr-McGee began Ra-226 precipitation treatment of its dewatering releases, but NMEID data indicate that treatment has often been incomplete. The effluent from both mines has been responsible for transforming the downstream portion of the Rio Puerco from a sporadically dry riverbed into a continuously flowing stream and has contributed to the current levels of background radiation along the river system (Table 1). This paper will summarize the postspill monitoring efforts and relate the assessment of this spill to the general question of evaluating the health effects of nuclear fuel-cycle wastes. The data pertaining to the measurement of radionuclides in the Church Rock environment and the radionuclide concentrations in animals will appear in forthcoming reports. CHURCH ROCK HEALTH EFFECTS ASSESSMENT APPROACH The initial health effects evaluation involved identifying the radionuclides that were released into the river system from the tailings pond. Table 1 lists the State of New Mexico maximum permissible radionuclide concentrations for liquids released to unrestricted areas, the typical tailings liquid concentrations, and postspill river water concentrations. The tailings liquid contained comparatively high levels of Th-230, Ra-226, Pb-210, and Po-210--all of which, according to postspill river water samples, had exceeded the state maximum permissible concentrations (MPC) at one time or another. After the radionuclides in the tailings were identified, pathways through which humans could be exposed were clarified. The latest estimates compiled by the Society of Exploration Geophysicists again indicate a moderate increase in mining geophysical activity in 1980 over the 1979 level. While North American activity remained at above the 1979 level, a considerable increase in mining geophysics was reported in South America, Australia, and the Far East. The total worldwide expenditures for mining geophysics were reported to be $53.7 million in 1980, compared to $44.4 million in 1979, and $31.6 million in 1978. During 1980, approximately 2% of all dollars spent on geophysics were attributed to mining geophysical activities; this percentage has remained relatively constant in recent years. Airborne surveys accounted for 51% of the total worldwide mining geophysical expenditure, 43% was spent for land surveys, and 6% for borehole surveys. Within the US, the breakdown of expenditures for land surveys was 60% for elec¬trical methods, 23% for gravity and magnetic methods, and 17% for seismic techniques. Electrical techniques remain the primary exploration tool for US mining geophysicists. Electrical Methods With inductive or electromagnetic (EM) techniques, significant developments were achieved in both frequency-domain and time-domain systems. Work continued on increasing the signal level on most time-domain (TEM) methods, to increase the exploration depth. The Crone group increased the power of its Pulse ElectroMagnetic system (PEM) to a 20-amp transmitter-loop capability. The GEOEX group is modifying the SIROTEM II system to obtain larger transmitter amperage from a portable motor generator. Geonics developed a new digital recording system (data logger) for its EM-37 system. This development should increase the productivity of EM-37 crews. A new ground, frequency-domain EM system was developed by the Scintrex group. This novel Genie system does not require a wire link between the receiver and transmitter. Because an amplitude-ratio is measured, the Genie data are reported to be relatively insensitive to coil orientation and distance errors. This new technique does not need extensive line-cutting or accurate station-chaining and would appear to be a good reconnaissance instrument. Scintrex also began marketing the new IPR-11 induced polarization spectral receiver. This receiver is microprocessor controlled, and can output to a cassette tape and record 10 windows of secondary voltage decay simultaneously from up to six receiver dipoles. The Phoenix group's new 100 kW induced polarization/resistivity (IP/R) transmitter began tests using their IPV-3 multifrequency, multichannel receiver. While this unit was primarily developed for "oilfield" IP exploration research, it has obvious application to "deep" mineral exploration. The Phoenix group also developed a new remote-reference, real-time magneto-telluric (MT) device in 1981. This five-component MT sys¬tem has a frequency range from 0.0005-384 Hz. Helicopter-borne electromagnetic (HEM) developments also continued in 1981. The mining in¬dustry increased its use of the new Geonics EM-33-3 multifrequency, multicoil instrument. In 1981, the Dighem group developed software for estimating magnetite as a mapping parameter from its HEM system. Dighem's work is said to complement airborne magnetic intensity surveys, since the HEM estimate is independent of remanent magnetism and magnetic latitude effects. Gravity and Magnetic Methods Probably one of the most innovative techniques in geophysics in 1981 was the use of airborne gravity surveys for both mining and petroleum exploration. The Carson group is using a modified, shipborne LaCoste-Romberg platform in helicopters. Data accuracies to 0.5 milligal have been achieved by flying gridded surveys. Although this airborne method is expensive-up to $186/ km ($300/line-mile)-the geophysical community has been excited by initial results. On the ground, the portable proton-precession magnetometers are becoming sophisticated. Both GeoMetrics and EDA recently introduced field magnetometers having data storage and processing capabilities. This development should greatly increase the productivity of ground-magnetic surveys. Seismic Methods Development of high-resolution seismic techniques continued in 1981. These techniques have primarily been directed toward coal studies for fault detection. OYO Instruments introduced their McSEIS-1500 seismic data acquisition system in 1981. This device contains a 24-channel recording capability, with digital output to 256-kbyte floppy disks. The high-speed data transfer using the disk media is considered a desirable feature. Borehole Methods The general decrease in uranium exploration, where borehole logging is extensively used, probbly led to the overall decline of geophysical logging activity in the minerals industry. However, a number of new sondes and logging systems were introduced in 1981: • Mount Sopris recently introduced their Series III logging system. This microprocessor-controlled unit records up to four channels of data on nine-track or cassette magnetic tape. The logging package is relatively light-weight, so helicopter transport to mountainous or roadless exploration sites is possible. (Both the Edcon and Woodware-Clyde consulting groups offer "slinging" capabilities for their Mount Sopris units.) Mount Sopris is continuing work on their 500-mm-diam (2¬in-diam) spectral gamma-ray sonde. This tool is expected to be available soon. • Owl Technical "slim-downed" its successful digital deviation probe to 380-mm (1.5-in) outside diameter. This new sonde will also measure inclinations up to 80° from the vertical, as compared with its older instrument that could measure inclinations to 30°. • A magnetic susceptibility sonde was introduced in the US by the OYO Instruments group. This Kappalog sonde contains two aircored coils for measurements slightly affected by thermal changes within the borehole. 24.1-BASIC PRINCIPLES JAMES R. TAYLOR The basic principle involved in maintenance is protection of a company's investment through the care and upkeep of machinery and equipment, buildings and properties. Maintenance is justified through making available to the maximum extent possible the facilities, machines and services required by the operating and production departments in their function of insuring optimum return on investment. As the size, cost and complexity of the equipment used in modern mining and related industries increase, operating and production personnel become proportionately more dependent upon maintenance engineering for its contribution to the overall profit picture. A greater part of the total cost now is charged to maintenance and the maintenance group becomes a major company unit. Basic maintenance concepts have changed very little over the years, but the advances in methods and technique have brought about tremendous challenge and have in consequence assumed a greater influence and importance. The older "sledge- hammer" methods have given way to engineered and scientific systems, while the accelerating development of intricate equipment and machinery, and the instrumentation essential for control, demand an ever-increasing upgrading of maintenance personnel. Regardless of this growth in importance, cost and complexity of the maintenance function, it is important to remember that it is not an independent and self-sufficient unit, but is a necessary facet of the entire plant operation and is subject to the company's total production effort. As one part of a team, maintenance is successful only when functioning cooperatively wit11 the other departments it serves Close collaboration in all areas served by n maintenance group is essential if it is to function fully to protect the company's investment, whether the assets are in machinery, materials or people. 24.1.1-MAINTENANCE SCOPE Maintenance functions may be grouped into the two general classifications of primary and secondary. This grouping constitutes a basic division although, in practice, the scope of the maintenance activity varies in each mine or plant and is subject to differences in size and type of operations and company policy, plus the influence of precedent, which may be sectional or industry-wide. The primary functions supply the justification for the maintenance engineering department. while secondary functions are variable according to plant conditions and needs. The primary functions may be categorized as follows: 1. Maintenance of existing machinery and equipment. 2. Maintenance of existing buildings and grounds. 3. Inspection and lubrication of equipment and machinery. 4. Utilities generation and distribution. 5. Alterations to existing equipment and facilities. 6. Installation of new equipment and facilities. 7. Instrumentation.

"As mine operators continuously look for ways to improve functionality and increase capacity of their facilities, existing process equipment is often pushed to and past its designed limitations. As a result, equipment manufacturers are asked for economical solutions to meet redefined processing needs. Newmont USA (Newmont) in Nevada was faced with such a need in their Carbon-in-Leach (CIL) train. In a collaborative effort between Newmont and Derrick Corporation, tests were conceived to assess the viability of drop in replacement screen using a three dimensional screen panel. The urethane pyramid screen panel was designed to increase flux while maintaining the existing equipment footprint. The test is the end result of an ongoing research and development project that Derrick undertook in an effort to refine and upgrade their urethane screening media. The application of this new screening media is presented for this CIL application along with future plans to implement the technology in trash, safety, carbon dewatering, carbon sizing, and other scalping applications.INTRODUCTION In Spring of 2014, Process Machinery Associates (PMA), Derrick Corporation’s (Derrick) local area representative, met with Newmont USA (Newmont) to discuss the operation of the in tank screens in the carbon in leach (CIL) train at Twin Creeks, near Golconda, Nevada. The process was originally designed for 13,000 +tons per day (tpd) but has been steadily rising since the plant was commissioned and now Newmont requires 16,000 tpd to be processed. The original screens were running at their maximum capacity and Newmont required additional screening capacity to continue the upward trend in production. Physical separation by use of screening is one of mineral processing oldest methods. Screening media can be prone to blinding and pegging and the necessary support structures required for finer apertures result in smaller open areas. Therefore, more screening area generally requires larger panels and therefore larger machines, screening media that doesn’t blind, or a combination of both. In an effort to avoid larger machines, Derrick Corporation had been working on a revolutionary three dimensional panel which would add non blinding screen area in the same footprint of an existing machine. Alpha testing of the new screen panels began at Newmont Mill 2/5 (Carlin, Nevada) in July of 2013 and was still occurring when PMA approached Twin Creeks about beta testing a refined machine and pyramid panel. After presenting the initial results, Twin Creeks determined that an upgraded machine and pyramid panels would be a possible solution to meet production needs. The testing of a machine eliminated the risk of capital costs to Newmont and provided Derrick the opportunity to obtain additional data."

"A cationic surfactant, tetradecyl tributyl phosphonium chloride (TTPC), was first used as a collector. Flotation characteristics of muscovite using TTPC have been investigated through flotation tests. And dodecyl trimethyl ammonium chloride (DTAC) was tested for comparison. Their adsorption mechanisms on muscovite surface were clarified using zeta potential measurement, DFT calculation and molecular dynamics (MD). The flotation results indicate that the recovery of muscovite decreases when the pH increases in the presence of surfactants. When a maximum of recovery was reached, the dosage of TTPC was 2×10-5mol/L which was half of that of DTAC. Zeta potential measurements confirm that the TTPC displays a stronger collecting power than DTAC and is a superior collector for the muscovite flotation. DFT calculation and MD simulations show that TTPC interacts with the muscovite mainly through electrostatic attraction and hydrogen-bonding. The conclusions drawn from purely theoretical computations matched remarkably well with the experimental results. Keywords: Quaternary phosphonium salts; Flotation collectors; Flotation INTRODUCTION Muscovite mica is widespread in igneous, sedimentary and metamorphic rocks. It has the general composition KAl2(AlSi3O10)(OH)2. Muscovite mica exhibits perfect cleavage along the (001) plane, which exposes interlayer potassium ions[1-3]. In aqueous suspensions, the potassium ions readily enter into solution, leading to the development of a net negative charge on the (001) plane[4-6]. The surface of the muscovite mica is negatively charged at all pH > 1[7]. Due to the negative charge surface over the whole pH range of 2–12, the muscovite mica would not be expected to respond to anionic collectors in the absence of an activator. Conversely, the mineral is readily recovered using cationic collectors. However, until now, various amines and their derivatives (with N atoms as the center) are still the only available cationic collectors which still had their shortcomings such as weak selectivity, low flotation speed. Since few types of cationic collectors could be used, in some sense, the limitations of collectors are increasing significantly and the cationic flotation is facing more and more challenges. Quaternary phosphonium salts, as novel cationic surfactants, are widely used in antimicrobial, catalysis and surface modification field due to their biological activities, hydrogen bonding capability, stability and superior group electrical property[8-10]. There are a number of processes where quaternary phosphonium salt surfactants have shown better properties than quaternary ammonium salts and amines. Although the adsorption of quaternary phosphonium salt surfactants at the bacterium–liquid interface has been studied thoroughly, the case of adsorption in mineral surface has not been investigated thus far."

"During the mid to late 1960’s, the underground potash industry in Saskatchewan was born. For the next four decades the industry operated at a steady pace supporting a large part of the world demand; until 2008 when the potash market heightened. Operators were motivated to capitalize on this increased demand, new mines were proposed while existing mines were planning upgrades. Potash Corporation of Saskatchewan (PCS) was one of the major producers planning upgrades to their production facilities quickly, including modifying the existing headframe and shaft at the PCS Cory Site to accommodate a new 1500 tonne per hour hoisting plant. Upon completion of the project, the innovative operations group continuously reviewed methods to increase production and reduce operating costs, allowing PCS to continue being competitive on the world market. This paper will describe the design challenges encountered during the development of the infrastructure around an operating facility whilst minimizing down time, schedule, capital and construction costs. This paper will also explore the strategy to increase capacity of the newly commissioned hoisting plant. BACKGROUND Potash production is a major Saskatchewan industry, and has played a significant role in the economy for over 40 years. Production commenced in 1962 when water problems were overcome at the International Minerals and Chemical (IMC) mine near Esterhazy, Saskatchewan. A surge of development by other companies resulted in twelve companies opening ten mines by 1970. In 1975 Potash Corporation of Saskatchewan was created by the government of Saskatchewan and shortly thereafter purchased several mines in Saskatchewan, including the Cory Mine (Cory), near Saskatoon. Cory has been in operation since 1968 and is located 12 km (7.5 mi) west of Saskatoon in central Saskatchewan. The mine is located in the center of the world’s largest potash deposit, 1,000 m (3300 feet) below surface. PCS has embarked on sizable capital expansions of more than CDN $7 billion. The Cory CDN $1.6 billion expansion was expected to raise operational capability of the site to 2.7 million tonnes per year, an increase of 1.9 million tonnes per year (MTPY). To achieve 2.7 MTPY of finished product and considering the 22.6% average production recovery1, 12 MTPY of ore must be hoisted to surface for processing. To deliver this rate required upgrading of the hoisting plant to 1500 tonne per hour (TPH). To accomplish this ambitious challenge, creative thinking, careful planning and a cohesive team was needed to work together to ensure the schedule, budget and most importantly the work was completed safely"

"Rare earth elements (REEs) found in coal are in the form of minerals, ion-substitution with clays and organically bound. Rare earth minerals (REMs) such as monazite exist in coal and have a grain size less than 5 microns. Froth flotation was successful in concentrating REMs existing in a thickener underflow material derived from Fire Clay seam coal which contained around 300 ppm of rare earth elements (REEs). Conditioning with fatty acid followed by processing using multiple stages of conventional flotation produced a final concentrate containing 2300 ppm REEs. Using a laboratory flotation column, a concentrate containing around 4700 ppm of total REEs was produced equating to an enrichment ratio of 10:1. INTRODUCTION Rare earth elements (REEs) are becoming increasingly important and have been widely used as raw materials for the production of phosphor, metal catalysts, magnets, and batteries [1, 2]. REE recovery from alternative sources such as coal and coal by-products has the potential to stabilize the economic viability of coal mining operations while providing a dependable supply of critical materials. The average REE content of the world coal is around 60-70 ppm [2]. There are many well-known coal beds with high contents of REEs such as those in the Far East coalfields in Russia (300-1000 ppm), the Fire Clay coal seam in eastern Kentucky (500 ppm), and the Sydney Basin in Nova Scotia, Canada (72-483 ppm) [3-6]. The occurrence of REEs in coal and coal by-products can be classified into three types, i.e., rare earth minerals such as monazite and xenotime, ion exchangeable REEs in clays, and REEs associated with organic matrix. The particle size of the rare earth (RE) minerals in coal and coal by-products is very small and normally in the micron and/or nanometer scales. The flotation technique, which is based on the surface chemistry difference, has been widely used to treat fine rare earth industrial minerals such as monazite, xenotime, and bastnaesite [7-11]. As such, flotation is a promising method for the concentration of REEs from coal and coal by-products. In the current study, rare earth flotation was conducted using the fine coal refuse generated at an operating coal preparation plant treating coal from the Fire Clay seam. REE concentration by flotation required an appropriate amount of grinding and control of the pH value while using typical collectors. Prior to RE concentration, release tests were conducted to evaluate the floatability of the recoverable coal and the REE distribution in the flotation fractions in the fine refuse. Mineralogy of the REE was studied using SEM-EDX."

"INTRODUCTION An inherent aspect of any tunnelling project is ground risk: how it is identified, quantified and managed throughout the various stages of a project. Early identification of these risks, such as adverse ground conditions or potential obstructions is crucial; however specific identification of all risks will inevitably be limited by the extent of the desk study, ground investigation and accuracy of as-built information available, particularly in a densely populated urban environment such as London with a long and varied history of development. It is therefore crucial to have suitable plans and procedures in place for dealing with potential ground risk scenarios if they occur during construction work to ensure the safety of the workforce and to protect nearby critical assets. A number of experiences of potential ground risk were encountered during the Sprayed Concrete Lining (SCL) works on Crossrail contract C300/410 which was constructed by a joint venture of BAM Ferrovial Kier (BFK). The C300/410 works, undertaken in the heart of London, comprised of over 2.5km of SCL tunnels for Bond Street and Tottenham Court Road Stations and the Fisher Street crossover, as well as two 6.2km TBM tunnel drives between Royal Oak and Farringdon. BFK appointed Donaldson Associates as their SCL designers for the works; together they were responsible for excavation stability up to the point of ring closure. Despite the majority of SCL tunnelling works at Bond Street and Tottenham Court Road being through the favourable medium of London clay, there remained areas of local geotechnical challenges. Some of these were expected from the outset, some became apparent as works progressed from records of other nearby excavation works, and some were only encountered during tunnel excavation. Being located in the centre of London meant that there always remained a possibility of meeting some form of obstruction from previous structures. However, identification of all potential issues prior to the commencement of the works was challenging and proved to not be entirely possible. Through a series of case studies, this paper documents incidents where obstructions or adverse ground conditions arose. These scenarios all had the potential to cause safety risks, impact the tunnel design and/or construction methods and ultimately add time and cost to the works. The paper discusses how these challenges were overcome which utilised in part, a progressive risk reduction cycle, feeding back knowledge gained at all stages of construction into the approach for undertaking the works. It also examines what could be done in future to further reduce the ground risk through early identification, communication, education and appropriate contingency measures."

INTRODUCTION The continuous mining machines which were introduced in the 1950's now account for more than half the production of coal from underground mines. Unfortunately, these continuous miners which were designed for increased productivity have also increased the concentration of respirable dust in mines. Inhalation of this respirable dust by miners causes the Coal Worker's Pneumoconiosis (CWP). Concern for CWP increased in the United States following a 1963-65 survey that showed definite x-ray evidence of CWP in 9.8 percent of working miners and in 18.2 percent of nonworking miners in the Appalachian bituminous coal region (Breslin and Niewiadomski, 1982). One of the major objectives of the Federal Coal Mine Health and Safety Act of 1969 was the prevention of CUP. This law initially limited the daily average exposure of coal miners to 3 mg/m3 of respirable dust. However, following the results of a probabilistic model developed by Jacobsen, et. al. in 1971, the limit was reduced to a mass concentration of 2 mg/m3 unless quartz content exceeds 5 percent, a rare occurrence in mines. Based on x-ray categorization prior to death it is believed that the mass concentration in the respirable fraction is a parameter of primary interest for hygenic evaluation of dust exposure. Elorgan, Burgess and Jacobson (1973) suggested that the quantity of inhaled dust, although influential is not the only aspect of exposure which is significant in producing CWP. Miners working in higher rank coal (anthracite) mines are more susceptible to CWP (Morgan, Burgess and Jacobson, 1973). This also confirms the findings of the British (Bennett, Dick and Kaplan, 1979 and Hurley, et. al., 1979) and Germany (Reisner and Rubock, 1977). Based on the cy to toxicological evidence Reisner and Rubock (1979) suggested that when coal properties are similar, increasing quartz and mineral content increases toxicity. David, Ottery and Le ?oux (1975) examined that as the severity of the CWP increased the retention of the coal dust content in the pulmonary region remained the same, while that of quartz content increased significantly . Leiteritz, Baur and Bruckman (1971) studied the various components of coal dust and showed that coal particles comprised a smaller fraction of the mass of fine dust than did particles , of quartz and minerals. "INTRODUCTION The Jefferson Barracks Tunnel is in the final stages of design and is expected to go out to bid in the summer of 2016. The project is located in south St. Louis County along the west bank of the Mississippi River, about 13 km downstream of the St. Louis Gateway Arch. The project consists of a deep sanitary sewer tunnel, a deep pump station and several intakes. The intakes will divert sanitary flows into the tunnel and the tunnel will convey the flows by gravity to the Lemay No. 3 Pump Station, which will pump them up into the Lemay Waste Water Treatment Plant. The tunnel is about 5.4 km long and will be lined with 84-inch (2.1 m) fiberglass pipe. It drops at a gradient of 0.1 percent from south to north and will be bored through strong limestone using a hard-rock tunnel boring machine (TBM). The diameter of the TBM is anticipated to be about 3.4 m. The Owner’s original concept was for a relatively shallow tunnel so as to minimize construction and pumping costs. The preferred alignment ran close to the river, next to the existing force main, which was to be abandoned. Keeping the tunnel close to the force main would reduce the amount of shallow sewer work needed to connect the existing feeder sewers to the new tunnel. Several geotechnical investigations were performed, with shallowly focused geophysics and the borings all going exactly down the conceptual tunnel depth. Limestone bluffs run along the Missouri side of the river. These bluffs are pockmarked by sinkholes and riddled with karst. Large, well-known caves occur both north and south of the alignment. Karst features, including deep crevices and possible caves, were found in those early geotechnical investigations. Many of these features extended well below the level of the Mississippi River and would have been encountered by the tunnel. If a large crevice or cave were encountered, then the inflow of mud and water into the tunnel could have been large and possibly uncontrollable. Something had to be done to mitigate this situation. Pre-excavation grouting was considered, but the program would have been huge and extremely expensive. Using a closed-face TBM with bolted and gasketed lining was also considered, but that too was both risky and extremely expensive. The Owner could have abandoned the tunnel altogether and built a new force main, but that would have brought a whole new set of problems and was undesirable to the Owner. The last option was to move the tunnel away from the karst."

"The Eglinton-Crosstown Light Rail Transit line is a 19-kilometre corridor with a 10-kilometre underground portion currently being constructed by Metrolinx in Toronto through a densely populated urban environment. Tunneling works are being completed through two separate design-bid-build contracts that include consideration for a phased handover to a follow on contractor who will take ownership of the tunnels, including warranties, and will complete the stations and remainder of the system through a PPP delivery model including maintenance of the system for 30 years. Warranty provisions, settlement apportionment, tunnel leakage, potential delays and conflicts between contractors are just some of the risks the team managing tunnel contract integration had to consider when developing the PPP contract. This paper will present an overview of how these risks were assessed and allocated through the contracts and the strategies adopted to minimize risks to Metrolinx while ensuring successful procurement was maintained. INTRODUCTION The Big Move is a 25 year, $50 billion ($-CAD) plan for coordinated and integrated transportation in the Greater Toronto and Hamilton Area (GTHA) in Ontario, Canada. This plan has been initiated by Metrolinx, a division of the provincial government of Ontario, and its vision, goals and objectives are rooted in creating a high quality of life; a thriving, sustainable and protected environment; and a strong, prosperous and competitive economy for the GTHA. The Big Move will triple the length of rapid transit service in the GTHA to 1,725km, significantly increase the percentage of the GTHA population living within two kilometers of rapid transit, reduce commute times to an average of 77 minutes per person per day and decrease greenhouse gas emissions from passenger transportation per person by 29%. Work is currently underway on several projects and more than $16 billion ($-CAD) of the planned investment has already been committed. A key project that forms part of this commitment is the Eglinton Crosstown Light Rail Transit (LRT) Project which is being delivered in the City of Toronto under Metrolinx’s Rapid Transit (RT) Program. This complex and sizeable undertaking has been broken into a number of separate contracts comprising: LRT Vehicles contract; Eglinton West Tunnels Project; Eglinton East Tunnels Project; and a Public-Private-Partnership (PPP) contract to complete the underground stations, at-grade stops, all rail infrastructure and systems and to maintain the entire system for 30 years. The PPP contract, which includes an initial capital portion of approx. $5.1 billion ($-CAD), has recently been awarded to Crosslinx Transit Solutions, a Special Purpose Vehicle (SPV) consortium comprised of SNC Lavalin, Aecon, Dragados and EllisDon."

A longwall coal mine in southwestern Virginia mining the Pocahontas #3 Coal Seam experienced a history of mining-related seismic events. The events were never large enough to cause concern until three major events occurred recording 3.4, 4.3, and 3.4 ML in 2005, 2006, and 2007 respectively. Two of the seismic events generated an air blast which damaged stoppings which fueled a mine fire in 2005 and 2007. The mine responded to the events by narrowing the longwall widths, changing ventilation controls, installing a 7-sensor seismic system around the active panel, and creating a mining hazard map to avoid similar geology in the future. All of the actions taken have prevented seismic activity from impacting mining operations, but it did not prevent other large seismic events from occurring (Van Dyke et al., 2018). In 2016, another large seismic event, measured at 3.7 ML, occurred in the gob that was reported by the local media and recorded by the United States Geological Survey (USGS). While the event did not directly impact mining operations, mine management wanted to know where and under what conditions could large seismic events occur in the future. The National Institute for Occupational Safety and Health (NIOSH) became involved to help understand what conditions precipitated these events so that future mine design could more accurately assess seismic hazards and reduce the risk to mine worker safety. Researchers at NIOSH and mine management created a seismic forecasting map that combined three factors: (1) overburden thickness, (2) sandstone thickness, and (3) the expected caving height of the gob. The criteria were developed based on the geologic data the mine collected from core drilling, e-log analysis, and fiberscope observations. The forecast map was able to provide an accuracy of 72%–85% of events 1.5 ML or greater and 100% accuracy of events that were 3.0 ML or greater based on historical data. The seismic forecasting map was applied to the entire future mining area to show areas where seismic potential could be the greatest based on current mine design planning, ultimately improving the ability of the mine operator to assess the seismic hazards surrounding active and future mining areas (Van Dyke et al. 2018). Following this research, the seismic forecasting map was implemented into the mine’s hazard recognition program and is continuously updated and evaluated prior to mining each longwall panel. This paper will cover in more detail how the seismic and lithologic data was processed and how the mine implemented a new seismic mitigation program based on the forecasting results.

"INTRODUCTION While the United States coal mining uses prescriptive regulation under the Title 30 Code of Federal Regulation enforced by the Mine Safety and Health Administration (MSHA), Australia uses a risk based approach. Australian regulation is individual to each state. In Queensland, Australia this risk based approach to address principal hazards is underwritten by the Queensland Coal Mining Safety and Health Act of 1999 and the Coal Mining Safety and Health Regulation of 2001. Each Queensland Mine is required to have an overarching Health Safety and Environment Management System, of which Safety Management Plans are an element. “Safety Management plans (SMP’s) are intended to formalise the process by which mines address principal hazards and other related matters to ensure the safety of mine personnel.”7 THE QUEENSLAND CONTEXT In Queensland, the basic elements of a coal mine’s safety and health management system must provide for risk identification and assessment, hazard analysis, hazard management and control, and the reporting and recording of relevant safety and health information and data.5 (Part 2 Division 1 Section 6(a-d)) The initial guidance for writing Safety Management Plans comes from the Approved Standard for Mine Safety Management Plans and Guidance Note QGN09- Reviewing the Effectiveness of Safety and Health Management Systems. The Guidance Note and Approved Standard are supplemental to the Act and Regulation, and put in place to provide more clarity around the topic. The definitions in this section come from the Approved Standard and are included to provide context for those who may not be familiar with Australian or Queensland law and regulation. A hazard is defined as a source of potential harm or a situation with a potential to cause loss, while a principal hazard is a hazard with a potential to cause multiple fatalities. A risk is the chance of something happening that will have an impact upon objectives which is measured in terms of consequences and likelihoods. It is important to be able to make the distinction between a hazard and a risk. For instance, a lion is a hazard, while the risk associated with a lion is getting eaten. As defined by the Queensland Approved Standard for Mine Safety Management Plans, a Safety Management Plan (SMP) requires a systematic definition of all the actions necessary to ensure that mining operations are carried out safely. A SMP includes, but is not limited to organisational structures, planning, activities, responsibilities, practices, risk identification, audits and reviews.7 The goal of a SMP management plans is to mitigate the risks to bring them to an As Low As Reasonably Achievable (ALARA) level."

INTRODUCTION A current need of the phosphate industry is the development of process technology for treatment of high carbonate content ore. About 75 to 80 percent of the world phosphate reserves (1) are associated with carbonate gangue, which is not amenable to treatment by the existing techniques. Numerous laboratory and pilot plant studies have been devoted to carbonate-phosphate separation. Different techniques were involved (washing, sieving, attrition, flotation, calcination, and leaching). For technical and economical considerations, flotation appears to be an efficient process for this problem. Separation of carbonates (calcite and dolomite) from igneous phosphate is applied in industrial plants in Brazil, Finland, and South Africa. But, for sedimentary phosphate, no industrial realization exists at this time, with the possible exception of some plants in China. The main problems with sedimentary phosphates are: • high substitution by CO3-2 and F- for P04 -3 in the apatite lattice. • larger surface area of the phosphate particles because their surface is porous and irregular. • tendency of sedimentary minerals to produce more slimes in conditioning than the crystalline minerals, hence, larger dosages of collectors are required for sedimentary minerals. Investigations reported in the literature indicate that most processes developed for sedimentary carbonate-phosphate separation involved inverse flotation of carbonates using fatty acids as collectors (2), (3) associated with phosphoric acid (4), monosodium phosphate (5), F- (6), or SiF6 -2 (7) as phosphate depressants. Later investigations, conducted by A. Henchiri and G. Baudet (8), (9), (10), (11), (12), (13) in the BRGM technology department, of phosphoric esters revealed that these reagents are selective collectors for carbonate-phosphate separation. The above investigators developed a process which used phosphoric esters to collect carbonate minerals and sulfuric acid or sodium fluorosilicate as depressants for phosphate. Extensive comparative tests for different processes on the same phosphate sample show that the process developed at BRGM is one of the most promising methods proposed for sedimentary phosphate-carbonate separation. It should be pointed out, however, that phosphoric esters make abundant froths. The handling of these excessive froths will be a problem in an industrial-scale plant. This paper outlines test work performed in order to optimize and to improve the efficiency of the flotation process on a sedimentary phosphate ore selected from the Sra Ouertane orebody. The Sra Ouertane flotation process, illustrated on Figure 1, first removes carbonate minerals by inverse flotation and then recovers phosphate as a froth product, leaving quartz as a cell product.

INTRODUCTION Certain activities, such as emergency work and rescue operations, often have to be undertaken in environments where heat loads often exceed limits prescribed for routine work. This is particularly relevant to the South African mining industry where heat stress-induced fatalities are not uncommon during rescue operations. In this regard an excessively hot environment is defined as one where either the dry-bulb temperature exceeds 37,0°C or the wet-bulb temperature exceeds 32,5"C, and it is therefore not surprising that heat stroke mortality escalates rapidly when these limits are exceeded. A review of both local and international standards indicated that the available information on safe exposure times under abnormal heat stress conditions is very limited. Where available, such standards are aimed at the general population, including both sexes, irrespective of age and also the least heat tolerant worker, and not at a medically-screened, all-male population free of grossly heat intolerant individuals, as is the case in the South African mining industry. It is suggested that available norms and limits are too conservative for local application and, therefore, unrealistic. In view of the definite need to protect workers when exposed to abnormally high environmental temperatures, the present study was undertaken to establish a scientifically derived data base. applicable to local conditions, to determine safe exposure times: Any exposure limit, especially in an emergency situation, should be easy to administer, valid and reliable. These aspects, there- fore, form an integral part of this paper. FORMULATION OF HEAT STRESS LIMITS The formulation of heat stress limits was based on the criterion that a heat exposed individual should experience a negligible risk of developing dangerously elevated body temperatures. In order to provide a database for risk assessment, certain physiological responses of heat tolerant men were determined at various combinations of thermal conditions and work rate in a climatic chamber. These physiological responses were analysed statistically in order to predict safe survival and tolerance times as a function of thermal conditions and work rate. EXPERIMENTAL DESIGN A total of 202 randomly selected, medically fit mine workers volunteered for the study on the basis of informed consent. These men were considered to be inherently heat tolerant in as far as they had successfully completed a heat tolerance test (Anon., 1991) or the standard climatic room acclimatisation procedure used on many South African gold mines (Anon., 1989). INTRODUCTION Today, flotation is clearly the dominant mineral concentration method. Since it is a relatively selective process, it is used for almost all sulfide minerals. A recent U. S. Bureau of Mines study indicated that approximately 225 million metric tons of sulfide ore was processed in 67 plants in the United States in 1980 (USBM Staff, 1981). There were over 100 plants processing copper ores in the world (Bassarear, 1985). Sulfide ores containing lead, zinc, copper, molybdenum, precious metals, cobalt, nickel, and iron are generally separated and concentrated by flotation. This chapter briefly reviews the plant practice related to the recovery of the above-mentioned sulfide minerals with emphasis on chemical reagents used for the production of marketable-grade products. PROCESSING OF SULFIDE ORES It is beyond the scope of this chapter to review the milling data for operating plants which have been extensively covered in literature (Tveter and McQuiston, 1962; Rausch and Mariacher, 1970; Weiss, 1985; Crozier, 1979). However, the following generalizations can be made regarding the processing of sulfide ores: 1. The primary grind is targeted to produce as coarse a product as necessary to provide an economic degree of liberation. 2. The concentration process, generally flotation, is designed to produce a concentrate consisting predominantly of valuable minerals and a middling fraction which may require further grinding to produce an optimum release of minerals. 3. The chemical reagent suite (collectors, frothers, etc.) for the separation of sulfide minerals from gangue minerals is generally complex. Total reagents used in the flotation process can be as high as six to eight, as is the case for lead and molybdenite separation from complex sulfide ores. 4. Chemical reagents are generally expensive because of the high selectivity for sulfide minerals. The process flowsheet is dependent on several factors, including ore grade and mineralogical composition. Generic process flowsheets for processing different sulfide ores have been developed for discussion in this chapter. However, there appears to be a consistent relationship between the reagent combination choice and the mineralogical composition of the ore (Tveter and McQuiston, 1962). These are covered in detail in the following sections. LEAD AND ZINC Galena (PbS) and sphalerite (ZnS), which commonly occur together, are the only minerals of lead and zinc, respectively, in most primary deposits. Feed ore grades are typically 1 to 5 percent Pb and 1 to 10 percent Zn. Very often economic quantities of copper and precious metals, silver and gold, also occur with these base metals.

1.1. INTRODUCTION Despite indications of a general economic recovery in the U.S. during the mid-19801s, mine operators have not generally shared in the benefits of these reported better times. Budgets have not rebounded, borrowing is tougher, parent companies' pockets are not as deep as in previous years, and margins are not any better -- if there is any left over at all. Managing capital equipment budgets is always a challenge, and the last five years of stretched equipment lives make these decisions more tenuous. Delays and avoidance of replacements due to tough times inevitably makes those decisions increasingly difficult and usually more expensive The purposes of this paper are to present. a brief refresher outline of the fundamentals of equipment replacement analysis, to suggest a number of papers where analysis methods are explained in greater detail. and to recommend the use of a widespread microcomputer software capability (1-2-3) as the easiest path to a much-needed solution for a very difficult, and usually under-supported management decision problem 2. REPLACEMENT DECISION POLICY Investments in equipment replacements must compete for capital with all other investment alternatives. Capital for replacements of ten represents a greater commitment than do the new projects which a company might handle in a given year. But. sadly, formal policies for standardized analysis of replacements rarely exist, even in major companies. Yet this policy can be very significant in determining the competitiveness of a firm. Whereas many replacement decisions are based on cost minimization, economists and financial analysts agree that wealth maximization or return on investment is the superior criterion. Though the difference may seem slight, agreement on a firm's financial objective is vital. Replacement policy should be straight- forward. have a logical basis, and a proper level of sophistication. There are several reasons to develop a policy for replacement decision-making: [I] 1 - equipment's optimal economic life is nearly always shorter than its physical life; 2 - mine life often exceeds equipment life, so management must face these decisions routinly 3 - replacement capital costs, on an undiscounted basis, can run 25 to 75% of original capital 4 - technological changes may result in a newer machine or system which can operate more efficiently, more safely. or less expensively 5 - estimates should be made consistently in the fundamental areas of life expectancy, availability, utilization. price escalation, taxes 6 - long-term and short-term budgeting without decent data and analytical capability is nearly impossible, or at least ineffective Policy development assumptions which must be considered include: the planning horizon, cost patterns, interest rates, availability of capital, replacement vs. expansion, technology advances.

INTRODUCTION Las Vegas has recently been termed the “Construction Capital of the Country”. It has also been acknowledged as the fastest growing city in the country for several years running. The rapid growth improves the economic picture of the region but it also imparts unusual demands on the area’s infrastructure. In 1994 the Southern Nevada Water Authority (SNWA)was working on a master plan to address the rapid growth. A long range analysis revealed the need for additional water treatment capacity and delivery systems. Short term projections showed that the current system would reach capacity in the summer of 1997 with the possibility of demand exceeding the supply. The River Mountains Tunnel #2 is part of a $150M scheme to quickly upgrade the system to meet the short term demand, improve the overall system operation and allow for a smooth transition into the larger additions being planned for the future. The SNWA selected Carollo/Black & Veatch (CBV) to provide detailed design and construction management for the upgrades. The River Mountains Contracts consisted of five separate contracts broken out separately to stream line a very aggressive (just 34 months) design and construction schedule. Necessary work included four miles of excavated and lined tunnel, connections from existing pipelines to and from the new tunnel including large diameter (to 3.66m(144 in)) valves, a 18 927cu m (5 million gal) regulating reservoir, a surge tank and a 170 343cu m (45 million gal) reservoir to add additional storage to the system. Notice to Proceed was given to Kajima Engineering and Construction (Kajima) on May22, 1995 for the construction of the River Mountains Tunnel #2. The design called for a tunnel to be driven 6070m (19 915ft) in rock and lined to a finished diameter of 3.71m (12.17ft).Excavation began on December 14, 1995 and holed through on April 30, 1996. Lining began on June 25, 1996 and a notice of Substantial Completion was issued on December 13,1996. This paper describes the measures taken to complete the tunnel in 18 months from Notice to Proceed to Substantial Completion. THE RIVER MOUNTAINS SITE The River Mountains are a low range of rugged mountains separating the Las Vegas Valley from its main supply of water, the Colorado River at Lake Mead. They are volcanic in origin and the harsh desert environment has left them as expanses of exposed bedrock rising 305m (1000ft) above the valley with little or no vegetation. An existing 3.05m (10ft)

Staurolite, an iron aluminum silicate mineral, is used industrially as a high value-in-use sandblasting agent, as a premium-grade foundry sand, and as the source of aluminum in portland cement manufacture in areas where the aluminum constituent is not economically available from shale or argillaceous limestone. Staurolite is produced by E. I. duPont de Nemours and Company as a coproduct of titanium mineral production in Florida, where it was first recovered on a commercial scale in 1952 from the Pleistocene sand deposit known as Trail Ridge. At this deposit it comprises about one-fifth of the heavy mineral suite, the principal other heavy minerals being ilmenite, leucoxene, rutile, zircon, kyanite, sillimanite, and tourmaline. Staurolite and other nonconductive silicate heavy minerals are separated from the conductive titanium minerals on high-tension electrostatic separators. The silicates are then separated from one another magnetically, the staurolite being relatively magnetic. In the total process, 200 t of raw sand yield 7 t of heavy mineral concentrate from which about 1 t of staurolite is recovered. The staurolite concentrates have the characteristics given in Table 1. Staurolite concentrates are sold in three grades having the size distribution of subrounded grains shown in Table 2. All three grades are used in sandblasting for cleaning metal, for paint removal, and for cleaning masonry buildings. Staurolite compares favorably with lower priced abrasives, such as boiler slags and silica sand, in applications where the abrasive is used only once and is not recovered and recycled. Because it is hard, fine, clean, and dense it can reduce blasting time, and less material is required to clean a specific area. Compared to higher priced abrasives such as aluminum oxide and garnet, which are recycled, staurolite costs significantly less per kilogram and cleans at equal or faster rates. Its breakdown factor, which is a measure of recycling performance, is intermediate between those for aluminum oxide and garnet. [ ] [Tables 3 and 4] show the blast cleaning performance of the three staurolite grades compared to other abrasives. [Table 5] compares the longevity of premium priced abrasives during recycling. [Table 6] shows a cost comparison between silica sand and staurolite. Fringe benefits claimed for staurolite's use as an abrasive are: 1. Good flow properties and freedom from the lumps or slivers found in silica sand or boiler slag abrasives. 2. Low dusting affords good visibility, permits faster working, and reduces cleanup problems. 3. Heated storage is not required during freezing conditions, because the mineral is nonhygroscopic.

Erratic distribution of values in free gold ore bodies presents severe problems in sorting material in large scale open pit mining. However, an intense program of modern blasting techniques and less than modern gold panning results in good sorting at Houston International's Manhattan mine. Blasting technique is important. If the sample location cannot be relocated because of blasted material movement, information acquired while drilling is useless. So short benches are critical to minimize dilution. Though not very accurate, panning allows for rapid analysis. The more accurate fire assay does not provide the needed speed. Panning can be fairly accurate and errors made are mostly on the low side. Samples of mixed and broken stockpile material are used as checks on pit procedure and to develop stockpile content in¬formation. Added tonnages discovered offset the higher labor costs of this careful sorting technique. The mine is located in central Nevada's Toquima Range in the small mining town of Manhattan, 322 km north of Las Vegas. The mine has four epithermal stockwork ore bodies that are mined in two pits. The mine problem was that conventional ore control was not separating ore from waste. Assays of 6-m blast-holes were used on 5-m centers. The blastholes were relocated by stadia survey after blasting. Holes assaying above the cutoff grade were flagged and mined as ore. This system is practical and useful in high concentration ore bodies or in evenly disseminated mineralization. But it was not practical at Manhattan because of large gold particles and small ore pocket sizes. Ore pockets were often missed when drilling a grid area for blasting. Also, large gold nuggets made assay results unreliable. The probability of a nugget being contained in an assay charge is small. So most results were low, compared to actual grades mined based on stockpile samples. When a nugget found its way into a fire assay charge, the result would be far higher than the actual grade of material represented. In addition, disturbance by blasting created problems in relocating ore. Because of these problems, the grade control program was intensified in September 1979. It successfully separated ore and waste in blocked out ore areas and located more ore in "waste" areas. Known reserves mined to date have increased up to 50% on some benches. The methods used are modern blasting techniques, detailed assaying, and traditional panning of samples collected from active mining faces. These methods allow mining flexibility, to match the variable mineral occurrence in ore bodies. The united effort of the geology and engineering departments has been important to the program's success.
