Steel supports are increasingly used in metal mines for junctions, chambers, and other wide openings and for semi-permanent areas such as haulageways, shaft stations, and pump rooms. Steel lasts longer-especially in wet mines and where foul air is present-and is procurable in different sizes that will carry heavier loads than any timber of equivalent cross section. The chief advantage of steel supports is that less excavation is needed than with timber support of the same strength, so that faster progress can be made. Since there is no possible way to calculate the exact weight on supports, sizes must be chosen by experience. Any adjustment and cutting of steel supports underground would be costly and time-consuming, and it is therefore advisable to cut sets beforehand at the surface workshops. This requires standardization of sets. All these problems have been recognized at Bawdwin Mines, Burma, where mining is difficult, slow, and very expensive. The lead-silver ore is very brittle, with hanging and footwall rock of unconsolidated rhyolite tuff. During the rainy season from May to October water penetrates into the mine, and where soft rock is present disintegration into a loose mass around openings can be expected. For this reason heavy 10x10-in. timber caps break quickly in drifts and crosscuts, and timber rots in a very short time. An average increase of 50 pct in both maintenance work and costs could not be avoided.

A photoguaplric method of measuring crack velocities has been applied to an examilzation of- the effect of hydrogen on the impact properties of zirconium. Results indicate that the crack in hydrided zirconit travels at about 26 m per sec whereas that in the uacuum -annealed is dependent on the speed of the hammer. RECENT work on the effect of hydrogen on zirconium reveals that when hydrided zirconium is tested in slow bending the crack producing failure can be unstable. The mode of fracture seems to involve the crack linking up broken hydrides by a ductile mechanism in the zirconium matrix.2" In order to compare the fracture of zirconium with that of other metals which show unstable crack propagation, notably steel, the rate of the crack spread must be known. As the methods previously used for measuring crack growth are either unsuitable for the present work5-'' or very costly," a simple technique has been devised in which the maximum crack velocity is derived from a series of crack position-time measurements. EXPERIMENTAL PROCEDURE The experimental arrangement is shown schematically in Fig. 1. The flash unit12 was triggered when the Charpy hammer touched the specimen and the 6-psec flash of the lamp delayed by a preselected time interval in the range 100psec to 10 msec. The camera was left on open shutter during the test. From direct measurement on a calibrated photographic print, Fig. 2, the position of the crack and the specimen was found at a known time after impact and, from a series of such tests using an appropriate range of flash-delay times, the crack velocity was estimated. Preliminary experiments were carried out on mild steel above and below its transition temperature. Vacuum-annealed zirconium, and zirconium hydrided to 100 ppm H were then tested at 20" and 150°C. For comparison, Zr-2.6 pct Nb alloy (in the 0 + transformed /3 condition), vacuum-annealed or hydrided to 100 ppm H, was tested at 20 C. The chemical analyses of these materials are given in Table I. For the high-temperature tests a fine chromel-alumel thermocouple was spot-welded onto each specimen close to the notch. The test piece was heated up to 200°C to allow enough time for preparing the apparatus while the specimen cooled to the testing tem- perature. It was assumed that the specimen cooled uniformly. The accuracy of the flash-delay time was better than i2 pct i 5 psec. The crack length was taken as its maximum distance traveled normal to the length axis of the specimen. Errors in these measurements arise as a result of the uncertainty in the location of the crack tip and the difficulty in defining the exact position of the crack starting point. The latter was due to the bonding and large amounts of deformation at the hammer contact point, especially in the vacuum-annealed specimens tested at 150°C. The maximum error in the measurements was however estimated to be about 5 pct. DISCUSSION OF RESULTS From the trial experiments using mild steel above its transition temperature it can be seen from the plot of crack length against time after impact, Fig. 3. that a reasonable estimate of the maximum crack velocity at the surface of the specimen may be made. Due to the coarse structure of mining explosives, one has to model a finite reaction zone,contrary to C-J calculations, describing their detonations, i.e they become dependent of diameter and confinement, An effort within the DYNO group has been to investigate if the DSD theory ( originally developed by Bdzil, Stewart / Los Alamos ) can be used to describe major groups of civil explosives.The groups of explosives tested are dynamite, anfo, aluminised emulsion, heavy anfo and straight emulsions.

The White King / Lucky Lass Superfund site consists of two former uranium mines in south-central Oregon. The remedy included consolidating two large mining overburden stockpiles that contained low levels of radioactivity and heavy metals by moving over 700,000 cubic yards of material and then capping the combined stockpile. An innovative armored cap design was used to minimize the stockpile footprint and thereby prevent disturbing existing wetlands. In addition, the stockpile was designed to merge into the surrounding topography. The remedy also included stream restoration and development of new wetlands. The stream was returned to its pre-mining channel, with the addition of hydraulic features to create new wetlands. The wetlands have greatly enhanced the ecological value of the remedy and the site.

"INTRODUCTIONOne of the key drivers for the selection of a mechanized tunneling method in good quality rock masses is a good excavation advance rate. In order to achieve satisfactory advance rates, a good geotechnical performance of the rock grippers in main beam open tunnel boring machines (TBM) is required, besides other factors related to ground support and the performance of the cutter head.One well known potential issue of main beam gripper TBM is associated with the bearing capacity of the tunnel side walls while crossing fault zones or poor rock masses in general. Such a mechanism is similar to that of footings on rock and perhaps the most common problem associated with gripper performance. Depending on the quality of the rock, the rock under the gripper contact may experience significant damage and crushing (Figure 1), eventually affecting the TBM advance rate due to a lack of tunnel wall reaction/support. Such a mechanism is primarily dependent on rock quality, applied loads and gripper size.In the case of large diameter TBMs (e.g., D > 12 m), a second potential failure mechanism may arise associated with the loss of confinement within a rock pillar between twin tunnels, even in good quality rock but particularly in bedded sedimentary strata. Punching of the rock pillar may develop due to excessive shear along bedding partings and sub-vertical joints (Figure 2), particularly under a low stress environment. As a result, this mechanism is dependent on both the rock pillar width between twin tunnels and stress conditions around the tunnels, in addition to the previous factors described for the first failure mechanism."

The flotation rate of coal has been studied using a continuous laboratory flotation cell and a multiple exposure photographic technique. The effects of particle size, reagent additions and cell turbulence on the flotation rate of coal have been investigated. The results have been analyzed in terms of current theories of particle bubble encounter and adhesion. The rupture forces which occur during particle bubble encounter have been shown to have an important effect on the flotation rate of coarse coal particles. The flotation of coal is a time dependent process, and in order to obtain a complete understanding of the operation it is necessary to determine the kinetics of the process. In coal flotation, the flotation rate and the associated values of cell productivity in terms of tons per kilowatt-hour and tons per cubic foot of cell volume are of greater importance than in most other mineral flotation operations. The reasons for this are associated with the low unit value of coal and the relatively high cost of coal flotation compared with other coal cleaning processes. An important aspect of coal flotation kinetics is the design of coal flotation cells. The cells used in many coal cleaning operations are often replicas of those used for the treatment of sulfide ores and are not necessarily suitable for the treatment of coal washing pulps, which usually contain a higher percentage of coarse and floatable material than the sulfide pulps for which the cells were designed. This disadvantage of many coal flotation cells has been recognized by Russian engineers who, by the addition of baffles to their subaeration coal flotation cells have increased the cell capacity by 100%. Investigation of the variables which influence flotation kinetics and cell productivity are, therefore, important to the coal industry where flotation is the only accepted method of cleaning the fine (-1/2mm) coal. The kinetics of the flotation process have been investigated in detail by Schumann10 who developed a continuous flotation cell which permitted the study of flotation under steady state conditions. This technique has been used by Gaudin et a1.,6 Brown and Smith and recently by Jowett and Savfi.7 The latter authors have developed a refined mathematical treatment for interpreting the results of continuous laboratory tests. Attempts have also been made to study flotation kinetics using conventional batch testing techniques. The experimental difficulties, however, and the problems of determining the order of the flotation rate process have made this technique of dubious value. A summary of the proposed rate equations for batch testing and the difficulties associated with this technique is given by Brown and Smith. 2 The experimental techniques described above are concerned with the rate of production of froth product. Mr. Chairman and Gentlemen: I tried to get Benedict Shubart to have some fellow-some big fellow with a big voice-to read this paper for me, but he said there was nothing doing; [ ] that I would have to read it myself. I will try and not take up too much of your time. It is a pretty big subject, Mechanical Loading, and it covers a good deal of ground. I have merely touched on the high spots, as it were. A great deal has been preached and written in the last year or so about mechanical loading, and I doubt if any other subject in the mining industry has been discussed so freely and as often in the last few years. It is not the intent of this paper to dwell on the merits or demerits of any [ ] particular type of loader, that is, to say what the writer thinks is the most efficient or most inefficient machine. There are several types of loaders on the market to-day that are proving very successful in many mining districts. THE U. S. Bureau of Mines has recently reported on the development of a process for preparing pure ductile titanium in substantial quantities' and on the physical properties that may be attained in the metal, when consolidated by powder metallurgy methods and fabricated by certain hot and cold-working procedures.2 The high physical properties (Table I), combined with excellent corrosion resistance and light weight, will lead no doubt to appropriate applications of the metal. In any application, methods of joining will be of considerable importance. The relatively high activity of the metal toward the common gases raises the question of weldability and conditions that might be required for successful welding. Since hydrogen, oxygen, and nitrogen embrittle the metal, it should be protected from these gases at elevated temperatures. This was demonstrated by a few preliminary trials of arc, gas, and resistance welding. In these trials only the resistance method proved to be immediately applicable. While atomic hydrogen welding of titanium may be successful in some instances, a considerable amount of development work will be required to prove its worth. In addition, it would be suited only to small assemblies that could be heated subsequently in a high vacuum to remove embrittling hydrogen. Heli-arc methods likewise would be limited to small assemblies, which could be completely protected by a helium atmosphere. It was quite apparent that titanium required more protection than was afforded by the normal flow of helium gas used for welding magnesium and aluminum. The hot metal is attacked by oxygen and nitrogen, and the rapid diffusion of the oxide into the metal prevents the formation of a protective coating and requires the helium atmosphere to be maintained until the weld has cooled nearly to room temperature. The preliminary work indicated that titanium could be successfully spot-welded without deterioration of the metal and that the microstructures of the welds were normal, exhibiting good bonding, and no oxidation. The high resistivity of titanium (53 microhms per cu. cm.) suggested that it would behave somewhat like stainless steel and allow considerable latitude in the several welding variables. The results reported here are intended to give a general picture of the effects of varying welding current, welding time, and tip pressure on single spot welds made on several different thicknesses of titanium sheet. The work was undertaken to demonstrate the feasibility of welding titanium rather than as an exhaustive study of spot welding. There are, therefore, a number of factors that will require further investigation to establish the specific controls required for actual applications.

IN making a study of the slip deformation and strain hardening of single crystals of aluminum, it has been found that considerable information additional to that provided by the usual metal-lographic techniques can be obtained by the use of the microfocus X-ray method described by Schulz.1 This technique has been applied to crystals both in the state as-grown by the strain-anneal method and after they have been subjected to increasing amounts of plastic strain. Kelly and Wei" have recently reported the use of this method for a study of lineage structure in aluminum single crystals grown from the melt. The crystals, prepared from aluminum of 99.99 pct purity supplied by British Aluminium Co. Ltd., were grown in the form of flat tensile specimens with a gage section of 0.3 by 0.1 in. and a gage length of 2.3 in. The microfocus X-ray tube used provided a focus of approximately 50 half width. Examination of a large number of as-grown crystals showed no sign of any macroscopic defects in the majority of cases. However, occasionally crystals grown by the strain-anneal method did show defects similar in nature to those shown by Kelly and Wei to occur in crystals grown from the melt. In some other cases a more regular appearance of black and white lines, corresponding to valleys and ridges, respectively, in the lattice, was obtained, Fig. 1. This particular crystal was slightly deformed as the lines when present in the image of an as-grown crystal are only barely visible. The angle of tilt of the blocks is of the order of 3. It seems likely that these tilt boundaries arise during the growth of the crystal due to the specimen not being perfectly flat on the hearth of the furnace and straightening under its own weight at the elevated temperatures. For the deformation studies the crystals were strained in a Polanyi-type tensile machine. A typical photograph taken after 1.2 pct elongation of a single crystal oriented with its axis fairly close to the center of the stereographic triangle is given in Fig. 2. The reflection, which is from the (111) planes, is traversed by two systems of parallel lines. These have been identified as traces of the primary (111) slip plane and the (110) plane perpendicular to the primary slip direction. The latter lines correspond to kink bands which could not be detected by the ordinary techniques of

The re-use of waste water from coal preparation plant tailings is always desirable, but is generally problematic in terms of solid-liquid separation. Dewatering and disposal of coal slurry at a high percentage of moisture poses a major technological and environmental challenge to mining and mineral processing industries world-wide. Characterization and flocculation of these coal tailings reveals that the separation of fine coal tailings depends on factors such as size distribution, slurry viscosity, zeta potential, pulp density, circuit design, plant operating parameters, and the behaviour of solid particles in an aqueous medium. The present study was carried out on -600 mm coal tailings collected from a coking coal preparation plant of Jharia Coalfield, India, using anionic flocculent. Characterization studies revealed that the zeta potential was highest (-44 mV) at a pH of 9.0, the size range of the particles was 1.1?700 m, and the average ash content was 62.57%. Flocculation studies were performed at different solid concentrations, pH values, and polymer dosages. The results indicated an efficient settling rate of 178.15 mm/min at 8% solid (w/w) at a flocculent dosage of 32.50 g/t solid. The turbidity of the clear liquid obtained was 7.42 NTU.

"Nippon Mining & Metals started a new copper anode slime treatment plant that processes slimes in a chloride media. The basic plan of the process was proposed by Mr. James E. Hoffmann and Nippon Mining & Metals modified and advanced to produce precious metals in acceptable qualities for the company. The process has a wet chlorination stage of decopperized slimes, a solvent extraction and a reduction stages of gold, a silver chloride reduction stage and separative reduction stage of its gold extraction raffinate. The new plant of this process started in 1997 and has been operated in good condition.IntroductionThe old anode slime treatment plant of Nippon Mining & Metals, located in it's Hitachi Refinery, processed anode slimes through a combination of pyrometarllurgical processing and electrorefining. There was a need to plan a future of the plant because the maintenance cost increased year by year and a new selenium registration was going to be applied to the plant. Some plans were discussed and the result was that the process proposed by Mr. Hoffmann was superior among them.Nippon Mining & Metals started 'to examine the process and the experimental result showed that the process had an ability to be its new process but further research should be necessary to produce precious metals in acceptable qualities. The final process flow has some modified and additional stages to be suitable for its requirement. Silver processing stage is quite changed. Gold extraction stage has a new step to polish gold loading DBC. Gold extraction raffinate treatment stage is advanced to be able to recover precious metals, selenium and tellurium, separately. Aeration under atmospheric pressure is employed in decopperizing stage.The basic design of the new plant started in 1995 and finished in 1997. The construction of the new plant began in April 1997 and finished in October. The plant has been operating well after the four months test operation."

The southern Mid-Atlantic Ridge (SMAR) is one of the least-explored spreading centers in the world with respect to hydrothermal activity. In the past 8 years research focused mainly on the ridge segments close to Ascension Island, discovering hydrothermal fields as far south as 14°20?S (Haase et al., 2007; German et al., 2008; Tao et al., 2011). However, the vast majority of the SMAR ridge axis has never been thoroughly investigated for hydrothermal activity. In the spring of 2013 we used segment-scale AUV deployments as well as conventional CTD surveys to identify areas of hydrothermal activity along 16 mid-ocean ridge segments between 13° and 33°S, thereby covering close to 5 % of the global ridge axis in a single cruise. During cruise MSM25 of the German research vessel Maria S. Merian, we tested longrange (>100 km lines per dive) AUV deployments of GEOMAR?s AUV ABYSS (a Remus 6000 vehicle) along the spreading axis for their capability to locate active venting in a fast and efficient way on regional surveys. "RÉSUMÉDurant les 10 dernières années, les améliorations dans le contrôle des limites des sautages et les arpentages après l’exploitation ont fourni des informations préalables et post facto sur les limites minières réalisées par rapport à la planification, plus particulièrement pour les grands chantiers exploités par les méthodes chambres vides et sous-niveaux. Cependant, peu de recherches ont été faites pour améliorer la délimitation du minerai, l’estimation des ressources et des réserves, les études de faisabilité et la récupération minière dans d’autres types de gisements. Il faudra d’abord expliciter les impacts des lacunes, en quantité et en qualité, de l’information géologique et de l’échantillonnage sur le rendement de l’extraction. Il faudra également remplacer la notion de « dilution générale » par celles de « dilution » et de « perte de minerai ». Les impacts des lacunes observées sur l’exploitation des gisements d’or filoniens sont démontrés par une simulation basée sur des résultats d’études antérieures à la mine Thompson d’Inco, des impacts financiers de densités d’information moindre et par des analyses des valeurs actualisées nettes (VAN) de deux cas types.Deux gisements similaires, de 3 m et 6 m de largeur et exploités par chambres vides sont étudiés, en prenant pour cas de base la délimitation sur maille de 7,5 m (25 pi.) et en étudiant les impacts des limites moins précises supportées par une maille de 15 m (50 pi.), en assumant que ces imprécisions moyennes de +/- 0,62 m se transforment soit en surestimation (dilution), soit en sous-estimation (pertes de minerai).Les impacts d’information moins précise ont été beaucoup plus forts sur le gisement plus étroit : autant la dilution que les pertes de métal ont diminué les VAN du gisement de 3 m de 39,27 % et 45,41 % respectivement, tandis que les VAN du gisement de 6 m ont été diminuées que de 12,84 % et 11,55 %.Ces impacts des valeurs actualisées nettes diminuées justifient et exigent des efforts plus systématiques pour s’assurer que les travaux de mise en valeur et les études de faisabilité soient réalisés de façon requise pour optimiser l’exploitation et offrir aux investisseurs des rendements compétitifs par rapport à ceux des autres secteurs industriels. ABSTRACTDuring the past 10 years, research work to improve the survey of stope limits after mining supplied useful information, particularly for large open and sub-level stopes, regarding waste dilution caused by differences between final production limits and planned stope limits.However, the efficiency of this information to improve the overall efficiency of the mining process is limited, given that the impacts of inaccurate ore limits leading to dilution (sensu stricto) and ore losses have not been considered adequately. Geological/sampling information should be optimized in terms of quantity and/or quality for the deposit characteristics and the constraints of the mining method used. As industry practice regarding the definition and recording of dilution is flexible and as ore losses frequently are poorly documented, the use of these terms is reviewed.Simulations were carried out on two typical narrow gold deposits, based on an earlier study at Inco’s Thompson mine regarding the impacts of wide to narrow delineation grids using recorded variations of vein geometry. Vertical to sub-vertical deposits of 3 m and 6 m widths mined by long-hole open stopes were considered. Deposits delineated by 7.5 m (25 ft) grids were taken as a base case and the impacts of less accurate limits in conjunction with long hole mining based only on a 15 m (50 ft) delineation grid. The impacts of overestimation (dilution) and underestimation (metal losses) were considered separately, using the average error of +/-0.62 m recorded in the Inco study.The impact of less accurate information was much stronger on the narrower deposit: both dilution and ore losses diminished the net present value of the 3 m wide deposit"

A low cost methodology that allows the determination of in- situ stress using oriented core specimens has been investigated. The technique can be used to determine the stresses during the early stages of a project, even in undeveloped areas of a mine. The research aim was to compare the experimental results estimated by the AE and DRA methods to those estimated by conventional ANZI (Australia and New Zealand Inflatable) cell measurements. The studies have been focused on fill stress tensor determination from a single oriented cored rock. In all cases the rock core specimens recollected the in-situ stress reasonably well compared with the standard overcoring method.

[Asphalt and road oil are the principal products manufactured from Wyoming ?clack oils? which, because of their high asphalt an high sulfur content, are difficult and costly to refine into high-grade motor fuels, burning oils, and lubricants by pre sent methods. Accordingly, the remand for these asphaltic residuums for the construction of roads and other uses is a controlling- factor in the quantity of crude oil produced. The increasing utilization of special asphalts as inexpensive bonding, insulating, and waterproofing materials indicates the desirability of obtaining information on the yield and properties, of asphalts from Wyoming black oil to promote a wider and more stable caret for these asphaltic materials.]

R. C. Gifkins (CSIRO)—In this paper evidence is put forward to support the idea of grain boundary shearing in aluminum at 4.2°K and the phenomenon is explained in terms of a low-temperature "equicohesive temperature". It is stated that these findings show that grain boundary shear cannot be ruled out in any situation by temperature arguments alone. We believe, from our own recent study of "apparent grain boundary sliding'' at temperatures below 0.45 Tm, that it can be ruled out on temperature arguments alone.45 Our experiments were made principally with magnesium and a Ms-A1 alloy ("Magnox A12"). Careful examination of offsets of marker lines or steps at grain boundaries produced by deformation at room temperature (-0.3 Tm) showed many of these to result from curvature over narrow zones along the grain boundaries. A significant residuum did, however, appear as a sharp offset or step. It was found that the mean value of this step rose to a small value during the first 0.5 to 1 pct strain and then remained at this level with further strain. Moreover, exactly the same values were obtained (at the same stress) at 75°C instead of 21°C. That is, in this range of temperature the "apparent sliding" was not temperature-sensitive. At 125°C and above there was sliding which increased with strain and which was temperature-dependent. When this temperature-dependent sliding was used to calculate the sliding to be expected at room temperature, this underestimated the "apparent sliding" by a factor of 104. The conclusion drawn is that the "apparent sliding" consists of shear localized in a zone of a width measured perhaps in microns, whereas it seems that true sliding occurs on the boundary surface.46 A model for such localized shear has been developed in general terms, following a similar model of Urie and Wain.47 Other strong evidence for low-temper- ature sliding has been based on the offsets of marker lines consisting of a substructure seen using the electron microscope to resolve these.48 We have repeated this work and found that the effects can be accounted for in terms of cracks in an oxide film which is produced when the substructure is etched up; these cracks form over zones of shear rather than at actual interfaces. However, in aluminum at room temperature (-0.3 Tm) there is no "apparent sliding" which is optically detectable. We suggest that, when the temperature is much lower (and possibly the rate of strain higher, too), as in the experiments of Chin et al., then aluminum behaves like magnesium at room temperature, and a narrow zone of shear develops to accommodate differences in strain across the grain boundary. Such a zone could well give rise to triple-point cracks. It is hoped to investigate these suggestions further. B. Y. Chin, W. F. Hosford, Jr., and W. A. Backofen (authors' reply)—It is interesting to have Dr. Gif-kin's comments, although it is somewhat difficult to reply in detail without the reference containing the work that he describes. Apparently though, he does find a low-temperature temperature-independent sliding that would seem to be related to the observations reported here. Perhaps some of the difficulty has semantic origins, growing out of the distinction between ('true sliding" and "apparent sliding".

"Quiet, please!" is the newest directive being thrust at industry by guardians of the environment-with good reason. In countless cases, industrial noise is literally deafening its listeners, and sooner or later the volume must be turned down. Noise control is already mandatory for many companies under a Federal regulation that went on the books when the Walsh-Healey Act was revised last May 20. The new rules impose specific noise limits on any plants supplying the Government with goods under a contract valued at more than $10,000.

"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."
