One of the predominant causes of fires in underground coal mines is flame cutting and welding. These fires can lead to major events such as the explosion that occurred on January 22, 2003 in an airshaft being constructed at the McElroy mine. Six contractors were inside the shaft at the time of the explosion and the explosion fatally injured three and seriously injured three [MSHA 2003]. Understanding the root causes of these fires and explosions will help prevent these types of events from occurring in the future. Fires and explosions require fuel, oxygen and an ignition source (heat). These three items are referred to as a fire triangle. The control of these three items in flame cutting and welding operations is required to prevent fires and or explosions. United States Department of Labor, Mine Safety and Health Administration (MSHA) Reports of Investigation (ROI), MSHA Mine Accident, Injury, and Illness Reports, and the MSHA Mining Industry Accident, Injuries Employment, and Production Data Base were analyzed and interviews and observations with mine workers and welders were conducted to determine the root causes of coal mine fires and explosions caused by flame cutting and welding operations. This paper will discuss the root causes and potential means to reduce the number of fires and resulting fatalities and injuries caused by or due to flame cutting and welding.

This paper describing the mining practice of the Hecla and Star mines, both located at Burke, Idaho, is one of a series being prepared by the Bureau of Mines on mining practices, methods, and costs in the various mining districts of the United States. It is partly a revision of articles by the author appearing in volume 72 of the Transactions of the American Institute of Mining Engineers, published in 1925, and in the June 1928, issue of the American Mining Congress Journal. About 26,000 tons per month of lead ore is produced at the Hecla mine. Costs at the mine are given for the year 1928. Stoping operations in the Star mine have covered such a short period, during which time the abnormal expenses of starting were incurred, that they are not given in this paper.

Over the past century coal miner safety and health has seen tremendous improvements, the fatality and injury rates continue to decrease while productivity continues to increase. Many of the hazards that plagued miners in the past, such as coal bumps, methane and coal dust explosions, ground fall accidents and health issues have been significantly reduced. The contribution of NIOSH research includes products for prevention and survival of mine fires, methane control measures, design procedure for underground coal mines, methods for excavation surface controls, methods and procedures for blasting, laser usage in underground mines and prevention of electrocution from overhead power lines that have reduced accidents and injuries in underground coal mines. Health research has produced products such as the personal dust monitor, noise abating technologies and ergonomic solutions for equipment operators. Research priorities at NIOSH are set by considering surveillance statistics, stakeholder inputs and loss control principles. Future research in coal mining is directed towards respiratory diseases, noise induced hearing loss, repetitive musculoskeletal injuries, traumatic injuries, falls of ground and mine disasters. The recent spate of accidents in coal mines resulted in the Miner Act of 2006 which includes a specific role for NIOSH in future mine safety research and development. The mine safety achievements in the USA reflect the commitment of industry, labour, government and research organizations to improving the safety of the mine worker.

This report describes reconnaissance by the Federal Bureau of Mines of titanium-bearing sandstone in Kemper County, Miss. The purpose was to determine the ilmenite, rutile, and associated heavy-mineral content of black-sand-bearing Eocene deltaic formations" The investigation was part of a survey of titanium resources of the United States. Ilmenite (FeTi03), an iron-titanium oxide, is the major titanium-bearing mineral consumed in the titanium pigment industry. Rutile (Ti02), titanium oxide, is used mainly in producing welding-rod coatings and titanium metal. The fieldwork was conducted during the first half of November 1959. Seven holes, totaling 304 feet of bore, from which 64 samples of cuttings were taken, were drilled in the north-central part of the county; and two holes, totaling 172 feet of bore, from which 34 samples of cuttings were taken, were drilled in the southeastern part.

Study Design. Survival analysis techniques were used to compare the fatigue failure responses of elderly motion segments to a middle-aged sample. Objectives. To compare fatigue life of a middle-aged sample of lumbosacral motion segments to a previously tested elderly cohort. An additional objective was to evaluate the influence of bone mineral content on cycles to failure. Summary of Background Data. A previous investiga¬tion evaluated fatigue failure responses of 36 elderly lumbosacral motion segments (average age, 81 ± 8 years) subjected to spinal loads estimated when lifting a 9-kg load in 3 torso flexion angles (0°, 22.5°, and 45°). Results demonstrated rapid fatigue failure with increased torso flexion; however, a key limitation of this study was the old age of the specimens. Methods. Each lumbosacral spine was dissected into 3 motion segments (L1-L2, L3-L4, and L5-S1). Motion segments within each spine were randomly assigned to a spinal loading condition corresponding to lifting 9 kg in 3 torso flexion angles (0°, 22.5°, or 45°). Motion segments were statically loaded and allowed to creep for 15 minutes, then cyclically loaded at 0.33 Hz. Fatigue life was taken as the number of cycles to failure (10 mm displacement after creep loading). Results. Compared with the older sample of spines, the middle-aged sample exhibited increased fatigue life (cycles to failure) in all the torso flexion conditions. Increased fatigue life of the middle-aged specimens was associated with the increased bone mineral content (BMC) in younger motion segments (mean ± SD, 30.7 11.1 g per motion segment vs. 27.8 ± 9.4 g). Increasing bone mineral content had a protective influence with each additional gram increasing survival times by approximately 12%. Conclusion. Younger motion segments survive considerably longer when exposed to similar spine loading conditions that simulate repetitive lifting in neutral and flexed torso postures, primarily associated with the increased bone mineral content possessed by younger motion segments. Cycles to failure of young specimens at 22.5° flexion were similar to that of older specimens at 0° flexion, and survivorship of young specimens at 45° flexion was similar to the older cohort at 22.5°. Low back disorders (LBDs) are a major cause of both short- and long-term occupational disability in the United States, and it is clear that workers in certain occupations are more susceptible to LBD.2-5 Specifically, epidemiologic studies have shown that jobs involving heavy physical demands such as construction,2,3 mining,2,4 and farming,5 engender increased risk of back pain. These occupations are thought to experience high LBD rates as a result of their extreme postural and manual lifting demands. Studies have shown that jobs involving significant lifting 6-9 and jobs that involve frequent bending10-12 are associated with increased LBD risk. Workers in the mining industry, in particular, often have to lift heavy materials in restricted workspaces that compel torso flexion. An understanding of the impact of repeated loading of the spine in flexion on fatigue failure of spinal tissues is thus a critical issue for our research agency, which is concerned with reducing the pain and disability associated with LBD in the mining industry. It is well accepted that loads experienced by the spine during manual lifting tasks are sufficient to cause fractures in the endplates of lumbar vertebrae, particularly on repeated loading. 13-15 Endplate fractures may not be painful in and of themselves 16; however, evidence suggests that the process of internal disc disruption and disc degeneration may be initiated via endplate fractures. 16,17 It has been shown that endplate damage will alter the distribution of stress in the disc, resulting in buckling of the lamellas of the internal anulus.17 Once this process has been initiated, repeated spine loading will cause additional disruption of the anular fibers, resulting in formation of significant fissures or tears. 11-21 If these tears occur near or extend close to the outer portions of the anulus, a wound healing process is initiated, which involves infiltration of mast cells, macrophages, and re¬lease of cytokines and growth factors. 21 The presence of these infiltrates has been strongly linked to discogenic low back pain. 21,22 Indeed, for patients experiencing chronic low back pain, the prevalence of internal disc disruption is at least 39%,19 which represents the most

The results of a program to demonstrate the technical feasibility of hydraulic borehole coal mining are presented in this report. The hydraulic borehole method of coal mining involves inserting a hydraulic mining device into a borehole that has been drilled from the surface into the underground coal bed. The borehole mining device issues a jet of water under high pressure, which breaks the coal. These pieces of coal, along with the water, form a slurry, which flows to the base of the borehole where it is pumped to the -surface. Phase I of the program encompassed the design, fabrication and factory-testing of a hydraulic borehole mining device. The program was successfully completed with the field tests being conducted at the Wilkeson-Carbonado Coal Field near Wilkeson, Washington. The technical feasibility of hydraulic borehole coal mining was demonstrated by mining 32 tons of coal at a depth of about 75 ft over a period of 4 hours.

Although the exact mechanisms underlying vibration white finger (VWF) are not clear, it has been speculated that VWF is associated with variations of the blood flow patterns due to the physical damage and/or degeneration in neural and vascular tissue caused by vibration loading [1]. Excessive dynamic deformation of the soft tissues in the fingertip under vibration loading is believed to induce multiple occupation-related hand/finger disorders. However, the in vivo distributions of the dynamic stress/strain of the tissues in the fingertip under vibration conditions have not been studied because they cannot be measured experimentally to date. The goal of this study is to analyze, theoretically, the location and frequency-dependent dynamic deformation of the soft tissue in the fingertip during vibration exposures. [ ] Methods The fingertip considered in the model is the distal phalanx, the portion from the distal end of the fingertip to the distal interphalangeal (DIP) joint articulation (Fig. 1). The external shape of the fingertip was determined using a smooth mathematical surface fitting to the observed fingertip shapes. The fingertip surface was then scaled to the dimensions of a typical male index finger: length 25 mm, width 20 mm, and height 18 mm. The fingertip was approximated to be symmetric, such that only a half of the fingertip was considered in the FE modeling. The fingertip was assumed to be composed of outer and inner skin layers, subcutaneous tissue, bone, and nail. The soft tissues (inner skin layer and subcutaneous tissues) were assumed to be nonlinearly elastic and viscoelastic, while the bone, nail, and outer skin layer were considered as linearly elastic. The simulations were conducted using a displacement-controlled protocol in two stages. First, the fingertip was statically pre-compressed. The contact plate was first displaced towards the finger to achieve a predetermined value of tissue deformation (i.e., 0.5, 1.0, 1.5, and 2.0 mm). Second, the steady-state dynamics responses of the fingertip were analyzed using a linear perturbation procedure. The fingertip was subjected to a continuous harmonic excitation (magnitude 0.5 mm) from the contact interface. The dynamic analysis was performed in a frequency domain ranging from 16 to 2000 Hz. The frequency-dependent distributions of the vibration magnitude and dynamic strain magnitudes in the soft tissues are investigated. Results Typical simulation results for the frequency-dependent distributions of the vibration magnitude in the soft tissues are shown in Fig. 2 (figures show the results with a pre-compression of 2.0 mm). The vibration magnitude at the contact surface is 0.5 mm (specified) for all frequencies, while the vibration magnitudes in the soft tissues are location- and frequency-dependent. It is clear that the fingertip has a major resonance around 125 Hz, at which the vibration magnitudes in the soft tissues are over four times greater than that of the contact plate (0.5 mm). It is interesting to observe that, at this resonant frequency (125 Hz), the soft tissues at the tip has the maximal vibration magnitude while the regions near the contact

The National Institute for Occupational Safety and Health (NIOSH) has conducted research to improve the ventilation of large-opening mines. Large-opening mine ventilation is unique for the following reasons: (1) it is challenging to keep airflow velocities high enough to effectively remove or dilute airborne contaminants, (2) large air volumes can be moved through the mines with little static pressure drop, and (3) stoppings to direct ventilation airflows are costly to construct and maintain. The research results suggest that by incorporating ventilation planning into the mine planning process, using propeller fans, developing new stopping materials and construction methods, and using long pillars to eliminate crosscuts where possible, the ventilation of large-opening mines can be significantly improved. The ventilation improvements created by incorporating these various techniques into the ventilation plan will help reduce the exposure of mine workers to airborne contaminants in underground large-opening mines.

An examination of the Census of Fatal Occupational Injuries (CFOI) data for the period 1992-1995 shows that 659 fatalities occurred in the mining industry as defined by the SIC. Oil and gas accounted for slightly more than one-half of all fatalities (figure 1-1). The coal industry had the highest fatality rate for the 4-year period, with 35.6 deaths per 100,000 full-time workers, followed by metal (23.1 per 100,000), oil and gas (23.1 per 100,000), and nonmetal (20.1 per 100,000). The fatality rate for all U. S. industries during this same period was 5.1 per 100,000 full-time workers. Figure 1-2 compares the fatal injury rates by year for the four major industry sectors of mining to the fatality rate for all industries combined. Figure 1-3 compares age-specific fatality rates in the mining industry to those seen for U. S. industry as a whole. Fatality rates for all industries combined were lowest for young workers and steadily increased in relation to workers' ages. In the mining industry, fatality rates by age group show a U-shaped curve. Young workers under 20 years of age had the highest injury rate (54.6 per 100,000), followed by workers aged 20-24 (50.8 per 100,000) and workers over age 65 (37.7 per 100,000). Table 1-1 shows the distribution of injuries by type of injury event. The mining industry, in comparison with all industries, had a considerably larger proportion of fatalities classified as caught in or crushed by collapsing materials (9.1% and 1.9%, respectively) and fires and explosions (10.0% and 3.1%, respectively). Transportation events comprised the largest proportion of fatal injuries in both the mining industry and all industries. Within the mining industry only, transportation accounted for the largest proportion of fatalities in all mining sectors except coal. A more detailed examination of the transportation events within the mining industry showed that highway incidents (50.5%) were the most common, followed by nonhighway incidents (22.2%), pedestrian incidents (10.6%), and those incurred during rail, water, and air transport (15.7%). Incidents involving caught in or crushed by collapsing materials comprised a higher percentage of injuries in the coal and metal sectors, while fall-from-height incidents were more frequent in the oil and gas sector. The classification system used in CFOI for type of injury event is not compatible with any variables contained in the MSHA data; therefore, direct comparisons are not attempted. [ ]

One of the primary methods for analyzing roof stability in underground mines is the age-old method of “roof sounding” where a miner taps on the roof and listens for the hollow sound of loose blocks of rock. This paper looks at the feasibility of using noncontact laser-based vibration measurements to detect roof fall hazards with the ultimate vision of improving, expanding and automating procedures for mine roof inspection. Vibration measurements made on loose blocks of rock in underground mines are summarized and compared to estimates of fundamental resonance frequencies for rock slabs of the size responsible for highly hazardous “skin failures.” Both laser Doppler vibrometry and full-field interferometric methods are examined and are considered to be feasible methods for detecting anomalous vibrations in loose roof rocks. Results from simple laboratory experiments using laser vibrometry demonstrate some of the proposed application concepts. While considered a challenge to move these techniques from the laboratory to heavy industrial or outdoor environments, the potential for success in the current application is enhanced by the reduced requirements of qualitative analyses.

"The handling and transportation of explosives and explosives supplies are inherently hazardous owing to their sensitiveness to impact and flame or electric arcs and sparks. Many accidents have occurred in the past during transportation of explosives, especially when they were carried by individual miners on man-trips. When electric haulage became the usual mode of transportation and shots were fired electrically, the hazards of carrying explosives and supplies on man-trips were fully realized, and in some mines all explosives were placed in one car, usually at the rear of the trip. Far-sighted managers saw the danger of this practice and provided separate trips for delivering explosives. This step was a definite advance in safety, but the latest achievement is the construction of special, rugged, nonconducting cars.Many coal-mining men have shown considerable interest in the construction of safe cars for explosives during recent months. Undoubtedly many others are interested in this phase of safety, therefore this circular is issued to give information on cars of this type now in use.Car A (fig. 1) is essentially a closed inner box separated from an outer box by an insulating lining of rubber. The double box is attached to a standard steel chassis that has spring journals and electric welding instead of certain rivets. The chassis is equipped with roller bearings. The over-all length from pin to pin is 11 feet, 9 inches; the wheel base is 54 inches, the track gage 42 inches, and the capacity eighteen 50-pound cases of permissible explosives. The box is 10 feet long, 6 feet wide, 3 feet high at the top of the arched roof, and 2 feet 9 inches at the edges. The top of the car is 46 inches above the top of the rail.The inner box is insulated from the outer box by two pieces of 1/4-inch switchboard.-insulating matting cemented together. To cover the top, two ends, two sides, and the bottom of the box requires 320 square feet of the l/4- inch matting.The walls are well-seasoned tongue-and-groove oak, 1 1/4 inches thick and 2 3/4 inches wide. A 1 1/4-inch air space is maintained by oak strips between the outer wall and the two thicknesses of 1/4-inch rubber matting. The inner box is made of 7/8-inch, well-seasoned, tongue-and groove oak fitted closely against the insulating matting. No nail, rivet, or bolt extends through the rubber insulation.The floor of the outer box is secured to the chassis by two sets of bolts through an intermediate piece of well-seasoned oak. One set of bolts is countersunk from the top of the intermediate piece of oak, and the other set is countersunk from the bottom. The countersunk portions are filled with ""insulate,"" an insulating compound. Adjacent bolts are at least 6 inches apart.The bed of the inner box is divided into six compartments by transverse strips of wood at the bottom and partitions near the roof. Each compartment accommodates three 50-pound cases of explosives. This arrangement prevents cases in adjacent compartments from touching, so that there is no possibility of end thrust of the entire load of explosives. Each inner compartment is 21 inches high, 5g inches long, and 16 1/4 inches wide. Separating strips at the floor are 3 inches wide and 1 1/4 inches thick. The outside dimensions of the 50-pound cases of explosives used are 17 1/2 inches long, 13 1/2 inches wide, and 12 1/2 inches high."

All general measures of the safety record of the coal mining industry worsened appreciably in 1970. Injury experience for the year was 260 fatal and 11,552 nonfatal injuries at respective frequency rates of 1.00 and 44.40 per million man-hours of worktime. The fatality total was 57 higher and the rate of occurrence was 18 percent higher than the corresponding figures of 203 fatal injuries with a frequency rate of 0.85 in 1969. The worsened fatality record in 1970 resulted primarily from a major disaster (a single accident which results in the death of five men or more) on December 30, 1970, that killed 38 men. There were no major disasters in 1969. The total of 11,552 nonfatal injuries in 1970 was the highest annual total since 1960. It was 1,635 higher and the frequency rate was 6 percent higher than the corresponding totals for 1969 of 9,917 nonfatal injuries occurring at a rate of 41.76. The injury severity rate of 8,308 days lost or charged per million man-hours was 13 percent worse than that of 7,359 in 1969. Similarly, the average severity of 183 days lost or charged per injury in 1970 was 10 days or 6 percent higher than in 1969. Aggregate worktime of 260.2 million man-hours and production of 609.8 million tons in 1970 were, respectively, 10 and 7 percent higher than in 1969. An average of 144,480 men were at work on an average of 227 active days in 6,316 mines and 544 mechanical cleaning plants during 1970.

"INTRODUCTION The relative value of the various individual physical and chemical properties of coke as a measure of its performance in the blast furnace as aroused considerable controversy but it is generally agreed that uniformity in these properties is extremely important if maximum mum pip-iron production is to be attained. The control of,the bulk density or weight per cubic feet of the coal charge in the coke ovens is a major, and frequently overlooked, factor in producing a uniform product and in efficient operation of the coke plant.This report presents a brief discussion of the general problem of bulk-density control and the results of tests on samples of coke-oven charges from several operating plants experiencing difficulties due to lack of adequate control. These tests covered the effect of moisture and oil content on the bulk density of the coal. Some tests on the effect of a wetting agent and furnenous of gained were also made. A description of the methods used to determine the bulk density of the coals studied in this investigation and of the methods used to apply oil to the coal at byproduct-coke plants are included in this product. No theoretical analysis of the data presented in this report, because the primary purpose of the investigation was to obtain data of immediate practical value to the plants concerned."

During the time period from 1990-2007, 1601 reportable fires occurred in the U.S. mining industry (an average of 89 fires per year). The leading causes of U.S. mine fires were flame cutting and welding operations, frictional heating and ignitions, electrical shorts, mobile equipment malfunctions, and spontaneous combustion. The fact that mine fires continue to occur with an alarming regularity reinforces the importance of recognizing and eliminating the potential hazards and the overall need for improved fire control and suppression technology to ensure the best possible outcome during a mine fire. The National Institute for Occupational Safety and Health (NIOSH) is conducting an evolving comprehensive program of research that is addressing mine fire prevention, detection, management and suppression. This paper presents a summary of recent research accomplishments and provides an overview of the next phase of the NIOSH mine fire research program.

"A new apparatus has been devised for quickly and precisely analyzing natural gas for its helium content. The following outstanding advantages make it far superior to any equipment now available for the purpose.3/4/5/6/1. It enables more rapid determinations to be made.2. It has far greater sensitivity than previous apparatus. 3. It is portable.Fifty or more samples may be analyzed in an 8-hour day with the new apparatus and procedure. A maximum of about 10 samples per day could be analyzed with previous apparatus. In the apparatus described, the volume of pure helium obtained from a sample is determined by measuring the pressures created by that pure helium when it is expanded into a constant calibrated volume. In older methods the pure helium separated from a sample is pumped into a burette and measured at atmospheric pressure. The new apparatus and Procedure used with it reduce the average time for an analysis from about 40 minutes to less than 10 minutes,Samples containing a few thousandths of a percent helium may be analyzed as readily as samples containing 1 percent or more, This was not possible with former apparatus. Duplicate samples containing a few thousandths of a percent helium have been analyzed, results checking to one ten-thousandth of a percent. Attempts to measure a corresponding volume of helium in a burette at atmospheric pressure become futile. The ability to determine very small quantities of helium has become of great importance recently in gas- and oil-field engineering studies such Os the utilization of helium as a tracer gas in following the movement of gases through underground structures.A complete apparatus was recently carried more than a thousand miles in a passenger automobile. It arrived without breakage, was .set up and ready to make an analysis in a few hours. .Former apparatus was not portable. The portability of the present apparatus permits taking the apparatus to the field for more efficient prosecution of field engineering problems. The only services required in the field are electricity, tap water, and a source of liquid air or liquid nitrogen.The principle involved is based upon the fact that large volumes of all the constituents usually found in natural gas, except helium, are adsorbed quantitatively on cocoanut charcoal that has been cooled to the temperatures of liquid air. A volume of sample is measured and transferred to a tube of cocoanut charcoal maintained at the temperature of liquid air. After adsorption of all the constituents other than helium, the charcoal tube is connected to a sensitive pressure-measuring device, and the Percentage helium originally in the sample is calculated from two pressure readings and the volume of the original sample."

Occupational exposure to hand transmitted vibration (HTV) arises from the hand held powered tools extensively used in the mining and construction industry such as rock drills, chipping hammers, chain saws etc. Regular exposure to HTV is the major cause of a range of permanent injuries to human hands and arms which are commonly referred to as hand-arm vibration syndrome (HAVS). In addition to this, the percussive tools generate overall sound power levels in excess of 110dBA in most cases. Such a high sound power level greatly exceeds the maximum permissible exposure limit (PEL) of organizations such as National Institute for Occupational Safety and Health (NIOSH) and the Occupational Safety and Health Administration (OSHA). Long term occupational exposure to this noise has been diagnosed as the main reason for permanent hearing loss in the operators. It is therefore important to develop an understanding of the mechanisms which lead to these high vibration and sound levels and in order to do this a detailed computational model of a pneumatic chipping hammer has been made. This paper presents a nonlinear computational model of a pneumatic chipping hammer. In order to better understand the dynamics of the chipping hammer, the hammer was subdivided into components that are shown in figure 1 (a) (based on a chipping hammer manufactured by Atlas-Copco). The hammer mainly consisted of a center body, a moving piston and a chisel. Compressed air is used to drive the piston inside of a cylinder and on the downward stroke this piston impacts the chisel to create the hammer effect. The machine has one pneumatic valve and this valve regulates the air supply either to the upper chamber or to the lower chamber. The valve changes according to the relative pressures in the two chambers and the supply pressure. There are also twelve different exhaust ports at two positions along the cylinder labeled upper ports and lower ports. As the piston moves the ports can be closed or open (allowing exhaust). Fundamentally, the computational model was made up of two different sub-models, a fluid model and a structural dynamic model as shown in Figure 3 (a) and (b) respectively. The first sub-model takes into consideration the fluid dynamics of the machine since the hammer is driven by compressed air. Equations for the mass flow rate though bleed orifices (assuming an isentropic process) 1 is used to determine the mass flow into and out of the upper and lower chambers.
