Throughout all segments of the United States mining industry, 163 independent contractor employees were fatally injured between 1990 and 1997. Factors that contributed to these injuries were identified after reviewing descriptive information contained in Mine Safety and Health Administration fatality reports for each of the incidents. Thirty-three unique contributing factors were identified and multiple factors were noted for each fatality. Those most frequently discerned were "failure to follow `standard operating procedures"', (35% of the fatalities), "poor positioning of employee" (26% of the fatalities) and "failure to maintain equipment" (23% of the fatalities). Contributing factors were then associated with the accident and job classifications of the victims. Factors having the highest frequencies in the top five accident classifications where most fatalities occurred were as follows: powered haulage - "failure to maintain equipment"; machinery - "poor positioning of employee"; slip/fall of person - "inadequate fall protection" and "failure to wear safety line"; electrical - "equipment contact with power line"; and falling/rolling/sliding rock - "poor positioning of employee. " Among the job classifications in which most fatalities occurred, leading contributing factors were as follows: truck driver - "failure to follow standard operating procedures" and "poor positioning of employee"; laborer/utility man/pumper - "poor positioning of employee"; mechanic/ repairman and welder - "failure to follow standard operating procedures." Recommendations for utilizing contributing factor information as a basis for safety training are proposed. Training programs may be developed that inform independent contractor employees of factors which, in the past, have contributed to fatal injuries in their job classifications. Then, by combining this information with contributing factors identified in "related" accident classifications, a truly focused training intervention that addresses very specific workplace hazards may he developed.

The Doan Valley Storage Tunnel Project in Cleveland Ohio was constructed to control Combined Sewer Overflows (CSOs). It is a complex system of three tunnels ranging from 8.5 to 18 feet in diameter with a total length of about 2.3 miles that was put into operation in mid-2021. The project lies within and below the cultural, educational, and medical hub of Cleveland, which presented unique design and construction challenges. This paper provides a case history of the design and construction, with particular attention given to lessons learned in devising a large tunnel system that can carry both dry and wet weather flows constructed in rock with limited cover, within a flood plain area and vibrant urban center

A numerical investigation of the effect of pore pressure regime on the safety factor and the critical failure mechanism is presented for fly ash storage facility. Pore pressures’ measurements from standpipe piezometers and pore pressure estimated from seepage analysis are used to compare the factor of safety for a fly ash slope. This was applied for considering static and seismic scenarios. A probabilistic approach was applied to account for the uncertainties resulting from the limited data available and support a qualitative risk assessment evaluation. Slope stability analysis is conducted in two and three dimensions, adopting the limit equilibrium analysis approach, and also a finite element seepage analysis, to assess the stability of the slope. The two-dimensional cross-sections were extruded to three-dimensional models to estimate the factor of safety and associated shear failure. The results from the performed analysis suggest an increase in safety factor values of 5%.

Slope failures very commonly occur during a long period of heavy rainfall and groundwater rise. The failures pose a substantial risk to people, infrastructure, and equipment downslope. In the recent past, the mining industry has faced an increase in tailings dam failure due to several reasons including pore pressure increases in the dams. Many failures have been observed to occur during times of water level fluctuations but the critical factors that influence the initiation of slope failures still need to be adequately clarified. To investigate these factors, laboratory experiments were conducted on model sandy slopes to determine pore pressure-induced slope failure initiation. This study also presents a method to examine water seepage through soil slopes using an ultraviolet (UV) dye and a UV flashlight. The small-scale model test simulation demonstrated failures induced by either water percolation from the upslope tank at a constant level or by raising the water level at intervals. A bottom chamber fully filled with water to replicate groundwater rise was also simulated. The soil slope was monitored at every stage to study the deformations and behavior until failure occurred. Results gathered from the controlled laboratory conditions were useful for the verification of the numerical modeling method created in the computer program Slide. The analysis showed that slope failure always occurred when the toe was fully saturated. High pore pressures and seepage forces reduce the shear force at the toe to almost zero causing it to slide. A comparison of slope height to water level also indicated an average of 96% water level to cause a failure. At this point, the soil slope is fully saturated and has no matric suction. The findings in this study show that, by monitoring the moisture content of slopes, failures can be predicted.

"PROJECTThe “Caltanissetta” Tunnel is part of the ANAS S.p.A (National Autonomous Roads Corporation) $ 1.3 Billion Highway 640 Caltanissetta-Agrigento (Sicily, Italy) Rehabilitation Project, consisting in upgrading of 62 km (40 miles) of highway. The Italian multinational construction company Cooperativa Muratori & Cementisti di Ravenna, is the General Contractor for the design and build contract. The highway S.S. 640 starts from Porto Empedocle, to the intersection with the Motorway A 19 Catania-Palermo.The entire project, double carriageway designed for speed 70-120 km/h and maximum longitudinal slope of 4.2%, is divided into two sections. First Section: just finished and opened to traffic. Second Section: mainly involving the province of Caltanissetta, length about 28 km (18 miles), works started in 2013. This second section includes the most challenging work of all the project, the “Caltanissetta” tunnel consisting of two separate tubes, each with 3.9 km length and inside diameter of 13.50 m, in order to host 2 traffic lines and an emergency line. HORIZONTAL AND VERTICAL ALIGNMENT - GEOLOGY AND HYDROGEOLOGYThe plan alignment of “Caltanissetta” tunnel is formed by two tubes which are located at a minimum mutual distance of 22 m to a maximum of 63 m, with n. 9 pedestrian and n. 4 road cross passages; ventilation shafts are not provided. Overburden over portals area is about 5 m till a maximum of 120 m. Tunnel sections have variable longitudinal slope from 2.6% to 0.5%, transversal slope of about 3.5%. Right tube length is 3,992 m, left tube length is 3,877 m."

Introduction Samarco Mineracao S.A. is a Brazilian corporation jointly owned by S.A. Mineracao da Trindade (SAMITRI) and Utah International Inc., with 51% and 49% interests respectively. Headquartered in Belo Horizonte in the state of Minas Gerais, Samarco owns and operates an open-pit mine capable of producing 10 Mt/a (11 million stpy) of iron ore; a concentrator rated at 7 Mt (7.7 million st); a 12-Mt (13.2-million st) capacity pipeline transportation system; a 7 Mt (7.7 million st) filtering and pelletizing plant; and a harbor facility. The mine, concentrator, and initial pipeline pump station are located at Germano, near the town of Mariana, about 300 km (186 mi) north of Rio de Janeiro by air. The pipeline terminal, pellet plant, and harbor are at Point Ubu, 70 km (43 mi) south of Vitória the capital city of Espirito Santo State, and about 300 km (186 mi) by air east of the mine. The 396-km (246-mi) pipeline uses 508-mm (20-in.) pipe except for a 53-km (33-mi) section where 457-mm (18-in.) pipe is used to absorb and control slack flow through increased velocity in one of the areas along the route. Iron Ore Slurry The hematite iron ore concentrate produced at the Germano plant is processed by grinding the average 53% run-of-mine itabirite iron ore to -150 µm (100 mesh) for gangue liberation. It is treated further by cyclones to remove the 10 µm (1250 mesh) slimes to prepare the ore for flotation. During the multistage cationic flotation operation, the silica is reduced to make low silica (-0.9%) concentrate for the direct reduction (DR) iron-making process or to make normal silica (-3.5% +2.5%) concentrate for the blast furnace (BF) iron-making process. The concentrate then is reground further to meet the particle size requirements for pelletizing. The reground hematite concentrate is screened in closed circuit with the regrind mills for close control of the maximum particle size, not to exceed 3.5% +75 µm (200 mesh) for pipeline wear protection. Thickened concentrates at 70% solids are pumped to four 5-kt (5,510-st)(dry) agitated slurry storage tanks that hold the feed before it goes to the first pipeline system pumping station. Pipeline Capacity The pipeline system was designed to transport 12 Mt/a (13.2 million stpy) at 65% solids with an average minimum safe operating velocity of 1.5 m/s (5 fps). The pipeline will transport 5 Mt (5.5 million st) in 1982. The system, therefore, is moving the slurry in 13-kt (14,330-st) batches, dispatched at intervals to move 20 kt/d (22,050 stpd). In operation, the pipeline has exceeded its 93% availability design factor. Pipeline Facilities Pump station No. 1 is located at the Germano minesite 1 km (3,330 ft) above sea level. It has the following principal equipment: • four 5 kt (5,510-st) (dry) agitate slurry storage tanks; • two centrifugal charge pumps (one stand-by); • a 217-m (712-ft) safety loop; • seven mainline positive displacement triplex plunger pumps (189 m3/h, or 832 gpm, at 14.9 MPa, or 2,162 psi, maximum output pressure); • four corrosion-erosion spools; • a control and command center. "GENERAL PROJECT DATAThe Sparvo Tunnel is part of the “Variante di Valico” project, a new 65km (40mi) highway link between Bologna and Florence, designed to renew the existing A1 route which connects Bologna and Florence. It is composed of 2 x 2,5km (1,55mi) twin tunnels excavated inside a complex geological context, characterized by the presence of a series of surface landslides and methane gas.The inclination is 2,6 % and the minimum radius is 1.400m (4.600ft). The two tunnel tubes were excavated by a 15,6m (51ft) diameter EPB-TBM from summer 2011 to summer 2013. The internal diameter of the concrete lining is 13,6m (44,5ft), made up of precast concrete segments 2m (6,5ft) long and 70cm (28”) thick, being the largest and heaviest segments ever produced. Excavation went smooth apart from initial problems concerning clogging problems and a main bearing damage of the TBM, which are discussed in a further paper during the congress. No structural damage was observed until the end of the TBM drives.SITE OBSERVATIONS DURING THE POST-EXCAVATION PHASEThe first evidence of the stress state of the segmental lining appeared on November 20, 2013 inside the northern tube (1st drive). Ring No. 884 showed obvious signs of overstress, resulting in spalling and crushing of the concrete, and the subsequent bending of the reinforcement, as visible in figure 1.At this time the tunnel invert was already partially backfilled, and construction traffic was running through the tunnel. First safety measures included closing of the tunnel, and initial measurements of the displacements."

I am Alfred E. Michon, Vice President- International for Burlington Northern Inc. I work for BN's holding company and my responsibilities include co-ordinating the activities of our eight subsidiary companies as they relate to international commerce--an increasingly important aspect of the overall business of our company. Until last April, I was President of the Fort Worth and Denver and Colorado and Southern railroads, subsidiaries of BN. Prior to that, I headed the marketing and pricing area of BN's coal transportation business. Based on my experience in the energy industry, both domestic and international, I would like to discuss the world-wide trend towards the reliance on low-sulfur coal in lieu of natural gas and oil for the generation of electricity. Since pontification on the energy scene world- wide is best left to consultants with scenario-spitting computers, let me in- stead emphasize this trend through two examples. First I will describe the regulatory and economic factors which have encouraged the replacement of natural gas by low sulfur coal as a boiler fuel in the US. BN's investments in track and equipment were instrumental in this conversion. Secondly, I will show how this same combination of factors has created a large demand for steam coal in the Far East. Again, our company's strategy is to facilitate conversion to coal overseas through timely and sufficient investments.

This paper presents the details of the Kramer tunnel project in the Federal State of Bavaria, Germany. This road tunnel, which has been under construction since December 2019, is the key component of the western bypass of the town of GarmischPartenkirchen. Once completed, it will relieve the town’s central district from enormous volumes of traffic. The contractor is the joint venture named ARGE Kramertunnel consisting of BeMo Tunnelling and Subterra. The whole project will be completed and handed over to the project owner, Staatliches Bauamt Weilheim, in 2023. The opening of the entire bypass section including the tunnel to traffic is planned for 2024. The tunnel, with a total length of 3,609 m (2.24 miles) and an estimated cost of EUR 178 million (incl. VAT), is designed as a single-tube, double-lane structure with one traffic lane for each direction of travel. The safety issue that exists for single-tube tunnels is solved by a parallel escape tunnel passable for vehicles. All tunnels and ducts are excavated according to the principles of the New Austrian Tunnelling Method (NATM). In order to reduce the geological risk for the owner, the escape tunnel was supposed to be completed before the design process for the main tunnel ever began. The exploratory tunnel was awarded to a competitor with an separate contract.

The Environment Weipa is located at 12° south latitude on the west coast of Cape York Peninsula, North Queensland, Australia. The climate is tropical with most of the 1 600 mm. average annual rainfall falling between November and March. During the short winter months (May/ August) the weather is clear and pleasant, with temperatures seldom exceeding 27°C.Throughout the remainder of the year, conditions are frequently oppressive due to consistent high ambient temperatures accompanied by high humidity. The terrain is of low relief, and generally not more than 6 to 10m. above mean sea level adjacent to the coast. The coastline features numerous short estuaries which discharge run-off from the heavy rains collected by creeks and rivers. The whole area is covered by an open forest, the predominant species of which is Eucalyptus tetradonta. These eucalypts have only limited commercial value and are heavily infested with termites. Despite the fact that Weipa is close to the location of the first reported sighting of the Australian coastline by a European (William Janz - 1606), it remains very isolated. There is no developed road or rail connection to centres of population, so that transport of practically all products, personnel and materials to and from Weipa is by sea and air. About nine years ago the President's Materials Policy Co commission under the chairmanship of William Paley released its report entitled "Resources for Freedom." Not long ago the Senate Select Committee on National Water Resources under the chairmanship of Senator Kerr released a study entitled Water Supply and Demand. Both reports contained projections of future water user the report for 1975, and the Select Committee print for 1980 and 2000, It is my task to compare the two reports. In the initial letter from Mr. Kazmann arranging for thus session he suggested that I present "a critical evaluation of the Paley Report's section on water ?ten years later,? which might include a comparison of predicted and actual water-use rates and would spotlight the assumptions which lead to errors in the predictions , . ." It is possible to compare the results of both studies, and it possible to explain art of the differences that can be found between them. The remainder must remain shrouded in doubt, since the Paley Report did not give all details of the methods used in estimating the com¬ponents of water use.

The Bullfrog and Montgomery-Shoshone deposits, near Beatty, NV, have a combined drill-indicated reserve of 15.8 Mt (17.4 million st) of ore. Average grade is 3.5 g/t (0.103 oz per st) gold and 8.2 g/t (0.24 oz per st) silver. The deposits occur in Miocene volcanic rocks that underwent large-scale movement within a detachment-faulted terrane. The principal host to gold mineralization is a strongly welded rhyolitic tuff that experienced brittle deformation during plate movement. Open space fractures and breccia were filled with quartz, calcite, and adularia accompanied by lesser pyrite, gold, and acanthite. Gold occurs as micrometer-sized particles within and adjacent to the low-angle fault and along associated listric-normal and tear faults. The Bullfrog and Montgomery-Shoshone are volcanic-hosted, detachment fault-related, epithermal quartz calcite stockwork gold deposits. Bullfrog's minable reserve is 13 Mt (14.3 million st) of ore grading 3.8 g/t (0.11 oz per st) of gold. At Montgomery-Shoshone, 2.7 Mt (3 million st) minable tons at 2.5 g/t (0.072 oz per st) are present. Preproduction stripping has begun at the Bullfrog ore body. A 7.7-kt/d (8500-stpd) carbon-in-pulp (CIP) mill is under construction. In 1990, Bond Gold expects to be producing about 6.2 t/a (200,000 oz per year) of gold.

The Indian Head fine has been in existence since 1922. For over 56 years there have been North Dakota people mining coal at this same location. The area affected will fit inside a circle 5 km in diameter, and the coal is lignite. The seam thickness has varied from 2.4 m to 3.65m, and the overburden from 6 m to 30 m in depth. me Lignite coal which is being mined at the Indian Head Mine is from 15.8 x l0 6 to 16.7 x 10 6 J per kg; it has a moisture content of 33 to 38%; an ash content of 5 to 10%; and a sulfur content of from 0.5 to 0.7%. The moisture content and the fact that we crush the coal co a top size of 14 cm x 0, are two of the characteristics or the product which cause it to freeze and remain inside of the railroad cars on the slope sheets. The market for the coal initially was almost entirely local coal sales. That is, coal was sold to people in the area who used the lignite to heat their homes, stores, offices, schools, and churches. Back in the early 1920's, there were no stoker-fed boilers, or pulverized fuel boilers, as we have in today?s marketplace. Basically, all of the coal was sold as lump coal, pieces about 30 to 35 cm square, to be used in furnaces: and nut coal with a top size of about 7.5 cm to 10 cm to be used in the old kitchen cookstove. All of the coal fines, less than 5 cm, were screened out and layed onto a pile. There was no market for the smaller sizes of the lignite coal. It has been our experience that when the coal is in lumps or chunks larger than 7.5 cm to 10 cm, there is a very slight possibility of freezing. That is the chunks themselves freeze, but by their nature, they do not cling to one another or form large masses of frozen material which are difficult to remove from the slope sheers of the railroad cars.

The Atlanta Plane Train Tunnel West Extension is being constructed at Hartsfield-Jackson Atlanta International Airport using the Sequential Excavation Method (SEM). The tunnel has shallow ground cover and mixed-face conditions and passes underneath the SkyTrain and MARTA stations and airport terminal buildings. It includes a twin tube Bifurcation with a span of over 45 feet and ground cover of 12 feet. A flat roof is being constructed at the Bifurcation to maximize rock cover and minimize surface settlements. This paper focuses on design and construction challenges for the Bifurcation and tunneling beneath the world’s busiest airport, while minimizing impacts to operations.

A compressed-water hydraulic pulse generator (HPG) for nonexplosive blasting of rock and con¬crete is described Energy is stored in the HPG by compressing water to ultrahigh pressure. A large, fastopening valve discharges the com¬pressed water, resulting in a high-energy impulse. The HPG can be used for small-scale bench blasting by discharging the water through a nozzle that is closely fitted into a borehole. A bench blasting system requires an HPG, a drill and an indexing mechanism to allow insertion of the HPG nozzle into a drilled borehole. Systems with discharge energies ranging from 40 to 250 kJ have been used to excavate hard rock, including andesite, granite and quartzite. The largest system can excavate 15 t/h (16.5 stph) of massive quartzite based on a 2-min cycle time for hole drilling. Blasting with an ultrahigh-pressure hydraulic pulse generator was demonstrated as an effective means of excavating hard massive rock under the confined conditions of a tunnel opening or underground sloping panel. Hydraulic blasting provides a continuous mechanized mining capability that is well suited for deep-level mining and urban excavation sites where explosives are prohibited

"INTRODUCTION Foam, unless intentionally created, in any industrial process presents operational problems, or at a minimum, is a nuisance. Some foam problems are unique to a given industry and others are common across many industries. A few problems foam presents to any particular industry are: (a) Poor Housekeeping, (b) Environmental impact, (c) Safety Hazards, (d) Slower Production Rates and, (e) Reduced quality of the end product. Defoamers address these issues. Sadly, the assumption in most industries is all defoamers are alike. In reality, a lot of chemistry is involved in the manufacture of even the simplest defoamer sold today and rarely are any two defoamers “just alike”. Today’s defoamers can include polymer chemistry, fatty esters, alkoxylated alcohols, silicone chemistry, silicone surfactants, ethoxylated and propoxylated block polymers, and a tremendous amount of emulsion chemistry just to name a few. This paper will discuss the various types of defoamers, defoaming mechanisms, the advantages and disadvantages of defoamer selections, and how to choose the proper defoamer for your application. Before a discussion of defoamer can be brought about, foam generation and composition must be discussed. WHAT IS FOAM? Foam is a colloidal suspension of a gas dispersed in a liquid. Most often foam consists of air dispersed in water. If a stream of air is introduced into a liquid, the bubbles produced assume a spherical shape. As the density of the bubble is less than the liquid the bubble rises. When the individual bubbles reach and pass through a surfactant free liquid they collapse. Air from the bubble dissipates and the liquid previously surrounding the bubble flows back.It should be noted a pure liquid will not foam. In the case of air dispersed in water a surfactant is necessary to make the water support a stable foam. A surfactant is a term used to describe a surface active agent; a compound with a hydrophilic end and a hydrophobic end on the same molecule."

In the last ten years the U.S. coal industry has experienced the collapse of several large silos. In addition, other problems such as extensive structural damage, small "live" capacities, inadequate feed rates, arching, coreholes, and particle segregation have been common to the industry. This paper explains the relationship between those problems and stored material properties, silo hopper configuration, silo structural design, and granular flowability. Methods of identifying, repairing, and/or modifying damaged silos, and designing new silos are given.

This paper describes the ventilation planning that was per- formed at the Barrick Gold Corporation, El lndio Mine. The El lndio Mine is located at an elevation of approximately 4,000 m in the Andes mountains east of the coastal city of La Serena, Chile. The ventilation planning program consisted of a detailed ventilation survey of the mine and the establishment of a correlated com- puter model. A series of network simulations were conducted to determine the optimum position, size, and configuration of new ventilation raises and main fan exhaust system. Modifications to existing mine infrastructure including; driving of drifts, slashing of airways, and positioning of ventilation controls were examined. Detailed costing information was developed to optimize the location of the new main exhaust raises and fan system. The final ventilation system is expected to increase the mine airflow from the existing 157 m=/s to over 315 mls. This increase was necessary to accommodate an expansion in mine production, diesel equipment utilization, and deeper mine development. Furthermore, the increased airflow was required because of the de-rating of diesel equipment and large air volumes necessary to dilute diesel exhaust at high elevation.

Geochemical studies were under- taken in the Breckenridge mining district to test the effectiveness of soil and rock-chip samples in high- lighting known mineralization in the district, and in locating sites of possible porphyry-type mineralization, if such mineralization is present in the area. Soil sampling can be done more quickly and more cheaply, but it does not provide a direct site- specific sample of mineralized rock. In well-developed soils, supergene effects may mask and/or displace primary hypogene effects; and even in places where the soils are poorly- developed, as in the Breckenridge district, the masking effects may be felt. Soil anomalies, if present, likely will be of lower amplitude and more diffuse (widespread) than corresponding rock-chip anomalies. On the other hand, detailed rock-chip surveys are more time-consuming and expensive, even though they yield direct geologic information in addition to the chemical data. The goals of the combined soil and rock-chip programs were (1) to com- pare the data from the two; and (2) test the effectiveness in identifying significant chemical anomalies with respect to the geology of the district, as it is presently understood.

Talc is an important industrial commodity due to its wide-ranging intrinsic properties; however, the presence of iron oxide hinders its industrial utilization. Herein, we report the purification of Pakistani talc through acid leaching assisted by chlorination techniques. Among various acids (i.e., HF, HCl, H2SO4, HNO3, H3PO4, Aqua Regia, H3BO3, and CH3COOH), HF was the most effectual, removing up to 67% of iron oxide at optimized conditions of temperature, 90–95 °C; time, 120 min; HF concentration of 5%; and at a pH of 1. In the second step, chlorination of the talc sample was performed separately by both wet chlorination and dry chlorination. Wet chlorination was carried out using KMnO4 and HCl in slurry phase which shown about 52% removal of iron oxide, whereas dry chlorination with CaCl2 at high temperature attained about 48% removal of iron oxide from talc sample. Further, the wet chlorination of HF leached samples was also performed under optimized conditions which showed up to 87% iron oxide removal. The original and treated talc samples were characterized by EDX and XRD analysis to monitor the changes in the elemental and mineralogical composition of the talc. It can be concluded that HF leaching followed by wet chlorination is an effective method for the upgradation of low-grade Pakistani talc ores.
