The objectives of this paper are to introduce the International Code of Practice for Risk Management of Tunnel Works and to illustrate to the participants of the 13th Australian Tunnelling Conference 2008 how the insurance industry would like to see the future of tunnelling insurance. Tunnel construction projects have suffered large losses over the years as can be shown from various loss examples and a list of major losses over the last ten to 15 years. Since the consequences of insufficient risk control had to be borne by insurers, these losses have heightened the need for improved and adequate risk management measures so that insurance cover can also in future be provided for this hazardous industry sector. Observations are made as regards to the prevailing trends in the tunnelling industry, which contribute to the increase in risk and have adverse consequences for the insurance industry. To find a solution to this dilemma various options are examined with the suggestion of a Joint Code of Practice for Tunnel Works being agreed by all stakeholders, which would become essential to secure future insurance protection for such project work. The British Tunnelling Society has, in cooperation with the Association of British Insurers, developed such a code, which the International Tunnelling Insurance Group has now adopted for global application in international markets. The content and concept of this code is explained to establish a framework of understanding and applied risk management principles for tunnel works. This initiative also has the support of the International Tunnelling Association and the International Association of Engineering Insurers. The international application of this new code is demonstrated with a few examples, which also includes Australian projects. It is the aim that this code forms in future part of the insurance coverage for tunnelling works in order to put this risk segment on a more sustainable basis. For that reason the insurance industry is seeking the support of all stakeholders of tunnelling construction projects.

If one can embrace the medical analogue, much of the mining industry currently suffers from a massive bacterial infection. When pyrite-bearing or sulfide-bearing rock formations, tailings, or mine wastes are infected by Acidithiobacillus ferro-oxidans, the likelihood of forming acid rock drainage (ARD) is almost guaranteed. The ?pharmacy? of antibiotics available is extensive, ranging from solid alkaline amendments like limestone to liquid ?medicines? such as sodium lauryl sulfate, sodium thiocyanate, waste milk, and bipolar lipids. Unfortunately, the ?geo-medical? teams of geochemists, microbiologists, engineers, and mine managers lack the tools to surgically apply these active ingredients where they are needed most with a minimum of waste. Distribution of fine grained limestone on the surface of an acidic mine waste dump is analogous to applying a bandage soaked in antacid to treat an upset stomach. The implementation of up-to-date best management practices has not healed the patient; an equivalent combination of hypodermic needle, cyber knife, and arthroscopic probe is clearly needed. Using an engineered, flow-able or pumpable foam or pHoam? as the medicinally analogous ?dextrose delivery solution? for solid and/or liquid ?geo-antibiotics?, the authors have combined off-the-shelf technologies that have been previously applied in solving geotechnical problems in the mining industry. A patent for the innovative process is pending. This paper discusses method concepts and the advantages it could provide over conventional BMPs.

Mr Kapp's paper helps considerably to clarify the subsidence/mine geometry relationships in New South Wales. It is in the best tradition of the presentation of field investigations such as has built up internationalknowledge over the years.The Author has plotted in Fig. 10, the maximum subsidence developed over each of a number of panels of different width/depth ratios and concludes that the maximum possible subsidence over a super critical extractionarea is around 0.65 of the mining height and that this is reached with the critical width/depth ratio of extraction of around 1.5.Although, as Mr. Kapp says, the maximum possible subsidence in the UK can reach 90% of the seam thickness extracted the more usual figure for the first seam is up to 80%. It has also been found in the UK that with shallow workings such as those at Kemiro Coliery the critical width of extraction is 2.0 or more times the depth.

Bituminous coal mining in the United States is currently experiencing a period of rapid growth which began in the early 1960's. Future demands are conservatively predicted to rise to 700 million tons by 1980 and to anywhere between 1 to 3 billion tons by the turn of the century. This means a continuous expansion of coal production by a factor of at least two can be expected before the end of the century. Over the last few years, production from underground mining has decreased; yet it is about 50% of the total coal output. In the absence of any discovery of near surface coal deposits, and even after conceding that some deeper deposits can be surface mined with changes in technology it can be reasonably assumed that coal production from underground will continue to play an important part in solving the energy crisis.

Laboratory and pilot plant investigations aimed at recovering gold as an electroformed sheet from gold eluate and gold leach solutions and slurries are discussed. The electrolytic test work undertaken makes use of high surface area electrodes, in particular nickel-chrome foam metal and activated carbon, as well as ultrasonic activation. The process involves initial gold electrowinning (or adsorption onto activated carbon), followed by gold electroforming, both operations being carried out in a specially designed reactor. The pilot plant work mainly involved the treatment of gold eluate solutions from a South African gold mine containing 600 mg/L Au: some tests were also carried out on gold leach solutions containing 6 mg/L Au. In the former case, very efficient gold extraction onto the foam metal cathode was achieved using electrolysis combined with ultrasonics. The polarity of the gold-loaded cathode was then reversed and, using a replacement electroforming electrolyte, gold was electroformed in situ onto a polished stainless steel mandrel, from which gold sheet was subsequently removed. In the case of leach solutions, pilot plant tests confirmed laboratory findings that in the presence of calcium (resulting from the lime used during the leaching process) no electrowinning is possible. Laboratory tests showed, however, that when sodium (in the form of NaOH or Na2CO3) is added to the leach solution, electrowinning can indeed take place. Alternatively, if a special activated carbon electrode is used (without current), the non-ionic calcium aurocyanide complex is adsorbed on to the activated carbon. When a gold-loaded activated carbon electrode is made anodic in a gold electroforming electrolyte, gold electroforming is feasible, although the efficiency is very low. The possible implications of these developments for gold recovery are further examined.

Helium was in abundant supply after August, and the informal allocation system which had been in use intermittently since 1955 was discontinued Delivery of 26 new tank cars in August and September increased the total in the tank-car pool to 163. Defense and space agencies and the Atomic Energy Commission continued to use more helium in their operations and research.

Zarshuran gold deposit, with the total reserves of some 12.5 million tons of 4.18 g/ton ore, is considered to be an important gold mining project in the area. In this paper the feasibility studies of the whole project is carried out on the basis of several economic variables. Given that mining data are often of uncertain nature, these economic variables have therefore been estimated under the conditions of uncertainty. The resultant Net Present Value is hence also obtained under the same conditions. For this purpose, a model is first devised with the help of Excel and COMFAR software. The model is then developed that considers different scenarios which would result in different expected values for the economic variables. It is concluded that the NPV of the project is most sensitive to gold and silver price and that other variables such as discount rate, operating and capital cost all affect the feasibility of the project with lower degree of severity. Finally, risk analysis is also carried out using Mont Carlo technique, in order to estimate the most probable value of NPV. The average NPV calculated in this way is US$ 49.7 Million, whilst without due consideration of risk, this would be US$ 36.6 Million. The techniques used, together with the procedures adopted in this paper, can be used in feasibility studies of all mining projects. Such decision would result in more accurate Net Present Values than expected from other frequently used methods.

Wettability of graphite by synthetic SiO2-Al2O3-CaO-MnO slags with varied MnO content and by industrial slags was studied at 1350-1600 °C using sessile drop method under argon atmosphere. Initial contact angle was the lowest (110-115o) for the MnO-free slag. For slags with 10-40 wt% MnO, the initial contact angle was in the range 125-135o. The contact angle decreased with the contact time. The change in the contact angle with slag mass was found insignificant. The surface tension of slags obtained from the drop shape analyses was 500-800 mN/m and varied in a narrow range with temperature and MnO concentration. The reduction rate increased with increasing MnO content in the synthetic slag from 20 wt% to 40wt% and temperature. Increasing temperature also increased the rate of reduction. No correlation was observed between the reduction rate and MnO content in the synthetic slags in the range 0-20 wt% and in the industrial slags. The rate of SiO2 reduction from the molten slag was close to the rate of MnO reduction. The MnO-free slag had the largest contact area with graphite substrate. "The paper describes in general the objectives, goals as well as theoretical background of "METSI"-project proposal submitted to German BMBF (Federal Ministry of Education and Research). The METSI project is a starting point for technology and know-how transfer of German water management and water treatment technologies to South African Mining industry. "METSI" is Setswana, one of the eleven official languages in South Africa, and means "water". It stands also for "Management System for the Implementation of Environmentally Sound Water Supply Technologies in South African Mining Areas with Involvement of all Relevant Stakeholders". The project focuses on development, design and implementation of mathematical models that consolidate and evaluate existing data and generate substantiated projections of "water" amount and quality. METSI covers a wide range of scientific, technical, social as well as economic questions related to "water in mining"."

The final objective of a mine ventilation project is to find a way of supplying the working faces with the necessary quantity of air to dilute all the possible air contaminants to safe limits and to maintain the temperature, humidity and oxygen content within desirable limits. To accomplish the aforesaid, decisions have to be made at early stages of planning and design of a mining operation. That is, ventilation and mine planning are closely related and interdependent, and experience has proved that improvisation as related to mine environment is rarely optimal in ventilation engineering. Mine management should be concerned with two general aspects of ventilation problems. The first is related to the statuatory requirements which are usually the minimum standards that have to be maintained. During the last two decades, mining laws have become increasingly stringent and management of operating mines had to face the effects of these externalities. In some cases these required either a production rate reduction or additional mine development. The second aspect that management should not overlook is the technical and economical importance of mine ventilation on the overall mining cost. In coal mining it is necessary to move, on an average, five tons of air through the mine for each ton of coal produced. The power cost incurred is perhaps the single most important direct cost assigned to ventilation but it is not the only one. Other costs that should be considered include the supervisory labor cost, fan depreciation and maintenance, doors, stoppings and regulators construction cost and maintenance, the additional mine airways development and maintenance because of ventilation requirements. DURING the last several years, the U-Zr system has been studied at many laboratories as one of the research programs sponsored by the Atomic Energy Commission. The equilibria in part of this system have been difficult to determine, although the phase diagram from 0 to 60 atomic pct Zr has been established with reasonable certainty. Existing diagrams of the system vary appreciably beyond 60 atomic pct Zr, particularly with regard to the existence of an intermediate phase. Most existing diagrams show a transformation or reaction horizontal near 600°C. In 1955, a collection of uranium phase diagrams' was prepared at the Battelle Memorial Institute and the U-Zr diagram in that collection contained a questionable region in the vicinity of 60 to 80 atomic pct Zr. Summer-Smith" has published a phase diagram of the system that shows no intermediate phase. Summer-Smith did his work with powders and found only the uranium and zirconium eutectoid mixture from X-ray patterns of material annealed below 600°C. Mueller3 at Argonne National Laboratory did similar work with filings, and he found that initially there was a pattern of an intermediate phase, but on long-time annealing

This work has as its objective to characterize ceramic bodies for vitrified tiles elaborated with the mixture of nepheline-syenite flux and kaolinitic clay. Formulations were prepared with 0, 30 and 50 wt.% of nepheline-syenite incorporated in a kaolinitic clay. To determine the physical and mechanical properties such as dry bulk density, linear shrinkage, water absorption and flexural rupture strength, specimens were prepared by 30 MPa uniaxial pressure-molding and then fired in a laboratory furnace at 1150oC. The microstructure of the fired ceramics was evaluated by scanning electron microscopy. The results indicated that the nepheline-syenite improved the evaluated properties of the clay and attended the specifications of vitrified ceramic tiles.

A seismic monitoring network has been installed in western Colorado (USA) in the vicinity of three underground coal mines to (i) distinguish and characterize seismic activity as either mining related or naturally occurring, (ii) implement a real-time event monitoring and notification tool, and (iii) collect data for use in research studies aimed at quantifying impacts from mining-related and natural seismicity. These potential impacts include dynamic rock mass failures such as coal bumps as well as strong shaking in the vicinity of critical structures such as impoundment dams, reservoirs, mine seals, mine openings, and steep slopes. Examples of two damaging seismic events are presented and the mining and geologic factors attending these dynamic failures are described.

Using mines in Canada, Boart Longyear, a leading provider of drilling services and products, has put its Stage 3 Coring Bit to the test and each time the bit has passed the test with great success. After more than a year in the field at a major mine operation in Sudbury, Canada, the Stage3 coring bit broke life records to become recognized as the most productive design in the industry. In addition to the proven advantage seen in regular bit life increases of as much as 300 percent in some cases, the design is quickly being recognized for its cost-saving operational impact. In Sudbury, the mine operation realized actual cash savings of more than C$45,000 within an eight-month period by making the switch to Stage3 bits. For years, the Sudbury Basin team has relied on the Alpha Extended Crown bit with the Express Waterway geometry option, ordering approximately 500 units per year. This 07 COM ECEX configuration performed well in medium-hard, non-abrasive to ultra-hard, non-abrasive formations with an average bit life of 150 m (490 ft). While satisfied with the performance of the Alpha Ex-tended Crown, the team agreed to put the new Alpha Stage3 to the test. Boart Longyear?s field engineers supplied five drill sites with Alpha Stage3 bits in a 07 COM configuration. After thoroughly testing the Stage3, the results were clear. Even with the well-known performance and versatility offered by the previously preferred configuration, the Stage3 set a new standard of bit performance. The new bit maintained its cutting edge for up to three times longer than the Alpha Extended Crown, a significant performance increase when compared to the 07 COM ECEX.

Kidd Mine in Northern Ontario is now producing from Stage 1 of the Mine D expansion project ? mining between 2100 m and 2700 m below surface. Planning for Stage 2 of the Mine D project is already well advanced and will see the mine extended potentially to 3100 m below the surface. To ensure adequate ventilation is maintained at such depths, two large axial fans have been installed on the 6000 Level (1800 m below surface) as exhaust booster fans to operate in series with the surface exhaust fans. The Kidd Mine 6000 Level Booster Fans are a unique installation with unique and often unexpected challenges. These challenges have been met and attacked through the combined efforts of mine and fan manufacturer personnel with a common focus.

We learn very early that stories entertain us, but also give us the ?rules? about what is expected of us and how to make meaning of our experiences. Stories are integral to our lives, and have great power to change or influence how we think or how we react. They are an innate part of how we learn, and have obvious benefits to trainers or educators who are trying to impact what and how we learn. This paper and presentation discuss the social and cultural power of stories, and how they have been used to develop training for skilled blue-collar workers. Included will be examples as well as suggestions for trainers on where to find stories, what type of stories to pay attention to, and how to use them to improve occupational safety training.

In this study, a computer program to design tunnel blasting pattern has been developed. The program consists of two parts; one is for tunnel blasting pattern design and the other is for blasting modeling to estimate the peak particle velocity, the distribution of fragmentation and the excavation damage zone. We modified the design method of tunnel blasting pattern suggested by Langefors because it provided undesirable pattern in blasting practices such as considerably large center cut and too large burden for Vcut as drilling length increased. As a result, the burden and spacing were reduced to practically appropriate amounts. In addition, the correlation between rock mass rate, RMR, and rock constant in blasting, c, was analyzed based on the data collected from twenty three tests of tunnel blasting. It was concluded that the correlation between them was fairly good enough to be applied in cut design. In order to check the validity of the modified methods and their practical applicability, test blasting was carried out at two different tunnel construction sites in Korea. The results were satisfactory in that the average rate of advance was 90% and the overbreak did not cause additional support. Futhermore, the developed program is capable of estimating peak particle velocity by using (a) the existing vibration equations, (b) the vibration equation obtained by test blasting to check out the practical applicability of the designed blasting pattern. Feedback is implemented into the program to adjust the designed blasting pattern and control the vibration.

It is interesting to study the noise standards that have been promulgated in the US over the last decade or so. These regulations are likely to have long-lasting impact. The Occupational Safety and Health Administration (OSHA) Hearing Conservation Amendment (March 1983) continues to have influence not only in the workplace but in the debate over new regulations. Both the Mine Safety and Health Administration (MSHA) and Federal Railroad Administration (FRA) regulatory preamble documents state the desire to be consistent with OSHA. An examination of Table 1, which compares the major components of the three regulations and the 1998 NIOSH “best practices” criteria, depicts the extent to which that intent is met. There has been some regulatory activity in the last decade which may give some hope for evolution and updating based on the wealth of science that has occurred during the last quarter of a century since the OSHA regulation was enacted. However there has also been some “back-sliding” toward more lenient standards. The MSHA noise standard made regulatory progress in September 2000 by emphasizing engineering and administrative controls, followed by personal protective equipment, in the hierarchy of noise intervention. MSHA’s requirement for technician certification (today only available from CAOHC) strengthened the training requirements for audiometric testing in hearing conservation programs and MSHA also added the requirement of dual hearing protection at 105 dB TWA. There were many subtle differences between OSHA and MSHA based on comments and a desire to clarify some of the vague aspects of the OSHA noise standard, and meet the needs of the regulated mining industry. One example pertains to the ceiling for exposures. OSHA says, “no exposures >115 dBA,” which is interpreted to mean no unprotected exposures above that level, giving credit for the assumed effectiveness of hearing conservation programs, hearing protection devices and administrative and engineering controls. MSHA specified that a “P” code1 violation be issued for any protected or unprotected exposures >115 dBA. The Federal Railroad Administration (FRA) noise standard for railroad operating employees, which went The Modern Evolution of Hearing Conservation Regulationsinto effect February 26, 2007, was expressly based on the OSHA standard but also uses MSHA for comparison. The preamble states that the FRA defers to OSHA as the “primary regulator of noise in the workplace,” but also acknowledges the need for some departure from the OSHA regulation (FRA Preamble II.B). As an example, FRA requires testing at 8000 Hz “because it will allow employers to identify hearing loss sooner.” The FRA rejected MSHA’s hierarchy of noise controls in favor of requiring specific engineering interventions and focusing on appropriate hearing protection which would still allow communication and audibility, and identification of excessive noise through employee-filed “excessive noise reports.”2 Where OSHA has no specific mandate requiring employees to take advantage of the employer-paid audiogram, it has been traditionally a condition of employment and is generally accepted that OSHA-covered workers require an annual audiogram. MSHA addressed this issue in its preamble; however, they made no significant change. MSHA employers are required to offer annual audiograms but MSHA stopped short of requiring employees to comply with annual audiometric testing. The MSHA preamble does allow that mine operators can also make audiometric monitoring a condition of employment. FRA requires employees to
