In May of 1980, a mine operated by Belmoral Mines Ltd experienced a failed crown pillar, resulting in the death of eight miners and putting sixteen others in harm?s way. The Ferderber mine was located in the Val d?Or region of the province of Quebec in Canada, and had been under development for 19 months and production for 9 months, since the opening of the portal. The crown pillar separating the 1?7 level from overlying overburden ultimately failed, permitting an estimated 1.5 million cubic feet of liquefied sediment to gain entry into the mine. This paper back-analyses the cave-in by applying decision error theory, in an examination of the organizational culture and the collective decisions contributing to the cave-in disaster. A case will be made for a greater understanding of mining methodology, geomechanics and risk assessment when operating in such challenging geologic conditions. Boundary condition and decision (BCD) analysis will be introduced as an innovative new method for the analysis of mine accidents and incidents. It will also be shown that BCD analysis can be applied to any event in an enterprise for which standards, norms or legislation exists. A cognitive profile will be presented that provides insight into the safety ethos existing within the Ferderber mine organization at the time of the tragedy.

In this study, the possibility of beneficiation of chromite tailings in the Uckopru/Fethiye-Turkey region by a combination of hydrocyclone and multigravity separator (MGS) was investigated. The two significant operational parameters of hydrocyclone, which were diameter of the apex and diameter of the vortex, and the three significant operational parameters of the MGS, which were drum speed, tilt angle, and wash water, were varied and the results were evaluated with the central composite rotatable design. In order to produce a chromite concentrate by hydrocyclone and MGS, mathematical model equations were derived by computer simulation programming, applying the least-squares method using Minitab 15. Second-order response functions were produced for the grade and recovery of chromite concentrates.

The ability to perform three dimensional imaging at micron-scale resolution of real mineral floc sediments and structures using commercially available Micro X-ray Tomography (XMT) instrumentation can be used for the analysis of aggregates and granules, liberation and dewatering of sediments. This paper presents are view of recent work by the authors relevant to the mining and minerals industries. It covers three main topics: X-ray imaging of agglomerates and multi-component mineralic particles,, permeability of sediments, and dissolution of granules with potential application to heap leaching.

A retrospective study was conducted on the Rand Mutual Assurance (RMA) noise-induced hearing loss (NIHL) compensation claims from 1998 to 2008 to determine if the 2008 milestone agreed at the 2003 Mine Health and Safety Summit was achieved. The number and costs of NIHL compensation claims in different commodity sectors and workplaces were collated. A secondary analysis of the ages of employees compensated after 2008 was conducted. A complementary retrospective analysis of audiogram data investigated the percentage loss of hearing (PLH) shift in different homogeneously exposed groups and occupations at two gold mines. The compensation claims analysis indicated a significant decrease in NIHL claims from 1998 to 2008, but the milestone was not achieved. The reason may be either that claims have not been submitted timeously as required by Instruction 171 and that the current submissions are a result of pre-2003 noise exposure, or that employees who were baselined are still developing NIHL owing to ineffective hearing conservation programmes in place in the mining industry. On the basis of best practice for hearing conservation, recommendations are made for leading indicators in hearing conservation programmes and for reducing the risks of NIHL in order to achieve the 2013 milestone.

The aim of this research was to investigate the effect of coke particle size on the thermal profile of the charge bed in the sintering process of iron ore. Six different ranges of coke particle size that could be used in practice in the sintering line of Esfahan Steel Company (ESCO) were used while parameters such as ratios and particle sizes of other ingredients, ignition temperature, and suction power were kept constant. Thermal profiles were obtained by measuring the temperature changes at three points in the sintering bed. The influence of coke particle size on rate of temperature rise, maximum temperature of the sintering bed, and the flame front velocity of the sintering bed was determined by analysing the thermal profiles. Moreover, the cold strengths of the sinters were measured using the Tumbler method. The results indicated that the coke particle size has a significant impact on the thermal properties of the sintering bed. Based on the results, a coke particle size between 0.212 mm and 3.350 mm was determined as the optimal range for the specific conditions in the sintering line of ESCO. Limestone formations in the United States can be subject to relatively high horizontal stresses due to the existence of tectonic loading of the limestone strata. Underground limestone mines use the room-and-pillar method, in which 12- to 18-m-wide rooms are typically excavated. The stability of these excavations can be compromised by the horizontal stress, resulting in a rockfall hazard. Rockfalls are the cause of 15% of all reportable injuries in underground limestone mines. Horizontal stress related damage can occur in the form of guttering along one or more sides of an excavation, roof beam buckling or oval shaped roof falls, with the long axis perpendicular to the major horizontal stress. Numerical analyses show that the pillar layout and orientation of the mine workings have an effect on the horizontal stress distribution within the roof. The effects of high horizontal stresses can be mitigated by orientating the heading development direction parallel to the maximum horizontal stress, reducing the number of cross-cuts and offsetting the cross-cuts to limit the potential lateral extent of horizontal stress related roof falls. The modeling approach described in this paper can be used as a tool to evaluate potential roof failure and optimize the stability of room and pillar layouts.

P-wave velocity measurements were carried out on 22 large limestone blocks in a stone processing plant. In the laboratory, P-wave velocity of core samples was measured to obtain intact rock velocity. The square root of the ratio of the velocity of the large stone block to the velocity of the core specimen was called the velocity ratio index (VRI) and a quality classification for large building stones was suggested. In addition, the applicability of the established classification to stone cutting with gang saws was checked. It was concluded that the quality classification and estimation of slab production efficiency of the building stones can easily be made by ultrasonic measurements. However, the study was carried out only in limestone. Further study must be conducted to check the validity of the obtained results for the other rock types.

Ground water and associated pore pressure represent elements which can have a negative impact on surface mining and slope stability. Ground water flow and deformation within a slope influence each other. An increase in pore pressure will result in a decrease of effective stress; and conversely if the pore pressure decreases, the effective stress increases. Slope analysis and slope design calculations require good information on pore pressure distribution around an open pit excavation. Until recently very little pore pressure data was available and assumptions had to be used. Location and design of measuring points depend on geological and structural setting, geotechnical domains and practicality. Good planning and the use of sensitive point piezometers to measure in situ pore pressures means that real-time data can be collected and used for the construction of flow nets, the determination of pore pressure distribution around an open pit mine, the validation of the dewatering/depressurization requirements, the calculation of the efficiency of the dewatering system and finally the use of pore pressure data in slope stability analysis. The recommended layout for a monitoring system, types and design of piezometers, data collection and use of the pore pressure data are presented.

Abstract Several zones with expansive rocks were encountered during construction of two tunnels of the water conduction system for the Chacabuquito Hydropower Plant, located in the Aconcagua river valley of the Chilean Central Andes Mountains, at elevation 1.075 m a.s.l., and 65 km north-northeast of Santiago. The expansive behavior was associated to three different geological features. Firstly with the crossing of important north-south trending faults, with montmorillonitic and caolinitic fillings. Secondly to the presence of zones of rock mass showing a penetrative presence of clay minerals (montmorillonite, illite and caolinite) and penetrative thin veins of ceolites. And thirdly to the hydrothermal alteration of the rock and the consequent generation of clay minerals (predominantly montmorillonite, sericite and minor caolinite). The geological and mechanical characteristics of the expansive rock domains, as well as the field and laboratory reconnaissance and characterization program, are described in the paper together with the bases for design and the guidelines for construction and definitive tunnels support. Resúmè Pendant l’excavation des deux tunels du système d’adduction d’eau pour alimenter la centrale hidroélectrique Chacabuquito, plusieures zones de rochers expansifs ont été découvertes. Ces tunnels sont dans la vallée du fleuve Aconcagua de la Cordillère des Andes du Chili Central, à une altitude aproximée de 1.075m s.m.n. et à 65km vers le nord-nord-ouest de Santiago. Cette conduite expansive est associée à trois différentes caractéristiques géologiques du massif rocheux. Un premier lieu, à l’intersection de plusieures zones de failles, d’orientation nord-sud et subverticales, avec remplissage de rochers désintégrés, montmorillonite, caolinite et sérite. En seconde lieu, a la présence de zones de rochers expansifs, avec des nombreux minces veines de céolite-calcite et aussi des minéraux argileux (montmorillonite, illite et caolinite). Et, dans troisième lieu, dût à la présence de zones d’altération hidrothermale du massif rocheux et la conséquente génération de minéraux de remplacement argileux (surtout sérite, montmorillonite et en plus petit degré, caolinite). This paper summarizes the characteristics of Southern African coals, presents a fundamental approach to the combustion performance of coal, and reviews some of the recent developments in the fields of technological evaluation and boiler design for coals typical of this region. This includes a summary of the petrographic constituents of coa~ highlighting the differences between the Palaeozoic coals of the northern and southern hemispheres (Laurasia and Gondwana respectively), and the effects of these properties on the combustion performance of coal as seen in terms of laboratory, pilot-scale, and full-scale operating conditions. Recent developments in boiler design linked to advanced characterization studies and commercial experience have led to unprecedented use, with maximum effect, of the low-grade, high-ash, high-inertinite coals typical of some parts of this region. In the light of this success, more adequate understanding and better methods of assessment of the coals typical of the Gondwana region are called for, particularly in the light of the price premium for high-grade coal and the more cost-effective lower grade coals on the international steam markets.

Backfilling at Vaal Reefs during the experimental phase has been carried out using cyclone-separated tailings which are transported underground hydraulically by means of a borehole. The selected tailings are piped from the borehole to the stope and then, after the addition of a flocculant, deposited in place via single or multiple nozzles. The successful performance of the backfill/stope system relies not only on the actual quality of the placed material, but also on the effective operation of stope filling. The success of stope filling relates to the complex, time- dependent interaction of backfill, stope and barricade materials, and the transport and placement procedures used. A comprehensive laboratory testing programme was carried out on backfill materials at Vaal Reefs in conjunction with the experimental stope- filling operation. The range of testing techniques corresponds to the requirements of each stage of the backfilling process. Where necessary, new or alternative techniques were sought during the course of the experiment to satisfy and facilitate better understanding of the different processes. These techniques comprise both routine quality control testing and research-oriented testing. The laboratory testing techniques appropriate to each stage of the backfill process are briefly discussed. This paper presents the main conclusions of the study and identifies areas for further research.

Since the dawn of mining, pillars have been used as primary support to ensure stable workings. Early designs were based on trial and error, after which more scientific means developed over time. A vast amount of progress has been made, especially in soft-rock room-and-pillar design methodologies, from which hard-rock design theories developed with minor changes to constant parameter values. The commonly used Hedley and Grant method for hard rock and Salamon and Munro methodology for soft rock draw on the tributary area associated with the pillar, the width-toheight ratio of the pillar, and a back-analysed strength reduction factor. In these methods, only the vertical stress, or stress normal to the pillar influences the load applied to the pillar. This investigation considers the possible influence of shear stresses on pillars in a room-and-pillar layout in single reef planes and multi-reef environments, based on elastic numerical modelling methods. The possible shear stress poses a safety and financial risk to the design process, whereby an undersized pillar would lead to unstable working conditions, whereas oversized pillars could lead to an under-utilized ore resource.

This investigation compares the gas adsorption behaviour and capacity of three bituminous coals from South Africa, of similar rank, by assessing the characteristics of the raw coal, as well as the resulting float and sink fractions (at 1.80 cm3/g) obtained by density separation of crushed coal samples. Calculations were also made to obtain the raw coal gas storage capacity from the weighted contribution of both float and sink fractions results, thereby permitting comparison with the analysed results of the raw coal. The study demonstrated that the clean fraction of a coal has the highest capacity to retain gas in the sorbed state, followed by raw coal, and lastly the sink fraction, and re?confirmed previous investigations that showed minerals to be inhibitors of gas adsorption and retention.
