INJURY DATA and experience at coal mines in the United States for 1956 and 1956 are presented in this publication with a minimum of interpretation and comment because of the numerous tables required to afford detailed comparisons between the 2 years. Corresponding table., for 1955 and 1956 follow in consecutive order under the following general sections: (A) General injury experience: Tables I to II: contain current data on number of fatal and nonfatal injuries, frequency and severity rates, and average severity. (B) Selected injury data: Tables 12 to 77: Contain current data on causes of injuries, distribution of injuries by part of body injured, clays of disability, extent of hand and machine loading, experience by employment and production groups, and various other operating factors and physical conditions related to the occurrence of injuries at the Nation's coal mines. (C) Injury experience, by States: Tables 78 to 87: Contain data on injuries, injury rates, employment, cause of injuries, aid severity rates by States. (D) Major disasters: Table 88: Contains data on major disasters, 1947 -56. A major disaster is defined as a single accident in which, or as the result of which, five or more persons are killed instantly or died at any time thereafter. (E) Historical coal-mine-fatality experience: Tables 89 to 93: Contain data on fatalities and their causes at bituminous-coal and anthracite mines from the earliest records through 1956.

The papers contained herein summarize results of various recent Bureau of Mines investigations in the area of mine power distribution systems. They are based on information from Bureau personnel, contractors and grantees, and Mining Enforcement and Safety Administration. Conclusions drawn from a series of parameter measurements on mine power systems, taken over a 3-year span, are presented. Personnel and equipment hazards due to faults on isolated, solidly grounded, and resistance grounded three-phase systems are discussed. Two papers cover dc applications of circuit breakers, including a novel protection scheme for trolley lines. The methods of suppressing induced arcing currents and voltages in a-c machines are evaluated. An evaluation program for ground monitors is described. Acceptance test procedures and the importance of various paparameters for trailing cable splice are covered. A number of techniques for locating faults in trailing cables are described.

The steeply dipping silver/lead/zinc orebodies at Mount Isa mine have been extracted most recently by a combination of cut-and-fill and open stoping methods. Bench stoping was first used at Mount Isa Mine seven years ago to allow safe extraction of a highly stressed cut-and-fill stope. Subsequent benches have shown that the method offers significant cost and safety benefits through increased productivity and reduction in man exposure. The success of benching has lead to cut-and-fill mining being replaced at Mount Isa.Significant resources have been focussed to optimise benching. This paper describes the operational, planning, and research and development aspects which have contributed to the method to date. A brief look is also taken at the future direction of benching at Mount Isa.

An instrument using a gamma-ray backscatter technique for determining the weight-percent incombustibles content of coal dust and rock dust mixtures in coal mines has been built and tested. The unit gives a direct readout in per-cent incombustibles for coal mine dust mixtures containing between 50 and 100 percent incombustibles; calibration of the device requires using a sample of the rock dust employed; this is usually limestone but occasionally dolomite or gypsum. A small americium-24l source is used with a scintillation detector whose output feeds a rate meter. The source-detector geometry used was chosen to reduce the effects of variation in bulk density. Accuracy of the system is within ±3 percent for most mixtures. The instrument is portable and completely self-contained; the rechargeable batteries supply power for 300 to 400 determinations.

Iran's petroleum industry is nearly 50 years old and the oldest in the Near East. According to published figures, Iran ranks third in production and reserves in the Near East. These reserves (including probable reserves) are estimated at 35 billion barrels and constitute about 11 percent of the world's total. Iran is the sixth largest producer of crude petroleum in the world. The 1961 output of more than 430 million barrels was 5 percent of total world output for the year. In the same year, Iran ranked fourth among world petroleum exporters, providing 8.4 percent of total world exports.

"SRI International has developed and patented* a castable dilute explosive tile @ET) that offersunique advantages over conventional high explosive. The primary advantages of DET are that itsdetonation pressure can be adjusted over a wick range and its impulse can be controlled independently by adjusting its thickness and configuration. Because DET has a much lower detonation velocity than conventional explosives, the pulse duration produced by DET is as much as 10 times longer than the pulse duration produced by conventional explosives for equal explosive weights. These features of DET present several advantages to the mining industry. For example, DET can be tailored so as to control fragment size and distribution to enhance extraction of desired elements from ore or breakup of specific regions."

The ability to detect and quantify ground movements in underground mine workings is of the utmost importance to the safety of miners and continuity of operations. Rib sloughage is one of the most common forms of underground deformation. Remote sensing techniques, such as laser scanning, can be used to quantify this rib sloughage, by recording precise, timelapse point clouds of mine workings. In order to determine sloughage volumes and locations, two sets of laser scans were performed 42 days apart at a working section of an underground limestone mine. During this period, significant sloughing and scaling occurred, allowing for differentiation between the initial and final point clouds. Volumetric changes of 2.3-2.6 m3 were detected on rib faces during the time-lapse study. In addition, displacement in the mine roof was, as expected, not detected, suggesting well surveyed scans and an appropriate scan resolution.

This paper provides an overview of the patenting process as it pertains to the fields of metallurgy and mining, and summarizes some general patenting strategies. A review of new "green technology" initiatives recently adopted by Patent Offices around the world is also provided.

Many mine samples of coal are analyzed each year in the laboratories of the Bureau of Mines. The analyses are made in connection with investigations relating to fuels belonging to or for the use of the United States Government, the causes of accidents in coal mines, the geologic relations of coal beds, and the character and the value of the coal and lignite in the public lands. The systematic collection and analysis of such samples in connection with fuel investigations was begun in 1904 at the Government fuel-testing plant at the Louisiana Purchase Exposition, and was under the direction of the United States Geological Survey for six years. Descriptions of the coal samples collected between the beginning of this work, July 1,1904, and the transfer of the fuel-testing and mine-accident investigations to the Bureau of Mines, July 1, 1910, were compiled and published in Bureau of Mines Bulletin 22.a Descriptions and analyses of samples collected during the fiscal years 1911 to 1913 were published in Bureau of Mines Bulletin 85.b The present bulletin presents analyses and descriptions of samples collected during the fiscal years 1913 to 1916. In order that the material in this bulletin may be used to supplement that presented in Bulletins 22 and 85, the same plan of geographical classification has been followed, the analyses and descriptions of the samples being grouped in alphabetical order according to the State, county, and town near which the mines or prospects sampled are situated.

"Upon successful commissioning of the Stage Grinding Project (Cormier et. al., 2002), process improvements predicted through pilot plant testing were obtained in several flotation circuits, resulting in higher grades and recoveries for zinc and lead concentrates. Subsequent to establishing new baseline operating conditions from the Stage Grind Project implementation, additional opportunities were identified, and through the application of Six Sigma methodologies further gains were made. Six Sigma improvement projects were carried out in the copper-lead, zinc, lead upgrading and bulk flotation circuits. The overall performance increase resulting from incremental improvement projects has been substantial.Most remarkably, given the operational maturity up to the late 1980’s, the Brunswick concentrator team has continued to deliver a steady stream of major improvements over the past fifteen years. While senior managers and their programs have changed several times, there have been some critical constants over the long run. Common elements have been a commitment to team work, a commitment to recruit and maintain technical personnel, a structured business improvement framework supported organizationally by senior management, and focus on a few priority initiatives rather than trying to fix everything at once.The paper briefly discusses the longer term setting and then endeavors to illustrate, with examples, how improvements are presently achieved by applying the Six Sigma quality program.INTRODUCTIONThe Noranda Inc. Brunswick Mine, located near Bathurst, New Brunswick, is one of the world’s larger lead/zinc mines. Milling of ore began in 1964 from the No. 12 orebody, with an expansion in 1966 to accommodate ore from the No. 6 mine. In excess of 100 million tonnes have been milled to date. The concentrator employs differential flotation to produce lead, zinc, copper, and lead/zinc bulk concentrates. The ore is massive sulphide, with approximately 20% valuable minerals, 60% pyrite and 20% silicates."

This paper documents a field survey using long-range terrestrial digital photogrammetry at the Palabora open-pit mine, South Africa, for multi-scale characterization of rock mass discontinuities. Stereomodels covering the entire mine were generated based on photographs taken from a distance of 1600m across the pit, using a series of f=20-400mm lenses. Details on the methodology used and its accuracy are discussed. Preliminary results on discontinuity orientation, persistence and intensity are presented, highlighting the effect of observation scale. The potential uses of long-range terrestrial digital photogrammetry in open-pit mine and large natural rock slope environments is emphasized. The use of an f=400mm lens from distances larger than 1.5km may represent an important step forward in the geotechnical characterization of inaccessible remote rock faces.

Mineral production was of little significance in the country's economy, and Bangladesh was not a world-ranked producer of any mineral commodity during 1977. Natural gas was the most important mineral exploited; however, reserves were sufficient to allow for much greater production, Economically, Bangladesh made some progress in 1977-782 in its struggle against poverty, underdevelopment, and rapidly increasing population. The gross domestic product (GDP) grew 8%, mainly owing to a 13% increase in food grain production, the key economic indicator in this highly agricultural nation. GDP at current prices dropped slightly in Fiscal Year (FY) 1976.77 to $6.6 billion3 and rose to $7.2 billion in FY 1977.78. The population rose from 81 million in FY 1975-76 to about 85 million in FY 1977-78, resulting in an increase of per capita GDP from $83 to $85 during the same period. The high annual population growth rate, over 3% in 1976 and about 2.8% in 1977, was recognized by the Government as the main problem facing the country. Average density was 525 people per square kilometer. A similar density would have the entire world population living within the continental United States.

This paper describes the successful management and construction of the 5.18 m (17 ft) diameter, segmentally lined, North Dorchester Bay CSO storage tunnel in Boston, MA. This 3,300 m (10,830 ft) long tunnel was mined through variable soft ground with only 6 m (20 ft) of cover, using an earth pressure balance tunnel boring machine. These extensive data collected from the projects unique automated and internet accessible geotechnical and TBM monitoring system, will be examined and used to explore the relationships between geological conditions, TBM operating parameters and measured settlements. The paper will also discuss the use of screw revolutions as an indicator of muck volume.

Hydraulic mining has been practiced in the United States since 1852 and many mineral commodities, such as gilsonite, phosphates, lead-zinc, silver ores and St. Peter Sandstone can be mined in this manner. In 1958, the U. S. Bureau of Mines initiated research into the mining of coal using these techniques.1-4 These early investigations were conducted in the 5000 psi pressure range. The large nozzle diameters used in this pressure range produced extremely high flow rates which created visibility problems at the coal face, and required removal of large amounts of water from the mining area. Besides, the production rates were not much better than with mechanical methods. This indicated that with higher pressures the water quantities might be reduced using the same or higher horsepowers but enhanced breakage. For this reason intensive investigation in material removal using fine water jets at extremely high pressures (50,000 psi to 100,000 psi) has been undertaken. This was not the entire reason for this contract from the Bureau of Mines, however. The contract objective is outlined later. IIT Research Institute has done pioneering work in high pressure jet development in the United States.5-8 Similar schemes have been investigated abroad. 9-13 Hydraulic mining of coal at relatively low pressures is successfully being done in the Soviet Union, Japan, Canada, and most recently, in Germany.

Environmental incidents involving mine tailings and waste rock inevitably become highly publicised and serve to damage the credibility of the entire industry. The recent serious incident involving a spill from a Romanian mine, was blamed for triggering æone of the worst environmental catastrophes in Eastern EuropeÆ. It is important to clearly demonstrate to the general public that these incidents are not representative of the industry as a whole. Where such incidents have occurred, the causes are generally related to poor design and/or construction, inadequate supervision and/or monitoring and maintenance, lack of independent expertise, inadequate risk identification and treatment and no provision to ensure that rehabilitation and long term management of the site occurs. Many people are unaware that these issues have been considered and provided for at the Martha Mine. Despite the fact that the first tailings storage facility has performed very well over fifteen years of operation, the long term safety and security of the tailings storage facilities continues to be questioned and debated by the public. The best way to address this is to provide quality information to enable a better understanding amongst the general public.

Case studies from four western states are used to illustrate the impact of the environmental compliance process on the mining industry. The case studies were not chosen as a statistically significant sample of all mining operations in the western United States but were chosen as representative of operations on both public and private lands and in locations that exhibited varying degrees of involvement by governmental units, non-governmental organizations and the general public. Basically, each case study was chosen because some aspect of their environmental compliance process helped to illustrate an interesting point in the regulatory process. The criteria used to select the case studies were: mines located in different jurisdictional areas, mines located in varying geological areas~ mines located in areas having different historical relationships with mining: and operations that are in production as well as those seeking an operating permit. The variety of such projects should provide the necessary foundation to evaluate the impacts of the environmental compliance process in each location.

In 1948 the United States became a net importer of oil. Coupled with an unprecedented domestic production of 5,900,000 barrels daily, imports approximating 500,000 barrels made available a total supply of 6,400,000 barrels a day. Thus, by a herculean effort, the oil industry was able to meet an extraordinary demand at home and, in addition, to ship abroad almost 460,000 barrels daily of petroleum and products. The country’s oil position now differs greatly from that prevailing at the beginning of World War II. Then, actual oil production averaged about 4 million barrels a day, and productive capacity probably was greater than 5 million barrels a day. As the demand of the military and industrial machine grew with the rising tempo of the drive toward victory, the United States not only met its own needs but produced about two-thirds of the total oil requirements of the Allied Nations, excluding Russia. As a people, we can be deeply grateful that the Nation’s petroleum resources were adequate and sufficiently developed to support such a tremendous effort. Even in those critical hours when immediate needs were the paramount issues, a prudent Congress paused to contemplate the importance of an assured long-term supply of liquid fuels. From these deliberations came the Synthetic Liquid Fuels Act of 1944, which authorized research and development work on new sources of oil – coal, oil shale, and agricultural and forestry products. Since World War II, a civilian demand for 6 million barrels of petroleum products has become the order of the day. By drilling as many as 39,000 new wells a year, the production rate has increased enough to meet domestic requirements by a bare margin. Some imports were necessary, however, to fill both domestic and foreign demands, and these doubtless will increase. Is this policy of exploration within the United States and the development of oil in foreign countries adequate insurance for the fulfillment of our future national needs? This question has two aspects – one purely civilian and economic in character and the other significant from the standpoint of our national defense. There are now approximately 4,000 producing oil fields in this country. At first thought, it would appear that production could decline in a considerable number of these fields without materially affecting the overall daily output. However, 48 percent of the oil comes from only 111 major fields which contain 60 percent of the total proved domestic reserves. It is apparent, then, that the flow from large fields – having proved reserves of 100 million barrels or more each – must be sustained if daily production is to be kept at the present levels. Essentially, this means that new major fields must be discovered about as rapidly, or perhaps more rapidly, than the older ones lose productive capacity. From 1940 through 1947, only eight major fields with estimated reserves of more than 100 million barrels each were discovered and proved – an average of one such field a year. The total estimated reserve of these eight fields was less than 1 year’s supply at the present rate of use. As drilling continues, some existing fields ultimately will prove reserves larger than those now known. Without a pronounced increase in the number of major discoveries, however, the prospects for increasing our present productive level are not encouraging. Any gap that appears between domestic productive capacity and demand could be filled with supplies from proved oil resources abroad. Waste phosphatic clay presents a difficult disposal problem and a significant loss of P2O5. Recent developments make this study on the use of phosphatic clay as a feed material for H3PO4 production worthwhile. (a) New data from Florida Industrial and Phosphate Research Institute (FIPR) suggest that the Bclay^ waste can be effectively treated by improved solid separation coupled with flotation to produce feed material for recovering additional phosphate. (b) The use of BOLI Flowsheet^ provides capability to predict performance of aqueous-based processes for a range of possible operating conditions. Using the new FIPR data and BOLI Flowsheet^ enabled the design of a new process that can potentially extend the usefulness of current P2O5 ore deposits. Predictions using OLI Flowsheet were able to identify process conditions that effectively overcome the difficulty of high Mg content of phosphatic clay. Estimated manufacturing cost in the current study lead to valuation of the P2O5 content of the phosphatic clay recovered/utilized at 60 to 70% of that of the phosphate rock feed material currently used. Phosphatic clay should be considered as a valuable material that is currently given zero value. The new FIPR data and BOLI Flowsheet^ enables the design of a new process that can potentially extend the usefulness of current phosphate ore deposits by extending the fraction P2O5 of recovered

"INTRODUCTION In a previous reports results were given of a manhole investigation carried out over a period from June,- 1929, to July, 1930, in cooperation with the Edison Electric Illuminating Co. of Boston, Mass., to determine the general hazards of combustible gases and vapors in manholes and other underground openings owned by this company. Considerable time was spent in this initial investigation in developing a suitable detector for determining the range of explosibility of the gases found in manholes. About 17,000 tests have been made with the detector since the investigation was started and its utility has been verified and its accuracy checked, by having complete analyses made of such samples that showed combustibles present equal to or above 25 per cent of the lower explosive limit, and those found deficient in oxygen.The initial investigation made in 1929 and 1930 demonstrated that the detection, control, and elimination of combustible hazards in manholes can best be obtained by a close cooperation of the interested parties through a systematic and continuous inspection of manholes, under proper supervision.COOPERATIVE INVESTIGATI0N DURING 1931The investigation carried out during the year 1931 has been along lines similar to that conducted during 1929 and 1930. In the present investigation the Edison Electric Illuminating Cc. of Boston and the Boston Consolidated Gas Co., have cooperated with the Bureau of lanes. The Boston Consolidated Gas Co. serves a major part of the territory under discussion with gas, but the tests covered the Edison territory irrespective of the gam company furnishing manufactured gas. A test crew consisting of two men, one an employee of the Consolidated Gas Co. and one an employee of the Edison Electric Illuminating Co., has been continuously employed during the year in the testing of games in manholes served by the Edison system. The test crew was under the direct supervision of John Campbell, superintendent of the special service department of the Edison Electric Illuminating Co., and F. M. Goodwin vice president of the Consolidated Gas Co.As in the previous investigation, all manhole tests were given test numbers. The test data sheets on which the information regarding the tests were recorded were prepared in triplicate, one copy going to the Edison Company, one going to the Gas company, and the third to the Bureau of Mines at Pittsburgh. These test data sheets were then filed (1) according to the town, (2) the street location, and (3) the manhole number; in this way a complete record pf each manhole tested was immediately available. For the last 15 years environmental Life-Cycle Assessments (LCA) have increasingly been used to evaluate the environmental performance of industrial processes and products and have been employed as a basis in decision making processes in the public and industrial sector. However, assessment methodologies for environmental protection measures and reclamation/remediation measures with respect to their consumption of natural resources are generally poorly developed. In this presentation a methodology for LCA of environmental protection measures and remedial actions, respectively is suggested, using the remediation/reclamation of mining sites as an example. Key aspect of this approach is the development of an evaluation method appropriate for environmental protection measures, which links the various impact indicators recognised in the impact assessments and allows their integrated assessment.
