"INTRODUCTION Strategies to improve performance in mining production, maintenance and mineral processing operations has evolved over time from straightforward time studies of individual machine performance in the 1960’s-1970’s to more sophisticated and better focused statistical and financial tools that have emerged in the 1980’s-1990’s . These newer strategies fall under the broad descriptive category of “Operational Excellence”. Operational Excellence is the term that embodies a number of performance evaluating strategies, the most popular and relevant to mining includes the following: • Six sigma for addressing errors within mining or milling process and work standards • Lean manufacturing to optimize organizational and operational performance through continuous improvement. There are other strategies that can be applied as well and this paper is not focused on a review of the pros and cons of any one strategy. However, the common threads in these strategies embody the expectation that expected results from these efforts can be quantified and that value will positively benefit the targeted work process, organizational unit, equipment deployment or time schedule when applied to it as part of continuous improvement efforts. The priority for proceeding with any one or more of these efforts is based on the calculated net value of the required resources to affect the change versus the estimated value gained. The practice is identified as the value mapping process. This discussion summarizes the value mapping process methodology used at a mature uranium mine and mill in northern Canada to work through the myriad of potential improvement efforts available through the operational excellence process for continuous improvement.THE RABBIT LAKE MINE AND MILL The Rabbit Lake Mine and Mill are located in northern Saskatchewan, Canada on the shores of Wollaston Lake (Figure 1).The mine was discovered in the mid-1960s and was built by Gulf Minerals, Ltd. The mill began processing uranium ore into yellowcake in 1975 from an adjacent open pit mine. A whole ore acid leaching followed by solvent extraction and precipitation forms the basic milling processes. The operation continued to produce yellowcake through a succession of open pits and finally opened an underground mine in the early 1990s. Since beginning production, over 200 million pounds of yellowcake has been packaged from the site, making it the sixth highest producing uranium mine of all time."

"The 4c segment is a part of the ""Oman National Railway Project” linking the mining area in the surroundings of Thumrait and the port of Salalah. It is 90 km long and the last 40 km stretch crosses a mountainous area near the city of Salalah. The project includes the excavation of a 33 km double and single bore tunnel and 10 km of safety tunnels. This mountainous area is a holiday destination for Omanis, thanks to its beautiful landscapes, especially in summer time due to the monsoon rains. The tunnelling design had to take into account both these environmental constraints and the limestone rock formations crossed, which are affected by significant karst features. Given the context, the conventional tunnelling method has been preferred. INTRODUCTION The Oman Railway Project (Fig. 1) covers the construction of approximately 2,150 km of railway line for freight and passenger transport which will connect together Oman’s main towns and industrial sites and it will form part of the Arabian Peninsula railway system “GCC Project”. The non-electrified double-track railway line must guarantee a speed of 220 km/h for passenger trains and 120 km/h for freight trains and must guarantee the transit of AAR (Association of American Railroad) H and D plates using clearance envelope according to AREMA (American Railway Engineering and Maintenance-of-Way Association) requirements. The design alignment must have maximum radiuses of curvature of 3,250m and a maximum gradient of 1.0%. The project is divided into 9 constructional and functional segments. •,Segment 4c will be constructed in the southern region of Oman, close to the border with Yemen, to connect the town of Thumrait with the Port of Salalah. •,Segment 4c will run through the Dhofar region and will be mainly used by the freight traffic which currently travels by road."

Knowledge of environmental risks associated with a property will allow better appraisal of a property's overall desirability prior to completing the transaction. It is possible that the cost of environmental cleanup could exceed the value of the property. Topics appropriately investigated include permit status, water quality, past use of the property, hazardous materials management, storage tanks, public records pertaining to the environmental character of the property, and environmental costs.

Geographic Information Systems (GIS) are becoming a more commonly used tool for resource estimation as they provide not only a database management platform, but also the graphical capabilities to visualize resource characteristics. Interpolation methods used commonly in these resource studies do not typically provide a quantitative answer to the question: "How accurate is the estimate?" By comparing localized measurements at several locations to the interpolated thickness distribution used in a resource estimate, a quantitative analysis of accuracy can be obtained. The technique is demonstrated with a case study from Central Appalachia.

"Historically there have been a number of different hypotheses and empirical models developed in an attempt to describe the nature of fracturing above longwall panels in underground coal mining. The motivation for such models varies, ranging from understanding the impact of mining on surface subsidence to back-analysis of caving behaviour in the immediate roof behind the longwall face. One of the most critical motivating factors that is taking on increased importance in many coalfields is the need for better understanding, and hence prediction of the impact of mining on overlying strata, particularly strata units acting as aquifers for different groundwater horizons.This paper reviews some of the major prediction models in the context of observed behaviour of strata displacement and fracturing above longwall panels in the Southern Coalfields of New South Wales, south of Sydney. The paper discusses the parameter often referred to as “height of fracturing” in terms of the critical parameters that influence it, and the relevance and appropriateness of this terminology in the context of overlying sub-surface subsidence and groundwater impact. The paper proposes an alternative terminology for this parameter that better reflects what it is and how it is used. The paper also addresses the potential role of major bedding shear planes mobilised by mining and their potential influence on overlying subsidence and groundwater interference.INTRODUCTIONKnowledge of the detailed nature of rock deformation and failure above any form of large-scale underground mining is always going to be limited to interpretation from a very incomplete set of data. It is extremely difficult, if not impossible, to directly measure the detailed nature of the rock failure, fracture networks, and deformational behaviour above an extracted mining area. Limited techniques such as borehole extensometry can provide some evidence of relative or incremental deformation in the direction of the borehole (usually vertical). However, such data cannot assist below the horizon where full caving has caused major rotation and dislocation of rock blocks and effectively destroyed the instrumentation borehole. Above such a horizon, the data is only valid along the axis and in the direction of the borehole, and to the level of detail defined by the extensometer anchor spacing intervals. It is often also restricted by major shear planes intersecting the extensometer borehole and cutting off the instrumentation below the level of the shear horizons."

Low-flow reactive-transport processes in Wightman Fork downstream of the Summitville Mine, south-central Colorado were simulated using the OTIS and OTEQ solute-transport models. The simulation was calibrated using data from a synoptic study conducted during October, 1998. Discharge over the 8-km reach from just below the mine site to the Alamosa River confluence ranged from 0.077 to 0.17 m3/s; travel time was 10.7 hours; pH ranged from 4.6 to 5.7. Simulations revealed that mass balance for SO4 was maintained, and that pH, Fe, and Al were non-conservative. Simulations allowing hydrous ferric and aluminum oxide or hydroxysulfate precipitation matched observed water quality conditions more closely than simulations without precipitation.

Particle-size Distribution Specifications for Particulates. Feed to and finished products from size-reduction operations are defined in terms of the particulate sizes involved. It is also well to know whether the ultimate individual particle is being measured, or, if any aggregation or agglomeration of particles exists, whether this has been created by the size-reduction operation. The most complete description of a powder is given by its particle¬size distribution. This can be plotted in terms of cumulative percent oversize or undersize in relation to the diameters of particles, or it can be plotted as a distribution of the amounts present in each unit of diameter against the several diameters. It is common to employ a weight basis for percentage, but there are some data in the literature where frequency, or number of particles, is used. The basis of percent¬age, whether weight, frequency, or some less commonly used factor, should be specified, as also should be stated the diameter, its units, and preferably whether determined by sieve, settling velocity, or other¬wise. Fig. I presents two sets of distributions-ore cumulative and the other in unit intervals. The slopes of the 5-µ intervals of the cumulative curves are converted to percent per micron and plotted as a block, or histogram, from which smooth curves are derived. Powder A has a narrower, or tighter, size range for the bulk of its weight than powder B. Both materials have the same weights below and above the size marked by the arrow. Simpler treatments of distribution are possible. In some cases the significant value is the top size. Since the 100% point is dubious, some amount, as 95 or 98%, so specified, can be called top size. In other cases the percent on some sieve helps define coarseness. Merely stating that all, or all but a small percentage, passes a given sieve is inadequate for defining the true fineness or a material. Complete parti¬cle-size analysis to show the distribution is essential for most compari¬sons and calculations. Particle-Size Equations. A number of equations have been pro¬posed to correlate the quantity of a particulate material with its parti¬cle size to obtain a distribution relationship. Brought on stream in 1975, the 10,000-tpd copper oxide processing plant of Anamax Mining Co. at Twin Buttes, Ariz., is now at design capacity. Based on acid agitation leaching, solvent ion exchange, and electrowinning, the Anamax plant bridges an ore to metal gap by producing mine site copper cathodes from lime-bearing oxidized feed. Until recently, such material was considered submarginal because copper recovery was uneconomic using conventional methods. The new Twin Buttes oxide plant, the largest of its kind in North America, is one stage in the expansion of Anamax, a joint venture owned equally by The Anaconda Co. and Amax Inc. Other phases include an accelerated stripping program in the pit, an expansion of the sulfide concentrator from 32,000 to 40,000 tpd, and construction of a 57 MW diesel generating station for the oxide plant. Once the basic oxide flowsheet had been selected, Anamax awarded a contract to a joint venture of Arthur G. McKee and Co.' (Western Knapp Engineering Div.) and Davy Powergas' (a member of the Davy International group). The contract covered process design, engi¬neering, procurement, and construction. The property, identified by Banner Mining Co. in the 1950s and developed by Anaconda, was put into production in 1965 as an open pit mine equipped with a 32,000-tpd sulfide flotation concentrator. The Twin Buttes mine has since become one of the largest earth¬moving operations in the world, excavating some 10 tons of waste per ton of ore. At the original sulfide mill feed rate, the mining fleet moved 375,000 tpd of material (225,000 tpd alluvium and 150,000 tpd of rock). The most significant copper mineral in the ore body is chalcopyrite, which is scattered in pockets of weak mineralization. Cap oxidation has progressed to depths of several hundred feet, principally as chryso¬colla. Present estimates place minable reserves at 347 million tons of sulfide ore grading 0.6% copper and 55 million tons of oxide ore grading 0.9% recoverable copper from 1.25% total copper. The nonre¬coverable fraction is composed of native copper and insoluble copper oxides and sulfides. The oxide ore has been partially stripped, and 25 million tons having a copper content greater than 0.6% have been stockpiled. The stripping of oxide ore to expose sulfides will continue for several years. The copper oxide studies launched by Banner Mining in the 1950s were continued by Anaconda. Process routes investigated included: acid leaching; cyanide leaching; segregation roasting; chlorination, oxidation, and reduction; flotation in combination with leaching; cal¬cite flotation plus leaching; copper oxide flotation; and sulfidization flotation. By late 1968 test work favored a low temperature roast followed by an ammonia-ammonium carbonate leach of the ore. During this period, acid leaching had been discounted because of excessive con¬sumption of expensive acid by the high limestone ore matrix. Restric¬tive SO2 emission controls in 1971 suggested an increase in acid production and availability at a lower cost. Research then switched to acid leaching, resulting in development of the present flowsheet. Mechanically agitated leaching in a cascaded series of tanks was selected in preference to vat leaching after piloting a stirred-tank system. Preliminary testing of vat leaching indicated that inter-vat solution clarification problems could be caused by gas evolution from leaching of high limestone ore. Lack of research time for resolution of this problem, plus success of the pilot agitation system, dictated the final flowsheet selection. In 1973 Anamax decided to proceed with agitation acid leaching to produce liquor for a solvent ion exchange (SIX) and electrowinning (EW) installation. At that time, such systems were proving their eco¬nomic viability, and they allowed direct recovery of high quality cath odes while bypassing the smelting and refining of lower grade cement copper precipitates. The Anamax oxide plant processes a grade of about 1% recovera¬ble copper from stockpiled sources and new mine production. Unit operations include size reduction, agitation leaching, countercurrent decantation washing of residue, pH adjustment of solution, clarifica¬tion, solvent ion exchange, and electrowinning of copper, with a mini¬mum of environmental impact. The feed to the primary crushers is composed of: 1) -10-in. stockpiled oxide ore, which has passed through the mine crusher and has been grade-segregated to allow blending for a consistent feed material. 2) -4-ft ROM oxide ore stockpiled in the early mining operation. 3) -10-in. ore currently being mined. Ore is delivered in 100-ton mine trucks to a dump pocket, where an apron feeder transfers the ore to an inclined 6-in. fixed grizzly. The grizzly oversize passes through a 48 x 60-in. primary jaw crusher, where it is reduced to -6 in. The fixed grizzly undersize and the crusher product are conveyed to a 4-in. vibrating grizzly. Oversize from this grizzly reports to a 7-ft Standard cone crusher, where it is reduced to -1 in. The crusher product and the vibrating grizzly undersize are conveyed to two 8 x 20-ft double-deck vibrating screens that operate in closed circuit with two 7-ft short head cone crushers. Screen undersize (-1/2 in.) is delivered to a covered conical fine ore stockpile with a designed live capacity of 15,000 st. Beneath the stockpile are ten Pioche belt feeders feeding two conveyor belts that supply the milling circuits. Each group of five feeders has two variable speed and three fixed speed drives. Grinding to 95% -48 mesh is done in two parallel lines, each consisting of an 111/1 x 18S -ft rod mill and a 12' x 30-ft ball mill in open circuit. Water is added to maintain a pulp density of 72% solids in the mills. Further water additions are made to the ball mill discharge sumps to create a 60% solids slurry for transfer to the leaching reactors. The nominal grinding rate is 440 tph solids. A splitter box on the product from the mills separates the incoming slurry into two streams, one for the leaching reactors and the other for the pH adjustment reactors. This split is determined by the lime content of the feed material and the pH of solution entering the pH adjustment reactors. Five 30-ft diam by 31-ft high mechanically agitated, rubber-lined leach tanks are arranged in cascade, with gravity transfer from one to another by enclosed launders. Concentrated sulfuric acid (93.2%) from storage is added to the pulp principally at the first leach tank, and the reacted slurry leaving the cascade after a 5-hr residence time at 50% solids is pumped to countercurrent decantation (CCD) wash thickeners.' At this point, most of the soluble copper has been leached from the ore. Up to 250 lb of sulfuric acid is consumed per ton of ore, the acid being supplied in tank cars from a nearby smelter. Leached slurry at 50% solids is washed countercurrently with return raffinate from the SIX plant in a series of four Doff-Oliver center caisson, 400-ft diam thickeners. Solids advance from thickeners No. I through No. 4 to the tailings dam. SIX raffinate advances from thickener No. 4 to thickener No. l, where a pregnant solution overflows to pH adjustment. To optimize solvent extraction, the pH of the pregnant solution from CCD thickener No. I is adjusted from 1.5 to 2.5 by adding unreacted ground slurry (split from the grinding circuit) to the preg¬nant overflow of No. I CCD thickener. The resulting slurry is retained for 45 min in three mechanically agitated leach tanks. CO, generated during leach and pH adjustment is ducted to a demister, where entrained acid droplets are removed. The cleaned CO, and the No. 4 CCD underflow are the only effluents from the plant. The underflow of No. 4 thickener mixes with tailings from the sulfide concentrator and is neutralized. Slurry exits the pH adjustment vessels at about 10% solids and

Resumen ¦ Un sistema de comunicaciones “a través del suelo” (TTE) comercial y disponible fue evaluado en una mina de carbón subterránea en la zona Este del Estado de Kentucky. Este estudio de campo fue realizado como parte de una evaluación multisitio mayor de sistemas TTE disponibles. La mina detallada en este estudio se encontraba paralizada al momento de la evaluación, lo que proporcionó la representación más cercana a la realidad del medio ambiente en una mina luego de una operación de cierre. Para este estudio se probaron las comunicaciones superficie a subterráneo y subterráneo a superficie. El despliegue estándar del sistema TTE usando los procedimientos recomendados por el fabricante logró obtener comunicaciones claras y nítidas entre el socavón y la superficie. Otras disposiciones de las unidades arrojaron resultados mixtos. Existen planes para examinar más los fenómenos observados en esta mina.

Unsaturated flow and solute transport were studied in three large columns measuring 1.5 m diameter x 2 m height, loaded with up to 3.7 t of oxidised waste rock, which was excavated from the Stratmat pile at Heath Steele Division, Noranda Inc. and contained elevated concentrations of zinc in the pore water. The bottom of each column was partitioned into 37 squares, each of which was drained independently. Following rain simulations at the top of the rock bed, the drainage in each drain port was monitored for volume and physicochemical parameters at pre-determined intervals. This paper describes some of the observations resulting from the experiments. The main conclusion of the study is that unsaturated flows in waste rock are heterogeneous, and flow channelling is ubiquitous. The results imply that the transport of solutes from oxidised waste rock to its toe drainage is subject to various controlling mechanisms, including geochemical equilibrium control and successive pore water dilution control.

The integration of the ESZ orebody and DOZ orebody resulted in a much larger footprint than the original DOZ Mine shape. The significant change in footprint geometry presented new cave front management challenges, specifically: longer undercut drifts. Using a constant blasting rate, more time is required to complete caving in each drift which ultimately influences the number of active faces at any point in time. The number of manageable active drifts is controlled by the planned square-meters of undercut blasting, lead/lag restrictions and other specific conditions such as the need to blast pre-charged explosives. In early 2008, an evaluation of the existing plan showed that significantly more active undercut drifts and longer cave fronts would occur in the next five years. Difficulties associated with the long cave fronts led to a decision to redesign the caving sequence. Several different boundary options based on operational, geotechnical and ore grade parameters were investigated. This paper describes the issues faced with long cave fronts, the constraints and requirements to effectively manage the caving sequence and the options considered to mitigate this situation.

The stability of shale gas wells drilled through current and future coal reserves can be compromised by ground deformations due to nearby longwall mining. Depending on the longwall-induced rockmass permeability, the high-pressure explosive gas from the damaged well may reach mine workings and overwhelm the mine ventilation systems. This study uses geomechanical models to estimate the rockmass permeability induced by mining. A two-panel longwall model of a deep, 341-m-cover mining site in southwestern Pennsylvania is constructed in 3DEC to explicitly model the rockmass by a Discrete Fracture Network (DFN) technique. Stress-induced fracture apertures and permeabilities are calculated across the model and are validated against permeability measurements. A Fracture Flow Code (FFC) is developed to use these results to predict potential inflow to the mine should a gas well breach occur. One hundred DFN realizations are simulated, and the results show that for a gas pressure of 2.4 MPa, the average of the predicted inflow rates to this deep-cover mine is 0.03 m3 /s, significantly lower than the average inflow of 0.92 m3 /s for a shallow-cover mine (145 m deep) studied in the previous work (Khademian, et al. 2021). The result can help assess the potential hazards of a shale gas well breach for mine safety and evaluate the ventilation requirements to mitigate the risk

"A commercially available through-the-earth (TTE) communications system was evaluated at an underground coal mine in eastern Kentucky. This field study was conducted as part of a larger multisite evaluation of available TTE systems. The mine detailed in this paper was idled at the time of the study, which provided the nearest practical representation of a postevent mine shutdown environment. For this study, surface-to-underground and undergroundto- surface communications were tested. Standard deployments of the TTE system using the manufacturer’s recommended procedures were able to achieve clear communications between underground and surface locations. Other arrangements of the units yielded mixed results. Future experimentation is planned to further examine the phenomena observed at this mine IntroductionThe ability to establish effective communication between surface and underground personnel after a mine event is essential for both evacuation and rescue efforts. Current wireless underground communication technologies are reliant on intermediary physical infrastructure, such as antennas and repeaters, to enable underground communications. As such, communications are easily disabled during a mine event because of the inherent vulnerabilities to physical damage (Damiano, 2012; National Academy of Engineers, 1970; Yenchek et al., 2011). A true emergency communications system would function regardless of a mine’s physical state. Through-the-earth (TTE) wireless communication technology only requires the use of a single transmitter and a single receiver to achieve point-to-point communication through solid strata. With this capability, TTE systems have the potential to serve as a true emergency communications platform."

Introduction The cost of grinding may be divided into a fixed equipment cost portion and an operating cost portion. The fixed equipment cost is the amount of money required to purchase the grinding mill itself. This amount may be kept separate from the installation cost which, often, is taken as a percentage of the fixed equipment cost. A good average is 43%; therefore a $100,000 ball mill would cost approxi¬mately $143,000 installed. Operating costs are variable and paid per unit time for power, liner replacement, grinding charge replenishment, labor, and mainte¬nance including major parts replacement. Such costs depend on the geographical location and will vary with the cost of labor. Operating costs are difficult to estimate. Most mining companies consider such information proprietary. This section provides a means to obtain an estimate of the equip¬ment cost of a grinding mill with a rated accuracy of±l0%. Compara¬ble methods to determine operating costs are not available. Such costs are presented in table form with suggestions for updating them. Esti¬mating the cost of liners and steel replacement is possible as described in a later section. Capital Costs of Grinding Mills Grinding mill costs have been related,124 by means of equations and corresponding graphs, to the nominal horsepower requirements of mills. All graphs are based on a Marshall and Stevens cost index for Mining and Milling of 300, where a cost index is a ratio used to estimate current prices of equipment from obsolete prices. Where (Cost Index Now) Cost Now = (Cost Then) (Cost Index Then) (I) For example, the cost of an autogenous mill that requires a 3000¬hp motor is estimated as $500,000 (Fig. 95). This was the estimated price when the M&S (Marshall and Stevens, Mining and Milling) cost index had a value of 300. If the index is currently at 400, then the estimated current price of the autogenous mill is: Cost Now = ($500,000) (400) (300) = $667,000 Most professional journals will publish current values of cost indices at least once a month. In subsequent pages, each graph of grinding mill cost vs. mill horsepower has an associated graph of capacity vs. horsepower along with a table that relates mill size to horsepower. The associated graph and the table must be used with caution, since both apply to an average ore under average conditions. Example calculations showing how graphs should be used are shown in the following. It should be stressed that the cost data incorporated herein ex¬cludes the cost of pumps, motors, lines, and classifiers, unless other¬wise stated. Costs are not direct quotes; they are the averages of quotes supplied by manufacturers and analyzed by means of regression analysis. Example 1. The McKinley Mine is located in the northwest New Mexico, in McKinley County, approximately 25 miles from Gallup. The mine is operated by The Pittsburg & Midway Coal Mining Co. (P&M), a wholly owned subsidiary of the Chevron Corporation. P&M is headquartered in Englewood, Colorado. Since the McKinley Mine began operations in 1962 a total of 83,391,000 metric tons of coal have been sold through 1990. The 1991 production was approximately 5.000.000 metric tons. The mine operations are contained on a 109.000 square kilometers lease area of which 44.000 square kilometers are on the Navajo Nation. The present customer base consists of four utilities and one industrial account, all of which are located in Arizona. The utility customers are the Arizona Public Service Co. Cholla Station at Joseph City, The Salt River Project. Coronado Generating Station near Joseph City, and The Tucson Electric Company, Irvington Generating Station in Tucson. The industrial customer is the Stone Container Corporation, Snowflake Paper Mill.

Sutter Gold Venture excavated an exploration decline into a mineralized zone in the Sutter Creek gold mining district. Delays in obtaining the conditional-use permit resulted in 7 to 10 million gallons of water accumulation in the decline containing elevated dissolved arsenic. Sutter received a permit to land apply the water after reducing the arsenic concentration to 0.05 mg/L. Initially the water was treated by co- precipitation arsenic with ferric hydroxide. Operational difficulties of this treatment system resulted in Sutter pursuing alternative treatment methods. The activated alumina adsorpting was selected due to its flexibility and ease of operation. Operating and maintenance data and costs will be presented.

Although Disputes Review Boards have been used for some time, they had not received wide publicity at the time Monroe County first used one. Public owners frequently resist adopting new contracting practices. This paper discusses an owner's experience with Dispute Review Boards, from learning about them to their use on two (2) projects. The experiences of potential and actual contractor claims are compared to a claim on an earlier contract and the ensuing litigation. Suggestions for possible use for Owner-Engineer disputes and recommendations for use are provided.

PPC (Public Power Corporation) operates a new power plant at the South-West part of Crete. Recently the extension of the Power Plant started. ATHENA SA the main contractor of the project assigned to EXORIXI SA the implementation of a blast vibration monitoring program and prevention-evaluation of risks caused by construction blastings inside or very close to the operating part of the power station. The blasting parameters and characteristic elements that had been evaluated, the measures that had been taken and materialized, the overall risk management and the results of the whole program are presented in this paper.

Geotechnical, geological, and hydrogeological fieldwork and data collection for the mining industry is often conducted by younger, less-experienced staff members. In many cases, fieldwork personnel are recent college graduates or are otherwise relatively new to the mining industry. Due to busy schedules, staffing shortages, project budgets, and in some cases, poor management, new fieldwork hires often do not receive thorough training. In the past and the present, many workers in the mining industry learn(ed) by “doing.” While this is still considered to be the most practical approach and the easiest way for a company to determine the skill set of an employee, there is always some risk involved in the “feet to the fire” method. The reality is that field personnel are often collecting data on which major operational decisions will be made or on which regulators will base enforcement decisions. Inadequately trained or inexperienced field personnel may collect inaccurate data or fail to recognize key conditions that may influence decisions made by regulators, design engineers, mine managers, and corporate leaders. Even the best engineers may come to incorrect conclusions if they are starting with poor input data. Decisions based on inaccurate data often result in delays, budget issues, unsafe conditions, or even worse consequences. This paper provides numerous mining industry examples of incidences where inaccurately collected field information did, or could have, resulted in major adverse consequences. The paper also offers some simple approaches to minimizing the potential for such problems.
