"SynopsisThis paper assesses the effectiveness of the Mine Occupational Health and Safety Leading Practice Adoption System (MOSH) and its potential to improve mine health and safety in South African mines. Developed by the Chamber of Mines, which represents the majority of the country's large scale mining employers, MOSH was devised to accelerate progress towards achieving health and safety milestones, which were set by tripartite agreement in 2003. The paper documents and builds on the findings of a study conducted by the Centre for Sustainability in Mining and Industry (CSMI) in 2011 that evaluated MOSH strategy, structures, and process of implementation.The study found that MOSH operated across the mining sector, was directed and dominated by experts and, despite best efforts to include other stakeholders, was led by employers. Statutory worker health and safety representatives and structures were not integrated into the complex change process developed by MOSH. The Mine Health and Safety Inspectorate (MHSI) and organized labour were ambivalent about direct involvement in MOSH and preferred regulatory measures to enforce the participation of mines. MOSH interventions were not targeted at mines with a poor health and safety record, and MOSH lacked a baseline from which to track impacts on sector-wide health and safety performance. The leading practices most widely adopted by mines were designed to improve, rather than fundamentally alter, existing practice. Although the depth of engagement with MOSH among stakeholders and on mine sites varied, mining companies, labour representatives, and the Mine Health and Safety Inspectorate (MHSI) saw the programme as significant.IntroductionIn 2011, the Chamber of Mines commissioned the Centre for Sustainability in Mining and Industry (CSMI) to evaluate the progress and effectiveness of the rollout of the Mine Occupational Health and Safety Leading Practice Adoption System (MOSH). CSMI, based in the School of Mining Engineering at the University of the Witwatersrand, is an independent research centre conducting health and safety research relevant to all major stakeholders in the mining sector. The purpose of the evaluation was to identify the factors that enabled or hampered the adoption of new technologies or practices. The timing of the evaluation, two years before the deadline for the 2013 safety and health milestones, allowed time for adjustments to be made. This paper summarizes the results of the evaluation and includes further analysis and discussion of the nature of the programme and the conditions that act to advance it or it hold back."

The Conference Minerals & Materials '96 was held in Somerset West, South Africa, from 31st July to 2nd August, 1996. It was organized by the Universities of Stellenbosch and Cape Town, the Cape Technikon, and the Western Cape Branch of The South African institute of Mining and Metallurgy (SAIMM). The three-day Conference was attended by 180 delegates, representing countries such as Australia, South Africa, United Kingdom, Finland, Sweden, Germany, Poland, Zambia, and Zimbabwe. The programme and associated exhibition were well organized, and each of the delegates received two volumes of proceedings containing the 77 papers presented at the Conference.

"A major shortcoming of the statistical back-analysis of coal pillar strength is that it relies on the asmined pillar dimensions, not taking into account time-related pillar scaling with subsequent reduction in pillar width. The paper describes an investigation where the rate of pillar scaling was applied to the pillars in the databases of failed and unfailed pillars. The reduced pillar sizes were then used in the same method of statistical back-analysis that had been used in the past.This resulted in an equation for pillar strength which predicts significantly greater pillar strength than the previous statistical analyses, which is similar to the strength which had been found earlier by direct testing of large specimens underground. It is concluded that the lower strengths found by previous statistical analyses are due to the incorrect pillar width being used and that by adjusting the pillar sizes to compensate for scaling, more credible results that correspond to direct testing are obtained.As the newly derived strength equation is time-dependent, it follows that the safety factors and probabilities of failure are likewise time-dependant. An equation to determine the probability of failure is also developed. This results in the safety factor being close to unity at a probability of failure of 50%, which is aligned to statistical expectation. IntroductionSince the Coalbrook mine disaster, caused by massive pillar collapse, there have been several attempts at analysing and defining the strength of coal pillars. It was realized in the very beginning that performing classical strength tests on coal specimens in a laboratory and then transferring those results to real coal pillars could not be successful. The scatter of results is one problem and, more seriously, there is a size effect (at least for small specimens) that made the transformation of strength results to much larger coal pillars underground all but impossible.Two main schools of thought then emerged. Some researchers preferred to perform direct strength tests on large specimens underground, while others relied on statistical back-analysis of failed and unfailed pillar cases, attempting to determine the differences in size and shape that would allow a satisfactory prediction of failure. The latter school of thought resulted in the method that was widely accepted by the South African coal mining industry, described in the landmark publication by Salamon and Munro (1967)."

Ferroalloys like FeCr and FeMn are mostly produced in submerged electric arc furnaces. Typical for these processes is that the amount of slag is quite high. The role of slag for the process is thus very essential and specific chemical, physical and electrical properties are required. Slags in FeCr process are Al2O3-MgO-SiO2based with minor contents of CaO and CrOx. Slags in FeMn process are principally similar originating from silicate minerals but can have quite a large composition range depending on the manganese ore and its gangue minerals. Characteristic for FeMn process is that the slag is rich in manganese oxide up to above 30%. A common feature is that the ?ore oxides?, CrOx and MnOxin slag are not only divalent oxides but also containing some higher valent of Cr3+ and Mn3+, respectively. In FeMn production conditions the slag consists mainlyMn2+. This is a clear difference from CrOx containing slags in which both two and three valent coexists in slags in FeCr process conditions. Oxidation state of the slag depends on the ambient oxygen partial pressure and temperature in the examined process. Thermodynamics of slags in ferroalloys production are discussed in terms of phase diagrams, activities and oxidation state of chromium and manganese oxides. Further, slag properties influencing reduction kinetics and furnace operation i.e. viscosity and electrical resistivity are discussed. Evacuation operations from work places in the case of an emergency are the key to minimising the effects of catastrophic type accidents such as fires, explosions, cave-ins, floods, etc. The correct planning of these operations, having the necessary means for sounding alarms and communicating and suitable training of personnel are the key factors for guaranteeing successful evacuation. The article gives an analysis of the problem in the case of subterranean mining and various types of work developed by AITEMIN in this field, including the development of new alarm and communication means specially adapted to the characteristics of mining operations, development of tools for help in the preparation of emergency plans and experiences in the education and training of personnel.

This paper deals with the selection of alternative accesses to a podiform chromite orebody in Turkey. The most important alternatives studied in a related project have been stated as a new vertical shaft and a ramp system. The comparison of these alternatives have been carried out on the basis of total investment costs, ore transport unit cost and net present value (NPV) of overall project for various depth. The results show that up to the depth of 700 m, the total investment cost and NPV favours the ramp access while unit ore transport cost make the shaft access preferable.

The South African based platinum mines produce the world?s majority of platinum metal that is used in autocatalysts and jewellery. As the platinum market is forecast to grow at healthy rates into the future, the South African producers, Anglo Platinum, Impala, Lonmin and Northam are planning to expand in the next decade. Furthermore a number of smaller producers who are accessing other mineral rights are entering the buoyant market. The South African platinum mines have traditionally exploited the Merensky reef of the Bushveld Igneous Complex (BIC); however, the mining of this reef is reaching depths that require significant vertical shaft systems and costly refrigeration for access to the ore bodies at deeper levels. In the past decade all of the producers have begun to mine the less attractive UG2 reef because of its cheaper mining costs. This UG2 reef contains significant quantities of chromite with typically a chrome-to-iron (Cr/Fe) ratio of 1.35. UG2 ores can contain between 10 and 25% Cr2O3 depending upon the reef width and the selected mining method. It is relatively cheap and easy to extract a portion of the chromite from a platinum concentrator tailings stream at a grade of between 40-42% Cr2O3. However it is only in recent years that some ferrochrome producers have begun to source some of their chromite from the tailings of platinum producers. South Africa is also the world?s largest single producer of ferrochrome and supplies much of the world?s stainless steel producers and thus the growing market for their product. As the traditional sources of high Cr/Fe ratio ores have been depleted at the shallow economic depths and thus have become more expensive sources of chromite, technical developments have improved the ability to use fine chromite economically to make acceptable grades of ferrochrome. Thus UG2 fines are now becoming possible cheap sources of acceptable feed to the ferrochrome industry. The abundance of feedstock for ferrochrome production and the lessening dependence on mined material will impact upon the structure of the ferrochrome business in South Africa. With continued platinum mining from UG2 reefs, South Africa is set to remain highly competitive in the world?s ferrochrome/stainless steel industry.

The challenge; to design production facilities and in particular separation equipment which have the ability to deal effectively with the ever changing composition of well fluids, over the life time of the field in which it is operating, ensuring maximum uptime of the asset coupled with consistently effective separation. Design engineers are, however, continually confronted with a dilemma when selecting materials of construction for Oil/Gas separation equipment, since they are generally obliged to select an acceptable material of construction, based on ?predicted? operating conditions, at the lowest acceptable cost. Consequently many designs are somewhat basic and do not take into account the constantly varying dynamics of the well fluids being released into the separator. As oil fields mature, oil companies endeavour to maximize revenue by maintaining oil and gas throughputs. This often results in an associated increase in water and sand volumes and, in order to maintain efficient separation, an increase in operational temperature and pressure; subjecting the equipment to operational conditions for which it was never designed. In reality, therefore, the limitations and constraints placed upon new construction design often result in Maintenance Engineers having to solve the ensuing problems in difficult ?in-field? environments. Should refurbishment or upgrade be required then the status of the platform and that of the vessel during shutdown will dictate how this is achieved. Hotwork on the vessel is generally not preferred due to the requirement for post weld heat treatment and re-classification of the pressure vessel, not to mention the added risk in terms of safety. Unfortunately, until recently, there have been few alternatives to conventional ?hotwork? repair and protection systems, especially since the performance of cold curing polymer based repair and coating systems, for use in oil and gas process equipment, has, until recently, been limited by their resistance to temperature and pressure in immersed conditions. In line with the continued development of high temperature / high pressure oil and gas fields however, recent breakthroughs in polymer technology can now offer both repair and protection solutions which are able to function effectively at temperatures up to 180°C in immersed conditions combined with resistance to the destructive forces of explosive decompression from pressures in excess of 90 bar. This paper will look at recent work that has been undertaken to determine the suitability of modern polymer based repair and protection systems as an alternative to hotwork; thus enabling newly designed and upgraded internals to be cold bonded into position and further protected with temperature resistant coatings, in order to minimize the downtime of the asset and further to protect the vessel internals from the Erosive Corrosive environment.

The project was aimed at evaluating the double-header trains implemented at Great Noligwa mine, looking at the changes from the old conventional trains to the new trains, and identifying possible improvements to obtain an even better and more productive tramming system. The methods used to evaluate the double-header trains at the mine included personal observations, time studies, personal interviews, technical research, safety analysis, cost analysis, production analysis, breakdown analysis, break test analysis, comparisons and working closely with engineering department and manufacturers. Issues that were evaluated are the improvements in safety, cost reductions, tramming congestion reductions, better efficiencies, labour optimization, enhanced driver safety and maintenance. Shortcomings of the double-header trains were identified and recommendations made to improve on the shortcomings. Findings of the evaluation study indicated the following: ?Drastic improvement on safety ?Extreme cost reductions ?Reduction in tramming congestions and bottlenecks ?Better tramming efficiencies ?Optimized labour placement ?Enhanced driver safety ?More effective maintenance. Although there is room for improvements, the double-header trains were implemented with great success at Great Noligwa mine. It is currently the preferred tramming methodology at Great Noligwa mine, and is also implemented at other Anglo Gold Ashanti?South Africa shafts. This tramming system has huge potential for growth, not only in Anglo Gold Ashanti, but in all other older mines that experience major tramming problems.

Finsch Mine is a Kimberlite diamond mine located at Lime Acres in the Northern Cape of South Africa. The mine was founded in 1961 and started surface-mining in 1964. Underground production commenced in 1990 using a Modified Blast Hole Open Stoping method for the mining of Blocks 1,2 and 3. Block 4 is currently being mined as a Block Cave. The process of identifying and optimizing a mining method to mine the Block 5 ore body started in 1991 and in 2006 Inclined Caving was identified as being a technically feasible method. This report aims to document the process employed in developing this method by the Block 5 Pre-Feasibility Team as well as discussing the technical challenges encountered during this process.

The effect of SiO2addition on the decomposition of Li2CO3was investigated at temperatures up to 1000ºC. It was found that addition of SiO2greatly enhanced the decomposition of Li2CO3. The main decomposition reaction started at around 600ºC, and completed just above the melting point of Li2CO3. The major decomposition product was Li2O?SiO2irrespective of the Li2CO3to SiO2mixing ratio while both reactants were available. It was ascertained that a liquid layer betweenLi2CO3and SiO2particles formed and facilitated the decomposition reaction. The governing reaction of the decomposition was the one between the dissolved Li2CO3and SiO2in the liquid layer to form Li2O?SiO2. The decomposition rate was independent of the Li2CO3/SiO2mixingratio until either one had been completely exhausted. When excess Li2CO3existed, it further reacted with the initial product of Li2O?SiO2to form 2Li2O?SiO2. When SiO2was found in excess, on the other hand, no further reaction took place. This is attributed to the fact that uponLi2CO3exhaustion there is no liquid phase available to facilitate further reaction. The apparent activation energy of the decomposition of Li2CO3in existence with SiO2is 198 kJ mol-1.

"Large quantities of slag are produced every year and dumped on slag heaps around the world, posing a potential environmental threat due to contained heavy metals content. Slags are waste minerals produced during furnace smelting of minerals. The contained values of base metals may be released into the environment under natural weathering conditions polluting ground and underground water. Copper losses into slag are a continuous problem in pyrometallurgical smelting operations. Due to shortage of ore concentrate, various mining operations have initiated the recovery of copper from dumped slag. The typical process uses crushing, milling and flotation. These operations create excessive liner wear rates during crushing and milling because the bond work index of copper slag is 22 kWh/t and this is considered a very hard slag. The process also has reduced flotation recoveries and requires high flotation reagent consumption. The present work shows that digestion of the milled copper slag slurry with concentrated sulphuric acid at a temperature of 250oC forms a solid cake and leaching of the solid cake with water is a possible option. However, the consumption of sulphuric acid can have a considerable cost on the recovery process. This can be limited by producing a leach residue containing gypsum and silicate materials as amorphous silica which are suitable for use as an additive for Portland cement. Laboratory results show that 90% copper recoveries can be achieved. The leaching is conducted to prevent silica gel formation and iron co-extraction. Leaching is optimum if silica gel-free solutions and good filterable residue are obtained. INTRODUCTION Slag is a waste by-product of smelting and converting operations in metallurgical plants. Both smelting and converting operations are aimed at iron separation from base metal sulphides in non-ferrous metallurgy. The iron sulphide component of the ore or concentrate is oxidized to iron oxide at higher temperatures in the molten form, and then fluxed with silica and other oxide rock minerals to produce fayalite (2FeO•SiO2) slag (Schlesinger et al., 2011). Slags are generally deposited on dumps and considered to be unreactive materials. Smelter copper slag contains significant concentrations of several potentially toxic elements, including arsenic, lead, cadmium, barium, zinc and copper. These elements can be released into the environment under natural weathering conditions and cause pollution of soils, surface waters and groundwater (David, 1998)."

Gullies are the vital in-stope excavations that provide access for mining personnel and material to, and mined ore from, the producing faces in the tabular hard rock mines. They pose a serious problem in most mines because they induce stress orientations, which, combined with blast damage, produce adverse rock fracture patterns that contribute to the loss of hangingwall and sidewall integrity. Rock-related injury statistics confirm this by highlighting gullies as the second-highest risk area in South African hard rock mines. Inappropriate gully geometries, poor blasting practice, and non-adherence to accepted standards worsen the problem. The aim of this work is, firstly, to provide guidelines to select the correct gully geometry and appropriate support for a particular geotechnical environment and, secondly, to review the practical issues behind gully safety and stability. In order to meet these aims, this paper reviews the available literature on gullies and their history, and reports on an industry-wide survey of practices and opinions. Then it covers observations from underground visits to 107 gullies situated in the respective geotechnical environments of the Basal Reef, Beatrix Reef, Vaal Reef, Carbon Leader Reef, Ventersdorp Contact Reef, Kimberley Reef, Kalkoenkrans Reef, Merensky Reef, and UG2 Reef. Despite the differing geotechnical conditions and gully geometries, common problems are experienced with geological structure, stress, and fracturing. Results from numerical modelling are also presented to help analyse the underground observations and to quantify the merits of the different gully layouts. It is concluded that there are no new techniques available to improve gully stability, but that significant improvements in safety are attainable by employing the correct gully geometry for the geotechnical environment, good blasting practice, and appropriate support.

The semi-solid metal casting process results in a unique solidification path. The microstructural features that were identified during the characterization of the as-cast microstructure, were used as evidence of the process history in order to determine the solidification path followed during the evolution of the microstructure. The morphology of the grains was characterized using SEM and the extent and distribution of the elemental segregation within the grain structure was analysed using EDS. The first documentary evidence for the use of fire to extract a metal was furnished by the wall paintings of the ancient Egyptians showing workers using the blowpipe and bellows to increase the temperature to melt gold in a crucible. Furnaces grew in size and complexity, coke replaced charcoal in the blast furnace, and the Industrial Revolution took place in England that gradually spread worldwide. More metals became known and new techniques were introduced for their extraction. Among these, metallothermic reactions became prominent in the nineteenth century immediately after the discovery of the electric current and its use to liberate the alkali metals from their salts. Once these metals became available, they were used to produce aluminium from alumina that resisted all other methods of attack. Aluminium, now available on a large scale, was used to decompose the most stable compounds and liberate more metals that became the basis of modern civilization.

This paper introduces the Quantified Value-created Process (QVP), a decision-making process for use in mine design and operation. The implementation of the QVP is founded on a ten-step rock engineering design process and a ten-step strategic planning process. The potential of this approach is demonstrated by examples of value created as a result of good research, design, and planning. The QVP is divided into two parts, namely, a ‘planning the process’ section and an ‘implementing the process’ section, and can be considered to be an extension of the risk approach. All costs, direct and indirect, associated with a significant mining decision (design, operational, investment) must be quantified in advance of committing to that decision, to determine the value that will be created in the short, medium, and long terms. Ethical values, associated with health, safety, the environment, public perception, and social aspects, may be difficult to quantify in economic terms, but must nevertheless also be considered. In contrast with a traditional risk approach, the QVP will identify the upside, and allow executives to make more informed strategic decisions. In this context it is essential to actively engage upper management in the decision process.

The kinetic and thermodynamic efficiencies of chromium recovery from the slag in an AOD process during the slag de-oxidation period with ferrosilicon is a complex process affected by slag basicity, temperature, the dissolution kinetics of the reducing agents, and intermixing of the slag/metal/alloy addition phases within the vessel. The present work focused on a study and simulation of the fluid dynamics and mixing behaviour of ferrosilicon additions in the AOD process for the production of stainless steel. In order to visualize the mixing behaviour, a two dimensional slice model of an AOD vessel was first built and filled with ZnCl2solution and then covered with silicone oil in order to represent the bulk liquid steel and slag phases, respectively. The injection of argon/oxygen process gases was simulated using air or helium. Particle trajectories and intermixing of the phases was monitored with a high-speed camera. To simulate the 75FeSi additions, glass spheres were used. A three-dimensional version of the AOD model was then constructed in order to confirm the adequacy of the slice model. Similar particle mixing behaviour was observed. Both models demonstrated that the ascending gas/liquid plume created two asymmetric mixing zones: a smaller rapidly re-circulating mixing zone adjacent to the side-wall locating the tuyere(s), and a larger mixing zone in the main part of the vessel. The process efficiency during the reduction period is governed by the thermal behaviour of ferroalloy as well as the mixing efficiency in the main large mixing zone. To enhance the recovery efficiency for chromium, small Fe-Si particles are suggested, given their wide distribution within the slag phase, and their large interfacial areas and rapid melting times for Cr2O3reduction from the slag. The aim of this paper is the construction of models of systems with ionic and ionic-covalent bonds (molten salts) using experimental diffraction data and the calculation of their structure and thermodynamic and transport properties. Modelling was performed using algorithm BELION, in which the non-coulomb part of interaction potential between ions in the model was determined by the method of iterative simulation which targeted agreement between calculated and experimental partial pair correlation functions (PPCFs). The ionic charges in salts, which can be fractional, were determined by minimization of internal energy. In this paper, the algorithm BELION is applied for chlorides of alkaline-earth metals. Pair potentials, PPCFs, atomization energies, self-diffusion coefficients and electric conductivity are calculated. The charge of alkaline-earth metals was in the range of 2.0-2.1, slightly increasing from Mg to Ba. Partial pair correlation functions and atomization energies of models agree well with experimental data for MgCl2, SrCl2and BaCl2. "SynopsisFluorine – in the form of hydrofluoric acid, anhydrous hydrogen fluoride, elemental gaseous fluorine, fluoropolymers, volatile inorganic fluorides, and more – has played, and still plays, a major role in the nuclear industry. In order to enrich uranium, the metal has to be in the gaseous state. While more exotic methods are known, the standard and most cost-competitive way of achieving this is by means of uranium hexafluoride (UF6). This compound sublimates at low temperatures, and the vapour is enriched using centrifugal processes. The industrial preparation of uranium hexafluoride requires both elemental fluorine gas and anhydrous hydrogen fluoride (HF). HF is prepared by the reaction of sulphuric acid with fluorspar (CaF2). Fluorine gas in turn is prepared by the electrolysis of HF. Yellowcake is first converted to uranium tetrafluoride (UF4), using HF, after which the compound is treated with fluorine to yield UF6. After enrichment, the UF6 is reduced to UO2 for use in fuel elements in pellet form.South Africa has the largest reserves of fluorspar internationally, and is the third largest producer after Mexico and China. Fluorine technology has many associated difficulties, because of the reactivity of fluorine and the toxicity of HF. The main barriers to entry into the fluorochemical industry are thus the abilities to produce both HF and F2. Both these substances are produced locally, at the industrial scale, at Pelchem SOC Ltd. Should South Africa contemplate developing its own nuclear fuel cycle as part of the awaited new-build nuclear project, it will be imperative to leverage the existing skills with respect to fluorine technology, resident at both Pelchem and Necsa, for this purpose. This paper summarizes the fluorochemical skills developed locally over the past several decades, and suggests strategies for maintaining the technology base and developing it for the next generation of scientists and engineers.IntroductionFluorine chemistry has always had a very close relationship with the nuclear industry. Although the first mention of fluorspar was recorded in the sixteenth century, and the fundamentals had been well-developed by the Second World War, it was only during the Manhattan Project (Banks et al., 1994), when almost unlimited funds were made available for the large-scale industrialization of fluorine and related compounds, that the technology took off. Expertise in both the chemistry and the engineering aspects of fluorine is essential for the design and running of several of the unit processes in the nuclear fuel cycle."
