"Unexpected movement of ground can potentially endanger lives, damage equipment or destroy property. Occupational accidents frequently occur with fatal consequences in developing countries with significant economic dependence on industries such as mining. Therefore, there is increasing need for miners' protection against such hazards. Slope stability and roof support accidents are two of the major causes of fatalities at surface and underground mining operations. According to national codes, employers are obligated to protect workers against accidents; however, these rules fall short of the standards in developed countries. National safety regulations should clearly specify support systems. The International Labor Organization (ILO) prepared two codes of practice, aiming to guide those responsible for improving standards of safety and to provide guidelines for drafting of safety regulations for the coal mine industry and quarry open cast mines. The practical recommendations of these codes in the ground control area have been analysed and the advantages and disadvantages of ILO codes concerning ground control are summarized.INTRODUCTIONTwo million people die every year from work-related accidents and diseases. An estimated 160 million people suffer from work-related diseases, and there are an estimated 270 million fatal and non-fatal work-related accidents per year. In economic terms, the ILO has estimated that 4% of the world's annual GDP is lost as a consequence of occupational diseases and accidents (ILO, 2009).Mining has always been dangerous due to many potential risks such as those associated with using machinery in enclosed spaces, rock falls, blasting operations and explosion of flammable gases, which endanger miners' safety and health. However, it is suggested that implementing new laws along with strict adherence would reduce such risks in the workplace.The severity and frequency of accidents are trending downward in developed countries, where the focus is on large private companies who invest heavily in mechanization and developing economies of scale. But in many parts of the world, particularly in developing countries, minerals are extracted by people working with simple tools and equipment; they work on a smaller scale, are more flexible, and can exploit irregular ore bodies and steep, dipping seams."

Total extraction mining of coal is being undertaken on a small scale by Anglo Coal South Africa. Longwall mining has been done in the group at New Denmark colliery since the 80s and recently the company started experimenting with pillar mining at Goedehoop colliery. Total extraction mining methods result in caving of the overburden strata that is often accompanied by displacement of air causing wind blasts. The windblasts can cause injuries to underground mining personnel, damage to mining equipment, and ventilation disruptions. Some incidents of windblast damage and injuries have been recorded at both New Denmark and Goedehoop Colliery that prompted mine management to experiment with the goaf warning device, an instrument developed by CSIR mining technology to give early warning of impending god. The initial version of the Goafwarn was originally developed to operate as a stand-alone unit, with LED lights mounted on the device that would flash if the unit was triggered'. That set up would be effective in small working areas where the flashing lights are visible to all workers. This requirement is more difficult to satisfy in longwall application of the unit where mining activities are spread over large linear distances of up to 250 m and the flashing lights would not be visible to all workers in the longwall face. It was also found that the single unit had limitations in distinguishing between man made vibrations and cracking in rockmass leading to an unacceptably high incidence of false alarms. The case studies describe the experiments that were carried out to ameliorate the two issues mentioned above through networking more than one unit. This was achieved through the development of radio communication from the warning devices to a central control (Trollex Data Acquisition Unit) from which the alarms are issued once the prescribed triggers are satisfied. The operating system was made up of 4 Goafwarn units installed 30 m apart with 2 in the tailgate and other 2 in the main gate. The alarm state was defined where 3 or more units were triggered simultaneously. The concept was applied successfully in real time during longwall mining, and management procedures have been developed to achieve safety through the system including switching off the shearer, and flicking the lights to warn people.

This paper concentrates on subsidence measurements, applied over the thick and steep seam mining in the Rocky Mountains Region of Western Canada. The studies to date indicate that two new subsidence monitoring techniques appear to be the most suitable methods for these conditions. The computerized telemetry and aerial photogrammetry systems are being tested for the reliability of the systems during the winter months within the net- work of laser surveying over the workings of the new hydraulic mine.

A procedure is presented for the design of an arch canopy for the application of rehabilitating a high-roof-fall area. This procedure is specifically developed for evaluating an unbackfllled arch canopy, which Is subjected to an impact load (roof fall). Dynamic line loading of the arch canopy at its crown is only considered and the procedure does not account for an unsymmetrical loading condition. The evaluation of whether an arch canopy is suitable for a particular installation is dependent upon many variables, including in-nine conditions and the engineering properties of the structure. However, a general evaluation can be based upon a comparison of the arch's maximum crown deflection and a prescribed allowable crown deflection. A single spring-mass system is used as an idealized model to represent the dynamic behavior of the arch canopy at its crown. An energy approach is taken to determine the maximum crown deflection of an arch rather than solving a differential equation of notion derived from dynamic equilibrium. The design procedure is based on the concept that when an arch canopy is subjected to impact loading at its crown and deflects from its unloaded state to maximum crown deflection, the structure absorbs strain energy, both elastic and plastic. As a result, this strain energy can be calculated from a static load-displacement diagram for the structure. The significance of this design procedure Is that It gives mine personnel an analytical tool to select an arch canopy to meet the dimensional and functional requirements of a mine entry and a prescribed allowable crown deflection.

"A coal burst occurred on 15 April, 2014 at the Austar Coal Mine, located west of Newcastle, NSW, Australia. The burst resulted in fatal injuries to two men working as part of the mining crew at the development face. At the time, a continuous miner was being used to mine a longwall development gate road through heavily structured coal, at a depth of approximately 550m. A number of pre-cursor bumps had occurred on previous shifts, emanating from the coal ribs of the roadway, in proximity to the coal face.This paper reviews the geological, geotechnical and mining conditions and circumstances leading up to the coal burst event; and presents and discusses the available evidence and possible interpretations relating to the geomechanical behaviour mechanisms that may have been critical factors in this incident. The paper also discusses some key technical and operational considerations of ground support systems and mining practices and strategies needed for operating in such conditions in the future.INTRODUCTIONAustar Coal Mine is an underground longwall coal mine located near Cessnock in the Hunter Valley of New South Wales (NSW), Australia and is the only underground mine still extracting the Greta Seam in this region. The mine was the first in Australia to adopt the Chinese-developed Longwall Top Coal Caving (LTCC) method for thick seam extraction. Typically seam thickness ranges from 4m to 7m and depth of mining from 480m to 560m, with future mining planned down to depths of up to 700m, making it one of the deepest operating coal mines in Australia.On 15 April 2014, a pressure burst occurred in the left hand rib at the active mining face of B Heading, 2 to 3 cut-through, Maingate A9 panel, during development of the gateroads for the ninth longwall top coal caving panel. Strata in the general vicinity was affected by disturbed geology and multiple geological structures. Figure 1 shows a section of the mine plan as at October 2014 (six months after the accident), indicating the current longwall extraction panel (AS) and the development panel A9 where the accident occurred. (Note: Neither development nor longwall face positions changed significantly between the time of the accident and the date of this plan). At the time of the accident the current longwall face was in excess of 1,000m away from the Maingate A9 development panel face position."

The excavation of a longwall panel causes a continuous stress redistribution in the surrounding strata for every face advancement. The understanding of the behavior of rock to high level of stresses is instrumental to the structural design of underground mines. In order to achieve a realistic analysis of the behavior of strata, the three-dimensional finite element method is frequently used. Also the model developed for the analysis has to be designed large enough to represent the influenced strata in the study area and a sufficient number of elements has to be used to provide detailed information on the stresses and displacements. Due to the size of the model and its numerical solution the CRAY X-MP/24 supercomputer has been used for this study. The mine site selected for the analysis is a coal mine located in the Black Warrior Basin in Alabama. The mine has two longwall sections at a depth of 2,000 feet with various dimensions of chain pillars, including yield pillars. Various research programs at this mine have been carried out by Park et. al. ( 1984 ). Therefore data were already available on the geology, and physical properties of rock and coal. In addition a rock testing program has been carried out to obtain rock mass classification parameters. The progressive failure technique has been utilized to simulate stress redistributions. The failure criterion after Hoek and Brown ( 1980 ) which is based on rock mass behavior was used for the iterative technique. Simulated stress redistributions are therefore very realistic. In order to automate the process involved in the application of progressive failure technique, a computer program has been developed. The combination of a large scale three-dimensional finite element model and the automation of progressive failure simulation provided a tool that can generate a large amount of information related to the behavior of rock and coal as a response to a longwall mining operation.

The Skybolt is a new bolt concept, point anchored with resin in large diameter holes of specific sizes 38 mm and 45 mm. The new design achieves the optimum annular spacing required for mixing the resin anchor components which is essential for optimum resin bonding of the bolt to the surrounding rock. This paper explains the load transfer mechanisms between bolt/resin/rock, unique to this bolt concept. It compares the pull-out test results to those of paddle and expansion shell type bolts, and highlights the versatility of Skybolt over existing, currently used, rock support systems and the benefits mine operators can achieve when using the Skybolt.

Patton Mining, LLC and J.H. Fletcher Company recently collaborated and mechanized the sometimes hazardous, and always labor intensive, process of installing standing steel supports during the construction of a slope entry of a new mine. The new innovation is a beam setting machine design that can lift and maneuver heavy steel beams into place over and around the belt and vent tubing. In addition, the machine is designed with an integral double extending lifting work platform to allow workers a stable work area from which to install metal sheeting between the beams. The previous method to install the heavy steel supports and the steel sheeting included multiple workers lifting heavy loads frequently on stepladders. This paper will inform the reader of how Patton?s major goal to develop the safest, most continuous slope development process possible was achieved and the beam setting machine that evolved from the project.

The subsidence over a longwall panel at Tahmoor Mine in the Southern Coalfield of NSW, Australia, was found to be approximately twice the size it had been in previous measurements. An investigation into the potential causes was conducted using computer modeling together with hydrological characterization and detailed geotechnical characterization of the strata. The abnormal subsidence was found to be consistent with localized weathering of joint and bedding planes above a depressed water table adjacent to an incised gorge. The study showed that other factors such as variation in stress field, joint zones, variation in rock strength and topographic factors did have sufficient impact to induce the abnormal subsidence. The results have significant implications to subsidence prediction in areas that may be prone to the phenomenon found at Tahmoor. Key indicators of the potential for this form of abnormal subsidence are presented.

The conditions of great mining depth and multi-seam¬longwalling require a special method of rockbolting, It can be described as a control loop which is based on the elements of planning, drivage and measurement. Different planning elements such as the results of the observation of physical models, numerical programmes for calculation of stress-distribution and support success, lab and underground support testing and the knowledge of strata behaviour underground including geotechnical classification are used to get best results. The increasing geotechnical information collected during drivage of roadways are used to estimate and minimise the risks of support-overload. The repeated plan/acutal comparison during the use of roadway and continuously monitoring of support and roadways provided the opportunity of optimising support design during the process of drivage and if necessary in front and after of the longwall face.

The successful extension of the applications in which rockbolts can be used in the German coalmining industry is, because of the very great raining depth combined with a high degree of stress in the native rock, only possible on the basis of a comprehensive quality management system. The quality assurance system consists of material tests on the individual component parts of a rockbolt, of test rig trials of installed rockbolts, of suitability tests underground and of measures for quality assurance during roadway drivage. Only on the basis of exact knowledge of the way in which installed rockbolts function in the underground and possible sources of error and faults can quality assurance be guaranteed during roadway drivage.

After 34km of roof bolted drivage the first fall of ground occurred at the Phalen Colliery in the maingate of the 8th East longwall panel. The fall occurred over the stage loader as the wall approached an area of poor roof geology. This area had developed a severe cutter since initial drivage and the longwall had yet to mine through 500m of even worse conditions. This paper describes the cause of the poor roof conditions, the use of polyurethane resin injection and cable bolting to re-stabilize the roof outby the fall, and the measurement of roof movement and performance subsequent to the continuation of mining under the rehabilitated roof.

Using 3D numerical modeling as a tool to predict rock behavior during mining extraction is still an uncommon practice at many underground operations. This paper will discuss how 3D numerical models were used to identify potential underground instability problems in the later stages of mining. Detailed modeling was then used to make modifications to the mine design in order to minimize the impact of the potential instability problems. This paper will also discusses how the back analysis of a previous underground collapse event at the same mine in a room and pillar area was used to identify the instability problems for a new orebody being developed deeper in the mine. A 3D numerical model was first created in the MAP3D software to determine the conditions of the pillars and the magnitude of the vertical displacement above the mining zone prior to the underground collapse. The results from the ground collapse model were then used as a threshold limit for predicting potential instabilities and developing a model for the new mining zone. sing the results from this analysis, mine management has been able to implement a much higher percentage recovery of the reserve in a new, high-grade area of the orebody while minimizing potential instabilities in the future.

The mechanics of load transfer of fully-grouted roof bolts in coal mines has been investigated in laboratory and analytical studies. The purpose of this research is to increase the understanding and better interpret data from pull tests which are routinely performed in the field. Laboratory work includes variation of bolt length, hole diameter and grout type in pull tests. Four-foot bolts were tested in holes of 1- and 1-3/8-inch diameters. Both polyester resins and inorganic grouts were used as the anchorage medium. The bolts were strain gauged at six locations to observe the rate of load transfer from the bolt to the rock mass when a force was applied at the bolt end using standard Jack pull testing procedures and equipment. Analytical studies using closed form solutions and finite-element techniques were compared to experimental results. The models are for the elastic regime only but do include slip elements. A detailed analysis of the load transfer mechanisms from the bolts through the grout to the rock mass is presented.

An investigation into the performance of support systems that are currently used in South African coal mines was conducted. Five most critical components of a support system have been identified. These components are resin, bolt, hole, machinery/equipment and the rock type. All five of these components are equally important as failure in any of these components will result in an inadequate support system. The performance of resin used in South Africa was investigated through in situ short encapsulated pull tests. The results indicated that in the majority of pull tests, failure took place at the rock-resin interface, indicating that the rock failed before the resin shear strength had been reached. It is therefore concluded that the strength of resin currently used in South Africa is adequate. It is found that although the maximum bond strengths achieved form both resin suppliers were similar, there was a considerable difference in the stiffhess of the resins. Roof bolt diameters and rib heights of bolts from three major manufacturers were evaluated (approximately 80 roof bolts from each manufacturer). The results showed there is a significant variation in bolt diameters and rib heights in the same batch in roof bolts supplied by certain manufacturers. The results also showed that on average there are insignificant differences between the parameters that determine the bolt profile (rib angle, spacing between the ribs and rib thickness). The influence of these parameters on bolt performance was very small and could not be established in situ. Roof bolts from four major suppliers were also evaluated through in situ short encapsulated pull tests. The results showed that the reinforcing system using roof bolts from all four manufacturers performed almost identically in sandstone, but somewhat differently in other rock types. The effect of bit types was evaluated. Both the stresses and the maximum loads obtained from the two-prong bits were greater than those obtained from a spade bit. Borehole annulus was found to be another important parameter that determined the support performance, and was investigated. The results from these tests showed that an annulus between 2.8 mm 4.5 mm resulted in the highest bond strengths. The results also showed that as the annulus drops below 2 mm, it appears to have a negative effect on the bond strength. The effect of wet and dry drilling on support performance was investigated. The results showed that wet drilling can-provide relatively greater system stiffnesses and slightly greater bond strengths. Short encapsulated pull test results showed very distinct differences between bolt system performance in different rock types and that sandstone produces significantly better results than shale and coal. From the results it is concluded that rock type is probably the primary factor influencing support system performance. A detailed investigation into the specifications of roofbolters that are currently being used indicated that the quality of installation of a support system is directly related to the performance of the equipment that is used to install the bolts. The important parameters, torque, thrust and speed of bolting equipment were investigated. The results showed that there are significant variations in these parameters in South African roofbolters.

Longwall mining technology without interpanel pillars is feasible from a geomechanical point of view. A set of pillarless longwall layouts are proposed including tailentry pillar recovery, headentry pillar recovery and drivage of a new tailentry, partial headentry pillar recovery and combination mining in which advancing panel entries are used for retreating panels. The following mitigative measures are proposed in order to enhance the stability of entries near the panel: driving crosscuts obliquely to the longwall face line, advance support of entries and crosscuts, localized destressing of the chain pillars by horizontal slotting, and partial stowing of the gobs behind the pillars being recovered. A new technology of recovering barrier pillars on the setup entry side is also proposed. This technology uses the same regular longwall face for interpanel pillars extraction.

Significant coal reserves in New South Wales lie under areas with steep topography such as valley slopes and cliffs. Subsidence predictions are difficult to make for these locations as existing empirical databases cannot cover all permutations of topography and mining geometry. Two dimensional numerical model alternatives are of limited applicability. Consequently a novel three-dimensional numerical model is developed including the detailed panel layout, realistic rock mass properties including the full stratigraphy, goat behaviour and actual surface topography. Model results show the same trends as subsidence measurements from Baal Bone Colliery in the Western Coalfield of New South Wales

Mobile Roof Supports (MRS) look similar to a longwall shield on crawler tracks. The MRS units offer a safe, productive method of extracting the 40% to 60% product left normally unrecovered in room and pillar mining. The units feature radio control, self-contained power units, crawler track drive, and a 600 ton yield. Cribbing and setting posts in retreat mining has been a necessary burden to production. The MRS reduces this burden. Long blocks of coal can be developed to further reduce move and tram time, thus increasing production. The MRS units have successful safety records. The workers are removed from the immediate fall area, and we feel back injuries have been reduced by the workers not setting posts. While MRS units have been damaged in falls, the other capital equipment has been unharmed. The MRS product line has undergone normal product improvement and evolution. Product evolution is seen in our development of machines for particular mine requirements. Through the continuing development of the MRS, Fletcher presents the coal industry with a safe, productive method of retreat mining with potential to develop new mining methods.

Pre-feasibility work at Goedehoop Colliery indicated that stooping of the undersized pillars could be undertaken safely and economically. In light of the findings a decision was taken in may 2003 to undertake the project in the 600 area where three panels (628, 630 and 632) were identified for pilot test sites. The findings of the pre-feasibility studies were presented in a paper to the 22nd International Conference on Ground Control in Mining held at Morgantown in 2003 (1). This paper is a progress report for the project 12 months down the line. Work commenced to prepare the sites for mining including, acquisition of equipment, equipping the panels, selecting and training people to undertake the project. Three monitoring sites were set up distributed evenly along panel 628. Each site comprised of a surface extensometer station in the overburden strata adjacent to a pillar underground containing hard inclusion vibrating wire extensometer cells. This combination would be used to record pillar loading ahead of stooping as well as the depth of caving and strata deformation in the goal zone behind the stooping face. In July 2003, support work began to secure the roof and rib sides. The support was required to be at least three-pillar center distances ahead of the mining front to ensure that the effective stooping zone is secured at all times. The NEVID method of pillar extraction developed at Sasol and described in detail by Ismet and Moolman (2) in a Coaltech 20-20 report was adopted for the pilot project. As part of training the selected stooping team was send for a week to Sasol's Bosmanskrans Colliery to familiarize with the NEVID method of stooping. Further training sessions were carried out on site especially on the use of Mobile Roof supports. The first cut was taken on 20 August 2003 by implementing a form of chequerbord mining taking every second row of pillars. Three rows were mined this way which period was considered training to get workers familiar with the new equipment and mining method. On the fourth row the full design span equivalent to mining 4 pillars in a row was implemented taking out all the pillars in successive rows. Production figures gradually increased from 300 to more than 1,000 tons run of mine coal per shift. Panel 628 was mined out at the end of December 2003 and the second of three panels (630) is being mined. Substantial data from the monitoring set up has been recorded since the start of the project. Monitoring will continue to the end of the pilot project and the data will be analysed and fed back into the design tools to improve the design in future undertakings. The pilot project has been a success so far and other old workings are being evaluated for stooping.

In designing a proper roof support system, one must know the features of roof geology in advance of mining. The drilling parameters obtained during normal roof bolt installation cycle can provide a large amount of information on the roof geology when properly interpreted. It is also an economic way for identifying the geological features of the roof. This research develops a method for predicting the roof geology based on the drilling parameters obtained during roof bolting operation. From the results of a series of laboratory and underground tests, feed pressure is found to be a good indicator for identifying the voids/fractures and estimating the roof rock strength. This paper describes the method for determining quantitatively the location and the size of void/fracture and estimating the roof rock strength from the drilling parameters of roof bolter.
