Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a ?plateau? and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have ?exemplary? safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA?s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization?s mindset? What characterizes these organizations? decision making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensive research since 1997 of organizations that suffered unexpected disasters. Their huge focus on safety, the achievement of lofty goals and celebration of safety successes led to ?mindset? that they are leaders in safety, protected by an extraordinary safety system and that their safety incident figures indicated a truly world-class safety performance. It led them into the trap of the seven deadly delusions?

"A process is proposed for the treatment of phosphate rock for the recovery of uranium and the lanthanides. The process assures the production of phosphatic fertilizers without polluting the environment with radioactive material.IntroductionWhile many processes for uranium recovery from phosphoric acid produced by the H2S04 route are known and utilised commercially, little is known about uranium recovery from phosphoric acid produced by the HN03 route1. Advantages of the latter route are both the elimination of the gypsum disposal problem, and the possibility of radium disposal in a controlled way2. Further, the lanthanides originally present in the rock can be recovered if the nitric acid route is used; they will be mostly lost in the gypsum produced by the sulphuric acid route3. In the present work a scheme for the recovery of uranium and the lanthanides from phosphate rock without interfering with the normal route for fertiliser production is proposed using HN03 as a leaching agent. Attention was also given to uranium decay products and their separation so that they might not contaminate the fertiliser4. Further, an attempt was made to defluorinate the leach solution in order to upgrade the fertiliser and at the same time recover the fluorine5. Rapid analytical methods for the determination of uranium, radium, and lanthanides in phosphate rock were developed6,7."

"Wholly owned by the Coniagas Mining Company, the oldest continuously listed mining company on the Toronto Stock Exchange, St. Andrew Goldfields is located on Hwy. 101 approximately 30 miles east of downtown Timmins, Ontario. Geographically situated on the Porcupine-Destor Fault, the mine and mill in Stock township is positioned in the heart of Northeastern Ontario's goldmining communities.In July 1988, a 600 ft. shaft, hoist, and compressors were already in place when construction started on a 500 T.P.D. capacity C.I.P. mill and crushing plant.By June 15, 1989 the mill was commissioned and attained the design capacity by August of that same year. "Access to the outside world is supplied by the Alaska Highway to continental Canada and the United States, the Haines Highway, and by the White Pass & Yukon Route to Skagway and Vancouver. Communities and mining developments located along the Alaska Highway are supplied daily by truck transport with commodities originating in Whitehorse, Edmonton, and Vancouver.The average payload on the Alaska and Mayo Highway is in the order of 16 to 25 tons and is subject to government regulations. Highway haulage distances to railhead at Whitehorse vary as does the direction. The Clinton Creek asbestos development is located 395 road miles north-west of Whitehorse whereas the United Keno Hill operation is 287 miles to the north. Cassiar Asbestos is located in Northern British Columbia at a distance of 357 miles south-east of railhead at Whitehorse. In order to service these various mines multipurpose highway vehicles have been designed to allow maximum backhaul of petroleum products. The White Pass & Yukon Route was established as an integrated transportation system from 1955 when it became apparent that three non-integrated systems (ship, rail and truck), with the various paper work and rising costs, could not continue to serve the needs of Yukon. Integration became the company policy and a system was developed. A special container ship (Clifford J. Rogers) was designed and built. This was the first specially built container ship in the world. The northern container integrated system was operational in 1955.In 1965 over 8.5 million dollars were spent in upgrading the container facility and the construction of a tanker-container ship designed specifically to serve Yukon and Northern B.C. This paper presents the history of uranium procurement and demand, and takes a look at future requirements. The three basic conclusions drawn are: (1) the productive capacity of former and existing producers whose operations may be profitably reactivated is insufficient to meet demand projected beyond 1975; (2) reserves in the $5 -$10 price range do not provide the flexibility to sustain production at the high rates required in the late 1970's; and (3) only higher prices can overcome these deficiencies, as this would provide sufficient inducement for the industry to explore for and develop new reserves and productive capacity.

"Production commenced at the Golden Sunlight Mine in 1983 and at Kidston Gold Mine in 1985. These projects were both constructed under the management of the Placer Dome Inc. (Technical Services) group of Vancouver, B.C.Golden Sunlight uses a conventional rod mill/ball mill grinding circuit while Kidston uses a SAG /ball mill, and later on, SABC circuit. Since the hardness of the ore and the fineness of the primary grind in both mines are about the same, a meaningful cost comparison can be made. The comparison is made on the basis of capital and operating costs.The result of this study indicates that a significant capital cost savings can be achieved in favour of the SAG /ball mill circuit when the ore is amenable to semi-autogenous grinding. With a good maintenance program, the operating cost for the SAG /ball mill circuit could also be lower that the conventional rod mill/ball mill circuit.HISTORYGolden Sunlight MineGolden Sunlight is a 5 530 t/d gold mine located near Whitehall, a small town 60 km east of Butte, Montana. Originally optioned by PDI U.S. in 1958, the development of the property waited until 1981 when economic conditions became favorable for the project to proceed.Semiautogenous grinding tests were conducted during the final stages of the feasibility study. The hard near surface ore was not quite amenable to semi-autogenous grinding. The best result produced from a series of tests using a 1.83 m diameter mill was a semiautogenous work index of 39. This is substantially higher than the 22 Bond work index of the sample used in the testwork. A conventional SAG/ball mill circuit for this ore will be extremely inefficient, and for this reason, this circuit was rejected in favour of a conventional rod mill/ball mill circuit."

"Hydrocyclone modelling and selection procedure is illustrated. Computerized procedure for hydrocyclone cut size determination for grinding and non-grinding applications is described.A standard procedure for evaluating operating hydrocyclones is described in detail. Many case studies are illustrated comparing actual plant performance with the computer simulation output in terms of overflow product size distributions.INTRODUCTION Cyclones are widely used for applications such as: close circuit grinding, backfill, tailings and dewatering. Normally, cyclones are used for very fine separations where screens are not suitable or too expensive. Figure 1 shows the separation size range of various classifying devices. Cyclones are popular because of their low capital and operating cost. Cyclone selection procedure for different applications depends on the amount of information available on the cyclone feed slurry characteristics. Closed circuit grinding applications will be discussed in detail in this paper. The Cut Size:The size of the material in the feed which reports 50% to each product stream (i.e. underflow and overflow), after correcting for the feed solids by-passing with the underflow liquid, is termed the cut size or ""d50c"". This is often used as the criterion and the basis for cyclone design.When the corrected recovery to underflow of each mesh size in the feed is plotted against the geometric mean size in microns on a log scale, the micron size corresponding to 50% recovery is the d50c size."

Modern underground mines operate fleets of mobile production equipment including drill jumbos, long-hole drills, load-haul-dump machines, trucks and bolters. Their availability has a substantial impact onto production cost and thus onto mine profitability. Through appropriate collection of pertinent data and information, use of adequate performance measures and their analyses, mine operators can identify the most promising and cost effective opportunities to get more from their equipment and to reduce the overall cost. This paper presents results of the study aimed to review and analyse means and measures used to quantify equipment availability and its contributing factors (reliability, maintainability and maintenance support) in several Canadian underground mines. It overviews current reporting procedures, sources, quality and quantity of the data and information collected as well as examines performance measures and their definitions used by the surveyed mines. Problems concerning data acquisition as well as meaningfulness and usefulness of the above mentioned metrics are also discussed. The paper concludes with some suggestions and recommendations concerning several aspects of equipment availability assessment. It is believed that the latter may become a more useful tool for mine operators in their efforts to reduce production cost than it seems to be the case at present.

"Heath Steele Mines is a base metal producer situated near Newcastle, N.B. presently treating 3500 TPD of complex Cu-Pb-Zn ore. The ore consists of massive fine grained sulfides averaging I. 1% Cu, 1.6% Pb and 4.5% Zn.The original mill started in 1956 at 750 TPD and was successively expanded through 1500 to 3000 TPD in 1969. In 1972 an expansion to 4000 TPD was initiated. It was planned to obtain the maximum benefit from equipment and manpower by adding some process equipment, instrumentation, an on stream analyser, and a process control system. The benefits from this approach could be greater than normally expected, because of the complex metallurgy of this ore.IntroductionThe crushing plant is of conventional design with two 5 ½ ft, Symons cone crushers with the crushed ore screened to -7/8 inch on a 61 x 16 1 Al I is Chalmers vibrating screen and stored in four 1000 Ion bins. There are two para I lei grinding circuits, each one consisting of the following: one Rod Mill in open circuit, one Ball Mill in closed circuit with two 20"" Krebs cyclones, and one Ball Mill in closed circuit with three 15"" Krebs cyclones, Al I the Mills are 9' x 121 Al I is Chalmers Mills. The secondary cyclone overflow from each grinding circuit is thickened and combined to form flotation feed. The final product is 65% - 400 Mesh. Two regrind mills have been installed as part of the expansion."

"INTRODUCTION Extensive studies have been carried on pure gold to determine the various factors that affect its dissolution in alkaline cyanide solutions. With the extension of milling to refractory ores more intensive investigations on the effect of impurities in the ore on the dissolution kinetics are needed. The present work describes experiments carried on refractory flotation tailings with the object to determine how the recovery of gold could be increased. The effect of arsenic and antimony in the ore and the excessive use of lime are discussed. "Since the late 1990s, increased prevalence of severe lung disease among US coal miners has been observed, particularly in central Appalachia where thin-seam mining is prevalent. While respirable silica exposures have been implicated in many cases, the available dust monitoring data from mines shows no explanatory trends. One possible factor could be differences in the relative toxicity of silica particles based on their surface condition. Previous research has indicated that particles with occluded surfaces (e.g., by a thin layer of clay) may be less toxic than those with non-occluded surfaces. However, this dust characteristic has not been investigated before now in central Appalachian mines. In the present study, 18 respirable dust samples from seven central Appalachian mines and two northern Appalachian mines were analyzed to assess the relative occurrence of surface-occluded silica. Results showed that the dust from central Appalachian mines actually had higher fractions of occluded silica, which may be due to extraction of large amounts of rock strata along with the coal. However, the central Appalachian mines also more silica particles overall (i.e., occluded + non-occluded) in terms of total number percentage of respirable particles.INTRODUCTIONResurgence of occupational lung disease among US coal miners, especially in the central Appalachian region, has been observed since the late 1990’s (Gibson, 2016; Zosky et al., 2016; Suarthana et al., 2011; Gamble et al., 2011; Antao et al., 2005; Attfield & Sexias, 1995). In addition to increased prevalence of simple Coal Workers’ Pneumoconiosis (CWP, or “black lung”), the prevalence of severe and rapidly progressive disease has risen dramatically (Almberg et al., 2018; Blackley et al., 2018a, 2018b, 2016, 2014; Graber et al., 2017; Cohen et al., 2016). While exposures to respirable silica have long been known to cause or play a role in diseases such as silicosis or progressive massive fibrosis (PMF), which is conventionally considered as the most advanced form of CWP, it is unclear whether silica is the only culprit in the recent outbreak of severe disease. Indeed, as disease rates have risen, both total respirable dust and silica concentrations generally appear to have declined since the late 1980s, based on regulatory monitoring data in the most affected mining districts (i.e., MSHA coal Districts 4, 5 and 12, which cover central WV, eastern KY and southwestern VA, and southern WV, respectively) (Figure 1). Of course, the latency of disease can vary and may be or more 10 years, but many of the most recent cases are likely to have resulted from exposures that occurred since the late 1980s."

The thermodynamics of the reactions taking place are examined with the aid of equilibrium diagrams for the Pb-O-S system. Based on investigations of chemical, mineralogical and differential-thermo analyses of intermediate products of different desulphuration degrees, an attempt is made to explain the mechanisms of the reactions in the smelting shaft of the KIVCET furnace. The differences between this and other direct lead smelting processes are demonstrated by these thermodynamic and kinetic considerations. Proceeding from these results, the practicability of direct lead smelting processes is discussed with reference to the composition of the primary feed materials to be processed. This comparison also considers the behaviour of minor metals, the products obtained and their characteristics in respect to further treatment. For assessing the operating costs, the over-all energy consumption is considered taking into account various price relations between electric energy and fuels/reductants. Based on the peculiarities of the various processes, an attempt is made to compare the necessary means of meeting the pollution control requirements. The paper concludes with reference to the criteria and problems involved in comparable evaluations.

"INTRODUCTIONA beneficiation plant to upgrade earthy iron ores is being installed by the Iron Ore Company of Canada in Sept-Iles, Quebec.The crude ore for the plant will come from the Knob Lake area, located in Northern Quebec, where mining has been in progress since 1954.Some 170 million tons of ""direct 11 ores have been mined and shipped in the past eighteen years, with an overall silica content of 7 to 8 percent. No beneficiation - except the reduction of the top size to about 2 inches - took place during this period, but extensive research work has been carried out, in order to find a feasible process.Since “washing"", gravity separation, roasting and magnetic separation experiments did not indicate positive enough results, flotation was tried and found to be feasible.The beneficiation complex being installed will consist of grinding, flotation, dewatering - i.e.: thickening, filtering and drying - balling feed preparation, and agglomeration - i.e.: balling and induration.This process will result in a product superior to the ""direct shipping"" ores and will also expand the ore reserves by allowing material with a higher silica content to be utilized.THE OREThe Knob Lake ores are grouped by mineralogical composition and by grade.BLUE ORES, representing roughly two thirds of the reserves, carry their iron content mostly in the form of massive, blue hematite and martite.YELLOW ORES, about one sixth of the reserves, have predominantly hydrated iron oxides as the iron bearing minerals.RED ORES, the other sixth of the reserves, carry the iron mostly in the form of red hematite.The gangue in all ores is mainly quartz, but aluminum oxides and hydroxides are also present, particularly in the red ores."

"Competitive pressures dictate that the mining industry moves to larger mobile underground haulage equipment with more advanced equipment mechanization, higher levels of autonomous operation of machines, and longer periods between access by maintenance personnel. Since such new machines will of necessity be more complex than existing designs, machine reliability has become a key issue in progressing toward advanced mobile underground haulage equipment. Reliability and maintainability models of such equipment are practically non-existent in the literature. This paper addresses the issue of reliability and maintainability of mobile underground haulage equipment on two fronts: reliability modelling to enhance decision making for maintenance planning; and reliability simulation to enable evaluation of more rigorous production planning scenarios. A case study from a gold mine in the Chilean Andes is presented, for which reliability models of mobile underground haulage equipment, specifically load-hauldump vehicles and underground haul trucks, are developed, and a reliability-based simulation implemented. IntroductionMaintenance accounts for 20% to 35% of the overall operating budget for a typical underground operation, and hence represents the largest portion of the mine’s controllable operating costs (Cutifani et al., 1996; Campbell, 1997; Goddard, 1998). "Bell Creek Mill which has been in continuous operation since 1987 has undergone numerous operational changes in the past three years. The addition of over 2. 5 million tons of resource at over 13 git from Hoyle Pond Mine precipitated an expansion program for the Bell Creek Mill. This paper discusses the major changes that have occurred over the past two years to increase throughput and efficiencies. In addition, the refractory ore treatment options are also discussed.IntroductionThe Bell Creek Mill is located in the Timmins Goldcamp approximately 8km NE of porcupine Ontario. (Fig I.)The mill was originally constructed by Camamax in 1987 to treat the Bell Creek ore at a design throughput of 350tpd. The flowsheet (Fig 2.) consisted of two stage crushing, grinding, thickening, leach and drum filtration with gold recovery by Merrill Crowe.As ore depleted at Bell Creek, the Mill was purchased by Falconbridge to process ore from their Hoyle Pond property. Kinross purchased the Hoyle Pond property and Bell Creek Mill from Falconbridge in 1992.The successful exploration program at Hoyle Pond has led to several expansions and process design changes, with further changes planned over the next twelve months.In 1994, the mine produced 61,865 oz. of gold at an average cost of 197US$ per oz. In 1995 production was 91, 611 oz. at a cost of 187US$ per oz. Projecting the nine months ended September 30, production for 1996 will be 155,000 oz. at a cost of 165US$ per oz.In early 1997, the CIP expansion will be complete, with a target of 175,000 oz. at a cost of 176US$ per oz for 1997. The mill throughput will have increased from 400tpd to 1500tpd between 1994 and 1997.The changes to Bell Creek since 1994 have been numerous with very little of the existing equipment still in operation. The total cost of the expansion to date is 15 million C$. This paper discusses the major projects carried out during in this time. The focus will be to highlight the justification of each project, the design and engineering, some of the initial start-up problems and solutions.Figure 3 shows a plan of the existing mill facility together with the various additions that were carried out."

Broad band mine communication today is characterized mainly by individual solutions, which have in common their long development phases and their high cost for development, operation and service. The use of Ethernet and TCP/IP on the other side provides an uniquely price efficient combination of high bandwidth open networking, universal use and accessibility of cheap development tools, especially because it forms the technical infrastructure for the worldwide Internet. TCP/IP and Ethernet assures reliable networking however it is not able to guarantee the latency timing of forwarding network information. Therefor, the network design requires strict load limitation and application level traffic control especially if moving equipment has to be manually remotely controlled via the network. To optimize the use of bandwidth on mining networks using load limited, ?bandwidth wasting? local Ethernet segments, long distance broadband traffic should be carried out using more efficient, deterministic communication backbones providing a guaranteed quality of service for each of the physically separated networks. Open networking and broadband systems however may lead to the anticipation that any demands on bulk data or multimedia traffic can be fulfilled in the future. This definitely will not be the case as the bandwidth demands of applications will be growing as least as fast as the bandwidth provided by communication systems. Looking on those problems from an overall system integration perspective, machines and equipment in the future have to be equipped with intelligent computing devices capable of an intelligent local data preprocessing. Such ?Embedded Machine Information Servers? running commercial computing power in the future will be the intelligent bridge between open networking and a secured, high safety local automation system. These ?Embedded Machine Information Servers? then also can make advantage of intelligent video and audio data preprocessing to store and transmit multimedia information ?on demand? or when the application requires it. Summarizing the potential for the future Ethernet based networking together with powerful communication backbones and intelligent local data preprocessing on the machines in the future will lead the way to integrative, reliable and cost efficient mine automation solutions.

It has long been appreciated that chloride chemistry has a number of advantages over the more traditional sulphate route for both extracting and separating metals. Modern materials of construction, coupled with the development of a more cost-effective and efficient hydrochloric acid regeneration technology mean that chloride-based processes are now more competitive and attractive. Such processing routes are able to recover more of the contained metal values in a feed, including iron in a benign and/or marketable form, and particularly the so-called rare and rare-earth elements which are increasing in demand in our electronic age. These factors, therefore, add appreciably both to the overall economics of a project, but also offer a much more sustainable approach to our dwindling natural resources. Additionally, chloride-based flowsheets can be much more environmentally-friendly, offering practical alternatives to two of the biggest headaches faced by the industry, namely cyanide usage and the generation of red muds. This paper presents a brief theoretical background, focusing on the latest developments of ferrous iron oxidation and the low-temperature, selective hydrochloric acid regeneration process, which is the key unit operation for any chloride flowsheet, and discusses in general the advantages of the chloride approach. Several recently-developed flowsheets, which have been tested at the miniplant level, are presented for gold, base metals, titaniferous magnetites and lead-silver ores wherein the advantages of considering a chloride approach are highlighted.
