The typical management systems used to address the issue of operating risk centre around technologies that identify, analyse and evaluate the risk factors. These logical systems such as HAZOPs, Fault Tree Analyses, etc rely heavily on technical considerations and process flow analysis. However, several research projects, across industries and countries have clearly shown that up to 80 per cent `operating risk' is determined by the actions of workers, supervisors and managers. Yet, most of the focus of risk management is on technical and procedural issues. This suggests that a significant imbalance exists in our management systems and that industries are unlikely to show significant improvement on current levels of safety performance. Effective risk management must include the intangible element of the organisation psychology towards risk. A practical and theoretically sound approach had been researched and developed in and for the mining industry. This paper describes the approach and methodologies applied at the Cosmo Howley Gold Mine in the Northern Territory, where an inclusive approach to managing financial and operating risk was followed. The Cosmo Howley Mine showed a dramatic reduction in lost time injuries and improvements in financial indices - providing conclusive evidence of the link between management effectiveness, safety and productivity. This `triangle' has often been the subject of speculation and theory. It is demonstrated in this paper how the management team of Cosmo Howley focused their practices and systems on the improvement of organisational dynamics, ensured tangible and clear focus on parameters of costs and safety and how the improvement process was managed. It is concluded that the improvement process did not require a `major re-engineering' of the organisation. The organisation culture changes that occurred were incremental, planned and effective. It entailed the implementation of sound risk management methodologies, and ensured that a significant emphasis was placed on an aspect that is defined as `organisational dynamics'.

THIS note gives comments on the paper by Kitaev et al on p. 68 of this book and compares results with those obtained by ourselves. We wish to say that the authors are to be congratulated on their sophisticated approach to an important problem and that it complements our efforts on flow maldistribution.1-5 It is our view that Eqs. (1)-(5) on p. 69 are impressive, but unfortunately with present day computers and computational facilities, it would be quite impractical to contemplate the numerical solution of these. Eq. (8) in the authors' paper, on p. 70, does provide the logical starting point for tackling flow maldistribution problems and strictly speaking this equation is more accurate than Eq. (1) in our paper in this book. We note in passing that there is direct experimental evidence to support the contention that our Eq. (1) is at least a reasonable approximation and one may question in a practical sense whether the refinement afforded by Eq. (8) in the paper by Kitaev et a/ is justified. While one must readily accept the appropriateness of Eq. (8) of Kitaev et al as a starting point, the subsequent mathematical manipulation by the authors is open to serious criticism. In order to solve this complex, non-linear equation Kitaev et al postulated irrotational flow, which afforded considerable simplification. One can argue simply on physical grounds that the assumption of irrotational flow is inappropriate, a fact which should be readily apparent on inspection of the flow fields shown in Figs. 2 and 3 of Kitaev's paper. It follows that while the starting point through the use of Eq. (8) is correct the results derived from this starting point may be suspect because of the erroneous nature of the irrotational flow assumption.

The Lafarge Marblehead Plant in Marblehead, Ohio, USA produces 4.2 million tonnes of sized aggregate yearly. In 1997, a program was instituted to maximise primary crusher productivity through the optimisation of blasting variables and procedures. A proprietary blasting shot report database and the Lafarge production database was statistically analysed to provide guidance in the program. Blasting powder factor (kg/m3) was the main control variable utilised to increase crusher throughput. In 2005, the program was refined to increase the production of certain high margin aggregate sizes and the reduction of fines. The principal blasting component affecting the percentage of high margin size production appears to be pattern area (burden and spacing area). This can be coupled with powder factor to optimise the production rate of high margin aggregate sizes. The size optimisation program incorporates the blasting shot report database, Lafarge production database, statistical analysis and aggregate sampling. A preliminary indication is that the production of high margin sizes (12.7 mm X 4.8 mm, 24.4 mm X 12.7 mm, and 50 mm X 25.4 mm) has been increased seven to 12 per cent utilising these techniques. As an added benefit, blasting costs have been reduced linearly as burden û spacing pattern areas have increased and powder factors have decreased. The program is ongoing and will investigate other blasting procedures and variables including electronic blasting caps to further increase the production of high margin products. The statistical methods involved in the analysis are described in this article for use in other mines and situations. The methods described can be used to optimise crusher throughput, digging rates and product size.

In this paper, it is briefly introduced that China's Ordovician limestone confined-water menaces coal mining and Ordovician water is prevented and cured to secure the safety of coal mining. The theory of "Down-Three-Zones" is presented on the basis of research and practice in recent ten years. The concept of "Down-Three-Zones" of floor water-eruption is described clearly. Finally, the principle and method of applying the theory of "Down-Three-Zones" are discussed. 1. An Introduction of Damage: China is rich in coal resources. The conditions of coal field geology are quite complicated. There is an area of two million square kilometre of Karst limestone in nine million six thousand square kilometres of territory. The resources of underground water are rather plentiful, but this menaces coal exploration. Sixty percent of coal mines in China are damaged by limestone confined-water in different scales, particularly coal mines located the Carboniferous-Permian coal fields of the Ordovician limestone distribution area of Northern China. Because there is the 500-800 metre thickness of Ordovocian limestone and the well developed Karst containsl plentiful water, Karst water can have more than 100 Kg/cm pressure. Limestone aquifers have only distances of several metres or several decade metres to coal seam, Karst water can enter the aquituge and even the coal seam, large eruptions often occur. For example, a mine of 3 million ton production capacity had an eruption of 2053 cubic metres per minute. This is the largest confined-water eruption which has brought enormous economic damage and threatened the safe production of coal mining. Therefore, the prevention and cure of Ordovician water has immense advantage to the development of the coal industry. There are several decades experience in the prevention of Ordovician water-eruption in China. The prevention of Ordovician water-eruption has two kinds of plans. 1) dewatering and depression: it refers to dewatering of aquifers as roofs of seam and depression of aquifers as floors of seam in mining areas. 2) comprehensive prevention under the condition of having water pressure in aquiters. Due to enormous water, electricity, equipment and lots of other factors, the former isn't used extensively, the latter tits to China's actual conditions and is used widely. Instability in spoil piles and in high wall slopes in natural overburden has been ex- perienced at Utah's Goonyella Mine since com- mencement of operations in early 1971. Annual production targets, although gener- ally met since the commencement of operations, required modifications to stripping and mining techniques and the rapid re-scheduling of oper- ations around unstable areas, but culminated in the earlier than planned acquisition of addit- ional stripping equipment. An understanding of failure mechanisms was developed within a collaborative study of pitwall instability between Utah Development Company and the C.S.I.R.O. Division of Applied Geomechanics. Results showed that pitwall instability occurs as structurally controlled deformations along planar failure surfaces; spoil pile failures are precipitated by moist- ure ingress and subsequent loss of strength of base layer materials; highwall failures de- velop over areally extensive bedding planes in conjunction with inclined and subvertical fractures caused by overburden blast damage.

Up to 80 per cent of the copper in a bornite ore can be quickly leached with percolating acidified ferric sulphate solutions. The upper limit for rapid copper extraction is set by the conversion of bornite to slowly dissolving chalcopyrite. The dissolution occurs in two stages: a rapid linear initial stage, followed by a slow, non-linear stage. The initial step is largely controlled by the rate of oxidant supply (Fe2(S04)3); temperature is not an important variable during the initial reaction. The second leaching stage is controlled by chemical and physical factors and is temperature dependent. The initial copper leaching rates are insensitive to both the concentration of the ferrous sulphate reaction product, and the sulphuric acid concentration.

The Taharoa ironsand deposit is 1600 hectares in size and is located 144 km south of Auckland on the west coast of the North Island, New Zealand. The sand predominantly contains heavy mineral-rich sediments, quartz, and feldspars. One hundred and two samples were collected from all areas of the mine, from the stockpile, black sand areas, tailings and Te Akeake Sands. Samples were collected with the primary focus on the next predicted mining path and secondly on gaining a good geographic spread throughout the mine. All samples were analysed for the dry, wet and particle density, the porosity and the magnetic, non-magnetic and clay minerals content.Stockpile samples had an average magnetic mineral content of 92 percent. Dry, wet and particle density values for stockpile material exceed all values from untreated ironsand samples with average values of 2.8g/cm3, 2.9g/cm3 and 4.4g/cm3 respectively. Black sand samples from the mine came from various areas such as the central area, the forest, the southern area and the benches. All black sand samples are remarkably consistent, with dry and wet density values between 2.3 û 2.5g/cm3 and particle densities that range from 3.7 to 3.9g/cm3. The porosity values are comparable to all other areas of the mine (average 37 per cent). The tailings and Te Akeake Sands have different physical properties due to their different mineral contents. The dry, wet and particle densities are all lower than that of all other areas with averages of 2.1g/cm3, 2.2g/cm3 and 3.3g/cm3 respectively. These lower values reflect the lower magnetic mineral content (average 18 per cent) and the associated increase in non-magnetic mineral content (average 78 per cent).

Bacteriophage (phage) are viruses that infect bacteria. Protein coatings on the surface of the phage particle can be genetically engineered so that they bind to inorganic molecules. The filamentous phage particle is about 930 nanometres in length and 6.5 nanometres in diameter and consists of a single-stranded molecule of DNA surrounded by five different coat protein molecules denoted P3, P6, P7, P8 and P9. Peptide sequences in the P3 coat protein that bind to sphalerite and chalcopyrite have been identified. The binding is selective; phage with specific P3 peptide sequences bind to minerals but do not bind to silica or to pyrite. Two peptide sequences in the P8 coat protein that bind selectively to chalcopyrite and pyrite have also been identified. The amino acids in the P8 binding sequences are hydrophilic and thus a coating of a mineral particle with phage having these peptide sequences may render the particle hydrophilic. Experiments are underway to confirm this. Phage are relatively easy to grow and are not living organisms requiring an energy source to maintain life. Phage can also tolerate a range of pH and temperatures. Given this, and the ability to indentify phage coat proteins that bind selectively to a variety of inorganic molecules, phage have the potential to be useful tools in mineral processing and waste treatment.

In the processing of refractory gold-bearing sulfides, pressure oxidation (POX) is commonly known for its role in liberating the gold in solid-solution via oxidation of the sulfide occlusions. For deposits that contain arsenic-bearing species including arsenian pyrite and arsenopyrite, POX serves an important secondary function in the fixation of arsenic, via the hydrothermal precipitation of stable arsenates such as scorodite and basic ferric arsenate sulfate (BFAS). There have been various investigations on the impact of different POX conditions on arsenic precipitation and the stability of the formed phases, but less is known about the changes to the arsenic carrier that occur in the curing stage, which follows directly from POX. The primary function of curing is to coax the dissolution of sulfates to reduce lime consumptions during cyanide leaching, and the slurry is kept at elevated temperatures near 100°C to achieve this. While the arsenic-bearing phases are expected to undergo transformation during this stage, there has thus far been limited experimental evidence. In this study, the effect of curing time and curing temperature on the arsenic-bearing species and arsenic deportment in the POX product has been investigated. Leaching of the curing products was also conducted to examine the stability of the arsenic species in the solid precipitates.

The McKinnons gold deposit is hosted by weakly deformed, Palaeozoic metasedimentary rocks of the Cobar Basin in the western Lachlan Fold Belt of New South Wales. Mineralisation comprised a largely supergene resource of 1.7 Mt of ore, grading 1.7 g/t gold, established over structurally controlled, primary gold and base-metal mineralisation in underlying, carbonate-rich meta-siltstones and sandstones of Early Devonian age. Mineralisation was associated with at least two major fluid cycles. The first cycle was characterised by four stages of vein formation. Early quartz and carbonate veins (Stages 1 and 2) and quartz-pyrite breccias (Stage-3) occurred prior to, or early in, the main regional deformation (D1). Continued D1 deformation produced syntectonic, carbonate-base-metal veins (Stage-4), primarily in dilatant zones adjacent to fold axes. The second cycle of mineralisation was an epithermal-style event, driven by a shallow, syn-D2 or post-tectonic intrusion, defined by aeromagnetic imaging. The intrusion may have exploited a jog, controlled along the basin margin by basement faults. Mineralisation produced gold-pyrite veins, associated with pervasive quartz-sericite-adularia and chlorite-sericite alteration. Highest primary gold grades are confined to quartz-adularia boiling zones. The McKinnons deposit offers an important example of mineralisation formed by multiple processes. Early-stage, base-metal mineralisation at McKinnons reflects structurally focussed, basin dewatering and metamorphic processes, typical of other deposits in the Cobar Basin. By contrast, late-stage gold mineralisation at McKinnons is magmatically driven and of epithermal style. Recognition of overprinting distinctive geological events is critical, not only in modelling ore formation, but in developing a versatile exploration concept.

One of the main challenges for current and future mining and processing methodologies is to understand the characteristics of the rock so that its behaviour in one or more parts of the mining process chain can be predicted. Features such as rock texture, mineralogy or border complexity between minerals control how the rock behaves from the processing plant through to the final products, including tailings disposal and thus can affect the feasibility of the project from technical, economic and environmental impact perspectives. Understanding how the mineralogy and texture of rock affect its behaviour in mineral processing is a demanding area of research and is essential knowledge in predicting the processability of complex ores in different stages. This study aims to demonstrate that different types of relevant information can be extracted from a range of currently available analytical methods. However, this information can be used in a complementary way for building a more comprehensive rock characterisation at the microscale and better-predicting ore processing characteristics from the intact rock through to tailing disposal. Carbothermic reduction of iron oxide is an attractive mechanism to produce metallic iron, since it can directly utilise coal as both reductant and fuel. To improve energy efficiency and productivity of the metallic iron production based on this mechanism, the combustion bed process packed with the composites of iron oxide and coal powder was experimentally evaluated. The process has a potential of high heat efficiency since it can automatically utilise the heat of reduced materials as that necessary to the subsequent reduction of the composite by a large heat exchange rate between the packed particles and gas flowed across the bed. However, it is hard to perfectly prevent the reoxidation of formed metallic iron in the bed. The present paper reports the effect of coal content, pellet size and so forth on the reduction/metallisation degrees of the samples after the reduction experiment. It also discusses the effect of pretreatment of the composite pellets using CaO bearing materials to suppress the reoxidation of formed metallic iron in the bed.

The 1100 orebody is a massive copper sul- phide orebody which contains a major portion of the Isa Mine copper ore reserves of 130 million tonnes at 3.2%. The underground mining of a deposit as large as the 1100 orebody is technologically a complex operation as extraction strategies have to be evaluated with reference to 1. rock mechanics considerations, 2. safety and environmental standards, 3. production and operating require- ments and 4. metal recovery and costs. The mining philosophy in the orebody has undergone considerable change over the past five years as a result of experience and improved technology. At this stage, primary stopes are extracted by high rise sublevel open stoping and pillars by slot and mass blast drawn either under rock fill or as an open Stope.

Constructed between 1885 and 1891, the New Croton Aqueduct (NCA) in New York State is a 50 km long (31 mi) brick-lined tunnel conveying water from the Croton watershed north of New York City to distribution systems in the Bronx and Manhattan. Except at two siphons, the aqueduct operates as an open channel conduit from the New Croton Reservoir downstream 39 km (25 mi) to the Jerome Park Reservoir in the Bronx. The remaining 11 km (6 mi) is operated as a pressurised conduit and includes a major siphon, of circular cross-section, under the Harlem River. The aqueduct has a flow capacity of approximately 1100 million litres of water per day (13 m3/s or 290 million gallons per day) and supplies on average ten per cent of New York CityÆs drinking water. The CityÆs water supply system is operated and maintained by the New York City Department of Environmental Protection (NYCDEP). This paper covers the work done during the three main stages of the program to rehabilitate the entire NCA: inspections, design and rehabilitation. It also includes some of the more interesting structural findings of the original construction as well as documents and drawings from the NYCDEP archives. The rehabilitation program has been underway in phases since 1993, with inspections scheduled to minimise outages, allowing this strategic aqueduct to remain in service during critical seasonal periods. The open channel flow section of the aqueduct in the north is horseshoe shaped, with inside dimensions of 4.1 m (13.6 ft) in width and 4 m (13.5 ft) in height. The pressurised section is 3.7 m (12.3 ft) in diameter, and includes a 90 m (330 ft) deep siphon under the Harlem River. A total of 163 million bricks were used in the construction, enough to build a 48 km (30 mi) wall around Manhattan Island, 3 m (10 ft) thick and 15 m (50 ft) high.Between 1993 and 1997 a series of in-tunnel investigations were performed in the open channel portion of the NCA, consisting of field inspection, non-destructive geophysical testing, and coring of the brick liner. Between November 2004 and September 2005, a major inspection program was conducted to assess the condition of the 11 km (6 mi) long pressurised section and all shafts, headhouses and blow-off structures along the entire 50 km (31 mi) alignment. Inspection methods included using an underwater remote operated vehicle (ROV) equipped with sonar and cameras to inspect the deep siphon, fibre optics examinations of probe holes drilled through and beyond the brick lining, and core holes and geophysical inspections to assess properties of the liner and behind-the-liner materials. A water pressure and test grouting program was conducted to assess methods and expected grout takes for the rehabilitation work in the pressurised sections. The rehabilitation program is anticipated to be completed in 2010.
