As the global mining industry continues to grapple with ‘black swans’ (high consequence / low frequency events) for health, safety and environmental losses, the focus is shifting from risk assessment of these events to risk management using selected purposeful controls that reduce the risk.Fundamental to critical control management (CCM) is a soundly based rationale for selecting the right controls for each and every risk. The approach should be based on a thorough understanding of the dynamics of the event: What causes it? How frequently will it occur? How likely is the consequence to be realised? This is typically done through a bowtie and methodical numerical risk analysis. Core to this step, then, is the structure and content of the bowtie; specifically, the analytic architecture that is used to construct the bowtie. Different architectures yield different constructions, and bowties are no different.Although bowties for operational health and safety (OHS) hazards appear to have a common architecture, they are not generic. It is important to stress that they contain site specific information, are prepared by site based operational teams, and vary from site to site to reflect each site’s operational conditions. They are often a product of the knowledge and understanding of the operational teams involved, may be prone to human error, and potentially miss latent hazards that may have been embedded early in the planning, design or construction phase of a mine or facility, or within the mining equipment. Overcoming these deficiencies through constant re-evaluation of the risks, and challenging the content on the bowtie will help to minimise the chance that something will be missed.This paper will cover key themes in bowtie architecture, introduce the concept of barrier models for high consequence event (HCE) bowties and present a relevant case study to illustrate the issues that determine quality in bowtie construction and critical control (barrier) selection and management.CITATION:Mills, R, Andrew, M and Reynolds, H, 2016. Mining plants and infrastructure, and the decisions made by their designers, can have large impacts on their environmental and social context, commonly referred to as the site or plant footprint. They can also be the source of constructive contributions to sustainable development when designers are enabled via the information, systems and tools to incorporate sustainability in engineering processes. A facility’s footprint and the project’s contribution to sustainability – economic, environmental and social aspects – are now key considerations in regulatory approval processes and are the fundamental drivers of support, or opposition, from the project’s stakeholders.Historically, plant and infrastructure designers have not been well integrated with the environmental and social professionals who are engaged to pursue regulatory approvals. Despite this, many objectives central to technical and process engineering have in fact contributed positively to a facility’s footprint and indeed its contribution to sustainable development. Examples of these objectives include increasing process and energy efficiency, and minimising the quantities of waste requiring disposal. With the framework of sustainability, mining plant and infrastructure designers are indeed at the forefront of incorporating sustainability into resource industry projects and are central to the process of obtaining regulatory approvals and ongoing stakeholder support. The process of incorporation starts with a shared understanding of sustainability and sustainable development, and continues with project processes that facilitate information and knowledge exchange throughout the project life cycle.CITATION:Murphy, A, 2014. A major Ventilation Upgrade Project commenced at the Fosterville Gold Mine (FGM) in 2017 to service the ongoing ventilation requirements of the underground mining operation. The project consists of three large diameter ventilation raises totalling 1100 metres, and new axial primary ventilation fans to upgrade the mine ventilation system. With a mining depth of 1200 metres and a thermal gradient of approximately 2.7 °C per 100 metres of depth, the completion of the Ventilation Upgrade Project is key to the ongoing success at FGM. The Ventilation Upgrade Project will allow continued mining, including high-grade gold deposits, lower in the mine where heat is increasing. Figure 1 below illustrates the underground workings at FGM, and provides a general view of the ventilation upgrade works to establish the new exhaust system from the lower workings to surface. Whilst raisebore pilot drilling the H4760 Return Air Rise (RAR) from surface to 395 metres, the raisebore drill string became stuck in the raisebore pilot hole at approximately 300 metres down hole, presenting a significant technical problem and delays to the project. This Case Study provides an overview of the recovery process undertaken at FGM including the recovery of the stuck drill string. The value of the stuck in-hole equipment, the investment in the shaft site preparation, the collar stabilisation, the underground development to the base of the ventilation raise, the surface footprint prepared for the Primary Fans construction and the limited available space on the site were all motivations to recover the raisebore drill sting and ultimately recover the ventilation raise location. More importantly, the long lead-time required should a new shaft location be the final solution, and the delay this would incur to the ventilation upgrade was unacceptable. Recovery of the drill string was not possible using the capability of the Contractor’s Raise Drill. An alternative method for freeing the drill string was required. A significant amount of engagement with industry was undertaken to seek out potential solutions and draw from the experience of others. All methodologies available were considered and assessed. Ultimately, after several attempts at freeing the raisebore drill string using various methodologies, Adams Drillers and Clear Cut Interventions were engaged to provide a turnkey recovery solution.

Recently Mount Isa Mines undertook a study to review the lead-zinc orebodies to extent of resource and reserves and determine the new life-of-mine schedule, as the Mount Isa and Hilton orebodies had not be reviewed for many years. Also, the recovered value of the Mount Isa, Hilton and George Fisher orebodies had been increased by major performance improvements in the lead-zinc concentrator. The Mount Isa Lead-Zinc concentrator processes fine-grained lead-zinc ore from the Mount Isa, Hilton and George Fisher orebodies. These orebodies have varying levels of pyrite and large variation in the grain size and texture of the galena, sphalerite and pyrite. This variation has a large effect on the recovery of the lead and zinc in the concentrator. Some ores achieve 90 per cent recovery at target grades, yet other ores are unable to achieve æsaleableÆ concentrate grade, with no metal being recovered to concentrate and therefore effectively losing the contained metal to the final tailings. Models were created to predict the metallurgical performance of these orebodies, to better reflect the variation that occurs. These improved metallurgical models and economic data allowed the determination of the Net Smelter Return (NSR) for each block in the geological block model. This NSR is the return to MIM of the metals contained in concentrate. The NSR was used as the cut-off grade criteria when determining the value of the ore. This formed the basis of the lead-zinc life-of-mine schedule and the optimisation of the lead-zinc business.

Nickel-iron sulphide mineralization at Nepean, Western Australia occurs within a small Archaean greenstone outlier in granite south of the main Coolgardie greenstone belt. Individual sulphide ore bodies are situated near the foot- wall contact of simply-differentiated en echelon ultramafic sills that have intruded a sequence of sulphide-bearing metasediments and amphibo- lites. The paper describes a detailed section through the No. 2 ultramafic sill and its associated sulphide ore zone, which gave Metals Exploration N.L. the first significant nickel discovery in the Nepean area in 1968. The sill is variable in thickness up to 30 m along a strike length of 600 m, and is truncated to the north by pegmatite intrusion and pinches out to the south. The body has border zones of pyroxenetic composition, now represented by felted aggregates of chlorite and amphibole, and a central peridotitic core in which olivine and orthopyroxene are concentrated towards the base. Textures within the peridotite are dominated by large lath-shaped olivine grains and their serpentine pseudomorphs, which delineate triangular areas composed of talc, chlorite, and amphibole. Orthopyroxene occurs in large ovoid patches and is often replaced by Talc.

The world-class iron ore mines at Mt Whaleback, Torn Price and Paraburdoo located in the Hamersley Province of Western Australia must mine the Mt McRae Shale to obtain high grade ore from the Dales Gorge Member of the Brockman Iron Formation. The Mt McRae Shale contains a black pyritic shale lithology that reacts with ammonium nitrate-based explosives. This paper provides an overview of the problems associated with mining this reactive rock type as well as the development of a comprehensive procedure to safely and efficiently blast, excavate and dispose of this material. The strategy used to develop the procedure involved raising the awareness of relevant site personnel to the risks associated in mining the Black Pyritic Mt McRae Shale, obtaining the commitment of these personnel that a detailed procedure was required and that any procedure developed must be followed, otherwise the potential risks such as premature detonation were high. It was also necessary that an on-going training program be implemented so that all relevant new employees would be educated as to the reasons for adhering to the Black Pyritic Shale Procedure.

Large scale cut and fill mining in narrow silver-lead-zinc orebodies began at Mount Isa in 1964. Initially each orebody was one stope and access was via raises from the level above. Upholing was the principal mining method, although flatbacking was employed where ground conditions were more difficult. Developments since have included access via footwall inclines and connection of all stopes with major crosscuts. Flatbacking is the principal method. The most recent cut and fill operation is now fully mechanised using drilling jumbos, rockbolting jumbos and load-haul-dump units. Improved ground support and drilling and blasting practices have enhanced safety, recovery and productivity in all cut and fill operations.

The challenge to the zinc industry is shifting from maximum metal recoveries to the environmentally safe processing of the ore concentrates. The main source of solid residues from hydrometallurgical zinc processing is the iron content of the concentrates. Unfortunately iron precipitates from leach liquors contain various compounds which cause them to be considered as environmentally unsafe, unless they have been cleaned in subsequent process steps. This calls for separating the iron from hydrometallurgical processing as early as possible. For this purpose a process has been designed to eliminate the iron directly after neutral leaching the calcine. This is being done by direct smelting the neutral leach residue with a cyclone/top blowing unit. The zinc is recovered from the neutral leach residue as a metal oxide to be recycled to the zinc process. The iron is converted to a slag, which is resistant to atmospheric attack. Key data of the integrated hydro-/pyrometallurgical process are presented as derived from pilot plant tests by cyclone smelting and top blowing the slag.

The concentrator at Bougainville Copper Limited has an annual throughput of 48.2 million tonnes/year producing approximately 175,000 tonnes copper, 16.1 tonnes gold, and 50 tonnes silver. Conventional three stage crushing and screening are used to reduce run of mine ore to an average product d80 size of 6.7mm. The primary objective of the crush- ing plant is to provide a continuous supply of ore to the 12 primary grind- ing mills via an intermediate 300,000 tonnes capacity stockpile. Since ball mill performance improves with decreasing mill feed size, the crushing plant continually strives to produce the finest possible product for mill- ing. This has been achieved by increasing plant availability and total installed power, and improving crusher manganese liner life, screen cloth life, and plant operating practices. A recent innovation was the installa- tion of a 35 million tonnes/year preconcentration screening plant in Janu- ary 1987 to recover high grade fines from low grade run of mine ore types in which the mineralisation occurs preferentially along fracture planes in the parent rock.

Zinc-bearing materials containing chlorides and fluorides of alkali metals, such as electric arc furnace dusts, Waelz oxides, galvanising ashes, brass foundry and converter fumes are marginally attractive, both technically and economically, for processing by the Imperial Smelting Process or in a conventional zinc sulphate electrolytic plant. The EZINEX« Process allows the recovery of high purity zinc in cathode form from these materials, together with regeneration of the leach liquor. The process is based on a new electrochemical system which depolarises the anode reaction with reduction of the cell voltage, compared with the traditional sulphate electrolysis method. The electrolyte, based on a chloroammino complex, permits the recovery of other valuable metals present in the zinc bearing materials such as lead, copper and silver. These are recovered as a metallic cement, consequently improving the economics of the process.

"It is well known that from time-to-time a vehicle will interact with a bund or windrow and often the barrier is insufficient to safely arrest that vehicle. The result of such an incident is often catastrophic. Regulations around the requirement for edge protection near pit edges vary between jurisdictions. Generally in Australia regulations tend to be around placing the responsibility on the mine operator to provide ‘adequate’ or ‘appropriate’ edge protection. Guidelines often place a minimum height of bund at between half and two-thirds the diameter of the largest tyre on-site.Safescape has identified the opportunity to improve safety in open pit edge protection and is currently developing the Edge Protector (EP) product. The EP is designed to create a vertical front face to a conventional broken rock safety bund. The fundamental premise of the product is that by creating a vertical front face to the bund, trucks cannot easily drive up over the bund and instead have to push through the rock to get to the other side.In addition to providing a safer system for arresting vehicles, the vertical front face also means that extra space is made available in the haul road. This can either be used to facilitate larger trucks or when designing a new mine, with an increase in overall pit slope.This paper outlines the research and development of a new edge protection product. The behaviour of tailings under cyclic loading has an impact on the stability and deformation of tailings dams and a good understanding of the cyclic behaviour of the tailings under earthquakes is important for tailings engineers and tailings operators for the design and safe operation of tailings dams. This paper presents an investigation into the cyclic behaviour of tailings using both laboratory cyclic direct simple shear (DSS) testing and numerical modelling. The laboratory testing involved cyclic DSS tests on gold tailings and zinc-copper tailings under different loadings and loading frequencies. The gold tailings specimen tested at a higher loading frequency and a higher cyclic stress ratio experienced spontaneous liquefaction during cyclic shearing and the zinc-copper tailings specimen tested at a lower loading frequency and a similar cyclic stress ratio exhibited ductile behaviour and liquefied due to cyclic mobility. The liquefaction behaviour of the tailings was modelled using an advanced constitutive model (UBCSAND), which was developed specifically to model soil liquefaction and strain softening under cyclic loading. The constitutive soil model presents the nonlinear soil behaviour under dynamic loading using a hyperbolic stress-strain relation. Dynamic analyses were carried out using a finite difference program FLAC. The cyclic stress-induced excess pore water pressures measured in the laboratory were modelled using numerical simulations of the laboratory tests under different loading conditions. The results of the non-linear dynamic analyses demonstrated the capability of numerical modelling of cyclic shearing induced excess pore water pressure and the onset of the spontaneous liquefaction. The laboratory investigation and numerical modelling technique provided the key properties and parameters for the dynamic analyses to assess the stability and deformation of tailings dams under design earthquake events. More importantly, the laboratory tests and numerical modelling provided some insights into liquefaction behaviour of the tailings during earthquakes. CITATION:Li, A, Laxman, M and Jinto, H, 2018. A two-dimensional finite element modelling technique is applied to predict the subsidence movements on a sloping ground surface above a completely mined panel. The modelling is carried out for the surface both directly and using a combination of an equivalent horizontal surface and a rays projection method and the result compared. The rays projection method provides the subsidence components on a sloping surface from the corresponding ones on an assumed equivalent horizontal surface through the point of mean elevation of the relevant part of the sloping surface. This method is used next to compare the subsidences, horizontal displacements and horizontal strains from the mining of a given panel at the surface for different inclinations. The results show that the magnitudes of the subsidence components are greater on the dip side of the slope from the point of the mean elevation, than the corresponding ones on the rise side and the asymmetry increases with the inclination of the surface. The findings are also compared with those of other research Workers.

Present capacity for molten iron production at Port Kembla is an average of 13 000 t/day from five blast furnaces. The most recently installed blast furnace, No.5, was commissioned in March 1972 with a design capacity of 4 500 t/day. For the first reline currently scheduled for 1977, plans are being finalized.for significant modifications and uprating of capacity. Achievement of the initial design capacity from No.5 blast furnace on a sustained basis was not realised until the third year of operation. Although the furnace is only 42% larger than No.4 blast furnace on an inner volume basis, the more stringent raw materials, operation and maintenance requirements of the larger furnace and more complex equipment have been found to be much more critical than anticipated in the planning stages. These problems have also been experienced by many other companies who have recently installed large new blast furnaces. Some of the principal problem areas are described together with measures taken to over- come the difficulties. In commercial mineral processing operations it has been found that the conditions in the drier have a considerable effect on the subsequent electrostatic separation of nttile and zircon. Laboratory scale studies simulating these heat treatment processes have shown that the electrical conductivity of nitile varies markedly with treatment temperature, time and gas composition. The zircon on the other hand is unaffected. By increasing the electrical conductivity of the rutile, significant increases in separation efficiency can be achieved. Electron microscope investigations have shown the increase in electrical conductivity to be associated with the production of planar defects in the crystal lattice.

Potential hlghwaJl mining reserves in Australia are large, but geotechnical and operating conditions are quite different from those where highwall mining has evolved in the USA. In order to achieve safe and effective resource recovery, equipment and procedures should be developed to penetrate 250 m in the Australian mining environment. This paper identifies key auributes to be incorporated in such machinery and methodology. These suggestions are intended to promote discussion on innovations to satisfy Australian requirements for highly productive, safe and environmentally acceptable techniques.

We are all no doubt aware that with today's high costs it is more important than ever to get the maximum possible return on capital invested in the mining industry. Screening is an important and often costly process in the size classification of industrial minerals and for this reason alone it is essential for both managers and operators to fully understand what type of screening equipment is best suited for a particular application and why. The advent of elastic materials such as rubber and polyurethane have added a new dimension to the process of screening. It is important to understand why the physical properties of these materials can be of benefit in the screening process, and recognise which type of screening applications best take advantage of these orooerties.

Polished discs of a complex sulphide ore were leached with three lixiviants; a mixed culture of Thiobacillus ferrooxidans and Thiobacillus thiooxidans, Thiobacillus ferrooxidanalone, and ferric sulphate. At either one or two week intervals the discs were examined by scanning electron microscopy, and the amount of soluble metals released from the discs were determined. The mixed culture solubilized more Cu, Zn and Pb than did either ferric sulphate or T. ferrooxidans alone. However, no differ-ences were observed between the three treat-ments by scanning electron microscopy during the first 44 weeks. The minerals were attacked in the following order: pyrrhotite, cubanite, sphalerite, pyrite and chalcopyrite.
