Rock reinforcement systems are used in open pit and underground mines to stabilize the rock mass in-situ and provide safe working conditions. Significant progress has been made to date in the design and implementation of rock reinforcement systems to accommodate bad rock conditions and larger excavations. However, the rationale for the design of rock reinforcement systems remains essentially the same and can be categorized into three groups, namely 1. to create a beam of competent rock in stratified formations, 2. to resist sliding motion in jointed rock, 3. to passively support the superincumbent loads in highly stressed ground. Case histories of the rock reinforcement sys¬tems that illustrate these design approaches will be presented in this paper.

The use of phosphate depressants in the cationic flotation of silica from francolite in the Florida phosphate industry is rarely practiced. The occasional use of gelatinized starch derivatives and pH adjustments to assure a slightly alkaline flotation feed slurry pH have been used in some processing plants to improve selectivity. This paper presents a preliminary laboratory comparison of several potential phosphate depressants for use with the flotation of siliceous phosphates. It was found that starch is ideal for floating fine (-35 mesh) silica from phosphate, while sodium tripolyphosphate may be the best for coarser (+35 mesh) feeds.

Studies were carried out to determine whether filtration rate changes observed in an iron ore concentrator were due to variations in particle shape. It was confirmed that ores mined from different locations differed in filtration rates by as much as 60%. However, when image analysis was used to quantify the shapes of the individual particles in the filtration feed, it was found that the particle shapes showed almost no variation, and the shapes did not correlate with the filtration rate. It was also determined that the filtration rate variations were not caused by changes in particle size distribution. These results suggest that the differences in filtration rate are due to factors other than size and shape, such as surface chemistry.

Three drill-blast cycles of 3.1 m advance achieved in one working shift of 7.5 hours at the Holsfjorden Tunnel project in Norway. The tunnel, 5235 m long, with a cross section of 8.6 m2, was completed several months ahead of schedule. This remarkable performance was achieved by a crew of 16 men, using machines specially designed for small tunnel excavation. The integration of drilling and charging into one unit, mucking and transport in another, reduced non-productive time considerably. The average cycle time of 140 min made it possible to complete three rounds in one 7.5 hours shift.

The mineralogy of the Biwabik Iron Formation changes dramatically from west to east as the formation nears the basal contact of the Duluth Complex. This reflects a contact metamorphism that took place with the emplacement of the igneous Duluth Complex at tem¬peratures as high as 1200 °C. However, the mineralogy of the Biwabik Iron Formation also varies vertically through the stratigraphy of the unit. This variability in both the vertical and horizontal dimensions makes it difficult to predict exact horizons where specific minerals will occur. The iron-formation has been subdivided into four broad stratigraphic units (lower cherty, lower slaty, upper cherty, and upper slaty) and into four lateral mineralogical zones (1-4). Zone 1, the westernmost zone, is characterized by quartz, magnetite, hematite, carbonates, talc, chamosite, greenalite, minnesotaite, and stilpnomelane. The silicate mineralogy in Zone 2 of the Biwabik Iron Forma¬tion changes very little. However, the minerals begin to change dramatically in Zone 3. Most significantly, Zone 3 is characterized by the appearance of grunerite in both a tabular form and a fibrous form. In Zone 4, the original silicate minerals have completely reacted, and a new suite of minerals occupies the iron-formation. These include grunerite, hornblende, hedenbergite, ferrohypersthene (ferrosilite), and fayalite.

Decisions made during a response to a mine fire or an explosion greatly affect the final outcome. The safety of miners should be the primary concern with accounting for all of the miners the first issue for resolution. If miners have not safely evacuated from the mine, the last known location of the miners at the time of the event and anticipated travel routes are primary exploration areas. After the mine is evacuated, there are many very important decisions that must be made quickly and often under great stress. It is critical that the mine operator establish an orderly method to direct the mine emergency response. Designation of a Command Center, staffed with personnel charged with developing plans, is a common method of establishing control. Scheduling of key mine managers will be critical, since decisions must be made 24 hours a day and people working for extended periods with little or no sleep do not always make clear or cognizant decisions. Internal communications, passing information on to subsequent shifts, and external communications with families and media must be accurate and timely. Assembling resources for responding to the mine emergency is critical for an effective response, and pre-planning can pave the way to a successful recovery. Sampling mine gasses and accurately determining the composition of gas mixtures is a common tool used to develop response plans in all fires and explosions. It is essential that preparations for emergency situations are made well in advance of a disaster. Effective communications are essential for mine management to successfully respond to a mine emergency. This paper discusses some considerations which need to be addressed in directing a response to a mine fire or explosion.

Mineral exploration activity in the US suffered through another depressed year in 1984. And continued strength in precious metals (and possibly industrial minerals) exploration failed to make up for declines in other minerals. 1984 offered little solace for the explorationist. But some new data on mineral exploration provided better statistics for analysts of US mineral exploration trends. The Society of Economic Geologists (SEG) published data from its survey of 1980,1981, and 1982 exploration by US and Canadian companies - 96 companies responded (Barber and Muessig, 1984). In addition, a Mining Engineering (April 1984 p. 345) article by Emerson and Ivosevic presented their data on annual exploration expenditures in the US for gold and for all minerals from 1961 through 1983. A review of 1984 domestic exploration and development activities reported in several industry journals indicates that almost 90% of those projects were directed at gold and silver deposits. Statistics on 1984 exploration expenditures are not available. But some industry observers believe that precious metals accounted for an even larger share of industry's exploration budget in 1984 than in 1983. The Owner Procurement Process or OPP is a proven, viable alternative contract delivery method that is potentially applicable to many tunnel projects. Using the OPP, the owner specifies and procures major tunnel construction items such as tunnel boring machines and tunnel linings and provides them to a tunnel excavation contractor in order to manage risk and accelerate project schedule. However, due to industry perceptions regarding potential owner risk, it is seldom considered or is rejected out of hand. In fact, past performance demonstrates that the process significantly minimizes owner, contractor and over-all project risk and mitigates the impact of any claims and differing site conditions. The paper outlines how the process should be structured to achieve schedule, cost, quality and risk management benefits. Potential and perceived disadvantages are discussed and how the process can be structured to mitigate or avoid them. The planned implementation of this process on the tunnels for the Silicon Valley Rapid Transit Project in San Jose, California is discussed.

Since Reserve Mining Co. began commercial operations at Silver Bay, MN, some 25 years ago, major improvements in taconite processing technology have become available. In addition, significant new government standards have been imposed on industrial operations. By implementing a $370-million modernization program, Reserve was able to accomplish two major company objectives: the quality of concentrates produced from the Peter Mitchell mine has improved, and the operation meets both present and foreseeable environmental standards. Process modifications at Reserve were designed to: •Reduce the pellet silica content from 9% to 5%; •Increase the pellet iron content from 62.5% to 65.8%; •Implement a tailings disposal system utilizing "best available technology" water quality controls; and •Reduce air emissions by installing "best available technology" air emission controls. By adding dry cobbing, fine screening, and flotation to the concentrating process, pellet silica content has been reduced; the process reclaims and reuses all process waters; all tailings are stored in a zero discharge tailings basin; and all air emission standards are being met. Initial Beneficiation at Silver Bay Reserve's Peter Mitchell mine at Babbitt, MN, is on the eastern end of the Biwabik iron formation. The open-pit mine is approximately 16 km long, 0.8 km wide, and presently has a maximum depth of about 50 m. Principal constituents of the ore body are magnetite, quartz, and iron-magnesium silicates. The company's large commercial plant began operating at Silver Bay, 75 rail km from the mine, in 1955. Facilities have historically had the capacity to process 31.5 Mt/a of taconite, containing an average 24% magnetic iron, into 11 Mt/a of iron ore pellets. Reserve's initial beneficiation process-which existed up to early 1979-consisted of two crushing stages at the Babbitt mine site, two additional crushing and screening stages at Silver Bay, grinding in a conventional rod-ball¬regrind mill circuit, size classification with cyclones, and concentration with magnetic and hydraulic separators. Concentrate produced by that circuit assayed 65% iron and 8.5% silica, with a screen analysis of 94% -44 µm, and a surface area of 9300 cm2/cm3. Pellets produced from this concentrate assayed 62.5% iron and 9% silica (dry basis). Construction began in 1977 to modify Reserve's operation. Process modifications were designed from pilot scale operations that had been operated and researched by Reserve for more than 10 years. There were two design restraints imposed on the process: economic constraints that Reserve continue to use much of the existing processes and structures in the new operation; and a substantial number of government-imposed permit constraints. In effect, the tailings process was designed first and then a concentrating process was developed that would "fit" the tailings disposal system. Operation of the modified process began in June 1980. Crushing Remains Unchanged No changes in process flow were made at either the Babbitt mine and crushing plant or at the Silver Bay fine crusher. Gyratory crushers reduce run-of-mine taconite to -90 mm-diam at the mine. Taconite is then transported to Silver Bay by intra-plant railroad. There are 10 fine crushing lines. Each consists of a double deck screen, followed by a heavy duty short head crusher, a second double deck screen, and second short head crusher. All screen undersize material bypasses the crushing lines, resulting in an open circuit crushing process. Fine crusher product is approximately 98% - 20 mm, 80% -12 mm, and 50% -10 mm in diameter. Each fine crusher line processes about 400 t/h of crude taconite. Cobbing Provides Aggregate for Tailings Dams The first major process change was the addition of dry cobbing, a dry magnetic separation step between the fine crusher and concentrator. Dry cobbing serves two major purposes in the new process. Its principal function is to provide large volumes of dry coarse aggregate for use in construction of tailings dams. This process rejects about 20% of the fine crusher product as tailings with a size analysis of 98% -20 mm and 50% - 10 mm. These tailings are free draining and have high frictional shear strengths-two key requirements for dam building aggregates. Fine crusher product is conveyed to a new dry cobbing building. Here crushed ore is discharged into one of two 30-minute capacity surge bins. This paper describes the original planning which actually resulted in the opening of a new aggregate plant located in central New York State. This plant, which is now named the Skaneateles Plant of The General Crushed Stone Company is unique in that the plant itself is a departure not only in design but in concept of operation from the normal aggregate plant the company has operated in the past. Whether or not the concepts which resulted in the Skaneateles Plant are economically feasible remain to be proven by the next few years of operation. By way of background data about our company which will help set the stage for this presentation, we currently operate seventeen stone quarries in the states of New York, Pennsylvania and Virginia. These quarries range in size from 200,000 tons per year to 1,500,000 tons per year with a total annual stone production of approximately 7,500,000 tons. We also produce about 1,250,000 tons of sand and gravel at six sand and gravel plants, 300,000 cubic yards of ready mix concrete, 1,500,000 tons of bituminous concrete, aglime and other minor construction materials. In many cases we are our own best customer consuming a large percentage of the total production of our own aggregates in our various ready mix and blacktop plants. Up to the concept of the Skaneateles Plant our operating facilities have been of the permanent type as opposed to the moveable or portable type plant. The steps involved in the case history of Skaneateles will begin with the basic economic requirements, market research, geologic surveys, land acquisition, zoning and financing and carry through to the final design and construction. We will attempt to show what some of the problems were, how we interpretted them and how we proceeded to solve them. The current results are apparent and can be analysed. The success of the total concepts of the Skaneateles Plant remains for future analysis.

Introduction Rock bolting is a general term that encompasses pinning, roof bolting, rib bolting, and cable bolting. Rock bolting contributes the highest nonproductive cost and is the most significant safety risk in mining today. Total installed bolt costs are now more than $20 per bolt. Rebolting or repair work doubles this. And bolting accidents account for about 30% of all fatal and nonfatal lost time accidents. It is only logical, then, for the mine operator to install the highest quality anchors possible. Every bolt on the market - tensioned, untensioned, full contact, or point anchor - has designed capabilities in axial, shear, and fluctuating ground conditions. Bolts will provide a designed support if material specifications and installation procedures fall within the manufacturer's recommendations. This also assumes that there are no unexpected geological or operational changes. Proper material specifications and installation procedures should guarantee the mine operator a quality anchor. To help assure this, the Mine Safety and Health Administration (MSHA), the US Bureau of Mines (USBM), and the AMC Manufacturers group developed stringent ASTM materials standards. These govern mechanical point anchor bolts and bolts using threaded and nonthreaded rebar. There are also minimum standards that suppliers must comply with in the manufacture of frictional bolts. However, most frictional bolt standards are based on the manufacturer's own specifications. It would be impossible to set uniform regulations on frictional bolts. Each bolt provides support in a slightly different manner and thereby requires different material needs. Quality Control In any case, material quality can generally be assured through tight order specifications and periodic quality control checks. The problem with installing quality anchors does not solely lie with the material. It includes all the events that occur from the time a bolt leaves a manufacturer to the time it is installed. The anchoring ability of any major type bolt can be affected by transportation, storage, mechanical performance of installation equipment, installation demands of the operator, proper drilling accessories, and local geology. Table 1 and Fig. 1 document the negative events that can affect each bolt and the degree to which it can be affected. Drilling a hole of oversized diameter, for example, is an operator controlled factor. It will adversely affect most bolts. An over-sized diameter reduces, or even eliminates, the anchorage capability of a mechanical shell. It will reduce the support capacity of a split set to nearly nothing. And it will cause a partial or incomplete grouting around the resin bolt. Frequently, due to bit wear, holes are drilled undersized in diameter and may not be properly cleaned out. In this case, it will be difficult for the operator to push the mechanical shell into the hole, thereby causing him to drill another hole. In cases where the bolt is obviously not providing anchorage, operators will probably drill another hole and place another bolt. Rock Bolt Regulations It is the situations where a bolt is improperly installed in the hole, but gives the appearance of a good bolt, that are frightening. In fact, some safety officials and engineers believe that only 50% to 60% of installed bolts are working. These bolts cause irregular ground load transfers in other bolts and overload them. This causes hazardous situations and overstressing. It is these bolts that fail prematurely 200 to 255 mm (8 to 10 in.) behind the face and possibly contribute to the high accident rate. The concrete lining for a long 8.05 km (5 mi) section of New York City Water Tunnel No. 3 was cast in place. The contractor, Grow-Perini-Skanska, placed over76,841 m3 (100,505 cy) of concrete, pouring 24 hours per day, 5 days per week. The concrete was dropped 204 m (670 ft) to the tunnel level, transported by rail to a concrete remixer and pump, and placed in 146.3m (480 ft) of full circle forms. Some of the many challenges include the sheer magnitude of the job, navigating through drifts, a raise and curves, dealing with ground water inflows, and placing the concrete within time limits.

METAFORA is a simulation model for determination and evaluation of strategies for operating loader/truck systems in mining. A specific version of METAFORA was used to evaluate the haulage system at Solider Mineral's Aitik mine in northern Sweden, one of the largest open pit mines in Europe with a production of about 11.5 Mtpy (about 34000 tpd) of copper are. A production level of 34065 tpd was estimated by simulation runs with METAFORA. Using METAFORA's dispatch rules (Fixed Dispatch, Maximize Trucks, and Maximize Loaders), the truck/shovel system in the Aitik mine was simulated for evaluating alternative dispatch control strategies. The Maximize Trucks rule gave about the same total production (ore + waste) as the Fixed Dispatch rule but a 9% higher ore production. The Maximize Loaders rule gave the lowest production. The marginal truck capacity decreases with increasing number of trucks serving the same loader due to increasing waiting times. In Aitik mine the low crusher capacity results in long dumping times. If the dumping time at the crusher could be halved, the ore production would increase by 14%. The paper indicates that the use of software like METAFORA can result in a substantial reduction of time, effort and cost in shovel/truck system design and evaluation.

Economically favored Cr and Cu leaching out process from spent power plant fly ash (PPFA) had been investigated. PPFA, bearing of C, S, V, Ni, Cu and Cr, has been widely used as an assistant fuel in cement industry. Recent environmental regulation in S. Korea prohibits using PPFA as a fuel. Many researchers have also studied on extraction V and Ni from PPFA. Due to low concentration of V and Ni, 2% and 1%, respectively, economical and environmental friendly process could not be found. In this study, Cu and Cr have been removed in an economical way and which satisfied with KS Extraction Test. The rest unburned carbon, V2O5 and NiO2 in residue were found to be applicable for indoor and outdoor colored bricks or deodorant panel. Various parameters such as pH, lixiviates and time on leaching behavior were investigated in order to leach Cr and Cu remaining C, V and Ni. Because of complex geological environments, a number of coal reserves occur in inclined or dipping coal seams in certain parts of the world. Longwall top coal caving using split-level a singleentry panel system (SSPS) has been widely used for extracting such coal seams with large thicknesses (greater than 6 m) in China because SSPS can reduce or mitigate problems of coal outbursts, gate road deformations, overturning of shields, downhill sliding of shearer and armored face conveyor (AFC), and so forth, during mining of inclined coal seams. Depth of yield is the range of yield coal from the rib of the pillar to the boundaries where yield ends. It plays a significant role in maintaining the gate pillar stability. This paper presents a study on the stability of an unconventional water-obstruction gate pillar (UWGP) formed by SSPS to prevent water inrush in mining of an inclined thick coal seam in a coal mine in China. As the shape of this pillar is non-cuboidal, nonparallelepiped, a conventional empirical or analytical method is not applicable for studying its stability.

Because of the lack of a universally accepted definition or standard for RCA, we are left to our own conventions in the marketplace as to what is most ?effective? or a ?Best Practice? for our own facilities or organization. This lack of uniformity in the application of RCA produces an equal lack of uniformity in the measureable outcomes of RCA on the bottom-line from site to site and industry to industry. What is the definition of success for an RCA? Is it regulatory compliance? Is it some quantifiable measure of bottom-line performance? Is compliance and bottom-line performance the same thing? Can we measure the ROI we receive by doing compliant RCA? These questions must be adequately addressed if any definition of RCA is to be effective on the bottom-line. Like our various Corporate Vision and Mission Statements, what would be our Vision and Mission for RCA? When pondering the answer to this question, we should consider: ? What Key Performance Indicators will the Success of RCA be Linked to? This provides a direct correlation to the corporate dashboard and therefore garners support for the RCA effort when implemented at the sharp end. Not until we know what KPI?s will effectively measure our maintenance and reliability objectives can we begin to establish which opportunities will afford the greatest returns1. ? What Bottom-Line Metrics will be Tracked to Measure the Effectiveness of Our RCA Effort? Will it be increased production, profitability, regulatory compliance, etc.? Will it be decreased injuries/fatalities, claims, customer complaints, risks, etc.? We should be specific about these metrics and their correlation to the metrics on our corporate dashboard. ? What Will Trigger an RCA Investigation? Normally companies will establish quantifiable thresholds to be surpassed before an RCA will be conducted. Examples may be an OSHA recordable injury, a failure event in which production losses exceed $250,000, an equipment failure in excess of $50,000 or a regulatory violation. This is the norm. However, we should realize this is using our RCA skills totally on a reactive basis. High Reliability Organizations (HRO) strive for proaction and they also use RCA proactively. Why can?t we use RCA to analyze why unacceptable risks from an FMEA are so high? It is perfectly applicable yet rarely implemented in this fashion.

Additional field evaluations of a modified continuous miner using a new spray system and modified scrubbers have been conducted by the authors. In this demonstration, a comprehensive package for dust control on a continuous miner which included previously developed modifications to the scrubber, scrubber intakes and the innovative chassis spray system was demonstrated. Side-by-side comparisons of the modified miner and regular miner indicated that the miner operator with the modified miner was exposed to 62% less respirable dust. Ram car operators with the modified miner had 38% less respirable dust exposure and the return air on the modified miner had 19% less respirable dust. The mining conditions during side-by-side testing were very comparable. These results support the findings of the modified spray system studies conducted earlier at different mines in Illinois on four occasions and the modified scrubber system field evaluation studies.

With all of the unknowns associated with tunnel construction, how does a contractor choose a roadheader? This paper presents a review of several models of roadheaders being used in today?s tunnel projects. The features of different models of roadheaders are presented and compared. Roadheader size and cutter power generally determine which roadheader is best for a particular job. We will also present a model for matching features of the roadheader to the anticipated ground conditions such that the most efficient machine is selected for the project.

The MTA in conjunction with LIRR, are in the process of expanding one of the largest commuter railroads in the country, with over 260,000 passengers a day. The LIRR, which provides 700 passenger trains every 24 hours, can?t continue on the present growth course as a joint tenant in Penn Station with Amtrak and NJT. For this reason, in conjunction with the fact that over 53% of LIRR?s current riders into Penn Station are ultimately bound for the East Side of Manhattan opening, have created the need for the expansion of the LIRR. The complete expansion will cost approximately 6.3 billion dollars and all of the contracts are to be completed by 2013.
