"Niobec mine, a producer of pyrochlore concentrates since 1976, became a producer of ferroniobium in 1994 and achieved accreditation to the ISO 9002 standard in 1995. The goal of ISO 9002 is to ensure that the products supplied to clients regularly meet their specifications, through explicit work standards and procedures throughout the production process, from deposit appraisal to ferroniobium production.A key element of ISO 9002 implementation at Niobec consists of the quality control procedures applied to the raw materials: the ore fed to the mill and the pyrochlore concentrate fed to the reduction plant. These controls extend from exploration, deposit appraisal, mine planning and reserves estimation, mining, mineral processing and, finally, ferroniobium production. The implementation of ISO 9002 has allowed more efficient control of the quality of the final product sold to clients. It requires interactive team work, the role of the geologist being to relate all information pertaining to raw material and product quality to the geological framework of the deposits."

"Due diligence reviews are an essential part of mineral property acquisitions. One of the primary reasons to complete an environmental review prior to exploration property acquisition is to assess the potential environmental liability. It is critical to identify the potential reclamation costs or existing environmental liability associated with the exploration property. A review of the existing condition of the property and its history can allow prioritization of exploration targets and ease future expenditures for historic damages. IntroductionIn Canada and some other jurisdictions, the purchaser of a property is assumed to know, and is (in the absence of deceit or fraud) financially responsible for, what is purchased. A plea of ignorance, at least from the perspective of environmental liability, is not an acceptable excuse to avoid responsibility (Nelson, 1994). Therefore, if an exploration company (for the purposes of this paper, any organization conducting exploration work) purchases a property that has environmental contamination, the new owner is liable for the cost to reclaim the site to an acceptable level. The cost to reclaim the site is the environmental liability. The new owner is responsible for existing environmental liability of the property, even if the exploration company’s personnel were not aware of the problem. In simplest terms, buyers beware. Therefore, in order to protect the exploration company’s interests, the exploration manager should determine the characteristics of the property of interest prior to purchase or other involvement, and exercise due diligence.The term “due diligence” is often used in the mining industry. To ensure clarity, in this paper’s context, the author has chosen to use the following meaning: “… due diligence is used to describe a detailed data review, generally in connection with the purchase or sale of a mineral property, the approval of a large capital expenditure, or the borrowing of a large sum of money” (Smith, 1997).Directors of the exploration company are held liable for the decisions made, unless they have relied in good faith on,“…a written report of the auditor, or a report of a lawyer, accountant, engineer, appraiser, or other person whose profession lends credibility to a statement made by any such person” (OBCA, 1994)."

"The aim of this study is to give a better understanding for the behaviour of the composite material RRS and the synergy effect of its components while an excavation face advances during tunneling. The lower part of the Q-system scale (i.e. Q-values of 0.4 or less for a tunnel span of 10 m) is commonly used for tunnel applications in softer rock formations. This is implied in the updated Q-chart, where the dimensioning of rock support by using Reinforced Ribs of Shotcrete (RRS) comprises the use of fiber reinforced shotcrete, S(fr), rock bolts and shotcreted beams reinforced with steel rebars. The newest part of the lower part of the Q-chart gives more information on required support compared to older Q-system versions. The RRS can be described as a three part composite material with the 3rd component being the reinforced concrete beam (with no fibers) and with steel rebars that are placed in groups and sprayed with pneumatically applied shotcrete in layers. The RRS usually have a total thickness varying between 0.20 and 0.50 m and a width between 0.25 and 0.60 m respectively. They are placed at different distances along the tunnel axis depending on the rock mass quality.In this study, an effort has been made to simulate RRS in a numerical code and to study its behaviour in a real case study. A double tube, triple lane tunnel in weathered tuff has been chosen as reference material for testing RRS under changing load. This was done by writing a FISH subroutine which is implemented in the discontinuous numerical code Universal Distinct Element Code (UDEC) that takes into account the characteristics of the reinforced beam (height, width, number of steel rebars, and spacing) in the calculation process. A rather extensive parametric study has been run by using the same key input rock mass parameters and in situ stresses (?H/?v = 0.5), and changing only the RRS key input parameters in order to simulate the changing loads during the face excavation process in tunneling. The critical part of this numerical modelling work that is under investigation in this study, is the period just after the excavation: i.e 4 hours; 16 hours after excavation; and finally after the complete curing (28 days) of the cement based components. The RRS are installed in three consecutive steps as follows: a) the application of S(fr) on the rock surface in order to even it up for the RRS application; b) radial bolting; and c) installation of the reinforcing steel rebars, that form the reinforced beam.The results show that: a) the S(fr) is almost overloaded in the first 4 hours after application in both tunnels; b) on the S(fr) and steel rebars, the stresses, axial and shear loads, as well as moments are decreasing after 16 hours and 28 days; and c) the bolts loads are increasing with time. The observed reduction in stresses and loads in both the S(fr) and reinforced beam is surprising. A possible explanation is that tunnel arches are self-supported to a lesser extent as time progresses due to redistribution of stresses in the nearby rock mass through the radial bolts. The loads in radial bolts are increasing by almost 25%. This can be seen from the overall results as we observe an increase in maximum principal stresses, displacements, shear displacements along joints and hydraulic apertures compared to tunnel baseline that has no support of any kind."

This paper presents the results of a research study that is being conducted by the Mine Effluents Program, Mining and Mineral Science Laboratory (MMSL), Natural Resources Canada aimed at evaluating biotechnology processes for the treatment of nitrogen compounds such as thiocyanide (CNS) and ammonia (NH4-N) which are present in gold mill effluents. A sequencing batch reactor (SBR) technology, commonly used for the biological treatment of municipal and industrial effluents, was used in this study. In the SBR process, the micro-organisms were able to degrade CNS to NH4-N and NH4-N to nitrate (NO3-N) at operating conditions of two 12 h treatment cycles per day, with pH maintained in the 7.4 to 7.6 range, and at room temperature (approximately 21°C) and also at 12 °C. The end products of CNS and NH4-N biological oxidation were NO3-N and sulphate (SO4) that are relatively non-toxic. Partial removal of NO3-N was achieved by biological denitrification reactions in the SBR process. The SBR process effluent was measured to be non-toxic to rainbow trout based on the 96 h acute toxicity test. The microbial consortium isolated from the SBR treating a simulated effluent was able to effectively oxidize CNS and NH4-N to NO3-N in water samples (under batch conditions) collected at three mine sites located in Quebec, the Northwest Territories, and Yukon.

Introduction 1. Factors on Which the Design of the Roaster Plant was Based The roasting operation at Lake Shore, it was realized from the outset, would be profitable only if costs and losses could be reduced to a low figure. It was absolutely imperative that the operation should be self roasting over the whole range of tonnages and sulphur grades which might obtain during the life of the mine. The roasting operation could not bear the cost in fuel and labour of even a partially fired roast. To meet this requirement, it was necessary to resort to additional insulation in the construction of the roaster and to provide a more flexible means of rabble cooling than the conventional water jacket (see p. 31). A second basic consideration was that of handling a wet, putty-like concentrate. The operating costs of a drier ruled out the possibility of drying the charge before feeding it to the roaster. Moreover, the dusting which would result from charging a dry product virtually all -40 micron (325 mesh) in size would seriously handicap the use of a dryer. Dust lubrication (p. 3S) due to the back-mixing action of the Edwards rabbles, and the use of a Sullivan slapper ( p. 27) on the concentrate filter, completely solved the problem of handling the roast feed. The third consideration was the reduction of dusting to a sufficiently low point that a Cottrell or other dust collecting system was unnecessary. The Edwards roaster when properly supervised, it is believed, can be operated to give the minimum possible dusting. At Lake Shore, the evidence has been that less than 1 per cent dusting occurs at any time and that the greater part of the dust produced is recovered either in the spray tower or in the dust chambers.

The complexity of the reduction process combined with the fact that under different experimental conditions the controlling stage for the reaction may not necessarily be the same, has led experimenters in this field to a wide diversity of opinion regarding the reaction mecl1anism. As a rule, each investigation has been restricted to the study of one or two variables over a comparative]~• narrow range of experimental conditions, and consequently, it be-comes necessary to examine the significance of each published work in the light of the results of other experimenters in the field.

Early in my term of office it was announced that my Presidential Address would make particular reference to the subject of Mine Taxation in Canada. With this purpose in view there was drafted a Balance Sheet and Earnings' Statement of a Gold Mining Company. This in turn was submitted to the Dominion Income Tax Authorities in Ottawa and to the Minister of. Mines of each of the several Provinces of Canada. The response to the invitation was not as complete as we had hoped it would be, but the figures that were obtained will be presented during the present meeting, and they constitute, I hope, a sufficient basis for extended study and additional enquiry during the year to come ( 1) . In the form letter that accompanied our request to have the submitted earnings assessed, I made the following assertion: "In this work we have three objects in mind. 1.--We will get direct comparison between the different Provinces in Canada. 2.-We will in this way educate our members concerning taxation as it affects the mining industry. 3.-We will publish the details as to how the assessment is calculated by each body, so that a Mine Manager in the most remote district will have a ready reference to guide him in his annual return to the taxing bodies. As things stand, there is no intention of making this a basis for a critical attack on the present rates assessed. It is intended as an authoritative summary of actual conditions as they presently exist".

On exploite le chrysotile dans le sud du Quebec depuis plus de cent ans. Au cours des dernieres decennies, cette variete d'amiante est devenue une des principales substances minerales produites au Quebec. Bien que la production de fibres d'amiante ait diminue de J 365 779 ten /979 a que!que 700 000 ten 1983, le Quebec n'en demeure pas mains une des principa/es sources d'approvisionnement mondial de ce mineral, la valeur des exportations quebecoises ayant ete de l'ordre de 315 millions de dollars en 1983. Dans la presente contribution, nous passons en revue que!ques-unes des notions mineralogiques sur l'amiante, depuis l'etymologie du mot et les proprietes physico-chimiques du mineral, jusqu'd ses principales utilisations commercia/es et industrielles. Un des deux objectifs principaux de noire article est de definir brievement le contexte geologique regional, la lithologie et /'age des roches ultramafiques serpentinisees et de decrire les principales caracteristiques des gftes de chrysotile qu'elles renferment et qui furent exploites a date dans /es Appa/aches du sud quebecois. Nous concluons en dressant un tableau synoptique de la metallogenie et en soulignant le role preponderant qu 'ont joue /es mouvements tectoniques majeurs, a la jonction des plaques et au sein des feuillets et copeaux de la croute oceanique (lithosphere et manteau superieur), sur la genese des gftes de chrysotile dans les roches ultramafiques serpentinisees, et tout particu!ierement dans celles associees aux series ophiolitiques du Paleozaique inf erieur.

"The operation of flotation circuits is often regarded as a combination of art and science. Expert systems offer a method by which the art may be captured and supervisory control of flotation circuits fully realized. Operating knowledge, the art, can be consistently employed to enhance regulatory control through the judicious application of an expert system. Using an expert system, a supervisory control routine was developed, and successfully implemented, in the copper flotation circuit at Brenda Mines Ltd.The expert system ran in conjunction with regulatory PI controllers on a personal computer. The expert system received assays via an on-stream analyzer and adjusted regulatory loop setpoints as required in real-time. From its commissioning in November 1989, to the end of the mine's operation in June, 1990, the ES was enthusiastically embraced by the operators and attained operating levels superior to those achievable manually.INTRODUCTIONExpert system (ES) software springs from research into artificial intelligence and is currently making significant inroads into business and industry. Proponents of ES's make many attractive claims; they are a means of capturing the knowledge of experts and they can be programmed by non-programmers. To date, most uses of this software in the process industry are in off-line applications, meaning that the applications are not tied directly to operating (on-line) environments. Moreover, in on-line environments, they are typically employed as advisors and only suggest changes. Human operators are required to close-the-loop to the process. Yet, ES's should be applicable to operating environments and, if the claims of the proponents are valid, should also be better than existing tools. As an intelligent controller an ES must use the knowledge of experienced operators, apply it directly to a process and do this without the intervention of human operators. On-line applications have been successfully employed in the closed loop control of SAG circuits (Herbst et al., 1989) and flotation circuits (Hales, 1989). Harris and Kosick (1988) detail an implementation for flotation control at the Polaris Mine."

"Abstract — The Detour Lake gold deposit is located within the northwestern limit of the Archean Abitibi Subprovince of the Superior Province. Mineralized zones are hosted by a sequence of pillowed and massive flows, hyaloclastite units, and altered ultramafic rocks, and are commonly oriented parallel to a series of high-strain zones that are co-planar to the regional-scale Sunday Lake Deformation Zone. Early in its kinematic history, the Sunday Lake Deformation Zone placed older (2.72 Ga) volcanic rocks of the Detour Lake Formation over younger (2.70 Ga) sedimentary rocks of the Caopatina assemblage. Gold mineralization is also parallel to the position of a laterally persistent siliceous rock unit, the 2.725 Ga ‘Chert Marker Horizon.’ The Chert Marker Horizon coincides with the boundary between the mafic-dominated upper Detour Lake Formation and the ultramafic-dominated lower Detour Lake Formation. Stratigraphic and geochronological data suggest that the Chert Marker Horizon is a deformed felsic volcaniclastic unit. At a cut-off of 0.6 g/t Au, the Detour Lake deposit contains 17.26 million ounces of Au within 445.9 million tonnes of rock grading 1.20 g/t Au. Gold occurs in shear-hosted and extensional vein arrays within rocks of the upper and lower Detour Lake Formation and within the Chert Marker Horizon. The Detour Lake deposit has many of the characteristics of a greenstone-hosted, quartz-carbonate vein deposit; its very large size may be attributed to large-scale rheological controls resulting in the formation of dilatant sites in permissive rock units. © 2012 Canadian Institute of Mining, Metallurgy and Petroleum. The behaviour of minerals during flotation of a fine-grained pyrite-rich base metal ore was investigated by studying the feeds. concentrate and tailings from every flotation cell in the mill of Brunswick Mining and Smelting Corp. Ltd. Bathurst. New Brunswick. The samples were analyzed for Zn, Pb, Cu. Fe, Ag. Sb, Sn, Bi, In. Hg, Mo and As. Mass balances were computed and recoveries of all metals from each cell. from each circuit and from the mill were calculated. The percentages of sphalerite. galena and chalcopyrite occurring as free grains in each product were determined with an image analyser, as were size analyses for free mineral grains and middling particles, and compared with screen analyses. The results show that the behaviour of minerals is defined much better by the recovery of free mineral grains than by the recovery of elements. The recovery of free sphalerite in the zinc circuit is 96 wt %, whereas Zn recovery from the same circuit is 88 wt %. the difference being due to unliberated sphalerite. The recovery of free sphalerite from the mill feed is 88 wt % and Zn 78 wt %. The recovery of free galenafrom the mill feed is 88 wt % and Pb 64 wt %. The recovery of free chalcopyrite is 75 wt % and Cu 45 wt %. Size analyses show that more sphalerite and chalcopyrite can be liberated by regrinding. but the galena is so fine grained that regrinding would not necessarily improve liberation. Silver occurs mainly in tetrahedrite and galena; the main loss in the tailings is due to large (+ 26 um) and small (-1 um} free and unliberated tetrahedrite grains.

"FLEX 31 is a chemically enhanced sodium isopropyl xanthate that has shown improved collection properties over SIPX for copper-bearing and copper-activated minerals. Characteristics such as improvement of recoveries with lower dosages and high pyrite and pyrrhotite rejection capabilities have all been exhibited by FLEX 31.The most improved results have been seen insphalerite flotation. Plant trial work carried out in the zinc circuit at the Louvicourt Concentratorin northern Quebec has produced data showing superior grade and recoveries from rougher concentrates that previously had not been attain-able with other reagents effectively. The improvement over SIPX adds up to an almost$1,000,000 annual improvement on the mine’s bottom line at present zinc prices.IntroductionSodium Isopropyl Xanthate (SIPX) is one of the most popular flotation chemicals on the market today. Somewhat more specific than amyl xanthates, SIPX is used in a variety of ores like copper/zinc, lead/zinc, arsenopyrite/pyrite, and many others (Melis et al., 1991). An investigation carried out by Tennants Research Technology Applications (TRTA), a division of Charles Tennant & Co. (Canada) Ltd., produced a chemically enhanced SIPX that has shown all the advantages of normal SIPX along with improved recoveries of copper and copper activated minerals. The new product has been named FLEX 31. Currently, FLEX 31 is manufactured at the Prospec Chemicals, another division of Charles Tennant & Co. (Canada) Ltd.A large amount of laboratory testing pro-grams have been carried out with FLEX 31 todate (MacPhail and Cousins, 1995; Cousins and MacPhail, 1996). One of the minerals that has a defnite affinity to improved fotation is sphalerite (ZnS). Once activated with copper sulphate, sphalerite has produced some of the most spectacular test work when FLEX 31 is compared directly to SIPX. Plant trials of FLEX31 at Aur Louvicourt Inc. carried out in 1995show that FLEX 31 is far superior to SIPX for activated sphalerite flotation."

"RÉSUMÉPour maintenir de bonnes performances technique et économique dans l’exploitation par chambres vides (open stope) il est primordial d’assurer un contrôle efficace des limites réelles des chantiers d’abattage. C’est pour cette raison que, de plus en plus souvent, les opérateurs miniers utilisent un dispositif de laser (Cavity Monitoring System: CMS) qui permet de mesurer les dimensions réelles de chantiers en vue de calculer principalement la dilution et les pertes du minerai ainsi que d’autres paramètres. Malheureusement, il n’existe présentement aucune méthodologie commune, qui permettrait d’interpréter ces résultats selon les mêmes critères pour faciliter d’évaluations juste et précise de la réalisation par rapport à la planification en vue d’en apporter de corrections, dans les différentes opérations minières, si nécessaire. Dans la première partie du présent article on discute les principales séquences d’exploitation utilisées dans l’extraction par chambres vides. Par la suite, à l’aide des sections transversales et longitudinales on illustre toutes les phases des travaux miniers. Ceci en vue de permettre au lecteur de comprendre mieux c’est qui peut se passer effectivement dans les chantiers d’abattage. Par après on donne les définitions générales de principaux facteurs de performance technique. Finalement, l’exemple numérique, élaboré à partir des résultats de l’arpentage au laser effectué par la mine Bousquet 2 dans un chantier d’abattage, donne les détails concernant les définitions ainsi que les valeurs de nombreux facteurs de performance technique, qui pourraient être utilisés, par après, par les opérateurs miniers dans le suivi de leurs exploitations souterraines."

"Coals from the Corporation's Lingan and No. 26 mines are classified as high-volatile A bituminous, with a very low inherent ash and containing all essential properties to provide metallurgical coal, with the exception of a high sulphur content. Only through proper cleaning at low specific gravity can the pyritic sulphur fraction of the coal be liberated to an extent where final sulphur meets coking standards. This first section of the paper deals with the operation of a Dutch State Mines heavy-media cyclone circuit to achieve an actual separation in the region of 1.30 specific gravity. A discussion of the parameter of control which must be applied along with an appraisal of the results obtained is included in the paper.Introduction Very high fluidity and unusually low ash are the outstanding characteristics of coal from Devco's mines in the Harbour seam of the Sydney coal field. However, sulphur content, mainly pyritic, ranges from 1 to 2.50Jo. To obtain the low and consistent sulphur level required in metallurgical coking coals, preparation at low specific gravity is required. Metallurgical coke is used as the fuel in an iron blast furnace. Its sulphur content largely determines the quantity of slag required to remove it and therefore influences furnace productivity and unit costs. Typical data on the various coal properties of importance in the production of coke are included in Table 1."

The subject of capital and the Canadian mining industry is so broad as to be impossible of comprehensive treatment in brief form. The purpose of the present paper therefore will be only to outline in a meagre and somewhat sketchy way the nature and extent of the mineral industry in Canada and to point out those features which are likely to be of general public interest. Canada's mineral industry has a long and creditable production record. The? value of the output per capita rose from $2.23 in 1886 to a maximum of $26.40 in 1920 and the value of the products from ten million dollars in the former year to almost two hundred and twenty-eight million dollars in 1920. Compared with other primary industries and on the basis of the value of production, mining held third place in Canada in 1920, the premier position going to agriculture and the value of forestry production placing that industry second in the list. In the mineral industry, between six and seven thousand properties are operated annually giving employment to up- wards of sixty thousand employees to whom about seventy-five million dollars is paid in salaries and wages. During 1923, seventeen mineral products reached a production value of one million dollars or over in Canada and in order of the values assigned these were: coal, gold, nickel, cement, cop-per, silver, clay products, lead, asbestos, natural gas, stone, zinc, lime, cobalt, sand and gravel, gypsum and salt.

CHROME ore, which is used in Canada in the production of ferro-alloys, in a variety of refractories, and indirectly in many chemical industries, is both commercially important and scientifically interesting. The sight of chromium metal is familiar to all of us in the home, the office, the automobile. Chromium is synonymous with armaments: it is understood to be one of several essential metals in tanks, which played an important role in the defeat of France in the spring of 1940. Chromium, in other forms, comes into our daily lives by way of paint and leather, of the familiar green ginger-ale and beer bottles, and more indirectly in every pound of steel produced in the .Dominion. Necessary to our civilization in peace and war, the ores of chromium occur in many corners of the earth. This paper attempts to describe the chemical constitution of chrome ores, their world distribution, and the various chrome products and their industrial applications in Canada.

"INTRODUCTIONThe Hemlo area is located in Northern Ontario, some 350 km east of Thunder Bay along highway 17. Hemlo is Canada's largest, recent discovery of gold ore. Three companies, Lac, Noranda, and Teck are currently developing mines and processing facilities in the area. A combined milling capacity of approximately 10,000 tonnes per day will ultimately be available in the area. The gold yield will be between 750,000 to 1,000,000 ozs. per year. This will slightly more than double Ontario's gold production but only represents 2.5% of the total world gold production.Lac Minerals Ltd. is substantially committed to the gold mining business. Lac currently operates four (4) gold mines and mills and produces 300,000 ozs. per year. Lac's Hemlo development will put the company in the position of number one gold producer in Canada.The Lac Hemlo ore body has an estimated reserve of 37 million tonnes at 6.2 g/t. The mining will take place at an initial rate of 3,000 t/d commencing in the first quarter of 1986. The mine and plant will be expanded to 6,000 t/d in 1988. At this capacity, Lac will be the largest producer in the Hemlo area.1. GEOLOGYThe orebody is divided into two (2) zones for mining purposes. The 'A' Zone is to the east and dips to the Northeast from surface to 300 m below surface. This zone has a strike length of 250 m to 300 m at an average width of 20 m. The upperpart of the zone will be mined by open pit and the remainder from underground. The 'B' Zone, to the north west of the 'A' Zone, although lined up is not necessarily an extension of 'A' Zone. 'B' Zone has a known strike length of 600 m and is open to depth and along the strike. It is over 70 m wide in the core. This zone holds most of the property's known reserves. 'C' Zone with narrow gold zones is under exploration in the south west portion of the property."

"Testwork was conducted in order to compare the effectiveness of carbon based catalysts while oxidizing a gold-bearing pyrite concentrate under atmospheric conditions. Without the use of a catalyst the oxidation of pyrite was incomplete after 96 hours, resulting in roughly 60% pyrite oxidation. Furthermore the resulting sulphate yield from unassisted pyrite oxidation ranged between 72 and 84%. It is evident that atmospheric oxidation of pyrite is not viable due to slow oxidation kinetics and high sulphuric acid production when using a ferric sulphate media without a catalyst. The effect of two different carbon based catalysts, one Lewatit® AF 5 a new microporous carbonaceous bead, and the second a granular coconut shell based activated carbon, were tested under the same oxidation conditions as the unassisted pyrite tests. After 96 hours, Lewatit® AF 5 assisted oxidation showed approximately 96% pyrite oxidation with a sulphate yield ranging between 26 and 35%, while the activated carbon assisted oxidation reached approximately 100% pyrite oxidation with a sulphate yield between 36 and 37%. Testwork indicated that not only does the addition of a carbon based catalyst greatly improve the oxidation kinetics and lead to almost complete oxidation, the sulphate yield is also drastically reduced, resulting in very high elemental sulphur yields.INTRODUCTION Pyrite is an important sulphide mineral due to its ability to contain invisible gold concentrations up to 132 g/t (Chryssoulis & Cabri, 1990). As a result, ores composed of only a small amount of pyrite still have the ability to contain high gold grades. This refractory gold cannot be accessed via conventional gold leaching processes, such as cyanidation, and additional oxidative pre-treatment is required to liberate the gold, making it more amenable to extraction (Marsden & House, 2006). Current oxidative pre-treatments do exist consisting of both pyrometallurgical and hydrometallurgical processes including high temperature and high pressure leaching within autoclaves, biological atmospheric leaching and roasting. Autoclaves are a proven process reaching desirable recoveries within approximately a 1–2 hour retention time, due to the increased temperatures and pressures, allowing for an increase in dissolved oxygen (Marsden & House, 2006). Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a ?plateau? and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have ?exemplary? safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA?s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization?s mindset? What characterizes these organizations? decision making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensive research since 1997 of organizations that suffered unexpected disasters. Their huge focus on safety, the achievement of lofty goals and celebration of safety successes led to ?mindset? that they are leaders in safety, protected by an extraordinary safety system and that their safety incident figures indicated a truly world-class safety performance. It led them into the trap of the seven deadly delusions?
