"Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a "plateau" and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have "exemplary" safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensiveresearch since 1997 of organizations that suffered unexpected disasters.The Piper Alpha Oil Rig disaster in the North Sea, on the 6 July 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry.In short, an explosion and fire occurred when a pipe started leaking gas and ignited. A temporary flange, with no safety valve, was used to block off this pipe during a maintenance operation the previous shift. The permit to advise operators not to start the pumps on this line was misplaced and lost. Several deficiencies, problems and system failures coincided. A key factor was that the water deluge system was inoperable at the time and failed to extinguish the large fire that erupted, followed by an even larger gas explosion. The accommodation unit, situated on top of the oil rig, was the main "killing field" - most men gathered here to await instructions (which never came) and where they died from smoke inhalation. Later, a large explosion of the gas pipeline from an adjoined rig exploded and killed more people.The line of events and the associated system failures are all very relevant in the analysis of the accident, and provide the only insight into what caused the accident. But much "further back", inside the organization’s culture, lurks more dark matter. The first inclination is to ask what was "deficient" in the culture. Did they not care enough about safety, or about their people? Did they have a culture where production decisions were more important than safety considerations? Did they ‘coerce’ supervisors and employees to ignore safety precautions or did they just "fly by the seat of their pants"?Not surprisingly, it is very difficult to identify the true nature of the culture in that business because in such an aftermath of death and destruction, no one dare say that the safety culture was a ‘positive one, focused and caring’ or that the company was doing well in safety and that this is only a freak accident. Indeed, there were many deficiencies in the organization’s systems and procedures, such as the permit to work system.But there are some niggling questions: How is then that the Piper Alpha Oil Rig was actually considered the company’s most productive and safest oil rig in the North Sea? Why would this rig win a safety competition, 6 months before the disaster, with the deficient permit as its most outstanding successful system?"

The Central Andes in Peru, Chile, Bolivia, and Argentina , contain two contiguous terranes which were accreted to Gondwanaland between the end of the Devonian and the end of the Triassic. These terranes, Arequipa in the north, and Chilenia in the south, differ from each other in the timing and style of their metallogenic development. However, they contain over 90% of the Central Andean porphyry copper resource. The copper porphyries in the Arequipa terrane are Paleocene to lower Oligocene in age, whereas the Chilenia deposits are mid-upper Cretaceous and mid-upper Miocene. Porphyry copper deposits occur outside these terranes, but are all Miocene in age. Miocene porphyry-style mineralisation also exists in the Arequipa terrane, but is copper-deficient and gold-rich. The accreted terranes may have favoured porphyry and other styles of copper mineralisation because they possessed relatively thin continental crust compared to adjacent parts of the Andes. Crustal thickening which commenced at about 30 Ma in the Arequipa terrane coincided with a major change to polymetallic, precious metal mineralisation which commonly displays high sulfidation characteristics.

The use of refuge chambers for use in handling underground emergencies is an issue of topical relevance within the US mining industry. Chamber design is of importance to meet some new safety requirements of US federal and some state government mine regulations. A review has been undertaken of a steel mine refuge chamber design to predict what will be internal temperature within the chamber over an extended period with miners present? The design has been examined for the situation when the chamber is sited in still mine air. Spreadsheets have been developed to allow various design assumptions to be tested. The evaluation has been undertaken using thermodynamic heat transfer models with some assumptions specified to produce a model of appropriate simplicity but with adequate degree of accuracy. Under normal still air conditions it can be seen that internal chamber conditions rise quickly to often arduous conditions. There will always be quite a few assumptions necessary in complex heat transfer process evaluations. Some important refuge chamber design parameters are: ? External surface area of the chamber, its wall thickness and construction. ? Metabolic heat generated and number of persons making use of the chamber. ? The temperature of the external mine atmosphere to the chamber over the year and the influence of geothermal gradient on temperature at depth. ? The maximum internal chamber Apparent Temperature considered safe. ? The distance inside the underground mine and adjacent ventilation conditions.

Demand for construction raw materials depends on investment in housing, other buildings and infrastructure. The provision of these materials requires early knowledge of future trends in construction investment. By using scenarios possible future developments and bandwidths can be explored. Policy makers in government organisations as well as in the private sector can base their policy decisions upon these insights. In this paper we discuss the case of sand and gravel in the Netherlands. We will show that, depending on the future role of government and the degree of internationalisation, the long term demand for construction raw materials will strongly diverge. Nevertheless there are some common developments such as a decreasing population growth, a smaller share of the labour force in total population and a growing wealth. In different scenarios we will encounter different conditions for minerals provision and for sustainable development.

This final report reviews installation, maintenance and inspection of mine pager telephone installations. A Technical Handbook was developed and distributed. Certain commercial telephone test equipment was modified and a tone source and switching circuit were developed for use in the testing of mine pager phone systems. Descriptions are included of the modifications to a commercial test set and of a tone source/ switching unit.

Sin embargo, cuando se da una excesiva profundizaciión del terreno, se emplea el método de pique vertical ya que logra el acceso de los operarios a las zonas más profundas de un modo más rentable y rápido a través de la extracción del mineral por izado de vagones o skip hasta el de nivel de superficie o intermedio. Por ello, es común que las minas replanteen la sustitución de su sistema de extracción habitual por el de izado por piques. En este sentido, el presente trabajo desarrolla la aplicación de los piques verticales en los sondeos de exploracion de la mina Hullera Vasco Leonesa. La primera parte del proyecto describe la ejecución de dos piques verticales (Aurelio y Emilio del Valle), y la segunda, en la ejecución de más de 17.000 m de galerías y obras de infraestructura de diversa índole.

The wastes from marble extraction in the Thasos- Kavalla-Drama region of northeastern Greece could be utilised as a new source of high-quality white carbonate fillers for the European market. Large deposits, satisfactory properties, acceptable processing costs and environmental benefits are among the factors that would have a positive bearing on a proposal for their use in this way. DTH, the waste arising from dolomitic marble quarrying operations on the island of Thasos, appears to be the more suitable material than DA waste from Nikissiani on the mainland because of the lower cost of primary grinding and the higher production rate of ultrafines with a mean particle size of 5 micrometres (the optimum size for filler properties in the case of both DTH and DA), its superior whiteness, the benefits that such use would bring to a very severe and complex environmental problem and the lower transport cost

The aim of this research was to study the absorption characteristics of thermally dried fine coal from South African collieries. After milling to ?150 µm and pressure filtration, the samples were subjected to either air drying or thermal drying at 150°C. The samples were then put into a climate box at 28°C and at three different relative humidity levels, namely 30, 60 and 94 per cent. It was found that the equilibrium moisture content of thermally dried coal at a specific relative humidity was lower than that of air dried coal. The relationship between the equilibrium moisture content and relative humidity is described by 1 ? RH = exp (-KTMen). There was no relationship between the volatile matter content and equilibrium moisture content, but there was an increase in the equilibrium moisture content of the coal with an increase in mineral content. The moisture uptake rate increased with an increase in temperature and a decrease in particle size. A model is discussed that fitted the experimental data and gave a good prediction for changes in the conditions. There was no evidence of greater moisture absorption by thermally dried coal. It includes specific data related to location, depth to the cap rock and to the salt, storage facilities for LPG, and production of salt, sulfur, petroleum, and rock related to or associated with the dome structure in the subject area. Also included is a brief history of the salt, sulfur, and petroleum industries as related to the salt domes. Gulf Coast salt domes were the scene of the first discovery of a large deposit of rock salt in North America, the birthplace of the Frasch sulfur industry, and the location of the first large oil gushers that ushered in the modern liquid fuel age.

Open pit mine plans define the complex strategy of displacement of ore and waste over the mine life. Various mixed integer linear programming (MILP) formulations have been used for production scheduling of open pit mines. The main problem with these models is the inability to solve real size mining problems. We present a mixed integer linear programming formulation for the open pit long-term production scheduling problem. To reduce the number of binary integer variables in the formulation, we aggregate blocks into larger units referred to as mining-cuts. We present a semi-supervised clustering algorithm which aggregates smaller blocks based on rock types and grade ranges into mining-cuts without introducing an error in the grade estimated values. The cluster-based aggregation algorithm is fully integrated into an MILP mine production scheduling prototype software. We have implemented, verified, and validated our models. The efficiency of the new platform is evaluated through iron ore mine case study.

Paper written on project work carried out at the University of Pretoria in partial fulfilment of the requirements of the B Tech degree at the Tshwane University of Technology This work tested whether ore particle size affects the carbon-based pre-reduction of titaniferous magnetite. Pre-reduction is performed at a temperature of some 1100°C in a rotary kiln, in the Highveld Steel process. Laboratory isothermal reduction measurements were performed with ore-coal mixtures at this temperature, to test the effect of ore particle size. The results show that size does limit the degree of reduction for particles larger than 15 mm.

The Velvet gold orebody (153 Koz Au) is located near the Kanowna Belle gold deposit in the Boorara Domain of the Kalgoorlie Terrane, a subdivision of the Archaean Eastern Goldfields Superterrane (Swager et al, 1990). Gold mineralisation at Velvet is hosted within an intrusion of intermediate composition that broadly follows a sub vertical hanging wall splay of the Fitzroy Shear Zone.Velvet was discovered in 2003 during an underground exploration drilling program targeting potential strike extensions to the Kanowna Belle deposit. Diamond drill hole KDU1595 returned a downhole intercept of 36 m grading 4.6 ppm Au approximately 500 m west of the deposit and at a vertical depth of 700 m below surface in the immediate hanging wall of the primary Kanowna Belle host structure, the Fitzroy Shear Zone. Subsequent drilling completed following the initial discovery, from both surface and underground collar positions, failed to replicate this result.In 2014, KDU1595 was resurveyed gyroscopically and the mineralised intercept correctly located. Additional drilling was undertaken to follow-up the repositioned discovery hole. Challenges included poor drilling angles through the Fitzroy Shear Zone, excessive deviation during drilling and the unknown geometry of the target. Navigational drilling helped to successfully penetrate the Fitzroy Shear Zone and in April 2015, drill hole KDU3234 returned a significant downhole intercept of 45 m grading 5.6 ppm Au.Additional drilling success resulted in a decision to develop a 600 m drive to serve as a dual-purpose access for further exploration and mining. This was a potential risk considering that the orebody geometry was unknown and all significant drilling results were clustered within a 50 m × 50 m panel. Development commenced in June 2015 and the first ore cut taken in April 2016. Geological mapping and structural interpretation, together with multi-element and spectral analysis of drill samples has enabled several other target areas to be identified for exploration. The Velvet mining area is critical to continued production at Kanowna Belle. Geological understanding must advance as mining progresses to ensure continued resource growth and optimal extraction of reserves.CITATION:Lorusso, G, Rogers, J and Tripp, G, 2017. Polaris Operations, a Cominco Ltd. lead-zinc mine, is located on Little Cornwallis Island, Northwest Territories, at Latitude 75°23' N and Longitude 96°75' w. When the Polaris Mine went into operation in late 1981, it became the world's most northerly base metal mine.The Polaris Concentrator, housed in a unique process barge, is a basic lead-zinc concentrator where flotation is used to produce the lead and zinc concentrates.The paper describes the ""coarse lead"" flotation feature incorporated in the concentrator flowsheet and describes the start-up problems and continuing operation of this circuit.

The primary water supply system for the City of San Francisco and adjacent peninsula areas consists of aging and deteriorated pipelines crossing under the San Francisco Bay (the Bay). This system is highly vulnerable to seismic damage from the nearby San Andreas and Hayward fault zones. The Bay Tunnel Project will replace the existing system with a 8-km-long (5-mi-long) tunnel underneath the Bay. The tunnel will be constructed using earth pressure balance methods and will have a two-pass lining system of precast concrete bolted, gasketed segments for initial support, and a welded steel pipe final lining. The project challenges include tunneling through both soft ground and rock conditions without any intermediate shaft access over the entire alignment. Seismic performance criteria include maintaining service flows after a large earthquake.

Energy West Mining Company (EWMC) has implemented a geologic program for improving stability and support requirements along deep two-entry longwall gateroads at its Deer Creek Mine operations located near Huntington, Utah. Together with geotechnical monitoring and concurrent review of mine seismicity in cooperation with MTI staff, it has become possible to improve longwall layout designs for two seam extraction of the Hiawatha and Blind Canyon seams. The geologic program consists of surface and in-mine exploration drilling, surface and in-mine geologic mapping, rock mechanics testing and detailed measurements of changes in depositional and structural conditions during development of every longwall gateroad. The geotechnical program consists of underground measurements at seven locations in two neighboring mining districts located in the Hiawatha and Blind Canyon coal seams under moderate to deep cover. Within the last five years, additional measurements, including data on seismicity and face conditions, have been incorporated into the data collection program for verifying the insitu coal strength and load transfer mechanisms for improving two-seam designs and support systems. Geologic mapping results are also used to predict local stability of headgate and tailgate entries, combining mine roof mapping, stratigraphic profiles of the coal and roof, and depth-of-cover information and dual-seam mine overlays, for use by operations personnel in planning roof and rib support for the retreating longwall sections.

INTRODUCTION LKAB was formed at the turn of the century when the railways between the ore fields and harbors were built. The mines are situated north of the Arctic Circle in Sweden. Kiirunavaara is the largest mine, with a pro¬duction capacity of approximately 30 Mt/a. Malm¬berget has a capacity of about 12 Mt/ a. Both are under¬ground mines. Svappavaara, the only opencast mine currently in operation, has a capacity of about 4.5 Mt/a. The ore harbors are at Narvik for the Kiruna and Svappavaara mines and at Lulea for the Malmberget mine (see Fig. 1). Production in the form of finished products is shown in Fig. 2a, b, c, and d. Due to poor ore markets in re¬cent years, production and shipments have been cut back, but there was a positive swing in 1978. LKAB has been 96% state-owned since 1957. In addition to the iron mines in northern Sweden, LKAB has subsidiaries in central Sweden which produce sulfide ores, quartz sand, and uranium (Fig. 3). The mining methods employed are opencast bench stoping at Svappavaara and sublevel caving at Kiirunavaara and Malmberget. GEOLOGY The Kiirunavaara ore body is 4000 m long and av¬erages about 90 m wide, with variations in width from a few meters to around 150 m. The ore has been ex¬plored by deep drilling down to about 1000 m, but mag¬netic measurements indicate a depth of at least 1500 m. The Kiirunavaara ore is oriented in a north-northeasterly direction with a number of ores in the boundaries be¬tween the wall rock types. The ore in Kiirunavaara is mainly a fine-grained magnetite with a varying content of fine-grained apatite. The apatite content of the ore decreases with increasing depth. The other ores are all apatite-bearing and a couple are hematite ores (Fig. 4a, b). The ores are sedimentary, tilted, and tectonically de¬formed. They have a lateral dip of 0.87 to 1.2 rad (50 to 70°). The contacts between ore and wall rock are distinct and sharp, but both the hanging wall and the footwall are brecciated and impregnated with magnetite. The footwall of the Kiirunavaara ore consists of syenite porphyry with a low quartz content and rela¬tively good strength; the hanging wall consists of quartz porphyry. Both types are known as Kiruna porphyries. The ore at the boundary with the hanging wall is often rich in kaolin. There are a few veins of syenite porphyry running through the ore, which has a few major faults (Fig. 4c, d). There are two sharply delineated ore grades in Kiirunavaara: low phosphorus ore with less than 0.1 % phosphorus (about one-third of the ore) and high phos¬phorus ore with greater than 0.5% phosphorus. The iron content is 67% in the low phosphorus ore and 60% in the high phosphorus ore. These ore types are sold as different grades and so are kept separate during mining. Copper production through smelting generates large volumes of material flows. As copper contained in ore becomes copper contained in concentrate to be fed into the smelting process, it leaves behind an altered landscape, sometimes mine waste, and always mill tailings. Copper concentrate, fluxing materials, fuels, oxygen, recyclables, scrap and water are in-puts to the process. Dust (recycled),gases — containing carbon dioxide(CO2) (dissipated) and sulfur dioxide (SO2) (mostly collected, transformed and sold) and slag (discarded or sold) — are among the significant process outputs. This article reports estimates of the flows of these input/output materials for a particular set of smelters studied in some countries. They include Chile (Altonorte, Caletones, Chagres, Chuquicamata, LasVentanas and Potrerillos), Japan(Naoshima, Onahama, Saganoseki,Tamano and Toyo), the United States(Garfield — Fig. 1 — Miami and Hayden) and Europe (Glogow I, Glogow II, Huelva, Norddeutscheand Ronnskar).
