"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

Introduction Atlas Consolidated Mining and Development Corp. (ACMDC) started gold production at its Masbate gold mine early in 1980. This involved an open-pit mine and a 3.5-k/d (3850-stpd) carbon-in-pulp processing facility (Pizzaro and Antonio, 1981). When this operation started, the mine began stockpiling sub-grade milling material averaging 1 g/t (0.029 oz per st) gold. Furthermore, geologists estimated that there were additional low grade reserves within the ultimate pit limits. These amounted to 2.4 Mt (2.6 million st) with an average grade of 1.1 g/t (0.03 oz per st) gold. The success of low cost heap leaching prompted an investigation to determine if this tech¬nique would be applicable for the low grade Masbate ore. The findings were favorable and a series of laboratory tests were undertaken. These tests indicated the amenability of this subgrade milling ore to heap leaching. Depending on ore size, gold extractions of more than 60% were obtained. To further define operating parameters for a commercial heap leaching plant, a 400-t (440-st) batch pilot plant was built and operated. These test results were used to design the first commercial heap leaching plant in the Philippines. One design concern was the control of solutions during the rainy season. Due to the material's low grade, every effort was made to minimize capital and operating costs. To this end, surplus mining equipment from the copper operations of ACMDC and used machinery in the crushing circuit were considered. Location: The Masbate gold heap leaching operations are located on the northeastern part of Masbate Island, Philippines. This is about 480 km (300 miles) south-east of Manila. Climate: The climate is moderate with two distinct periods. The dry season runs from January through June, and the rainy season extends from July through December. During the wet season, the rainfall at the mine site averages 300 mm/m (11.8 in. per month) for a total of about 2000 mm/year (78 in. per year). However, heavy downpours in excess of 100 mm/h (4 in. per hour) can occur. Geology: Four distinct rock types have been identified in the pit area. They are tuffaceous agglomerates, andesitic agglomerates, metasedimentary rocks, and hornblende augite andesitic porphyry. The tuffaceous agglomerate occupies the northern and western flanks of the mine. Andesitic agglomerates are localized distinctly in the central portion of the volcanic plug within the mine site. The eastern boundary of the mine is occupied by metasedimentary rocks, whereas, the hornblende augite andesitic porphyry occurs as an intrusive within the andesitic agglomerate and tuffaceous agglomerate. Some areas of the pit contain high proportions of clay minerals, requiring an agglomeration step in the flowsheet. This ensures satisfactory percolation through the heaps. To keep Homestake's Bulldog Mountain silver and lead mining and milling operation in southwestern Colorado workable, natural ground water and decant water from the mine's sand backfilling operation must be continually pumped from the workings. Left untreated, this water exceeds maximum allowable levels for various contaminants that may be discharged to the environment. To comply with regulations, a National Pollutant Discharge Elimination System (NPDES) permit was obtained and a mine water treatment plant for heavy metals removal was constructed in 1977. Due to equipment capacity and operational problems, and the lack of on-site technical personnel and analytic equipment, the first water treatment plant could not consistently meet permit limits. In 1979, Homestake entered into a consent decree with EPA and the Colorado Department of Health, calling for the construction of a water treatment facility to meet NPDES permit limits. Initial construction began in the spring of 1980 and the plant now produces effluent acceptable for discharge.

There have been fundamental structural changes in the industry. And I think that SME has been quite successful in coping with these changes as well. There has been a reorientation in how the minerals industry does business. It used to be called rationalization. Today we call it reengineering. In addition, the minerals industry has become more complex and more global. Shareholders have become more vocal and demanding in the management of the business, asking for protection of their shareholders' value. In the 1980s, a fundamental change was precipitated by a merger and acquisition fever in all industries, including the minerals industry. Raiders such as T. Boone Pickens put enormous demands on companies that, in turn, had to spend substantial management time to fend off the raiders. This resulted in a major restructuring of the mining houses. Today, for example, Anaconda, Kennecott and Amax are only historical names.

The former Homestake gold mine at Lead, South Dakota, has been selected by the National Science Foundation as the proposed host site for the Deep Underground Science and Engineering Laboratory. The deep laboratory and associated research campus will be developed at approximately the 7700-ft level. Plans also include the development of the Sanford Underground Science and Engineering Laboratory at the 4850-ft level. After the mine?s pumps were shut off in June, 2003, water levels rose to approximately the 4650-ft level by early 2008. Pumps in the 8150-ft deep mine were turned on again for dewatering in April, 2008. Instrumentation is being installed in the mine to measure water-level declines during pumping, for later analysis of permeability and related hydraulic properties of the formation. Pressure-sensitive transducers and data loggers will provide real-time information on water levels that will be available to interested parties. An assessment of U.S. western hard-rock miners? noise exposures was conducted as part of a multi-year National Institute for Occupational Safety and Health (NIOSH) survey of noise exposures in each sector of the mining industry. Noise from selected mining equipment and operator noise exposures were measured, analyzed, and tabulated for dissemination to the participating sites and are being used to direct NIOSH research and interventions to address the greatest noise hazards. Eighty-two noise dosimeter measurements were obtained, along with time-motion studies as the miners operated hard-rock mining machines. Ninety-six percent of the operators had daily noise doses that exceeded the Mine Safety and Health Administration?s permissible exposure level. The average gold miner dosages while operating the following equipment were: haul trucks - 261%, load-haul-dumps (LHDs) - 235%, single boom drills - 221%, bolters - 214%, and dual boom drills - 163%. The worst exposure level was a silver miner with a daily dose of 873%. Time-motion data showed that this miner?s exposure accumulated most rapidly while operating a jack-leg drill. These results will be used to help prioritize noise control development by NIOSH and other partners. "The Kittila underground mine is extracting one of the largest known gold deposits in Europe and has an estimated mine life through 2034. The Kittila mine is located in the Lapland region of northern Finland, approximately 900 kilometres north of Helsinki and 150 kilometres north of the Arctic Circle. The ore at the Kittila mine is refractory, making gold extraction relatively difficult because the gold is generally locked inside the two main sulphide minerals – arsenopyrite and arsenic-rich pyrite. The orebody consists of a complex pinch-and-swell structure including up to 100 different gold mineralization lenses. Fluctuating gold prices and other economic considerations increase the need to maintain and update the mining plans more frequently. Over 3,000 stopes need to be designed and optimized once a year for the LOM-plan and more frequent updates are needed for mid- and short term plans as well as production studies. As a large part of ore body is very sensitive to cost and gold price changes, stope optimization is crucial to find the best possible production scenario for every moment. Kittilä has gone through several methods for stope design and optimization. Most recently the Deswik Stope Optimizer (Deswik.SO) has been successfully used. Dewsik.SO has helped Kittilä to design and optimize stopes in a timely and cost efficient manner. Together with the Deswik suite of planning tools, reliable and auditable plans are created. This paper describes the methods used for stope design and optimization at Kittilä. THE KITTILÄ MINE The Kittila underground mine in northern Finland is extracting one of the largest known gold deposits in Europe. Kittila achieved commercial production on May 1, 2009, becoming Agnico Eagle’s first mine to open outside of Canada. The Kittilä orebodies were initially mined from two open pits – Suuri and Roura – and underground operations were added in October 2010. The pits were mined out in November 2012, so mining is now entirely underground in the Suuri and Roura deposits. Additional, small open pits will mine any remaining near-surface reserves in the future. The Kittila mine is located in the Lapland region of northern Finland, approximately 900 kilometres north of Helsinki and 150 kilometres north of the Arctic Circle. With a mine life estimated through 2034, Kittila is Agnico’s longest-life mine. The mine’s proven and probable reserves contain 4.7 million ounces (32 million tonnes at 4.6 g/t gold). Ore has been mined from underground since 2010. The original 3,000-tonne-per-day operation underwent an expansion project to increase the throughput capacity of the mill by 1,000 tonnes/day, starting by the end of 2014. The region around the Kittila mine is underlain by mafic volcanic and sedimentary rocks of the Kittila Greenstone Belt oriented nearly vertical. This greenstone belt is similar to those hosting Agnico Eagle's Canadian deposits in Quebec’s Abitibi region and Nunavut. At the contact point between iron-rich and magnesium-rich rocks lies a 100- to 200-metre-thick structural zone known as the “Suurikuusikko Trend”. This trend hosts the Kittila deposit, where multiple mineralized zones have been traced over a strike length of more than 25 kilometres."

Separation of minerals by shape has received little attention, since minerals normally do not break into different shapes during crushing and grinding. However, comminution of consumer wastes generates opportunities for novel separations based on particle shape. For instance, ductile plastics are broken into longer curved shapes, whereas brittle plastics yield angular particles. Using spouting beds for particle separation in water requires controlling the spout's instability. The preferred geometry for such redesigned spouting beds is reported. Plastic cylinders, discs and spheres were separated as a function of rising water velocity and amount of particles in the system. By using 5 mm particles, differences in settling behavior could be video recorded.

Over the past 100 years we have witnessed a revolution in permanent magnetic materials. Prior to 1932 electrical machinery depended upon steels for both the soft and the permanent magnet structures. Because permanent magnet steels were easily demagnetized, motors, generators, loudspeakers and similar devices often used the interaction between two electromagnets. Motors and generators were mostly of the induction type, depending for function upon induced fields. The discovery of Alnico in 1931 and its rapid commercialization presaged an expansion of the industrial revolution and today, permanent magnet structures are widely utilized. [ ] DEVELOPMENT OF PERMANENT MAGNETS Act 1: Early Days of Permanent Magnets Alnico, which stands for aluminum, nickel and cobalt, is a family of alloys with similar structure and processing requirements. Additional elements, such as copper and titanium, are present in minor amounts to optimize magnetic properties. Alnico is known by many trade names around the world including Alcomax, Hycomax, Ticonal, Nipermag, Koerzit, Oerstit, Nialco, etc(1,2,3). In addition to the generic name ?alnico?, it is usually appended with a number representing the magnetic grade.

Since 2001, CVRD is carrying out a modernization project in it’s pelletizing plant’s automation systems, that comprehends instrumentation, control and information systems. In addition, the project includes implementation of advanced control systems, using artificial intelligence techniques, including fuzzy logic, expert systems and dynamic physical models, to increase production, reduce fuel and energy consumption and improve quality. This paper reports CVRD’s experiences in carrying out an optimization project.

?Classification System Efficiency (CSE)? is a simple, powerful concept which can be used to measure and improve ball mill circuit efficiency. This paper describes six plant case studies which demonstrate the use of CSE to diagnose and/or improve grinding circuit performance, with related improvements in circuit efficiency ranging from 3 to 30 percent. The increased effective mill grinding energy is recaptured from energy that was going into over grinding, which can greatly benefit downstream mineral separation. The achievable CSE for given circuit design/operating variables is described, providing every plant metallurgist with the means to determine the potential for increasing their particular grinding circuit?s efficiency through increased CSE.

The Empire concentrator was built in four stages, with the latest stage accounting for 50% of the overall plant capacity. Over the past 5 years, the average throughput of the three grinding lines has decreased by 50 LTPH (15%) without significant changes in ore characteristics and despite the addition of secondary grinding capacity to supplement the existing pebble mills that are the constraint of the grinding circuit. This paper discusses the different areas under study that are aimed at optimizing grinding circuit utilization which includes improving system logic, pebble crushing operation and pebble mills operation.

Mycobacterium phlei has very good potential as a hematite flotation collector. The flotation recovery of hematite is dependent upon the solution pH and concentration of the microorganism. The critical pH for maximum recovery of hematite is around three, which corresponds to the point of zero charge of the microorganism. An excellent correlation between the flotation recovery and bacterial adhesion on hematite is observed. The adhesion of Mycobacterium phlei on hematite can be described by the potential energy of interaction calculations using an extended DLVO theory.

In processing coal, the low concentrate value requires high capacity production and sharp classification/separation techniques to reduce specific production costs. The allflux separator meets these criteria with a unique two-stage design. Through a combination of rising current and fluidized bed technologies, the allflux has extended the state of the art by expanding the particle size range that can be efficiently processed. Using concepts that have proven successful in processing concrete sands, small-scale industrial testing with coal feed stocks looks very promising. Attractive applications for the allflux are projected in fine coal recovery from old settling ponds or as a retrofit in existing fines circuits.

The block cutting machine is one of the most important noise sources in the stone processing factories. In this study, the assessability of the noise from the block cutting machines in the stone processing factories using the P-wave velocity was investigated. The noise levels of some block cutting machines were firstly measured during the cutting of the three different rocks. Then, core samples of the same rocks were cut by an automatic cutting machine in the laboratory and noise levels were measured. A conversion factor was obtained by dividing the site noise levels by the laboratory noise levels. Then, thirty seven different rocks were cut by an automatic cutting machine in the laboratory and noise levels were measured. The P-wave velocity of rock samples were also measured in the laboratory. A strong relation between the laboratory noise levels and the P-wave velocity was found. The noise level from the block cutting machines can be assessed using the derived relation and the conversion factor.

The Wyoming bentonite industry finished 2005 with strong growth. For most of the markets, demand exceeded supply. The industry continues to be heartened by the strength of its longtime markets, oilfield and iron ore pelletizing markets. But overall, all of the major markets enjoyed good to excellent commercial conditions. Growth at the macroscale was fueled by the continued strong domestic economy, by the exchange rate of the U.S. dollar in global markets and by strong worldwide global demand. As result, two new greenfield operations are in the early stage of development in the Wyoming region for the first time in more than two decades.

Risk assessment is an essential component of mine feasibility studies. However, it is often challenging to identify the appropriate analysis and derive the required input data. The uniqueness of a mining project and the associated technical issues require customization of the risk assessment approach to meet both requirements and characteristics. The uncertainty driving the operating risk in mine feasibility studies and its assessment through Monte Carlo simulation as a component of a cash flow analysis are discussed in this paper. The paper introduces the use of the analytic hierarchy process for derivation of qualitative input and conducts quantification of the three main sources of operating performance delays, equipment, ground and schedule problems, in a cash flow simulation. A methodology for the assessment of operating risk in mine feasibility studies is presented.

The influence of solution ionic strength (IS) and electrolyte valence on the flotation behavior of malachite has been investigated. The microflotation tests were conducted over a range of solution IS (1?300 mM) at a constant speed (340 rpm), pH (9.5), flotation time (10 min), and sodium oleate dosage (5.3×10-7 moles/g). The size of malachite ranged from 45 to 53 µm, and two different types of electrolytes (NaCl and CaCl2) were employed. Electrophoretic mobility was also measured over the same IS range employed in the microflotation study. Overall, strong coupled effect of solution IS and ion valence was observed. Specifically, the flotability of malachite increased with increasing IS in the presence of monovalent cation (Na+) while the flotability increased up to 30 mM and decreased with increasing IS in the presence of divalent cation (Ca2+). Notably, the flotability of malachite was greater with the presence of Na+ compared with Ca2+ under high IS conditions (IS>30 mM). Consistent trend for Na+ versus Ca2+ was observed from the flotation results with different SO dosages. The characterization and flotation results suggested that the trend was likely attributed to the increased unfavorable interaction between malachite and bubble via the charge reversal of bubbles and malachite at higher IS in the presence of Ca2+.

Indigenous people are nearly always affected when projects enter their territory. Crop and other asset compensation can help or undermine a project’s progress depending on the approach. When compensation is handled poorly, a technically viable project can get indefinitely entangled by local unrest. Stake holders associated with compensation issues include; the state, the indigenous people, non-governmental organizations, and the developer. Appreciation for the unique perspective of indigenous peoples and the influences affecting them can avert problems and prevent delays. A review of the compensation issues encountered in Ghana during its recent gold boom can assist in avoiding problems associated with handling poor and uneducated people.

Mine production crews and drill-and-blast crews may be exposed to unexpected movement of ground when working under pit slopes. Without specialist geotechnical advice and appropriate measurement systems, mine crews are faced with conducting risk assessments and relying on subsequent controls that may or may not be adequate to ensure their safety. Mine crews need an ?early warning? monitoring system to give them the confidence that they will have time to walk out of the danger zone if the pit slope begins to move. Slope stability radar technology is used by mine geotechnical engineers to assess overall slope stability over an extended period, typically days or weeks, and to critically monitor slopes that are actively moving and may become unsafe. The complexity of slope stability radar allows for geotechnical engineers to assess movement types and movement rates across multiple work areas of a mine from long ranges, with alarm capability to a central location. However there are many mine work areas where production and drill-and blast crews are not adequately covered or alarmed by slope stability radar systems. What is required is an easy-to-use, short-range, fast-scanning tool that can be operated directly by a work crew and can provide a local alarm with sufficient warning when a wall movement occurs within the work area. The Work Area Monitor (WAM) concept has been developed specifically to be operated by a mine production crew. The system comprises a sensitive, fast-scanning radar coupled with a high-resolution camera, built into a mine-standard light vehicle. The system has been designed specifically for ease of use with the output being a local area alarm to warn workers of slope movement in their vicinity, as opposed to the broader pit monitoring and long-term deformation measurements of the known slope stability radar systems.
