Fault tree analysis is a systematic safety analysis tool that proceeds deductively from the occurrence of an undesired event (accident) to the identification of the root causes of that event. One recurring mine safety problem—a dozer falling into a void over a drawpoint on a coal surge pile—was analyzed using available, inexpensive fault tree programs on a personal computer. The analysis identified basic and intermediate events that led to the burial of the dozer and graphically depicted the interrelationship between these various subordinate events as well as the various chain of events leading up to the primary event. A sensitivity analysis on these probabilities showed which events had the greatest influence on dozer burial in a coal surge pile.

The Bureau of Mines performed experiments to determine concentrations of toxic gases and liquids, and smoke-particle diameters and diameter-concentrations evolved during degradation of PVC-derived brattice with and without nylon-woven fabrics, fiberglass, cotton, and jute brattices, and other mine materials. The samples were exposed in a 20-L chamber at temperatures of 1500 to 4500 C, with a chamber airflow of 10 L/min; 27.3 cm3/s of which was directed into a submicrometer-particle detector-analyzer. During the degradation of PVC-derived brattice, large concentrations of hydrogen chloride (TLV, 5 ppm; STEL, 100 ppm) evolved at a temperature as low as 2000 C. The minimum toxic load of 3.1 mg or 40 ppm in 50 L of dilution air, with O.l~g weight loss, may be reached at a mine face with 314,000 L of dilution air, at the 5th min, during which less than 1 kg of brattice undergoes combustion. Fiberglass, cotton, and jute brattices, pine wood dust, and coal dust showed small concentrations of carbon monoxide evolving during early stages of combustion. Hydrogen cyanide gas evolved from the PVC-derived, nylon-reinforced brattice; at a later stage of combustion, large concentrations pf carbon monoxide and dense smoke evolved from all PVC-derived brattices. Experiments to characterize smoke particles showed that marked differences exist in particle diameter-concentrations among the materials.

"Introduction.The Bureau of Mines has taken up the problem of determining a method or methods whereby underground compensation insurance rates for metal-mining companies can be equalized in proportion to the underground hazards involved. This study has been started by the writer with the assistance of Mr. I. M. Wolfin* as consultant.The present paper briefly reviews the origin of accident compensation, gives a digest of State compensation laws, and describes types of insurance carriers. This will be followed by study of the basis of premium rate, risk differentials, and cost of compensation insurance to mine operators. These data will be supplemented by an analysis of the accident hazards in different mining methods and practices, and the severity and frequency of accidents classified by time lost and period of exposure to hazard, for different States, mining methods, occupations and nationalities. Attention will be given to standardized preventives for underground accidents in metal mines, and, finally, the classification of underground metal-mining hazard for workmen's compensation insurance premium rating."

"A recent premature explosion in a quarry that resulted in the death of nine men and the injury of several others, was directly traceable to a spark from a steam-shovel working under the face directly in front of the hole being loaded. This accident and similar accidents on record in the Bureau of Mines indicates the necessity for pointing out the dangers in quarries of explosives being ignited by sparks emitted from steam shovels and locomotives, and making recommendations for greater safety.Many premature explosions of which the following are examples, have resulted from sparks, both from steam shovels and passing locomotives. (A) The premature explosion occurred during August, 1920 at a limestone quarry, in a 100-foot drill hole, loaded with 1000 pounds of high-grade nitroglycerin and gelatin dynamites. This hole had been loaded with 500 pounds of 75 per cent strength gelatin dynamite, 300 pounds of 60 per cent straight nitroglycerin dynamite, and 200 pounds of 50 per cent straight nitro¬glycerin dynamite; but no steaming had been placed in the hole. The powderman loading the hole saw the spark enter the hole and was able to run a short distance before the explosion took place, thus escaping serious injury. The explosion of this first hole was transmitted to nearby hole similarly charged. The fall of rocks from these two holes buried the steam shovel and some of the gang of men working around it.(B) In September, 1909, in the Canal Zone, a steam shovel uncovered a misfired charge of dynamite. While the dynamite was being picked up the steam shovel continued working nearby. A spark from the shovel set fire to some loose dynamite spilled on the ground from the troken cartridges. This fire was transmitted to a hole which was being loaded close by and caused a premature explosion. One man was injured."

This paper is one of a series being prepared by the Bureau of Mines describing; gypsum mining and crushing methods and costs at various operations throughout the, United States. These papers are designed to dissemminate technical information regarding methods used. The cost tabulations represent local operating expenditures only and not total production costs. It is recognized that publication of total production costs may in many instances cause embarrassment to the individual operator as well as to the industry as a whole; on the other hand, operating costs are essential to the technical discussion and. study of the methods employed. The attention of the reader is specifically called to this differentiation in order that no misunderstanding of the scope of the cost tabulation shall ensue. The following report deals specifically with the mining and primary crushing methods employed at the plant of the Ebsary Gypsum Co., Inc., Wheatland, Monroe County, N. Y. This company is capable of maintaining a maximum yearly production of 220,000 tons of crushed gypsum, thereby ranking as one of the largest gypsum operators of the State. The methods and practices employed typify the standard methods followed by gypsum producers in western Now York State, The bed of gypsum mined lies only 35 feet beneath the surface and is above average thickness for this vicinity.

"Standard rock dust for use in the rock dusting of coal mines might be defined tentatively as powdered mineral, light colored and free of carbonaceous matter and free silica, all of which will pass a 20-mesh screen while 50% of it will pass-through a 200-mesh screen.Such dust may be prepared from limestone, gypsum, anhydrite, or shale free of sand and flint. For the initial rock dusting of the average nongaseous bituminous mines enough standard rock dust should be applied so that the combustible content of the resulting mixture of rook dust with mine dust shall not exceed 45%, a range somewhere between 35% end 45% being the practical objective sought.All entries, slopes or passageways and room necks should be rock dusted.Additional safeguards are:a.,Rockdusting roomsb.,Scattering of dust in vicinity of shots before firing.c.,Placing of barriers in the mouths of panels, cross-entries and other key positions.Redusting becomes necessary whenever the combustible content of the mine dust exceeds the permissible maximum. This should be determined by regular, systematic sampling, followed by a simple analysis of the samples for combustible content. A Bureau of Mines volumeter for measuring the density can be used for the determination of combustible content."

This report describes Bureau of Mines investigations of a low-grade scheelite-bearing granodiorite intrusive situated on Henderson Creek, Granite County, Mont. The intrusive is in a narrow, V-shaped valley at the head of extensive placer workings. Since May 1943 the H. & R. Mines Co. of Portland, Oreg., has recovered scheelite sand and gold from the placer workings. A modified Yuba, 4?cubic foot bucket dredge was used in the operation. By February 1947 this operation had recovered and marketed over 215,000 pounds of scheelite concentrates containing 63 percent WO3. The scheelite in the placer was found to be derived principally from erosion of the granodiorite intrusive. The intrusive is one of a series of four similar, dikelike masses of granodiorite whose outcrops are aligned along a 10-mile zone of weakness trending N. 60° E. The intrusive has a triangular outcrop exposed over an area of 57 acres. Trenching and sampling by the Bureau of Mines demonstrated that scheelite is sparsely distributed throughout the granodiorite. This work delimited the outcrop of the intrusive and indicated a large submarginal reserve of low-grade tungsten mineralization. The deposit is ideally suited for exploitation by large-scale, open-pit mining operations; it would be easily available in a national emergency. Additional exploration would be necessary, however, before large-scale production plans could be made efficiently or accurately.

"INTRODUCTION The Mining Branch of the Bureau of Mines has used the wagon drill to sample mineral deposits in New Mexico. This method of sampling was successful and con¬stituted an important phase of the work on five exploratory projects undertaken in 1943, 1944, and 1945. The deposits varied in topographic features, geology, mineral content, and in the physical characteristics of the rocks drilled. The altitudes ranged from 630 to 7,800 feet. Three hundred and seventeen holes totaling 6,852 feet were drilled. The deepest hole was 66 feet; the average depth was a little less than 22 feet.The results of the above-mentioned work form the basis of this report. Special attention is given to the later and more improved techniques used. This report was written with the expectation that the procedures might prove advantageous to others in the exploration and sampling of near-surface mineral de¬posits, and that a more extended use of the technique might result.AcknowledgmentsAcknowledgment is due Charles H. "INTRODUCTION Laboratory mineral-dressing investigations covered by this report show that ores from Montgomery County, Ark., and Morgan County, Mo., are potential sources of barite and zinc. Fine grinding and froth flotation yielded drilling-grade barite from the former and chemical-grade barite and zinc concentrates from the latter.Samples from these deposits were subjected to a laboratory investigation to develop an economic and feasible treatment. The Arkansas ore, submitted by the Husky Refining Co., consisted of an intimate mixture of the economic mineral and quartz. Fine grinding and fatty acid flotation were utilized to effect a good recovery of a drilling-grade product. The other was a zinc-barite ore submitted by the Rocky Ford Mining Co., Stover, Mo. There was no problem in the recovery of zinc (most of which occurred as sphalerite), but the recovery of barite by flotation was complicated by the presence of gangue carbonates. Ultimately, an excellent recovery of barite was achieved with a relatively new type of flotation promoter, petroleum sulfonate.In recent years, barite has become one of the major industrial minerals as an essential ingredient in heavy drilling fluids, pigments, and chemicals. New sources of this important commodity are always interesting; thus, it is the purpose of this report to present information that may permit commercial exploitation of two deposits not in production."

The Bureau of Mines conducted corrosion research to determine suitable construction materials for geothermal resource recovery plants. Weight loss, pitting and crevice corrosion, U-bend stress corrosion, and electrochemical polarization measurements were made on selected metals in brine and steam process environments produced from high-enthalpy hypersaline brine from geothermal well Magmamax No. 1 at the Salton Sea Known Geothermal Resources Area, Imperial Valley, Calif. Cadmium (and by extension cadmium coatings) and a 6061-T6 aluminum alloy were unsatisfactory because of high general corrosion rates and, in the case of aluminum, severe pitting Molybdenum and niobium (columbium) were resistant to general corrosion, pitting, and crevice corrosion, Copper alloys corroded at rates that may preclude their use in wellhead brine, The presence of iron accelerated the corrosion of the copper alloys Titanium alloys were resistant to general corrosion and stress corrosion cracking in all of the environments. They exhibited crevice corrosion in some of the brine and steam environments and Ti6A14V pitted in the brine environments. They exhibited passive behavior over a broad range of potentials. Exposure tests were conducted on a high-temperature polyphenylene sulfide thermoplastic (Ryton) in the same environments. A 40-pct-graphite fiber-reinforced Ryton composite deteriorated readily in wellhead brine and failed in all of the brine and steam process environments; Ryton coatings on 316L stainless steel were adherent and performed well.

On March 14, 1921, an explosion occurred at about 6:15 A.~. on the premises of an iron-mining concern throug:1 which two magazines, one containing upwards of 9,000 pounds ot a high explosive of the dynamite class, and a second, 60 feet distant, containing upwards of 10,000 No. 6 detonators, were completely destroyed, and a ncratertt,? f3et deep by 30 feet in diameter, was excavated in the gro~ at ~he place whare the mag~iine containing the high explosive had been lo~ated. The Bureau of Mines collected coal and coal waste aggregate material from two different mine sites to determine their strength characteristics for use in concrete. The concrete would be placed underground as support walls for ground control in adverse conditions or unique mining applications. A standard mix design was established for a "wet" mix using a water-to-cement (w/c) ratio of 1.0 to facilitate pumping into place. Later, material was collected from four more mine sites and used to make 2-by 2-by 2-ft test blocks with the same w/c ratio. A pull-out cone device and borehole shear tester were used to determine the in situ material strength of the test blocks. These techniques will provide a simple and quick way of determining support wall strength in an underground environment. Test results from the pullout cone de-vice showed a good correlation of 0.98 to test cylinder compressive strengths of the same mix. Results of the borehole shear tester were inconclusive.

The publication, of this information should be of interest to the industry at large and the exchange of ideas thereby made possible should be of economic value to operators in all harts of the country. The last previous Bureau of Mines publication relative to details of permissible methane detectors (Bulletin 331, published in 1930) describes detectors of four different types, namely flame, combustion, catalytic, and a combination Weatstone-bridge and catalytic type in which catalytic action at the surface of a platinum filament when in methane causes a change in the temperature and resistance of the filament, thus unbalancing the bridge circuit and giving a reading proportional to the methane concentration. Three detractors of the latter type have been approved since 1930. A brief description of each follows: U. C. C. Detector Approval No. 902, covering the U. C. C. indicating detector, as described in Bulletin 331, was extended. January 27, 1930, to cover important changes in design that permit checking the calibration of the detector at any time whether in gas or fresh air. Essential features of the detector are shown in figure 1.

This report, the 22d in a series, gives analytical data showing composition and quality of tipple and delivered samples of coal collected during fiscal year 1972. This coverage lists mines located in 20 States. Ultimate analyses are given for most of the coalbeds listed. Tipple samples were collected by Bureau of Mines engineers at tipples as coal was being loaded into railroad cars or trucks. The deliveries were sampled in accordance with instructions issued by the Bureau of Mines, and the samples were analyzed in the Bureau laboratory at Pittsburgh.
