South Deep Gold Mine, owned by Gold Fields Ltd., is situated near Westonaria in the Gauteng Province of South Africa and mines the conglomerate bands of the Upper Elsburg reefs (Mondeor Conglomerate Formation) of the Witwatersrand Supergroup and the Ventersdorp Contact Reef (VCR) of the Ventersdorp Supergroup. The stoping and underground developments are mechanized. The Upper Elsburg reefs are mined by a variety of mining methods, including mechanized drift and fill, modified drift and bench, longhole stoping, and low-profile mining. Optimal mine design and scheduling for deep-level mechanized mining are complex, and success is highly dependent on detailed, robust, and accurate geological and geostatistical models. Geological structures significantly influence the sedimentological characteristics, distribution, and preservation of the Upper Elsburg reefs and VCR. Accordingly, particular emphasis is placed on the generation of a mine-scale structural model that accommodates the relationships between the older north-trending fault systems (West Rand and Panvlakte faults) and younger east-trending dextral wrench faults. Results from underground mapping, borehole intersections, and highresolution three-dimensional seismic data have been integrated to produce coherent three-dimensional geological models. The Upper Elsburg reefs suboutcrop against the VCR and comprise an easterly diverging clastic wedge, thickening from the suboutcrop position, to approximately 130 m at the mine?s eastern boundary. The Upper Elsburg reefs are characterized by conglomerate and quartzite bands forming multiple, stacked, upward-fining unconformity-bounded couplets. Palaeocurrent directions are dominantly from west-northwest to east-southeast, indicating that the more proximal deposits are preserved close to the suboutcrop, with distal facies to the east. Sedimentological modelling is applied to individual stratigraphic units and caters for facies definition. This is achieved through channel width (CW) kriging and fitting of type sections to borehole and mapping data. Homogenous geological geozones for each stratigraphic unit are thus defined within individual structural blocks on the basis of those sedimentological parameters that have been found to have a positive spatial correlation to gold concentration. These geozones then serve as constraints to the evaluation of the orebody. This contribution presents a summary of the modelling processes that are currently applied in the development of high-confidence, timeously produced geological models that are essential input for mineral resource estimation and mechanized mine planning and scheduling.

Over the last three decades, the mining industry has been moving towards underground mechanized mining methods and the number of load haul dump (LHD) vehicles utilized has increased. The growth of mechanization and automation has benefited both workers and mining companies. However, due to the design constraints with LHD vehicles and the limitations of the operating environment, restrictions to operator visibility has contributed to a number of accidents, including fatal injuries. Past researchers have used the light filament technique to collect obscuration zones around mobile equipment and have produced 2D visibility charts (shadow diagrams) of this information. The light filament method involves manually collecting visibility data, which results in errors and is time consuming, requiring around three hours to complete. This research utilizes a MENSI GS100 laser scanner along with 3D modelling software (3Dipsos) to collect and process operator visibility profiles for underground mobile equipment. Results from this test work indicate that a laser scanner can be successfully used to rapidly collect this data and utilize this information for improved mobile equipment design from a visibility perspective. The following aspects are discussed: improved rotary-drill bit performance, use of trucks and load haul- dump units underground, long-hole raising, blasting techniques, and the use of butterfly roofbolt plates for support.

Zincor operates four Lurgi fluidised-bed roasters at its roast-leach-electrowin refinery in Springs, South Africa. In 2006, Zincor?s four roasters had reached ages ranging between 26 and 38 years, while the expected life of this type of roaster is generally 20 to 25 years. Based on its condition, Roaster 3 was the first to be rebuilt in 2006. Roaster 4 is being rebuilt in 2007 and the last two roasters will be rebuilt over the next few years as necessary. The main damage to Roaster 3 was caused by buckling of the roaster steel shell. This caused the freeboard and dome areas to shift, resulting in cracks in the dome brick lining and crumbling of the bricks lining the freeboard. The steel shell buckled due to the combined effects of too-high operating temperatures and corrosion caused by the following two main factors: ? Thicker external insulation was installed during a previous maintenance shut which resulted in and increased steel shell temperature and exacerbated the normal metal fatigue; ? SO2/SO3 containing gas that had permeated through the brick lining, passed through its dew point during frequent planned and unplanned shuts over the life of the roaster. This caused acid formation between the refractory brick lining and steel shell, resulting in corrosion of the shell. The initial plan was to replace only the damaged areas of Roaster 3, but upon dismantling more damage was found and the roaster was rebuilt from the tuyeres upwards. During the rebuild, the opportunity was taken to modify the design of some of the brickwork to extend its life and make future repairs easier. This paper discusses the mode of failure of Roaster 3 and the lessons learnt during its dismantling and rebuilding.

The Limpopo Metamorphic Belt is a complex geological terrane with at least three major deformational events. Tectonically juxtaposed lithology, open to isoclinal folding, crosscutting and re-activated shear zones and metamorphic gneisses and schists make pit slope design and maintenance a risky business. Venetia diamond mine?s Cut 3 is planned to reach 360 m by 2011. The difficult rockmass created problems in the original pit and a new more comprehensive slope design was necessary. Nearly 9 km of geotechnical drilling were undertaken to produce a new geological model and provide geotechnical information near the final Cut 3 face. All existing data from the pit were brought together into a database. SRK Consulting were brought into the project to audit the process and provide most of the manpower for the data analysis. Existing failures were back-analysed and the geotechnical domains identified. Analyses of only the southern domains are discussed in this paper. Geotechnical engineers can design slopes at any angle, but it is up to the managers at the mine to decide what risk they are willing to accept in a slope design. Once the issue of risk was discussed and settled in respect of the consequences of failure, each of the main geotechnical domains were analysed to produce a slope design at5% probability of failure with 15% above the first ramp. A risk assessment of the design was then undertaken to determine what risk the design placed on the equipment, personnel and the profitability of the overall mining operation. The risk assessment indicated minimal risk to the operation. One of the main assumptions of the final design is that the pit slopes will be dewatered. A project was immediately put in place to determine a groundwater model for the mine and to thereby determine the best dewatering procedure. However, it should be obvious that it is practically impossible to prevent benches from getting briefly saturated with water during storm conditions. It was therefore necessary to look specifically at supporting the crests of the ramps in the problematic southern domains to guard against the possibility of crest failure during heavy rains. The data available were re-analysed toward this objective. Spiles were recommended as the most cost-effective support system, and a probabilistic approach was used in the design. The final design has a 5% probability of failure, and the analyses indicate a strong sensitivity of the required spile spacing to the strength of the reinforcement material that will eventually be used. The pit slope parameters derived as per the discussion in this paper have just begun to be implemented at Venetia Mine.

A South African gold mining rock-related fatal accident database has been analysed in order to determine rock-related fatality trends in the gold mining industry over the period 1990 to 1998. This analysis was extended further to include some of the individual reefs mined and focused particularly on the Carbon Leader Reef, the Ventersdorp Contact Reef, the Vaal Reef and the Basal Reef. Rock-related accident locations are also presented. A stope rock-related risk is proposed, where the fatality rate in terms of rock-related fatalities per million square metres mined for the gold mining industry as a whole is compared to individual reefs, allowing a stope rock-related risk to be attributed to the individual reef. This comparison is extended further by removing the contribution by the reef being evaluated from the industry data, thereby allowing this reef to be directly compared to the rest of the industry with respect to a stope rock-related risk.

"Occupational noise-induced hearing loss (ONIHL) is classified as the leading work-related disability in the mining industry. ONIHL has a negative impact, on not only the health and occupational productivity of affected individuals, but arguably also on the country’s mining industry and economy. Hearing conservation programmes (HCPs) are an effective strategy in the management of ONIHL. However, current literature indicates that HCPs are not achieving the anticipated and desired outcomes in the South African mining sector despite the efforts focused on the management of ONIHL. Therefore, the purpose of this study is to propose the use of a feedback-based noise monitoring model as a tool for monitoring and managing ONIHL in the South Africa’s mining sector. This model is a basic static feedback model with practical applications such as estimating, monitoring. and providing quantitative information to aid miners, mining administrators, and policy-makers in decision-making around HCPs. Additionally, the model could form part of an early intervention and management strategy for ONIHL in the workplace. The strength of this model, although currently static, is that it encompasses all the pillars of HCPs and takes into account the policies concerned with the management of ONIHL in the mining sector. IntroductionHazardous noise levels (above 85 dBA) in the workplace place exposed employees at risk of developing a disabling occupational noise-induced hearing loss (ONIHL). ONIHL is the ‘partial or complete hearing loss in one or both ears as a result of one’s employment’ (Nandi and Dhatrak, 2008, p. 1). This type of hearing loss develops gradually as a result of being exposed to continuous or intermittent high levels of noise over a long period of time (McBride, 2004; Patel et al., 2010; Rappaport and Provencal, 2001). A hearing threshold below 40 dBs is classified as a disabling hearing loss (Yadav et al., 2015), resulting in a hidden condition that does not readily manifest itself (Tye-Murray, 2009, p. 3). Dugan (2003) describes disabling hearing loss as the ‘most prevalent, least recognized and least understood physical disability’, while Copley and Frederichs (2010) and Hermanus (2007) argue that permanent disabling hearing loss is a major contributor to the global burden of disease on individuals, families, communities, and countries."

Paper written on project work carried out in partial fulfilment of B. Eng (Mining Engineering) degree The Union Section of Anglo Platinum is situated ±18 km west of the town of Northam on the north-western rim of the Bushveld Igneous Complex. Both the Merensky and UG2 reefs are mined. Support of the Merensky stoping areas with rock bolts is specifically problematic due to the stoping width restrictions varying around 90 cm. The installation equipment of in-stope rock bolts and the effectiveness thereof in the Merensky stoping areas was critically evaluated. In order to do this a background research on the need for better on-the-face support had to be conducted. The need for improving the rock bolt drilling and the equipment had to be investigated and the financial implications and justifications for the improved equipment examined. To conduct the search an overall study was done on the rockfalls and safety aspects throughout the platinum industry in South Africa and specifically Union Section. It was found that most of the rock falls in the Merensky stoping areas of which ± 70% are less than 1.2 m in thickness generally occurs within 4 m from the face and causes ± 60% of the in-stope injuries. Serious shortcomings had been identified with respect to the airleg, rock drill machine and the drill steels used in the drilling of the rock bolt holes. Through experimentation, using a testing frame built for this purpose on surface and with the co-operation of mine staff and suppliers, a number of possible modifications were identified and tested. This resulted in the final product with which it was possible to drill a 1.2 m hole in the hangingwall of the stopes ranging with an angle to the hanging from 90° for a 90 cm stoping width and 67° for a 80 cm stoping width. It is confidently concluded that this system will effectively improve the total face support and result in a substantial reduction of rock falls and consequently the occurrence of injuries and fatalities in the Merensky stoping areas. The installation of rock bolts on the face with the modified drilling equipment will increase the direct support, but the system has a potential of a substantial reduction in the profile stick support density and an increase in productivity in the stoping areas due to fewer occurrences of rock falls. It was finally concluded that effective modifications to the drilling equipment for rock bolt in-hole drilling could be made and the installation thereof in the stope face is adequately justified.

To remain at the leading edge, in any business, depends on accurate but flexible design thereby allowing operational managers to provide alternatives to improve their operations. The ultimate goal is to deliver to plan, with optimum economics. Mining is no different, with falling prices and escalating costs, improved margins become the focal objective. There are many ways to achieve this, ranging from operational efficiency improvements through production scenarios to planning initiatives. Of all, planning usually receives the least attention, yet it is the area of most influence on the NPV of the reserve. With computer technology, numerous concepts and iterations are possible thereby closing the gap between optimal and actual design. This paper discusses the application of the split shell design at the De Beers Venetia open pit in South Africa.

The approximately 2800 Ma old Ventersdorp Contact Reef is a unique Witwatersrand orebody, from which about 6% of the world?s gold production is derived. A major portion of this production is mined at depths exceeding 2500 m and deepest levels of mining are at about 3600 m. The orebody?s hangingwall consists of volcanic rocks. This differs from the older Witwatersrand orebodies, which have quartzitic hangingwalls. The thickness of the Ventersdorp Contact Reef is generally about 1.20 m, but is highly variable, at an average dip of about 18°. A pronounced unconformity beneath this orebody is also expressed by varying footwall rock type characteristics and frequent rolling of the reef plane. These rolls follow topographic variations of the palaeo-surface. More geological than rock mechanics studies were performed on the Ventersdorp Contact Reef prior to 1992. Recently, however, a multidisciplinary team comprising various rock engineering and geological disciplines has delineated geotechnical areas by a novel technique. Geotechnical areas are defined by distinct footwall/hangingwall rock assemblages. The presence of three footwall and two hangingwall rock types results in the definition of six geotechnical areas across a distance exceeding 60 km, including about 10 gold mines. Distinct rock mass behaviour has been documented in three of the six geotechnically defined areas. Different rock mass behaviour is expressed by features such as mining-induced fracturing, closure rates, fall of ground characteristics and seismic character. Some geological features override the rock mass behaviour as defined for the individual geotechnical areas. These are undulations of the orebody or the frequency, characteristics and attitude of major discontinuities. It is concluded that the concept of geotechnically subdividing orebodies is not only important for facies delineation and the associated grade, but also needs to be considered for safety aspects, in determining factors such as the appropriate support to be employed.

A number of challenges face platinum concentrator plant operators. These challenges include the increase in operating costs, the increase in smelter cost for the processing of concentrate, the shortage and cost of power, and the tightening of specifications on concentrate quality by the toll-smelting operations. Over the years the focus has moved from extracting the platinum group metals (PGM) from the Merensky Reef to the UG2 reef. This has a number of advantages, including the higher ?basket price? for UG2 concentrate, reduced mining cost per unit volume as a result of the higher density of UG2, and the reduction in overall concentrate tonnage to be smelted. In many cases the Merensky ore has been fully exploited and it makes sense for the focus to shift to the UG2 ore that can be accessed through the Merensky shaft infrastructure. The presence of relatively high levels of chromite in UG2 concentrate is, however, a major disadvantage due to the problems associated with smelting such a concentrate in conventional submerged arc furnaces. In addition to increasing the specification on the minimum PGM grade of concentrates, smelters have had to impose strict specifications on the levels of chromite in the concentrate. The threat of high penalties has forced concentrators to change their modus operandi, often resulting in a significant loss in recovery. The final concentrate grades and PGM recoveries are shown to vary significantly throughout the industry. The reasons for this include varying ore mineralogy and different operating philosophies. This would therefore imply that the opportunity exists to optimize the operations by considering fundamental aspects such as the PGM mineralogy and the application of appropriate technologies. By returning to the fundamentals of flotation and applying the findings of detailed process reviews, it has been possible to increase the concentrate PGM grade, reduce the concentrate chromite grade, and in some cases increase the recovery of PGM to concentrate. This paper presents case studies where this approach has been used to successfully optimize concentrator performance, resulting in lower operating cost and higher PGM production. A leaching model for gold ore that takes into account the impact of cyanide and oxygen concentrations as well as gold surface exposure on leaching performance is developed. Cyanide consumption is estimated by an empirical model that is a function of ore particles size distribution as well as cyanide, copper and sulphur concentrations. Increased contamination due to sand in spiral feed has had a negative effect on the efficiency of spiral concentrators at Navigation Plant. The no. 1 seam at Kromdraai opencast, which was previously mined by underground bord and pillar methods, is now susceptible to spontaneous combustion because of air ingress into the old workings which are exposed in the opencast high wall. This posed a risk in the loss of reserves to burnt coal. As solutions, buffer blasting of the preceding cut to collapse bords and soft overburden dressing of un-collapsed bords were implemented to prevent air entering the workings. The latter remedy resulted in increased contamination due to addition of sand in Run of Mine. After evaluating alternatives, a low capital option was selected. The 110t/hr Teetered Bed Separator (TBS) has been installed, primarily to desand and upgrade the feed to the spiral concentrators. The unit indicated an upgrade with a yield increase of 0.4% during test work and is currently showing an overall plant yield increase of 0.97%.

The work described forms part of a Safety in Mines Research Advisory Committee (SIMRAC) funded project, currently an ongoing programme of research aimed at providing ?improved criteria, guidelines and the use of numerical models to design mine layouts at depth, so as to reduce seismicity and enhance worker safety?. The entire project deals with a number of distinct, albeit interrelated, research areas such as: subsidence and caving in hardrock, general numerical criteria for layout design, regional benefits of backfill, concrete pillars, strike stabilizing pillars and bracket pillars. This paper will focus on the last two items only. New considerations for the design of both stabilizing pillars and bracket pillar layouts are briefly discussed.

"In self-aerated flotation machines, the gas rate depends on operational variables (e.g. froth depth and impeller speed), pulp properties (e.g. solid content and viscosity), and reagent addition (e.g. type and concentration of frother). The gas rate has a strong correlation with the flotation performance by influencing the gas dispersion properties and froth retention time. A factorial experimental design was used to study how the gas dispersion properties, the froth retention time, and the flotation performance respond to changes in froth depth and impeller speed (as the most common operational variables). An in-depth understanding of the effects of impeller speed and froth depth on the gas dispersion properties, especially the bubble surface area flux and froth retention time, is necessary to improve operating strategies for self-aerated flotation machines. All experiments were carried out in a 50 m3 selfaerated flotation cell in an iron ore processing plant. The results showed that the froth depth affected the metallurgical performance mostly via changing the froth retention time. The impeller speed had two important impacts on the metallurgical performance via varying both the froth retention time and the bubble surface area flux in the froth and pulp zones, respectively. The interaction effects of the froth depth and impeller speed were also established. This allowed us to develop a strategy for operating selfaerated flotation machines based on varying the froth depth and impeller speed with regard to the cell duty. This manuscript focuses on the design and development of steels for the railroad industry and methods of testing them. Here are summarized the results of research focusing on alloy development and design, with particular focus on advanced rails and wheels. The high-performance steels are characterized for their mechanical and service characteristics. The relationship between properties and cleanliness is discussed and compared to the regular railroad steel. Some of the tests presented are standard tests that are complemented with the nonstandard test recommended by the Association of American Railroads (AAR) and recommended modifications of this. The paper presents the results of metallurgical analysis, mechanical, and residual stress tests. The laboratory methods include microstructure, residual stresses, tensile, hardness, fracture toughness, and microcleanliness tests.

Pushback 8 South is the final cut back on the south wall of the East Pit at BHPBilliton?s Mt Whaleback iron ore mine in the Pilbara region of Western Australia. Development of the cutback commenced in 1999 and is now nearing completion with ultimate pit walls up to 300m high. This paper describes the geotechnical, structural and hydrogeological setting of PB8S, reconciliation between design and slope performance and the slope engineering techniques used to effectively manage the risks associated with the slope. These techniques include slope buttressing and sequential mining with waste backfilling, as well as a detailed program of displacement and hydrogeological monitoring, structural mapping and slope stability assessment. During the course of slope formation recent developments in remote slope displacement monitoring using radar and remote structural mapping using terrestrial photogrammetry have also been applied. Aspects of the PB8S slope management plan are relevant to many large scale open pit slopes. This paper provides geotechnical practitioners with information which may be helpful in managing geotechnical risk in similar environments.

Shotcrete has become a strategic component of support in underground mines. The ductility of reinforced shotcrete makes it a safe and economic means of surface support under adverse static and dynamic loads. The ability of ductile shotcrete to sustain load at large deflections enables excavations in squeezing conditions to be stabilized and the safety in excavations subject to rock bursting, to be assured. A Shotcrete Working Group was formed to find the configurations of fibre reinforcement that would give shotcrete the same ductility as when reinforced with mesh. Beams and panels reinforced with various types and lengths of fibre were accordingly tested. A single mix design consisting of river sand, ordinary Portland cement, fly ash, condensed silica fume and gunite dust and segregation suppressant was used. Steel and polypropylene fibre varying in length from 25 to 50 mm were used in nominal contents of 5 % and 0.5% by mass respectively. Beams, panels and control specimens were shot into specially made formers. Suitable mixing procedures were developed for the different types of fibre. The test specimens were shot in a semi-dry mix process. The shotcrete was cured until tested 28 days after shooting. The fibre contents and uniaxial compressive and Brazilian tensile strengths of the shotcrete were determined from the control specimens. The beams and panels were tested in bending in specially manufactured test rigs. The beams were examined for simple supports and fixed ends. The panels were fixed in a manner that simulated underground conditions and were loaded statically under uniform pressure. The beams were loaded and the corresponding deflections measured up to failure. The panels were loaded up to mid-span deflections of 150 mm. The results are presented in the form of load-deflection graphs. The peak loads and ultimate deflections for the fixed end beams were considerably larger than for the simply supported beams for the various types of fibre considered. None of the beams tested, however, were able to sustain load at relatively constant value beyond the peak load. The panel tests showed that 40 mm and 50 mm long steel fibre and 30 mm, 40 mm and 50 mm long monofilament polypropylene fibre reinforcement gave shotcrete the same ductility as mesh.

Techniques used to safely control the roof in UK coal mines for some 10 years have been applied to a South African coal mine. This new approach known in the UK as advanced rockbolting technology, is based on applying four fundamental principles: ?Understanding the roof failure mechanisms ?Using an effective roof support system ?Designing this support using measurement ?Monitoring the performance of the system. The paper summarizes the approach in detail and describes how it is being applied by Anglo Coal, giving results obtained to date. Investigations at a number of South African coal mines, including stress measurements at two Anglo Coal mines, have confirmed that the mode of roof failure (lateral shearing due to horizontal stress) is the same as in other coalfields worldwide, and aspects of current world best practice, including the use of advanced technology rockbolting, are therefore relevant in South Africa. The stress measurements indicated a high level of stress field anisotropy and further investigation of stress conditions in South African coal mines is recommended. The most effective bolting system to resist shear failure is one with high bond strength and stiffness. Short encapsulation pull testing of existing South African systems confirmed that they have low bond strength and stiffness. An improved rockbolt system with the required performance, and features allowing rapid installation and installation quality and performance audit, has been developed and is currently under full-scale trial. Design by measurement and routine monitoring procedures including the use of a rotary telltale device are also under trial. It is anticipated that South African coal mines will be able to obtain significant safety and productivity benefits from the application of this technology.
