The mining industry has the highest average annual fatality rate among major U.S. industries. To address this, the National Institute for Occupational Safety and Health, Pittsburgh Research Laboratory (PRL), is conducting major research programs to reduce the hazard exposure of miners. One of the recently concluded programs at PRL developed the enabling technology for remote-controlled mining. An application involves the manual process of extracting and hauling coal where operators, in the tight confines of a mine, can be struck or caught by mobile machinery. The approach to remedy this problem uses a guidance system on the continuous haulage equipment so that it follows the continuous mining machine that extracts coal. This, in essence, involves sensor-based docking of the machines. Sensors functioning in the hostile mine environment of dust, methane gas, and water play the key role. Computer analysis of the mining machine's movements and empirical machine characterizations established operating requirements and spatial limitations to ensure proper loading of coal into the haulage equipment. These data served in the selection of a guidance sensor. Technologies such as scanning laser systems and ultrasonic sensors have frequently been used in other applications, but were found unacceptable. However, a near-infrared sensor employing active targets met the requirements. The sensor has a nominal 75b conical field of view and a range from 0.1 to 18.0 m. For the single-target mode, nominal range accuracy was 4.3% at a distance of 3.65 m. Correction algorithms were generated, which reduced the error to 0.6%. Airborne dust testing showed less than an additional0.8% accuracy (worst case) degradation at dust levels up to 15 mg/m3. Using four targets, the nominal range accuracy was 0.4% without correction algorithms. Analysis of dynamic testing of a continuous miner and haulage showed that accuracy was maintained and total target loss did not occur. A guidance system for the haulage system to follow the mining machine does not exist commercially. Such a system can reduce fatalities, injuries, and exposure to dust and noise.

The Bureau of Mines conducts several of these seminars each year in order to bring the latest results of Bureau research to the attention of the mining industry as quickly as possible.

This paper has been abstracted from a forthcoming Bureau of Mines bulletin on the concentration of copper ores in North America. GENERAL DEVELOPMENTS IN CONCENTRATING COPPER ORES FROM 1912 TO 1932 During the east 20 years the milling of copper ores as practiced in the larger concentrators has chanced to such an extent that comparatively few of the machines in use at the beginning of this period remain in service today. Concentrating Practice in 1912. In 1912 the usual equipment for concentrating copper ores comprised jigs for the coarser sizes, tables and sand vanners for the intermediate sizes, and vanners for the finer crushed and ground material. Rolls operated in series under graded crushing conditions, were in common use for the liberation of the copper minerals; grinding by Chilean or Muntington mills ordinarily was confined to the coarser tailings of jigs and tables.

The revised estimate of coal production in January 1977, is 39 million tons -- down 12 mi 11 ion tons compared with production in January of last year, largely as a consequence of the snow storms and cold. The major coal-hauling railroads operating in Eastern and Midwestern States reported to the Bureau of Mines that they loaded approximately the same number of cars for the week ended February 5, as in the preceding week. The Bureau of Mines estimates that U.S. coal production for the week ending February 5, 1977, was approximately 8.5 million tons, or nearly 3.5 million tons less than production for the corresponding week in 1976. Coal production from January 1 - February 5, 1977, was approximately 47.8 mi 11 ion tons compared with production of 63.5 million tons for the corresponding period in 1976. As of the first week in February, river and truck shipments of coal in the Eastern U.S. were about one-half of the normal traffic. Despite the freezing weather and ice and snow conditions of rivers and highways and reduced rail shipments, there was reportedly little or no shortage in coal supply to consumers. Inventories at electric utility plants and steel company coke plants at the beginning of the severe weather were sufficient in most cases for at least 30 days" operations. There have been no reports of coal shortages at other industrial plants.

This Bureau of Mines report presents a Fortran IV computer program for the structural analysis of (1) protective canopies used in underground mines to protect equipment operators from roof falls and (2) rollover protective structures (ROPS) used in surface mines to protect equipment operators during vehicle overturn accidents. The program, entitled PLASTIC CANOPY, permits rapid and inexpensive calculation of the ultimate strength and deformation characteristics of a canopy for a variety of static loading conditions. The structure is modeled as a series of interconnected beam and/or plate elements. Stresses in, and displacements of, each structural element are computed and printed out at the yield strength of the canopy, at several intermediate load levels between the yield and ultimate strength of the canopy, and at the ultimate strength of the canopy. Other features of the program include prediction of buckling of a beam member under combined axial compression loading and bending, plotting of the input geometry of the structure, and plotting of the deformed shape of the loaded structure. Program description, input instructions, program listing, and four example problems are included.

"INTRODUCTION In a previous reports results were given of a manhole investigation carried out over a period from June,- 1929, to July, 1930, in cooperation with the Edison Electric Illuminating Co. of Boston, Mass., to determine the general hazards of combustible gases and vapors in manholes and other underground openings owned by this company. Considerable time was spent in this initial investigation in developing a suitable detector for determining the range of explosibility of the gases found in manholes. About 17,000 tests have been made with the detector since the investigation was started and its utility has been verified and its accuracy checked, by having complete analyses made of such samples that showed combustibles present equal to or above 25 per cent of the lower explosive limit, and those found deficient in oxygen.The initial investigation made in 1929 and 1930 demonstrated that the detection, control, and elimination of combustible hazards in manholes can best be obtained by a close cooperation of the interested parties through a systematic and continuous inspection of manholes, under proper supervision.COOPERATIVE INVESTIGATI0N DURING 1931The investigation carried out during the year 1931 has been along lines similar to that conducted during 1929 and 1930. In the present investigation the Edison Electric Illuminating Cc. of Boston and the Boston Consolidated Gas Co., have cooperated with the Bureau of lanes. The Boston Consolidated Gas Co. serves a major part of the territory under discussion with gas, but the tests covered the Edison territory irrespective of the gam company furnishing manufactured gas. A test crew consisting of two men, one an employee of the Consolidated Gas Co. and one an employee of the Edison Electric Illuminating Co., has been continuously employed during the year in the testing of games in manholes served by the Edison system. The test crew was under the direct supervision of John Campbell, superintendent of the special service department of the Edison Electric Illuminating Co., and F. M. Goodwin vice president of the Consolidated Gas Co.As in the previous investigation, all manhole tests were given test numbers. The test data sheets on which the information regarding the tests were recorded were prepared in triplicate, one copy going to the Edison Company, one going to the Gas company, and the third to the Bureau of Mines at Pittsburgh. These test data sheets were then filed (1) according to the town, (2) the street location, and (3) the manhole number; in this way a complete record pf each manhole tested was immediately available. Objective The main purpose of this video is to teach mine rescue personnel certain aspects of rescue work that are not traditionally taught. These include the strain caused by working continually under adverse conditions; the "survivor guilt" that occurs from being spared the fate of one's deceased cohorts; and the posttraumatic stress that may be experienced during the sometimes lengthy task of recovering the victims' bodies. Background On November 20, 1968, an explosion rocked the Farmington No. 9 Mine in northern West Virginia (figure 1). The explosion killed 78 of 99 miners who were working in the mine, including all but one member of the mine's rescue team. Some 30 years after the event, researchers from the NIOSH Pittsburgh Research Laboratory (PRL) conducted a video interview with Danny Kuhn (figure 2), the only Farmington No. 9 mine rescue team member who was not working that day and who did not die in the explosion. According to the Mine Safety and Health Administration (MSHA), there are about 80 underground coal mine rescue teams in the United States, and according to the National Mining Association, there are more than 850 underground coal mines. This represents a significant decrease in both over the past decade. The low number of rescue teams is a concern because serious incidents, though fewer, still happen, and if a disaster occurs, there will be fewer rescue teams to respond. This would result in an increased need to use available human resources very efficiently. Much training to prepare these rescue teams still takes the form of what educators call "teaching to the test." In other words, teams practice in order to win mine rescue contests. This test-oriented approach has traditionally been augmented by interactions with others who have responded to real situations or by participating in actual incidents. Given few incidents and fewer rescue teams from whom to derive enrichment, the preparation of coal mine rescue personnel becomes a problem. Recently, more emphasis has been placed on active learning, such as working out problems in smoke. These sessions more nearly simulate the actual conditions that today's team members might rarely encounter. To properly prepare and retain the current coal mine rescue personnel who are left in the workforce, it is necessary not only to offer such hands-on experience, but also to replace the learning narratives that once were passed from team to team. The present video is intended to contribute in that capacity. Approach NIOSH researchers first learned of Mr. Kuhn in the fall of1998. Mr. Kuhn was contacted and agreed to an interview. An oral history interview was conducted with Mr. Kuhn in early November1998. After reviewing the videotape of the interview, researchers determined the content to be so rich as to warrant a second interview in a more formal setting. The second interview was done in early December 1998. During this interview, Mr. Kuhn spent about 2 hours describing the Farmington disaster, including the workplace climate leading up to the explosion, the events that took place in the 10 days after the explosion, and the nearly 10-year effort to safely and successfully recover most of the victims' bodies. The videotape of the second oral history interview with Danny Kuhn was reviewed and edited by NIOSH researchers for content. Careful attention was used to ensure that valuable information was presented, but within a reasonable timeframe. A detailed map of the Farmington No. 9 Mine was edited into the final video and used to highlight critical features as they were being described by Mr. Kuhn. Results and Accomplishments A 60-minute videotape and instructor's guide for use in safety training or other settings was completed. The target audience consists of all those who work in underground coal mines. This training video will help safety instructors better prepare mine rescue team members for the situations that they will encounter when engaging in extended rescue and recovery work. The objective of the study is to examine the apparent influence of soil and near surface rock strata on the severity of damage experienced by surface structures subjected to mine subsidence. The report discusses: 1) modes of ground movement that might complicate or be confused with mine subsidence; 2) prevailing concepts concerning mine subsidence in the Northern Appalachian Coal Field and Illinois Basin; 3) apparent influences of soil and near surface rock strata on the character of subsidence-related ground movements; 4) classifications and criteria for characterizing damage due to ground movements; and 5) types of subsidence damage experienced by homes above active mines in western Pennsylvania. Presented is a damage classification system incorporating a severity index for characterizing subsidence damage to homes with basements above active mines. A series of case histories illustrate subsidence damage to structures above active and abandoned mines.

Ecuador a gross domestic product (GDP) grew 8% despite a slowdown in production and sales of petroleum, the dominant economic sector. Crude petroleum exports, which amounted to about 35% of total exports in 1977, were valued et $484 million,2 a decrease of 14% from 1976 values. Petroleum export sales were depressed because foreign buyers were somewhat uncertain that State-owned Corporación Estatal Petrólera Ecuatoriana (CEPE) held legal title to the oil until final settlement with Ecuadorian Gulf Oil Corp. In addition to the legal complication, Ecuador maintained a relatively high export price for oil until late in the year. There was also a shrinkage of traditional U.S, markets owing to the Alaskan pipeline becoming operational and an increased availability of Mexican supplies. The export market was also affected by increased domestic production in Peru and Chile, which lessened their import needs, and the pressure of greater competition from the Middle East in supplying crude oil to Caribbean refineries. About 20.4 million barrels or 40% of Ecuador's total crude exports went to the United States. Only 27 wells were drilled in Ecuador during L977; of these, 16 produced oil, 3 were dry holes, and 8 were suspended. About 31 wells were planned for 1978. Three new oilfields were discovered during 1977; two by CEPE in northeastern Ecuador and one by the Argentine State oil company Yacimientos Petraliferos Fiscales (YPF), operating in Ecuador under a service contract. During the present century powerful tools have been made available through the development of ferrous and. nonferrous alloys containing such elements as molybdenum, vanadium, titanium, tantalum, and tungsten, which were little used before 1900. One of the most important of these so-called rare elements is tungsten; its use ins alloys for high-speed cutting tool is partly responsible for the era of mass production. Those flat lying, bedded or replacement, non-fossil fuel deposits which exist within the U.S.A., and which are amenable to longwall mining, are identified by reference to three principal factors associated with their physical/geological environments. These are their depth, extent and structural continuity. Technical feasibility is assessed in relation to the likely cutting speeds which can be achieved in the identified minerals with existing longwall shearer machines. Economic comparisons are made between traditional mining operations in those deposits for which technical feasibility is demonstrable. Continents are made concerning those technical deficiencies which appear to preclude the application of existing longwall equipment to the identified deposits and suggestions are made regarding the lines of research aimed at improving longwall technology.

"Nickel is an essential metal in stainless steel, armor plate, and nickel steels and also in nonferrous alleys, such as monel metal and cupre-nickel, which are used in marine service. Nickel is one of the metals that must be greatly restricted in civilian use to provide the amounts needed for our War program. Consequently, it is desirable to investigate every possible substitute.The 5-cent coin no z in use contains 2.3 percent nickel. At the end of 1941, about 450 tons of nickel was being consumed annually to produce these coins. If the nickel used for that purpose in 9141 had been diverted to military use, 15,000 tons of armor plate could have been produced. As nickel is vitally needed for military as well as essential civilian products, it would seem desirable to substitute some other alloy for the 25 percent nickel alloy now being used. The Bureau of Mines has been asked to suggest possible substitutes.The substitute should have physical properties similar to the alloy now being used. It should have a metallic appearance and ring and a reasonably white color, so to be distinguishable from the present 1-cent coin. It should also be reasonably resistant to corrosion."

Workers at Anaconda Aluminum Company's primary aluminum smelter at Sebree, Ky., were back on the job December 1, after agreeing to a new 3-year contract. The old contract had expired November 15. On December 20, the Aluminum Company of America reportedly shut down a 42,000-ton-per-year potline at its Wenatchee, Wash., primary aluminum smelter and half of a 23,000-ton-per-year potline at its Vancouver, Wash., smelter as a result of a power cutback by the Bonneville Power Administration. A1 so in December, Kaiser Aluminum & Chemical Corp. reduced overall power at its Mead and Tacoma, Mash,, smelters and shut down one of eight 27,500-ton-per-year potlines at Mead. Reported shutdowns for the firms since September have amounted to 76,000 tons for Alcoa and 50,000 tons for Kaiser. Alcan Aluminum Ltd. announced plans to double the generating capacity at its Kemano hydroelectric power plant in British Columbia to 1,916 megawatts. The additional power would be equal to that required of a primary aluminum smelter with an annual capacity of 567,000 tons.

This paper briefly describes the progress in mine safety in the United States, with emphasis on recent events that have followed since the explosion at the Sago Mine in January 2006. Legislation following the mine accidents last year and areas of work carried out by the National Institute for Occupational Safety and Health (NIOSH) related to the accidents are discussed. American coal production in 2006 was 1,150 million short tons. Most of this coal is consumed domestically, with annual exports of coal about 50 million tons. One third of annual production is produced at surface mines and the rest from underground. This coal is produced by about 82,000 miners working at 1,400 coal mines. Major improvements in coal mine safety have been made in the US since the Federal Mine Safety and Health Act of 1977. The fatal injury rate in underground coal mines between 1977 and 2004 was reduced by 47.8% to 0.036. The annual number of fatalities dropped from 112 to 14 during that period. Likewise, the non-fatal days-lost injury rate was reduced by 42.6% from 10.87 to 6.24. The improvements in mine safety during this period can be seen in the graph of underground coal mine explosion fatalities and injuries from 1980 to present, which is shown in Figure 1. Between 1984 and 1993 (9 year period) there were 5 explosions and one major mine fire, for a total of 55 fatalities. Over the seven year period from 1993 to 1999 there were no fatalities from coal mine fires and explosions. However, in the six years since 2000 there have been 5 explosions and one fire, which have caused 37 fatalities. [ ]

A job analysis is a written list of the tasks associated with doing a particular job. In drilling, describing the steps associated with adding rod to the drill steel would be an example of a task associated with the job. How to hammer casing or fish a lost drill bit out of a hole would also be job tasks. Develop a written list of job tasks. Ask drillers in your company if the tasks listed are correct and accurately reflect how to perform the job. While each driller may do the job a little differently, some tried?and?true methods are required to perform certain tasks safely and correctly. You may find a separate analysis is needed for different drilling jobs (e.g., a residential well or a monitoring well). Also remember to include the tasks of both the helper and the driller.

Since the promulgation of the MINER Act and the follow-up changes to the regulations governing mine seal construction and maintenance, mine operators must be acutely aware of the atmosphere in sealed mine areas and prepared to deploy control technologies when the conditions are warranted. On Site Gas Systems1 in Newington, Connecticut was awarded a contract by the National Institute for Occupational Safety and Health (NIOSH) to construct a novel in-mine nitrogen (N2) generation plant using pressure swing adsorption (PSA) technology. PSA technology utilizes carbon molecular sieve material and pressure to adsorb oxygen (O2) molecules while allowing N2 molecules to pass through the sieve material. The prototype PSA unit was tested at the NIOSH Safety Research Coal Mine (SRCM) where a 62,000 ft3 area of the mine was rendered inert during a series of two tests. The resultant data were then used to construct a CFD simulation of both injection tests. In addition, O2 depletion and gas leakage rates were quantified in the model and were compared to actual values. Once the model was calibrated, the sealed mine area was doubled and simulations were made with various injection rates and injection site locations to determine the impact on the time needed to render this area inert. This paper presents an overview of the PSA plant, the details of the gas injection tests and CFD modeling work.

It is presented in a step-by-step "user?s manual" format. Appendixes contain the technical assumptions, program default values, and a summary printout detailing capital and operating costs and cash-flow analysis.

A knowledge of the mechanics of wave propagation through the earth is essential if reliable communication systems are to be designed for coal mine environments. To obtain a reliable basis from which accurate propagation predictions could be drawn, a series of theoretical Studies have been prepared for the Bureau of Mines by Mr. Ramsay Decker of Spectra Associates, Inc. through a subcontract with Collins Radio Group. These reports formulate the analytical techniques from which wee may derive frequencies and antenna configurations that yield the optimum signal-to-noise ratios over the greatest distances in mine environments. The reports of sections 2 and 4 deal with analytical techniques for determining the signal-to-noise ratio within a mine environment for loop-to-loop and line source-to-loop antenna configurations. The analytical results of these reports were then implemented into computer programs to obtain predictions concerning through-the-earth signal propagation. These preliminary predictions are based on noise data compiled by Bensema, Maxwell and Stone, and WVU prior to the extensive research conducted by the National bureau of Standards (NBS). These initial measurements indicated a 20- to 30-dB lower noise level than that measured recently by N BS. The recent NBS data is that used by Collins in the final predictions.

"Interest in the nitrogen content of steel has increased sharply in the last few years. Investigations have indicated that its presence in steel has an important bearing on high temperature brittleness3, magnetic properties4, room temperature hardening5, and shock resistance. The steel producer may therefore look forward with certaintly to the necessity of controlling this element in a considerable proportion of his product. The literature, however, reveals a great many contradictory statements concerning the percentage of nitrogen which must be present to produce given effects on the properties of iron and steel. Most of these arise from a misunderstanding of the nature of the iron-nitrogen diagram near the iron end and a lack of recognition of the effect of state of aggregation on physical properties although both have been fairly well worked out.My thanks are due to Mr. J. L. Gregg, Prof. R. H. Dowdell, Mr, R. F. Mehl and Mr. H. C. Vacher for suggestions in connection with the preparation of this paper, and to Mr. C. H. Hefty and Mr. G. T. Motok for furnishing a valuable bibliography and translations of articles by Must and Duhr.The iron-nitrogen diagram in the solid state, so far as it concerns the nitrogen content of commercial iron and steel, consists simply in a solubility curve increasing with temperature. The important thing about this solubility curve is that without cold wore the iron nitride when precipitated does not go into solution on heating for long periods below 300°C. This slowness of solution is probably due to the fact that iron nitride when present as a separate phase has at these temperatures an equilibrium pressure of several thousand atmospheres7 and that therefore each particle of iron nitride surrounds itself with a layer of nitrogen. The precipitation of such an unstable crystal presents points of interest thermodynamically but we can not go into these here. It is quite well established that it precipitate. It can not diffuse then unless this layer is broken through or the temperature and pressure are increased to a point where the nitrogen is absorbed at an appreciable rate in the surrounding iron. This rate will depend on the degree of unsaturation of the iron and the excess pressure of the nitrogen above equilibrium pressure. The degree of unsaturation is determined by the temperature alone so that the temperature at which the rate of solution of the nitride becomes appreciable will depend on the pressure of nitrogen developed. This, in turn, will depend on the balance between rate of decomposition of the nitride and the rate o: flow of the iron. In annealed iron, this temperature of appreciable solution rate is between 300 and 350°C.8 By cold work, however, the elastic limit of the iron is increased, and conditions for the nitride to go into solution are reached at a much lower temperature, giving rise to the so-called blue brittleness. This brittleness and hardening is due to iron nitride going into solid solution and is not precipitation hardening."

Introduction This paper is a descriptive analysis of injuries and fatalities at small surface bituminous coalmining companies in Pennsylvania. The purpose of analyzing a specific industry sector and location is to identify issues that might be missed in a wider analysis. Other studies of small mine safety (e.g., Peters and Fotta, 1994, Randolph and Boldt, 1997) have shown that the relationship between safety and mine size is not a simple one. Some injury types occur at higher rates at small mines, while others occur at lower rates. Consequently, an analysis limited to a specific type of mining can help identify hazards and issues specific to that sector. The results of this analysis will be useful for directing training, intervention, and research efforts towards the most prevalent causal factors and incident types. Procedures and Definitions Data sources The primary sources of data for this analysis were files provided by MSHA containing information collected under Title 30 Code of Federal Regulations, Part 50. These files, commonly referred to as “Part 50 data”, are provided though MSHA’s Program Evaluation and Information Resources directorate. Two primary types of injuries were selected from the Part 50data: Fatalities (MSHA “degree of injury” code 1) and lost-time injuries, otherwise known as nonfatal with days lost (MSHA “degree of injury” codes 2 through 5). Lost-time injuries involve either actual days away from work or days of restricted activity or both. Fatality and lost-time injury incidence rates were calculated using the standard MSHA formula: Number of injuries per 200,000 employee-hours. Additional information on company sizes and mine ownership was provided by the Pennsylvania State University College of Earth and Mineral Sciences. Time span A five-year time span was selected for this study. More than a single year of safety experience is usually needed to capture a broad range of incident types. Many types of injuries, especially fatal injuries, can be extremely rare, and may be missed if too short a time span is selected. However, a time span that extends too far back may begin to suffer historical threats to validity. That is, incidents that occurred several years ago may be caused by hazards that have since been remedied or may be related to mining practices and conditions that no longer exist. A five-year span offers a practical compromise between these two concerns. Consequently, the MSHA Part 50 data files from 1993 through 1997 were used. The 1997 file was in “preliminary” form at the time of the study, so small variations from the final 1997 “close out” file can be expected. Based on past experience, we do not expect the close-out variations to have a significant impact on the findings.
