Improving the safety culture and performance of any operation is a fundamental goal of every mine operator. Methods of implementing, or changing a culture have developed from the early methods of operant conditioning, for example rewarding crews for achieving nominated safety milestones i.e. 3 months Lost Time Injury (LTI) free through to the current model of engaging a workforce through education and empowerment. As a large scale and mature operation with multiple contractors, Telfer has been exposed to numerous programs throughout its history. This has provided the opportunity to build on and complement these systems by integrating key elements from each. SAFE 360° is a new approach developed to engage the workforce in a holistic approach to encourage positive behaviours across all facets of an individual’s life which results in a focus on situational awareness at all times, rather than being workplace specific. It is based on three key modules: • Emotional Intelligence • Mindfulness • Situational Awareness Each module has a series of modules that help enhance the individual’s sense of self, leading to an ability to self-reflect and self-manage whilst empowering them with the moral courage to challenge their peers to raise standards. Since commencing this SAFE 360° behavioural safety risk management journey in November 2017, we have witnessed a change in the way our workforce engages with each other, their workplace and with management. There has been an increase in hazard perception, identification and ownership of issues, and we have seen a significant decrease in site TRIFR rate from 13.7 to 5.7 (as of April 2019). This paper will provide an overview of the program, how it has been implemented, case studies to demonstrate outcomes and KPIs used to measure effectiveness.

Maintaining stable high walls can be a challenging task. Complex geological and structural features, alteration zones, rock mass properties, and hydrogeological conditions must be well understood to design a safe mining environment. Economic considerations must also be taken into account as the mine operator must provide ore deliveries and protect reserves. However, even when good engineering practices are followed, and proper blasting and mining techniques are implemented, slope movements may still occur. When these slope instabilities are minor, the mine is often able to continue operations with minimal impacts on safety or production. When these occurrences are excessive and coupled with high rates of movement, a robust monitoring system is crucial to continuing mining activities while protecting personnel. This paper summarizes the approach an open pit mine in Nevada took as excessive and rapid high-wall movement was experienced in an integral area. Creative mining approaches, combined with slope monitoring radar technology, were utilized to maintain safe and successful mining activities despite active movement that exceeded an average velocity of 30 inches per day.

A matrix analysis of the differences between the camps and communities associated with the Britannia Mine is used to develop an analytical tool. The tool is intended to assist the mining industry as it embraces the many facets of sustainable development. A case study is used to support the identification of key factors that may predict the sustainability of a new mine town. Sustainable communities, like Britannia Beach, are discussed in the context of three pillars of sustainability: value of mining products to society; creating wealth; and, environmental integrity. A sustainable mine town may be viewed as a socio-economic asset as well as an asset to the mining industry.

To better understand the load shedding and the stress transfer on coal pillars due to room-and- pillar retreat mining in deep cover panels, researchers from the National Institute for Occupational Safety and Health (NIOSH) conducted a monitoring field study. Two sites, at overburden depths of 1,000 ft (304.8 m) and 1,500 ft (457.3 m), were selected in a room-and-pillar mine operating in Southern West Virginia. The deformation and stress changes in the roof and two adjacent pillars at each site were monitored during the retreat mining process. The monitoring results and field observations were used to calibrate large-scale FLAC3D models for each site. The calibrated models were used to assess different sizes of roof bolts installed in deep cover areas. INTRODUCTION Deep cover room-and-pillar retreat mining is an important emerging issue that will intensify in the future as shallower reserves are depleted (Mark, 2009; Chase et al., 2002). During pillar retreat mining, the likelihood of local and/or global instabilities increases due to elevated stress levels near the pillar line because of the abutment loading. In the last decade, approximately 31% of the room-and-pillar mining groundfall fatalities in the United States occurred during pillar retreat mining. Between 2013 and 2017, this proportion increased to 55%, of which 50% of these fatalities were due to falls of roof and rib that occurred during pillar recovery (MSHA, 2018). One interesting piece of information about pillar recovery fatalities was that the victim was nearly always under bolted roof. Sheared and broken #5 (5/8-in), fully grouted rebar bolt failure contributed to three of four fatal roof fall incidents that have occurred in deep cover retreat mines (NIOSH, 2010). This claim raised the following question at the study mine: Is it safe to use a 4-ft long #5 (5/8-in) rebar bolt grade 60 at a depth of cover more than 1,000 ft (304.8 m)? There was some concern about shear failure of the #5 (5/8-in) rebar bolt at a depth of cover over 1,000 ft (304.8 m). To answer the question and to reduce accidents and fatalities caused by ground falls due to room-and-pillar retreat mining in deep cover panels, the National Institute for Occupational Safety and Health (NIOSH) monitored the roof and rib deformation and the change in vertical pressure within two adjacent pillars during pillar recovery. The study was conducted at two deep cover sites in a room-and-pillar mine located in Southern West Virginia that operates in the Lower War Eagle (LWE) seam. The monitored sites were selected at 1,000 ft (304.8 m) and 1,500 ft (457.3 m) depths of cover. In this paper, site-1 represents the monitored site at 1,000 ft (304.8 m) of cover, while site-2 represents the monitored site at 1,500 ft (457.3 m) of cover. For this study, large-scale FLAC3D numerical models were calibrated based on both field observations and instrumentation results. The calibrated models were then used to compare the ground response and the induced stresses of two pillars at instrumented sites 1 and 2. The maximum lateral displacement (roof shift) obtained from the calibrated large-scale FLAC3D models was applied to a small-scale FLAC3D bolted models, the safety factor for axial stresses of the simulated roof bolts was calculated at the deep cover sites.

THE first catalogue of Geological Reports of the United States was prepared by Prof. O. C. Marsh, and published in the American Journal of Science and Arts for 1867, vol. xliii, second series. Since then the list has more than doubled, and it was thought that the publication of a new catalogue, brought down to date, would be useful to many of the members of the Institute of Mining Engineers. The compiler will be most thankful to have any omissions or inaccuracies in the list sent to him, to be published as a supplement in the next volume of the Transactions. Especially to have the correct title, number of pages, plates, and maps of any volume on the list to which an * is prefixed, as he has been unable to personally consult such works. A preliminary list was issued last February, which has been almost doubled by the kindness of many friends. A powerful methane explosion in the mid-shaft pump station of a main service and ventilation shaft killed several persons on the station; more workers were killed when a mancage travelling down the shaft fell to the shaft bottom under a pile of broken concrete slabs from a brattice wall destroyed by the explosion venting into the shaft and a tangled mass of twisted steelwork from the equipped compartment. The newly-commissioned shaft serves a recent extension of an established mine on the Orange Free State (OFS) goldfield in South Africa. After outlining the circumstances in which the paper was written, and reviewing the nature and extent of the methane hazard in OFS mines, the paper describes the physical layout of the shaft and summarises evidence given by witnesses at the official inquiry concerning events prior to the explosion. No attempt is made to apportion blame for the incident.

The summit of L_ihi submarine volcano changed dramatically after seismic activity beneath L_ihi during the month of August 1996. Bathymetric re-surveys of the L_ihi edifice using SeaBeam, Simrad and deep-towed Reson-Focus multibeam systems showed the presence of new north-south striking fissures, normal faults and a new pit crater located at the site of a former 300-meter high volcanic cone. Comparisons between the pre-1996 SeaBeam data and the new bathymetry show a net loss of about 100 million cubic meters of rock volume on the summit. This loss is localized at a new pit crater -- the 300-meter deep by 1,300-meter wide Pele's Pit. Submersible observations with Pisces V show the northern edge of Pele's Pit to be bounded by a normal fault located north of the site of the former Pele's Vents cone that has disappeared into the new pit crater. A new hydrothermal field, the Ikaika Vents, extending from a water depth of 1,311 meters to 1,287 meters has formed at the northeastern edge of the floor of Pele's Pit. Submersible observations of these new Ikaika Vents during September 1996 showed vigorous venting at the site with extensive white-colored bacterial mats hanging off the rock face above the vents. Follow-up mapping and sampling of the vent sites during August 1997 showed vent temperatures of up to 198°C, dense populations of vent shrimps around the vents and a centimeters-thick veneer of fine-grained polymetallic sulfide mineralization along the rock walls above individual vents. The mineralogy of the sulfides shows a barite, chalcopyrite, sphalerite, and pyrite composition.

Since zinc was first deposited at Risdon in February, 1917, provision of the most economical cathode assembly has been a subject of extensive research, the major problems being corrosion and efficient use of power.From the corrosion viewpoint, a perfect cathode is one in which the life of the headbar and the deposition sheet are the same. However, corrosion usually concentrates in a narrow zone across the face- of the sheet, resulting in breakages which, before the advent of argon arc welding, meant a loss of a 'complete assembly.Attempts to extend sheet life included use of thicker sheets, multitudinous protective coatings applied with varying degrees of success, re-rolling, and for a time, aluminium consumption was reduced when broken sheets found re-use as protecting strips. This latter economy became impracticable when electrode spacing was reduced for greater efficiency in power utilization.As an integral part of the electrode, the headbar was modified accordingly. Copper headbars, although more corrosion resistant, were replaced by aluminium due to a possibility of dissolved, copper promoting deterioration.Thicker electrodes and regulated spacing reduced internal shorting, whilst provision of knife-edge type supports on both contact and insulator ends of headbars aided in electrode centralising and continuous electrical conduction.After much experimeIitation, an assembly has been evolved which, with the aid of argon arc welding and tbe use of minimum additional aluminium, permits comparable headbar and sheet lives, combined with good electrical conduction throughout the life of the cathode.INTRODUCTION Over 300 tons of zinc are produced daily at Risdon from a purified zinc sulphate solution by electrolysis, the zinc being deposited upon all-aluminium (except for copper contact) cathodes. It is stripped in 72-hour cycles from the cathodes, melted and cast into the customary rectangular slabs.

"In 2017, one of the international authorities on coal bursts, Dr. Chris Mark, published a paper entitled “Coal Bursts that Occur during Development: A Rock Mechanics Enigma,” in which several relevant technical issues were identified, the most pertinent being:i.whilst development coal bursts are commonly associated with geological faults, understanding which specific faults result in burst-prone development mining conditions and why, remains undefined.ii.conventional wisdom in relation to strong roof and floor geology of the coal seam might be limiting, based on certain burst examples in Colorado.iii. development coal bursts can occur without the local ground stresses being substantially elevated from their in situ levels.This paper outlines what is considered to be a credible, first-principles, mechanistic explanation for these three current development coal burst conundrums by reference to early published coal testing work examining the significance of a lack of “constraint” to coal stability and an understanding of how very specific structural geology and other geological features can logically cause this to occur in situ, albeit on a statistically very rare basis. This basic model is examined by reference to published information pertaining to the development coal-burst that occurred at the Austar Coal Mine in New South Wales, Australia, in 2014 and from the Sunnyside District in Utah, USA.The “cause and effect” model for development coal bursts presented also offers a meaningful explanation for the statistical improbability for what are nonetheless potentially highly-destructive events, being able to explain the statistical rarity being just as important to the credibility of the model as explaining the local conditions associated with burst events.The model could also form the basis for a robust, risk-based approach utilising a “hierarchy of controls” to the operational management of the development coal burst threat. Specifically, these include the use of pre-mining predictions for likely burst-prone and non-burst-prone areas, the use of the mine layout to avoid or at least minimise mining within burst-prone areas if appropriate, and finally the development of an operational Trigger Action Response Plan that reduces the likelihood of inadvertent roadway development into a burst-prone area without suitable safety controls already being in place."

The refractory nature of residual gold in Kalgoorlie calcines is caused by occlusion in dense pyrrhotite formed by fusion within pyrite grains during decomposition. Fusion of pyrite increases as the rate of heating exceeds the rate of sulphur expulsion. By controlling these rates in relation to each other in a fluidised system fusion is limited and an expanded structure is formed. The increased area presented to cyanide solution, and the decreased diffusion distance, makes gold more accessible for extraction. Gold extraction from expanded pyrrhotite, before oxidation for porosity, is comparable with extraction from fully oxidised calcines from Edwards roasters. A method is suggested for standardised comparison of cyanidation residues.

The stabilities of rehabilitated landforms are often threatened by the altered surface hydrology, especially the excessive surface run-off, which may cause severe water erosion and associated pollutant transport to surrounding areas. These effects would lead to pollution in waterways and render the failure of newly established vegetation. An accurate quantitative evaluation of run-off problems are consequently essential for the proper rehabilitation designs which can help minimise these harmful consequences. For this purpose, in this study a novel run-off model, RunCA, was developed based on Cellular Automata (CA), integrating fundamental hydraulic principles. In this model, a series of rectangular cells were employed to cover the study area. Run-off production within each cell was simulated by determining the cell state (water surface elevation) that consists of both cell altitude and water depth. The distribution of water flow among cells was determined by applying CA transition rules based on the improved minimisation-of-difference algorithm and the calculated flow travelling time. RunCA was applied to various aspects in mined land rehabilitation planning, including landform designs for both hill-slopes and tailings dams, revegetation strategies and cover constructions. Simulation results showed that surface run-off increased with slope gradient and more run-off was produced on convex and straight slopes than on concave and S-shape slopes. Both mounds and rip lines constructed on the tailings dam surface could help reduce the water discharge. Vegetation planted in rows were more effective than those in columns or patches in controlling run-off. Increased cover thickness had the potential in reducing run-off rates, and the covers with an increasing thickness from top to bottom of the slope tended to be more efficient than those with a constant thickness. All these results have demonstrated the efficacy of RunCA as a simulation tool in assessing the hydrologic behaviours of different rehabilitation designs. Compared to the traditional hydrologic models, most of which were developed for the catchment scale studies, RunCA has the potential advantages in capturing the dynamic and spatially varied run-off behaviours at different spatial and temporal scales.CITATION:Shao, Q,?Baumgartl, T, Huang, L and Weatherley, D, 2014. A two dimensional cellular automaton (CA) model coupled with finite element (FE) method has been developed for simulating the formation of solidified microstructure during beam blank continuous casting, which represents columnar and equiaxed dendrites growth in the case of detailed secondary cooling boundary conditions. In this model, a new adaptive mesh division is proposed to couple CA cubic lattice and irregular FE mesh of beam blank. A double-accuracy liner interpolation method is employed to obtain micro CA temperature field via macro FE calculation. The nucleation process and the velocity of dendrite tip are calculated by continuous nucleation model and KGT model, respectively. Moreover, the preferred orientation of dendrite growth is determined using a probabilistic model. The effects of super heat and casting speed on the microstructure characteristics are analyzed. This model can be applied to optimize processing parameters for beam blank continuous casting basing on microstructure evaluation.

A HISTORY of the ore-mining and iron-smelting industry of the Adirondacks comprises a century and a half of pioneering by rugged individualists, both men and women. By geographical location, the climate is most severe-long, cold winters with heavy snowfall, and short seasons of vegetation growth; this, together with its remote location from large centers of population, adds considerably to the difficulties of conducting mining operations in this area.

Smelting of nickel-copper sulfide ores at Vale Inco’s Sudbury operations commenced in 1888 and the first converting plant went into operation in 1892. As the demand for nickel grew, the smelting operations expanded and by the 1960’s the Copper Cliff Smelter featured the longest converter aisle in the world with space for nineteen Peirce-Smith converters. During this long period, different converting techniques and vessels were employed. The evolution of converting is reviewed with special emphasis on the modifications made to the Peirce-Smith vessel to accommodate the metallurgical and process needs of the changing times.

Our purpose is to record and to explain the widely changing numbers of the huge total of black workers on the gold mines of South Africa over the century from 1896 to 1996. These men were recruited from up to twenty territories, and their contributions are examined. The major areas providing labour over this long period have been Lesotho, the former Transkei within South Africa and Mozambique. From 1989 to 1996, for instance, these three areas accounted for over sixty per cent of the total labour force (tlf). Our principal Figure 1 shows the tlf fluctuating through time to reach a peak of 480 000 in 1988. Irregular falls are also documented with the most serious in 1995 to 180 000. Based on this graph we consider the political, economic, sociological and historical reasons behind these changing employment opportunities. These have included external world-scale events such as war or depression, as well as internal changes of national economy and policy. We show the importance for numbers employed of the discovery of gold in the Orange Free State in 1946, the influence generally of the ?Winds of Change? and the firm maintenance by the State of law and order. Varying gold production costs and profitability against high tax rates, allied with increasing wages and a lack of development capital, have led in the last decade to employment falling by fifty per cent, despite an actually higher value of gold output. Finally, this analysis considers the role of the migrant labour system as it affects, and is affected by, conditions in the gold mining industry, with special reference to its less than humane consequences in the numerically dominant territories of Lesotho and Transkei.

GENTLEMEN : If my first words were other than those of thanks for the high honor of being called to preside over the American Institute of Mining Engineers, I should do injustice alike to you and to my own sense of the obligation, under which I have been placed by your voluntary choice. When the position was first suggested to me, I resolutely declined to allow the use of my name, on the ground that, by training and occupation, I was not entitled to this honor, which, it then seemed to me, should be conferred only on a professional engineer. In fact, the history of the Institute, which had honored itself, as well as my distinguished predecessors, David Thomas, Rossiter W. Raymond, and Alexander L. Holley, by elevating them to its highest office, seemed to indicate the line and the limit of safe precedent, in which I did not then see how I could be included. But my scruples were finally removed by the consideration that, as in the course of human industry, the pioneer, of whom Mr. Thomas was so marked a type, must precede the mining engineer, so fitly personified in Dr. Raymond, and be followed by the mechanical engineer, whose incarnation we behold in Mr. Holley; so all these must be supplemented by the man of affairs and finance, in order that "enterprises of great pith and moment" may be prosecuted to a successful issue. I felt, therefore, that I had no longer any right to resist your preference, or to deprive myself of what I must regard as the crowning honor of an active career. On an occasion like the present it seems appropriate to review the history in this country, during the last hundred years, of those industries which the Institute of Mining Engineers specially represents. There is much in the story with which other members of this body are more familiarly acquainted than myself; yet I fancy that to all of them, and not less to our distinguished and most heartily welcome guests from abroad, a comprehensive outline of the progress of mining and metallurgy in the United States will prove interesting. I ask your attention, therefore, to a condensed account of that subject, embracing a statement of the beginnings of the industry on our shores, of the notable events attending its increase, and of its present
