This report, sponsored by the Bureau of Mines, presents the results of an examination of the safety of the Blind Shaft Borer (BSB) shaft sinking system and an alternative modified BSB design concept in comparison to conventional shaft sinking technology. Detailed study of the two systems was also performed to identify and recommend design changes for optimum safety conditions. A comparison of the BSB systems with conventional shaft sinking was made an the basis of relative risk levels. Risk incorporates both the frequency and the potential consequence of accidents. A safety evaluation of the component operations of the two BSB systems was performed using a Failure Modes and Effects Analysis (FMEA) methodology. The results of this analysis show generally that both BSB systems are safer than conventional shaft sinking. The potential risk of conventional shaft sinking operations could be reduced by over 30%, on the average, with the use of the original BSB system, and by an average of about 10% with the modified BSB. Design change recommendations to further improve the safety of these systems include a better fire suppression system, improved man haulage systems and more compact hydraulics. An operational safety manual was also developed for the modified BSB system.

Extensive thick lignite apparently lies within 250 feet of the land surface in Dyer, Lake, Lauderdale, Obion and Tipton counties. Information on geology, land use, soils, groundwater and instability is compiled, along with a bibliography. A test and monitoring site was investigated at Fort Pillow Prison in Lauderdale County. Analyses were made of lignite, overburden, associated water and leachate. Lignite has high BTU (about 7800) and low sulfur (1.1% avg.). A year or more of data are presented on rainfall, water levels, and chemistry of surface water, groundwater, leachate and water from test pits. Main potential environmental problems are quality of surface water, and possibly shallow groundwater. The main aquifer of the region will likely be un-affected by mining. Low transmissibility of shallow aquifers will minimize lowering water levels of wells. Soil reclamation and conservation should be readily accomplished, Dangerous earthquakes have a low probability and mines are not significantly more hazardous than steep bluffs.

The STATE MINERAL SUMMARIES publication provides estimated data and summaries of mineral activities at the State level for 1993. Most of the estimates are based on nine months data. Individual State summaries are published separately as State Mineral Industry Surveys and copies can be obtained from the appropriate State mineral officer. In an effort to facilitate the rapid dissemination of information and publications produced by the U.S. Bureau of Mine, an electronic document dissemination system is now available for use. MINES FaxBack is a simple-to-operate automated fax response system that operates 24 hours a day, seven days a week. All a user needs is access to a fax machine with a touch-tone telephone. Many aspects of water use in mining are driven by the technical requirements for water. There has been little consideration given to both the safety and the health requirements. Workers in the mining industry are exposed to water from a range of treated and untreated sources. The intended use of this water dictates the required quality of the water, sometimes with limited consideration for contact with miners. Particularly for underground workers, there is exposure while working in and walking through groundwater possibly contaminated by hazardous substances. Where controls are inadequate the outcomes of this exposure can result in disease, illness and injury û the costs of which can be significant. The situation could also arise where, due to the lack of human exposure, the controls are unwarranted and the financial cost unjustified from a health perspective but warranted from an equipment management standpoint. In addition to the health aspects, water can impact on a range of safety issues resulting from high pressure usage, dust suppression, fire fighting and other processes. The protocols and procedures in place need to consider the safety issues in addition to managing water usage. The current state of water management in mines from an occupational health and safety perspective has been ascertained from a survey targeting underground and surface coal and metalliferous mines. This survey considered the health and safety issues associated with water use, the types of surveillance, control protocols and the strategies that the mines have in place to manage the health and safety risks associated with water systems. The results assisted in the identification of potential occupational health and safety risks associated with water systems and the management processes that may be appropriate from a risk management perspective. Health and safety issues are often neglected but remain critical issues affecting the value of water to the mining industry.

"The execution of classical building services in underground mining operations is more truth than myth. From small concrete foundations for conveyor belts to the construction of huge workshops for large machinery maintenance, nearly every operation requires building services. By creating such fixed facilities, these services ensure the efficiency of the production. A distinction must be made between fixed facilities which directly enable the production and auxiliary facilities which only indirectly support the production. The construction costs of such auxiliary facilities are often roughly estimated because they do not immediately contribute to mineral extraction. The research project ‘Development of a systematic cost-calculation method for building services in underground mines’ seeks to address the question: ‘Can the construction costs concerning fixed auxiliary facilities be neglected e.g. within a prefeasibility study?’ The presentation will show the monetary relevance and significance of these auxiliary facilities and introduce a guideline for cost estimators. PROBLEM DEFINITION The overriding interest of an underground mining operator is to exploit the commodity with the maximal quality by minimum cost and time expenditure. If a new operation shall be installed, it has to promise profitability. Therefore, predictive economic feasibility studies are performed in advance of installation. Herein, all cost expenditure for installation and operation of the future underground mine is determined and compared to the expected income by selling the raw material. The described interest of the mining operator is the starting point of the research project ‘Development of a systematic cost-calculation method for building services in underground mines’. While economic feasibility studies for future operations are focused on the profitable exploitation of raw materials and the efficiency of the accompanying necessary production processes, auxiliary actions are only considered rudimentary. Necessary production processes, as well as auxiliary actions regularly require classical building services, which are normally performed in construction business on the surface in the same manner. By performing building services like masonry or concrete works in underground mines, fixed facilities shall be set up in order to enable or support the raw material production. Production enabling fixed facilities are varying from small concrete foundations for conveyor belts or other infrastructural elements to upstream processing plants situated in the mine workings. While such fixed facilities are essential parts of the production process chain, supporting facilities underground shall increase the efficiency of the mining operation. Workshops, control rooms and social installations are only a few examples for supporting facilities. Those could be situated on the surface as well, but the efficiency increases significantly by installing them underground. Especially supporting facilities are evading from the main interest of the mine operator, since they are only indirectly connected to the production process. The building costs are oftentimes only roughly assumed."

There is increasing demand for geotechnical contractors to commit to carbon reduction, net zero or science based targets. A lot of focus has therefore been placed on using low carbon materials and low emission rigs. However, whilst these are ways to help reduce carbon, there is a need to look beyond just substituting materials and fuels to meet net zero. Using the IEMA carbon hierarchy, there is a need to first look at eliminating and reducing emissions, before considering substitute materials. For example, in geotechnical designs, this may mean considering using ground improvement or reducing the number of piles, before considering using low carbon cements. Equally, for remaining emissions, this hierarchy means considering carbon negative products to offset emissions, such as ground source heat pumps. This hierarchy can also be applied throughout geotechnical contractor organisations. For example, considering video conferencing for meetings, before considering public transport or using an electric vehicle. This article explores the carbon hierarchy and how it can be applied to all areas of a geotechnical contractor’s business. Only through embedding this hierarchy across the business can geotechnical contractors achieve the step change required to meet net zero.

This Bureau of Mines report describes the history, characteristics, distribution, and mineral development potential of 21 lode gold deposits in or near the Chugach National Forest (CNF), AK. It includes findings from a 4-yr (1979 to 1982) mineral evaluation of the CNF conducted by the Bureau and the U.S. Geological Survey (USGS). This evaluation, undertaken to provide the U.S. Forest Service with mineral resource data for use in making decisions on land use, included site-specific mapping and sampling of more than 200-lode gold deposits. The data presented here were compiled from the findings of this 4-yr evaluation and from existing literature. Fourteen of the 21 lode gold deposits described in this report contain a combined identified resource of 117,750 short tons (st) averaging 0.55 oz/st Au and 0.2 oz/st Ag. Two mineralized felsic dikes in the Summit Lake-Palmer Creek area contain a combined identified resource of 18 million st grading 0.02 oz/st Au and 0.16 oz/st Ag. Identified resources were not determined for the remaining five deposits because of a lack of information.

ABSTRACT This study analyzed the publicly available health and safety policies of 26 mining companies to determine the quantity of health and safety (H&S) practices that mining companies encourage in relation to the Plan-Do-Check-Act cycle. A thematic content analysis of the policies identified elements and practices within the text. On average, companies communicated information on about 7 elements (range 1–14, standard deviation = 3.49) and discussed 15 practices (range 2–34, standard deviation = 9.13). The elements in which companies highlighted the most practices were risk management, emergency management, leadership development, and occupational health. A discussion of the policy trends shows areas that mine sites can improve upon within their Plan-Do-Check-Act cycle, in addition to encouraging the use of both leading and lagging indicators to manage health and safety performance. INTRODUCTION Mining companies and their employees are expected to identify and manage risks at their worksites to ensure the health and safety (H&S) of everyone at the site. The primary mechanism in place to control these risks is a company’s health and safety management system (HSMS) (Boyle, 2012). Broadly, an HSMS is a set of standard, interrelated, and interacting elements used to promote and achieve occupational H&S goals (ANSI/AIHA Z-10, 2012; BS OHSAS 18001, 2007). The practices conveyed within an HSMS are expected to minimize incidents, injuries, illnesses, and even save worker lives (Alsop and LeCouteur, 1999; Arocena and Núñez, 2010). In the United States, both the Occupational Safety and Health Administration (OSHA) and Mine Safety and Health Administration (MSHA) regulate and encourage aspects of an HSMS (US Federal Register, 2011, 2010). Many HSMS documents (i.e., Chemical Industries Association, 1995; Health and Safety Executive [HSE], 1995; International Labour Office [ILO], 2001) attribute the source of their basic management system to Deming’s Plan-Do-Check-Act (PDCA) model of continuous quality improvement and organize their practices within this cycle (Johnson, 2002). The PDCA cycle is a well-adopted approach in health and safety management and promotes continuous learning and adaptability (Robson et al., 2007)."

Researchers at the U.S. Bureau of Mines collected and analyzed overcore measurements and other indicators of in situ stress characteristics at the Lucky Friday Mine, Mullan, ID. An analysis of these data revealed that significant local variations in in situ stress are present in this mine and that these variations existed prior to mining. High stresses were found to be associated with the most competent strata and a locked section of a fault. An estimate of the predominant stress field was developed for mine-wide modeling. The estimate shows that the greatest principal stress was oriented north of northwest. This information should provide a basis for mine-wide structural modeling of the Lucky Friday Mine and should generate important insights into localized stress conditions, which may significantly affect both local and mine-to-mine variations in seismicity induced by excavation.

A series of controlled experiments was conducted at four surface coal mines to study the effects of certain blast parameters upon the ground and air vibration. Preliminary scale-model blasts were conducted at a quarry to provide basic data. It was found that the amplitudes of the ground and air vibration were controlled by the maximum explosive weight per delay. Either cube root or square root scaling could be used for the seismic amplitudes, and cube root scaling could be used for the overpressure amplitudes. Both amplitudes were not significantly affected by the total charge weight per blast, the attitude of the boreholes, or the charge length or diameter. The overpressure amplitudes were affected by the charge depth, the use of cover over surface detonating cord, and the use of adequate stemming. The length of the delay intervals and the spacing of holes were important in determining the potential for reinforcement of the amplitudes in certain directions from the blasts. In downwind directions from the blast, the attenuation of the overpressure with distance decreases. Surface terrain did not significantly affect the overpressure amplitudes within the range investigated. The type of overburden affects the generation of both amplitudes and the propagation of the seismic amplitudes. Spectral analysis of the ground and air vibration is reported. A bibliography of pertinent literature is included as an appendix. A scaled distance of 300 ft/lb1/3 should produce a peak particle velocity of less than 1 ips and an overpressure of less than 128 dB linear peak. For square root scaling, a scaled distance of 75 ft/lbl/2 should limit the ground vibration to less than 1 ips.

"Abstract - Shallow submarine hydrothermal fluids from Paleohori Bay off the Greek Island of Milos have been investigated in detail. The extensive gas and warm (max. 93°C) water vents are enriched in major components such as C1-, Na+, Si, Ca2+, Mn, PO4 3-, NH4 +, K+, Sr2+, H2S and CH4 and depleted in Mg2+, O2 and SO4 2- relative to ambient seawater. Based on zero-Mg calculations, it is deduced that the submarine vent fluids are related to the geothermal reservoir beneath Milos which has a similar chemical composition. Fluid samples from each vent in Paleohori Bay represent a mixture of hydrothermal end-member fluids derived from the upper and deeper geothermal reservoir systems of Milos, a mantle-derived source, a high-chloride fluid resulting from phase separation of geo thermal fluids and a steam-heated CO2-rich meteoric water. Résumé - À l’île de Milos dans la mer d’Égée, des fluides sous-marins hydrothermaux en eau profonde ont été récherchés en détail près de la baie de Paleohori. Les larges émanations de gaz et d’eau chaude (temperature maximale: 93°C) sont enrichies en constituants principaux tels que de Cl-, Na+, Si, Ca2+, Mn, PO4 3-, NH4+, K+, Si, Sr2+, H2S et CH4 et appauvries en Mg2+, O2, SO4 2- comparé à l’eau de mer environnante. Adequate planning for the development, operation, and reclamation of a surface mine requires attention to present and future uses of land, both on the mine site and in the mine vicinity. This paper outlines land use implications of surface mining, and suggests procedures that could link mine planning, mine operation, and reclamation to more general land use concerns. Mining is viewed as a temporary use of land that may irretrievably commit land resources away from some future activities, but may allow other uses, some of which may have been impossible without mining. The paper accepts the premise that surface mining should attempt to minimize environ-mental damage and to maximize future alternative uses of land by appropriate reclamation. I. This paper presents a case study where Cenozoic age detritals and an ancient buried landslide have been modelled in the final walls of an iron ore pit. Orebody 25 is located in Western Australia’s Pilbara region and is an operating pit where a redesign of the final wall cutback was required. The existing slope design utilised a simplistic geological model of mixed conglomeratic, calcretic and clay –dominated detritals overlying deformed bedrock sequences. This simplicity resulted in a conservative design that was not reflective of the slopes performance to that point. Further field investigation was carried out with the results assessed in conjunction with historic drilling, mapping and laboratory testing. A detailed geotechnical model was developed including three conglomerate and two clay dominated units and laterstage calcrete overprinting. The clay units were of significant importance to the stability assessment and resulting slope design due to the thickness and poor strength conditions indicated by the laboratory and field testing. However, the drilling investigation shed only limited understanding as to the origin of the clays and thereby how they should be treated as a component in the stability analysis. A comparison of the ratio of various geochemical elements measured by assay was made between the clays and 1) shale bedrock and 2) clays of known detrital origin from other nearby sites. This indicated the clays had a strong affinity with weathered bedrock. This and other textural features resulted in the interpretation that the clay units were relic debris preserved following ancient landslides in bedrock shale. Using laboratory testing data and calibration of results for a landslide origin, stability analyses were run, resulting in a steeper slope design being implemented. Excavation of the final cutback is now in progress with the model confirmed as appropriate.

Loudoun Water is currently implementing a quarry to reservoir conversion that will improve water supply reliability in periods of drought and flexibility in water supply operations when turbidity in its Potomac River supplies are high. The Milestone Reservoir Project consists of an inter-tie to the existing Potomac River Pipeline; new pipeline and tunnel; and a pumped intake and outlet works. Planned construction involves significant risks due to underground conditions which includes hard, igneous rock and high groundwater at the creek crossing. In addition, the site is constrained by Goose Creek, an adjacent residence, active mining in a quarry next to the new reservoir, public use of adjacent park space, and the Washington and Old Dominion Trail. Working with these constraints will require close coordination with active mining operations and public and private property owners. Safety of personnel involved with construction also represents a concern with quarry high walls over 30 meters in height near the planned construction. In consideration of key projects risks, Loudoun Water determined that an innovative CMAR delivery approach provided the best opportunity to achieve its goals in quality, innovation, front-end cost and schedule certainty, and operational flexibility and reliability. What matters most to Loudoun Water is the value in having a builder participate in the design process where key decisions can be made based on consideration of risks, cost, schedule, quality, and operability and maintainability of the system. While a progressive design-build delivery model may have produced similar outcomes, Loudoun Water had already invested in substantial development of the design for the work, and ultimately, CMAR provided Loudoun Water the opportunity to retain the partnership with the design team while gaining valuable input from a contractor. Among the challenges with CMAR in the Commonwealth of Virginia is a substantial limitation on self-performance corresponding to 10 percent of the construction value of the work. A significant industry outreach program was initiated by Loudoun Water to encourage participation with incentives in contract terms that allowed for the CMAR Contractor to make a reasonable profit. This paper reviews the CMAR procurement process and discusses contract incentives that were adopted to promote the work within the constraints of contract law in Virginia.

"Design for reconstruction of the East Flagstaff traffic interchange project located in Flagstaff, Arizona posed particular challenges. The proposed design included construction of a nearly 50-foot tall mechanically stabilized earth (MSE) wall given rightof- way constraints and soft subgrade soils. The tight right-of-way, approximately 15 feet in certain locations, adjacent to existing structures limited the number of viable remedial alternatives to provide grade separation at the proposed bridge abutment without significant settlement. Preliminary analyses indicated the existing subgrade soils - consisting primarily of relatively soft silts and clays with decomposed pumice - would settle approximately 12 to 14 inches resulting in unacceptable wall deformation. Stone columns placed via vibratory methods (vibro-replacement) consisting of circular excavations which penetrate to firm bearing soils and which are filled with crushed stone were selected to densify the existing subgrade soils. The implemented mitigation measures helped limit settlement to less than one inch and allow for successful construction of the project. The project history - including site constraints, subsurface conditions, evaluation of remedial alternatives, stone columns methodology, and effectiveness of ground improvements - will be discussed.IntroductionThis paper describes the subsurface investigation performed for the reconstruction of the Business Route 40 (B-40) (Country Club Drive) and United States (US) 89 Traffic Interchange (TI) located within the city limits of Flagstaff, Arizona. This project is currently referred to as the East Flagstaff TI. The Project Site Vicinity Map is shown below.The former B-40/US 89 TI was considered ineffective in handling the current and projected volumes of traffic which transition from northbound (NB) B-40 to US 89 (both directions), as well as from southbound (SB) US 89 to SB B-40. The intent of the project was to reconstruct the TI to better accommodate projected traffic volumes. The project eliminated the existing ramps from both NB B-40 to SB and NB US 89. The new interchange includes a Tee intersection from B-40 to US 89. In order to accommodate this alignment, the profile of US 89 was raised several feet (on nearly the same as existing horizontal alignment) to match the elevated profile of B-40. B-40 was shifted just west of its current alignment to accommodate construction of a new, wider bridge structure which spans over Industrial Drive, the Burlington Northern Santa Fe (BNSF) railway, and old Route 66. South of the new bridge, B-40 transitioned back to the existing alignment just north of the I-40/B-40 TI. Within the project limits, both US 89 and B-40 were widened to three travel lanes in each direction."

The revitalization of Pittsburgh’s historic Strip District has led to the redevelopment of existing properties and the development of new properties. At one time, the Strip was home to wholesale market products. Now, the historical warehouse buildings are being converted to multi-use residential and commercial properties. One such property, 1600 Smallman Street, consisted of 1 to 6-story connected buildings that housed night clubs, retail shops, and office space. The lower story portion of the building, on the southwest side of the property, was torn down to make room for a new 5-story- parking garage with street level retail space. The remaining 6-story-building, on the northeast, was gutted to be renovated into high-end condominiums, keeping the exterior historical building facade.

This paper presents a case history for a unique application of a system of vertical micropiles with horizontal soil nail bracing and shotcrete facing for underpinning. The system was used to support and protect an existing 2-story hospital building during demolition and renovation. The goal was to limit building movements and have the building remain in use during construction of an abutting addition. Soils at the site consisted of clean, loose to dense, sand and gravel. The original underpinning design was revised after the initial excavation revealed unsuitable soil conditions for traditional underpinning pits. An alternative system consisting of vertical micropiles (hollow bars) through the footing, combined with shotcrete and soil nails was proposed, accepted by the owner, and successfully constructed. Excavation depths were up to 15 feet (4.6 m) below the existing footings. Wall movements were monitored at regular, horizontal and vertical intervals along the hospital wall, and less than 0.6 inches (15 mm) of settlement and 0.1 inches (3 mm) of horizontal displacement throughout underpinning and construction were reported. The challenges with the original proposed support of excavation design and other alternative approaches considered will also be discussed.

This general report considers what promoters of deep basement projects want from their teams and whether they get it often enough. Most large projects are based on complex financial models where the funding is related to a future income stream. Project delays and cost over-runs can critically affect the financial model. Too often, delays and cost over-runs originate in the substructure and foundations. We must take positive steps to improve our industry's reputation for reliability.
