Near misses can enhance awareness of the potential causes of injury and prompt safety management initiatives. Although most companies require near-miss reporting, the value of these reports and how they influence subsequent actions or controls to reduce on-the-job risks are unclear. Researchers at the U.S. National Institute for Occupational Safety and Health (NIOSH) conducted a case study with an aggregates company in which near-miss reports were analyzed at each of their locations over an entire quarter during the summer of 2018. Within that quarter, workers recorded 249 near misses. The analysis provides insight into ways that risk-communication and management programs can be improved to reengage workers and their situational awareness on the job.

As an advanced method of rock engineering construction safety monitoring, microseismic monitoring technology can monitor the micro-fracture of rock masses within an effective range, and then perform forecast analysis of rockburst, cracking, falling blocks and other disasters. Although the application of microseismic monitoring technology has become more mature, how to improve its application efficiency remains to be further studied. At present, artificial intelligence has been extensively developed, and the use of intelligent algorithms to improve the efficiency of rockburst forecast can provide a valuable reference for the safe construction of underground engineering. To this end, this paper proposes a microseismic multi-parameter rockburst early warning model based on dynamic Bayesian networks. First, the sensitivity of microseismic source parameters to rockburst forecast is analyzed based on microseismic monitoring data and actual rockburst information of a hydropower station. Second, the main seismic source parameters and their data are selected and imported into a dynamic Bayesian network for learning to obtain the rockburst forecast model. The model is confirmed in its self-validation and K times cross-validation, and the receiver operating characteristic curve of the model is also analyzed, it is found that the area under the curve is close to 1, indicating that the model has good accuracy. Finally, the analysis of the influence strength of the parent and child nodes in the model reveals that the moment magnitude and source scale are the most sensitive parameters, which can be used as an important reference for rockburst forecast.

"Cyanide heap leaching of gold ores in areas with extremely low winter temperatures requires special considerations to prevent freezing of the leach solution. Factors that impact the solution temperature include ore temperature at the time of stacking and during operation, burial of drip irrigation lines, use of geomembrane covers and heating of the leach solution. Thermodynamic modeling was performed to simulate expected ore and solution temperatures under different operating strategies for Kaminak Gold’s Coffee gold project in the Yukon Territory. Mean monthly temperatures at the site ranged from a low of -22.5 °C (-8.5 °F) to a high of 13.6 °C (56.4 °F), while in situ ore temperatures ranged from -0.5° to about -1.5 °C (31 ° to 29 °F). The modeling results were used to optimize the operational parameters and indicated that a combination of drip line burial and heating of the leach solution during the winter months was required to maintain solution temperatures above freezing during the initial years of stacking, although as the size of the heap increased, heating was no longer necessary.Modeling objectivesThe successful operation of a cyanide heap leach under extremely cold winter temperatures requires the implementation of operational strategies to prevent freezing of the leach solution and the ore within the heap. This paper presents an algorithm for optimizing the classification of surface mine material subject to excavating constraints. High-resolution expected profit models are input and optimized to classification maps subject to site-specific rectangular excavating constraints. This optimization problem defies traditional closed-form analytical solutions; a practical heuristic algorithm has been developed to quickly determine the optimal final destination for the material subject to realistic constraints. The optimization is fast and generates results that achieve up to 98–99% of the total expected profit achieved with free selection. This algorithm provides a fast viable option for practical application in short-term grade control and in managing multiple realizations in long-term resource estimation.

Understanding relationships between mineralogy, texture, microstructural features, and element distribution and residence in minerals, is critical for accurate modeling of mineral weathering in waste-rock piles. The morphology of minerals and deformation structures, such as veins and faults, are important factors that control: (1) fluid access to the mineral surface to initiate dissolution, (2) the distribution of trace elements, and (3) formation of secondary minerals. We characterized the mineralogy, textures, and chemical properties of waste-rock material at three sites at scales ranging from outcrop to micrometer scale. A comprehensive hand-sample to thin-section scale methodology utilizing light microscopy, scanning electron microscopy, electron microprobe, and laser ablation ICP-MS was used to thoroughly characterize the waste-rock material.

Compagnie Miniere Esperance (CME), a mining company based in French Guyana, operates a gold primary ore body in open pit. The ore is processed through crushing, grinding and gravity separation. Tailings are stored in a ?fine? pond. Analyses of the fine ponds proved a residual gold grade sufficient for a chemical treatment. However, direct cyanidation would require a too large installation, regarding the local conditions (accessibility, supplying and rainforest meteorological conditions). It has been then decided to test the feasibility of a pre-concentration using flotation. Lab tests have been carried out by SGS Lakefield (Canada) in March 2007, which proves the technical feasibility. Therefore, CME asked Caspeo to: ? Carry out an on-site flotation pilot campaign. ? Design an industrial flotation circuit. The tailings reprocessing will contribute to a better tailings management and a dry up of the gold resource, limiting the risk of illegal mining after remediation of the CME site. CME MINE The Esperance gold mine is located in French Guyana, in the vicinity of several gold deposits as Omaï, Rosebel or Camp Caïman, holded by Iamgold and Newmont. These main gold deposits are located on the figure 1. Esperance was put into operation in the early 90?s. The first years were devoted to the mining of alluvial gold in the gravel of the Esperance Creek. Since then, operation was led in the primary gold deposit. Annual production is on average 254kg for a total production in fifteen years of 4300kg. Geology Esperance deposit is located in the greenstone belt of the Guyana Shield. Mineralization is in tectonic contact between sedimentary basin of Maroni and a granitic massif. Clastic series, composed of conglomerate and lamina sandstone, overlie a volcano-sedimentary basement (series called "Paramaca"). The area is strongly folded by shear movements, resulting from granite deposit. Gold is in interlacing quartz veins intersecting the clastic series. Gold is present in association with pyrite. Other elements, as silver or arsenic, are in negligible quantity. Exploitation The Maroni River is the only access to the Esperance mine. Only the saprolite and a part of the transition rock are extracted in the open pit. Excavation works are done using shovels Hyundai 450 (4 units) and haulage works using articulated dumpers Terex TA35 (4 units). These two sorts of equipments are presented figure 2. Mill The mill is composed of crushing and grinding processing plants. After gravity concentration, both plants produce tailings that are stored in two ponds: the first pond contains coarse product which segregates the particles crushed to 2mm upstream and settled clays downstream; the second pond contains the fine product made of particles ground to 200µm. These two ponds, bordered by dams and diversion canals, are located on former alluvial sites of Esperance Creek (see figure 3). [ ] The CME mill is currently producing gold concentrate, with a global recovery of about 67%. The ore is crushed by one jaw crusher and two hammer crushers. This presentation in the open pit session deals with the development of a phosphate rock strip mine by CF Industries, Inc., a company that does not presently mine in the Central Florida Phosphate District. Land acquisition, engineering, financing and environmental permitting activities were begun in early 1975; site preparation, environmental monitoring and dragline construction began in mid-1975. The operation is due on-stream in late 1978. The reserve acquisition, permitting, and financing could each easily be developed into a separate paper of interest. Each, however, has been covered before for other mining ventures with similar capital and environment requirements. For that reason, I have chosen to discuss what CF Industries, Inc. is, why it is entering this new sec-tor of the agricultural chemicals business, and, to keep at least some technical basis, what design modifications are possible when the total mine output is used captively and it is unnecessary to have the flexibility to meet the numerous transportation and customer specification considerations. In an attempt to show the size and complexity of the CF organization, a brief description of existing facilities should help show why the big interest in mining phosphate rock.

Newmont Mining has a large, integrated gold-mining operation in northern Nevada with several mines, stockpiles and processing facilities. There are more than 90 defined metallurgical ore types and more than 60 defined gold-recovery process options. Newmont developed a mine plan and process model to take better advantage of the available synergies at its Nevada mines. This plan includes a mixed integer and linear programming optimization tool. This system is maintained and operated by a Nevada planning group. Standard in-house planning software tools and methods have been implemented at all mine sites. These changes have allowed a reduction in mine engineering staff and have increased the effectiveness of mine planning.

"The history of Duluth commences with Daniel de Greysolon du Lhut, one of the explorers of the Upper Mississippi who came to the head of the lakes in the summer of 1679. Radisson and Groseillier, and Claude Allouez, a Jesuit priest, are supposed to have visited the head of the lakes, -but there is no authentic record previous to that of du Lhut.In 1792, the fur traders established a fort at Fond du Lac, fifteen miles up the St. Louis River from Lake Superior, but within the limits of the present city •of Duluth. In the early 50's, there were a few scattering settlers at Oneota and around the George Stuntz trading post on Minnesota point. In 1855-6, the settlement on Minnesota point was called Duluth, commemorating the name du Lhut, and organized as a town in 1857. The first board of trustees and town council consisted of -William Nettleton, J. B. Culver; Robert E. Jefferson, Grin W. Rice and William Old: In 1870, the city of Duluth was incorporated, and five years later a portion of the same territory was incorporated as the village of Duluth, and the two municipal corporations were still in existence on March 1, 1887, when the city and the village of Duluth were incorporated as the present city.The first saw mill in Duluth was erected in 1856-57, on the present site of the ship canal. It cut logs from a virgin forest at the upper end of Minnesota point.The first railroad was built to the head of the lakes in 1870. The charter for this road had been granted in 1861 to the Lake Superior and Mississippi Railroad Company, afterward called the St. Paul and Duluth, and which is now a part of the Northern Pacific Railroad."

"The Central Bayside System Improvement Project (CBSIP) is a critical element of the Sewer System Im-provement Program (SSIP), a 20-year, $6.9-billion investment by the San Francisco Public Utilities Commis-sion (SFPUC) to upgrade San Francisco’s 100+ years old seismically vulnerable combined sewer system. The CBSIP’s centerpiece, a proposed 10,000-foot-long (3,048-meter-long) Channel Tunnel, will provide reliable and redundant gravity conveyance and storage of combined wastewater flows from the northern and central parts of San Francisco to the Southeast Water Pollution Control Plant (SEP) while ensuring compliance with regulatory requirements for combined sewer discharges to the Bay. A tunnel with a diameter of 23 to 30 feet (7 to 9 meter) is being planned using pressurized-face tunnel boring machine technology. Project challenges include tunneling through Franciscan Bedrock, mixed ground conditions in close proximity to the Bay, connector tunnels and shafts, construction in congested urban environment, and challenging real estate issues for shaft access. BACKGROUND AND NEED FOR TUNNEL The City of San Francisco is one of the most beautiful cities in the world. About 850,000 people live in the city and many more come to work, for tourism, and for attending conferences. The SFPUC is a department of the City and County of San Francisco that provides energy, drinking water and wastewater services to San Francisco, wholesale water to three Bay Area counties, and green hydro-electrical and solar power to San Francisco municipal departments. The SFPUC wastewater system includes tunnels, large storage/transport boxes, 1000 miles of collection sewers, 28 pump stations, and three wastewater treatment plants. San Francisco’s combined sewer system collects both wastewater and stormwater in the same sewers, which is then pumped, treated to stringent state standards and discharged into the San Francisco Bay and Pacific Ocean. After decades of service, much of the sewer system is nearing end of its working life. During the 1989 Loma Prieta earthquake, damage to SFPUC water and wastewater infrastructure was extensive. The 11,200-foot-long (3,400-meter-long), 66-inch-diameter (1,676-mm-diameter) Channel Force Main, built in 1976, and a critical component of the SF wastewater system was also damaged. This force main carries 70 percent of the bayside dry weather sewage flow from the northern and central part of the city to the SEP. The force main is at the end of its life and was not constructed to the current seismic code; in addition it has no redundancy. Figure 1 shows the project background and the force main that will be provided redundancy by the new gravity tunnel."

From a distance, the 0.75-km (0.5-mile) rail line that sits in the shadows of Vale-Inco?s Stobie Mine outside of Sudbury, Ontario could be mistaken for a standard conveyor belt hauling ore from the nearby mine. But with its large, yellow-painted loop and interconnected green rail cars that invert themselves on the loop before correcting with a smooth rollover further down the track, one might assume that this is some sort of test track for a new roller coaster. The fact of the matter is that it is the proving ground for Rail-Veyor technology, a technology that the company hopes will one day change the face of ore transportation in surface and underground mines around the world. With a line of linked, low-profile rail cars that are capable of carrying up to 1 t (1.1 st) per car of ore by way of simple, above ground, lightweight rail track, the technology is not groundbreaking. In fact, it was developed in the 1960s for the French State Railroad. A demonstration plant for the Florida Institute of Phosphate Research was installed in 2000 and the first commercial under-ground installation was in 2007 in South Africa at Harmony Gold?s Phakisa Mine. There, a 4.7-km- (2.9-mile-) long tram between two shafts operates three Rail-Veyor trains on a shared track. The difference is that the application that is being tested day-and-night in Sudbury is getting closer to wide production, thanks to the use of computer controlled automation that has helped turn an old idea into a cost-effective modern solution.

"Mining is an inherently risky business. Over the years, major mine disasters have resulted in the deaths and injuries of thousands of miners in the United States. These disasters also resulted in innovations in approaches to mine design, operations and the training of miners and mine rescue teams. The U.S. Congress responded to these disasters with new laws designed to create mandatory safety and health standards as a means to eliminate fatal accidents; to reduce the frequency and severity of nonfatal accidents; to minimize health hazards and to promote improved safety and health conditions in the nation’s mines.Following a mine emergency preparedness and response holistic gap analysis in 2012, the U.S. Mine Safety and Health Administration (MSHA) identified the need for the development of risk and readiness assessment models for MSHA and the mining industry. MSHA’s objective was to evolve existing theoretical concepts for risk and readiness assessment into simplified tools that can be applied by the mining industry for use at operational levels. In 2012, MSHA hired ABS Group, a recognized industry leader in safety risk management and a wholly owned subsidiary of the American Bureau of Shipping, to develop self-assessment models to help meet this objective.In September 2012, MSHA chartered a project to supply the coal mining industry with a pro-active toolset for mine operators to:1. Self-assess the risks associated with underground coal mining operations in order to prevent major mine emergencies.2. Assess the preparedness of mine operators to respond to an emergency.3. Measure the readiness of mine rescue teams and responsible persons to execute emergency plans.MSHA’s objective was to evolve existing theoretical concepts for risk and readiness assessment into simplified tools that can be applied by the coal mining industry for use at operational levels."

The Burnaby Mountain Tunnel was an integral part of the Trans Mountain Expansion Project. General Contractor, Kiewit Ledcor TMEP Partnership (KLTP), contracted with McNally Construction to build an underground tunnel between the Burnaby and Westridge Marine terminals under Burnaby Mountain. The pipeline was constructed within a 2.6-kilometre tunnel, eliminating the need to run the pipeline over the mountain and minimizing community impacts. The complexities of this Project led to multiple innovations, including the design of a Tunnel Boring Machine (TBM) capable of excavating through mixed-faced conditions concurrently thus allowing the Project team to complete the tunnel with limited geotechnical information.

For the REM (Réseau Express Métropolitain) project, a 2500 m tunnel was built using a tunnel boring machine (TBM) under the Montreal Pierre Elliott Trudeau Airport (ADM). Although the geology under the Montreal Airport (ADM) would have been suitable for an excavation with a hard rock machine, the rst section reuired an earth pressure balance (EPB) TBM due to the soft soil located under wetlands. After excavating 280 m, the TBM was blocked in a transition zone of unconsolidated, water-bearing soils directly above the underlying bedrock. A free air intervention was not possible due to these geological conditions. After several failed attempts to create a slurry cake for hyperbaric interventions, a ground freezing system was designed and implemented, incorporating angled pipes drilled from outside the wetlands to directly in front of the cutterhead. This paper discusses the engineering reuired to form a frozen, water-tight zone in front of the cutterhead using liuid nitrogen. It also describes the complex method of replacing the disc cutters while holding the ground freezing system active in a manner protecting the health and safety of the workers.

Tunnel approach structures situated in slow moving earth flows are subjected to significant earth pressures. The authors have developed a system, utilizing collapsible concrete columns, that limits such loading. The system uses an array of collapsible concrete columns in front of the structure to absorb downslope movement and limit-imposed loads. This concept has been modelled using 3D FEM analyses and a rudimental lab test. For implementation, the collapsible columns would be prefabricated in strengths to mimic the soil strength profile. This concept has merit, and analytical results demonstrate that collapsible columns are a viable means to limit loading.

"Water is a primary commodity for most industrial processes. It has become a global focus in light of climate change and population growth. Reuse of process water has two major benefits - minimize the amount of fresh water added now and minimize the amount of discharge water that requires treatment later.Mining and mine product processing has its share of reused water. For example, reclaim water from a tailings impoundment, overflow from thickeners and leach solution to raffinate and back again all demonstrate the use and reuse of water in mining circuits. Eventually, a mine reduces production or ceases operations completely, eliminating the option to reuse the impacted water and, thus, requires treatment of the remaining water.Current methods of water treatment remove the impacts, providing clean water for discharge, but leaves behind a concentrated waste that requires further treatment or disposal of a hazardous waste. Freeport- McMoRan is currently testing three water treatment technologies specific for sulfate and/or metal removal from mine impacted water with the approach of transformation not concentration. These technologies provide clean water for alternative use while creating byproducts that could be marketable, or disposed as a nonhazardous material, thereby reducing the cost and longterm liability of disposal.The selection of a water treatment process is site-specific. The volume of water, contaminant to be removed, disposal options, length of time for treatment, location of plant and reagents and discharge requirements are some of the factors that need to be considered when selecting a water treatment process. Common water treatment technologies for mine impacted water consist of low- or high-density sludge (LDS/ HDS), reverse osmosis (RO) and evaporative technologies. In addition to operating costs, these processes require additional handling of the concentrated sludge or brine. Advanced water treatment technologies include ion exchange, solvent extraction and metals precipitation with or without the aid of a biological system. Some of the more complex systems have a series of unit operations to achieve required discharge regulations."

Introduction and Historical Background The office and mining operations of Pacific Asbestos Corporation are located near Copperopolis, California in the foothills of the Sierras about fifty miles east of Stockton. This is historic country in the heart of the old Mother Lode gold district. Mark Twain's famous story of the jumping frog, written here, is still commemorated in an annual contest in Calaveras County,' and the camp at Poker Flats, described by Bret Harte, was only a few miles from the present mine.2 Joaquin Murrieta, the famous Mexican bandit started his three year reign of terror in this vicinity,3 and Black Bart, the P08 laureate of the stagecoach robbers, held up his first stage at Funk Hill, just three miles north of the mine. His last robbery was at almost the place, on the top of the hill overlooking the Stanislaus River ferry crossing. Although this was his favorite country, and he traveled extensively through it, he was not a credit to his acclaimed profession of Mining Engineer because there is no evidence that he ever recognized the asbestos, and he was finally caught as a consequence of the robbery at the ferry.4 One of the old stagecoach roads crossed the Stanislaus River at the mine site, and the ferry landing may still be seen from the main haul road into the pit. The road itself is partly buried under the rock dumps. In spite of its location and apparently prominent exposure, the first work on the asbestos deposit was not recorded until 1904.5 Development of the present operations was started in 1959 by Jefferson Lake Asbestos Corporation, a subsidiary of Jefferson Lake Sulphur Company. Management control of the company changed hands several times and it was later reorganized under the name of Pacific Asbestos Corporation. In December, 1968, a controlling interest was acquired by H. K. Porter Company, Inc. Figure (1) shows the mill and office buildings as seen from a site near the entry road. The offices are at the left with the mill warehouse in the foreground. The conveyor at left center carrys mine concentrates up the hill from the crusher. In the background are the dry rock storage building and mill tailings piles. Since the beginning of mining operations in 1962, seven and three-quarter million tons of ore and fifteen million tons of waste have been removed from the open pit which now measures about 1400 x 1800 feet at its widest points. The lowest operating level is at an elevation of 726 feet about 325 feet below the original surface. Figure (2) shows the open pit looking toward the west end. The main haul road can be seen along the side of the pit. A shovel is operating at the bottom of the pit while a front end loader on the level above provides ore for blending. In the background are rock dumps for waste stripping. The company markets an annual production of 30-35,000 tons of chrysotile fibre in groups 4 through 7, under the Mother Lode Brand
