This paper briefly describes the progress in mine safety in the United States, with emphasis on recent events that have followed since the explosion at the Sago Mine in January 2006. Legislation following the mine accidents last year and areas of work carried out by the National Institute for Occupational Safety and Health (NIOSH) related to the accidents are discussed. American coal production in 2006 was 1,150 million short tons. Most of this coal is consumed domestically, with annual exports of coal about 50 million tons. One third of annual production is produced at surface mines and the rest from underground. This coal is produced by about 82,000 miners working at 1,400 coal mines. Major improvements in coal mine safety have been made in the US since the Federal Mine Safety and Health Act of 1977. The fatal injury rate in underground coal mines between 1977 and 2004 was reduced by 47.8% to 0.036. The annual number of fatalities dropped from 112 to 14 during that period. Likewise, the non-fatal days-lost injury rate was reduced by 42.6% from 10.87 to 6.24. The improvements in mine safety during this period can be seen in the graph of underground coal mine explosion fatalities and injuries from 1980 to present, which is shown in Figure 1. Between 1984 and 1993 (9 year period) there were 5 explosions and one major mine fire, for a total of 55 fatalities. Over the seven year period from 1993 to 1999 there were no fatalities from coal mine fires and explosions. However, in the six years since 2000 there have been 5 explosions and one fire, which have caused 37 fatalities. [ ]

An inert-gas fusion technique was applied by the Bureau of Mines to the oxygen analysis of mixed-fluoride electrolytes used in electrowinning uranium and the rare-earth metals. Method development included: (1) Thermodynamic and experimental evaluation of the temperature required to quantitatively remove oxygen from a fluoride melt; (2) installation of a water and an acid trap to entrain the volatilized fluorides; (3) development of a suitable sample-preparation technique for the heterogeneous fluoride baths in which both pulverizing and pelletizing were necessary to achieve reproducible results; (4) comparison of platinum flux and dry crucible techniques, which showed that the use of platinum was not necessary. The accuracy of the method was evaluated by analyzing fluoride baths containing known amounts of oxygen added as the metal oxide. In the 0.05-to 0.5-percent range, the relative error was about ±5 percent.

Objective The main purpose of this video is to teach mine rescue personnel certain aspects of rescue work that are not traditionally taught. These include the strain caused by working continually under adverse conditions; the "survivor guilt" that occurs from being spared the fate of one's deceased cohorts; and the posttraumatic stress that may be experienced during the sometimes lengthy task of recovering the victims' bodies. Background On November 20, 1968, an explosion rocked the Farmington No. 9 Mine in northern West Virginia (figure 1). The explosion killed 78 of 99 miners who were working in the mine, including all but one member of the mine's rescue team. Some 30 years after the event, researchers from the NIOSH Pittsburgh Research Laboratory (PRL) conducted a video interview with Danny Kuhn (figure 2), the only Farmington No. 9 mine rescue team member who was not working that day and who did not die in the explosion. According to the Mine Safety and Health Administration (MSHA), there are about 80 underground coal mine rescue teams in the United States, and according to the National Mining Association, there are more than 850 underground coal mines. This represents a significant decrease in both over the past decade. The low number of rescue teams is a concern because serious incidents, though fewer, still happen, and if a disaster occurs, there will be fewer rescue teams to respond. This would result in an increased need to use available human resources very efficiently. Much training to prepare these rescue teams still takes the form of what educators call "teaching to the test." In other words, teams practice in order to win mine rescue contests. This test-oriented approach has traditionally been augmented by interactions with others who have responded to real situations or by participating in actual incidents. Given few incidents and fewer rescue teams from whom to derive enrichment, the preparation of coal mine rescue personnel becomes a problem. Recently, more emphasis has been placed on active learning, such as working out problems in smoke. These sessions more nearly simulate the actual conditions that today's team members might rarely encounter. To properly prepare and retain the current coal mine rescue personnel who are left in the workforce, it is necessary not only to offer such hands-on experience, but also to replace the learning narratives that once were passed from team to team. The present video is intended to contribute in that capacity. Approach NIOSH researchers first learned of Mr. Kuhn in the fall of1998. Mr. Kuhn was contacted and agreed to an interview. An oral history interview was conducted with Mr. Kuhn in early November1998. After reviewing the videotape of the interview, researchers determined the content to be so rich as to warrant a second interview in a more formal setting. The second interview was done in early December 1998. During this interview, Mr. Kuhn spent about 2 hours describing the Farmington disaster, including the workplace climate leading up to the explosion, the events that took place in the 10 days after the explosion, and the nearly 10-year effort to safely and successfully recover most of the victims' bodies. The videotape of the second oral history interview with Danny Kuhn was reviewed and edited by NIOSH researchers for content. Careful attention was used to ensure that valuable information was presented, but within a reasonable timeframe. A detailed map of the Farmington No. 9 Mine was edited into the final video and used to highlight critical features as they were being described by Mr. Kuhn. Results and Accomplishments A 60-minute videotape and instructor's guide for use in safety training or other settings was completed. The target audience consists of all those who work in underground coal mines. This training video will help safety instructors better prepare mine rescue team members for the situations that they will encounter when engaging in extended rescue and recovery work. This report summarizes completed research pertaining to the reaction of human subjects exposed to Self-Contained Breathing Apparatus (SCBA) stressors during escape conditions (viz., resistance breathing, C02 breathing, hot air breathing, and static positive facepiece positive pressure breathing) and the development of an automated breathing and metabolic simulator (ABMS). Volume I contains summarizes of the studies performed under this contract. Volume II is an appendix which contains more detailed descriptions of these studies.

Researchers, medical organizations, statisticians, and others often use data on the nature and rate of occurrence of occupational diseases in directing their efforts and plans. Very often this information is developed from the reports of the States which describe the nature of work¬men's compensation claims and awards and the frequency with which they are made. To be meaningful, the statis¬tical information must be viewed in the context of the State laws involved. There is considerable variation among the laws of the different States. The Bureau of Mines has responsibilities for protecting the health of persons employed in the mineral industries and for the development of useful mineral products. To assist the Bureau in the accomplishment of its objectives, this compilation has been undertaken. For the benefit of the mineral industries, and any others who may have a similar interest, this volume is made available. An earlier compilation of the Bureau of Mines on this subject was published in 1952.4 However, during the intervening years, sufficient changes have been made in State laws to warrant completely recompiling the information. This publication includes the laws of the States, the District of Columbia, Puerto Rico, and the Virgin Is¬lands through 1964. Only those portions of the workmen's compensation laws which apply to or as ' related to occupational diseases are presented, and in each case enacted additions to and deletions from the basic law have been incorporated into the text of the law, thus snak¬ing it unnecessary to search out the various amendments made each year.

Tungsten metal, chemically equivalent to commercial grade, is produced by electrolysis of impure tungsten compounds or scheelite in an alkali, phosphate or borate electrolyte. A preliminary investigation of fused-bath electrolysis shows the effect of electrolyte composition, temperature, and current density on current efficiency, yield, crystal structure, and purity of product. Experiments show that tungsten can be electrowon from scheelite in a fused alkali, phosphate, or borate bath with similar results. Studies on electrolyte composition show that the addition of sodium chloride is essential to both electrolytes in treating scheelite. Although its main function is to react with or flux the CaO, it also increases the bath fluidity. Temperature studies revealed optimum ranges between 900° and 1,100° C.; 900° C. proved satisfactory for tungsten trioxide, and 950° to 1,050° C. for scheelite. Current efficiencies were satisfactory for all current densities ranging from 10 to 57 a./dm.2. Yields were 400 to 650 g./kw.-hr. over the same current-density range. Crystal size and purity of the product are functions of temperature, current density, and amount of impurities in the electrolyte. Deposited crystals vary in shape from spherical grains to clusters of dendritic hexagonal needles, and in size from smaller than 35 to 570 microns. The purity of the tungsten metal produced ranges from 99.7 to 99.94 percent.

This report presents a quality control approach to the derivation of strategies for monitoring a work environment; specifically, the environment in underground mines where diesel-powered equipment and blasting agents are used. The approach is presented as a series of 22 steps called the Monitoring Strategy Development Scheme. The scheme is an orderly method for selecting from a number of options a strategy or tactics to cost effectively meet the requirements of a chosen objective related to the monitoring of breathing zone air quality. The scheme provides a format to quantify many of the factors that influence the selection of on strategy or tactic over another, and to ensure that important considerations are not neglected. The premise that breathing zone air in work environment can be considered to be manufactured product and that, therefore, conventional quality control approaches are applicable, is introduced. Typical approaches to the quality control of manufactured products and how these approaches can be applied to the selection of monitoring strategies are discussed.

Introduction This Instructor’s Copy contains most of the information needed to use the Wearing Hearing Protection Properly 3-D training reel. It offers practical suggestions on how to use the reel as a training aid, performance objectives for deciding if the training is appropriate for your needs, and discussion notes that provide additional information to the user. The 3-D reel was designed as a self-teaching aid that drillers and others who work at a drill site can use wherever the opportunity for such training presents itself. The latter could be a company preshift requirement or a tailgate session where workers would have an opportunity to view and discuss the reel. It could also be an individual looking at the reel in a pickup truck prior to beginning work at a drill site. The 3-D training reel is versatile in that it can be used for a small group of persons or for a single worker. For any of these situations, information is transferred to the individual by viewing each of the seven colorful, in-depth 3-D scenes and relating the words that appear in each scene to what is depicted in the visual. Presented in this volume are the basic mining techniques and production operations associated with underground bituminous coal mining. Chapter 2, "Multiflow Process Charts", presents, in sketch form, a description of the various operations characteristic of conventional and continuous mining. In order to focus attention upon the major components and operations in each work cycle, certain items are not shown in the sketches, e. g., machine electrical cables, brattice, and rib posts. The elapsed time shown for each sketch is considered representative for the various work elements summarized by the sketch. More detailed work element times can be found in Chapter 3, "Typical Work Cycle Times", and Chapter 4. "Standard Time Data".

Aruba, formerly a. part of the Netherlands Antilles, separated on January I, 1986, and became an autonomous part of the Netherlands. Full independence of the island from the Netherlands is scheduled for 1996. In this chapter, however, Aruba's statistics are included with those of the Netherlands Antilles. Tourism continued to be the main industry of the island, which continued to depend heavily on imports, mostly from the United States. During the year, the Government was preparing legislation to regulate the exploration of oil deposits in the relatively shallow sea around Aruba.

A continuous mining machine was redesigned for the installation of an integrated illumination system. The illumination system was designed by first calculating the theoretical number of luminaires required to meet the Federal Illumination Standards. Luminaires initially were placed using isointensity diagrams. A darkroom was used for "fine tuning" and demonstrating that the system met the illumination requirements. The luminaire mounts were installed on a machine underground. The performance of this illumination system was monitored in the working environment of an operating coal mine. Design changes were made as necessary during this evaluation period to improve the system.

In the manufacture of steel by any process the refining period involves the partial elimination of the metalloids carbon, manganese, phosphorus, and silicon, by virtue of their reactions with ferrous oxide. At the end of this refining stage the metal consists chiefly of iron in which small amounts of these materials are dissolved. Unless some deoxidizer is added to the steel at this time, the reaction between carbon and ferrous oxide will continue with such force that it is impossible to form sound steal in the ingot molds. Instead, the product will exhibit a marked porosity due to the formation and trapping of carbon monoxide-the product of the reaction mentioned. However, if deoxidizers are added, steel may be obtained which is dense and free from gas bubbles. At the same time the deoxidation products will remain in the steal as impurities. For example, silicon additions form Si02 or combinations of Si02 and FeO. Additions of manganese form deoxidation products consisting of various proportions of MnO and FeO. It is also quite obvious that simultaneous additions of manganese and silicon will form oxide combinations either in the binary system MnO-SiO2 or in the ternary system FeO-MnO-SiO2. Work at the Pittsburgh Experiment Station of the United States Bureau of Mines, in cooperation with the Carnegie Institute of Technology and the Metallurgical Advisory Board, involved the study of such deoxidizing reactions and their products. In this connection considerable information is necessary concerning FeO, MeO, Si02, and Al 203, and their various combinations. Some of the most needed data are the melting points of all of these materials. At the outset of this work ordinary methods of melting point determinations were found to be inadequate for various reasons, and the procedure finally selected involved the use of the micropyrometer with occasional corroboration by quenching methods, The reasons for this selection, together with a description of the apparatus, are given in the following pages.

As part of its overall program of coal carbonization, the Bureau of Mines is studying the coking properties of coals that may be used for metallurgical purposes. The objectives of this study were given in detail in a previous report.2/ The suitability of coal for metallurgical use depends primarily on (1) chemical composition, (2) preparation characteristics, and (3) carbonization behavior. The first two factors, as well as the available reserves, are considered in selecting coals for carbonization tests. According to the estimate of reserves by Dowd,3/ the recoverable coal in Raleigh County in beds 28 inches or more thick, in thousand tons, is: [ ]

Mining activity during 1977 was limited to the production of limestone, marl, and sand and gravel, all of which were used domestically. Public works projects consumed the major portion of the construction material produced. The consortium of oil companies operating as Esso Ventures Inc., drilled one exploratory well off the coast of southern Belize. For drilling the well, Exxon Corp. received a 57% working interest in the consortium's concession located 40 kilometers east of Puta Gorda. No other offshore drilling was attempted in 1977. Esso also drilled a well on the Anschutz Overseas Inc. concession in the Toledo District of south- eastern Belize. The attempt wee abandoned, but seismic activities continued.

This report describes the initial phase of Bureau of Mines research in the hydraulic mining of bituminous coal. The mining site and equipment are described, and the test procedures and results obtained with a bar-mounted monitor are given. Tests showed that hard Pittsburgh-bed coal can be mined hydraulically from a solid face at water pressures of 4,000 pounds per square inch.4 Coal was penetrated to a depth of 4 feet in 20 seconds with 225 gallons of water per minute coming from a 3/8-inch nozzle at 4,000 pounds per square inch pressure. Some difficulty was encountered in cutting the binders occurring in the coal; it was not possible Lu penetrate directly the overlying draw slate.

The Bureau of Mines (Bureau) and the U.S. Geological Survey (USGS) conducted a 4-year (1979-1982) mineral assessment study of the Chugach National Forest (CNF), Ak, as part of the Roadless Area Review and Evaluation (RARE II) Program. The USGS remapped the geology and distribution of mineral resources. The Bureau investigated mines, prospects, and mineral occurrences. This report describes the results of Bureau work in the central 790,880 acres of the Forest. Copper, with by-product zinc, silver, and gold, has been the major metal produced from the study area as well as from the entire Chugach National Forest. The majority of the copper deposits are concentrated within basaltic rocks on Knight Island but the major producers are hosted by sedimentary rocks on Latouche Island. Approximately 182.6 million lb of copper, 1.5 million oz of silver, and an unknown but substantial amount of gold were produced during a 30 year period. Currently no mines in the area are producing. Inferred, indicated and measured reserves having mineral development potential total 4.6 million tons, with weighted average grades of 1.6% copper, 0.30 oz silver/ton, and 0.04 oz gold/ton.

As part of the overall development of a borehole mining system -develop backfilling techniques to reduce possible damage to the environment caused by either the cavities or the piles of sand tailings that result from in-borehole hydraulic mining. (Borehole mining is described in technology News No. 56.) Approach Water filled cavities at the bottom of boreholes are rapidly and economically backfilled by jetting a sand-tailings slurry underwater. How It Works The Bureau of Mines borehole mining system creates a cavity having a horizontal diameter of more than 50 feet. (See photograph). This cavity can be backfilled with much of the sand previously mined. The sand tailings or similar backfill material is dumped by a front end loader into a slurry mixing tank at the rate of about one ton every three minutes. Enough water is added to give a thick, but pumpable slurry. The slurry is pumped down the borehole through a string of 4-inch pipe to the cavity. After passing through a right elbow at the end of the pipe, the slurry is ejected through a nozzle. The resulting slurry jet emplaces the backfill at the outer wall of the cavity. To obtain an even distribution of fill in the cavity, the nozzle is slowly rotated. The nozzle and pipe assembly is supported from a turntable on the surface. See diagram.

Research on the solid-fuel reduction of Knob Lake iron or was conducted to upgrade lean and high-grade ores and recover metallized products suitable for ironmaking or steelmaking. The program included bench-scale tests plus a number of continuous campaigns in a 34-inch by 36-foot rotary kiln. In the laboratory, metallizations in excess of 90 percent were achieved by reduction of crushed ore with lignite at 950° C. Subsequent grinding (to minus 200 mesh) and wet magnetic separation yielded concentrates analyzing less than 3.0 percent SiO2 and more than 90 percent Fe. Virtually complete recovery of the iron occurred. During early continuous tests the crushed ore created a serious ringing in the kiln which was solved by balling the fines with bentonite. Unfortunately, at this temperature (950° C) the bentonite militates against the concentratability of the metallized product. For example, a run on direct-shipping ore produced concentrates of 89.5 percent Fe and 4.5 percent SiO2. At 500° C the adverse effect of bentonite disappeared and concentrates of 88.4 Fe and only 2.5 percent SiO2 resulted. However, with comparable feed r Los, the metallization dropped to 73 percent.

"Purpose of InvestigationThe process of removing solid material from liquid suspensions has been studied at the Northwest Experiment Station, United States Bureau of Mines, in cooperation with the University of Washington, as part of a general program to investigate the dewatering of Pacific Northwest plastic kaolins, which is difficult by ordinary filter-pressing methods. The present paper is a progress report of the work began by Vance S. Cartwright in 1928 and continued in 1931 and 1932. Spray evaporation is believed to be valuable for the following classes or grades of clay and clay mixtures:(1) Highly colloidal, plastic-type clays, which release their mechanically attached water with difficulty; (2) clays prepared for shipment in pulverized or powdered condition; and (3) synthetic, ceramic, porcelain, and tile mixtures prepared as aqueons suspension, dried and pulverized before molding. Dilute suspensions can be dewatered by spray evaporation to produce concen-trated casting slip or plastic clay with a lower water content.The most common dewatering method used in preparing powdered kaolin for the market comprises the following steps: Reduction of the water content by filter-pressing from approximately 75 percent in the thick ""slip"" condition to about 30 percent in the condition; further reduction of the water in the filter-press cakes by heat to about 5 percent; and pulverization of the dried cakes in hammer or dust. These three steps can be simplified to a single process by spray evaporation."

This paper is a preliminary report on recent lode gold mining activity, operating conditions, and mining and milling methods at properties in production in September 1936 in the gold belt of northeastern Oregon. A few inactive properties are described also. Districts, inactive in 1936, that have been important producers at some time in the past are mentioned briefly. This is one of a series of similar papers describing western mining districts. The past production record of the gold mines of northeastern Oregon entitles the region to be ranked as one of the important gold-mining regions of the United States. Although comparatively inactive for many years, lode mining in the region has recently undergone a marked revival.
