Sixty years ago, underground coal mining was the most hazardous job in the United States. Roof falls were a big part of the problem. They killed about 100 miners every year, more than all other causes put together. Fast forward half a century to 2016, which is the first year ever with zero roof fall fatalities. Just three miners were killed by roof falls during the following 6 years. How was this historic goal achieved? This paper starts with a modern analysis of the causes of the roof fall fatalities in 1968. Then it follows the reductions over time by category, using snapshots of the fatalities occurring in subsequent decades. Along the way, it evaluates the influence of the regulatory environment, changing mining methods, and better ground control technology. The paper shows that in 1968, more than half of roof fall fatalities at large mines were attributable to an inadequate safety culture. The immediate effect of the 1969 Coal Mine Health and Safety Act was to reduce the riskiest activities, like needlessly going under unsupported roof. Other hazards, like large roof falls, required technological developments before they were brought under control. Roof control plans, which the U.S. Bureau of Mines had been advocating since the 1920s, played a significant role throughout the process.

Safety management is an important mortal and business function in construction. Contractors have traditionally tracked and reported lagging indicators, e.g. fatalities and lost-time accident rates, to measure their safety performance and stay in compliance with relevant regulations. Over the last several decades, contractors became more proactive in their approach to safety and developed programs that track leading indicators, e.g. safety audits and safety climate. In particular, safety climate measures workers? perception of safety management and its effectiveness in the workplace. Past literatures indicated that, while some industrial construction contractors ignore safety climate measures, others are limited by a lack of a formal means to measure safety climate. In this study, a survey approach is used to measure an industrial construction contractor?s safety climate through three key areas: management commitment, job control, and general safety climate. As a pilot study, a total of 214 individuals at a fabrication facility participated in the survey to verify the validity and effectiveness of the proposed survey approach. This survey study also confirms that job control and management commitment have a positive correlation and that worker demographics have an effect on respondents? perceptions of management commitment.

Early in 1944 the Chilean Government expressed a desire for assistance from the Bureau of Mines in a study of the coal deposits, coal-mining methods, and preparation and benefication of coals in Chile. A Memorandum of Understanding setting forth the purpose of the work was executed by the United States Government and the Government of Chile. Parts of this agreement are quoted: On February 4, 1944, the Ambassador of Chile in Washington expressed the interest of his Government in obtaining the loan of a mission of experts to assist in the conduct of a survey looking toward the utilization of the low-grade coals of Chile and the application of such technology as may be developed for the distillation of these coals and the use of oil shales. The matter has since been the subject of correspondence between the Secretary of State and the United States Department of the Interior, and the latter Department has agreed to cooperate with the Government of Chile, through the Bureau of Mines, subject to the following understanding:

Strabag, in partnership with Arup, is constructing the Advance Tunnel for the Scarborough Subway Extension (ATSSE); a project by Metrolinx and Infrastructure Ontario (Sponsors). The 10.7 m internal diameter single bore tunnel represents a significant departure from the traditional arrangement of twin subway tunnels in North America. ATSSE is the first-time steel fibers are used to solely reinforce a tunnel of this size in Eastern Canada. This paper discusses the challenges with design and acceptance of SFRC segments and the state-of-the-art numerical models employed to check the joint performance. The advance procurement approach for TBM and PCTL’s procurement to accelerate the project schedule is also presented.

Devido ao aumento da restrição do teor de enxofre nos aços torna-se necessário investigar a eficiência do processo de dessulfuração de ferro gusa. Portanto, o objetivo deste trabalho é empregar diferentes matérias-primas e fundentes como cal calcítica, sodalita, pré-cal, mistura padrão, fluorita, escória de KR, borra de alumínio e B2O3, a fim de encontrar a composição com maior eficiência de dessulfuração, relacionando-a ao fator de dessulfuração, obtido pela termodinâmica computacional. Os testes foram realizados a 1350°C, em uma atmosfera inerte com agitação constante. Os resultados mostraram que a mistura com maior eficiência foi aquela com 69% de cal, 5% de sodalita, 15% de pré-cal, 6% de escória KR e 5% de B2O3.

The mineral processing industry is in boom times. The focus on mill availability and therefore profitability has never been greater, as grinding mill maintenance often dictates the entire plant shut down duration. Advances in mill systemsÆ quality and reliability and the faster, safer relining afforded by RMEÆs Mill Relining Systems have significantly enhanced grinding plant availability and utilisation. Yet relining performance is still affected by the design and planning by a team of stakeholders long before the maintenance team set foot in the mill. For a greenfield grinding plant project, these team players include: 1. project owner, 2. project manager, 3. mill supplier, 4. liner supplier, 5. plant operating personnel, 6. plant maintenance personnel, 7. mill relining crew, and  8. mill reline system supplier (RME). The complex and sometimes competing design demands of mill construction and operation together with the pressure of tight timelines can result in even simple maintenance requirements being overlooked let alone optimising plant design and planning for maintenance. This paper identifies all the involved stakeholders in an effective mill relining 'team' and gives principles and considerations for each to more effectively achieve the goals within their vital roles. This paper is aimed to be a practical guide for grinding mill designers and planners to more fully exploit the advances in Mill Relining Systems and improve their mill profitability.

The Bureau of Mines examined the Bear Canyon RARE II area in 1982. The area has no identified mineral deposits. The few claims that were located in the study area are devoid of any workings. No concentrations of economic minerals were found in the samples taken. There are no mineral, geothermal, or oil and gas leases in the area; the nearest oil well is 10 mi (16 km) east. The nearest mining operation was the Ruby Gold Placer worked in 1914, 7 mi (11 km) southeast (Hart, 1966, p. 45); the Pliocene Paso Robles Formation which contained the gold does not extend into the area. SETTING The Bear Canyon RARE II area covers 19.7 sq mi (51 sq km) or 12,600 acres (1500 ha) in west-central California. It lies in the Santa Lucia Range 10 mi (16 km) southwest of King City and adjoins the southeast side of the Ventana Wilderness. It is 10 mi (16 km) from the Pacific Ocean. Primary access is by the San Antonio River road on the Hunter Liggett Military Reservation, which parallels the area's southern border. Further access to the area's interior is by jeep and foot trails.

"An underground environment with high air temperature and humidity conditions generated from various heat and moisture sources can significantly affect the thermoregulation processes of the human body. This can lead to a series of heat-related illnesses such as heat exhaustion, heat cramps and heat stroke. The majority of the metabolic energy of a mine worker results in the production of heat. If the human body is to maintain thermal equilibrium, then this metabolic heat must be transferred to the ambient surroundings at the same rate. The heat loss from the human body occurs through a combination of heat transfer processes, such as respiratory heat exchange, convection, radiation and evaporation. If the heat loss from the human body to the ambient surroundings is less than the metabolic heat produced, the excess will be accumulated within the human body, which will result in a rise in the core and skin temperatures. This can ultimately lead to a heat-related illness or a combination of heat-related illnesses. This paper aims to discuss major heat-related issues and provide an overview of various mine ventilation and cooling systems, which can be employed to overcome high levels of heat and humidity in underground mines. In this paper, the effects of heat exposure on the health, safety and productivity of the mine workers are also highlighted. IntroductionMining in the United States remains one of the most hazardous industries, despite significant reductions in fatal injuries over the last century (Coleman and Kerkering, 2007; Saleh and Cummings 2011; Jacklitsch, Musolin and Kim, 2016). Occupational health hazards within the mining industry include hazards that are physical, such as traumatic injury or hearing loss; chemical, such as silica or diesel particulate matter; biological; ergonomic; and psychological (Donoghue, 2004; Saleh and Cummings, 2011). The effects of some of these hazards can be diagnosed immediately, while many, like black-lung and silicosis, will have long-term effects on the health, safety and life quality of mine workers (Donoghue, 2004; National Institute for Occupational Safety and Health, NIOSH, 2000). Among these hazards, exposure to heat and humidity can significantly affect the safety, health and productivity of mine workers, particularly in deep and hot underground mines (Roghanchi, Sunkpal and Kocsis, 2015). Short-term acute heat exposures can cause a rise in the core temperature of the human body, which can result in a heat-related illness or combination of heat-related illnesses. Adverse long-term chronic heat exposure can generate serious occupational heat-related illnesses, such as cardiovascular diseases, mental health problems and chronic kidney diseases. High core body temperatures coupled with dehydration can generate negative behavioral effects such as physical fatigue, irritability, lethargy, impaired judgment, loss of dexterity and loss of concentration (Xiang et al., 2014)"

WHEN certain binary Fe-Ni alloys are worked cold and then stabilized by a stress-relief anneal, their Young's moduli are nearly invariant over a substantial temperature range determined by composition and work-anneal history.' In the present investigation addition of molybdenum to such alloys (besides increasing the yield strength) was found to diminish the sensitivity of the thermal coefficient of Young's modulus to composition changes. Such alloys are of interest for applications requiring 1 a metal with controlled, low temperature coefficient of Young's modulus and substantial magnetic permeability.' Young's modulus ordinarily decreases with rising temperature. In ferromagnetic alloys a change in modulus on heating due to loss of ferromagnetism modifies the .temperature dependence. Far below the Curie temperature ferromagnetism alters the modulus in two ways: a—Addition of the energy of magnetization, a function of interatomic distance, changes the relation between interatomic energy and distance.' This lowers the modulus in the Fe-Ni and Fe-Ni-Mo alloys of this investigation. b—An applied stress changes the ferromagnetic domain arrangement so that linear magnetostriction contributes to the strain and further lowers the modulUs.~ Since in the alloys of this investigation both effects lower the modulus, loss of ferromagnetism on heating changes the slope of the modulus-temperature curve in a positive direction. With increasing temperature the slope of the modulus-temperature curve, due to progressive loss of ferromagnetism, becomes less negative, goes through zero (the modulus is minimum), becomes positive, and resumes its normal negative value above the Curie temperature.' The increase in modulus and decrease in curvature in the modulus-temperature curve occurring on work hardening have been attributed to a reduction in effect (b).'. Vn the absence of ferromagnetism work hardening through introduction of residual strains would be expected to decrease the modulus.' Alloy Preparation—Test Methods Four series of alloys having nominal molybdenum contents of 5, 7, 9, and 10 pct and containing 47 to 58 pct Fe (the analyzed compositions are given in Table I) were melted in air and cast as bars weighing from 2 to 6 lb. The charge consisted of electrolytic nickel, Armco iron, molybdenum (99+ pct) or ferromolybdenum (62 pct Mo), manganese, and aluminum as a deoxidizer. The bars were swaged cold to the desired diameters with intermediate anneals. Alloys in this composition range are reported" to be face-centered cubic and ferromagnetic at room temperature. The Curie temperatures" decrease with molybdenum. For example, keeping the Ni/Fe ratio at unity and increasing the molybdenum from 0 to 17 pct decreases the Curie tempera- ture from approximately 500°C to room temperature. (The 17 pct Mo alloy requires quenching to retain a single phase.~) Young's modulus at a series of temperatures from —50" to 100°C was determined by a dynamic method previously described.', " In this method Young's modulus is calculated from the resonant frequency (in this case approximately 50,000 cycles per second) of a rod sample (approximately 2 in. long) vibrating longitudinally in the first mode. The densities of the samples at room temperature, Table I, were determined by the standard method of weighing in air and reweighing in redistilled bromobenzene. The linear coefficients of expansion, Table I, needed to calculate the sample length and density at the temperature of measurement, were obtained by observing the change in length (22" to 100°C) of an 8 in. gage distance with a two microscope cathetometer. The values of density and coefficient of expansion in the few samples checked are independent of treatment to the accuracy reported. This was assumed to be general for all the samples. The modulus values are accurate to &0.05x101' dynes per sq cm. However, values of thermal change in modulus are accurate to 20.005~ 10" dynes per sq cm. Young's Modulus Values at 25°C: The modulus values of O', 5, and 10 pct Mo alloys at 25 °C are plotted in Fig. 1 as functions of nickel. After working cold, the samples were either recrystallized at 950" to 1000°C or given a low temperature stabilizing anneal at 400° C. "This paper presents the design of a raft enhanced pile group recently constructed for a hotel development in Perth, WA. At the time of construction in 2016 the hotel was the tallest prefabricated hotel in Australia.The structure covers the entire 12 m by 32 m development footprint, located in an inner city environment, and directly surrounded by existing structures. The small site and lack of space required innovative thinking in terms of the substructure design.The ground conditions comprise a layer of medium dense sand underlain by variable layers of stiff clay, silts and sands. Groundwater was around 1.5 m below ground level.The substructure design was carried out on the basis of a raft enhanced pile group. This allowed the use of much shorter piles than a conventional pile solution, thereby permitting the adoption of CFA piling techniques. This also allowed the raft thickness to be reduced to such that the substructure works were maintained above the groundwater level.The design was carried out in general accordance with the Australian Standard AS 2159 and using conventional piled raft design techniques involving iteration between elastic soil settlement software and structural analysis software until spring reactions were obtained with convergence of the two models. Following this springs were introduced to model piles in the structure.Due to the variable nature of the ground and resulting uncertainty in pile base capacity it was not considered appropriate to design the system as a pile assisted raft with piles loaded beyond their ultimate geotechnical design strength. As a result, all piles were designed based on a maximum load maintained within the elastic response region, with the majority of the load taken by the piles. However, the combined capacity of the raft and the piles allowed for a saving in pile lengths and numbers of around 40%, when compared to a conventional pile group designed in accordance with the Australian Standard AS 2159.The raft slab was monitored during construction in order to check the actual displacements against the design displacements. At the end of construction, the settlements were found to be within 2 mm of the design values.INTRODUCTIONThe Wellington Street area of Perth in the CBD is undergoing recent regeneration which includes the King’s Square development, Perth Arena and the Wellington Street Bus Station. The proposed hotel development is close to the recent developments on a Lot previously occupied by a low rise development. Due to the small size and constrained nature of the site, the substructure design for the tower needed to take into account available space and the impact of the works on adjacent properties. This paper presents reasoning for the choice of substructure design, details of the design methodology adopted, results from limited post construction monitoring and potential improvements that could be considered for similar future developments."

Photogrammetry surveys were used to document a footwall slab failure that occurred at a surface coal mine located on the eastern slope of the Rocky Mountains. Photographs taken before and after the failure provided the pre- and post-failure geometry of the slope and the failed slab. A 2D finite element model was constructed using slope profiles. The model contained joint elements that represented the bedding surface upon which the slab slid and the model was used to analyze the footwall slope. The in situ stresses, rock properties and the bedding surface shear strength and stiffness properties are discussed. The process of mining out a valley and exposing a highwall was modelled. As expected, elastic valley rebound occurred in the model, but shear stress and shear displacement reversals along the slip surface were also observed. The modelling results provided new insight into shear strength degradation that occurs when the pit floor approaches and then passes a specific point in the highwall.

The conduct of work at elevated air pressures , such as in caissons and in tunnel - driving in subaqueous strata , poses safety problems not encountered in mining and tunneling operations at ordinary atmospheric pressure . This publication discusses some of the problems involved and presents information on the following subjects : ( 1 ) Physiological effects of gases at elevated pressure . ( 2 ) Use of respiratory protective equipment in rescue or recovery operations in event of fire or other emergency in pressurized tunnels . ( 3 ) Effect of elevated pressure on performance of some gasdetecting instruments ; interpretation of the results . Various types of respiratory protective devices are discussed in relation to their possible use at elevated pressures , especially the 2 - hour , selfcontained , recirculating - oxygen - breathing apparatus , which has had wide use in the United States during rescue and recovery work in the mining industry . The scope of this paper is such that it was deemed advisable to group the contents into several main sections. They are: Section 1. Geography and Geology. Section II. Historical Review. (a) Mining and Smelting in Alabama. (b) Milling. (c) Summary of Milling Research. These two sections are primarily a bibliography and summary of the research on the Birmingham Red Mountain ores up to the middle 1040's. Section III. Status of the Birmingham Iron Industry. This sect ion points out the extent to which the findings of the iron-ore research have been applied on a commercial scale. Also, a very brief description of the dependent and allied industries is included. The value of the Birmingham iron ore is greatly enhanced by the proximity of coal and dolomite. Thus the coal and coke industry and the dolomite industry are closely allied with and to a large extent dependent upon the possibilities of economic utilization of the Red Mountain iron ores. Section IV. Beneficiation Investigations of the Red Mountain Iron Ores. This is a record of the most recent, hitherto unpublished work of the Southern Experiment Station of the Federal Bureau of Mines, Tuscaloosa, Ala., on the chemical and physical composition not only of the Big seam but of most of the iron-bearing rock strata of Red Mountain.

A new waterfront park was developed on Long Island in close proximity to the East River and Newtown Creek. The existing ground consisted of fill over soft organic clay, medium dense silty sand, glacial till and schist bedrock. The ground was initially raised to preload the underlying soft soils to stabilize the ground in preparation for site development. The ground was subsequently cut down to temporary grades for installation of foundation piles for the overlook structures and construction of reinforced concrete cantilever retaining walls forming pathways located adjacent to the water bodies. The retaining walls were located in two rows at different elevations along the sides of a steep embankment. Where the pathways converge, some of the walls were too close to each other, and were redesigned as a combined U-wall with a common base slab. The individual retaining walls were designed for adequate bearing, sliding and overturning resistance. Ground anchors were installed to enhance global stability of the retaining walls. This paper presents the design considerations for the retaining walls for various loading conditions, and evaluation of the trial and proof tests for the anchors. INTRODUCTION The New York City Master Plan included the transformation of an approximately 30-acre parcel of land in Long Island City, Queens, New York from a former manufacturing zone into a residential, commercial and recreational zone for middle-income community. The site was located at Hunters Point South along the eastern shoreline of the East River and Newtown Creek (Figure 1). The development included low to high- rise residential buildings, a public school, and retail structures. Open spaces were provided along the East River and Newtown Creek waterfronts in the form of a continuous park providing for recreational and community-related activities. Waterfront structures included small park buildings, walkways and viewing platforms. The shoreline was surrounded by revetments as part of the flood mitigation strategy. The project involved extensive site re-grading and several retaining walls were constructed. The earthwork volume amounted to approximately 47,000 cubic yard of imported fill and 93,000 cubic yard of cut. The exposed height of the retaining walls ranged from 3 to 15 feet. This paper focuses on the design and construction of the retaining walls for the project.

THIS REPORT give's the results of a study of the central and south parts of the Cushing oilfield, including a description of the geology, a review of the production history, and estimates of oil reserves available by secondary-recovery methods. Each of the major producing zones of the field is treated separately. The report, which is one of a long series of field studies the Bureau of Mines has conducted over the past 40 years, was prepared to aid operators to increase oil recovery and to help prevent underground waste in the Cushing field, in the State of Oklahoma, and in the United States. The geology and production history are presented not as a review of past behavior of the Cushing field but rather to interpret the possibilities for increased oil recovery by future development of this and similar areas. The field, which is in the northwest corner of Creek County, Okla., was discovered in 1912 and developed during 1913 and 1914. For several years it, was one of the most important areas producing high-grade crude oil in Oklahoma. Although some oil has been recovered from each of 10 separate reservoirs, nearly 98 percent of the total recovery of oil in the area studied was from 4 major producing formations (table 1). A comprehensive study of the effects of hydraulically fracturing water-flood wells in Oklahoma has been made by the Federal Bureau of Mines. This report presents results of that study and an analysis of some of the factors governing the results of fracture treatment on waterflood properties. Originally, hydraulic fracturing was used to stimulate oil production from old wells producing by primary methods, and its application to water-flooding was regarded with skepticism by most producers. However, since 1953 the number of fracture treatments of both oil and input wells in waterfloods has increased steadily. Hydraulic-fracture treatments in waterfloods have generally been successful in stimulating production from oil wells and increasing the rate of injection into input wells. Hydraulic fracturing has enabled several producers to turn probable failures into economically successful projects.

The integrity of mine structures, such as the roofs, ribs, face and supporting pillars, is difficult to assess beyond the exposed surface. To mitigate potential rockfall hazards, the U.S. Bureau of Mines is assessing the usefulness of Rayleigh wave dispersion analysis to detect damage and stress-relieved zones in mine structures. Classical surface wave dispersion modeling is utilized at both laboratory and field scales in damaged and undamaged material to assess the integrity of a mine pillar and roof. To more fully exploit Rayleigh wave propagation characteristics, derivation of a novel dispersion parameter was conducted using a convolutional model. The development of a suitable Rayleigh wave dispersion system based on insight derived from forward modeling is presented. The application of inverse modeling for damage and stress assessment in the mine environment is also discussed.

The solubility of carbon in electrorefined vanadium was determined to be approximately 0.27 wt pct at the eutectic temperature 1,650° C and 0.03 wt pct at 700° C. The tensile strength and hardness of vanadium increased with car-bon content to the solubility limit, but this effect was small, probably because of the narrow solubility range. Between the solubility limit and 0.3 weight-percent carbon, the second phase had essentially no strengthening effect in vanadium. The second phase, observed metallographically at the grain boundaries and within the grains, was shown by autoradiography to contain high-carbon concentrations. This second phase was identified as VgC by X-ray analysis. The VgC in the grain boundaries contributed largely to the poor cold-and-hot workability of vanadium-carbon alloys. Increased concentrations of V2C occurred at the grain boundaries of the high-carbon alloy ingots as a result of homogenization at 1,600° C for 1 hour. Alloy ingots containing more than 0.36 wt pet carbon (a calculated 2.9 wt pct VaC) following homogenization, cracked severely and failed by intergranular fracture upon swaging at 900° C.

The Interior Department's Bureau of Mines is the Federal Government's primary research arm in the mineral and fossil-fuel fields. In accordance with its Organic Act of 1910, the Bureau's responsibilities have been reinforced since the passage of the Federal Metal and Nonmetallic Mine Safety Act of 1966, the Federal Coal Mine Health and Safety Act of 1969, and the Mining and Minerals Policy Act of 1970. The production of coal from underground mines in Washington declined during the recent war owing to shortage of manpower, and coal was shipped to the Pacific Northwest from mines in the Rocky Mountain region. The movement of this coal into Washington required the use of railroad equipment that otherwise could have been used for transporting war materiel and personnel. Strip mining uses a minimum of manpower, and the development of coal areas in the Pacific Northwest that can be mined by the strip method should increase production in the area, thus releasing railroad equipment from the necessity of transporting coal long distances and also providing consumers with a lower-cost product. The development of a strip mine to supply the demand at low cost requires a large investment in heavy equipment. In order that the fixed charges due to large investment in plant and equipment may be kept at a minimum, large reserves of coal must be proved. A reconnaissance of an area near Toledo, Lewis County, Wash., indicated a large bed of lignite that could be mined by strip methods. The area is approximately midway between Portland, Oreg., and Tacoma, Wash. (refer to fig. 1), and the opening of a large mine in the area would supply fuel to these and other nearby centers.
