Occupational health and safety is regulated by statute and common law. South Africa has, like many other countries, legislated extensively on occupational health and safety. Despite the body of legislation, the common law continues to play an important role as far as it concerns contractual, delictual and criminal liability. Two major Acts regulate occupational health and safety viz. the Occupational Health and Safety Act, No. 85 of 1993 (?OHASA?) and the Mine Health and Safety Act, No. 29 of 1996 (?MHSA?). The MHSA applies to mines and works as defined and related aspects. OHASA applies to other industries but does not apply to employers and workplaces to which the MHSA and certain matters covered by the Merchant Shipping Act, No. 57 of 1951 apply. See section 1(3) of OHASA. 2 The employer?s health and safety obligations in terms of the MHSA 2.1 Section 2(1) of the MHSA provides amongst others, as follows: ?The employer of every mine that is being worked must: (a) ensure, as far as reasonably practicable, that the mine is designed, constructed and equipped: (i) to provide conditions for safe operation and a healthy working environment (ii) ... (b) ensure, as far as reasonably practicable, that the mine is commissioned, operated, maintained and decommissioned in such away that employees can perform their work without endangering the health and safety of themselves or of any other person?.

INTRODUCTION The design/build (D/B) philosophy of contract delivery is neither new nor unusual when it comes to at-grade, elevated or high-rise structures. But when it comes to constructing underground facilities, there were no examples to be followed in early 90sin the continental United States or its territories. When DMJM+HARRIS was selected as general architectural and engineering consultant for the Tren Urbano (TU) project in Puerto Rico, one of the requirements of the Federal Transit Administration (FTA) for funding commitment was to use TU as a demonstration project utilizing design/build contract delivery method. Now fifteen years later, New York City Transit (NYCT) has instituted a similar requirement for their first Tunnel Boring Machine (TBM) contract for the Second Avenue Subway. As with most construction mega-projects, lots of things have happened and there were several lessons learned from the TU experience that are now being applied to Second Avenue. Both of these projects require building tunnels and underground transit facilities in heavily traveled corridors of an urban city, in the vicinity of active business and commercial enterprises. To enhance the challenge, there are noise and vibration sensitive and historic urban receptors either over or along both the transit corridors. Column free Tren Urbano station caverns over18 m (60 ft.) wide were built in soft ground conditions within 3.6 m (12 ft.) of the surface under an aging heavy utility corridor, while NYCT tunnels with station structures stretching the limits of rock caverns to 34.8 m (100 ft.), were planned in rocky geology with the presence of shear and fault zones and some mixed face conditions. BACKGROUND During the early period of design/build experimentation, the speed with which the contract procurement packages could be put together enabled the owners to take advantage of the major efforts underway to rebuild the infrastructure that was suffering from post-World War II neglect. Use of design/build for new starts was unheard of and most of the projects were smaller in nature in order to allow the methodology to be tested. The combination of Stage 1 Geoinformatics legacy data intervention processes with the Stage 2 ultra-detailed airborne geophysical prospecting demonstrates a new approach to converting data to information to knowledge, and converting that knowledge into exploration results.   The Coromandel region of the North Island of New Zealand has long been know to host gold deposits of economic viability (Hauraki Gold Fields). To the SE of this area lies the Taupo Volcanic Region (TVR) where the geology and structural setting mirror the Hauraki Gold Fields, however mineral exploration in the past has been hindered by the blanketing volcanic ash deposits. Glass Earth Limited recognised that this whole potential epithermal gold province could be effectively explored utilising rapid, risk orientated processing to review legacy geoscience data: The Geoinformatics Data Intervention Process; which in turn directed ultra detailed airborne geophysical surveys over the TVR. Results have outlined 21 new drillable potential epithermal gold targets; and a plethora of other targets with high prospectivity.

The Menominee Range of northern Michigan was a major producer of direct shipping iron ore for nearly a century. When the last underground mine on the Range ceased operation in 1978, it marked the end of an era. For since the start of mining in the early 1880's the district has gone through a Lull cycle from the start-up of mining through close-down. During this period over 200 million long tons of iron ore were produced and shipped from nearly 100 mining operations. This development and production spanned the time from when the United States advanced from an essentially rural-agriculatural society through the industrial revolution and into the beginnings of the computer and space age. There are many lessons that can be learned from such a rich history. This report attempts to provide ouch a lesson. Through the use of case histories, deficiencies in past mining practices and mine closure procedures that have resulted in current mine related problems are documented. These problems -an be grouped into three areas; 1) subsidence 2) hydrological or drainage problems and 3) problems related to inadequate mine shaft sealing and capping practices.

A wide-ranging base of jig data was collected from various types of commercial jigs in 9 coal preparation plants in China, and a complete data analysis was made. It was found that the sharpness ofseparation was very variable, which demonstrates the complex interplay of the many factors which influence jig performance. Tests showed that there exist optimum frequency ranges in each compartment of a jig. Stepwise regression analysis indicated that the sharpness of separation is most sensitive to the intake fraction ofthe air cycle, the frequency, and the feed rate. By means of non-linear curve fitting techniques and statistical analysis, several mathematical models were developed to describe both partition curve and the jig performance, for simulation purposes.

"Gravity concentration remains the main separation method for fine iron ore and is used extensively for treating various minerals. Gravity concentration methods separate minerals of different specific gravity by their relative movement in response to gravity and one or more other forces, the latter often being the resistance to motion offered by a viscous fluid, such as water or air (Wills, 2006).Spirals concentrators are one of the gravity concentration equipment used for beneficiation of minerals. Spirals are typically used for processing of fine minerals (-1mm) size fraction. The main operating variables for a spiral concentrator are the feed rate, the solids concentration, the wash water addition and the position of the cutters or splitters used to separate the tailings, middling and concentrate streams (Sadeghi, Bazin & Renaud, 2014).The aim of this study is to investigate the effect of feed rate, solids concentration and wash water on grade and recovery on iron ore material. The Multotec SC20HC/7 WW spiral was used for the experimental test work. The spiral was tested at varying solids concentration and feed rate. The wash water rate was also varied to establish the best washing rate. The splitter positions were kept constant. Optimization of these variables is expected to provide optimum conditions for a good compromise between grade and recovery.INTRODUCTIONSpiral concentrators are simple low energy consuming devices that separate minerals mainly on the basis of density. Spirals are widely used in mineral processing as a method for preconcentration and have proven to be metallurgically efficient and cost effective.Spirals have a wide application, they are used in iron ore processing, coal, gold, chromites, mineral sands, glass sands, and in soil cleaning.Separation on a spiral is achieved through a combination of forces that act on particles as they move down the trough of the spiral. The main forces known to act on the particle on a spiral are the gravitational forces, centrifugal force, hydrodynamic drag, and lift and friction forces (Kapur & Meloy, 1998). Apart from the forces acting on a spiral, the properties of the slurry flowing on a spiral including, solids concentration, feed rate and wash water also plays an important role in the separation on the spiral.The relationship between separation efficiency and feed rate was shown to be linear, with a decrease in efficiency at higher flow rates. Previous work on flow profile by Holland - Batt (1990) indicated that any increase in the feed rate to a spiral results in a greater part of the additional flow volume reporting to the outside of the trough. The flow on the outside of the trough becomes energized and consequence to that the heavy particles find it hard to settle and escape this region to the inner region. This effect has a high impact on recovery of heavy minerals, since any increase in volumetric flow on the spiral is not made available for recovery in the inner region of the spiral."

The Virginia State Capitol was designed by Thomas Jefferson and constructed in 1785. The Capitol is of great historic and architectural significance to not only the Commonwealth of Virginia but to the nation. It is the oldest operating Capitol in the U.S. and required a total restoration/renovation as well as an expansion to continue to remain functional. Site conditions dictated that the expansion (a new visitor center) be placed underground on the south side of the Capitol building, requiring a 40-foot deep excavation approximately 5 feet from the Building. Because of its historic significance, every measure had to be taken to ensure the safety and function of the Capitol during construction. The construction techniques that were used to make the deep excavation and protect the historic Capitol included a tied back concrete slurry wall, jet grouting, compensation grouting, and instrumentation monitoring. This paper describes the history of the Capitol building, the techniques that were employed for construction of the deep excavation, results of the latest movement monitoring, and factors contributing to meeting the restrictive movement goals.

The data reported in this bulletin are largely the result of experiments conducted by the Salt Lake City station of the Bureau of Mines in cooperation with the department of metallurgical research of the University of Utah. The work on lead is only one of the phases of the investigations carried on at that station during the years 1914 to 1916. The results of similar work with zinc ores and with copper and other ores will be published in future reports of the bureau.

According to the data provided by the most recent European Survey on Working Conditions, roughly 25 per cent of the European working population is exposed to mechanical vibrations for no less than one quarter of the work shift duration, while around ten per cent are exposed for the entire work shift. The occupational exposure to whole-body vibration (WBV), in particular, represents a major health risk in mining, quarrying and civil excavation activities. In accordance with European Directive 2002/44/EC, the latest version of the Italian law on physical risks has recently confirmed the possibility of using relevant literature data on vibration magnitude in order to evaluate personal occupational exposure (vibration database or data provided by the manufacturer). Still the alternative option, based on field measurements, represents the most appropriated method of risk assessment, as it accounts for the actual operating condition at the workplace; actual operator and vehicle under investigation and specific working cycle. The article discusses the vibration measurements taken on a number of heavy machinery (excavators, loaders, trucks, dumpers, etc) employed for talc extraction in a mine located in Sardinia and the resulting personal exposure for the drivers. The assessment was carried out in accordance with the specifications of ISO 2631-1 and EN 14253.

"Enterprises are faced with increasing economic competition and managers are obliged to look for methods that will ensure a competitive edge in their companies’ markets. These methods include managerial concepts that employ common sense and are low cost. One of these approaches is the KAIZEN methodology, of Japanese origin; kaizen means continuous improvement (CI) when translated. A mining company, despite some of its idiosyncrasies, is just an enterprise where the application of KAIZEN can be advantageous. The company investigated, OKD, is the only domestic producer of hard coal in the Czech Republic and operates four mines that produce 11 Mt of coal annually. The OKD management team opted for KAIZEN as their principal method of continuous quality improvement. This paper suggests ideas for development and application of processes that provide for continuing improvement of production and management. The authors have taken the OKD Company to be their benchmark in the field of mining activities. The paper also focuses on possible difficulties accompanying the introduction of CI sustainable methods. Three years of application of CI methods has increased the income of the company by almost US$38.1 million, which provides a strong argument for continued application."

A method is derived for obtaining the distribution of sphere diameters and plate thicknesses (or pearlite spacings) from size distribution functions obtained along randomly oriented lines. The method described offers several advantages over existing methods. IT is often of interest to measure not only the average particle size but also the distribution in sizes of particles dispersed in an opaque material, such as an alloy. The Bureau of Mines conducted surface joint and stress measurements, airborne remote sensing imagery studies, compass-oriented borehole televiewer surveys, numerical simulation of fractured wells, and well productivity studies in a four-quadrangle study area of Ohio, to determine the effects of earth fracture systems on the deliverability of gas storage fields. The results of these studies indicate that much useful information about a gas storage reservoir can be obtained and put to practical use by studying--(1) oriented cores for fractures and directional permeability, (2) orientation of induced hydraulic fractures for alignment of well patterns, and (3) aerial photos and multispectral scanner imagery for general surface geology comparison and to determine if some, development patterns can be generated by such results. The absence of known tectonic elements associated with flat-lying sediments, such as faults or fracture zones that can be seen on the surface, indicates that the use of remote sensors such as radar or aerial photography is not likely to be useful in picking well locations for drilling. Studies similar to those discussed here should be conducted in gently folded and strongly folded sedimentary formation areas to determine if the increased knowledge of earth fracture systems under these conditions might be beneficial in gasfield and gas storage reservoir development.

A study was conducted to investigate the damage potential to underground multiseam openings caused due to vibration generated by open-pit coal mine blasting. The objective was to recommend optimal explosives weight per delay to be detonated in open-pit blasts to ensure the safety and stability of underground openings. Sixteen blasts with varying charge weights and initiation sequences were conducted at Samleshawari open-pit mine and eighty four blast vibration data were recorded in the Hingir Rampur underground colliery in India. Analysis of vibration data showed that roof of the underground openings vibrated with much higher amplitudes compared to those in pillars and floors. The amplifications of vibration in the roof at the junction were upto 4.3 times the pillar vibration. It was observed that coal chips detached from roof when the peak particle velocity reached 131 mm/s or higher. The vibration magnitude recorded was higher in upper-seam openings than in belowseam openings. Rapid attenuation of vibration was recorded in below-seam openings at similar locations. Appropriate particle velocity-charge weight-distance equations were established to calculate the safe explosive weight per delay to be detonated in open-pit mine for the safety of multi-seams workings of underground mine. The support for the galleries and junctions were further modified for the working seams to ensure greater safety.

Increased mechanization .of coal mines has emphasized the need for study of actual power requirements of the various machines employed in the production of coal and losses in the distribution of power to these machines. These studies are being made in order that hazards, excessive use of power, and high maintenance cost of equipment resulting from these losses may be analyzed and eliminated where possible. Research on the use of electric power in-coal mines was suggested by the Advisory Committee of the Bureau of Mines in 1937, and it was decided to begin with a study of individual machines of various types under operating conditions in modern coal mines. The transmission of power, particularly line losses and inadequate return circuits, should be a part of the investigation. This report presents some of the data of an initial study in commercial mines. ACKNOWLEDGMENTS The superintendents, engineers, and other officials of the two mines studied cooperated fully and aided in setting up and transporting the test equipment, giving such information as was requested.

South Africa?s Mining Industry operates in an uncertain world. The industry is buffeted by problems regarding retrenchments, unemployment, low productivity, rising costs, volatile exchange rates, commodity prices and government regulations. Modelling languages with risk analysis have been available for decades, but in the past their use has been very expensive, requiring large mainframe computers and lots of computer time. Today, PCs and laptops have incredible power, which makes interactive risk analysis affordable and viable for all enterprises, whether small or large. ?What is risk analysis? ?What is Monte Carlo simulation? ?What kinds of computer models need to be created to use risk analysis and Monte Carlo simulation effectively? This paper addresses these questions and illustrates, with a practical example in the mining industry, how risk analysis techniques can be used to support decisions in the climate of uncertainty.

The emergence of new steel grades is largely driven by the need to produce high quality steel at a competitive price. The principal aims are to attain ultra-high purity, (especially low interstitial elements) and ultra-cleanliness (by the control of the inclusion content and size). Whereas the steel used to build the Titanic contained 650 ppm sulphur, resulting in MnS stringer-inclusions 25 mm in length, modern quality requirements for arctic pipelines would only allow 10 ppm of sulphur and inclusions in ball-bearing steels must be smaller than 5µm. Most developments are aimed at hot-metal treatment before refining and ladle treatment after decarburisation. Because ferroalloys are added during or after the refining step, it is the secondary (ladle) refining production technologies that are of specific interest to the ferroalloy producer. Through ladle treatment, hydrogen and nitrogen contents are reduced, ultra-low levels of solute impurities are attained and the shapes as well as the composition of inclusions are modified. Steelmakers are concerned about the cost effectiveness of the use of ferroalloys such as consistent product analysis and delivery, improved financing arrangements and technical support. Minimal variation in product sizing is vital because it impacts on handling problems and because particle size determines the rate of particle dissolution. The required ferroalloy product quality depends on the specific use but in general sulphur, phosphorous and nitrogen contents are of concern. Apart from the presence of trace and impurity elements, ferroalloys also contain inclusions. For example, ferromanganese may contain MnO-MnS-SiO2 inclusions while high-carbon ferro-chrome may contain different kinds of Cr-Mn-spinels. The presence of these impurities and inclusions has a significant impact on steel production techniques and cost and hence, on the selection and use of ferroalloys. The interrelationship between ferroalloy quality and steel production technology is plant and product specific and hence, effective communication between ferroalloy suppliers and steel producers is essential.

An inter-disciplinary geotechnical study has been carried out in an Austrian magnesite mine. The paper describes the how geological data, in situ and laboratory tests as well as subjective, empirical, geophysical and numerical methods have been used to identify critical areas in the mine. Middle Proterozoic sulphide ore mineralisation occurs in isolated blocks of the NE-SW trending Delhi fold belt composed of Delhi Supergroup rocks. The Ajari-Basantgarh-Pipela belt forms part of the longest, widest and westernmost tectonic unit of the Delhi fold belt and consists of amphibolites, metapelites, quartzites and carbonates. The metapelite lenses and associated amphibolites host pyritic copper mineralisation. Mineralisation takes the form of sheet-like bodies consisting of ore lenses of various sizes interlayered with host or associated rocks. Massive, banded and disseminated varieties of ore are found. Pyrite is dominant with subordinate chalcopyrite and sphalerite. Pyrrhotite, galena, cubanite, mackinawite, marcasite and freibergite constitute the minor minerals. The characteristic features of the ores suggest a possible correlation with Besshi-type deposits, and the closely associated amphibolites are interpreted as low-K tholeiites of a fore-arc setting

Today, the mining and mineral sectors are being impacted by highly fluctuating supply ? demand, extreme price volatility and tremendous market uncertainty on a world-wide basis. This pattern is expected to continue for the foreseeable future. In particular, large consumers of tonnage quantities of industrial gases (oxygen, nitrogen) all have to make informed investment decisions in this uncertain economic environment. Plant owners ? operators need to carefully examine their current and future industrial gas needs in order to develop the most cost-effective industrial gas plant configuration. This can be done by partnering with an experienced engineering company that offers a complete technology portfolio for producing industrial gases. Ideally, the mineral and engineering companies have to work together as a single team to identify the most efficient and economic plant design. This includes an in-depth assessment of capital and operating costs, in order to deliver an optimal solution, taking plant reliability and process safety into account. It is important to consider all the industrial gas needs at a given manufacturing complex, in conjunction with available utilities and infrastructure to develop the best possible long term solution. Oxygen (O2) requirements include: oxygen enriched and oxy-fuel combustion, smelting and hydrometallurgical operations. Nitrogen (N2) is used for annealing. All of these industrial gas requirements have to be examined at a given site over a long term horizon in a holistic manner. Cryogenic air separation was first practiced at a commercial scale in 1902. Since then, the technology has been constantly refined to reduce cost; and improve reliability and flexibility. Today, using advanced engineering techniques, including proprietary process simulation, and modern fabrication techniques, it is possible to build air separation plants that are both low cost and high quality at the same time. It is important to work with an engineering company that owns a complete portfolio of industrial gas technologies, functional engineering capabilities and a variety of plant solutions. Such a partnership can quickly identify a ?sweet spot? for a given situation. By drawing on experience gained from hundreds of projects, it is possible to quickly optimize a given requirement. In situations where a customer wants a complete solution it may be appropriate to provide an air separation plant on a turn-key basis. This will enable the mining and mineral processor to focus on their core business. By using global procurement and construction capabilities it is possible reduce cost and improve project execution, thereby making industrial gas plant procurement simpler and cheaper for the mining and mineral sector.
