Resin bolting has enjoyed wide acceptance for coal mine roof support. Beginning in 1973, the first year of significant resin bolting in coal mines, we estimate that 2.5MM resin bolts were installed. This grew to 7MM resin bolts in 1974 and 13MM in 1975. Resin bolts are currently being used in over 300 coal mines throughout the major coal producing areas of the U.S. and barring major work stoppage we expect more than 17MM will be installed in 1976. Although not a cure-all, the number of unintentional roof falls occurring in many mines has been dramatically reduced as a result of converting to this "new" method of support. 1 The improved anchorage strength of resin bolts provides better roof support. (Slide #1) This anchorage strength provides a mechanism to resist changing roof stresses along the entire length of the fully grouted resin bolt, thereby assuring stabilization of rock strata over long periods of time. The ability to resist roof displacement is borne out by readings obtained from strain gauged bolts. These findings can be summarized as follows.2

Cerro Colorado lies on the eastern edge of the Atacama Desert in the foothills of the Andean Cordillera of northern Chile. Compañía Minera Cerro Colorado SA (CMCC) operates Cerro Colorado. CMCC, headquartered in Santiago, Chile, is wholly owned by Rio Algom Ltd., of Toronto, Ontario. Cerro Colorado is located 120 km (75 miles)east of the port city of Iquique. Access is by 30 km (19 miles) of paved highway and 70 km (43 miles) of upgraded road. The mine site is located at an altitude of 2600 m (8531 ft) on a gentle west-sloping plateau adjacent to a deep canyon (Quebrada de Parca) to the north. Temperatures are moderate, ranging from 5° to 30° C (41° to 86° F) with an average annual rainfall of 20 mm (0.8 in.). The plateau is generally covered with 20 to 30 m (65 to 100 ft) of Quatemary sand and gravel overlaying bedrock. In the immediate vicinity of the Cerro Colorado deposit, a semicircular embayment has been eroded into a steep canyon wall that cuts through the surface gravels to expose a considerable section of the underlying bedrock.

The power industry throughout the world is in the midst of a significant transition that will result in a major restructuring. Deregulation of utilities in the United States is continuing, resulting in increased competition among power providers. The traditional ways that electric power is globally bought, sold and delivered will also change. However, according to the Department of Energy's (DOE) Pat Godley, one thing is clear: "Those who provide the power during the 21 st century will be those who are willing to change - those who are willing to accept new ways of doing business and willing to employ new technologies." Godley is assistant secretary of fossil energy with DOE. Her comments came during the opening remarks at the Fourth Annual Clean Coal Technology Conference, held in Denver, CO, Sept. 5-8, 1995. The conference was sponsored by DOE and the Center of Energy and Economic Development. Under the theme "Global Opportunity," more than 300 coal and utility executives spent three days discussing the new face of the electric-power industry throughout the world and the new, mostly clean coal technologies (CCT) that will be available to power providers.

INTRODUCTION Spokane Mining Research Center (BuMines) is researching sealants for underground coal mines to: 1. Reduce sloughing of shale roofs. 2. Reduce sloughing of coal ribs. 3. Seal block stoppings against air losses. 4. Construct stoppings by spraying over jute cloth or metal lath. 5. Render incombustible in-place polyurethane foam. Originally the work was directed towards reducing sloughing of shale roofs. Many underground coal mines have shale directly over the coal which weathers severely after exposure to the mine atmosphere. Lab investigations(l, 2, 3, 4) have shown that many shales separate into layers when immersed in water or stored in a high relative humidity environment. Upon drying or after removal from high humidity, the shales continue to separate. Underground coal mines become wet in the warmer months from condensation of the ventilating air. Conversely, the mines become dry in the cooler months from evaporation. The mines experience severe shale break-up during the summer and much less break-up during the winter. Figs. 1, 2 and 3 show weathered shale roofs. in underground coal mines. Fig. 4 shows fallen shale on the entry floor. Sealing the shale reduces this sloughing problem.

The McDonald gold deposit is located in the upper Blackfoot Valley in Lewis and Clark County in west-central Montana, about 65 km (40 miles) northwest of Helena and 13 km (8 miles) east of Lincoln (Fig. 1). The McDonald project area covers about 20.7 km2 (8 sq miles) of elevated terrain on the north side of the Blackfoot Valley. The Blackfoot River lies 1 km (0.6 miles) south of the McDonald deposit. East Butte and West Butte rise 300 m (985 ft) above the Blackfoot Valley to an elevation of 1,750 m (5,740 ft). They conveniently divide the area into two parts. The Seven-Up Pete Joint Venture (SPJV) is a partnership between Phelps Dodge Mining (72.25%) and CR Montana, a subsidiary of Canyon Resources (27.75%). SPJV controls about 83 km2 (32 sq miles) of mineral and surface rights in the Lincoln area. The McDonald project is on state-owned and private land. The joint venture has conducted exploration since 1989 at the Seven-Up Pete, McDonald and Keep Cool areas (Fig. 1).

INTRODUCTION Los Angeles is one of the world's largest cities in area with a span of 70 km between its north and south limits. The population in the Los Angeles Basin exceeds 11 million people. Los Angeles is second only to New York City in density of U.S. urban population and it operates one of the largest city bus fleets in the world, 2,500 buses. After many years of planning and many consultants' reports, it is planned to initiate a 30 km (18.5 miles) starter line subway -- the "Wilshire Corridor" (Figure 1). Construction is to start in 1985. In several locations, subway routes ultimately will skirt or cross old oil fields, which may cause gassy tunneling. Of particular note are the La Brea Tar Pits, which overlie a part of the old Salt Lake oil field. Tunneling through tar sands for about 3 km will be unique for subway construction and will occur on the starter line. Experience with previous gassy tunnels in the greater Los Angeles area shows that tunnels can be completed safely if precautions are taken. Abundant ventilation is the main solution, although other precautions are discussed herein.

This paper describes the techniques implemented at Randfontein Estates to develop twin-haulages at 'high speed'. An operation which culminated in the world record advance for twin-end multiblast cyclic development in hard abrasive quartzites. A major expansion project was planned for the mine, with the sinking of a third twin-shaft system (to be known as Cooke No. 3), to raise the level of ore production from 3 500 000 to 5 000 000 tonnes per alumm. Subsequent redesign of the proposed shaft system as a single 9 m brattice wall shaft took place, reducing the planned shaft sinking period by 12 months and saving an estimated R 250 000 (U.S. $ 330 000) in sinking costs. The advancement of the Cooke No. 3 Shaft sinking and development programme necessitated a rapid excavation schedule for two interconnecting twin-haulages from the existing Cooke No. 2 twin shaft complex to provide a second means of egress. An early holing between the two shafts would provide significant financial and technical benefits. Pre-development and equipping of shaft pillar excavations would enable a rapid shaft production build up and blocking out of at least one year's ore-reserves.

Brenda Mines, located 22 km Northwest of Peachland in British Columbia, Canada was an open pit copper- molybdenum mine which closed in 1990 after 20 years of operation. The primary concern in Brenda's tailings and waste rock drainage is molybdenum at a concentration of approximately 3 mg/L. The mine drainage is alkaline and contains little or none of the typically problematic heavy metals. Given that the waters downstream are used for municipal water supply and some irrigation, a discharge limit of 0.25 ma molybdenum was imposed with specific water quality guidelines in the receiving creek. A review of all existing and potential molybdenum removal methods was undertaken prior to mine closure. The chosen process is a two-step iron co-precipitation with clarification and sand filration at a slightly acidic pH. A 4,000 usgpm (912 m3/h) treatment plant was constructed and commissioned in 1998, at a cost of $10.5M. The successful removal of molybdenum from the drainage water is explained with details on some design innovations and operational challenges encountered during plant start-up. Investigated sludge disposal options are discussed although the long term disposal scenario has not yet been finalised.

Many coal preparation plants discard fine coal (generally minus 100 mesh) because it is difficult to beneficiate economically. At the Kentucky Energy Cabinet Laboratory (KECL), the waste streams from several Kentucky preparation plants were evaluated for cleaning with counter-current column flotation. A series of performance evaluations were conducted using a 0.05 m (2 in.) internal diameter (I.D.) 7.9 m (26 ft.) high laboratory column. The objectives of these tests were to assess the performance in terms of recovery and percent ash in clean coal and to optimize the operating parameters relative to combustible recovery and grade. Refuse streams containing 30-50 percent ash were upgraded to 2-8 percent ash with Btu recoveries of nearly 90 percent. A pilot scale column flotation cell was installed at the Bell County Coal preparation plant. This pilot plant utilized a 5.79 m (19 ft.) high and 0.15 m (6 in.) I.D. column to verify the performance data of the 0.05 m (2 in.) I.D. column for two different waste streams. Performance evaluation and pilot plant testing results related to scale-up are discussed.

Sedimentation in highly, flocculated suspensions of fine particles, such as clays, occurs through individual settling units (flocs) that have very open structures and correspondingly low densities (typically less than 5% solids by volume). Such settling processes rapidly lead to the buildup of a bed of flocs that are essentially close-packed, with an inter-floc porosity of perhaps 50%. However, the overall solids content is no more than about 2% to 3% by volume. Consequently, the thickening process is dominated by the bed consolidation rather than by the simple settling of the individual flocs. Experimental measurements of sediment consolidation in flocculated clay suspensions indicate, for a given system, that consolidation rates are essentially determined by the solids concentration and can be evaluated using the classical Kynch treatment. However, these rates are strongly dependent on the physical, as well as the chemical, conditions of the flocculation process. Furthermore, the extent to which gravity consolidation proceeds depends on the total weight of the sediment (i.e., on the bed height and the solids concentration). The implications with respect to thickener operation and the requirements for the laboratory testing for thickener design are discussed.

The U.S. Bureau of Mines (USBM) and Diesel Controls Limited evaluated a new diesel emission control system on an underground mine vehicle. The system was a catalyzed ceramic wall-flow diesel particulate filter (CDPF) combined with an catalytic converter, or diesel oxidation catalyst (DOC). This paper describes the design and installation of the system on a load-haul-dump (LHD). The results of screening tests conducted by the USBM are also given. The screening tests were conducted to determine if the device's particulate collection efficiency, regeneration temperature, and effect on gaseous hydrocarbon (HC) and carbon monoxide (CO) emissions changed over the period it was being used. The system was removed from the LHD and evaluated in the laboratory after operating for 308 and 1200 hours. After 308 hours of operation, laboratory testing revealed its particulate collection efficiency varied from 41.0 to 93.5 percent, depending on engine operating conditions. Its regeneration temperature was about 415° C. It reduced HC emissions by up to 97.6 percent, and CO by up to 95.0 percent. When tested in the laboratory after operating for 1200 hours on the vehicle, no significant change in performance was observed.

Around the beginning of the 20th Century, annual aggregate production in the United States amounted to about 50 million short tons. Today annual aggregate production is nearly 2.5 billion tons. Over a third of the States in the country produce more aggregate than the Nation did at the turn of the century. I would like to describe some of the historical events that allowed this nearly 50 fold increase in aggregate production. You might wonder why I want to talk about history. Some of our best ideas lie buried in the past, waiting to be rediscovered in the present. Almost every issue that the aggregate industry experiences today has been recognized for at least a decade; many of those issues have been recognized for a half a century; and a few of them have been recognized for nearly a century. The main reason for this article is to save the past from the forgetful indifference of the present-to faithfully preserve the past, to illustrate the insights of our predecessors, and to demonstrate how little of what we know today is really new.

Introduction Compared to the gyratory crusher, the cone crusher is character¬ized by its higher speed and a flat crushing chamber design which is intended to give a high capacity and reduction ratio for materials suitable to this type of processing. The aim is to retain material longer in the crushing chamber to do more work on material as it is being processed. A concept of how a piece of stone might flow through a secondary crushing chamber is shown in Fig. 45. The number of times material will be nipped during crushing will depend on material size, friability, and the geometry of the crushing chamber as well as its speed and eccentric throw. Cone crushers are usually specified in terms of closed-side setting (CSS) and a given quantity of material passing a particular square-mesh screen size. A criterion of product passing closed-side setting is often available. This value will vary according to the particular crusher design and the details of its applica¬tion. Essentially, cone crushers can be classified into distinct types, depending on their duty. Crusher sizes for all duties are described by terms arising out of common industry usage, such as 3-ft, 7-ft, etc., which refer to the crushing-head (mantle) diameter. At present the range of sizes available varies from approximately 2 to 10 ft diam. Weights vary in the range of 5 to 200 tons, and connected horsepowers range from 10 through 700. There are so many variations that specifics should be obtained from machine manufacturers. The crushing chambers that are fitted to various machines are also often referred to by terms arising out of common usage, such as standard for secondary crushers, and short head for tertiary crush¬ers, which refer specifically to the Symons design. Also used are more specific but equally vague descriptions, such as coarse, intermedi¬ate, and fine chambers. Some manufacturers such as Allis-Chalmers with their Hydrocone crushers refer to the model number of the machine by the feed opening and the diameter of the mantle. For example, a Model 10-84 Hydrocone crusher has an 84-in. diam man¬tle, and is capable of accepting a 10-in. diam sphere at the feed point to the crushing chamber. Rexnord and Telsmith define crushing cavi¬ties in their capacity tables as feed openings at minimum recommended discharge settings (closed-side settings). Each practice offers guidelines which should be carefully defined for the specific crushing problem. The utilization of a cone crusher depends on how well it is controlled and the features of the circuit in which it operates. Normally, it will operate in the range of 75 to 85% of the peak capacity. It is likely that industry pressures will call for a standarized de¬ applied crushing power. Experience has shown that the crushing process is more controllable if certain reduction ratios are adhered to for each stage. Technically, this is defined as that size of feed (in. or mm) of which 80% will pass, divided by the size of the product (in. or mm) which 80% will pass. Secondary Crushers These are the cone crushers which accept suitable feed size material either prepared by a primary crusher or occurring naturally, and reduce it to a size suitable for marketing in one stage, or make feed for subsequent crushing or grinding stages. Secondary crushers have feed openings of 4 to 25 in. in the larger (7-ft) models down to a 2'/a to 4 in. in the smallest 24-in. models. Reduction ratios normally range between 3:1 and seldom more than 5:1. In the 7-ft models, the cone crusher typically makes a product all passing 2 in., but this is dependent on the machine design and properties of the material being treated. Secondary crushers most commonly operate in open circuit. They also are operated in closed circuit with a vibrating screen, depending upon product requirements and conditions where the reduction ratio does not cause excessive power draw and build-up of circulating load. Screening ahead of secondary crushers is generally recommended, especially where the feed contains more than 25% material smaller than the desired closed-side setting. Tertiary Crushers These are cone crushers that normally take secondary crusher product and reduce it to a marketable product or make it suitable for subsequent comminution steps. The reduction achieved is a func¬tion of the crusher design and the properties of the material to be treated. The reduction ratio is normally in the range between 1.5 and 2 to 1, and seldom more than 3 to 1. The tertiary crusher normally operates in closed circuit with a vibrating screen and makes a product smaller than all passing 'h or 5/8 in. Fine Crushers Sometimes these machines are called sand crushers and are called by various manufacturer brand names, such as Gyradisc, Hydrofine, V.F.C., and others. A cross section of a Symons Gyradisc crusher is shown in Fig. 46. Essentially, these crushers would be used in a fourth-stage crushing operation, but could be called on to reduce a screened feed size fraction from a secondary or primary crushing operation. This type of crusher normally receives feed no coarser than 1'h in. that is scalped of all over-sized material and operates in closed circuit with a vibrating screen. For average materials, typical product is '/e in. top size, although a -10 mesh product can be produced on those materials having suitable characteristics. The crusher is nor¬mally operated in a separate circuit from the main crushing plant because of the variations in output rates that are caused by varying physical properties in the feed. The reduction ratio is generally less than 2:1, and circulating loads are generally high. More interparticle crushing takes place in these machines than with conventional cone crushers and capacities can be more sensitive to changes in moisture content, feed size graduation, and other physical properties. Design Features For the various applications, crusher manufacturers normally have a range of crushers of different sizes and power ratings to select from.

6.1 PRINCIPLES OF PLANNING GROUTING OPERATIONS FOR SHAFTS, DRIFTS AND TUNNELS The planning of integrated grouting of vertical, inclined or horizontal underground workings in saturated, fractured rock includes the following phases: 1. Planning for the collection and analysis of geological information concerning the characteristics of the struc¬tural deformation(s) that produced the details of the frac¬ture system being studied. 2. Planning for the acquisition of initial hydrogeological information produced by the flowmetric and hydrody¬namic investigations (described in Chapters 2 and 3). 3. Computing the initial engineering calculations of the ex¬pected or planned dimensions of the isolation curtains for each water-producing stratum (fracture zone) and the expected areal spreading pattern of the grout in each of these water-producing horizons. The selection of the lo¬cations, the initial optimal number of holes and the vol¬ume of grout needed to create each isolation curtain also are included in this phase. 4. The selection or development of a grout formula for the specific hydrogeological conditions that exist at the site. 5. Identification of a resource (deposit) required to produce the grouting clay. The planning of grouting operations must be supported by the acquisition of all necessary initial data, which in¬cludes: 1. The geographic location and administrative responsibil¬ity (ownership and regulatory constraints) of the mineral deposit and any peculiar features of the area where ex¬cavation of the underground workings is planned. The surface relief, the hydrographic network of the region, and the local climatic conditions must be evaluated prior to planning for buildings and waste discharges if any. 2. The economic status of the region, the available trans¬portation, the nature of the energy base, the water and heat supply sources, the location of construction mate¬rials suitable for making the grout all must be deter¬mined. 3. The geological and hydrogeological characterization of the work site must be accomplished. 4. The technical features of the shaft(s) and underground works must be characterized. 5. The organization's principal initial design plan features and the construction sequence of all buildings and struc¬tures must be developed. These must be correlated with the organization and schedule for excavating the under¬ground works. The technical characterization of a shaft or of other un¬derground workings must contain information about the purpose of the workings, the design depth, the planned excavation procedures along with cross-sections, and the parameters of the lining, if a lining is required. The geology of the section to be penetrated must include geological data obtained from existing exploration activities at the mineral deposit and conclusions about the shaft ex¬cavation conditions obtained from drilling technical moni¬toring holes and exploration drillholes. The interpreted stratigraphic regime, including the thickness, strike and dip of each stratum, must be shown in cross-section. The tec¬tonics, including all geologic structures, must be presented by cross-sections. The bedding orientations must be used to identify anticlines, synclines, monoclines and faults. The lithologic composition and the drillability of the rock com¬prising the section must be examined. The character of the rock's fracture pattern must be presented. Finally the per¬meability, porosity and storage characteristics of each aqui¬fer must be determined by in situ hydraulic property testing (injection tests or pumping tests, as described in Chapter 3). The fracture pattern of the indurated rock is presented in reports derived from geo-exploration borehole studies and from studies of outcrops on the surface and in all nearby underground workings. The extent of each fracture zone is mapped in three dimensions. The distribution of fracture apertures, the nature of the fractures according to each litho¬logic stratum, and the relationship of bedding planes to each fracture system are all determined. Acoustic logs, downhole cameras, caliper logs, oriented core, and any other appro¬priate geophysical tools available are used to construct the data base for this purpose. The hydrogeological characterization of the geologic section to be penetrated by the workings is based on the analysis of hydrogeological data collected during the drill¬ing and testing of technical monitoring holes and from in¬formation on ground water inflow collected during the ex¬cavation of nearby underground workings, if such workings exist. The hydrogeologic characterization must contain the following information: 1. The identification of all aquifers, their thicknesses and their locations in the section, 2. The distribution of hydrostatic head in the ground water in three dimensions, 3. The coefficient of saturated hydraulic conductivity (per¬ meability) of each aquifer, 4. The porosity and storativity of each aquifer, and 5. The projected rate of water inflow into the shaft, drift or tunnel for each aquifer prior to grouting. This step is used to identify the zones to be grouted.

INTRODUCTION Several major factors introduce uncertainties into the assessment of radon progeny exposure to miners using time-weighted average radon progeny concentrations: uncertainty in the measurement of radon progeny concentrations in specific areas, assignment of an individual miner's time to those areas, variation in radon progeny concentration between measurements and potential human errors involved in calculating concentrations and handling data. The currently available grab-sampling methods for determining working level were analyzed to determine the magnitude of the uncertainty due to each of these factors. For all measurement methods studied, the variation in the airborne concentration with time in operational areas of a mine is the dominant factor in the uncertainty in determining annual radon progeny exposures for individual miners. Uncertainties relating to accuracy of the method and precision of measurement were found to contribute a significantly greater portion of the total uncertainty than human errors. Under normal conditions, if the technicians performing the measurements are conscientious and well trained, human error contributes little to the total uncertainty of the radon progeny exposure determination. The primary goal of radiation monitoring is the reduction of radiation exposure to the lowest reasonably achievable level below regulatory limits. Monitoring personnel in mines should be trained not only to obtain accurate estimates of miner radiation exposures but also to recognize and, when possible, to implement correction of situations which result in unnecessarily high radon progeny exposures. ESTIMATION OF UNCERTAINTY DUE TO HUMAN ERRORS Human errors affecting the assignment of annual radon progeny exposure to individual miners can be placed in two categories: those related to the measurement of radon progeny concentration in specific mine areas and those related to estimation of occupancy time for individual miners and transcribing data to permanent records. The former are specific for the measurement method used; the latter are common to all methods. Errors in Determination of Working Level All systems for determining radon progeny concentration require measurement of several parameters, which include volume of air sampled, count rate and decay time. These quantities and appropriate constants are used in a basic equation, specific to the system, which estimates working level. An unintentional random mistake in measurement of any one of these parameters or in the selection of proper constants will contribute to the uncertainty in the determination of working level. In our analysis of human error we separated each measurement method into a sequence of independent operations, with each step subject to operator error. For each operation we estimated the probability of occurrence and the consequence of errors to obtain a resulting uncertainty. Certain types of errors result in specific consequences. For example, we assumed that an error of 5 seconds in timing of a 5-minute sample results in a fractional error of 1/60 (1.7%). Other types of errors can result in a range of uncertainty. Transposing digits read from a scaler can produce errors ranging from near zero to approximately 60%. In these cases we calculated the statistical variance for the distribution of errors. We assigned the square root of the variance divided by the mean as the consequence factor for that type of error. This is essentially the same as a coefficient of variation. The product of the probability of occurrence and the consequence factor is the fractional uncertainty in the measurement due to that particular error. The total uncertainty due to human errors is calculated by taking the square root of the sum of the squares of the uncertainties generated by all manual operations. Uncertainties Due to Human Error for the Kusnetz Method One of the techniques most commonly used to estimate working level in U.S. uranium mines is the Kusnetz method. A generalized way to express the equation used to compute WL by this method is: WL = (Net Alpha Counts)/(V)(ST)(CT)(E)(K) where: V = sample flow rate in liters/min ST = sampling time in min CT = counting time in minutes E = absolute counting efficiency K = Kusnetz conversion factor (dis/min-L per WL), as a function of decay time in minutes. The example of human error analysis presented here is based on the Kusnetz procedure having a timing sequence of 5 minutes sampling time, 40 minute decay time, 2 minute counting time. During the sampling procedure a stop watch is used to determine the timing interval. We assume that it is common to make small timing errors of a few seconds, but larger timing errors occur infrequently. Errors greater than 30 seconds are considered to be essentially non-existent since we assume that the

Introduction Coal mining in Alaska's interior, specifically in the Healy area, began as early as 1918 with the construction of the Alaska Railroad. Mining was originally limited to underground operations but has expanded to entirely surface operations. In 1943, the Usibelli Coal Mine was formed and started developing Alaska's first surface mine east of Suntrana (Usibelli Coal Miner, 1984). Production from the local coal deposits has steadily increased and, in 1978, surface mining of Poker Flats was initiated (Fig. 1). Currently, a 25-m3 (33-cu yd) walking dragline strips two coal seams, using an extended bench on the second pass. In addition, a fleet of trucks and shovels are used for coal removal and some limited overburden stripping. In 1984, a contract was signed between Usibelli Coal Mine and Sun Eel Shipping Co. in 1984. Since then, production has nearly doubled to more than 1.3 Mt/a (1.5 million stpy). This article will discuss geotechnical constraints on mining within the steeply dipping coal deposits that exist within the Poker Flats mining area. Specifically, the article will describe how the mining operation retriggered an historic landslide on the No. 5 coal seam (Fig. 2). And the article tells how a mine plan was developed that allowed the coal to be safely removed without inducing additional movement. Regional geology The coal-bearing group in the Nenana coal field is of Tertiary Age. It is overlain in some areas by several thousand feet of Tertiary gravels - the Nenana Gravels. In areas mined by surface methods, the Nenana Gravels have been eroded off, and up to 30 m (100 ft) of quaternary outwash gravels overlay the coal-bearing formations. The coal-bearing group is divided into five formations: Healy Creek, Sanctuary, Suntrana, Lignite, and Grubstake (Wahrhaftig, 1969). Lignite Creek lies on the north limb of a west plunging anticline. This has brought the Suntrana coal-hearing formations near enough to the surface to allow surface mining. Mining is presently in progress on the south side of Lignite Creek in the Poker Flats area. The coal-bearing formation is cut off to the south by a fault having perhaps several thousand feet of vertical displacement, with the upthrust side to the north. South of this fault, Nenana Gravels are exposed on the surface. The Suntrana Formation contain the minable reserves at Poker Flats. This formation is a repeated sequence of poorly consolidated pebbly sandstone near the bottom, grading through a silty fine sandstone to a footwall clay unit immediately below a coal seam cap. The footwall clays are high plasticity clays to silty clays. It has been reported that they contain 30% to 50% montmorillonite (Usibelli Coal Mine Inc., 1982). There are six coal seams in the Suntrana Formation, No. I (the lower seam) through No. 6. Only the top four seams are currently exposed. No. 3, No. 4, and No. 6 seams are the only mined seams. The No. 5 seam is very thin or not present. Portions of the undisturbed Suntrana Formation are overlain by up to 15 m (50 ft) of Quaternary outwash gravels or recent landslide rubble. The surface is overlain by a very thin layer of muskeg and isolated areas of permafrost. In many areas, the outwash gravels are found immediately below the surface muskeg. Numerous landslides have been documented along the north facing slopes of Lignite Creek (US Geological Survey, 1970, and Wahrhaftig, 1958). These appear to be surficial solifluction or skin flow types of landslides. In addition, deep-seated structurally controlled slides are also evident on both the north and south sides of Lignite Creek. Structural features Premining aerial photographs (Fig. 3) of the Lignite Creek slopes in the Poker Flats area indicate substantial evidence of deep-seated landsliding. The landslides noted in Fig. 3 are both inside and outside of the current mining area. Surface mapping and geologic exploration indicate that the coal seams are dipping out of the slopes within the noted slide areas. It is suspected that, historically, these landslides were triggered by undercutting of the toe of the slopes by Lignite Creek. And sliding it thought to have taken place on one or more of the clay beds underlying the coal seams (Golder, 1985). The slide areas are characterized by semicircular head scarps and slumped topography. Based on the premining photographs, these slides do not appear to have been recently active. INTRODUCTION The inactive uranium mill tailings investigated in this study are located in two mining districts, Elliot Lake and Bancroft, Ontario, Canada. The sites exhibit a mixture of surface features consisting of dry areas, with or without vegetation and areas covered with water. On the edges of the water bodies, indigenous vegetation has invaded the tailings beaches [ Typha] spp. (Tourn.) L. a dominant plant on these tailings beaches, has been studied for the uptake of radium-226 and lead-210 (Kalin and Sharma, 1981b) from the tailings. It was found that most of the radium-226 remains in the roots of the plants, and that the solubility of radium-226 in control soil differs from that in tailings. The uptake of radium226 by vegetation and other biota is related to the solubility of the element in water. Factors controlling the solubility of radium-226 in uranium mill tailings are of interest in assessing the environments effects of these wastes. Rusanova (1962) found that the soluble or extractable amount of radium from the soil is inversely related to the total concentration of calcium and magnesium in the soil. Experimental work has clarified some aspects of the leachability of radium from uranium mill tailings (Levins, et al., 1978; Wiles, 1978, and others). Halvik, et al., (1967) studied the effects of pH and chemical composition of surface water on the liberation of radium from uranium mill tailings and uranium ore. He found that an increase in pH, up to a value of pH 9, decreases the amount of radium released from the tailings and the ore. A positive effect of calcium chloride was noted on the leachability of radium. Benes (1981) reviewed the physicochemical forms of radium and its migration in water. Based on experimental work, he identifies primary factors which determine leaching of radium from uranium mill tailings. The ratio of the volume of the leaching solution to the weight of the leached sample; the composition of the leached solids and the leaching solution, and finally the pH of the leaching mixture are of importance. He emphasized that a scarcity of field data exists, which would relate experimental work to the actual situation in the tailings ponds. Inactive tailings ponds in Ontario are 16 to 23 years old, and processes which are of importance in evaluating the long term effects of uranium mill tailings in the environment can be studied. The objective of this work was to investigate the leachability of radium-226 from the tailings under field conditions. MATERIALS AND METHODS Sample Collection A description of inactive tailings sites in Ontario, where the tailings and the water for this study were collected, can be found in a report by Kalin (1981). [Typha] spp. specimens were excavated from 16 different locations along with the tailings attached to the roots of the plants. A tailings sample from the surface area, around the plant, was secured before the excavation. Tailings samples were also collected from dry areas free of vegetation in the vicinity of the wetland stand. Surface samples (depth 0-5 cm) and samples at a depth of 20-25 cm were secured. Water was collected from the shallow tailings beaches around the vegetation stands. Sample Preparation The roots of the plants were washed free of tailings with distilled water. The resulting thick slurry of tailings was allowed to settle in the wash basin for five minutes and the supernatant water was decanted. The remaining saturated tailings were brought to 400 ml volume with distilled water in a beaker. The slurry was mixed with a magnetic stirrer for two 24-hour periods. After the first 24-hours, the slurry was allowed to settle for 20 minutes and the supernatant water was removed. The remaining sediments were suspended again in distilled water (400 ml total mixture) and leached for a second 24hour period. The tailings slurries from the different locations had a solid to liquid ratio, which ranged from 0.5 to 1.4 grams of dry tailings material per millilitre. The variations in the ratios are the result of differing fractions of coarse and fine tailings on the sites. In the final leachates, the solid to liquid ratios of the samples were 0.17 (±0.1) g/ml. The pH of the surface water and the leachate was determined with an I.L. Portomatic pH meter. All the tailings samples were homogenized in a mortar and brought to dryness at 75 to 85°C. Rather than attempting to present a summary of the many and highly varied papers that have been presented at this symposium on sampling and grade control, I will attempt to extract the general philosophy of analysis and approach, and attempt to identify the trend of future developments. First, the term "sampling" is used with its broadest connotations. A sample consists of a representative portion of a larger mass, and must represent the mass not only in the grade of contained metals or minerals, but also in all other respects in terms of mineralogy and mineral quality (1, 5), deleterious materials, recoverability of economic components, physical behavior, geophysical response (1), and even archaeological and environmental aspects (7, 11). The sample must be taken from a locality and in such a manner and quantity that it is representative of the larger rock mass. This calls for complete and accurate geological control and an understanding of the nature and distribution of the contained chemical and physical elements and a record of the effectiveness of the different sampling methods. Second, value of a given mass of ore material is based upon its profitability - the difference between recoverable value and costs to achieve recovery, beneficiation and sale. There is a strong movement in mining geology control toward more complete analysis in determining cutoff grades and in grade control, as illustrated by the kriging of metallurgical recovery factors as well as grade at the Mercur Mine (8). To achieve a "profitability factor" as a guide for economic mining practice requires further integration of: 1) the value of contained metal or mineral, 2) percentage recovery of values, 3) dilution of ore with waste rock, 4) addition to, or loss of value as a consequence of by-product materials or deleterious components, 5) cost of producing a saleable product plus minimum profit to justify the effort (cutoff), and 6) cost of land restoration (7, 11). All these parameters vary with the rock type, rock structure, mineralogy, depth, geometry, mining and metallurgical methods, but they must be sampled and analyzed if sampling and grade control are to reflect profitability. A wide variety of deposits has been presented at this symposium; each deposit with its own problems and special solutions. Deposits containing high unit-value components, e.g. precious metals and diamonds, present special problems in the obtaining of accurate samples and generally require statistical analysis control methods or may disregard or modify occasional high or occasional low values, based upon experience (12). Grade control may be accurate for the long term but may vary for the short term. Bulk sampling is always essential. Deposits containing metals or minerals with low unit value are very sensitive to transport costs, and they are often very sensitive to small amounts of deleterious components or differences in physical or chemical behavior. Problems of sampling and grade control change with the genetic type of deposit, with the stage of deposit development and with the size of the information base. Precious metal epithermal deposits (2, 6, 8), because of rapid vertical zonation and erratic lateral distribution of values, have always been difficult to evaluate and maintain grade control and ore reserves. On the other hand, evaluation and grade control are relatively easy in bulk-lowgrade deposits (4, 13). However, these deposits generally have a low margin of profit and are sensitive to mining and beneficiaton costs, price fluctuations and political costs. Industrial mineral deposits (5) often must be evaluated on the basis of their behavior, rather than by chemical analysis. Environmental impact generally increases with the scale of the operation, but certain elements or minerals have especially high impact effects (7, 11). In the exploration phase there is no production control of sampling procedures and careful geological observations are particularly essential. The greatest number of problems is related to the oxidized outcrop where the chemical environment of the ore body has changed and the contained values may have been enriched, depleted or values left unchanged (2, 6). Present evidence suggests that gold values may be very mobile under certain conditions (2, 6) and stable under others. Everything must be sampled in detail. Principal values and by-product or deleterious elements may vary dependent upon their position within the soil profile. Such factors as geomorphic position, erosion rate, vegetation, climate, etc., may affect the interpretation (1, 3). "Native antimony metal is rare and antimony generally occurs together with gold, lead, copper and silver. There are more than 100 antimony minerals, although the sulfide mineral stibnite is the main economic mineral.Antimony enters the supply chain either as a result of mining (primary production) or recycling (secondary production). China has by far the largest antimony resources and is, as a result, the world’s center for antimony mine production. While China remains the leading primary producer, accounting for 69 percent (80.5 kt or 88,700 st) of the global 117.5 kt (129,000 st) in 2017, market consolidation and regulatory inspections across China leading to closures of facilities have caused a significant decrease in Chinese output from 115.8 kt (127,600 st) in 2015 (81 percent of global production).Tajikistan and Russia are the next largest producers of antimony after China, both ramping up production in recent years. Russia could significantly increase its supply of antimony if Polyus moves ahead with supplying 10-12 kt/a (11,000–13,200 stpy) of contained antimony from gold tailings. Australia and Bolivia are also significant mine producers outside China. The value of the different ores varies based on the metal content of the concentrates, with the most valuable gold-bearing ores sourced from Australia’s Costerfield Mine and the least valuable mercury-bearing ores sourced from the Anzob Mine in Tajikistan. On average, Bolivian and Russian ores trade at an intermediate value between those from Costerfield and Anzob. Nearly all of the rest-of-world production is exported to China, where the ores and concentrates are processed into antimony ingots and oxides.Antimony ingot production is not as widespread as is the case with antimony mine production, owing to the high levels of pollution that are associated with antimony smelting. It is for this reason that smelting has been largely confined to China and other Asian countries ,such as Vietnam and Myanmar, where environmental legislation has historically been less stringent. Tajikistan has become a growing producer of antimony ingot since 2014, coming from the commissioned metallurgical plant at the Anzob Mine.Antimony oxide production is undertaken on four continents with China being the biggest producer. There is also a considerable amount of oxide production in Europe (mainly Belgium and France), which relies on imported antimony metal feedstock, mostly from China.A new antimony project under construction by Strategic and Precious Metals Processing (SPMP) in Oman made progress in 2016 and 2017, and after producing first metal in April 2016 in test facilities, was set to be commissioned in the first quarter of 2018. Commissioning had been moved back to June 2018, with a potential ramp-up to 2-3 kt/a (2,200 to 3,300 stpy) of antimony metal by the end of 2018. SPMP is also reviewing producing antimony oxide as part of its flow sheet."

Incident investigation is of utmost importance in most high-risk industries. This study was undertaken to study the content of past investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of 5 elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of 3 Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed. INTRODUCTION The importance of learning from incidents is well documented in the literature. For instance, some scholars regard learning from incidents as important for improving safety [1,2]. Thus, many organisations spend considerable resources in identifying lessons from incidents and subsequently on the process of improvement that follows the identification of lessons. It is therefore important for such organisations to determine the effectiveness of their learning from incidents processes, in order to identify specific areas where improvements are needed, so that interventions can be as specific as possible. The learning from incidents process models the plan-act-check-do approach of management systems, often consisting of four major stages, namely; data collection and incident analysis, planning interventions, implementing interventions and follow-ups and evaluation [3,2]. The investigating and analysis of incidents have been regarded as one of the vital stages of the learning process since it is the stage where lessons are identified [3,2,4]. It has also been noted elsewhere in the literature that results of the previous stage of the learning process are important for the next stage [3]. For example, ineffective incident analyses could result in the planning of ineffective remedial measures. One of the primary aims of incident investigations is to identify lessons from previous incidents which when implemented can prevent future recurrences or minimise the effects of future incidents. There is thus the need for organisations to assess the quality of their investigations periodically since it is one of the means they adopt to improve safety and the quality of remedial measures to some extent depends on the effectiveness of the investigations. Although previous studies [5,2] have assessed the effectiveness of the learning process, and some aspects of the learning process, such as incidents reporting, there is less focus on the effectiveness of incident investigations. The research described in this paper was therefore conducted to contribute to this gap, focusing more closely on incident investigation reports.
