"On January 9, 2014, a large chemical spill occurred on the bank of the Elk River near Charleston, WV. Within hours, the potable water supply for more than 300,000 people was contaminated, and the incident captured national headlines for weeks. Although the primary chemical, crude 4-methylcyclohexanemethanol, or MCHM, is deemed nonhazardous, the impacts of the spill are significant – with all stakeholders, including the public, members of industry, and relevant government agencies experiencing losses. This case highlights the need for a more proactive approach to environmental management, shifting from crisis control and remediation to crisis prevention and risk management. INTRODUCTION On the morning of January 9, 2014, approximately 10,000 gallons of crude 4-methylcyclohexanemethanol (MCHM) leaked from a 48,000-gallon storage tank sitting on the bank of the Elk River near Charleston, West Virginia (WVDEP, 2014; CSB, 2014). MCHM is a chemical commonly used in preparation of fine coal, and is deemed nonhazardous in terms of transportation and storage regulations and also toxicity (CDC, 2014a). Freedom Industries, Inc., a producer and vendor of specialty chemical products, had acquired the storage facility where the spill occurred from Etowah River Terminal, LLC just a few weeks prior in December 2013 (WVDEP, 2014; Freedom, 2005). The direct cause of the spill was ultimately determined to be two small holes in the base of the 76-year-old tank, which were created due to corrosion damage (Bauerlein & McWhirter, 2014). The spill site was located just over a mile upstream from the intake of a municipal water treatment facility operated by West Virginia American Water (WVAW), which serves a population of 300,000 across nine counties in central West Virginia (CDC, 2014a). In a matter of hours, the leaked MCHM made its way through the treatment facility and into the water of nearby homes and businesses, where its strong smell was noticed almost immediately. On the evening of the spill, WVAW concluded that its conventional treatment processes, which include carbon filters to remove a variety of contaminants from water, were not capable of fully removing the MCHM and a “Do Not Use” order was issued for all customers (WVAW, 2014a). With little information readily available regarding the potential human health effects of MCHM, the US Centers for Disease Control and Prevention (CDC) quickly issued an advisory ruling that set a maximum screening level of 1 ppm for safe drinking water (CDC, 2014a). Based on this recommendation, the “Do Not Use” order remained in effect for a total of eight days, until January 17, when the WVAW distribution water was confirmed to have MCHM concentrations below this level (WVAW, 2014a). However, due to the pungent odor of the chemical, its presence was still recognizable in the WVAW facility and in consumers’ tap water after the order was lifted (Manuel, 2014). Recent studies have shown that the chemical can be detected in water at concentrations as low as 0.55 ppb by individuals experienced in odor identification (Rosen et al., 2014); and even individuals with no such training can detect pure MCHM at just 7 ppb (WVTAP, 2014). The fact that this chemical can be sensed at such low levels – indeed far below those considered safe – undoubtedly added to growing public concern. In response to numerous complaints and demands, WVAW continued flushing the treatment facility and pipes until February 25 when MCHM levels were measured below 2 ppb (WVAWb, 2014), and eventually replaced the affected filter media (WVAWc, 2014)."

Design and construction of temporary shafts for underground facilities often require evaluation of bottom stability due to rupture (blow-ins) caused by water pressure acting on a bottom plug. This paper provides an overview of shaft bottom stability evaluation methods for the case of a relatively watertight soil plug which overlies an artesian aquifer. Methods of considering plug perimeter shear, plug buckling and pressure relief are discussed. Minimum factors of safety are suggested. Future use of the shaft subgrade is also considered. Bottom plug failure case histories at several shafts are evaluated. This paper should be useful to project designers, ground support system design engineers and contractors involved with construction of shafts for underground facilities

This paper reflects the challenges encountered during the construction of the light rail tunnels under Boyle Heights, in Los Angeles, California. Equipped with two state-of-the-art EPB TBMs, the construction team undertook the mining of twin bored tunnels in the Los Angeles alluvium. The gassy underground conditions dictated many aspects of the design of the tunnels, as well as the tools and means used for their construction. This paper will detail the challenges encountered during construction, as well as the means, methods and logistics used on the project. Discussion includes: EPB TBM machines with some very unique/specific features; complex assembly sequence due to limited shaft access; final precast concrete liner with double gaskets; NATM cross passage excavation in soft ground and below the water table; compensation grouting; mucking shaft arrangement.

Resource Engineering and Development, Inc. is desigining and building a portable 150 to 225 tpd, flotation-cyanide leach plant with unitized, skid-mounted components. This plant design, with its portability, low capital investment, and high recovery of silver and gold from many partially oxidized ores, is well suited to treat many small, highgrade deposits, old waste dumps, and old mill tailing piles. A description is given of flotation and/or cyanide leach tests run on 18 partially oxidized, newly mined ores; waste dump materials; placer slimes; and old mill tailings.

The East Side Access (ESA) project is the largest underground rail infrastructure project under construction in New York City, USA. The Manhattan segment of the project is to be mined through rock and consists of 4.1 miles of Tunnel Boring Machine tunnels for track work and mined excavations to house rail switches, air plenums, and other rail facilities. This paper considers different methods for concrete lining, such as the use of fixed forms, adjustable forms, and pneumatically applied shotcrete. The advantages and disadvantages of these methods are discussed and the basis for considering one over the other are illustrated. Further discussions on design development and preparation of documents to facilitate quality and constructability in the field are presented.

Significant advances in longwall mining technology and equipment have occurred over the last decade by the late 1990s, longwall mine output accounted for 40% of all underground output in the U.S. and today longwall mines account for approximately 50% of coal produced underground in the United states. A 51% increase in average shift production rates has occurred over the last 15 years. this increased longwall productivity has meant that far more dust is being produced and controlling respirable coal dust presents an ongoing challenge for coal mine operators the National Institute for Occupational safety and Health (NIOSH) conducted a series of benchmark surveys at longwall operations across the country to identify current operating practices and the types of controls being used. Gravimetric and instantaneous dust sampling was completed to quantify the dust levels generated by major sources on the longwall section and to identify different control technologies in use today substantial reductions in dust levels were realized at sampling locations on the face when compared with ongwall surveys conducted in the 1990s results from the underground dust surveys and current longwall dust control technology and operating practices will be discussed. When the Trelleborg Divisions of Boliden Allis and Mineral Processing Systems (MPSI) merged, a unique opportunity occurred for two independent mill fabricators to compare design pro¬cedures and the research upon which they were based. Several semiautogenuous (SAG) mill analyses are compared using Boliden Allis and MPSI methods. The differences and similarities are then used in a correlation of critical stress definitions and the acceptable design and inspection criteria. Given the opportunity to compare the total philosophies and methods of several companies involved in the design and fabrication of grinding mills, it was important to first develop the comparison method and ground rules. One option was to compare analytical methods by details such as numerical techniques and accuracies. However, this option was discarded in favor of a more general approach. The question that was finally posed was in two parts. Given an identical mill requirement: • What would be the difference in the analytical design result, and to what aspect of the analytical process could this difference be attributed? • What would be the difference in approaches to obtaining the final hardware in subcontracting, both in specifying and Quality Assurance surveillance? By using this comparative approach, questions of relative conservativeness and reliability would automatically be answered.

Research studies have shown that the flow within feedwells displays a vortical structure and high velocities at the locations where standard tangential inlets force its way into the feedwell swirling flow, producing high local shear rates and flow non-uniformities. These hydraulic deficiencies can be attributed to the discrete feeding through a few entrances, usually one or no more than a few tangential feedpipes, which practically project jets into the vortex within the feedwell. The main consequences are unequal flocculation with aggregates subjected to different amounts of shear and a non-uniform discharge from the feedwell. This paper presents results on the application of CFD and small-scale models to develop a new design that overcomes these problems by using an involute shaped inlet that converges around the feedwell. The result is a new feedwell with enhanced hydraulics that perfectly couples with the FLSmidth E-Duc® Feed Dilution system by protecting the flocculated aggregates that have been carefully produced upstream, and that favors uniform sedimentation conditions within the thickener thanks to a uniform discharge, while maintaining a practical and trouble-free geometry with excellent turndown.

The Tren Urbano project in Puerto Rico includes a 19 m diameter tunnel constructed by the stacked drift method, which involves installation of a pre-supported concrete arch using multiple tunnels. Geotechnical conditions consisted of weak rock and highly weathered, unstable alluvial soils. Due to low cover, reinforcement of the arch was required. Ground behavior ranged from firm ground to mixed-face to raveling and flowing ground. Consequently, construction of the individual drifts involved many different soft-ground techniques such as steel set supports, shotcrete support, shield tunneling, forepoling, and consolidation grouting. During construction, the design was modified to improve constructability. Although settlement was experienced during excavation of the drifts, arch movements were within predicted limits and minimal surface settlement was observed upon excavation of the interior.

In 1994, the East Bay Municipal Utility District (EBMUD) commenced a system-wide Seismic Improvement Program to protect their facilities from catastrophic damage. A primary focus of this program has been on retrofitting the 75-year old Claremont Tunnel, which crosses the active Hayward Fault zone. A large earthquake on the Hayward Fault could generate enough right-lateral fault slip to effectively sever this important lifeline. The retrofit project involves construction of a new bypass tunnel through the Hayward Fault zone, together with systematic contact grouting and localized structural repair of the existing concrete liner. Several innovative design measures have been incorporated into the seismic retrofit design, including an oversized tunnel cross section, backfill concrete side drifts, seismic isolation of an internal structural pipe, and shear fuses in the final lining.

INTRODUCTION The Macassa Division of Lac Minerals Ltd. is a high grade gold mine that has been operating in Kirkland Lake, Ontario for the past 51 years. It produces approximately 120,000 tonnes of ore per year, averaging approximately 18.7 grams per tonne (0.45 ounzes per ton) of gold. Over the years, the ore body has been developed to a point where mining takes place below the 1,220 m (4,000 feet) elevation and 1,830 m (6,000 feet) west of the original shaft. The mine is currently serviced by two surface and two winze shafts. (Figure 1). The ore body extends below the 1,930 m (6,400 feet) Level which is the bottom of the existing mining operation. An ambitious underground exploration program has substantially increased these reserves. Concurrent with the discovery of greater reserves, has been rapidly escalating mining costs, particularly labour which constituted 66% of the total cost in 1981. These costs continued to escalate as the major proportion of mining was carried out to the west, far away from the existing hoisting systems. This required three lift hoisting for men, materials and ore. I n addition to the increased travel time and ore handling expense, increased humidity and temperature and ventilation and compressed air losses have added to the inefficiency of the operations. As a result, studies were initiated in 1980 to assess the future of the Macassa operation.

Local community acceptance is a key aspect of planning and managing mining projects, whether through sustainability assessment and reporting or corporate social responsibility programs. However, there has been very little research to understand the determinants of community acceptance. This paper seeks to identify key mine characteristics and demographic factors that influence individual acceptance of mining projects. Six demographic factors and 16 mine characteristics were selected based on literature review. Four of the six demographics factors were confirmed to be significantly (p < 0.05) correlated with people?s attitude of the importance of the mine characteristics. All sixteen mine characteristics are identified as important factors. The most important mining factors are Job opportunities, water shortage or pollution, air pollution and land pollution. Both the mining and control groups have same opinion of 12 mining characteristics and differ on four (infrastructure improvement, labor shortage for other businesses, noise pollution, and mine life). During design and management of mines, this data can be used as a reference. This result will facilitate better choice experiment (survey) design for discrete choice modeling. Such discrete choice models can provide a viable framework for data-driven community engagement and sustainable mine management.

INTRODUCTION Before a mine is planned in detail, a preliminary analysis is often conducted. This preplanning permits the mining engineer to make a rapid assessment as to whether or not a particular mining property warrants further consideration. In fact, a properly conducted preplanning phase can come within 10% to 20% of the actual results, thereby saving the mining engineer valuable time before committing himself to a fine-tuned analysis. The questions to be answered by mine preplanning are (1) What will be the operating conditions for the mine? and (2) How long will it take to complete the project? To answer these questions, calculations dealing with reserve estimation, production planning, man- power planning, and project scheduling are conducted. The following sections discuss these various phases of mine preplanning in greater detail. RESERVE ESTIMATION The estimation of the various characteristics of a reserve, such as quantity, grade, and thickness, is an ongoing process throughout the life of a mining venture. However, the reserve-estimation phase of mine planning is more closely associated with the preplanning phase. There are many different techniques employed in reserve estimation of solid mineral deposits; variations among them are introduced because of such considerations as ease of application or level of accuracy. In any event, each technique is developed in the following manner: (1) the mineral body is represented pictorially and then subdivided into blocks related to one or more samples of exploration data, (2) the area and volume of each block is determined, (3) the volumes are converted to representative tonnages (or, for example, grades), and (4) the results are tabulated. To illustrate this procedure, the polygonal (area-of- influence) technique will be presented due to its ease of application, flexibility in dealing with different deposits, and general acceptance. The polygonal method assumes that all characteristics of a mineral body extend halfway between a point of observation and any other point of observation. The explored portion of the mineral body is represented by polygonal prisms whose depths relate to the characteristic in question (grade, thickness, etc.) and whose plane bases relate to the area of influence of each point of observation (drill hole, etc.) as shown in Fig. 1. The polygons must be constructed in a definite order, usually clockwise and from the periphery to the center of the deposit. Table 1 can then be used to calculate the characteristics in question. Since, by definition, the polygonal method assumes that the characteristics of a mineral body extend half- way between points of observation, it becomes readily apparent that the evaluation of a vertical-drill hole exploration program requires that the sides of the polygons be formed by the intersection of perpendicular bisectors (Fig. 2). Figs. 3 and 4 show correct and incorrect construction of the polygons. The criterion for correct construction of the polygons is that all points within a particular polygon must be closer to its rallying point than those located in any other polygon. This is clearly not the case for certain points that are located in the polygon containing rallying point B in Fig. 4. Although this method can be used where the drill holes are irregularly spaced, calculations become less tedious when a square net (Fig. 5) or a chessboard (Fig. 6) grid is adopted. The numbers shown in Figs. 5 and 6 represent the relative weight for each of the shaded areas. In general, the greater the number of polygons, the more continuous the mineral body, and the more regular the grid, then the more accurate the computations will be. PRODUCTION PLANNING Before an acceptable evaluation of the production potential of a mine can be determined, a thorough exploration program should be conducted. It seems obvious that the nature and properties of the deposit and adjacent strata, the presence of impurities, and other mining conditions should be adequately determined prior to production planning and equipment selection. The lack of sufficiently detailed knowledge of the many mining parameters has led to the failure of numerous operations. For example, targeted production tonnages and equipment sizes for a coal mine are intimately related to the seam thickness. Needless to say the quality of information that is gathered from an exploration program can have a tremendous impact on the production plan. "Low-grade iron ores are finely ground and processed to recover iron-bearing minerals. However, the iron-rich powder is too fine to be used directly in blast furnaces and most direct reduction furnaces, and must be agglomerated or pelletized into a usable product called “pellets.” Bentonite clay has traditionally been used as a binder to make iron ore concentrate pellets, but it is expensive and contaminates the pellets. Organic binders may be a viable alternative to bentonite clay. This manuscript reviews organic materials that have been used to make iron ore concentrate pellets. Included are discussions on the development of organic binders, common pros and cons to organic binder use, organic binder structure and its effects on agglomeration, and additives to organic binders.IntroductionIron ore agglomeration turns finely disseminated, low-grade iron ores into a usable product, called pellets. During pellet production, fine-grained, iron-rich minerals are liberated from low-grade ores, concentrated, and agglomerated into round balls that are thermally hardened or cold-bonded to form pellets. Pellets are then transported to blast furnaces or direct reduction operations where they are used to produce iron; iron is further refined and shaped into the wide variety of steel-based industrial and consumer products in use today.Over the years, pellet production has become increasingly more prevalent with the increased exploitation of low-grade iron ores and increased iron and steel consumption around the world. In 2010, world iron ore pellet production was 388 Mt. It could be argued that the current pellet production process has been enabled by the use of binders – practical experience has shown that it would be nearly impossible to agglomerate iron ore concentrates using rotary drums and discs without using a binder."

Several bauxite residue sands and muds produced by Bayer plants in the U.S. and overseas have been fractionated using dry and wet magnetic separators. The magnetic sand fractions typically contain about 60 wt% iron and the bulk of the radioactivity in this waste. The muds are more difficult to process than the sands, but some fractionation is possible using a combination of flotation and magnetic separation techniques.

Portland, Oregon’s $370 million West Side Combined Sewer Overflow Tunnel Project has completed the final phase of construction and commissioning. The construction contract, which began in September 2002, was a reimbursable cost plus fixed fee contract that followed a qualifications-based contractor procurement process and apre-construction planning phase. This paper addresses the final results of the contracting method and provides a lessons-learned case history. Specific items addressed include the benefits realized from the contracting method, areas of improvement, final costs compared to the original contract estimates, design changes and schedule impacts, and subcontract management issues.

Numerous fatal/nonfatal accidents involving underground workers struck by powered machinery occur yearly, with continuous miners and roof bolters involved in the majority of these accidents. In an attempt to reduce these accidents, researchers at the the National Institute for Occupational Safety and Health (NIOSH) Pittsburgh Research Laboratory conducted studies of operator interactions with the motions of continuous mining machines and roof bolter boom arms. These operators generally perform their tasks in close proximity to the equipment in confined workspaces, often employing awkward postures. Since experiments with human subjects using the actual equipment were not feasible due to safety concerns, researchers opted to conduct controlled experiments in laboratory settings. Utilizing motion capture technologies and computer simulations, the studies collected data on equipment and operator movement in a range of seam heights and working postures. This article details the results of these studies to examine operating speeds based on usage and seam height.

Laboratory and pilot-scale test data have shown that the innovative feed presentation of the CrossFlow separator provides improved metallurgy when compared to traditional hindered-bed or teeter-bed classifiers. This design feature prevents excess water from entering the separation chamber and disrupting the overall fluidization flow rate within the teeter zone. Recent side-by-side industrial-scale evaluations have verified that this technology can improve overall efficiency, reduce the cut point of separation, and increase throughput capacity. This paper describes the important features that make this novel separator ideally suited for size classification and density concentration. Test data from heavy mineral, phosphate, and industrial mineral applications are also discussed.

Rio Tinto Coal Australia (RTCA) developed the Meandu Mine in Queensland, Australia in 1983 to supply thermal coal to the adjacent and newly constructed Tarong Power Station (TPS). In designing the mine, RTCA selected Meandu?s best economic reserves for a 27-year life based on station owner Tarong Energy Corp.?s (TECs) estimated life for TPS. That 27-year plan changed dramatically in the early 2000s when TEC approved life extensions to TPS?s four existing units plus construction of a fifth unit. After reviewing Meandu?s remaining reserves, RTCA decided the nearby Kunioon reserve offered the more economic option for maintaining deliveries beyond 2010. Those plans changed again in 2007 when TEC chose to purchase the Meandu Mine and nearby reserves. TEC then selected Thiess Pty Ltd. (Thiess) to continue operating Meandu until the new mine at Kunioon could be developed. After assuming operation of the mine in 2008, Thiess began a review of Meandu?s remaining reserves and found considerable coal available with economic potential. Working jointly with TEC, Thiess performed several analyses on Meandu and subsequently the Kunioon coal reserves. The results changed TEC?s decision from expediting Kunioon?s construction to instead continuing Meandu?s operation for up to 15 additional years.
