As the global mining industry continues to grapple with ‘black swans’ (high consequence / low frequency events) for health, safety and environmental losses, the focus is shifting from risk assessment of these events to risk management using selected purposeful controls that reduce the risk.Fundamental to critical control management (CCM) is a soundly based rationale for selecting the right controls for each and every risk. The approach should be based on a thorough understanding of the dynamics of the event: What causes it? How frequently will it occur? How likely is the consequence to be realised? This is typically done through a bowtie and methodical numerical risk analysis. Core to this step, then, is the structure and content of the bowtie; specifically, the analytic architecture that is used to construct the bowtie. Different architectures yield different constructions, and bowties are no different.Although bowties for operational health and safety (OHS) hazards appear to have a common architecture, they are not generic. It is important to stress that they contain site specific information, are prepared by site based operational teams, and vary from site to site to reflect each site’s operational conditions. They are often a product of the knowledge and understanding of the operational teams involved, may be prone to human error, and potentially miss latent hazards that may have been embedded early in the planning, design or construction phase of a mine or facility, or within the mining equipment. Overcoming these deficiencies through constant re-evaluation of the risks, and challenging the content on the bowtie will help to minimise the chance that something will be missed.This paper will cover key themes in bowtie architecture, introduce the concept of barrier models for high consequence event (HCE) bowties and present a relevant case study to illustrate the issues that determine quality in bowtie construction and critical control (barrier) selection and management.CITATION:Mills, R, Andrew, M and Reynolds, H, 2016. On the first of July, 2012, the Australian GovernmentÆs Clean Energy Act commenced. From this point, a price of $23/t of CO2-e will apply to facilities that trip the liability threshold. This first part of this paper concentrates on the mechanics of the Clean Energy Future legislation and how it will be applied to the mining and mineral processing industry.The second part of this paper contains analysis of the data presented for typical mine operations. Methodologies for constructing a carbon strategy are also presented. This will entail determining what the objectives of the carbon strategy are, and defining what the strategic priorities for a business should be. An action plan to address the risks identified from the Clean Energy Act is also developed as part of the strategy.Details such as the advantages and disadvantages of opting into the carbon price scheme for liquid fuels are shown. In the current cycle of low commodity prices and rising costs, mine managers continue to focus on optimising mine performance and productivity. A critical component of strengthening performance and productivity is ensuring material is routed correctly and efficiently through the core mine production activities of drilling, blasting and grade control.By the time a blast is planned, an enormous amount of intellectual property has been invested in the exploration, resource development, feasibility studies and mine planning activities. The value of that knowledge is either realised or diminished during mine production. Efficient execution of the process is critical and is reflected by the quality and timeliness of the information produced.Multiple teams are involved in the mining extraction process. These teams are all under time pressure to keep the mine equipment operating. One of the root causes of this pressure is the dependency between the teams. They do not operate in isolation, but are a link in a chain depending on the outcomes of each activity. It is essential communication is efficient so tasks can be completed according to the production schedule. Unfortunately, communication fails when unintegrated systems are used because of:- different teams seeing different data, hence there is not an overall, up-to-date view of progress and priorities- lack of cross-functional visibility of data leading to failure to identify errors in time to address or correct issues- teams tending to be siloed and focusing on their part of the process rather than the overall objective.With different systems used for the parts of a common goal, supervisors and managers lack visibility of the process. This makes it difficult to identify process improvement opportunities.Streamlined communications help consolidate all data and activities resulting in improved cooperation between mine production teams. This outcome increases the confidence and efficiency of material routing decisions. It also reduces potential impact to mine performance from errors in early stage tasks like drilling and grade control. The Mt Keith Mining Alliance operates a fleet of large capacity hydraulic face shovels and more than 40 X 240 t capacity haul trucks to deliver annual production in excess of 40 Mbcm per annum. The Mt Keith Nickel mine is currently operating at a depth to 300 m below surface with a planned final depth of the order of 500 m. As is the case with any large open pit managing the health of the workforce on continuous shift operations is of critical importance to achieving outstanding safety and production outcomes. Fatigue management at Mt Keith is effectively managed by the personnel operating the mining equipment, with assistance and understanding of the management team and supervisors. The program has been driven through the introduction of Fatigue Champions and a comprehensive management plan that includes education, individual health plans and the use of new technologies. Mt Keith has been instrumental in trialling new technology which assists in monitoring fatigue in operations personnel. Insight into these developments is provided in the context of the overall fatigue management plan. The field trials of the OptAlert(TM) system and how it assists in measuring fatigue is explained. The implications for individual operators and the context for a fleet wide approach to fatigue management is also discussed. Mt Keith has recently completed the second phase of trials with the result that this new and exciting technology is being considered for adoption globally across BHP BillitonÆs open pit mining operations.

Phyllite waste rock at the Simandou Pic de Fon and Oueleba Iron Ore deposits (Guinea, West Africa) variably weather to form clay-rich rocks. This weathering modifies the mineralogical, geochemical and mechanical properties of the phyllites. Final pit walls will largely be formed in phyllite, so it is critical that the slope design takes account of the distribution and variability of weathering within this material. Phyllite intact material strength has been correlated with geochemistry (ie mineralogy) facilitating the establishment of domains of intact rock strength (IRS). In order to calibrate this geochemically-based strength model, core from diamond drilling (DD), comprising a variety of phyllite material types (from soft clay to moderate strength rock), was geotechnically logged to assess intact strength; further validation was achieved by uniaxial compression (UCS) testing and point load tests (PLT). The correlation between geochemistry and strength offers a potentially powerful tool for differentiating basic mechanical properties of the rock mass, especially at Simandou where a significant proportion of the resource drilling is done by reverse circulation (RC). Unlike DD, reliable measurements of material strength from RC samples can prove difficult, especially in clay-rich material. Therefore, gaining information from routine geochemical assay results, independent of the sampleÆs physical integrity, is a valuable and cost-effective source of reliable empirical data. Data acquired using this approach provides an opportunity to develop strength domains, which are integrated into mine planning block models. In combination with other geotechnical characteristics, this can be a valuable input to the development of geotechnical models. "In 2010, two McLanahan RSS14×50 scrubber screens were installed at Fortescue Metals Group’s (Fortescue) Christmas Creek site. By 2015, the trommel (screen) section had effectively reached the end of their serviceable life. This was determined by regular measurement of structural components over the previous 18 months that tracked wear life. At the same time, numerous trials and optimisations had taken place on screen panel design and material composition. During 2015, a new set of trommels were designed, manufactured and installed that incorporated many new features: increased open area to facilitate high throughput, improved wear protection for structural members, simplified trommel panels and realigned discharge ports to reduce wear on exit. Numerous other upgrades have taken place on the drive system, feed chutes, liner and weir configurations and park brake, to name a few. This paper will introduce some innovative strategies for simulator training that are intended to improve the level of safety and operational performance of heavy equipment operators. The key strategy is to design simulator training to deliver improved work performance outcomes. This strategy is underpinned by a process for engaging workers in knowledge discovery and transfer that has been developed as an outcome of an Australian Coal Association Research Program (ACARP) funded project conducted at open cut coal mines in Australia. The process is expected to deliver improvements in the safety and operational performance of heavy equipment operators by informing training content about how experienced operators manage unexpected situations around the site. Tapping into the tacit knowledge that experienced operators gather over time can provide site specific and timely information about situational cues used to manage the unexpected. This type of information complements basic skills training by helping the trainee understand the operational context in which the acquired skills will be applied. Inexperienced operators have traditionally gained experience through their own actions, observing the actions of others and by taking the advice of mentors who have æbeen there, done thatÆ and developed operational decision making skills for safe and productive work performance. Leadership and mentoring techniques that contribute to effective knowledge delivery will also be discussed.Other simulator training strategies include refresher training, change management, emergency response training and testing the usability of new equipment and technologies prior to their introduction on-site. A key challenge ahead for the resources sector is the development of knowledge elicitation techniques that suit the operational workforce. Blast vibration analysis is one of the important topics in the control technology of blast vibration damage. Nowadays, blasting vibration safety standards are based on both the particle velocity and the frequency of vibration and they have been adopted widely and internationally. However, the analysis on vibration frequencies is less studied compared to investigations of peak particle velocity. For the vibration induced by a spherical charge in a viscoelasticly infinite rock mass, an expression of vibration spectrum taking into account the damping effect of rock mass is inferred. Then the attenuation laws of dominant frequencies and centroid frequencies are compared theoretically. For the case of cylindrical charge, dynamic finite element method (DFEM) software of LS-DYNA is employed to analyse the attenuation laws of characteristic frequencies. The research demonstrates that, no matter whether it is a spherical charge or a cylindrical charge, there is a local mutation or fluctuation at some distance when the blast proceeds, while the centroid frequencies conform to a smooth attenuation law. Both the multipeaks structures of vibration spectra and the decay rate difference between high- and low-frequency components contribute to the mutation or fluctuation investigated in this study. Additionally, an increase in longitudinally sonic velocity or the quality factor of rock mass produces an increase in the vibration frequencies, whereas an increase in blast source diameter or the rise time of blast load produces a decrease in vibration frequencies from a spherical charge. In borehole blasting, there is a positive correlation between the vibration frequencies and charge diameter, whereas there is a negative correlation with charge length. And altering the initiation plan will change vibration frequencies to various degrees. After multiple tailings dam failures in recent times, the owner organisations have made increased efforts to step up the governance systems to industry best practise in the design, construction and operational management areas. The measures include but are not limited to, site investigation, rigorous peer review (both internal and external), certification through appropriately qualified personnel and improvement in operational management practises. While a lot of focus is on design and operational practices, the construction stage requires adaptation (both in terms of materials and approved design) and immediate decision making on site. Work undertaken in these areas is mostly irreversible and sometimes poses serious threat to the future TSF raise/ expansion planning and may lead to reduced life of the TSF and additional risk to the Owner.This technical paper will discuss some challenges and highlight the importance of appropriate processes to achieve a successful construction regime. The modern workplace has changed dramatically in recent years with an increased emphasis on the psychological well-being of employees. While adhering to a certain standard of psychosocial safety at work is now not only a moral, but legal obligation in Australia, it begs the question: how can organisations begin to ensure psychosocial safety? A positive psychosocial safety climate is one where employees feel they are psychologically safe, feel supported in their work and are not exposed to work-related stressors that can negatively affect their mental health. Such a climate can help boost productivity and create a more positive working environment. This presentation will provide a twopronged approach to understanding and managing psychosocial safety: pairing a traditional risk assessment with climate measurement. Risk assessments can provide several benefits, including identifying and characterising work-related hazards, identifying the nature and severity of the risks to employees and prioritising necessary interventions for reducing or preventing psychosocial harm in the workplace. However, these assessments can have certain limitations, especially when it comes to capturing some of the less obvious causes of psychosocial harm. Climate measurement can provide valuable insights into employees’ perceptions of the work environment, identify areas of concern that may not have been identified in the risk assessment and help to guide organisational changes that will enhance employees’ psychosocial well-being over time. It will also be argued that climate should be assessed from a broad perspective exploring general, physical, social and psychological aspects of safety climate. This presentation will explore why conducting risk assessments in conjunction with detailed climate measurements is integral to protecting the psychosocial safety of employees in the workplace and demonstrate why employers should view the two approaches as complementary and interconnected. Best practice recommendations will be provided for the risk assessment, climate measurement and psychosocial interventions

Global iron ore demand, sparked by ChinaÆs rapid industrialisation and emergence as the worldÆs newest steel market, has seen iron ore groups investing heavily in production capacity as well as infrastructure. The success of these new projects depends heavily on reliability, competitiveness and quality control in an increasingly demanding trading environment. Furthermore, the iron ore industry continues to improve its competitiveness in mining practices, technology innovation and management processes. The continued fine tuning of these sectors will be critical to the success of the industry in the years ahead, and this mining sector will continue to demand process improvements from their service suppliers to maintain competitiveness. Automation offers a real solution to these demands. A crucial quality control element in mining, beneficiation, stockpiling and shipping stages is ensuring representative samples of ore are taken along each step of the process. When these samples reach the laboratory it is imperative that the sample is representative of the entire stream and that no induced errors occur. Examples of automation start with mechanical primary and secondary samplers in the run-of-mine processes through to sample tower and laboratory crushers, dividers and collectors. Automation is widely used in many forms to provide reliable and repeatable samples for analysis whilst providing a safer working environment. Laboratory automation systems in use today vary from simple linear systems to sophisticated robotic cells. Sample automation in the iron ore industry has resulted in many benefits for both producers and their customers. The most significant benefits have come in the form of higher safety standards whilst reducing sample error, sample handling and sample contamination during collection. These benefits have been further augmented by reliability and repeatability of automating the final sample preparation and analysis stage. Today these benefits are reflected in reduced costs of sampling and improvement in both turnaround time and quality of results, whilst complying with Australian safety and ISO sampling standards.

Precast concrete segmental linings have enjoyed widespread use around the world, and are the lining type of choice for tunnels constructed using a shielded tunnel boring machine. However, there is significant variation in the design of such linings from one country to the next in such areas as materials, geometry, fixtures and fittings, waterproofing and reinforcement to name but a few. This arises because the speed at which the technology has developed within some countries and regions has sometimes outstripped the transfer of such technologies to the rest of the world.This paper provides some insights into the variations in lining design in different parts of the world with the aim of providing designers and constructors alike a reference for the different methods adopted. It provides a context for these developments by briefly summarising historical reviews of this subject before looking at some of the developments around the world over the last 15–20 years. The developments are presented with some reference to the reasons for implementation, and the advantages and limitations commented on. Classical electroslag remelting of consumable electrodes is a drop-by-drop melt feeding through a slag layer into a renewing molten metal bath, slowly solidifying into a homogeneous, dense, defectfree ingot in a copper water-cooled mould. The new look at the additive nature of the electroslag remelting (ESR) process in a protective atmosphere allowed us to formulate the principles of a comprehensive thermodynamic-based model that can predict dynamic changes in slag and metal composition at certain ingot remelting. The model considers drastically different slag-to-metal mass ratios at the beginning and end of remelting and predicts gas, slag and metal composition in a chain of thermodynamic subsystems. Despite consisting of calcium fluoride and stable oxides, the ESR slag can oxidise main and active elements from steel and alloys (primarily aluminium, titanium and silicon) due to chemical reactions between slag and metal. ESR is not an electrochemical process in its nature. However, slags are ionic melts and deviation in their composition causes a change in their properties, affecting both operation mode and ingot quality. Another important understanding derived from the modern metallurgy technological route is that the ability to refine metal from impurities is not a priority for the ESR because consumable electrode has already passed all stages of refining and deep degassing at ladle treatment that changes ESR slags engineering principles. The critical importance became slag’s ability to generate process heat and keep the melting composition in the metal bath unchanged (except for non-metallic inclusions assimilation). Slag engineering for ESR required a compromise between chemical inertness to a metal composition and desired physical properties deriving from technological reasons. The Directed Chemical Bonds Concept (DCBC) in a multicomponent oxide system was used to build predictive models of electric conductivity and the melting temperature of fluoride-oxide slag based on their chemical composition. Both models help to design a customised composition of effective slags for steel and alloy groups or individual grades, and they are significant steps in the development of a comprehensive model of electroslag remelting.

Efficiently controlling foam/froth within mineral processing continues to pose a challenge to the mining sector. The foaming characteristics of aqueous solutions for two-phase systems of air/liquid are well-studied with theories and mechanisms proposed to describe and predict foam formation and stability. The types and structures of reagents that either promote or prevent foaming/froth formation and stability are well-defined in these studies and practiced across various applications. Three-phase systems of air/liquid/solid, such as froth from mineral slurries, however, pose a challenge with the presence of a large quantity of relatively fine mineral particles. Theories and mechanisms that rationalise the effects of chemical additives as a defoamer or foaming agent in aqueous can be applied as a general rule, but do not provide the most effective solution for mineral slurry systems. Silicone-based defoamers, particularly their O/W emulsions, have been widely and successfully used at various stages of mineral processing to efficiently control the mineral slurry foaming properties. However, their foaming control ability could be significantly impacted by different processing aids used during the upstream process. In a zinc-rich mineral system, a silicone-based defoamer starts losing the properties necessary to effectively control foaming in the leaching circuit when throughput is increased. In this study, the synergistic defoaming/anti-foaming effects from surfactants and silicone-based defoamers are discussed to effectively control foaming/froth in the zinc leaching circuit, even with lower total defoamer dosage. Improvements during mineral processing can also lead to a cleaner mineral grade by reducing waste.

Floor failure on the longwall face is a complex problem due to the dynamic interaction between the surrounding strata and powered roof supports involving multiple variables. It can also be highly problematic and costly when not well understood as there are significantly less remedies available post event. This was the early experience at the case study mine where significant lost time was incurred due to unplanned delays in the first two panels. To detect and prevent such occurrences is clearly the best approach, hence a tool to predict both floor failure occurrence and magnitude prior to mining was developed. Several parameters were determined as the key drivers for the failure events that had been experienced at this site. These parameters were, in list of influence from highest to lowest, material strength, retreat rate and the presence of water. The parameters were evaluated using a semi quantitative risk rating system ‘Heave Risk’ and given a weighting per their apparent influence. This ‘expert system’ methodology was selected as the use of risk rating systems in underground coal mining is well established. The heave risk model was first applied to previously mined panels. It accurately identified all areas of known major floor heave, with clearly defined areas of low and high risk apparent. The heave risk model was then used prior to mining and evaluated post mining based on observed conditions. It was found to also be highly accurate for prediction. The variability of each parameter and its influence on the overall risk rating is illustrated and discussed.

At Newmont’s Jundee Operation, exploration and resource geologists collaborated to produce an open pit resource model. Using a variety of modelling and estimation techniques but no new data, an uneconomic underground target was converted into a potentially economic open pit evaluation. During an industry downturn, it is more important than ever to efficiently share expertise, ideas and resources to problem solve and produce meaningful project outcomes. The two departments brought together different methods and ideas, and the process showed that certain methods worked better than others in different situations, thus the appropriate method was applied to each part of the evaluation. The results were used to create new procedures and improved lithology modelling and resource model estimations. Many young mining engineers do not consider consulting as a possible job, believing that many years of working on-site is required to achieve practical experience, develop expertise and acquire industry contacts. However, mining consultancies employ engineers with varying levels of experience, including part-time university students, graduates and engineers who are relatively new to the industry. This paper outlines the role of a young engineer in a large mining consultancy and the benefits to both the consultancy and the client of having a mix of consultants with a diverse range of experience. From the authorÆs perspective, the work experience, lifestyle and rewards of mine consulting are compared to site based work, including a discussion on whether consulting is able to offer the professional development that a young engineer needs.

Ion exchange (IX) as a primary production technique has been applied commercially since the 1950s; its application in the uranium industry continues to be widespread and is utilised to a lesser degree in the gold industry. The majority of ion exchange applications, outside of gold and uranium, have primarily been for secondary metal recovery and impurity removal. Newer developments in the application of ion exchange, as well as changes in regulatory requirements, are bringing about the wider use of ion exchange as a primary production technique not only for high value metals such as uranium, platinum group metals (PGM), refractory metals and rare earths, but also looking at base metals such as nickel and copper. Successful management of geotechnical risk at a mining operation is more than just having a good ground control management plan or using advanced numerical modelling and the latest monitoring technologies. The fundamental requirements for effective geotechnical risk management include identification of potential failure modes, identifying and implementing appropriate controls, and demonstrating the effectiveness of those controls. Effective geotechnical risk management requires an integrated approach by confirming that appropriate risk management systems have been identified and implemented (management plans, procedures etc), and that these systems are executed consistently in the field across all working areas. Development of a thorough integrated auditing process executed by an experienced team of technical subject matter experts with substantial operational knowledge, provides a more complete picture of catastrophic hazard management. Following consistent evaluation criteria allows benchmarking of systems and implementation compliance between the sites being audited, and the results can then be ranked against other operations in an industry database.
