In some parts of the world explosions in mines are more commonplace events. Even in the more developed nations explosions have occurred in recent times. In some cases these mines have significantly lowered their lost time injury frequency rates and as such considered themselves to be ?safe? operations. The consequence of these explosions has been devastating. It is not just the loss of those underground, but also to the families that have lost people in them and the broader community that has lost friends, income, business opportunities, and in many cases support and sponsorships. It begs so many questions, ?What went wrong??; ?Why were the circumstances not foreseen??; ?What could have been done to prevent it??; ?How is it that no one stopped it, reported it, fixed it etc.??. Being held accountable for negligence, should not be overlooked, however this may not aid the processes required to find all the answers. If the nature of explosions is understood, the circumstances and tools to prevent them are readily available then the occurrence and reoccurrence are due to weaknesses or inadequacies of the systems of work or in the implementation of them. This paper restates the fundamentals of gas explosions, examines the options to control the underground environment through ventilation, gas management and monitoring, education and training. Case studies have been used to examine systematic failures in ventilation management. In some of these cases it is not so much, the system that had been developed, but the interpretation and implementation of the system that resulted in the failure and subsequent loss.

In the Evraz Highveld production process, iron ore is prereduced in rotary kilns prior to charging into submerged arc furnaces (SAFs). Owing to the highly aggressive slags that are formed within the combustion zones of these kilns, chrome-oxide-containing lowcement castables have been used for some years in the kiln lining. Due to concerns around the potentially harmful effects should Cr(V1) be present in the spent linings, Highveld and its suppliers of kiln castables devoted time to developing alternative chrome-free lining materials. Such materials needed to be cost-effective and ensure good performance and improved kiln availability. This paper discusses all the parameters surrounding this development. Southern Africa has a history rich in the pyrometallurgical processing of ores. Two of the alloys produced are high-carbon ferromanganese (74?83 per cent Mn) and silicomanganese (59?67 per cent Mn and 14?31 per cent Si). The largest land-based manganese ore deposit in the world is based in the Northern Cape Province of South Africa. In 2010, 6.3 Mt of ore from the deposit was exported, mainly to China, Norway, Japan, and India, and an estimated 1.1 Mt beneficiated locally to produce manganese ferroalloys. In southern Africa the producers of manganese ferroalloys are Metalloys and Assmang, and of silicomanganese Transalloys, Mogale Alloys, and Match Corporation. Submerged arc furnace technology is utilized in the production of high-carbon ferromanganese (HCFeMn) and silicomanganese, applying the discard slag practice in both instances. The carbon content of HCFeMn is reduced using converter technology and that of SiMn using the Perrin process. Ninety per cent of the world?s manganese ferroalloys production is consumed by the steel industry. The consumption (and production) of manganese ferroalloys therefore closely follows worldwide steel production. From 2001 to 2010, the doubling of the worldwide production of manganese ferroalloys can be attributed to an increase in production capacity in Asia and Oceania, with the largest increase in the production of SiMn. Manganese ferroalloys produced in Asia and Oceania are mainly for the local market, with North America, Europe, Russia, and Turkey the major markets for alloys from southern Africa. The major challenges faced by the southern African manganese ferroalloy producers are increasing electricity tariffs and productivity of labour, which require a concerted effort by all parties involved to achieve a satisfactory solution. The history of mill liners has evolved over time and this paper tracks the changes and development of liner technology. It provides an overview of the primary types of liners, the design and the materials of construction. The design of a mill liner is driven by application, materials of construction, casting technology and handling issues. The choice of solid steel, grid liners, rubber or composites and magnetic liners is based on many factors.The methods used for monitoring wear and predicting eventual change out time are many and varied. The contribution of discrete element modeling (DEM) analysis in predicting wear profile is discussed.Mill trajectory and DEM software programs are used with great effect to predict the effect of lifter height, face angle, high low lifters or mill speed. In the field, trial and operational experience is used or improvements/design concepts at one operation are applied to another. Broken mill lifters, accentuated wear and other problems can be better understood using hardness testing, chemical analysis and photo micrograph analysis of pearlitic or martensitic structure along with fracture face observations. This forms a part of the typical root cause analysis. However given correct and sound liner and lifter, feed and discharge design media selection, and materials of construction, the majority of premature liner or bolting failure is normally attributed to incorrect operational methods such as excessive high ball to feed ratio, incorrect mill speed relating to lifter ball throw, incorrect media selection, "tramp" material entering the mill charge from upstream or recirculating process and understanding the specific mill operation in question.It is hoped that this paper will benefit mill operators who are faced with challenges of improving wear life and at the same time optimizing mill throughput and performance.CITATION:Connelly, D and Thompson, C, 2013. Studies to enhance the FLSmidth forced air machine through changes in the design, showed important improvements through changes in the number of Rotor blades, stator designs, rotor air distribution and other designs features. The main outcomes from this study highlighted that increasing the number of rotor blades increased the selectivity while decreasing power requirements while stators with slots have better results in terms of recovery, flotation efficiency and selectivity than without slots.Also the rotor air distribution makes a difference in the design when comparing different distribution criteria whilst keeping all features of shape and number of blades etc constant. At the most efficient rotor air distribution design, copper recovery reached one of the highest values while keeping good selectivity. The possible explanation for that is the increase in the pump capacity indicated by the increase in the specific power.It was seen as significant that reduction in specific power for some designs tested compared with the then current standard design had an average of 30% less power consumption.As a result, FLSmidth was able to identify new designs that decreased power and improved recovery, as well as, identifying some designs that specifically improved fine and/or coarse particle recovery.CITATION:Silva, R, Caldwell, K, Bell, D, Olson, T and Vianna, S, 2013. Ernest Henry Mining (EHM) is a copper, gold and magnetite mining and processing operation located 38 kilometres north east of Cloncurry in the Mount Isa - Cloncurry mineral district of north west Queensland, Australia. The EHM ore body is an iron oxide copper gold deposit with an average grade of 1.1 percent copper, 0.5 g/tonne gold and 22 percent magnetite.At the end of 2009, after a series of promising laboratory test work, EHM converted the ball mill media charge from forged steel to high chrome media. Laboratory test work also indicated that with the use of high chrome media, the pH was able to be reduced with no negative impact on plant performance. At high pH values (above 10.5), the forged media is passivated, so as the pH of the system drops the media corrosion rate increases and floods the system with corrosion products, affecting flotation. As the high chrome media is much more inert, pH has less of an effect on copper flotation response.This paper provides an overview of the effects of reducing the pH in the EHM copper rougher flotation circuit. The pH in the roughers was dropped from 10.6 to 9.6 to determine the impact on the rougher performance and the subsequent performance of the whole copper flotation circuit. It was found that there was a slightly increased improvement in copper recovery and no statistical change in either the rougher or final copper concentrate grades for copper and gold.With the reduction in pH, there was a noticeable reduction in lime usage from 0.85 kg of lime per tonne of ore processed to 0.55 kg/t. "Los Pelambres, majority owned by the Chilean company Antofagasta Minerals, is one of the leading producers of copper and molybdenum in the country. It is located high in Andes Mountains, over 3000 metres above sea level, 316 km from the nation's capital, Santiago.The maintenance service supplied to Los Pelambres over the past 13 years has evolved to the point that FLSmidth now provides an integrated service crucial to the management of their customer's assets. The contract initially involved specialty mechanical, electrical and instrumentation maintenance and has developed to encompass maintenance planning and plant control systems.The integrated partnership has delivered excellent results in the areas of improved availability, reliability and return on investment. The operation is now amongst the industry leaders in plant availability.The integrated maintenance service has delivered: An increase in plant availability from 93%, in the first year, to 96.5% today, generating approximately $50 million additional income per year, Quality work procedures, Process and equipment improvements, Major shutdown planning involving more than 700 personnel, 6000 parts and over 2 000 discrete activities.The ongoing success of the partnership requires a high level of understanding between both companies. "The SUSOP« Framework is a new technique that helps incorporate sustainable development principles and objectives into the design and operation of businesses. Developed through the Co-operative Research Centre for Sustainable Resource Processing, it brings a design methodology to sustainability, similar to what hazard and risk techniques such as HAZOP did for safety.Metallica Minerals Limited (æMetallicaÆ) adopted the SUSOP« framework to help identify opportunities and risks that could directly improve or affect its flagship SCONI (nickel-cobalt and scandium) project. This project has the potential to become the worldÆs largest producer of scandium oxide (æscandiaÆ) which is used in solid oxide fuel cells and strengthens aluminium alloys. Located northwest of Townsville in Queensland, the plant site will be located within the previously mined and partially rehabilitated Greenvale Nickel Mine. By applying the SUSOP« framework through a series of multi-disciplinary workshops and offline analysis, Metallica was able to identify numerous sustainability opportunities and risks. Opportunities were clustered together to form concepts to develop a more holistic understanding of the projectÆs overall potential contribution to sustainable development as well as determine where key risks could be mitigated. Four key concepts were identified: Waste and Water Workforce Community and Enterprise Development Energy and Greenhouse Gases (GHG). Metallica was able to include a range of initiatives that could deliver enhanced environmental, social and community outcomes into their mine development plan. These included measures to reduce reliance on water, reduce the volume of waste, provide local opportunities for training and skilled employment and create support for local enterprise development. To achieve an efficient separation in the flotation process, valuable particles need to be selectively segregated from the gangue while utilising a minimum number of flotation stages. TodayÆs existing flotation technologies are effective for fine particle size classes; however, limitations due to particle buoyancy and bubble-particle detachment restrict the effectiveness of traditional flotation approaches on coarser particles. As a result, flotation circuits are generally configured to process only fine particle size classes in order to maximise recovery in the primary stages. Eriez has invented a novel flotation technology, the HydroFloat, which overcomes these limitations by carrying-out flotation in a dense, fluidised-bed medium allowing for the recovery of coarse particles (>0.250 mm). Over the last 15 years, this technology has been successfully applied to industrial minerals with several full-scale units installed to recover particles up to and exceeding 3 mm diameter in primary flotation circuits. This paper includes discussions on the history of this technology, process theory, existing applications, and a review of recent sulphide-based laboratory test work conducted for Newcrest mining and others. Feed slurries are typically flocculated in a thickenerÆs feedwell to increase the settling rate, underflow density and overflow clarity. High molecular weight polymeric flocculants are normally used, but present a number of challenges in terms of make up, dilution, delivery and mixing with the feed slurry.Various aspects of the flocculant dilution and delivery system have been studied with a combination of flow visualisation and computational fluid dynamics (CFD). An open topped flocculant dilution/mixing box has been developed to replace a conventional flocculant manifold, with improvements to the mixing, fault diagnosis and ease of determining the flow split to the various dosing points. The addition points to the feedwell have also been studied, both in terms of where the flocculant is added, and how. While conventional multi-hole spargers are effective in dispersing flocculant, they are prone to blocking and have been replaced by open-ended pipes. The required dispersion is provided by an efficient mixing element placed in the dilution port. Adding the flocculant into the feedwell dilution stream prior to mixing with the slurry also provides super-dilution of the flocculant that can further improve the flocculation process and reduce flocculant consumption.CITATION:Heath, A R and Arbuthnot, I, 2013. In June 2012, Lucara Diamond Corporation officially commissioned its Karowe Diamond Mine in Letlhakane, Botswana. The processing plant circuit incorporates variable speed autogeneous milling, a first for African diamond processing - only previously practiced in Russia. The paper discusses the basis for the selection of milling against the more traditional circuits incorporating crushing and scrubbing. It covers early commissioning challenges of operating a mill in an æalien environmentÆ with no immediate reference plant to look up to for bench-marking.The paper describes how these challenges were overcome and learnings quickly incorporated into the mill operating philosophy for maximum diamond liberation and minimum product damage. The plant was commissioned in three months and by August 2012 the production ramp up had reached name-plate capacity. The feed to the mill has varied from highly weathered material to extremely hard kimberlite and the operating philosophy had to be optimised for the full range of ore types. Nowadays, the demand for a finer grind in minerals processing has set new challenges within the industry, particularly for grinding technology. Apart from a finer grind, energy efficiency is also a key driver in optimising comminution technology.Stirred milling technology, already proven in many other industries, delivers a modern, sophisticated, flexible and energy efficient grinding solution for fine and ultra fine grinding in minerals processing. Fine grinding technology is utilised particularly in concentrate regrinding to complete the liberation of valuable minerals after the first stage of concentration. Maintaining the correct particle and particle size distribution of the regrind product is crucial for downstream process performance. Stirred milling technology offers the unique possibility for online control and optimisation of the regrind product particle size. This paper describes the basis of this online control possibility and discusses the implementation of stirred milling technology in minerals processing, particularly in regrinding applications. The Autogenous Kelsey Axial Displacement (KAD) mill is essentially a short retention time centrifugal mill, designed to effectively harness extraordinarily high concentrations of input energy for efficient particle size reduction. The mill which has an operating duty band extending from primary to ultra-fine also has the unique capacity to generate fine and ultra-fine products from single stage size reductions of either coarse or fine mineral feeds, in both wet or dry operating modes. This paper covers developments following a recent optimisation programme which confirmed that mill shell speed and the velocity differential between the shell and the rotating internal disc assembly tips could be optimised in relation to specific energy for a given feed rate and feed size providing the product size remains constant. Subsequent test-work further demonstrated the versatility of the mill with duties ranging from the removal of surface contamination associated with heavy mineral concentrate particles to the production of premium ultra-fine talc, from a single stage of size reduction. Open circuit grinding of a rougher gravity concentrate to liberate gold ahead of leaching also highlighted the scope for the application of low cost KAD milling for a range of hydrometallurgical feed preparation duties, which involve fine and ultra-fine grinding.CITATION:Kelly, J and Kelsey, C, 2013. Ernest Henry Mining (EHM) is a copper, gold and magnetite mining and processing operation located 38km north east of Cloncurry. The EHM ore body has an average grade of 0.9% copper (chalcopyrite), 0.5g/t gold and 22% magnetite.Open pit mining commenced in March 1998, but EHM transitioned to underground mining in December 2012, extending the life of operations beyond 2024. The underground mine currently produces up to 3Mt/year of ore, hauled up a decline by truck. This figure shall double in 2014 with the completion of a 1000m shaft. The EHM Concentrator has a nameplate capacity of 10Mt/year. In June 2011, Xstrata Copper acquired the Mount Margaret resource to supplement underground production and utilise the full capacity of the Concentrator. The Mount Margaret Mining (MMM) operation was established to develop the five deposits making up the resource by way of open pit mining.Copper concentrate is produced by the EHM Concentrator via grinding, flotation and dewatering circuits. The grinding circuit comprises a SAG and ball mill in a closed AB configuration. Flotation consists of a rougher train, regrind and three stages of cleaning, with the tail from the first stage reporting to the tailings thickener.The mineralogy of the MMM ores is complex, comprising fine textures of active pyrite, copper oxides, native copper, clay and secondary copper sulphides. This paper provides an overview of the metallurgical test work completed to forecast the performance of the MMM ores through the EHM Concentrator and identify flow sheet modifications for maximising copper recovery and producing a saleable concentrate.
