The Holmes Safety Association was organized and named in honor of the late Dr. Joseph A. Holmes, first director of the United States Bureau of Mines. The association is a national organization and operates through local, sectional, or State chapters in mining, petroleum, and allied industries. The National Council of the Holmes Safety Association consists of representatives of State councils and local chapters, with the Director of the Bureau of Mines as president.

During the fiscal year ended September 30, 1932, there were 27 fatalities in the coal mines of Indiana. Four of this number occurred in small mines employing less than 10 men, over which the State Inspection Department has no jurisdiction. In addition to these coal mine fatalities, an explosives accident in a clay mine resulted fatally to one man. Tables 1 and 2, which follow, list the fatalities as to cause and occupation. This U.S. Bureau of Mines report identifies the types of serious accidents that occur most frequently at small underground coal mines and describes the strategies that could help prevent these accidents. A wide variety of methods for improving safety are suggested, including improvements in the design of equipment, work procedures, work schedules, safety programs, and emergency response plans, as well as techniques for diagnosing the potential hazards associated with new technologies and work procedures. Some of the papers in this volume focus on preventing specific types of mining accidents--ones associated with materials handling (primarily back injuries), equipment maintenance, improper machine guarding, and ground failure during retreat mining. Although the recommendations in this volume are heavily influenced by research performed at underground coal mines in the Appalachian coalfields, most of the papers contain advice that is equally pertinent to almost any type of mine.

West Virginia University researchers examined risk and exposure in the roof bolter operator's job and developed approaches for preventing accidents and for evaluating system and human performances that may be appropriate as a general application to improve safety and efficiency in the industry. Using a microanalysis of accident data and underground observations, the researchers classified bolting injuries within one of four work routines: 1) drilling bolt holes; 2) bolt installation; 3) machine tramming, (re)positioning and setting the ATRS; and 4) general face preparation. Time and motion studies were then combined with injury data analysis to develop risk and severity indices, as well as a severity-weighted index that served to determine the degree of risk and exposure by work routine. More than two dozen bolting-related problems were identified as potential situations that could lead to injuries or expose workers during their job. Approaches to avoid these situations were suggested and then applied at six mining operations to evaluate specific problems in roof bolting operations. These suggestions encompassed the three E's of Safety (Engineering, Education, and Enforcement) and emphasized the analysis of worker behaviors that lead to human error accidents.

Cable reel systems for shuttle cars were investigated to determine whether an improved system could be identified. Interviews and mine visits were conducted to better define limitations and problems with present systems. Although simple and relatively inexpensive, present cable reel systems are not fully satisfactory. Several concepts for a different technical configuration were formulated and evaluated. An all ectric system using electronic control and external sensors was recommended for further development.

The Missouri-Kansas-Oklahoma lead and zinc district, better known as the Joplin district, includes the mines in southwest Missouri and in those parts of Kansas and Oklahoma that are directly adjacent. (See PI. I.) This report of investigations carried on by the Bureau of Mines gives the methods used in mining and milling and indicates in some detail the conditions that affect the efficiency of those methods; it does not attempt to discuss the geology of the district, except incidentally, as this has been described in numerous reports by several geologists. Some of the more important recent publications are those of the United States Geological Survey by Smith and Siebenthal, a Bain, b and also those of the Missouri State geological survey by Buckley and Buehler." Other reports are those by Schmidt and Leonard," and Winslow,e of the Missouri geological survey, by Haworth and Crane,! of the Kansas geological survey, and by Jenney.g Although the lead and zinc ores are closely associated, the proportion of zinc ore mined at present is far greater than in former years. For this reason and because the losses in milling zinc ores are proportionately greater than in milling those of lead, the zinc ores are here given most consideration. Only the mines and mills actually examined and tested are discussed in detail, statements of results being restricted to the mines in general, with no special reference to anyone mine. Several mill tests were made to determine in a general way the efficiency of the milling practice in this district and to discover at what stage of the process the losses were greatest. Making these mill tests proved difficult,for, because of the arrangement of the mills and the necessity of estimating the tonnage treated, systematic mill testing and sampling can be done at only a few plants. Many tables giving results of mill tests and screen tests have been omitted because they would serve only to emphasize the important points shown by other data given. The writer has tried to bring out the possibilities of making certain improvements that would effect a greater saving, and has endeavored to avoid the use of technical terms and phrases in order that all statements may be clear and easily understood.

A series of 15, stations beginning with Potsdam and Budapest and ending with Budapest and Potsdam were occupied inn the period August, 1908, to January, 1909. The intermediate stations were all at points which had been connected by Baron Eötvös with torsion-balance traverses. The observations were made with an invariable pendulum of the standard four-pendulum type of instrument of the Geodetic Institute, made by Stückrath at Berlin-Friedenau. The observations at each of the field stations lasted-one to three days with one exception; two or three time determinations were made; four to eight series of pendulum observations were made; the error of determination of the elevation of the instrument above mean sea level ranged from 0.30 meter to 1.10 meters, with the exception of the Arad and Baja stations, where the elevation was known more accurately and averaged about ± 0.8 m, which would cause an error of ± 0.25 x 10-3 dynes. No topographic correction was applied. The values for ?g were taken from Eötvös: Report on Geodetic Observations in Hungary, especially Observations with the Torsion Balance, Budapest, 1910. The differences in ?g as determined, by the torsion balance and by the unvariable pendulum were: Kuvin-Hidegkut 1.2 km. 0.001, Livada-Kuvin 9 km. 0.001, Pankota-Livada 13 km. 0.004, Kuvin-Arad 20 km. 0.000, Pankota-Arad via Kuvin 42 42 km.0.005, Pankota- Arad via Kurtics 50 km.0.000 dynes. Oltay concludes that if the stations are close enough together and if the terrane is rather flat, the measurement of ?g with the Eötvös torsion balance is as accurate as with the invariable pendulum. -- Donald C. The work of the Mining Division is divided among seven sections, each in charge of a supervising engineer, as follows: 1. Metal-mining methods section, the supervising engineer of which is also supervising engineer of the Southwest Experiment Station at Tucson, Ariz. 2. Metal-mining research section. 3. Nonmetal-mining section. 4. Coal-mining section. 5. Mine-ventilation section. 6. Mineral-industries survey section. 7. Electrical section. The names of the sections indicate broadly the fields covered by each, and in the following, pages the objectives and type of work of each section will be summarized briefly and their active problems listed. The work clone during the fiscal year 1937 will also be summarized.

This report examines the feasibility of Employee Assistance Plans for the coal industry. Three issues are examined in detail. First, the prevalence of off-the-job factors such as alcoholism, drug abuse, and marital problems are examined in a sample of miners from the eastern, midwestern, and western underground coal fields. Then data were presented that examined the link between these off-the-job factors and behaviors critical to the mines such as absenteeism and turnover. Lastly, we examined the effectiveness of the major Employee Assistance Program in underground coal mining--the HELP Program. The report contains recommendations on whether coal mining companies should implement these programs and how these programs should be designed.

In an earlier series of 15 Reports of Investigations on Smelting in the Lead Blast Furnace, experimental data were given on the operations of a custom smelter, a smelter treating rich lead products, and a smelter treating highly zinciferous charges. Five "reports" were written on each of the three types of operation, which were later compared in a summary written for the Institute. Developments in each of these types of operation in lead smelting since the last of the above reports was written have continued and have followed modifications necessitated by changes in the nature of the products received for smelting. The sintering-blast furnace-smelting plants have improved their sintering operation .and have made "2-pass" sintering a more standard operation; the richer lead feeds are diluted when marginal precious metal ores are not available to prevent fusion of the sinter cakes and to permit air to contact the sulfides. Means have been devised to make it easier to remove metallic lead from the wind boxes, but no attempts have been observed to use the D & L somewhat as an ore hearth to make more metallic lead. Except in those cases where zinc-rich lead blast-furnace slags were to be retreated in a "slag-fuming furnace," the sintering and the blast-furnace operations were so carried out that a relatively "clean," lead-poor, final slag was discarded.

The national mine-safety demonstration at Pittsburgh, Pa., was projected and undertaken in the hope that it would aid the increase of safety in the mining industry. That the national mine-safety demonstration was warranted and has served a useful purpose was evidenced by the attendance, not only at the public demonstration in Forbes Field, but also at the more special exhibit of the work of the Bureau of Mines at the arsenal grounds and at the experimsntal. mine. Nearly 2,000 persons witnessed the demonstrations and exhibits of the Bureau of Mines at the arsenal grounds on the morning of October 30, 1911. The spectators were all mine operators, miners, or persons interested in mining, and represented operations in all parts of the United States. Nearly 1,200 of these persons visited the bureau's experimental mine at Bruceton to witness ~th~explosion of coal dust. Had the weather, which was exceptionally inclement, been more favorable, there would doubtless have been a much larger attendance. At the public demonstration in Forbes Field, which· was witnessed by President Taft, from 12,000 to 15,000 persons, chiefly mining men, were present and attested their interest and enthusiasm in the minesafety exhibits by remaining throughout the proceedings, regardless of the rain which fell all the morning .. Sincethis demonstration, the organization of first-aid corps and of rescue corps has been undertaken at mines where there had previously been no such organizations. The bureau has received letters of commendation attesting the value of the demonstration in drawing· attention to the dangers and to possible means of greater safety in mining. Many of these letters have urged that the results of the demonstration be published for the guidance and instruction of those who were present. In response to these reauests the following account of the mine- BlIIeLY demonstration lias been prepared. IIi its preparation various division and section chiefs of the Bureau of Mines have cooperated. The authors acknowledge the assistance they have received from those section chiefs and other members of the bureau who contributed detailed descriptions of apparatus exhibited at the demonstration or of investigations represented.

This report is the 20th in a series summarizing Bureau of Mines research and technologic work on coal and related investigations; it is the second4/ to be issued on a calendar-year basis. Previous reports related largely to publications by Bureau authors during the reporting period. This one discusses the publications issued during 1955 and in addition describes briefly the work in progress during the year. Further details on the published work may be found by referring to the publications cited in the footnotes. The Bureau continues to direct its research and technologic work on coal toward safer and more efficient mining, improved preparation, and more effective utilization, with the ultimate twin goals of promoting conservation and insuring that coal will make its maximum contribution to the Nation's industrial capacity. Significant advances toward these goals were made in 1955.

This paper describing the mining practices at the Magma mine, Superior, Ariz., is one of a series being prepared by the Bureau of Mines on mining prac¬tices, methods, and costs in the various mining districts in the United States. At present (1929) practically all the mineral production in the Superior district is from the Magma mine of the Magma Cooper Co. The ore is mined for its copper but contains associated silver and gold. Both direct smelting ore and ore of concentrating grade are produced. The mining methods now in use at the Magma mine were adopted or developed as knowledge was gained of the physical condition of the ore bodies and the inclosing formations; the wall rock is soft and crushed and the ore is relatively hard. In general, existing methods have been adapted to fit local conditions, but some of the mining practices used were originated at this mine. Most of the ore is mined by a combination of rill stopes and pillar stopes. The pillars are extracted by underhand mining.

Increased application and interest in the use of microseismic monitoring to deal with ground control problems, Particularly those resulting in violent failures, has brought about the need for practical guidelines with respect to all phases of microseismic monitoring. This report presents the results of an 18-month effort to review the microseismic method and develop needed practical guidelines. Microseismic activity and properties of microseismic events are treated so that these parameters can be taken into account when developing monitoring equipment. Microseismic equipment is discussed to provide familiarization with the components of monitoring systems. Factors to be considered and procedures for developing a microseismic monitoring system are presented. Both physical and electrical considerations for the installation of a monitoring system are discussed in detail. The operation of a microseismic system including data analysis procedures is presented. Field examples to illustrate use of microseismic data in evaluating ground control problems are presented in a number of case histories.

Through the series of 18 presentations compiled herein, Bureau personnel covered a wide range of topics including U.S. chromite and chromium trends and usage patterns; foreign and domestic resources; and research activities on mineral characterization and beneficiation, chemical and pryometallurgical processing of chromite and chromium-bearing wastes, new low-chromium alloy and coating substitutes, corrosion- and oxidation-resistant stainless steels, and chromium-containing refractories.

From September 1978 to December 1983, ESD Corporation (formerly a Division of FMC Corporation) undertook the development and testing of a bidirectional auger under contract with the U.S. Department of Energy. Underground testing began in August 1981 at Peabody Coal Company's Marissa Mine near Marissa, Illinois. The auger was developed to increase mine production and coal recovery without increasing land subsidence. It bored 4-foot-diameter by 10-foot-deep holes into pillars on 7-foot centers (each hole recovering 5 tons). All operating during this test was performed by maintenance personnel with the goal of finding ways to increase the machine potential. The maximum excavation rate achieved was 1.4 tons per minute. It was concluded that the concept of drilling short holes to extract coal from underground pillars without surface subsidence was practicable with a wheel mounted auger, but that more power was desirable for this mine, and that the hydraulically driven augering system that provided experimental versatility was not sufficiently reliable for production use.

In the last several years, the coal mining industry has made significant changes in the design of longwall mining systems to make them more productive. Some changes, such as increasing the length of the coal faces and increasing the capacity of the face conveyors, have resulted in problems with achieving sufficient motor torque to start and operate the longwall face conveyors. Other changes, such as increasing the size and/or decreasing the percent impedance of longwall power center transformers, have resulted in problems with the short-circuit currents on the transformer secondaries exceeding the interrupting ratings of the molded case circuit breakers used to protect the secondary circuits. A survey of 24 longwall mining systems in northern West Virginia was conducted to investigate both problems. The results of the survey are presented and analyzed. Several recommended solutions to the problems are presented to enable the coal mining industry to design safe and productive longwall mining systems.
