The identification of Critical Risks, their associated Critical Controls, and the implementation of processes to ensure their ongoing management can be difficult and resource intensive. Answering the following Critical Risk Management related questions can be challenging: • What is a Critical Risk? • What is a Bow-tie Analysis and when should it be used? • What is Critical Control? • How is the effectiveness of a Critical Control assured, and how is this measured? • Who is responsible for managing Critical Controls? • What are the implications if a Critical Control is not effective? It appears that in the mining industry’s efforts to implement the Critical Control Management Process outlined by the International Council on Mining and Metals (ICMM), instead of embedding an integrated, well understood, value adding process, we have instead created a resource hungry monster that is poorly understood by numerous layers of the organisation.

The easiest way to mine valuable orebodies is through the use of explosives and well-planned blasting layouts that produce fragmentation profiles designed specifically for conveyance equipment in underground mines. The design of blasting patterns is specific to not only the explosives being used but also the rock or ore type being fragmented. Many blasting designs are based solely on the experience of those developed for similar orebodies in terms of properties and orientation or individuals that have some blasting experience.The primary technical objective of this paper is to outline a unique methodology for determining underground blasting methodologies (developing specific blasting parameters) commensurate with explosive energies and rock/ore equations of state for a narrow-vein gold property located in Northern Quebec and for underground mines in general.The procedures developed are to define the desired fragmentation specification so that a specific thermodynamic break is generated, taking into account powder factor, energy factor, tonnages, explosive energies and distribution along with blasthole diameter and orebody orientation for a specific set of explosive and dynamic rock/ore properties. Determining dynamic modulus values for the ore using seismic sensors to measure P- and S-wave velocities, the aforementioned parameters will be utilised to set the design constraints to maximise recovery and minimise overbreak and dilution.New underground blasting software (AEGIS) will use thermodynamic break in conjunction with defined fragmentation profiles to create an array of blasting parameters used to design and plan ring layouts. This software will also allow mining planners, prior to drilling and loading a production blast, to appraise future intended blasting patterns to minimise the potential of diluting ore with host rock and the likelihood of damaging mine support structures.CITATION:Roy, D, Williams, T and Preston, C, 2015. Simultaneous mass-balancing of a mineral processing flow sheet results in adjustments to each data point that reflects the accuracy and precision across the full set of data. In this way, all the good work that goes into designing the sampling and analysis protocol for each sample point is leveraged in order to produce a æbetterÆ set of balanced numbers for every sample point in the circuit. Few operating plants employ whole of circuit balancing techniques to produce reported operating results. This situation is changing with the application of mass balancing technology which utilises redundant data for production accounting and metal reconciliation from product to ore source across multiple phases and multiple time periods. Mine and plant data can now generate credible information to improve productivity. Mine and plant personnel can now use consistent data sets for review and planning. Decisions based on balanced data can be made quicker and with increased confidence.

The arbitration process, and its drawbacks: Any time you have taken part in a major arbitration, and the decision, when it comes out, is not what you thought it would be (which is bound to happen occasionally), the normal reaction is something like æthe arbitrator got it wrongÆ. Again normally, when you have cooled down a bit, and you look in more detail at the decision, you can see, probably, æwhere he got it wrongÆ (it is only male arbitrators who get it wrong). Ultimately, when reason prevails, and you reflect on it, you can see that what really happened was that you failed to get the arbitrator to see what you were getting at in the point or points concerned. This is doubly frustrating, when you consider what has been involved in getting to this point, having been through it all at least twice. An alternative to this process may be the dispute review board.

The Miocene Chichibu granitoids associated with the Chichibu skarn deposit are divided into North Body, South Body and Daikoku altered stock. Orebodies of the Chichibu skarn deposit are mainly distributed in the north-western part of the North Body and Daikoku altered stock. The North Body consists of magnetite-series granitoids ranging from tonalite to granodiorite and to monzogranite from the margins to the centre of the intrusion. The South Body consists of tonalite and granodiorite, and most of these rocks are also classified as magnetite-series granitoids. The Daikoku altered stock consists of a quartz diorite porphyry. The SiO2 contents of the granitic rocks of the North Body and South Body range from 61.6 to 70.7 wt per cent. During magmatic differentiation, Cu, As and Zn contents in fresh granitic rocks of the North Body and South Body increase with SiO2 content up to 63 wt per cent SiO2, and then progressively decrease with increasing SiO2 contents as SiO2 exceeds 65 wt per cent. Estimated pressure for the emplacement of the granitoids is about 0.5 kbars. Cathodoluminescence studies on quartz from those granitic rocks show that vapour rich two-phase fluid inclusions tend to occur in the core of quartz crystals, while halite-bearing poly-phase fluid inclusions occur in the margins of quartz crystals. The distribution of fluid inclusions suggests that fluid composition changed from low to high salinities during the growth of magmatic quartz crystals. Texture showing quartz crystals with an oval shape enclosed by radial aggregates of a mixture of fine-grained quartz, plagioclase and K-feldspar composing the groundmass of the monzogranite also suggests a pressure decrease during solidification of North Body. These findings show that high salinity fluids were present during magmatic differentiation and that Fe-Cu-Pb-Zn-Au-Ag skarn mineralisation of the Chichibu deposit was possibly associated with these high salinity fluids. Uncertainties in large open pit geotechnical modelling has been a major concern for slope stability analysis. The level of confidence in the in-put assumptions, such as structural and rock mass model, dictates the modelling results to some extent. This article is to detail a case study of Kalgoorlie Consolidated Gold Mines (KCGM) regarding a predicted inter-ramp failure on an as-built pit walls by a 3D numerical modelling. Significant concerns were raised across site and multiple preventive plans were considered. After intense data collection work, it was confirmed that the risk of failure had been overestimated. The case study has improved the understanding of geotechnical data in-put and has enhanced the process of creating and updating geotechnical structural models at KCGM.

For the last 50 years, traditional safety management has improved safety performance by: • Fixing the environment (eliminating the hazard). • Improving the system (more/less rules, procedures etc). • Making safety more conscious (more/better interactions about the importance of safety). Although these approaches certainly have merit, and may be part of an overall solution, they are not enough to prevent many modern-day incidents. Today, these three approaches are used more than before, and are used better than ever before. Yet, workplace fatalities in Australia have stagnated from 2013 to 2019, as have the number of incidents, even serious ones. In the USA, workplace fatalities have increased by 15 per cent in the same period.

At some time in the mining life of an open cut coal deposit, consideration is usually given to determine the point at which underground mining becomes an economic proposition or even necessary. This can occur anywhere from the pre-feasibility project stage to decades after commencement of mining. Options which can be considered include the extension of the life of the open cut by use of larger equipment, the utilisation of highwall mining techniques and the introduction of low capital start-up or full underground development. The primary focus is always the minimisation of the total unit cost (including capital )of clean product. The transition point at which an undergound mining operation becomes more economic than an open cut operation is not a simple evaluation and depends on many factors, particularly on the nature of the deposit itself. The main general determining factors of the transition point are related to the three factors of mining resource, the mining method that can be used and mining cost issues. Mining resource issues include geography, geotechnics, strip ratio, geology and the size of the deposit. Mining method issues include highwall mining, extending the life of an open cut, the environment, available roof support technology, product yields and the availability of appropriate workforces. The mining cost factors discussed include capital and operating costs and the impact of productivity and management. All these factors impact to a varying extent on the transition point.

Employing gravity recovery for coarsely liberated gold and sulfide ores has potential to lead to a signifi cant reduction in the energy required for comminution and ultimately can have a significant impact on gold processing. Most of the recognised metallurgical tools at our disposal currently have been designed around recovery of relatively finely ground minerals. In order to change the paradigm of fine grind for liberation, a new set of tools need to be recognised that are capable of separations in the coarser size fractions. Some of these tools already exist and some need to be developed but the key is the way in which the fl ow sheets are arranged. A very strong understanding is required in the existing tools and their capabilities before these new flow sheets can be optimised. Only when we understand the existing gaps in technology can we start to fill these positions with new candidates. Many ores when crushed liberate their gold bearing minerals at coarser sizes than current practice would suggest (Figure 1). In order to characterise this response in ores it is necessary to utilise some new techniques in the lab such as gravity separation and ore sorting at the sizes greater than 15 mm followed by gravity recovery on fractions from 15 mm down to 150 ¦m. The most difficult range has been the mid-sized fraction from 4.0 mm down to around 150 ¦m. This is the size range in which many ores show economic liberation starting to occur and coincidentally this is the range in which many commonly used metallurgical processes reach their operational limits due to physical constraints. Examples of these processes are flotation, hydrocyclones and whole ore leach in agitated tanks which are currently the mainstays of the processing fraternity. This is an ABSTRACT ONLY. In hard rock mining, large-scale bulk mining methods such as block or panel caving (BC/PC) and sublevel caving (SLC) typically generate significant surface deformations such as subsidence. Surface deformations need to be considered in the project evaluation and planning stages of cave mines, for example, planning and location of critical underground mine planning infrastructure such as portals, haulage and vent shafts, access roads, or potential damage to existing infrastructure, changes to surface water management, waste dump, stockpile and tailings dam stability, and environmental impacts. Assessing the impact and risks associated with subsidence is a key component in robust mine planning and operations management.

Post-mining landscapes are required to geomorphologically and ecologically integrate with their surrounds (Martín Duque et al., 2015). Designing and constructing an erosionally stable landform is the first and arguably most important part of the rehabilitation process. Without an erosionally stable landform topsoil and nutrients are lost, shallow groundwater is depleted as well as when excessive erosion occurs, expose suboptimal materials. Here we examine the ideas of geomorphic design and rehabilitation and how the landform can be tested and enhanced using computer based design and landscape evolution models. Specifically we evaluate the complementary capabilities of landscape modelling (SIBERIA) (Hancock and Willgoose, 2017) and geomorphic design software (Natural Regrade with GeoFluv) (Bugosh et al., 2016), in order to maximise erosional stability on post-mining landscapes (Zapico et al., 2018). From its mining lease at the northern end of Cohens Reef at Walhalla in the North Gippsland Mountains, the Long Tunnel Extended Gold Mining Company produced 13,695 kg of gold, which at 1980 prices represents a value significantly in excess of $200 million. In the period 1885 to 1908 the mine was one of Australia's principal reef gold producers, being top Victorian producer in six of these twenty-four years. Prior and subsequent occupants of the lease produced an additional 160 kg of gold. The Extended and the other mines on Cohens Reef whilst contributing importantly to the financial resource of Victoria, were the sole important support for the substantial town of Walhalla that had grown up around them. When the mines closed, Walhalla gradually reduced from a bustling developed town to the present attractive mountain village so favoured by tuurriSts. Open stope mining is the most common mining method employed in underground, hard rock mines in Canada. It is characterised by relatively small, single lift stopes (20 000 to 100 000 tonnes), and fast stope turnaround times. Host wall rock mass conditions, which are typically poor to fair, are the controlling parameter in stope dimensioning. Rock mass stability is maintained by designing all stope surfaces at a stable dimension, and through extensive use of cablebolting for stope backs and frequently stope hanging walls. Dilution control is a high priority. The typically small, high-grade, deep and irregularly shaped orebodies are incrementally developed, usually in a bottom to top direction. In many cases, high stress is encountered due to the depth of the orebodies. Overstressing of pillars and hanging pillars are avoided through the use of pillarless or early pillar recovery mine sequencing strategies. Stopes are invariably filled, and in higher grade and wider orebodies, cemented fill is commonly used. This paper generalises many of the similarities found in Canadian open stope mines.

A major problem in the continuous casting of aluminium killed strip steel is the incidence of black and grey lines in the strip surface caused by the alumina inclusions. One place for limiting the number of alumina inclusions is by providing favourable fluid flow conditions in the ladle so that the inclusions float out to the surface or adhere to the ladle wall. In laboratory equipment 20 kg of steel was melted in an alumina crucible of 14 cm diameter in an induction furnace. Deoxidation was carried out while the melt was constantly stirred by argon bubbling. Bubble frequencies were measured at the lance tip using a microphone technique. High speed cine photo- graphy was incorporated to measure melt top surface velocities. Experimental results indicate that the rate of removal of inclusions is a function of both the gas input and the power input. However due to unfavourable surface area to volume ratio, reoxidation of the melt took place. Some other experimental difficulties in working at high temperatures are also discussed such as lance design, melting arrangement and sampling techniques.

WMC Resources Ltd nickel operations have expanded significantly over the last three years. Production capacity has increased at mine/concentrate, smelting and refinery processes to such an extent that WMC now ranks within the top three or four producers of nickel metal in all forms. The stainless steel industry remains the dominant user of nickel. WMC's forecast is for stainless steel production to grow at 2.8 per cent per annum off of the 1995 peak, with much slower growth in the short-term as the stainless steel inventory overhang unwinds, followed by a return to higher trend growth rates. WMC's forecast for the long-term growth in demand for primary nickel is three per cent per annum off the 1995 peak; again, slower growth in the short-term before a return to higher long-term trend rates. This projection is towards the optimistic end of the range of industry experts. It equates to a requirement for new primary nickel of 25 000 to 30 000 tonnes of annual capacity each year.

This paper examines the interplay between longwall support design/geometry features, operational controls and geological features on ground response. Experience from several investigations is used to demonstrate the influence of factors such as longwall support capacity and geometry, setting pressure, coal seam strength and stiffness, tip-to-face distance and hydraulic supply and control system parameters on longwall ground response. These factors are then used to outline the requirements on key controls such as retreat rate and cutting height and their influence on allowable roof convergence. The ground response curve concept as a means to provide graphical representation of longwall support and strata interaction processes is presented. The approach was developed to address the requirement for a practical longwall support evaluation and selection tool that can take account of support load influences such as changes in roof geology or cover depth. An example of the comparison between single pass longwall and top coal caving in a thick seam environment is given to demonstrate the influence of the various factors discussed.

This study examines the application of Landsat TM data to the identification of hydro- thermal alteration in the arid terrain of the El Salvador region of north Chile. Numerical reflectance values were extracted from the digital Landsat TM data for a variety of rock surfaces, including four parts of the El Salvador gossan, for each of six spectral bands. These reflectance values were analysed statistically in order to select the three spectral bands, combined as a colour composite image, that are most efficient in discriminating different varieties of alteration and for general geolog- ical interpretation. The most cost effective com- posite image for this area is a combination of bands 1,4 and 7 as the blue, green and red components respectively, with simple contrast enhancement. This image is superior to some much more expensive enhancement techniques and allows unambiguous identification of areas of hydrothermal alteration larger than about 50m. The past decade hap seen revolutionary advances in com-munications and information-handling techniques which have been stiriulated by "cold war" military requirements and by, current programmes for probing into outer space: The tools used in exploration for petroleum and minerals have been improved significantly as a result of this new space age technology and even greater improvements are on the way.Two areas of technical activity have been particularly fruitful in enhancing the effectiveness of exploration, methods. These are (1) The development of new approaches, based on information theory, for extracting signal from noise more efficiently than before, and (2) The use of high-speed digital computers for storing and processing all kinds of exploration information, not only geophysical but geological as well.In seismic prospecting, use of information theory has led to new techniques such as time-domain filtering, wave-number filtering and correlation methods which have made it possible to obtain usable reflection data from areas which were previously considered NR. Complex playback and other data processing facilities have been developed to apply these approaches.

The efficient operation of large haul trucks in large open pit mines requires an economic trade off between the haul route quality, the haul truck payloads and the level of haul truck reliability. This paper describes an investigation into the relation- ship between haul route surface quality and the level of structural reliability of a haul truck main frame. An instrumentation system for unattended recording of data on a haul truck operating normally in a mine and com- puter based analytical procedures were developed and used to identify which road segments contribute significantly to fatigue cracking at various sections of a haul truck main frame. This paper discusses the implementation and potential limitations of the method for estimating sampling and analytical errors of multiple source mine output blended into a shipping product, described by Everett and Ziegelaar (2010). The practice of quality measurement varies between iron ore miners. The common objective is to monitor effective resource utilisation and control of product quality to customers. A flow diagram is given presenting the various options used by iron ore mine operators and the merits of the different options are discussed.
