Injury Prevention: Dependent, Independent and Interdependent In recent years, management-directed (dependent)safety programs have lost ground to the more successful self-directed (independent) programs, where employees take responsibility for their own safety. Today, with the growth of the team concept in industry, a mutual-caring (interdependent) approach to workplace safety is gaining wide acceptance. This presentation compares characteristics and results of the three progressive levels of safety responsibility and explains how to develop a successful interdependent program. The presentation will also detail the common principals that are necessary for safety excellence under any of the management styles.

The design requirements for mine ventilation seals have undergone a radical transformation in recent years, and these revisions have greatly increased the cost of the seal designs and their construction. For example, in the past two years, new federal regulations have increased the minimum design requirement to withstand explosion pressures from 138 kPa (20 psig) to 345 kPa (50 psig) or 827 kPa (120 psig), depending on whether the sealed mine volume is monitored continuously or not. Moreover, there is still a possibility that under certain conditions (such as detonations) even higher pressure requirements may be necessary. The ability of a monolithic, stand-alone mine seal to reliably withstand the full range of current and future pressures is becoming increasingly difficult and expensive. In an effort to develop a practical alternative, the West Virginia Office of Miners? Health Safety and Training (WVOMHST) and the National Institute for Occupational Safety and Health (NIOSH) have collaborated in a research effort to develop a practical, economic, and safe mine sealing technique that can enable mines seals to meet the full range of new explosion pressure design requirements. The basic idea is to use a barrier of common mine gob and rubble in combination with a conventional mine seal so that the pressure resulting from a gas explosion is reflected, absorbed, and attenuated, so the pressure on the ventilation seal is reduced. This paper discusses some concepts and preliminary test results for a ?Passive Mine Blast Attenuator? (PMBA) that can provide a useful alternative to increasingly larger and stronger stand-alone mine seals. Numerical models and full-scale experiments conducted at the NIOSH Lake Lynn Laboratory Experimental Mine (LLEM) show that the use of a PMBA can signficantly reduce the blast pressure and impulse on conventional ventilation seals. In 1997, a group of Canadian mining companies met to review the Mine Environment Neutral Drainage (MEND) program and to consider future activities. They concluded that (1) that Canadian industry and Natural Resources Canada (NRCan) should fund a small secretariat in Ottawa to support technology transfer in Canada and, (2) that it was important for industry to continue the momentum of MEND on an international scale. The need for the International Network for Acid Prevention (INAP) was strongly endorsed by consultants, company, university, and government experts from Australia, Canada, and the United States who convened in Vancouver in February 1998. The initial focus was the development of a solid knowledge base covering the collective experience of INAP?s members to promote the dissemination of successful practices, the identification of knowledge gaps, and the formulation of effective research proposals. The sharing of information on past successes and failures alone was viewed as sufficient justification for the formation of INAP. INAP was officially launched in October 1998, and is now an effective, industry-based initiative to globally coordinate research and development in the management of sulphide mine waste. Significant improvements in the management of sulphidic mine materials through knowledge sharing, research, and development of technology by INAP will reduce liability associated with acid drainage. Nowadays, there are some new requirements for the supermarket floors. The floors are loaded by high point- and moving-loads. In spite of this fact, it is necessary to ensure the stiffness of the floor and the stiffness of its support. When pile foundations are designed in the case of weak soil, the piles are recommended under the foundations of the supermarket and also under the floor layers. The example of foundation of a typical supermarket located on long­standing municipal dumping ground is presented. Indirect foundation - CFA piles, 0.5 m in diameter and 4.5 to 20.0 m long, was de­signed. Fifteen trial loading tests were carried out.

El presente trabajo plantea un proceso para minimizar el contenido de bismuto en las celdas de electrorefinación, a través de la adición de carbonato de bario a la solución electrolito, de forma que el bismuto soluble presente en fase líquida, se precipite sobre la superficie del precipitado insoluble de BaSO4, bajo una coprecipitación por adsorción e intercambio debido al equilibrio dinámico entre el precipitado y la solución de electrolito conteniendo al bismuto. En este sentido, se reduce la presencia del bismuto en el electrolito de cobre en un 75%, adicionalmente se tiene la disminución del contenido de plomo en un 96%, de estaño en 48% y de plata en un 18% aproximadamente.

The U.S. Bureau of Labor Statistics reports that the mining industry has the highest average annual fatality rate (31.9 per 100,000 workers) among all major American industry. To address this, a major research program to reduce hazard exposure of miners with computer-assisted mining equipment was initiated by the U.S. Bureau of Mines.1 One application involves the manual process of extracting and hauling coal where operators, in the tight confines of a mine, can be struck or caught by mobile machinery. The approach to remedy this problem uses a guidance system on the haulage equipment so that it follows the machine that extracts coal. This, in essence, involves sensor-based docking of the machines. Sensors that can survive the hostile mine environment of dust, methane gas, and water, play the key role. Computer analysis of the mining machine's movements and empirical machine characterizations were conducted to establish operating requirements and spatial limitations to ensure proper loading of coal into the haulage equipment. These data served in the selection of a sensing system. Various technologies such as scanning laser systems and ultrasonic sensors have frequently been used in other applications but were found unacceptable. However, a near infrared (IR) sensor employing active targets met the requirements. The sensor has a nominal 75° conical field-of-view and a range from 0.1 to 18.0 m. For the single target mode, nominal range accuracy was 4.3% at a distance of 3.56 m. Correction algorithms were generated reducing the error to 0.6%. Airborne dust testing showed less than 0.8% accuracy (worst case) degradation at levels exceeding (by a factor of 7.5) concentrations permitted by Federal law. The sensor can track multiple active targets providing five degrees-of-freedom (DOF) measurements. Using four targets, the nominal range accuracy was 0.4% without correction algorithms. A guidance system for the haulage system to follow the mining machine does not exist commer¬cially. Such a system can reduce fatalities and injuries by current haulage mining equipment, and is a viable alternative to current haulage control.

The 6-m- (20-ft-) diameter cast-in-place tunnel lining for Phase 3 of the Mill Creek Tunnel (MCT3) has been completed. This article summarizes aspects of liner design and ad-dresses the construction approach in placing the liner in areas of tunnel that experienced crown overbreak. Tunnel lining production rates, installation procedures and form buoyancies experienced during concrete placement will also be presented. The Mill Creek Phase 3 tunnel (MCT-3) is expected to be completed this year. It is one of the largest tunneling projects undertaken by the Northeast Ohio Regional Sewer District (NEORSD) to date. The tunnel horizon is situated within the Devonian Chagrin Shale rock formation at an average depth of 85 m (280 ft). The tunnel was excavated using a two-pass method. A full face, fully shielded, Robbins 7.2-m- (23.8-ft-) diameter tunnel boring machine (TBM) was used to excavate approximately 4,500 m (15,000 ft) of tunnel and facilitate installation of initial supports (first pass). The final lining (second pass) consists of 305-mm- (12-in.-) thick cast-in-place reinforced concrete and integral low flow channel. Seven shafts were constructed on the project. The excavation of the tunnel commenced at Shaft 14 and proceeded down grade to Shaft 9, the terminus shaft. Because of the presence of overbreak in the tunnel crown, measures were required to support the form and counteract the effects of buoyancy during concrete placement.

THE PROBLEM Two and a half years have gone by since the December, 1976 Civil Engineering article on subway costs caused such a furore. What has happened in this interval? Even after discounting the crushing inflation rate, costs are still increasing, and there is no sign of a turndown, even in the rate of increase. Some steps outlined in the article have been taken. In April this year, the first D.O.T.-sponsored Peer Review Group, made up of managers of other transit properties, met in Boston to review MBTA's $500 million Red Line Extension-Northwest for cost effectiveness. While the impact of a review at the end of the design phase, with substantial segments under contract, cannot be as effective as a review early in the design phase, it is nevertheless a start. This first meeting will be followed by a review of Boston's Orange Line relocation, the final design for which is just underway. In the industry at large two facts are evident: 1. We are failing to reduce underground construction costs. 2. Underground construction is in danger of pricing itself out of the market place. It has always been a greater challenge to design a Chevvy than to design a Cadillac, but it is the Chevvy we need today. Much attention has been focussed in the past on engineering design in the narrow sense: areas such as ground loading conditions and concrete design. "SummaryBritish Petroleum plc is developing a new gold recovery process known as coal gold agglomeration (CGA) for the recovery of gold from its ores. Liberated gold grains are recovered with high efficiency by adhering to agglomerates of coal and oil. The process is simple, can recover gold grains down to very fine sizes, avoids the large scale use of cyanide and provides the industry with an important new process option.A technical appraisal of the process has been performed on an extensive range of gold bearing materials. Ore types have been identified from which high gold recoveries can be achieved. A spreadsheet based computer program has been used to estimate the preliminary capital and operating costs of the process. Comparison with established gold production costs shows that the process costs of CGA, on the appropriate ore types, are potentially very attractive, which will enable economic treatment of lower grade ores than is currently possible using conventional cyanidation. Treatment of known placer deposits or gravity concentration plant tailings may be the most rapid means of demonstrating the process at a commercial scale prior to more widespread applications.1. INTRODUCTIONWestern world gold production for the 1980s has shown rapid growth sustained by a demand for gold at a favourable price (Fig. 1). Heap leaching and CIP/CIL operations have been responsible for the major part of new production during the 1970s and 1980s with present overall production costs averaging US$200/oz in Canada, USA and Australia and US$265/oz in South Africa [l]. A great deal of research and process development effort in the 1980s has been towards the treatment of refractory ores. Operating costs for the economic processing of these ore types by currently available routes are around US$300/oz [2]."

City Water Tunnel No. 3 is being built in order to provide a redundant system for delivering 4,542,500 m3 (1.2 billion gallons) of water per day throughout the five boroughs of New York City. The present tunnels, City Tunnel No. 1 and 2 (Figure 1), were put in service in 1917 and 1936 respectively, and both have been in continuous service. Tunnels No. 1 and 2 can-not be shut down for inspection or repair until City Tunnel No. 3 has been completed. City Tunnel No. 3 has two stages (Figure 1).Stage One is 20.1 km (12.5 mi) long and is presently delivering drinking water to the boroughs of the Bronx and Manhattan. Stage Two consists of the Brooklyn-Queens Tunnel 17.7 km (11 mi) and the Manhattan Tunnel 14 km (9 mi). The Brooklyn-Queens leg is complete and is set to be put in service soon. The Manhattan Tunnel is being constructed and is the subject of this paper. The Manhattan Tunnel (Figure 1) is comprised of three legs: the north, south and east legs. The con-tract includes the excavation of 8,976 m (29,500 ft) of tunnel and the lining of 14,561 m (47,773 ft) of tunnel. The south tunnel, 5,705 m (18,720 ft), was excavated but not lined, under a previous contract. The tunnel ranges from 181 m (600 ft) to 152 m (500 ft) in depth in the bedrock under Manhattan. The tunnel contract also includes the construction of nine new shafts, 161 m (530 ft) average depth, and the completion of a tenth shaft (the previously built construction shaft). The Manhattan Tunnel contract was awarded to the sole bidder Schiavone/Shea/Frontier Kemper Joint Venture (SSF) for $668,532,680.00, on January 31, 2005. The Manhattan Tunnel was designed ?in house? by the New York City Department of Environmental Protection (DEP) and the construction management was provided by a combined staff of the DEP and a joint venture of Jenny Engineering and URS.

The objectives of this study were to: (a) determine the state of the art for the control effectiveness of various chemical dust suppressants, (b) field test the effectiveness of the dust suppressants, and (c) develop technical guidelines for selecting and implementing cost effective tailings pond dust control programs. Chemical stabilizers evaluated under field conditions were: Coherax, Calcium Lignosulfonate, Nalco 656, 655, and Magnesium Chloride. Testing in the field utilized a portable field in-situ wind tunnel. The wind tunnel was placed on the tailings surface and simulated wind speeds of up to 80 mph were used to determine wind erosion threshold velocities. Emission rate testing was performed with the wind tunnel through the use of a real-time cascade impactor. A control cost effectiveness evaluation was performed to determine how efficient the chemical stabilizers were on a dollars expended per acre basis.

"The application of telerobotics to mining is currently being introduced into production systems around the world. Mining companies in Canada, Sweden, South Africa, and Australia have tended to lead the international charge to this form of technology for mining. While this introduction is taking place, a few basic questions have yet to be answered. How many machines can a single operator run? How many types of machines can a single operator run?A Canadian Research Chair in Robotics and Mine Automation has been established at Laurentian University to investigate these questions and many more through a series of experiments in a telerobotics laboratory that connects Laurentian teleoperation workstations to model mining machines at Cambrian College’s eDome. The laboratory supports many experiments allowing researchers to physically run multiple robot scenarios differing the numbers and types of machines at one quarter scale. Further experiments are being designed to investigate the potential for managing time delays in telerobotics. This paper describes the laboratory, experiments, and some preliminary results."

Oxide ores can consume enormous amounts of expensive sulfuric acid. The problem is amplified with increasing heap heights. One solution is to provide a thin liner and drainage system between each lift of ore, thereby intercepting the pregnant leach leach solution (PLS) after it exits the active lift. However, these liner systems require special attention to hydraulics, slope stability and geomembrane design criteria. Inadequate attention to the first two can result in reduced recoveries, increased acid consumption andsystem failures. The geomembrane selection criteria are different from the base liner, and optimizing its selection can have significant effects on the operation’s bottom line.

The performances of the three bituminous coal demonstration methane re¬covery boreholes, located in Greene County, Pennsylvania, are reviewed. Development of these vertical holes was largely underwritten by project funds. Reference is also made to an anthracite coal methane project in Luzerne County, Pennsylvania. This involved one vertical hole and was underwritten by a utility company. In addition, an exploratory drilling in the southern anthracite, which provided the project group with coal cores for methane gas analysis, is also cited. The technical and economical data that were gathered from the various methane projects are analyzed. Finally, the advantages and disadvantages of methane recovery by vertical boreholes are assessed.

Cemented rock fill (CRF) is often used to support open stope voids. This allows for the full recovery of ore while achieving global mine stability. In such cases, the exposed CRF masses require adequate compressive strength and stiffness to resist the forces and limit the displacement associated with movements in the rock mass surrounding the excavations. The CRF material preparation often involves the use of large particles, either from crushed rock or development mining waste. This means that conventional laboratory samples cannot be readily used to determine the laboratory strength. Consequently, a research project was undertaken to determine the influence of sample size on uniaxial compressive strength. This paper presents the results from CRF samples that were prepared using the same mix design and then cast into moulds with diameters of 150, 240, 300 and 400 mm. The laboratory testing also allowed a better understanding of the effect of particle size distribution upon the overall strength of a rock fill mass. The laboratory results were compared to a database of large-scale results from the testing of fill masses from a number of mine sites.CITATION:Cordova, M, Saw, H and Villaescusa, E, 2016. The works recorded in this Test Report have been performed by Rade Roncar - Electrotechnical Institute of Zagreb, SFR Yugoslavia in collaboration with Bureau of Mines. The main objective of these investigations was to investigate the possibility of fabrication of thin-walled square enclosures of satisfactory safety factor in accordance with requirements of effective technical regulations. The results and the investigations can be divided in three main fields: - investigations of thin-walled enclosures with the view of determination of pressures causing technological deformations before performing tests in accordance with effective applicable regulations, - finding the safety factor by means of tests of metal-foam pressure vents, - comparison of enclosures with and without pretreatment by technological deformation.
