The National Institute for Occupational Safety and Health (NIOSH) has developed a computer program called the Air Quantity Estimator (AQE). The purpose of the program is to provide a starting point for estimating the air quantity needed to dilute diesel particulate matter (DPM) in an underground large opening mine, albeit taking into account various input assumptions. The AQE estimates the total air quantity needed to dilute DPM emissions that enter the main airstream of the mine to current statutory levels. It helps operators pinpoint vehicles that are high DPM contributors. In addition, it allows the user to make “what if” scenarios by varying the input parameters to achieve the most efficient and practical ventilation system. The AQE is a stand-alone software program that does not require any type of spreadsheet software. Engine data must be known or estimated with some accuracy and assumptions must be made in order to limit the variability of the program output. Program calculations and two case studies, for validation purposes, are included in this report. It should be noted that the program does not ensure adequate face ventilation and personal exposure compliance.

In the process of pillar extraction, pillars are seldom if ever extracted completely. The pillar remnants, or snooks, play an important role in the extraction process. At the working face, they need to be stable to provide a measure of support in the immediate area. Further back, they have to fail in a controlled manner to prevent undue load build-up on the pillars that have not yet been extracted. The stability condition at the working area is the result of interaction between the stability of the pillar and that of the roof rock. It is shown that the primary mode of roof layer failure is by bending induced tension. Direct tension can be excluded in most practical conditions, and only in exceptional cases will the first failure be in shear mode. Panel spans required to cause the first goaf to be formed, predicted using analytical considerations, are of the same order as has been observed in practice. Once the first goaf has formed, the horizontal stress is reduced and failure will occur at more frequent intervals. It is shown that bending induced tension is the most likely mode of failure in an unjointed roof layer. In roof layers with a high frequency of weak or uncemented jointing, failure can be expected to occur at spans equal to the joint spacing. In this context, failure will occur at the smallest of the spans required to induce tensile roof failure and the joint spacing. A fundamental procedure to evaluate the likely roof layer failure in a stacked beam system is developed. This can to be used to estimate the loading condition on snooks. It is shown that snooks assist in stabilizing the immediate roof beam. The sizes and positions of the snooks relative to the position of the next pillar to be stooped, control the magnitude of the contribution to stability. The further the snook is from the next pillar, and the larger it is, the greater the contribution to the stability of the immediate roof. Under the condition of loading by the immediate, continuous roof beam, the position and size of a single snook controls the load acting on it. The closer it is to the nearest solid, provided that the roof overhang extends beyond the snook, the greater the load on it. However, once the immediate roof beam fails, the loading conditions change. The snook is then loaded by the tributary area load. A procedure is developed to assess the condition of snooks for different situations. Using the fundamental considerations developed in this paper, snooks can be designed to be stable during the active mining phase in their immediate vicinity, but to fail once the next line of pillars has been extracted.

"The adaptation to platinum-group minerals (PGMs) of a protocol developed to predict gold recovery by gravity is presented. The protocol is based on three different data sets: ore characterization of gravity-recoverable minerals, description of their behaviour in cyclones and ball mills, and performance of recovery units. Examples of these are given. It is shown that the gravity-recoverable PGM (GRPGM) content can be very high. A case study at the Clarabelle mill is presented. Sperrylite is the dominant platinum mineral, and its partition curve is very similar to that of gold, despite its lower density. Its grinding kinetics is significantly higher than for gold. As a result, sperrylite accumulates in finer sizes than native gold, but can still be recovered by gravity or flash flotation. Recovery projections based on gravity recoverable data are presented.BACKGROUNDReliable modeling of gold recovery by gravity or flash flotation from a grinding circuit is based on the ability to describe the properties of gold-bearing particles in the ore, their behaviour in grinding and classification units and the performance of the equipment used for recovery, be it gravity or flotation. "Industrias Peñoles, S.A. de C.V. in Mexico, has nine mining operations, six managed by Peñoles (base metal mines) and three managed by Fresnillo plc (precious metal mines). Seven of the operations use flotation and the other two are heap leach operations, one for gold and the other one for copper.This paper gives a brief introduction of Peñoles, shows mine locations and describes the main process characteristics of each concentrator. It also presents four of the new operating practices that were implemented over the last four years to improve the economics of the mining operations. These include (1) the use of centrifugal gravity concentrators to increase the recovery of gravity recoverable gold (GRG) and silver (GRS), (2) the use of expert systems to reduce process variability while increasing grades and recoveries, (3) the application of the Six Sigma methodology to key projects to ensure success and personnel development and (4) the use of water chemistry fundamentals to identify chemistry problems and design adequate solutions for improving grades and recoveries.INTRODUCTIONPeñoles was founded in 1887. It is a mining group with integrated smelting, refining and chemicals producing operations. Peñoles is the world largest producer of refined silver and metallic bismuth, the leading Latin American producer of refined gold and lead. It is amongst the world leading producers of refined zinc and sodium sulfate.Peñoles operates and controls six underground mines in Mexico and is the largest producer of zinc and lead of the country. It also produces copper concentrates and cathodes. Through its 77.1% equity stake in Fresnillo plc (a London Stock Exchange listed company); Peñoles is also a significant producer of gold and silver. Fresnillo plc operates three precious metal mines in Mexico. While Fresnillo is the world largest primary silver mine, Ciénega and Herradura are two leading gold mines of Mexico. Figure 1 shows a map of Mexico indicating the locations of the Peñoles mines. The main process characteristics of each concentrator are listed in Table 1."

The Plumduff sinter occurs at Whenuaroa, near Puhipuhi, approximately 30 km north of Whangarei. It is one of three residual sinters in the southern portion of a mid- to late-Pliocene geothermal system. The deposit consists of many large sinter breccia boulders, including rudrocks (coarse breccias), scattered around the flanks and summit of the small conical hill of Plumduff. Three types of sinter breccia occur: (i) rudrocks consisting of large, angular to sub-angular clasts of layered sinter up to 0.8 m across, as well as smaller clasts of sinter (20 - 100 mm dia), and fragments of stromatolitic structures; all clasts are supported in a matrix of microcrystalline, transparent to translucent to milky quartz; (ii) fragments of layered angular to irregularly mottled sinter of white to grey or bluish hue supported in a translucent to opaque silica matrix; (iii) smaller, matrix-supported breccias with clasts both partially surrounded and/or penetrated, by a clay-like weathering rind, and sinter conglomerates with rounded to oblate clasts. Three principal types of fabrics constitute the bulk of the sinter: (i) stromatolites, including columnar, palisade and filamentous networks, showing progressive silicification, in fenestral, tufted to clotted fabrics; (ii) peloids and pisoids; (iii) plant-rich facies in which various silicified plant species occur in a silica matrix. All fabrics have counterparts in hot spring deposits in the Taupo Volcanic Zone formed at temperatures from 75¦C to 35¦C. Also present is steam-condensate-produced alteration: kaolinite and alunite as well as boulders of quartz lattice texture. Cinnabar only occurs in sinter on the north and south central slopes of the hillside. No quartz veins or geyserite were recognised.

This report presents evidence indicating that when rock is stress-relieved during the course of mining or in drilling a core, it may be altered or damaged to the extent that the physical properties of the relieved specimen are not the same as those of the in situ rock. Furthermore, this type of damage may result from stress change other than that accompanying stress relief, such as the application of a relatively large compressive stress. In some instances the damage is microscopic, manifesting itself as incipient cracks or the failure of individual crystal bonds. However, if the sample is taken from a high-stress area, macroscopic damage may occur.

One does not know which has the ultimate maximum effect. In his presentation yesterday, Alan Peers, our SHEQ leader, covered a lot of detail with the way we are managing safety at Modikwa. I have attempted to identify some of the key essentials in making the mine safe. Passion Safety is addressed with a passion that is unbelievable at times. When I get phoned because an incident has occurred and in reporting the incident, there are already action plans being put in place to prevent re-occurrences, you understand the commitment to put employees? safety first. You hear the disappointment in your subordinates? voices when they have to share the fact that an incident or accident has occurred.

The discovery of the Nyidinghu Iron Ore Deposit totalling 2.46 Bt of resource continues FortescueÆs highly successful exploration record of discovering multi-billion tonne deposits under cover with little to no surface expression. Since inception in 2003 Fortescue have discovered and delineated the Christmas Creek and Cloudbreak deposits in the Chichester Range totalling 3.38 Bt and 3.25 Bt in the Solomon region. Together with other deposits Fortescues resource portfolio now approximates 10 Bt of hematite - goethite mineralisation.The Nyidinghu deposit is situated on the southern margin of the Fortescue Valley, 35 km south of FortescueÆs Cloudbreak mining operations on the north margin of the valley. Nyidinghu comprises approximately 70 per cent BID martite - goethite mineralisation hosted by the Brockman Iron Formation and 30 per cent CID beneath alluvial outwash of the Weeli Wolli Creek in to the Fortescue Valley. Minor DID mineralisation occurs at the deposit margins.BID mineralisation is strongly controlled by the pre-existing NE/SW trending Weeli Wolli Fault system and bounded to the north by the Poonda Fault. Both fault systems are interpreted to have had a long and complex history commencing pre Ophthalmian Orogeny (ca 2.2 Ga) and remaining intermittently active through the Proterozoic.CID at Nyidinghu is the downstream continuation of the combined Weeli Wolli/Marillana CID palaeochannel system being mined by Rio Tinto and BHP Billiton at Yandicoogina and Yandi, respectively.Exploration commenced in August 2010 with the inaugural resource announcement of 1 Bt being made just six months later. To date over 1700 RC and 85 diamond core holes have been drilled forming the basis for the resource of 2.46 Bt and ongoing feasibility studies. "The Aurora North mine has been operating for more than ten years, and is the source of over half of Syncrude’s bitumen production. In order to minimize front-end rejects, and to allow for high throughput, the ore top size at Aurora has been set at a nominal 125 mm. These relatively large lumps have a significant effect on both pump performance and pipeline pressure gradient. Current design methods underestimate the solids de-rating effect of lumpy slurries in pumps, and do not accurately describe the distribution or magnitude of the lump effect on pipeline pressure gradient. Data from the Aurora hydrotransport systems has been compiled, to quantify the effect of lumps on the pump head ratio and pipeline pressure gradient, and how this effect changes as lump ablation occurs along the length of the pipeline. A model has been built, using a logarithmic curve, which predicts the effect of 125 mm lumps on pipeline pressure gradient as a function of velocity and the distance from the start of a hydrotransport pipeline. This model is applicable only to Aurora hydrotransport operation, but similar models could be developed for other operations using the same methods. Using accurate pipeline pressure gradients and pump head ratios when designing future hydrotransport pipeline systems will help improve their reliability and operability."

The use of stable isotopic measurements as a technique for elucidating the origin and history of the water in hydrothermal fluids is relatively new. Recent studies have emphasised the importance of meteoric water in the formation of many hydrothermal ore deposits including many epithermal gold-base metal deposits which occur in faulted volcanic terrane. Other vein-type gold ores are believed to have formed from fluids of magmatic or metamorphic origin and exhibit characteristic d180 values. These values also give information regarding temperatures and the degree of equilibrium. Preliminary oxygen data for some important gold deposits of eastern Australia are evaluated in the context of styles of mineralisation.

Greensand, which is widely distributed in the Eastern United States, was produced in 1976 only by the Inversand Co., a subsidiary of Hungerford & Terry Inc., near Clayton, N. J. Production and sales information is withheld by the Bureau of Mines. Raw greensand produced by the company was sold for agricultural use as a soil conditioner. Processed greensand was sold for use as a filter medium for the removal of manganese, iron, sulfide, and other elements from water.

This report describes the second phase of an investigation aimed at obtaining a wider knowledge of the problems involved in smelting titaniferous magnetites. Liquidus temperature measurements were made on selected synthetic slag compositions in the Ti02 -A1203 -FeO-Si02 -CaO-MgO system. Both the hot-stage microscope and the strip furnace (melting-holding-quenching technique) were used during test procedure to determine the effects of CaO, MgO, and SiO2 on the liquidus temperature of slags. The liquidus temperatures of slags studied ranged from 1,241° to 1,900° C. The lowest liquidus temperature for each Ti02 plus A1203 plus FeO weight-percent generally occurred when the difference between CaO plus MgO and Si02 was less than 12 weight-percent. In this investigation substitution of either CaO or MgO for SiO2 and MgO for CaO raised the liquidus temperature of more than 50 percent of the reported base slags when Ti02, A1203, and FeO contents were held constant. The results of this investigation may serve as guidelines for determining flux requirements in smelting a wide variety of titaniferous magnetite concentrates or ores.

The importance of the Broken Hill mining field may be gauged from the fact that since the discovery of the ore-body in 1883, till the end of 1931, approximately 43,000,000 tons of ore have been raised.The lode covers a length of 31 miles, and in places is upwards of 300 ft. in width. The ore, as at present mined, consists of an intimately blended mixture of lead and zinc sulphides with silver and gangue minerals.Structurally the lode conveys the impression of a huge longitudinal arch (when viewed from the east or west), having its apex or crown near the centre of the field, in the leases of the Broken, Hill Proprietary Co. Ltd., where the lode outcropped at the surface.The minimum depth of the lode in the Proprietary mine was some 500 ft. from the surface, but huge undulations, having an average pitch of 45°, carry the extensions of the arch gradually deeper, to those places where active stoping operations are now in progress in the workings of the North mine at one extremity, and the South and Zinc Corporation mines at the other. The depth of the lode from the surface, as exposed at present, is approximately 1700 ft. at these extremities.The structure and physical character of the ore-body, and its containing walls, are so complex and varied that almost every type of mining problem is met with.

Introduction An integrated oil company has two options in approaching strategic planning. It has to decide whether it is in business for the long-term. I state this in all seriousness. Those who help manage public companies have the responsibility of good stewardship of other people's money. We must decide whether continued investment in our basic business will generate a reasonable rate of return. With a world surplus of petroleum production capacity, and continuing downward pressure on prices, this question should be addressed by all oil companies. And a positive answer is not necessarily a foregone conclusion in all cases. The decision should be made based on proven competence in finding and developing petroleum reserves on a cost-competitive basis with other producers, and on a frank analysis of future prospects. This should include a realistic assessment of the projected rate of return on specific investment projects. If a company did not pass these tests, its strategy could take one of four paths. Diversification First, it could diversify. But recent history suggests that oil companies are not good at managing other businesses. Second, it could seek a buyer for its business at the optimum return for its owners. Third, it could sell its assets on a piecemeal basis. And, fourth, it could adopt a longer-range plan of self-liquidation by living off current recoverable reserves. In other words, by not investing in exploration and development, it could maximize short-term profit and pass the money back to shareholders in the form of dividends. The Conoco component of DuPont has a below-average cost of finding petroleum, a good track record of efficient production, and a full plate of projects that pass the test of return-on-investment. In short, Conoco is in business for the long-term. That fact, in itself, determines at least one key element in strategy. This is to extend the life of the petroleum reserve base while at the same time maintaining maximum efficient rates of production to provide a strong cash flow. A company that is able to maintain a strong reserve base will ultimately see the value of those reserves escalate as world oil supplies tighten. Our planners expect this tightening to occur within the next decade as demand grows and production in non-OPEC areas of the world begins to decline. An exploration program tailored to meet the requirements of reserve replacement and current production should have two basic parts. Rockbursts are a major hazard in deep hard rock underground mining and tunneling. To date a significant number of rockbursts still defy conventional explanations. A review of rockburst case histories in the literature has shown that pillars and tabular excavations under shear loading are prone to rockbursting in situations where they are least expected under conventional knowledge of rockburst causes. Despite the evidence of the effects of shear loading on underground infrastructure, and the frequency of their occurrence, little is known about how this loading mechanism causes such large magnitude events and extensive damage. In this paper analytical and numerical modeling are used to understand the behaviour of pillars and tabular excavations under shear loading. The numerical modelling results show that pillars under shear loading are less confined than their equivalents in loaded in pure compression in the same geologic environment, and are therefore more brittle and weak. For tabular excavations under shear loading, the behaviour depends on whether such excavations are continuous, co-planar or offset with rock bridges. Analytical results based on slit theory show releasable potential energy in tabular excavations increases nonlinearly with excavation half-span. The study results are important for the safe and economic extraction of orebodies under shear loading.

OHSAS 18001 is a series of international standards for occupational health and safety management. It is comprised of two parts, 18001 and 180029. OHSAS 18001 was formulated by international certifying bodies and is based on BS8800 and was first published in 19996,11. OHSAS 18001 was developed to be compatible with ISO 9001 and ISO 14001 in order to facilitate integration of quality, environment, and occupational health and safety management systems5. It is a voluntary standard and the overall aim of OHSAS 18001 is to enable organizations to control their occupational health and safety (OH&S) risks and improve overall health and safety performance5. OHSAS 18001 is widely accepted in the UK, Europe, and Asia, is gaining momentum in North and South America, and it was used as the basis and conforms to the requirements of ANSI/AIHA Z.10-2005: American National Standard for Occupational Health and Safety Management Systems. It defines a set of requirements for an occupational health and safety management system suitable for any kind and size of organization4. Businesses of all sizes are developing and implementing occupational health and safety management systems (OHSMS) that are in compliance with the OHSAS 18001 standard.

The current phase of office development at Canary Wharf in the London Docklands includes twelve multi-storey buildings varying from eighteen to forty stories in height. The area of Canary Wharf South comprises five multi-storey buildings with low level in fill podium and garden areas. Three of these multi-storey buildings have "pile-cap" designs where the entire building load is applied to the piles. The remaining two buildings have "piled-raft" designs where allowance is made in the design for part of the building load to be transferred to the raft formation. Using a piled-raft design approach has enabled savings to be made on the number of piles used below the raft. This paper describes the calibration analysis undertaken to validate the design method. The ground conditions at the Canary Wharf site are typically fill and recent gravel deposits over the overconsolidated clay and sand of the Lambeth Group. Below these strata is the Thanet Sand. The Lambeth Group provided the founding layer for the raft and contributed to the pile shaft resistance. The Thanet Sand stratum provided the bearing layer for the 1.5m diameter base grouted bored piles. The piled-raft design required an assessment of the building load transfer through the raft to the formation layer. This load transfer led to significant increases in effective stress in the soil below the raft and thence a corresponding increased the capacity of the piles. This interaction of the raft with the prepared formation contributed to between 27% and 45% increase in total vertical load carrying capacity of the piled-raft substructure compared with the pile-cap design method. The development of the piled-raft design was carried out in three main phases: ? Development of the approach and validation analyses. ? Design work on various schemes resulting in detailed design packages. ? Validation analyses using full 3D finite element analysis (presented herein). The validation of the piled-raft design approach was initially carried out against published case histories for piled-raft interaction. To improve on the validation work a full 3D finite element analysis of a simplified pile raft and superstructure (which contributes stiffness to the raft) was undertaken; the analysis included only a quarter of the structure due to assumed symmetry of the structure and vertical loading. This 3D validation analysis is presented herein. The analysis was carried out using the Oasys LS-DYNA finite element program. Prior to carrying out the analysis of the piled-raft an initial finite element analysis was carried out to back analyse a fully instrumented field pile test providing accurate modelling of a single pile performance. This back analysis allowed confidence in the soil parameters used in the 3D model; comparisons of shaft and base resistances between the pile test and the FE model calculations for the single pile are presented. The parameters provided in this work were then carried into the full 3D model of the piled-raft. The 3D model of the piled-raft structure included 18 piles (12 full piles and 6 half piles on an axis of symmetry of the model).

A program of stress change and roadway deformation monitoring was undertaken to measure the forward abutment load distribution about a retreating longwall panel at Dartbrook Mine. The results of this monitoring allow roadway conditions observed at various stages of mining to be ranked according to the estimated vertical stress they have experienced. This ranking also provides a means to predict and visualise future roadway conditions at various stages of mining. This paper presents the results of the monitoring and describes the approach developed to predict future roadway conditions. The monitoring results show the vertical stress distribution associated with the front abutment from a longwall goal decreases exponentially away from the goaf edge. The vertical stress distribution peaks at the goaf edge at 5 to 6 MPa stress increase and decreases to 1 MPa within 50 m becoming imperceptible beyond 100 m By scaling this measured distribution to reflect the loading environment, the vertical stress can be estimated for different overburden depths and different stages of mining. Photographs of roadway conditions at particular vertical stress levels provide a way to visualise roadway conditions at various stages of mining in the future

This paper updates an earlier report by the authors that studied electrical injuries from 1992 to 1998. The previous information is expanded and supplemented with fatal and nonfatal injury rates and trends through 2002. Injury numbers and rates were used to compare and trend electrical injury experience for various groups and categories. This information allowed identification of at-risk groups that could most benefit from effective electrical safety interventions. The data presented in this paper are derived from the U.S. Labor Department’s Bureau of Labor Statistics’ Census of Fatal Occupational Injuries (CFOI), Survey of Occupational Illnesses and Injuries (SOII) and Current Population Survey (CPS). Between 1992 and 2002, 3,378 workers died from on-the-job electrical injuries. Electricity remained the sixth leading cause of injury-related occupational death. From 1999 through 2002, 4.7% of all occupational deaths were caused by electricity, down from 5.2% in the 1992 to 1998 time period. The cause of death was listed as electrocution in 99.1% of fatal cases. Contact with overhead power lines was involved in 42% of all on-the-job electrical deaths. The construction industry accounted for 47% of all electrical deaths between 1992 and 2002, but showed overall improvement from 1995 through 2002 by reducing its electrical fatality rate from 2.2 to 1.5 per 100,000 workers. An additional 46,598 workers were nonfatally injured by electricity. Contact with electric current of machine, tool, appliance, or light fixture and contact with wiring, transformers, or other electrical components accounted for 36% and 34% of nonfatal electrical injuries, respectively. Contact with underground, buried power lines was involved with 1% of fatal injuries and 2% of nonfatal injuries. NIOSH research aimed at evaluating commercially available overhead power line proximity warning alarms is described. This research is expected to be the initial step for eventual development of a performance standard for such systems.

The regional structure of shear and deformation zones in the belt, which is recognisable in plan view on Landsat TM images, is interpreted in terms of a conjugate-shear model. Gold mineralisation is related to this framework at a variety of scales. The principal recurrent structural geometries hosting deposits at mines such as Motapa, Durban, Charliesona, Cecil, Sunace, Turk group, Queens district, How, Peter Pat, Matabele Sheba, Goodenough, Old Nic and Bushtick are associated with shear zones and folds and include conjugate systems (shears, shear intersections, kink sets), side-step systems (en echelon segmentation, duplexes, monoclinal inflections, antiform-synform couplets) and interactions of the two. Linear sections of shear zones are generally poorly mineralised. The same geometries are identifiable on all scales from local to regional, so sections of regional structural lineaments or areas within mines should be prioritised for further investigation or development on the basis of the gold-bearing geometries observed to be dominant locally
