This paper explores the use of a Process Safety Management (PSM) system to manage risk in the mineral processing and metallurgical refining industries. Important elements of a PSM system are described. International regulations and non-regulatory guidelines covering PSM for high hazard industries are reviewed. Case studies of incidents in the mineral processing and metallurgical refining industries are presented to understand the lessons learned from failure to manage risks using a PSM system. Conclusions are drawn to adopt PSM in the minerals and metals processing industry as a tool for loss prevention and risk management.

In the smelting section, flexible fuels such as coal or coke are used in place of heavy fuel oil in order to utilize energy sources more efficiently. Heat recovered from the off gas of the furnace has been converted by the waste heat boiler into steam, which is used for self power generation and for heating or maintaining various temperatures. The amount of the recovered steam in the smelter is currently increasing due to maximum utilization of powdered cokes. The heat source required to maintain the temperature of the electrolyte in the tank house is electric power and steam. However until recent year this steam was provided by its own heavy oil combustion type boiler in the tank house since the amount of steam consumed in heating the electrolyte is large. Though experimental work over the last few years has shown the advantages of reducing steam in the tank house by fully covering the cell, the implementation of the full cover can lead to other operating problems. However various improvements, mainly the covering of the cell with a simple device, have led to greatly reduced steam consumption. As a result, steam for the tank house is now provided by the waste heat boiler only bringing more efficient energy usage.

The Morning Star Gold Mine has operated over a 101-year period, producing gold from narrow quartz veins and sulfide stockworks hosted mainly within a dioritic-gabbroic dyke. The deposit has been developed over a vertical depth of 980 m with drilling extending the vertical depth to greater than 1200 m. Currently there is a total Inferred Resource of 4.6 million tonnes at a grade of 6 g/t for 910 000 ounces of contained gold resources, which comprises an underground project and a surface project. Significant variation exists between the grade of drill core intersections and that observed in face sampling and subsequent production reconciliation (250 - 650 per cent). Studies were carried out on ten major production reefs, where the grades of drill intercepts were compared with face samples collected during development and mining. The implications of this study have been used as justification to assign production or stope/development grades where applicable to resources that are direct extensions of the historical development headings. Grade assignment and its impact on sensitivity for gold resources are detailed and show a 40 per cent variation in the gold resource, depending upon whether a production or calculated stope grade is used in grade assignment for the major reef deposits. Recent diamond drilling has intersected approximately 40 new gold bearing quartz reefs and sulfide zones that contain all the key mineralogical and structural indicators, and traditional large variations in gold grades similar to the historical major production zones. The implications of grade factoring and grade assignment are the greatest challenges confronting the resource estimation team in defining grade for these new zones. Grade assignment is seen as a method of balancing known historical production observations with the irregular grade distribution inferred from drill core assay results. Ongoing mine development and bulk sampling works are planned to validate the grade assignment process. The first stage of this process is the trial mining of the newly discovered Maxwell Reef (MR). The stope drawpoints were completed in August 2008 and the MRS1 stope is scheduled for completion in September. The stope reconciliation results will be presented at the October 2008 conference.

Mining companies are increasingly faced with the challenge of reducing operating costs while maximising throughput and profitability. At the same time, the grade and quality of treated ores are becoming poorer and more variable, resulting in suboptimal process performance.It is now widely accepted that strategies to drive operational goals must revolve around improving plant availability, reliability and process efficiency. In particular, progress in plant automation and control systems has seen a trend towards unified frameworks for control and optimisation of mineral processing plants. These frameworks seek to seamlessly integrate different roles and tasks involved in process operations - IT, automation, plant maintenance and operations. An integrated solution framework for reducing process variability, increasing process efficiencies and maximising plant availability and reliability is discussed within the context of a typical milling circuit. The framework exploits a real-time analysis ægapÆ evident in production operations that prevents timeous identification of equipment and process failures. This is achieved by leveraging predictive analytics for equipment health monitoring, control loop monitoring and advanced regulatory control solutions towards enhanced process performance, improved asset management and regulatory compliance. The framework provides for early and actionable warnings on equipment and process degradation using real-time operational data.Resolving these problems can potentially avoid unplanned downtimes, process instabilities and other operational surprises, thus ensuring optimal plant throughput and optimal process behaviour for improved recoveries. Integrating control systems and plant automation with advanced analytics to analyse and optimise processes enables operations to achieve reductions in the variability of key process parameters. In addition, early detection of equipment malfunction or suboptimal process conditions leads to improved equipment availability and reliability. Ultimately, these operational gains reflect in profitability because of plant throughput and recovery gains. The Boddington Gold Mine (BGM) is managed by Worsley Alumina Pty Ltd (Worsley) and is located 110 km south-east of Perth, Western Australia. The gold mine, which has operated since mid-1987, was originally constructed to treat 3 Mtpa of gold bearing oxide ores, has subsequently been expanded to treat 7 Mtpa and is presently one of the largest gold producing operations in Australia.During 1990 a Supergene/Basement Plant was constructed to treat 250 000 tpa of copper supergene and hypogene ores and basement ores which contain significant gold grades and which are encountered at the lower levels of the existing oxide orebody. The SupergeneBasement Plant has the versatility to treat a wide range of ore types, producing both gold bullion and copper-gold concentrates. It is a stand-alone facility adjacent to the existing 7 Mtpa gold plant.

The Cadia Hill concentrator at Newcrest’s Cadia Operations has undergone several modifications and upgrades to treat an increased throughput rate of harder ore from the Cadia East block cave mine.Subsequent to the commissioning of the expanded circuit, the flotation cleaner block has undergone a systematic optimisation programme tackling all facets of circuit operation. The Western Mining Corporation mining lease on the Bendigo Goldfield measures 5krri x 18km with more than 20 anticlinal folds parallelling th long north-south boundaries. Numerous closely spaced shafts and extensive underground workings on the reef lines along the folds flooded with groundwater following the progressive abandonment of the field. Preliminary water sampling and assessment demonstrated that a major dewatering programme and acceptable disposal methods were necessary to dewater interconnected old workings in areas of interest prior to any underground work taking place. A field wide geohydolic investigation was undertaken to establish geohydraulic parameters, groundwater recharge rates and water qualities. Data from the field investigation together with estimates of the volume geometry of the submerged workings as obtained from old mine plans, production history, and limited pump testing results were used with an appropriate analytical model to obtain estimates of water discharge rates required to reach various dewatering targets. Water disposal options were limited by relatively high salinities, arsenic levels, contained hydrogen sulphide, and the mine locations within the urban areas surrounding Bendigo. Direct disposal to the surface drainage system could not be considered, treatment to acceptable discharge standards was not economically feasible, and it was necessary to adopt a strategy of collection, containment, and evaporation.

Recognition of prospective sites for accumulation of ore depends on understanding the heterogeneity of the ore environment and the distribution of the two styles of mineralisation. The internal stratigraphy of the Broken Hill ore deposit and its enveloping rocks, is described, and a generalised stratigraphic setting of the stratiform mineralisation is outlined. Reconciliation of metal production from a sublevel cave (SLC) mining operation is a complicated process. SLC reconciliation is unlike reconciliation for open stope operations, where a tonnage of extracted ore can be assigned to an exact volume of in situ ore, and a direct comparison made between the estimation of contained metal and actual metal production. In SLC operations the ore extracted from a particular SLC drawpoint will be composed in part from the parcel of freshly blasted in situ material, in part from varying components of ore material (from cave stocks) flowing down from other previously mined areas vertically above the current draw position and in part from external dilution. This leads to difficulties in reconciling production grades against estimated grades. The problem is that we know how much metal we have produced but we can not precisely say where it has come from. Metal production reconciliation is fundamental in assessing ore reserve estimation accuracy and performance and in determining a draw management strategy for SLC mining. In terms of contained metal, differences in extracted metal compared to in situ metal represent changes to cave stocks. To generate local estimates of the cave stocks, differences in metal for each ring are treated as additions to or subtractions from a series of vertical columns of stocks extending throughout the height of the cave. The sum of metal differences (extracted versus in situ) for each vertical column results in a residual metal map at any particular mining level. The metal maps show areas where the metal production has been greater than or less than expected. The maps also highlight areas in the cave where residual metal is located, areas where targeted overdraw can reduce cave stocks.

Longwall mining is by far the most common method of underground coal extraction in Australia. Industry trends and expectations are placing increasing emphasis on the reliability and productivity of these operations. Longwalls are becoming wider and longer while retreat rates are continuously increasing due to significant improvements achieved in longwall equipment reliability and automation. This increased longwall productivity is placing significant emphasis on the reliability of longwall panel development.Although there have been significant improvements in the reliability of continuous miners and hydraulic drill rigs, the traditional resin-encapsulated bolt installation is still the principal method used in all major coal producing countries around the world. Anglo American realised that an opportunity existed to introduce an alternative roof bolt installation technique called ‘spin-to-stall’ (STS) into Australia. STS was first introduced by Anglo American Coal in South Africa, where it has been successfully used for over a decade, though implementation of South African STS resin in Australia has proven to be near impossible. Therefore, a new STS development project was initiated between Anglo American Coal and Jennmar Australia. Gravity methods of recovering alluvial gold are not very efficient at particle sizes below about 0.1mm. Whereas such gold may not have been economic to recover in the past, the same may not be true today. Centrifugal concentrators, like the Knudsen and Knelson bowls, are showing promise after an inauspicious start, at least in New Zealand. The large proportion of heavy mineral in many auriferous beach sands and the natural hydrophobicity of fine gold should make flotation an attractive contender for its recovery. The conventional mechanical flotation cell is effective, but roughing, scavenging and cleaning are necessary to achieve both high recovery and low mass yield. More problematic is the difficulty of maintaining coarse and dense sands in suspension. The air sparged hydrocyclone is effective at recovering fine gold from a fine Colarado River sand, but is not effective for the coarse and dense beach sands of New Zealand. The flotation column shows much promise for these sands, when recoveries of over 80% and mass yields of less than 1% can be achieved in a single pass. The froth separator of the USSR and flash flotation could also be worthy contenders. It is felt that flotation could be limited to naturally and artificially de-slimed concentrates.

Minnovare identified the potential to improve production drilling outcomes in underground long-hole stopes through the adaptation of new technology which was under ongoing development. An approach to Evolution Mining resulted in the companies agreeing to a collaborative agreement to further develop the technology for use on underground long-hole drilling equipment. During a period of six months in FY2018, a series of trials were conducted to compare drilling accuracy using two different methodologies and the subsequent impact this had on ore dilution. Testing was conducted at Cracow underground gold mine in Queensland, Australia. The drill rig’s existing setup/alignment process was compared to the new technology system with the following observations: • Using the existing process only 20% of holes surveyed recorded toe points within the +/ 300mm (11.8in) tolerance blast-hole deviation. • The new technology was proven to limit rig setup/alignment error, reducing average blasthole deviation and increasing the number of holes drilled within tolerance by 160%. The increased accuracy reduced stope dilution by reducing over-break of subgrade ore: • Stopes drilled using the new technology recorded 62.6% less average dilution. • This contributed to an overall reduction in dilution for the full year (FY18) of 5 percentage points compared to FY17 - from 22% to 17% (a net decrease of 22.7%). • With the system implemented as standard practice following results of the FY18 trial and continued success, overall dilution for full year FY19 showed continued reduction to 12% Evolution has continued use of the new technology at Cracow, where the improved accuracy has allowed the extraction of narrower stopes where drilling accuracy is critical. Evolution has also deployed the technology at the Mungari operation and has stipulated that the contractor use the technology at the Mt Carlton operation, which is currently under development. Future implementation of the Development Optimiser system should be verified by commencing additional trials to include a larger data base for validating the initial trial results.

The diverse priorities, drivers and concerns of multiple stakeholders in the minerals industry complicate the integration of sustainable development considerations into mine design, as well as operational and strategic decisions. The ability of the industry to address all role-playersÆ needs, or indeed, to achieve consensus among all role players on the plethora of sustainable development issues seems a noble yet unattainable goal. One of the difficulties in assessing the relative importance of sustainable development issues across many stakeholders is the inevitable lack of impartiality in valuing these issues. Starting from a premise that transparency and fairness of the process are absolute prerequisites for progress, but that consensus is not, an approach has been developed in which all stakeholders are asked to assess the importance of all other stakeholdersÆ priorities, excluding their own. The approach recognises that stakeholders have different levels of contribution, understanding, vested interests, and perceptions of legitimacy and entitlement, across the diversity of sustainable development issues and decision criteria. Using Triangular Value Analysis, the relative weightings of each of the stakeholderÆs priorities are determined through a regression-like technique. The uniqueness of this approach is the manner in which self-assessment bias and overemphasis of stakeholdersÆ own priorities are avoided. The methodology can be used for both internal and external stakeholders, and in conjunction with other group decision making techniques, such as the nominal group or Delphi techniques. This paper will illustrate how Triangular Value Analysis may be applied in the minerals industry to determine priorities in various decisions relating to potentially conflicting sustainable development priorities within the industry.

Fosterville Gold Mine is an underground operation located approximately 20 km northeast of Bendigo and 170 km north of Melbourne. Fosterville is the most productive gold mine in Victoria, producing over 100 000 ounces of gold annually. Access to the underground is via two portals cut into previously mined open pits. Fosterville is 100 per cent owned and operated by Northgate Minerals, a gold and copper producer with mining operations, development projects and exploration properties in Canada and Australia. (Northgate Minerals Corporation, 2010).Underground operations commenced in 2006 with ore production focused on three orebodies – Phoenix, Ellesmere and Falcon. Primary ventilation to the existing operation is provided by wall mounted exhaust fans located underground. This system provides approximately 220 m3/s of primary ventilation to the operations with intake achieved through the portals. Exploration drilling during recent times has uncovered another lode, the Harrier orebody, located 2.2 km to the south of the existing workings. Due to the location of the new Harrier orebody, vent analysis revealed the requirement for a separate source of primary ventilation to service Harrier. Figure 1 shows a current underground long section and the location of Harrier in relation to the existing workings. A number of options were considered, including a parallel decline from one of the existing two portals, however, a 4 m diameter, 350 m deep surface ventilation shaft was selected as the most suitable and feasible option for the mine.In December 2009, geotechnical drilling commenced to determine the most suitable location for the shaft. Site locations were constrained by a number of factors including interaction with current and planned surface infrastructure as well as proximity to nearby community members at the southern end of the mining lease. Results from the geotechnical drilling indicated that the majority of the proposed shaft would be suitable for a 4 m raise bored shaft. However, the top 40 m of the proposed shaft were deemed unsuitable for raiseboring due to zones of weakness that would likely influence the stability of a 4 m diameter shaft (Robinson, 2010). Following independent geotechnical advice and input from experienced raiseboring contractors, it was decided to construct a 40 m presink prior to raiseboring activities.A crane mounted auger was chosen as the preferred method to construct the presink. This method differs substantially from traditional blind sinking methods which are highly labour intensive. The auger, or drill, is designed and built to be attached to a crane base using engineered pivoting cradles. The auger is suspended from the crane jib by pennant ropes attached to hydraulic rams. The auger applies rotation and axial loads to the drilling tools via a telescopic steel bar (Kelly Bar). Drill tools are attached to the bottom of the Kelly Bar and comprise an assortment of drilling buckets and reaming barrels. Drilling buckets cut in situ material and collect the spoil for removal and are used for the excavation of the ‘pilot hole’. Reaming barrels are used to expand the diameter of the excavation by directing cuttings into the pilot hole which are then removed by the drilling bucket. In June 2010, the presink was constructed utilising a crane mounted auger with support to the shaft walls applied via a remote fibrecrete unit. The excavation method involved cutting a series of smaller diameter holes (2.4 m wide) before reaming out to the final diameter (4.7 m wide) in 4 - 5 m vertical increments. Fibrecreting activities were carried out on the backshifts, to ensure the stability of the excavation and minimise interaction between the two units. The presink was completed on time and offered many advantages compared to traditional blind sinking methods, including safety, costs and timing. This paper will examine the benefits of this method as well as lessons learnt and future applications in the mining industry.

Environmental regulators are increasingly adopting risk-based approaches to protect aquatic ecosystems. The Australian and New Zealand Environment and Conservation Council (ANZECC) and the Agriculture and Resource Management Council of Australia and New Zealand (ARMCANZ) (2000) introduced a risk-based approach to water quality guidelines that use decision frameworks to adapt water quality guidelines to local environmental conditions. Environmental quality objectives are no longer a simple ‘pass or fail’ number such as restricting total suspended solids (TSS) concentration to a specified limit for a particular rainfall event. This approach ignores spatial and temporal variability and the complexity of aquatic ecosystems. The risk-based approach establishes levels where adverse effects may occur. If these objectives are not met, further risk-based investigation is undertaken to assess the potential impact to the ecosystem. Klohn Crippen Berger (KCB) developed a dynamic simulation to predict the variability of sediment discharges, which is applicable for risk-based impact assessment of aquatic ecosystems. The application of the methodology over the life of a mine site is illustrated using a proposed South Pacific mine.The site location is in extremely rugged terrain with a high annual rainfall of approximately 4.7 m. There were two major sediment ponds proposed to reduce sediment discharges. A sediment and hydrology model was developed in Goldsim to provide daily estimates of run-off flows and sediment concentrations. A modified algorithm based on the Australian water balance model was created and incorporated into the Goldsim model and calibrated using recorded field data.The sediment module was developed based on a Revised Universal Soil Loss Equation (RUSLE) assessment, an annual prediction of soil loss over a catchment. A land-use type erosion vector was developed for different land-use types and for each of the subcatchments, a subcatchment erodibility vector was calculated. This vector and the daily rainfall erosivity factor were used to estimate the daily soil loss. The estimated soil loss and the modelled run-off yielded predictions of daily sediment concentrations. Finally, a sediment settling module was used to predict the daily efficiency of the sediment ponds and hence discharged sediment concentrations.The results of this modelling form the first part of a quantitative risk-based assessment into suspended solids impacts. The second part is the assessment of the effects of these predicted TSS concentrations on aquatic ecology. As part of the feedback-loop required in a risk management framework, if the environmental quality objectives are not met then further management decisions and actions may be required to reduce the risks presented by suspended solids. This may include modifications to: the mine-development plan; construction techniques and scheduling; processing techniques; rehabilitation strategies; or hard-engineering solutions such as sediment basins or flocculation.The Goldsim water-balance model provides a quick and efficient method of testing potential remedial actions on predicted sediment loads and concentrations. In combination with a cost-benefit assessment, options for reducing sediment releases from the mine site can be effectively and quantitatively compared thereby providing mine operators with a basis for decision-making. Regulators can also be satisfied that potential impacts to aquatic ecology have been comprehensively assessed and the necessary management decisions have been initiated to limit risk to the environment.CITATION:Ind, M and Usher, B, ?2014. "The Geometallurgical Mapping and Mine Modelling (GeM) comminution index (CI) was developed as part of the AMIRA P843 GeMIII project. Drill core geochemistry samples were passed through a Boyd Crusher with standardised settings and the resulting particle size distribution (PSD) was measured using sieves.Disadvantages of the original measurement were the relatively coarse crush size, requiring a second pass of crushing for geochemical sample preparation, and the limitation of throughput caused by the sieving step. At Antamina, the measurement has been modified to use a 2.2 mm crusher size setting, which permits a single pass. The PSD is measured using a Camsizer, an automated optical sizing instrument. The Camsizer sample is a +180 µm fraction, used to avoid passing too many fines through the instrument and slowing down the measurement. In the sieving step, the weights of the two fractions are recorded and subsequently used to normalise the distribution measured by the Camsizer. In this way, the throughput of the Camsizer can match that of the Boyd Crusher. The PSD data are therefore measured without penalising the throughput of the sample laboratory.The normalised PSD distributions are used to derive modified CI parameters, based on the slope and intercept of the breakage product size distribution, which are indirect measures of the material breakage hardness.Fifty samples were selected as a training set, each sample a composite of five or six individual assay intervals and each with its own measured CI. The average PSDs were calculated for the composites and used to calculate a composite CI. The intact half of the core, approximately 40 kg, was tested to measure the BWi, A*b and crushability. The results were used to develop predictive algorithms for these hardness parameters.A further 50 composites were chosen for lab testing, to serve as a blind test of the algorithms. A synthesis of reported All-In Sustaining Costs (AISC) for 57 Australian gold mines in the period 2014–2016 reveals for the first time the clear influence of grade upon cost performance, most evident in gold-only underground mines. Past syntheses of costs on a single quarter basis have viewed grade as potentially subordinate to other factors such as production scale, mine geometry, mineralisation style and a combination of other mining-related costs. Better understanding of the grade to cost relationship on both an individual mine and whole-of-portfolio portfolio basis is needed. Insight into the roles and trade-offs between other cost-determining factors will aid the future sustainability of the gold mining sector in Australia and assist in targeting prospects for future resource growth and development. While the overall trend is toward higher-grade mines displaying lower costs and vice versa, the grade to cost functions appear radically different between mines based on time-series data. The median cost of Australian gold production in the final quarter of 2016 was A$1081/oz. The threshold for lowest cost quartile production was A$965/oz. Underground production now dominates Australian gold output – with 71 per cent of ounces originating from operations that are either entirely underground or hybrid pit/underground mines. Scale economies are evident, with smaller mines predominately among the higher cost producers. For mines of equal grade, open pit mines are lower cost than underground operations. Reserves are modest, with a median mine life for underground operations of 4.7 years based on current production rates. World urban expansion puts constraints on the available land surface area of our major cities both for accommodating people as well as providing infrastructure and services whilst at the same time attempting to preserve the remaining natural environment. Whilst consideration is often given to underground planning for private and public transport infrastructure, the author is of the opinion that the current lack of underground planning of our cities that are suitable for underground development will have major long implications when finally the need for underground space planning is critical and ultimately recognised. This paper identifies the need for our city planners to integrate surface planning and underground planning as a ‘total planning solution’ where the city plan incorporates the elements of underground space planning as identified in this paper. Investigations by the author reveal that there are very few cities in the world that actually incorporate underground space planning in their city plans. The author uses the City of Brisbane as a case study for the potential introduction of underground space planning as part of the city plan. Despite efforts by the author to include the planning concept of underground commercial development in the latest Brisbane City Plan 2014 as well as four underground space workshops in Brisbane over the past 20 year through the Australasian Tunnelling Society (ATS), underground space planning for the City of Brisbane, apart from infrastructure projects, is not considered of significance to the City of Brisbane.CITATION:Robertson, A C, 2014. The Mount Wright gold deposit is located near the town of Ravenswood, ~90 km south of Townsville, in north Queensland. The deposit has been mined by underground methods since 2006. It produced approximately 138 koz Au in the 2011 - 2012 financial year and has a total endowment of approximately 1 Moz Au. To facilitate near mine exploration, a re-logging campaign of 600+ diamond and RC drill holes was recently conducted, resulting in a revised three-dimensional (3D) geological model and improved understanding of the ore controls and the mineralised system. The deposit is within a Permo-Carboniferous breccia complex approximately 350 m diameter and more than 1200 m deep. It consists of early peripheral granite-rich breccia, polymict breccia, a rhyolite spine and a late tuffisite (fine-grained milled breccia) pipe. The rhyolite spine is approximately 60 m + 250 m + at least 1200 m deep and is zoned with massive, flow-banded and breccia phases that are in part gradational with the other breccias. There is extensive sericitic alteration centred on the tuffisite and rhyolite units and fading out into the shallower and peripheral polymict and granite-rich breccias. There is also a broad pattern of metal zoning with elevated Pb-Zn ¦ Ag in the upper 400 m, Au-Cu-Bi below 400 m fading out below ~850 m, and a poorly defined deep Mo-W-Te-Sb anomaly. The majority of the Au ore is between 400 and 850 m below the surface in the rhyolite spine. Below this, Au mineralisation is less coherent, and more localised in the margins of the rhyolite and in the adjacent tuffisite. The developing model is that the ore is not confined to or necessarily directly genetically related to the rhyolite spine, but is driven by other phases of intrusion that also generated the tuffisite pipe. Mineral deposits in New Zealand whose origin can be ascribed to primary magmatic segregations are associated with mafic-ultra- mafic complexes related to two orogenic episodes which took place during Paleozoic and Mesozoic times. These mafic-ultramafic complexes are essentially of two types. One is ophiolitic and has podiform chromite mineralisation presumably formed below a spreading ridge. The other type occurs as intraplate layered intrusions in which Fe-Cu-Ni sulphides and Fe- Ti-V oxides have formed as a result of liquid immiscibility with the host magma, and have subsequently accumulated by gravity means. Generalised descriptions are given for some deposits in NW Nelson, Fiordland and Marlborough districts.
