During the ten year span from 1978 to 1987, the number of fires which occurred in under-ground coal mines doubled from the previous ten years. Anyone involved with underground coal mines is well aware of the associated tragedy that can result from these fires, both to personnel and to property. Every day mine management and mining personnel face this threat of a possible mine disaster or fire. Their ability to deal with such a situation safely and effectively depends on the level of emergency preparation developed and maintained. As a direct result of the ever-increasing interest in the Mine Emergency Preparedness Program and the Mine Fire Training Programs currently being conducted, the Emergency Preparedness Center was created. As a branch of the Mining Extension Service at West Virginia University, the Center provides preparedness training at several different levels on a varying range of topics. Classes are conducted at a new especially-designed training laboratory at the National Mine Safety & Health Academy in West Virginia. By maintaining the proper equipment and levels of training allowing for immediate response, the personnel who are part of the Center believe that the impact of a mine fire or other such emergency situation can be minimized. This paper will present recommendations for maintaining an efficient mine emergency preparedness program and will provide information regarding the Mine Fire Training Program as conducted by the Emergency Preparedness Center and describe the advantages of the newly constructed training facility.

The Shoreline Consolidation Sewer is a CSO control project in Cleveland, Ohio that requires the installation of 5,000 feet of 72-inch inside diameter pipe using microtunneling methods. After award, the contractor submitted a value engineering proposal to eliminate two deep shafts and install the tunnel in two drives instead of four. In an example of successful collaboration, owner, contractor, and engineer worked together during the initial construction stages to modify the design and update the contract documents. This paper examines the importance of exible requirements in contract documents and a collaborative environment between all parties to successfully deliver a project.

SX operators have conducted extensive shut-downs and repairs over the years to mitigate solution leaks from failed stainless steel false floors and walls in rectangular mixersettler mix boxes. Hydrodynamic modeling using first principles and constitutive equations of fluid mechanics can inform an efficient structural redesign sufficient to eliminate the plate failures. Updated mix boxes have been performing without failure for over a decade. Given the extensive construction of the original Holmes and Narver mixer-settler design throughout the world that includes stainless steel lined rectangular primary and auxiliary mix boxes, SX operators may benefit from the approach, eliminating costly shutdowns, leaks to containment, and hazardous repairs.

"Underground refuge alternatives (RAs) require an air source to supply breathable air to the occupants. This requires pressure relief valves (PRVs) to prevent unsafe pressures from building up within the RA. The Mine Safety and Health Administration (MSHA) mandates that PRVs prevent pressure from exceeding 1.25 kPa (0.18 psi or 5 in H2O), or as specified by the manufacturer, above mine atmospheric pressure when a fan or compressor is used for the air supply. Research to date by the National Institute for Occupational Safety and Health (NIOSH) suggests that the design and implementation of PRVs used in RAs has not yet had sufficient performance analysis or technical development. In response to this need, NIOSH tested a variety of PRVs using an instrumented test fixture consisting of data acquisition equipment, a centrifugal blower, ductwork, and various sensors to determine if subject PRVs meet the MSHA requirement. Relief pressures and flow characteristics including opening pressure and flow rate were measured for five different PRVs under a variety of conditions. Subject PRVs included two off-the-shelf modified check valves, two check valves used in MSHA-approved built-in-place (BIP) RAs, and a commercially available valve that was designed for a steel RA and is currently being used in some BIP RAs. The test results showed relief pressures ranging from 0.20 to 1.53 kPa (0.8 to 6.1 in H2O) and flow rates up to 19.3 m3/min (683 SCFM). INTRODUCTION The Mine Improvement and New Emergency Response Act of 2006 (MINER Act) was enacted in the wake of three mine explosions/fires that claimed 19 lives that year. Intended to help improve underground coal mine accident preparedness, the MINER Act includes provisions that target mine safety issues in areas such as emergency response planning, adoption of new technology, training and education, and mine safety standards enforcement [1]. Section 13 of the MINER Act specifically directed NIOSH to provide for research into the effectiveness and viability of refuge alternatives (RAs) for underground coal mines. This mandate culminated in the 2009 adoption of changes to 30 CFR mining health and safety regulations, requiring underground coal mines to provide mine emergency RAs and associated components, to provide a life-sustaining environment for persons trapped underground. Such RAs can be either self-contained mobile units or built-in-place (BIP) facilities. The regulatory changes also include provisions establishing requirements for Mine Safety and Health Administration (MSHA) approval of RAs and their components, and among these provisions are numerous criteria for providing a safe breathable atmosphere under positive pressure within the RAs. One specific criterion for maintaining a safe RA atmosphere requires the inclusion of an air pressure relief valve that will activate at a maximum of 1.25 kPa (0.18 psi or 5.0 in. H2O), or at a pressure above mine atmospheric pressure in the RA as specified by the manufacturer when the breathable air is supplied by a fan or compressor [2]."

The addition of new data and corrections can be made under the control of a simple editing session. All programs for this system are written in FORTRAN and executabe "MACROS".. After the addition of new data, the whole file Is reinitialized to update the binary file and cross-reference tables. Updating the binary file involves rerunning the initializing program. The creation of graphic plots like cross sections and plan views is menu-driven from a terminal and very user-friendly. Small files containing not more than 10,000 entries (survey and assay points) are suitable for this setup, particularly for computers with less than 2 Megabytes of real and virtual memory. The largest file processed contained 35,000 lines of survey and assay information and required 4 megabytes of virtual memory. Working with this size file slows down the time for the creation of plots and cross sections. Another independent system is more akin to a financial type of data base. It was installed in one Anaconda mine as part of the operation's inventory, warehouse, day-pay payroll, and purchasing data bases. It is a highly hardware-dependent system, in particular, IBM's CICS/VS (Customer Information Control System/Virtual Storage). This is a true relational data base that optimizes storage requirements, supports online inquiry, update, data entry, and data security. While this system has been tested and proven in accounting applications, it requires programming expertice in PL/I and other CICS utility programs. It is well suited for huge data bases (like the 35,000 lines of data), but cumbersome for small engineering files.

So now you ask, Where are the transcripts? The well-organized co-chairs, Harry and Mary, did their job. Doug Halbe was active in the audience encouraging people to speak up and keeping everything moving along. So where are they? Were the comments so provocative that they could not be written? Was the profanity so bad (even for miners!) that it couldn't be repeated? It appears that at the first session of the Plant Operators' Symposium a glitch occurred with the recording equipment. Thanks go to everyone who did comment during the dis- cussion period. The People I session had a lot of lively conversation and we are sorry that it will not be printed. However, there is now another reason to hold the plant operators' sessions on a regular basis! Another People session can and will be held in the future.

The use of the underground space has been a recurring issue in human history, whereas the reasons for building subsurface structures have changed over time. In the decades to come, the world will be marked by a variety of underground projects never before seen. This paper describes a series of characteristic challenges of designing and building underground freight transportation systems, with reference to the example of Cargo sous terrain, a privately funded underground logistics project in Switzerland. It provides an overview of contracting practices that allow these challenges to be addressed with success.

"In Part I, Gibbs’ phase rule was applied to mass-balanced EH-pH diagrams for the Cu-As-S-H2O system. The diagrams were generated using the stoichiometry of enargite as a basis. In this study, the calculations were expanded to cover the stoichiometries of three other Cu-As-S minerals – tennantite, sinnerite and lautite – and compared with enargite. Stability regions within the resulting EH-pH diagrams were found to change with stoichiometry such that a particular mineral would predominate if its stoichiometry was used in the calculation. Solution speciation was also found to vary, with thioarsenate being the best example. This stoichiometric effect helps to explain results reported in the literature and would be difficult to realize if it were not for mass-balanced calculations. IntroductionA comprehensive thermodynamic review has been conducted on the aqueous copper-arsenic-sulfur (Cu- As-S) system (Gow et al., 2014, 2015a, 2016b; Gow, Huang and Young, 2016c). Theoretical calculations agreed well with the experimental results under alkaline conditions (Gow et al., 2015a), acid conditions (Gow et al., 2014) and alkaline sulfide leaching conditions (Gow et al., 2015b).In the present study, additional mass-balanced EH-pH diagrams were generated by changing the Cu-As-S stoichiometry from enargite (Cu3AsS4) to tennantite (Cu12As4S13), sinnerite (Cu6As4S9) and lautite (CuAsS). Regions for the thermodynamically stable species were shown to change with the four different mineral compositions. The Henderson Mine, a new large scale underground development, is being developed by AMAX. The mine site is located about 50 miles west of Denver, Colorado, adjacent to the east side of the Continental Divide. The mine's three access shafts are in a narrow alpine canyon at an elevation of 10,400 feet. The ore body of molybdenite is to the south of the shafts, 3,000 feet below the surface of nearby Red Mountain. The ore extends over 800 feet vertically and is about 1,700 feet by 2,200 feet horizontally. The eastern and southeastern boundaries of the deposit are still to be defined at the present time. Development mining is advancing at the mine site on a three shift, seven-day basis at the rate of about 2,000 feet per month. About twelve miles of heading have been driven at the present time, and work to sink No. 3 Shaft has started. Development mining was begun in 1967 with the sinking of No. 1 Shaft. This shaft is fully concreted, 23 feet in diameter and 2,441 feet in depth. It is the mine ventilation exhaust conduit. It is also used for development muck hoisting, stand-by mine water pumping and secondary access to the 8,035 foot level. This level is the mine exhaust ventilation level and was mined principally by conventional rail mining methods. From this one station at the 8,035 foot elevation a 5% ramp was driven to the initial production horizon at 8,100 feet with diesel rubber tired mining equipment. Subsequent mining has been entirely by mobile diesel equipment at the mine site.

Spontaneous combustion presents a very serious hazard to both safety and economics of mining operations. Notwithstanding considerable research that has been done over the years to present and control spontaneous combustion the risk still exists. Consequently, this problem poses a major challenge to mine safety researchers; new and improved techniques are constantly being developed to deal with this major hazard. This paper reviews the current practices to prevent and control spontaneous combustion in coal mine operations highlighting new developments in this area. Research and development work presently being carried out in Canada to' deal with this problem is also discussed which includes monitoring, inertization and application of biotechnology.

The Advisory Conference on Tunnelling of the OECD, held in Washington, D.C. in June 1970, has established the following definition: "Tunnelling refers to the construction by any method of a covered cavity of predesigned geometry whose final location and use are under the surface, whose cross-sectional area is greater than 2 square metres". (1) The definition makes clear that the term includes all types of underground construction, be it in hard rock, soft ground, under mixed conditions, underwater, etc. Small-sized tunnels in rock therefore constitute but one part of the tunnelling scene. With an ever-growing urbanisation taking place in industrialized as well as developing countries, the demand for small-sized tunnels is, however, rapidly increasing and requires a far-reaching mechanization of excavation and support functions. This trend is accentuated by the fact that small-sized tunnels are far more labor-intensive than large ones, and, with wages raising at a much faster rate than costs for plant and equipment, this will accelerate the introduction of new tunnelling systems. In 1986, a subsidiary of Echo Bay Mines Ltd. purchased the McCoy property, 55 km (35 miles) southwest of Battle Mountain, NV. Echo Bay then launched a two-year, $1 15-mil lion capital program to build a mill to extract gold and silver from high-grade ore mined in two open pits - the original McCoy pit and a more recent discovery, the Cove. Engineering on a new mill began in 1987 with the facility coming on line in 1989. It was comprised of a semiautogenous (SAG) mill and ball mill grinding, leaching, flotation, counter current decantation (CCD) and Merrill-Crowe processes. The mill started up on McCoy ore, a rocky material that settled very well in the new CCD thickener system. Production levels were maintained at about 7.2 kt/d (8000 stpd). The situation changed as the McCoy ore played out and increasing amounts of material from the Cove pit-with its oxide, sulfide and carbonaceous ore - were fed to the mill. The problem was the high clay content of the Cove ore. The new ore immediately created production problems, mainly loss of control on the CCD thickeners. With the McCoy pit slated to be shut down in early 1991, it was essential to find a way to efficiently process the Cove ore. Initially, the only way to regain control of the thickeners was to slow down the processing rate and increase flocculant usage, thereby getting the solids to settle in the thickeners. But this had its cost. Production dropped to about 5.9 kt/d (6500 stpd), additional operators were needed and flocculant costs, on occasion, would reach an unacceptable $80,000 per month. Over the past few decades, the mining and minerals industries for their part have undertaken significant and intentional changes at every level to become more responsible — no longer to just their shareholders, but also to the environment, to the communities in which they operate and to society at-large. Led by some of the global majors, these industries have indeed helped to define and operationalize the basic principles of sustainable development. Now in the midst of a full-on cultural shift, they are also key players in the continual effort to re-examine and calibrate practices to meet dynamic and increasingly complex objectives.SME’s technical position statementAs a home for so many technical experts and practitioners, SME has long recognized the importance of a commitment to sustainable development by the mining and minerals industries. In April 2016, the Society formally adopted a technical position on the topic following years of participation in, and support for, related activities. The full position statement appears below. In the underground type of mining operation, the finer portion of the waste material commonly is pumped as a hydraulic slurry to a tailings disposal pond. (1)* The tailings slurry is deposited via a pipe manifold system located around the perimeter of the pond. This method of deposition produces zoning because the coarser material tends to settle out immediately while the finer particles tend to remain in suspension and are deposited progressively nearer the center of the pond. The manifold system must be raised periodically as the pond fills with the tailings material. Over a period of time, this procedure will produce a zoned embankment known as a tailings dam. A typical pond and dam are shown in Figure 1. In plan view, the dimensions of tailings dams can be on the order of miles, with the height of the dam reaching 100 to 200 feet. Due to such factors as inadequate compaction, inherently low strength of the tailings material, and the possibility of high phreatic surface, many dams at some point become unstable, i.e., the factor of safety against a slope failure approaches 1.0. Remedial measures are required to improve the safety factor in order to prevent massive failure of the dam. In most cases, stability can be improved by effectively flattening the slope of the dam as shown in Figure 2, but this is not possible in every case. Occasionally, a tailings dam is constructed adjacent to a natural or legal boundary so that it is not possible to construct a conventional berm or to reduce the slope angle with earth moving equipment. Even a terracing process would require removal of the material and this may not be practical. In such a situation, an alternate scheme to improve stability must be found.

INTRODUCTION Under certain circumstances the closure of former mines which are located above a certain flood level can result in problems such as the emanation of detrimental substances after having completed filling and reclamation operations. This especially applies to uranium mines in which the radiation dose could far exceed the dose of natural background radiation. By means of an example of the uranium mining in Germany in the following it will be demonstrated how to cope with this problem. On the basis of comparative investigations in various vein deposits and using ventilation scheme calculations proposals for the optimization of the necessary forced ventilation can be submitted. REPORT ON SITUATION In the period 1946 - 1989 the former Soviet-German joint- stock company "Wismut" developed into the biggest European uranium producer with a total output of about 220.000 t of uranium. A major mineraldeposit district was the deposit of Schlemaf Alberoda in the Saxon Ore Mountains, in which 80.000 t of uranium were produced. Thus it is among the biggest uranium de- posits of the world, from which various other metals were at- tracted for many centuries. The exploitation of the Schlemal Alberoda deposit involved steep veins in regions near the surface as well as depths of 1.800 m. Until 1991 a total excavation space of 40 million m3, which is flooded at present, was produced. With the average increase in the water level of 80 cm per week the final flood level is expected to be reached in the year 2003. The shaft 373 at present still being used for ventilation will be no longer available since the second quarter of 1998 after flooding the -540 m level because it is not connected with the excavation system near the surface. As a study shows, a radiation dose far above the natural back- ground radiation has to be expected for the town of Schlema due to the extensive mining activities near the surface and due to the subsequent displacement with missing depression fo the main mine ventilating fan. An uncontrolled air flow containing radon leaves the open mine excavation due to the effect of the natural ventilating pressure and emanation caused by the barometric pressure drop with atmospheric pressure fluctuations. This mine air with its high-level radioactive equilibrium results in a high radiation dose in buildings (see Figure l). After having switched off the main ventilating fan in order to investigate the effect of the missing depression the increase in radon concentrations amounted up to 700% in various buildings of Schlema. This was partially due to the inversion state of the weather at that time. The high radon concentration has detrimental effects on the health of the population and of the miners working on the further reclamation in regions above the flood level. ANALYSIS OF THE RADON EMANATION RATE EXPECTED Considering the composition of the radon inflow from the mine workings it becomes evident that 80 % of the radon inflow originates from abandoned excavations and only 20 %from open ventilated mine excavations. This fact has to be taken into account for the ventilation after having reached the final state of flooding. After completing ventilation the radiation dose on the surface is mainly due to the radon emanation from excavations close to the surface. Investigations of the Wismut GmbH showed the in- crease in the specific radon emanation rate by a factor of 100 for abandoned excavations as compared to new drivings. One reason is the larger specific surface of abandoned galleries caused by displacements due to mining activities as well as by fall of hanging. Furthermore the radon can enter the gallery through joints, which have subsequently opened by convergences. All these effects result in a larger free surface available for radon diffusion. The large number of drivings in the deposit sections near the surface and the fact that the highest uranium contents are found near the surface as well as the high fracturing are further reasons for higher emanation rates. Considering these facts it can be expected that the radon inflow of 10.000 kBq/s, which refers to an open mine excavation of about 1.4 million m3, represents a minimum. Only by increasing the specific surface, for which a numerical value has still to be determined, this value will increase with certainty. An extensive radon emanation from the residual excavation, which cannot be flooded, can only be prevented by maintaining the ventilation system. The low pressure produced by the fan in the mine openings prevents the emanation of air containing radon due to the effect of the natural ventilating pressure. Without the controlled withdrawal of the radon the population as well as the miners working on the further reclamation in areas above the flood level would be endangered. Therefore the follow-

The Anderson Dam Tunnel Project is a high-priority, urgent project under construction by the Santa Clara Valley Water District in California. The project will provide a new low-level outlet for the Anderson Reservoir enabling the seismic retrofit of the Anderson Dam as mandated by the Federal Energy Regulatory Commission. This paper describes planning for construction of the tunnel through the dam’s right abutment crossing several faults in challenging and variable soft and hard ground conditions. The construction methods include SEM, hard rock drill and blast techniques, and a mechanized 8-foot (2.5 m) diameter microtunneled lake tap.

The Jones Hill prospect is a Proterozoic age, volcanogenic, semi-massive to massive sulfide deposit. The deposit is located on the western edge of the Pecos greenstone belt in the southern Sangre de Cristo Mountains of northern New Mexico. The prospect is found in a block (West Block) of meta- igneous rocks, primarily diabase, metavolcanic rocks, and metasedimentary rocks (fig. 1). This assemblage is typical of lithologies found in the greenstone belt. Metamorphic grade is greenschist to lower amphibolite. The West Block is bound- ed on the west by Proterozoic age (1.7-1.4 b.y. by Rb/Sr), granitic plutons collectively called the Embudo granite, and on the east by Paleozoic sedimentary rocks, primarily sandstones, shales, and limestones of Mississippian and Pennsylvanian age. The mineralization occurs in a single horizon which has been overturned and cut into two blocks by a high-angle reverse fault (fig. 2). The mineralized horizon is found at the contact of felsic metavolcanic and rnetavolcaniclastic rocks with overlying intermediate metasedimentary rocks. The local stratigraphic sec- tion from bottom to top is as follows: 1) porphyroblastic quartz- sericite schist (originally a rhyolitic, subvolcanic intrusive); 2) quartz-sericite-chlorite-biotite schist (felsic metavolcanic and metavolcaniclastic rocks); 3) chlorite-talc-biotite-sulfide schist (altered felsic metavolcanic rocks); 4) carbonate-quartz-sulfide [ ] chlorite-tremolite-talc schist (mineralized exhalite); 5) quartz- chlorite schist (silicified tuff); and 6) amphibole-chlorite-feldspar- biotite schist (occasional mafic to intermediate metavolcanic flows).

As the existing major U.S. - Canada rail tunnel was too small to accommodate double-stack container cars, Canadian National Railways is constructing a new parallel tunnel with a 50% greater diameter. Starting in Sarnia, Ontario, the new shallow-cover bored tunnel runs under a large research building and several other structures in an oil refinery before travelling under the St. Clair River. Maximum predicted settlements to be caused by the 9.2 meter diameter earth pressure balance TBM in the soft St. Clair till were in excess of 100 millimeters. As such, these settlements had the potential to cause severe damage and disruption to the existing refinery. In order to reinforce the soft clays below the refinery research building and other structures, and to compensate for settlement caused by the tunnelling, the ground improvement method of Soil Fracture grouting was successfully applied for the first time in North America. This paper will describe the details of both the soil fracture grouting application and the elaborate electrolevel remote monitoring system used.
