Incident investigation is of utmost importance in most high-risk industries and is often used to improve organisational safety. This study was undertaken to examine the content of past incident investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of five elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of three Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed.

This paper describes the geology of three European cities and outlines how the geology has lead to the development of specific foundation construction techniques. Comparative tables of techniques commonly employed are given in Section 5. It can be seen that despite the presence of the 'single market', techniques still have wide diversity despite some similarities in geotechnical conditions.

Augered-Cast-in-Place piles have been used successfully for many deep foundation applications. The four case studies discussed herein describe ACIP pile projects where problems developed during ACIP pile installation requiring changes to the method or sequence of installation. The case studies illustrate the importance of understanding existing subsurface conditions and project constraints when considering the use of this pile type. Ground loosening and over-excavation of soil during the augering process, a significant concern for fine to medium sands and non-plastic silts below the water table, can significantly impact adjacent structures. This risk needs to be evaluated during the pile selection process and generally can be mitigated, but not eliminated, by requiring the use of piling equipment with high torque and slow auger rotation speed to advance the auger with minimum amount of rotation. In some applications where high lateral stresses in the soil are present such as at the base of retaining structures, the use of ACIP piles may not be appropriate if structure movement is unacceptable. On the other hand, ACIP piles have been successfully used to support deep foundations in preconsolidated varved deposits where the new structure was designed to accommodate recompression settlement.

The introduction of social media platforms has ushered in a new era of interaction and information exchange, which impact business-to-society relationships. Mining companies are closely watching these platforms, as the industry takes a cautious approach to control flows of information. This paper explores the risks, challenges and opportunities of using social media, based on a qualitative mixed methods investigation with stakeholders from the Canadian mining industry. Generational and language barriers, misunderstanding and limited motivation from regulators make implementing social media platforms challenging and risky at an organizational level. The benefits of social media include being able to mobilize communities and share information in real time. Despite the challenges, the study found that 62% of mining stakeholders are incorporating social media tools, mostly in an ad hoc manner. Flexible strategies for transparent communication and public engagement are creating new opportunities for information exchange.

This paper describes the preliminary design of the fourth bore of the Caldecott Tunnels along State Route 24 in Oakland, California. The paper focus is on two major aspects of the project: initial support system design and seismic design. The initial support system was designed based on the New Austrian Tunneling Method (NATM).NATM provides the required flexibility to accommodate the variable ground conditions in the weak, folded, and faulted rock along the alignment. The seismic design provides a final lining system that meets serviceability requirements for a 1,500 year seismic event with an estimated peak ground acceleration of 1.2g.

Summary During the last three years the discovery of gold in the belt of volcanic and sedimentary rocks extending eastward from the Porcupine and Kirkland Lake gold-fields has caused a great deal of excitement in mining circles. The area is heavily drift covered, and every aid to the location of veins is eagerly grasped. A knowledge of the physiographic history of the area is of the utmost importance in the application of many of these aids. Evidence is presented to show that glacial erosion has lowered the general elevation very little below the pre-glacial level, and that many glacial erratics have been carried less than three miles southward. Probably a knowledge of the general geology of a: region was never more necessary to the understanding of the distribution and continuation in depth of the mineral veins than in this area. Four distinct granites occur in the region, and gold-bearing veins are genetically associated with only one of these. Two types of quartz-feldspar porphyries have been found, only one of which has any bearing on the locus of the mineral veins. Many other examples of this complexity of associations might be cited, but everyone goes to show that in most cases the economic importance of the deposits can be realized only after the regional geology is understood. A series of sandstone, limestone and shale intruded by granite lies unconformably below the Keewatin lavas. The latter are intruded by a gneissoid granite and are overlain unconformably by the thick series of Timiskaming greywackes and silts. These are intruded by carbonated syenites and syenite porphyries which have since been rendered somewhat schistose.

"As part of a larger research program, a number of MEGATEM airborne electromagnetic (EM) test flights were flown over the Gallen massive sulfide deposit in northwest Québec. A particularity of this test site is that a major part of the ore body was extracted before the MEGATEM survey. Therefore one of the purposes of this study was to verify the ability of the system to detect the remaining massive sulfides below the water in the open pit. The open pit is also surrounded by a metallic fence, and a power line is present in the vicinity. A large part of the case study involved accounting for the impact of infrastructure and acidic water on the survey, which will help in the interpretation of airborne EM responses in complex exploration situations.The Gallen deposit was also previously flown with the INPUT and GEOTEM airborne EM systems before the major exploitation period. A comparison between the results from the different survey results allows estimation of the physical properties of the Gallen deposit, which is a relatively poor conductor.A new method for removing the power line signal shows amplitudes with a smaller residual associated with the emanating fields, and more compact, cleaner responses associated with induced currents. This will make it easier to identify bedrock conductors close to power lines"

Alberta Piling Contractors very rarely are required to perform major marine construction in the prairies, so it became as a big surprise to AGRA Foundations Limited to be invited by Transalta Utilities Corporation to bid on a major marine project for the Sundance Cooling Pond Expansion at Lake Wabamun, Alberta. Even more of a shock, the work had to be undertaken during Alberta's harsh winter conditions with temperatures plunging to -30 C. Not a happy situation for prairie construction workers to be in, working from barges in the middle of a lake in the dead of winter. However, it is well known that prairie boys make the best sailors.

Longwall mining has existed in Utah for more than half a century. Much of this mining occurred at depths of cover that significantly exceed those encountered by most other U.S. longwall operations. Deep cover causes high ground stress, which can combine with geology to create a coal burst hazard. Nearly every longwall mine operating within the Utah’s Book Cliffs coalfield has been affected by coal bursts.

The paper focuses on construction of a 2.55-km (8,365-ft) long, 2.4-m (8-ft)diameter storm drain tunnel in Evansville, Indiana. The tunnel was driven through saturated soft ground varying from clay to fine sand. The paper presents: an overview of design and design rational including specific EPB machine requirements; the contractor’s proposed and accepted alternative construction methods and TBM design; and construction issues and performance. Some of the key points discussed are the use of the TBM in different operational modes for alternative ground conditions, the effectiveness of dewatering systems, production rates, and issues such as surface settlements and gas.

global mining boom and in-creased demand for iron ore has lead to a resurgence in the mining industry in Minnesota. At the combined 81st Annual Minnesota Section of SME Meeting and 69th Annual University of Minnesota Mining Symposium, April 15-16 in Duluth, MN, this resurgence was the hot topic as new mining projects were featured in the keynote session. The meeting attracted more than 400 mining industry professionals, including engineers, executives, mining managers and suppliers from around the world. This year?s theme was ?New Ventures in Mining.? The meeting included concurrent technical papers sessions, a keynote session that featured three distinguished speakers and a panel discussion that provided updates on new mining developments in the state. The Minnesota Section of SME technical papers were presented in concurrent sessions, one on mining and one on mineral processing. The mining session included papers on drilling, ore blending, slope stabilization, dust control and operations management. The mineral processing session included papers on plant-control technology, ball mills, pellet strength and fans. Also offered was an SME short course titled ?Simulation based pellet induration optimization.? The Minnesota Section of SME held its annual business meeting during the conference. The 69th Annual University of Minnesota Mining Symposium technical sessions were held on April 15 and also consisted of concurrent sessions on mining and mineral processing. Mining session topics included logistics management, real-time performance measurement, uses for taconite aggregate and environmental issues. The mineral processing session included papers on ore liberation characteristics, zeta potential studies, induration modeling, fuel optimization and conveyor belts. The combined conference included a tradeshow with more than 80 booth spaces featuring vendors offering products and services to Minnesota?s booming mining industry. More than 900 people, including conference registrants and special invited guests of the vendors, at-tended the tradeshow opening reception. This was the largest trade show in many years ? an indication of the vitality of the mining industry in Minnesota.

Pamour Porcupine Mines, Limited has been actively engaged in the ''Custom Tolling'' of Gold-Bearing materials throughout the past decade. Experience acquired in this area was utilized to design and commission a custom gold milling facility at its operations in Timmins, capable of:(a) Accepting a bulk ore test sample at the rate of 150 t/d for crushing, sampling, and metals analysis;(b) Processing gold-bearing ores at a rated capacity of 150 t/d, to produce a goldbearing sulphide concentrate;(c) Processing free milling gold ores at a rated capacity of 80 t/d for total cyanidation;(d) Cyaniding custom gold flotation concentrates at a rated capacity of 80 t/d; and(e) Processing of complex gold/copper or carbonaceous ores, requiring an impurity flotation concentrate separation stage prior to cyanidation, at a rated capacity of 100 t/d.Items considered in the process design and development included:- various gold ore types available for processing;- flowsheet flexibility required;- sampling strategy as related to head grade determination and metals accounting; and- contract considerations.This paper highlights how these various considerations were used to design Pamour's ''Custom Gold Milling Facility''.

The following observations on the mineral composition of the lead-zinc ores at Captain's Flat, New South Wales, are based upon the examination of a collection of specimens made during a brief visit to the mine in January, 1942, in conjunction with earlier studies of specimens and mill products, which have 'been the subject of various Mineragraphic Reports, Captain's Flat is about 35 miles south of Canberra, and is connected with the main line from Goulburn to Bombala by a branch line to Bungendore, 23 miles away from the mine.The geology of the mine has been described by Kenny and Mulholland2 and by Harper.3 The country consists of Silurian sediments, which have been invaded by a composite sill of quartz-felspar-porphyry. Both sediments and sill have been folded gently, the axial lines of the folds trending north and south. In the vicinity of the orebodies the folds are steeper, and thru,st faults are associated with the anticlines. The intense compression and shearing that accompanied the faulting has converted the adjacent porphyry to schist. The Captain's Flat orebodies occur along such a zone of shearing and faulting, on the eastern limb of a steep, north-pitching anticline. At their southern end the orebodies are enclosed in the slates underlying the porphyry, but the northerly pitch of the fold carries the slate to greater depth in the northern part of the mine, where the orebody lies beween schistose porphyry on the hanging wall and massive porphyry in the footwall.THE OREBODIESThe orebodies occur along a N.-S. strike length of 4,200 ft., and from the point of view of mining, fall into three sections: the Elliot's Section to the north, the Central Section, and the Keating's Section to the south. The Keating's Section is separated from the Central Section by 500-600 ft. of mineralised ground of so low a grade as' to be called barren, and is offset about 125 ft. to the 'east of the Central body by an oblique fault striking N.300E. "Once every four years, the global mining industry gathers in Las Vegas, NV for MINExpo International. According to the show’s organizer, The National Mining Association (NMA), about 44,000 people attended the three-day show from Sept. 26-29. It was a show that featured the latest and greatest in equipment and technology. It also served as the backdrop for a snapshot of the state of the industry and while the picture of a global industry is never completely clear at any one time, one thing that is evident is technology has become a major force in the way mining is done.Had it not been for the awe-inspiring “iron” that was on the show floor in the form of haul trucks and shovels weighing hundreds of tons, one could be forgiven for thinking that they were at the Consumer Electronics Show. From training simulators designed to give a realistic experience of operating mining equipment to virtual reality technology that can take the user into the depths of an underground mine or the middle of a mapped ore deposit, an overriding theme of MINExpo 2016 was that the mining industry is connected like never before.In the north hall of the Las Vegas Convention Center was the GE Mining booth that was designed to showcase its efforts to help create a mine that will be connected from the front office to the face of the underground wall or bottom of the openpit. GE Mining called its booth a digital mine, and it included a suite of technologies that can improve performance, reliability, safety and operations. GE Mining said it views each mine as a mini-city, requiring power, water and transportation. Intelligent, real-time monitoring across the mine allows operators to make data-driven decisions that improve equipment reliability and optimize mine operations for substantial savings. And GE Mining was just one of many exhibitors showing off software that allows various types of equipment to communicate. Its booth sat in the shadow of the Komatsu booth with its new 360-t (400-st) 980E-4 haul truck that included GE’s 360-t (400-st) electric wheel motor. This is just one example of how technology is connecting machines, operations and people around the world and how mining is moving forward."

Of the five provinces and territories to table budgets as of March 31, 1993, the industry's scorecard shows: one win, one tie, and three losses.IntroductionBy the end of March, five of Canada's twelve provincial and territorial governments had tabled their 1993/94 budgets. Only Newfoundland appears to be paying any attention to the industry's concerns about the impact of taxation on competitiveness. "Dating back to 1934, Con Mine is located directly south to the city of Yellowknife in the NWT, Canada. With mill operations ceasing in 2004, a challenging surface remediation situation resulted which required the treatment of close to 40,000 m3 of Arsenic impacted material.Through much of the period of 1941 to 1970, Con Mine treated their refractory arsenopyrite ore through the use of an Edwards Heath roaster. With mounting environmental pressure and through Canadian governmental studies during 1950 to 1970, attention was directed to the Arsenic Trioxide material being collected by two Doyle scrubbers on the Con’s roaster stack. In 1970, the roaster was decommissioned leaving the resulting Arsenic material in two containment areas on surface.As a result of Con Mine entering into remediation in 2004, the Con’s autoclave, commissioned in 1992, was dedicated to the immobilization of high impacted Arsenic material through the combination of excess Iron under high temperature and oxygen overpressure. Examined, herein, is the history and chemistry of the Con Mine autoclave as well as the two stages of chemistry for the Water Treatment Plant used to remove arsenic, heavy metals and WAD cyanide from tailings water. Simplified capping plans of the tailing containment areas and a high arsenic impacted area are also presented.INTRODUCTION TO CON MINECon Mine (Con) is situated just south of the City of Yellowknife in the North West Territories (NWT) Canada and is bounded locally by Yellowknife Bay (North Extension of Great Slave Lake) to the east and Kam Lake to the west (Figure 6). The discovery of gold at Con was made in 1934 and mineral rights were acquired by Consolidated Mining and Smelting Limited (Cominco) and Falconbridge soon thereafter. Nerco Minerals Limited (Nerco) took control in 1986 and in 1993, was subsequently sold to, Miramar Mining Corporation which in turn was purchased by friendly takeover involving Newmont Mining in November 2007 as a result of acquiring the Miramar’s Hope Bay project. Con’s underground operations ceased in September 2003 and have since entered into the remediation of the site. The current surface lease holdings are about 340 Hectares (Table 1) which sits on top of the Archean-aged Yellowknife Greenstone Belt."

The practical concern of economic minability of large, high-grade manganese nodule deposits in the northeast Pacific Ocean is addressed in this Bureau of Mines report. Principal objectives are to (1) estimate tonnage and grade of deposits with significant potential and (2) describe and estimate profitability of operations designed to mine and process deposits with greatest apparent potential. Analysis of data from over 800 ship stations identified three areas for detailed study. Average metal contents of these areas range from 1.30 to 1.45 wt pet nickel, 1.00 to 1.24 wt pct copper, 0.21 to 0.26 wt pct cobalt, and 26.8 to 27.8 wt pct manganese. Estimated recoverable nodule resources are 67.0, 66.9, and 148.8 million dry metric tons (t). A system to mine, transport, and process nodules from the three sites is described and costed. Although hypothetical, the system utilizes hydraulic mining and Cuprion (Kennecott) processing, which have been successfully tested at pilot scale. Nickel, copper, and cobalt are the three primary products, but ferromanganese is a considered option. Estimated capital requirements are approximately $1.5 to $1.7 billion for three-metal production. If ferromanganese were recovered, an additional investment of about $130 million would be required. Operating costs range from $71 to $83 per dry metric ton of nodules without manganese, and from $103 to $123 per dry metric ton with ferromanganese. Discounted cash flow analyses predict low returns, ranging from 2.7 to 5.2 pct with ferromanganese and from 4.1 to 6.0 pct without.
