Background Carbon monoxide (CO) is an odorless, colorless gas that can cause illness and death by asphyxiation. Although the toxicity of CO is understood, CO exposure can occur from unrecognized sources. On April 2, 2000, a couple and their newborn infant were poisoned by CO migrating through the ground from blasting at a nearby coal strip mine into their home near Kittanning, Armstrong County, PA. They were treated at the hospital and diagnosed with CO poisoning. The infant had a carboxyhemoglobin level of 31%, the father 28%, and the mother 17%. Carboxyhemoglobin is the compound formed in the blood when CO is breathed. Persons not exposed to CO have carboxyhemoglobin levels of 0.3%-0.7%, and cigarette smokers have levels of up to 9%. Following release from the hospital, the family installed a CO detector in the basement of their home with the help of the local fire department. This Technology News briefly describes this incident to help increase awareness of the problem. It is discussed in more detail by Eltschlager et al. [2001]. Figure 1 shows the coal strip mine believed to have generated the CO. The initial suspect for the source of the CO was the furnace in the house. Only later did it become apparent that blasting at the nearby strip mine might be the culprit. A State investigation concluded that CO had migrated approximately 500ft from the blast, through the ground, and into the home. Figure 2 shows the locations and order of blasts at the mine. Initially, the mine did not plan to blast. Overburden generally consisted of weathered shale that could be dug without blasting. However, the mine decided to blast when hard sandstone that could not be dug out was encountered within 3 ft of the coal top. The blasting was intended to break up the sandstone. Blasting was done using bulk ammonium nitrate-fuel oil (ANFO) in 6.25-in holes 25-30 ft deep on a 16-ft by 16-ft pattern, with charge weights ranging from 17 to 170 lb. The closest blast to the home was 430 ft on April 17, 2000. The distance from the home to the blast area on the day when CO was first reported (March 31, 2000) was 500 ft. Twenty blasts were detonated at the mine between March 7, 2000, and April 20,2000. The blaster was careful to prevent fly rock since another home was only 150 ft away and a garage was 50 ft away. The quantity of explosive used in the blasts was sufficient to break the sandstone, but was less than would normally be used to blast overburden. The holes were typically one-third explosive and two-thirds stemming, with powder factors averaging 0.43 lb/yd3. Additionally, the muck was not immediately excavated after each blast. The heavy confinement on the explosive acted to prevent the typical release of blasting fumes into the atmosphere. Instances of CO migrating from blasts into homes have occurred before, but generally the distances involved were under 150 ft. It is believed that the blast design contributed to the confinement of CO in the ground, and discontinuities in the ground strata facilitated migration toward the residence. Upon inspection of the blast pitwall, a regular system of joints was observed. These joints are oriented roughly from the blast pit toward the house (figures 3-4).Blast-generated CO appears to have been driven into the joint system, displacing the air. When the joint system was saturated, the next succeeding blast pushed the CO into an uncased well next to the house (a high level of CO has been measured in the well shortly after a blast). From there, the CO entered the French drain system and then the basement of the house. The joints are vertical cracks through rock layers that occur on a regular spacing (figure 3). They are most common near valley slopes and are caused when valley walls converge as the stream cuts down through the rock layers. Often called hill seams, they roughly parallel the valley walls, but can curve and split, creating a pathway for gas movement (figure 5). These fractures are often encountered when underground mining proceeds close to outcrop. The system of fractures at the Kittanning blast pit was well developed and provided a path for transmission of CO both through the sandstone and up and through the overlying shales. Blasting engineers should be able to distinguish naturally occurring fractures from blast fractures and note the frequency, development, and orientation of the hill seams and their proximity to occupied houses. Synopsis Underground stone mining Is an emerging sector of the U.S. mining industry. As this expansion takes mines under deeper cover, and as more efficient mining methods are utilized, effective stone pillar design methods will become even more Important. Current design practices are examined and a discussion of safe mine layouts is presented as a first approach towards weighing the demands for increased production against increased risk. Risks to underground stone-mine workers include rib Instabilities, pillar failures and roof falls. Seventy-two stone-mine pillar designs were examined. Pillars with width to height ratios of less than 1.5 appear more likely to fail when subjected to excessive stress levels. When width to height ratios fall below 1.0 defects in the pillars, such as through-running discon¬tinuities, can have a significant influence on stability. Discontinuity persistence, dip, material properties and orientation are also important determining factors in pillar strength. During the past three years the number of active underground stone mines in the U.S.A. has ranged between 90 and 100. This number is expected to increase as the crushed stone industry responds to growing demands for its products.1 As more of the industry moves to benefit from the advantages of underground mining production from underground stone mines is expected to increase above its current level of approximately 66 000 000 r/year. Parker2 identified four advantages of underground mining operations: (1) surface developments, zoning laws and environmental concerns are often less of an issue; (2) stripping and restoration requirements are eliminated; (3) additional reserves are often available beneath the quarry floor, under pit slopes or under an adjoining property; and (4) space is created for secondary use. Underground mines enjoy the added benefits of work in a constant underground climate, rather than in the variable surface climate, minimization of community concerns by placement of the crushing, sizing and stowing operations underground and reduction of surface vibration concerns through smaller blasts. The drawbacks of underground mining include added health and safety hazards for the stone miners, whose health can suffer from increased exposure to falls of ground, airborne contaminants and fog in large underground openings. Injuries from falls of ground in stone mines have occasionally exceeded the incidence rates for other mineral resources mined underground. 3 Existing underground stone operations, by comparison with stone operations a decade ago, mine more stone at a faster rate and with larger equipment. Because of high demand mines face increased pressure to yield more stone per production blast. A majority of the miners use the V-cut blasting pattern, which limits the depth or pull for each shot to about 4 m. Therefore, to produce more stone, either the mines must work more faces or existing faces must be enlarged. The enlargement of underground openings poses problems in the maintenance o€ strata stability, First, the widths of the mine entries partially control the amount of sag or deflection that any given roof beam can withstand before failure. This deflection can take pi ace quickly after an opening is excavated or much later as weathering processes form additional, thinner beams. Because increased deflection increases the potential for roof beam failure, there is a limit to how wide a room can safely be made. These limits depend on local geological and stress conditions and, to some extent, the cost effectiveness of roof-bolting. Given these room width limitations, underground stone mines have placed more attention on expansion of production through benching where mining thicknesses permit. The heights of rooms partially control the strength characteristics of adjacent pillars: as the room height increases for a given pillar width the pillar width-to-height ratio (w/h) decreases. In general, stone pillars are very strong, but when the width-to height ratio diminishes the potential for pillar rib instabilities and pillar failures increases (see Fig. 1). [ ] In the underground environment the desire to utilize wider rooms, higher benches and multi-level mining results in the potential for unstable ground conditions. Although the number of failed pillars is currently very low, the potential for additional failures will grow if more slender pillars are developed, especially under deeper cover, with wider mining sections and in multi-level operations. It should also be noted that the passage of time acts to decrease the strength of many existing pillars. An examination of current design practices

The sudden, violent collapse of large areas of room-and-pillar mines poses a special hazard for miners and mine operators. This type of failure, termed a 'cascading pillar failure' (CPF), occurs when one pillar in a mine layout fails, transferring its load to neighbouring pillars, which causes them to fail, and so forth. Recent examples of this kind of failure in coal, metal and nonmetal mines in the U.S.A. are documented. Mining engineers can limit the danger presented by these failures through improved mine design practices. Whether failure occurs in a slow, non-violent manner or in a rapid, violent manner is governed by the local mine stiffness stability criterion. This stability criterion is used as the basis for three design approaches to control cascading pillar failure in room-and-pillar mines-the containment approach, the prevention approach and the full extraction mining approach. These design approaches are illustrated with practical examples for coal mining at shallow depth. Cascading pillar failure {CPF) in room-and-pillar mines can go by many other names, such as `progressive pillar failure', `massive pillar collapse', 'domino-type failure' or 'pillar run'. In this kind of failure when one pillar collapses the load that it carried transfers rapidly to its neighbours, causing them to fail, and so forth. This failure mechanism can lead to the rapid collapse of very large mine areas. In mild cases only a few tens of pillars might fail; in extreme cases, however, hundreds, even thousands, of pillars can fail. CPF can have catastrophic effects on a mine, and sometimes these effects pose a greater risk to health and safety than the underlying ground-control problem. Usually, the CPF induces a devastating air blast as a consequence of the displacement of air from the collapse area. An air blast can disrupt the ventilation system totally by destroying ventilation stoppings, seals and fan housings. Flying debris can seriously injure or kill mining personnel. The CPF might also fracture a large volume of rock in the pillars and immediate roof and floor. In coal mines and certain other mines this can lead to the sudden release of large quantities of methane gas into the mine atmosphere; a methane explosion might result from the CPF. CPF is at the far end of the spectrum of unstable pillar failure. At the other end are slow `squeezes' that develop over days to weeks and, because of their slow progress, do not pose an immediate danger to mining personnel. There is ample warning time for men and machinery to get out of the way of the failure. In a CPF, however, the failure progresses so rapidly that men and equipment cannot be evacuated in time. Significant seismic energy is released as a consequence of the rapid failure and collapse. CPF should not be confused, however, with coal-mine bumps and rockbursts. In some cases the damage can appear similar, but the underlying mechanics are completely different. As will he shown later, the mechanics of CPF depend on the applied vertical stress and the post-failure, i.e. strain-softening, behaviour of the pillars. In a CPF pillars shed their applied load rapidly and have very little residual strength after failure. The collapse itself may release significant seismic energy, but otherwise the mine is seismically quiet. Alter a CPF the openings in the affected mine workings have usually closed completely. In contrast, coal-mine bumps and rockbursts occur in seismically active mines. Research has shown that coal-mine bumps and rockbursts are seismic events induced by mining that damage underground mine workings. Bumps and bursts are thus a subset of a much larger set of mining-induced seismic events.[ Only some of these seismic events damage mine workings; fortunately, most do not. The mechanisms by which a mining-induced seismic event can lead to a damaging coal-mine bump or rockburst is still a significant research area.2 After a bump or rockhursr the affected mine workings may or may not be completely closed. For example, in the coal-mine bump described by Soler and co-workers2 the mine workings remained open even though the pillars were destroyed during the bump event. Because CPF differs from a coal-mine bump or rockburst, the design approaches to control their occurrence also differ. The design recommendations developed here apply to CPF and not to bump- or burst-prone mines. Cascading pillar failure examples Unfortunately, CPF has occurred in room-and-pillar coal, metal and non-metal mines. The most infamous example is the Coalbrooke colliery in South Africa, where 437 miners perished where [si >1 3at1ttsumsus7car kky vArtMixiL7i nesafr tramwsamn ures] on 21 January, 1960.1 Table I summarizes the mining dimensions of 13 examples of rapid pillar collapse in U.S. coal mines. All occurred during the 1980s and 1990s, and all happened suddenly or without significant warning. Most resulted in air blasts and damage to the ventilation system. Table 2 gives the mining dimensions of six room-and-pillar metal and non-metal mines in the U.S.A. where failure occurred in all probability by the CPF mechanism. The collapse areas can be huge. Fortunately, some of these failures gave advance warning. At the lead-zinc mine slabbing from pillars and roof falls began four weeks prior to the main collapse.3 At the copper mine considerable rock noise and smaller failures preceded the main collapse by five days.5 At the copper-silver mine it is not known if nature provided a warning.6 Evidently, the trona mine collapsed without warning.7,8 Rock noise and other failure warnings preceded the silica mine collapse by three weeks. v The presence or absence of warnings at the salt mint is not known. to Most of these collapses induced a substantial air blast; damage to the ventilation systems was limited, however, to bent airdoors and a few downed stoppings, except at the trona mine where extensive damage to the ventilation system resulted.

The practicability and dependability of two permissible methane-indicat- in detectors recently developed were proved conclusively by extensive tests in the return airways of all mines operating in the anthracite region in most of which samples and readings were taken with one or the other of the two in- dicating detectors. These detectors roved dependable and accurate within practical limits, and their general use should be encouraged as an aid to bet ter ventilation, more accurate control of air currents, and greater safety in mines.

Part I discussed the use of power shovels, draglines, and excavator cranes; part II discussed the use of power scrapers, slackline cableway excavators and hydraulic monitors; and part III discussed the use of hydraulic, clamshell, ladder, and dipper dredges. This circular discusses haulage equipment in general, particularly the use of industrial cars with locomotives or hoists as the motive power. Part V, to follow, will discuss the use of other types of haulage equipment.

A simple , quick, and relatively accurate device is needed for determining the presence of very small concentrations of organic halide gases in various atmospheres . Such a device would enable safety engineers , health officials , and others to discover quickly the presence of these gases in a suspected atmosphere and thus be able to apply protective measures promptly. This detector should prove useful in detecting leakage of halide gases from refrigerating and air - conditioning systems and in determining the presence of halide solvents in the atmosphere of dry - cleaning and degreasing establishments where organic halide solvents are used . Virtually all poison war gases are halide compounds . Because of the high toxicity of these gases , even in very low concentrations , a detector having the proper sensitivity should be useful in civilian defense for determining when atmospheres are relatively free of poison gases .

The electrodeposition characteristics of seven binary alloys of titanium have been investigated. The object of the electrorefining experiments with an alloy was to determine the efficiency of titanium separation from individual alloying elements under established conditions. Reported results are only indicative. It is conceivable that numerous duplications of tests might give average maximums that would vary from those noted in this report; however, in the few tests that were repeated, the magnitude of transfer of the alloying elements was of the same order. The results detailed herein are for a given set of conditions. Many combinations of operating variables are possible, and the results achieved under different conditions probably would differ from those reported.

This preliminary paper is intended to answer briefly some of the inquiries coning to the Bureau of Mines regarding the so-called "paraffining", of oil wells and methods of preventing and remedying that trouble. The increased difficulties in operation and the losses of production caused by the deposition of gummy and waxy hydrocarbons, commonly called paraffin, in the wells, tubing, other pumping equipment, and in the pores of the productive sands, have long been recognized but the trouble has not been overcome. The nature and causes of the trouble, together with some of the possible methods by which it may be diminished or overcome, are outlined below. Character of the Peraffining Materials. The so-called paraffin that collects in oil wells end in the pores of the oil-producing sands immediately around the wells, is generally made up of amorphous wax or uncracked paraffin, asphalt, or other gummy hydrocarbons mixed with more or lees water, oil, and inorganic silt. Some of the samples of this "paraffin" collected from oil wells in different fields and examined by the Bureau of Mines contain as mach as 20 per cent by weight of water and inorganic silt, whereas other samples are composed entirely of waxy hydrocarbons and oil. The inorganic silt accompanying this "paraffin" is mostly fine sand, clayey materials, common salt and finely disseminated precipitates of calcium and magnesium carbonates. Sulphates of calcium, barium, and strontium are less common constituents of the "paraffin."

An ever-increasing demand for beryllium and beryllium alloys has directed attention to the milling of low-grade domestic deposits as a possible means of supplying part of the Nation's ore requirements. The United States generally produces less than 10 percent of its annual supply. For example, new supply in 1953 was estimated at about 9,250 tons, of which the United States imported 8,500 tons. Domestic production was only 750 tons.2/ Since record high prices for beryl in 1952 and 1953 did not greatly increase domestic product ion, the United States must develop mechanical beneficiation processes to use low-grade deposits and reduce dependence on foreign sources

This Bureau of Mines report presents information on the origin, collection, movement, and consumption of iron and steel scrap in California and Nevada. As far as practicable, information is presented on a grade basis. The iron and steel scrap industry in California is important from the standpoint of employment, economic essentiality, and dollar value. About 16,000 persons are either directly or indirectly employed on a full- or part- time basis in the collection, preparation, and sale of iron and steel scrap in California; in 1957 about 70 percent of the tonnage of products of California steel mills, valued at $271 million, was derived from scrap; the value of scrap in 1957 ($117 million) is about $10 million greater than the value of portland cement produced in California that year. The iron and steel scrap industry in Nevada is small.

THE BITUMINOUS coal and lignite industry declined slightly in 1961, compared with the 1960 figures. All of the major items, production, consumption, average value, exports, and employment, declined. Even the steady upward trend in mechanization failed to materialize; the percentages of total tonnages mechanically loaded and cleaned remained the same as in the preceding year. However, tonnage per man per day increased from 12.83 tons in 1960 to 13.87 tons in 1961, owing partly to the increased tonnage produced by continuous mining machines. Also, the percentage of total production mined by stripping and by augers increased slightly. Production.-The output of bituminous coal and lignite in the United States in 1961-403 million tons-was 3 percent less than the 416 million tons produced in 1960. Production was retarded in 1961, owing largely to the business recession and reduced exports.

This Bureau of Mines report describes photo lineaments obtained from stereoscopic examination of vertical aerial photographs and by the Ronchi grating study of aerial photoindex sheets. The photolineaments are compared with the trends of oriented fracture systems procured by measuring the compass direction of joints in bedrock. The relative reliability of the methods is shown, and the results are presented in graphical, tabular, and written form. Analyses of field measurements of strikes of bedrock joints in the project area indicate for the most part two intersecting sets of major joints oriented N 76° W -N 15° E, and N 57° W -N 27° E. The analyses of lineaments determined by stereoscopic examination of aerial photographs show dominant trends of lineament zones of weakness to be an average of N 65° W -N 20° E. Analysis of lineaments found by Ronchi grid viewing of aerial photoindex sheets indicate dominant trends of N 70° W -N 27° E. The three methods give similar findings and verify the work of previous authors. The Ronchi grid method is a useful guide, but it cannot be substituted for the two more painstaking methods.

Three long horizontal holes were drilled from a directionally drilled sur¬face hole at the Emerald Mine near Waynesburg, Pa. The purpose was to adapt the technique of directional drilling for use in draining methane from coalbeds. A 504-m (1,652-ft) long, 76-mm (3-in) diam¬eter, circular arc pilot hole was drilled, using a downhole mud motor, to enter the Pittsburgh coalbed at a verti¬cal depth of 305 m (1,000 ft). The hole was reamed to 222 mm (8-3/4 in) and was cased to a measured depth of 486 m (1,595 ft). Three 76-mm (3-in) diameter horizontal drainage holes were then drilled, totaling 2,909 m (9,544 ft) of horizontal hole. Improvements in drilling methods increased the average drilling rate from 20.4 m (67 ft) to 64.9 m (213 ft) per day. The cost of drilling the directional and horizontal holes (using a Government-owned drill rig) was $1,169,530, a figure inflated by inexperience and by delays caused by lost-circulation problems and fishing operations. An estimated total drilling cost of $960,000 (including rental of a drill rig) for an improved system, was determined by a detailed cost analysis. Initial gas and water production from November 1979 through May 1980 was low because of caving of the horizontal holes drilled in shale near the bottom of the casing.

This report presents the Phase I progress in the development of a Radar Coal Thickness Sensor (RCTS) being conducted by Boeing Aerospace and Electronics for the Bureau of Mines. A method is developed for measuring both the dielectric constant and depth of the coal shale interface using a modified UHF network analyzer and antennas in proximity to the surface of the coal. Both bistatic and monostatic systems, are compared. The algorithm technique uses synthetic range gating for clutter rejection and full polarimetric scattering matrix techniques to extract the dielectric constant and depth to sub-wavelength accuracies. Tests were run both in the lab and in a coal mine to build an error budget and demonstrate a validation of the model. The development of a prototype radar suitable for coal mine robotics is specified.

Lignite mining in North Dakota was begun about 1884 to supply the domestic needs of settlers. Annual production has grown rather slowly but steadily, exceeding 3 million tons in 1950. The western part of the State has lignite reserves, amounting to 351 billion tons, underlying an area of 28,000 square miles. Contour strip mining has almost completely replaced underground mining as the method of production. Lignite is often found under rather shallow and easily removed overburden, and this is the primary reason for the change in mining practice. Current mining operations are conducted where the overburden is 30 to 70 feet thick. Contour strip mining results in high production rates, at present 27 to 56 tons per man-day. These high production rates result in relatively low production costs.

A small-scale method was developed for determining the free-swelling index of coal samples of limited quantity, especially the macerals. Results of a series of tests using a 0.3-gram sample correlated well with those by the standard method using a 1-gram sample. The standard and small-scale methods are modified to give unrestricted swelling for those coals having swelling index numbers above 7.

"Initiated as a part of the Bureau of Mines program for development of strategic minerals for national defense, various mines in the Poughkeepsie Gulch and Mineral Point mining districts were examined by C. R. Wilfley, W. R. Hamilton, .and N. E. Wvvlwy, Jr., Bureau Of 'Mines engineers, in July and August 1943, July 1944, and September 1945. In a subsequent report a proposal was made to evaluate this isolated area 'of some 15 square miles located in Ouray and San Juan Counties, Colo.Reconnaissance work in the area was begun in June 1946. Mapping and sampling were begun in July and was completed in Pougkeepsie Gulch and extended westward to include the Hendrick Gulch area of the east side of Ironton Park. Project work was recessed for the winter on October 15, 1946 after approximately 6 square miles of the Poughkeepsie Gulch,district been mapped and 801 samples had been taken.In this report the physical features, climate, and ore deposits districts are described, and a brief history of mining and production lead, zinc; and silver is given.The individual mines and prospects in, the area are described in detail.A list of patented claims and ownerships is given in appendix I, and analyses of samples are tabulated in appendix II.The San Juan preliminary project was directed by Paul T. Allsman, chief, Salt Lake City Branch, Mining Division and was supervised by William M. Traver, Jr., supervising engineer, Colorado Field. Office.Chemical analyses were made by the Salt Lake City laboratory under the direction of H.E. Peterson, chemist, and S. R. Zimmerley, chief, Salt Lake City Branch, Metallurgical Division.Harold A. Waddington, president, Great Divide Mining & Milling Corp., and Ernest Miller, owner, Como Consolidated Mines, were particularly cooperative in making available maps and data concerning their properties."

Properties obtained for columbium carbide-carbon castings ranging in car-bon content from 12 to 17 weight-percent were compared with those of hot-pressed composites of similar composition. The properties were oxidation resistance in static air between 500° and 700° C, linear thermal expansion between 20° and 2,400° C, electrical resistivity between 20° and 1,500° C; microhardness between 20° and 1,500° C, and modulus of rupture between 1,300° and 1,800° C. CbC-C castings differed markedly from hot-pressed material with respect to microstructure, electrical resistivity, and modulus of rupture, but behaved quite similarly with respect to oxidation resistance, thermal expansion, and hot hardness. In oxidation resistance, thermal expansion, and hot hardness both materials were independent of carbon content. Both castings and hot-pressed composites oxidize rapidly in air above 600° C, have thermal expansion coefficients very similar to columbium metal, and the logarithm of the hardness decreases linearly with increasing temperature. The microstructures of hot-pressed composites contained varying amounts of graphite in a CbC matrix but did not contain significant amounts of a eutectic structure present in all castings, nor did they contain graphite needles which were present in the hypereutectic castings. The higher electrical resistivity of hypereutectic castings over hypereutectic hot pressings was attributed to the presence of highly oriented graphite needles in the castings. The modulus of rupture of castings was 1-1/2 to 2 times that of hot-pressed composites. Both materials displayed bend ductility above 1,800° C.
