When one looks back and does assessments of what has been the key ingredient in achieving success in our safety efforts, there are many issues that spring to mind and cover a wide array of elements. One does not know which has the ultimate maximum effect. In his presentation yesterday, Alan Peers, our SHEQ leader, covered a lot of detail with the way we are managing safety at Modikwa. I have attempted to identify some of the key essentials in making the mine safe. Passion Safety is addressed with a passion that is unbelievable at times. When I get phoned because an incident has occurred and in reporting the incident, there are already action plans being put in place to prevent re-occurrences, you understand the commitment to put employees? safety first. You hear the disappointment in your subordinates? voices when they have to share the fact that an incident or accident has occurred.

The mineral sector is a fundamental part of the U.S. economy. It contributes to the real gross domestic product (GDP) at several stages —basic (mining), intermediate processing, manufacturing of finished goods and construction. The real GDP increased by 3.5 percent from 2004 to 2005. The nominal GDP was $12.5 trillion at the end of 2005. Housing starts were up by more than 5 percent for the year, and the unemployment rate decreased to 5.1 percent in 2005 from 5.5 percent in 2004. The prime interest rate increased from 5.25 percent at the beginning of the year to 7.25 percent at the end of the year. The value of minerals mined in the United States in 2005 rose significantly because of increased unit prices for some metals —particularly molybdenum — and significantly increased production for some industrial minerals, mainly cement, construction sand and gravel, and crushed stone.

The kinetics of reduction of copper (cuprous) sulphide with CO-CO2-He gas mixtures, using an excess of CaO(s) as a sulphur-scavenging substance were investigated in the temperature range 855° - 950°C by a thermogravimetric method. Several gas compositions were employed at each temperature. Potential carbon deposition was overcome by preheating the reducing gas mixture. The phase-composition and the morphology of partially reactedsamples were analysed by a SEMlEDAX unit. The reduction rates showed strong dependence on the bulk gas compositions (CO, CO2) and, in most cases, the rate declined with rising temperature. The rate-controlling step appears to be the diffusion of CO2 gaseous species through the micropores of the CaS(s) - product layer which builds up around the CaOcore.

Samples collected during the exploration phase of mining represent baseline conditions at the site. As such, they can be very important in forecasting potential environmental impacts should mining proceed, and can become measurements against which future changes are compared. Constituents in stream water draining mined and mineralized areas tend to be geochemically, spatially, and temporally variable, which presents challenges in collecting both exploration and baseline water-quality samples. Because short-term (daily) variations can complicate long-term trends, it is important to consider recent findings concerning geochemical variability of stream-water constituents at short-term timescales in designing sampling plans. Also, adequate water-quality information is key to forecasting potential ecological impacts from mining. Therefore, it is useful to collect baseline water samples adequate for geochemical and toxicological modeling. This requires complete chemical analyses of dissolved constituents that include major and minor chemical elements as well as physicochemical properties (including pH, specific conductance, dissolved oxygen) and dissolved organic carbon. Applying chemical-equilibrium and appropriate toxicological models to water-quality information leads to an understanding of the speciation, transport, sequestration, bioavailability, and aquatic toxicity of potential contaminants. Insights gained from geochemical and toxicological modeling of water-quality data can be used to design appropriate mitigation and for economic planning for future mining activities.

Transportation accounts for some 26% of all mine accidents in South Africa. Although this figure may not be as prolific as fall of grounds accidents or accidents caused by seismic events, the enormity of transportation accidents demands attention, especially as many of these accidents are avoidable. This paper investigates vertical (i.e. shaft), horizontal (i.e. haulages), and in-stope transportation systems and their function as it applies to safety. Recent transportation accidents are discussed looking at current trends within the three transportation areas. In the shaft area, discussions will focus on handling of material cars, loading and off-loading of the workforce and management of shaft orepass systems. The horizontal component will focus on personnel transportation, safety around trains and hoppers, and handling of material in the cross-cut. The discussion on in-stope transportation will focus on the general congestion of the stope environment and how it affects the overall movement of personnel, material, and rock. Of concern are the large numbers of material handling and scraper winch accidents that occur and which will be discussed in some detail. The paper will conclude with a section on proposed guidelines within the three above-mentioned systems for mine engineers to consider in mine design and life of mine planning and scheduling. "The number of investigations reported during the past year indicates that control of the health hazard due to abnormal air conditions is still a matter of serious consideration in the mining, metallurgical, and allied industries.Australia. - In an investigation into the health and working conditions of employees in the mining industry of Tasmania and Victoria3, it was found that although pulmonary tuberculosis may occur early in life, silicosis is e disease of slow evolution and does not appear frequently under the ace of 40. A period of at least ten years in the industry was found necessary to produce silicosis, and in no case was the disease found in those who had not worked underground. A very high incidence of silicosis was found in all fields among those who had worked underground 20 years and longer. There was some evidence that silicosis complicated by tuberculosis may occur earlier than uncomplicated silicosis.England. - In a paper presented before the Section on Occupational Diseases of the British Medical Association, Dr. E. L. Middleton, the medical inspector of factories, called attention4 to the occurrence of silicosis in various industries throughout the country.punitive on some quite harmless industries. Thus Middleton says: ""Where the amount of dust is small the action of the phagocytes is likely to be effective, but where it is great . . ."" What dosage is indicated by ""small"" and ""great"" in this connexion? If the meaning of ""small"" were known, would it be correct to go on to say, as he does, ""each successive dose of silica produces a summation of effects""? Then, again, would it be fair to say ""a comparatively brief period of exposure may be the only determining factor""? What, in minutes, hours, or days per week, per month,' or year, is intended by ""a comparatively brief period""? ""Intermittency of the process"" is another expression used in summing up the reasons for and against a standardization of air purity by a gravimetric process or enumeration of the particles; the arguments against such a standardization are, by Middleton, considered strong, but they are by no means convincing to us. A standard has been accepted in respect of the amount of lead in milligrams per 10 cubic metres of air; Dr. Ronald Lane showed in another paper read that adherence to this makes the occurrence of poisoning highly improbable. Mavrogordato has placed on record figures of silica dust at different stages of the improvements in the workings of the Rand mines, and such a control is in constant operation in them today.We emphasize these points because apparently the Government has taken the unusual course of legislating first and postponing inquiry to a later date, as in the ""Various Industries (Silicosis) Scheme, 1939."" The insurance companies also have a considerable interest in the matter of compensation for the silicosis risk. Where exposure - as in the refractories industries, the grinding industries, sandstone industry, and pottery industry - is day in and day out, they rightly demand a premium far higher than for any other disease scheduled under the Workmen's Compensation Act - 5 per cent on the wages bill. row every industrial employer, such as the paint and varnish manufacturer, who has a sack of crushed silica in his store .room, from which he extracts once a week enough to make a paste With spirit and add to his other ingredients in order to give them the necessary ""bite,"" becomes involved, and may have to abandon the use of the material or pay 8 per cent on his wages bill for an absolutely negligible risk.. From 1990 to 1996, 16 states reported 92 injuries from falls of roof, rib or face in the more than 90 underground stone mines in the United States (Fig.1). Missouri, Pennsylvania and Kentucky accounted for 48% of the total number of injuries (Fig. 2). Of this total, 11 miners were killed. Of the 11 fatalities, 10, or 91%, were associated with unrecognized loose or failed rock within the roof beam. Additionally, unrecognized roof beam failures resulted in a majority of the falls of ground injuries occurring in underground US stone mines from1990 to 1996. This number is not large in magnitude. But a workforce of fewer than 2,000 miners makes for a high fatal-accident rate. The severity of the typical fall-of-ground injury is, in general, very high. About three-fifths of all roof, face and rib fall injuries were designated by the US Mine Safety and Health Administration (MSHA) as some kind of lost time accident. MSHA assigns each accident a severity value from one to six. A one represents a fatality, a two, a permanent disability and three, a lost time accident (Fig. 3).

Low sulfidation epithermal deposits are important sources of gold and silver, and are considered the extinct analogues of modern geothermal systems. The aerial extents of alteration halos that surround twenty-two epithermal deposits range from 4 to 315 km2, with many less than 60 km2. Deposits with alteration halos <30 km2 typically have well defined limits, whereas deposits with alteration exceeding 30 km2 are commonly complicated by overlapping alteration centres related to adjacent epithermal deposits or other deposit types (i.e. high sulfidation, porphyry). Irrespective of the physical coverage of vein or alteration halos, both large (>60 km2) and small (<20 km2) deposits can contain large amounts of gold and silver. The size ranges for twenty-one analogous liquid-dominated geothermal fields from around the world have a restricted size range of 5 to 60 km2 and irrespective of physical size, commonly have multiple zones of focused fluid upflow. From an exploration perspective the comparison between epithermal deposits and geothermal fields provide the following four important points. 1) Alteration halos surrounding epithermal deposits as well as those for analogous geothermal systems show a range of sizes, although most are less than 60 km2. Therefore, where alteration exceeds 60 km2 multiple vein systems related to several paleogeothermal systems are predicted. 2) Multiple upflow zones are common in geothermal fields of all sizes (5 to 60 km2) and although difficult to demonstrate in epithermal deposits there is a strong possibility of discovering several discrete adjacent mineralised centres. 3) The orientation of veins parallels the principle orientation of the surrounding alteration and where alteration is wider than the vein strike length, this could suggest the occurrence of adjacent veins. 4) The physical size of both veins and alteration zones is not important in relation to amount of contained gold, as both large and small can contain large amounts of gold. These conclusions provide insight into the physical size limits and hydrologic structure of these paleogeothermal systems and highlight the potential for multiple mineralisation centres within a single alteration halo.

Through a unique partnership between the Colorado Mine Safety and Training Program (CMSTP), the Colorado Rock Products Association (CRPA) and the Higher Education and Advanced Technology (HEAT) Center, a video packed, dazzling CD-ROM training program was developed for new miners entering the aggregate industry. Why this product was developed, and why the aggregate industry, will be discussed along with the challenges faced throughout the project. Cooperation from every one involved including MSHA made this the product of which all other training materials in the mining industry will be measured against in the 21st Century. This product's versatility and wide application makes it a tool that is as critical to a mine's success as any earth moving machine. Developed with both state and federal grant moneys allows this product to be affordable to small operators and independent contractors. The quality of this two CD-ROM set rivals any commercially made CD-ROM training.

"Abstract: Diaphragm walls are used to provide a stable and watertight confinement for the construction of tunnels, underground railway stations, deep basements and other structures requiring deep excavations. Water seepage and soil loss through the joints have occurred on several occasions, causing extensive damage. This may have been due to uncontrolled concrete flow between the joints, unexpected geologic conditions or just poor workmanship.Different ways of quality assurance are applied on construction sites. Most of them rely on recording parameters of the construction equipment and conventional concrete quality control. In the frame of the EUREKA/ZIM project DiWaQ we have looked at several ways to perform non-destructive quality control. Some methods are to be applied after excavation before concreting (phase 1); others after the wall is completed (phase 2).For phase 1 we have used two different principles for checking the shape of the excavation, especially the joint to existing panels. A mechanical device which can be adapted to the excavating equipment was developed; field tested and validated on test and construction sites. It is described in detail in another paper in the same volume. A sonar-based device with a 10 mm resolution has shown good performance in the lab, but is still under development.For phase 2 different temperature sensors have been lowered into access holes to monitor the hydration heat developed during concrete curing. The same tubes can also be used to perform electromagnetic and ultrasonic crosshole measurements across or along joints to detect flaws. Especially the ultrasonic method has shown promising results on validation specimen at the BAM test site at Horstwalde, but limitations as well. Success substantially depended on the joint type, flaw size and distance of the sensors to the joint."

The study of the hydromechanical behaviour of fractured rock masses requires a detailed knowledge of the isolated fractures. The aim of the paper is the simulation of the evolution of the void space, located between both walls of a fracture, during a shear test under constant normal stress. Results of the mechanical simulation model are first validated then analyzed in detail. We observe that, during the shear process, there is an opening of the fracture with the creation of preferential channels. These channels are determined at the early phase of the shear process (pre-peak phase), and we observe during the post-peak phase that void areas increase in size rather than the creation of new apertures. Die Untersuchung des hydromechanischen Verhaltens eines zerlegten Felsmassivs erfordert genaue Kenntnisse der einzelnen Spalten. Ziel dieses Artikels ist die Simulation der Evolution des Hohlraumes zwischen den beiden Wänden eines Spaltes während eines Abscherungsexperimentes unter konstantem senkrechtem Druck. Die Ergebnisse der Modellierung werden zunächst mit dem Abscherungsexperiment verglichen und dann im Einzelnen ausführlich analysiert. Während des Abscherungsprozesses tritt eine Öffnung des Spaltes auf, die die Bildung von bevorzugten Fliesswegen fördert. Diese Kanäle werden bereits in einer frühen Phase des Abscherungsprozesses (pre-peak Phase) festgelegt. In der post-peak Phase tritt dann eher eine Vergrößerung dieser Kanäle, als die Bildung neuer Hohlräume auf. L’étude du comportement hydromécanique d’un massif rocheux fracturé ne peut se faire sans une connaissance approfondie des fractures le constituant. L’étude présentée dans cet article concerne la simulation de l’évolution de l’espace des vides présent entre les deux épontes d’une fracture au cours d’un essai de cisaillement à contrainte normale constante. Les résultats du modèle sont tout d’abord validés puis analysés en détail. On constate que, au cours du cisaillement, il apparaît une ouverture de la fracture permettant la création de chenaux préférentiels. Ces derniers sont déterminés très tôt dans le cisaillement (phase pré-pic), et on observe dans la phase post-pic davantage un accroissement de la taille des chenaux plutôt que la formation de nouvelles zones de vides.

Shear-flow coupling tests are conducted using an original apparatus, and a shear-flow coupling model using Geographic Information System (GIS) is developed in order to evaluate shear-flow coupling properties of a rock joint. Shear-flow coupling tests indicate that the hydraulic conductivity of joint once decreases, and then rapidly increases at the shear displacement near the peak shear stress. The proposed model simulates the test results relatively well. However it indicates that it is difficult to apply the local ‘cubic law’ model in the initial shear stage, in which the change of selective flow path is remarkable. Scher-Fliess-Kopplungsversuche werden mit einem neuartigen Versuchsgerät durchgeführt und mithilfe von Geo-Informationssystemen (GIS) gedeutet, um so ein Modell für die Scher-Fliess-Kopplung in Gebirgstrennflächen zu entwickeln. Die Versuche zeigen, dass die hydraulische Leitfähigkeit zuerst abnimmt, dann aber schnell wieder zunimmt sobald die maximale Scherfestigkeit erreicht ist. Das vorgestellte Modell bildet die Testergebnisse relativ gut nach. Es zeigt auch, dass es schwierig ist, das lokale Cubic Law im Initialstadium der Scherbeanspruchung anzuwenden, in dem die Variationen selektiver Fliesswege gross ist. Les essaies hydro-mécaniques couplés en cisaillement sont réalisés grâce à un appareil éxperimental prototype et un modèle de couplage hydro-mécanique utilisant le S.I.G (systeme d'information géographique) développé dans le but d'évaluer les propriétés hydro-mécaniques des joint rocheux. Les résultats indiquent que la conductivité hydraulique des joints décroit puis augmente rapidement en se rapprochant du pic de cisaillement. Le modèle proposé simule relativement bien les résultats expérimentaux ; mais indique également qu’il est difficile d’appliquer localement la loi cubique au début du cisaillement où, les changements d’orientation dans le réseau d'écoulement sont considerables.

In New South Wales, development applications for major coal mining proposals are determined by the Minister for Planning and Environment and applications for mining leases and other mineral titles are dealt with by the Minister for Mineral Resources. The environmental conditions placed on the development consent, the coal mining lease and the approvals and licences under the pollution control legislation, amount to a substantial component of any new project's planning and commissioning considerations.An outline is presented of the framework within which various approvals are granted and conditions for environmental management are imposed upon mining projects. The principles and concerns, as perceived by the regulatory authorities, on which the majority of approval conditions are based are also outlined.No attempt has been made to analyse the extent to which these principles or standards are achieved in practice, nor to discuss the degree to which some of the conditions are achievable with the current status of technology and economics in the coal industry.

"The theory of flocculation usually is in a state of flux, involves many diverse phenomena, and is complex and sometimes contradictory in details. Instead of attempting a general discourse, I shall discuss selected observations made at our Stamford Laboratories, which impinge on theoretical aspects in two somewhat distinct areas of flocculation. Since some of you many not be familiar with flocculation, and :.n order that we all attach the same meaning to the jargon used, I first would like to review briefly a generalized picture of what goes on.Assume we have a suspension of particles in water, with a distribution in the sizes of particles. The surfaces of the particles interact to some extent with the water--· tending to alter the extent of hydration, may generate some ion exchange, all being somewhat variable with time--developing a surface condition which is very complex in details. We refer to this suspension as the ""substrate.""In the usual situation, if the suspension is allowed to stand quietly, the large particles settle out first, with somewhat smaller particles settling more slowly, and then there is a portion of very fine material which takes a long time or doesn't settle out at all. This resistance to settling t~ the very fine particles results from many factors, but all relate back to the size and density of the particles, to their surface properties, and to the other components in the aqueous suspending medium."

Remediation of contaminated soil represents a significant market for application of Deep Soil Mixing (DSM). Eighty-one percent of U.S. Environmental Protection Agency (EPA) Superfund Program’s site remedies have involved contaminated soil. Contaminated soil is frequently treated by In-situ Solidification/Stabilization (ISS). ISS treatment of contaminated soil involves mixing binding agents into soil using DSM techniques. DSM is a core business for many DFI members. Members may improve and broaden their services to clients by learning about the recent ISS treatment of contaminated soil at a Superfund site. ISS treatment is part of the EPA-selected remedy for a Superfund Site in the metro New York City area. The property has seen heavy industrial use for more than a century. Soil at the site is contaminated with a variety of volatile organic compounds (VOCs), polycyclic aromatic hydrocarbons (PAHs) including coal tars and heavy metals, principally arsenic and lead. Beginning in 2017 and continuing through 2018, approximately 150,000 cubic yards (115,000 cubic meters) of soil will be treated by ISS at the case study site. Portland cement and slag cement are mixed into the soil by DSM techniques using GPS guided excavators to immobilize the hazardous constituents within the treated soil. ISS treatment by DSM will contribute to redevelopment of this valuable property. This paper will describe ISS constructability and performance, case study grout mix design with geotechnical results for strength and permeability, and deep soil mixing techniques. INTRODUCTION In-situ solidification and stabilization (ISS) is a deep soil mixing (DSM) technique applied in the environmental remediation field to physically encapsulate, or chemically bind, contaminates of concern such that the impacts to human health and the environment are mitigated. Common remediation applications of ISS include mixing cementitious reagents into contaminated soils to form a monolithic mass with desired performance characteristics, and the installation of vertical subsurface walls designed to control groundwater movement. A key characteristic of ISS is that the contaminated soils are treated in their existing location at the project site and then are left in place after treatment. Typical performance characteristics of the ISS treated soils include improved unconfined compressive strength (UCS), reduced hydraulic conductivity (HC), and reduced leaching potential of soil contaminants. Increased shear resistance of the treated soil may also be a performance requirement where seismic loading is a concern.

River Des Peres Foul water Interceptor (Skinker–McCausland Tunnel) is a gravity sewer tunnel constructed to eliminate dry sewage overflows during high river stages and to improve the wastewater collection and treatment. The original 78-inch (2m)finished diameter Skinker-McCausland Tunnel started as a 21,500 feet (6.5km)interceptor with 13 work, drop and access shafts. The tunnel constructions included soft ground, mixed face and rock tunneling in karst formation with maximum overburden of 175 feet (53m). Due to the success of the project, the tunnel alignment was extended another13,361 feet (4.1 km) and 7 new structures by negotiating two additional segments with the contractor and the designers.

The Anglo American phosphate mine is located in the Brazilian state of Goiás and possesses one of Brazil’s most important phosphate rock deposits, with the site being the country’s second largest producer of phosphate rock concentrate. Despite the site having been in operation since 1979, the mining team only started using explosives in 2004 when rocks with greater hardness started to appear in the mine’s geological profile. Rock excavation was previously performed mechanically. Until 2013, only rocks with high hardness were blasted, this following the standard drilling grid that was defined in 2004 without any changes in the blast parameters. The aim of this paper is to show how fragmentation optimisation has yielded positive results at all stages of the production chain, from the mine to the mill passing through an impact hammer crusher, a rod mill and ball mill, operating in close circuit with hydrocyclones.While incorporating the mine to mill holistic approach during the operational integration, the focus was on defining the blasting domains and respective optimal fragmentation and measuring the benefits of blasting in terms of reduction in energy and grinding media consumed in the comminution processes. In the first stage, the blasting team made changes to the blasting parameters and assessed the effects of these changes on fragmentation using computer software and on-site measurements. This generated a database that made it possible to divide the mine into blasting domains. For the second stage, teams from the plant, geology, mine planning and operation departments jointly made comparisons between the performance when processing stockpiles, which only included high hardness blasted ore (20–30 per cent of blasted ore). Changing the drilling grid’s size from 3.0 m × 3.8 m to 2.0 m × 2.6 m for high hardness rock with the presence of silicates resulted in a 40 per cent reduction in the P80 for the blasted material (from 459 to 278 mm), while changing the grid from 3.0 m × 3.8 m to 2.6 m × 3.2 m for high hardness rock without the presence of silicates generated a reduction of 25 per cent (from 270 to 204 mm). There were gains observed from the measurements at the mineral processing plant in terms of specific energy spent on comminution and on the consumption of grinding bodies in the rod mill.
