Because of the lack of a universally accepted definition or standard for RCA, we are left to our own conventions in the marketplace as to what is most ?effective? or a ?Best Practice? for our own facilities or organization. This lack of uniformity in the application of RCA produces an equal lack of uniformity in the measureable outcomes of RCA on the bottom-line from site to site and industry to industry. What is the definition of success for an RCA? Is it regulatory compliance? Is it some quantifiable measure of bottom-line performance? Is compliance and bottom-line performance the same thing? Can we measure the ROI we receive by doing compliant RCA? These questions must be adequately addressed if any definition of RCA is to be effective on the bottom-line. Like our various Corporate Vision and Mission Statements, what would be our Vision and Mission for RCA? When pondering the answer to this question, we should consider: ? What Key Performance Indicators will the Success of RCA be Linked to? This provides a direct correlation to the corporate dashboard and therefore garners support for the RCA effort when implemented at the sharp end. Not until we know what KPI?s will effectively measure our maintenance and reliability objectives can we begin to establish which opportunities will afford the greatest returns1. ? What Bottom-Line Metrics will be Tracked to Measure the Effectiveness of Our RCA Effort? Will it be increased production, profitability, regulatory compliance, etc.? Will it be decreased injuries/fatalities, claims, customer complaints, risks, etc.? We should be specific about these metrics and their correlation to the metrics on our corporate dashboard. ? What Will Trigger an RCA Investigation? Normally companies will establish quantifiable thresholds to be surpassed before an RCA will be conducted. Examples may be an OSHA recordable injury, a failure event in which production losses exceed $250,000, an equipment failure in excess of $50,000 or a regulatory violation. This is the norm. However, we should realize this is using our RCA skills totally on a reactive basis. High Reliability Organizations (HRO) strive for proaction and they also use RCA proactively. Why can?t we use RCA to analyze why unacceptable risks from an FMEA are so high? It is perfectly applicable yet rarely implemented in this fashion.

Mining is big business in Ontario. The province is Canada?s leading mining region. It consistently ranks among the top 10 jurisdictions worldwide for exploration spending, reaching about C$1 billion in 2011. More than 26 percent of all exploration and appraisal dollars spent in Canada are invested in Ontario. Minerals produced include nickel, copper, platinum group elements (PGE), cobalt, zinc, diamonds, gold and silver. It was reported that there are more than 600 mining exploration projects in the province. And, recent discoveries of chromite in northern Ontario?s Ring of Fire will add to the province?s mineral production in the coming years.

Six different types of quartz and quartzites used as raw materials by ferroalloy plants have been investigated with respect to their thermo mechanical properties. In order to investigate the effect of the thermal shock caused by the rapid heating of quartz when it enters the furnace, the following techniques have been used for both in situ investigations and investigations of pre-heated quartz: -Heating stage microscopy for in situ observation of heating effects. -Shock heating of quartz in an induction furnace An optical microscope-mounted heating stage was used for in situ observations of the microscopic effects of heating up to 1300 ºC. The effects of heating were observed at different temperature intervals in different samples. The main temperature intervals were below 573 ºC and above 1000 ºC. The cause of the observed effects is interpreted as fluid inclusion decrepitation for T<573 °C and structural implications (micro cracks, grain boundaries and remaining fluid inclusions) at T>1200 °C. Shock heating of the samples was carried out in an induction furnace to 1300 ºC for ten minutes. The effects of the heating were studied with different techniques such as direct observation during and directly after heating, optical microscopy and XRD. Combined transmitted and fluorescence light microscopy shows that the heated quartz show an opening of the grain boundaries and evenly distributed micro cracks that were not present in the unheated reference samples. In some of the heated samples, weak generation of cristobalite was detected by XRD.

In planning activities the aim is to adapt them to changes in conditions, in the needs of the mining industries, and in the public interest; those that contribute most to solving the present problems are emphasized, investigations of less immediate importance being recessed if necessary. The strategic minerals investigations of the bureau afford an example of this change in emphasis to problems of immediate and urgent interest, and during 1941 they comprised, upon the basis of appropriations and expenditures, more than 90 percent of the work of the division.

"Locked cycle testing has become more common practice in the last 10 years. Reasons vary from a desire to avoid pilot plant testing, demand for more variability testing within deposits, and perhaps most importantly, a request from the financial institutions who invariably finance many of the projects. Surprisingly, many people don’t understand locked cycle testing, analysis and balancing. This paper provides a brief overview of locked cycle testing, and then progresses into a more detailed discussion on the analysis and balancing of locked cycle test data. The paper endeavors to educate CMP personnel on how to determine if the test was good and if a reasonable metallurgical projection was made. The paper uses case studies to illustrate the key points.INTRODUCTIONA locked cycle test is a repetitive batch used to simulate a continuous circuit. The basic procedure has a complete batch test performed in the first cycle which is then followed by similar batch tests which have “intermediate” material from the previous cycle added to the appropriate location in the current cycle. These batch tests, or cycles, are continued in this iterative manner for an arbitrary number of cycles. The final products from each cycle, i.e. final concentrate and final tailings, are filtered and thus removed from further processing. At the end of the test, all the products, final and intermediate, are dried, weighed and subjected to chemical analysis. The test is balanced and a metallurgical projection is made.This description can be found in many of our classical textbooks for the mineral engineer 1, 2, 3,4. Unfortunately, none of these textbooks provide any discussion beyond the basic procedure. Often, they provide comments such as in Taggart’ where he states; “It is questionable whether in any case it approximates mill results any more closely than the standard batch test”. It is truly surprising that our classic textbooks promote that locked cycle tests are more art than science, and suggest that they can be of dubious value. None of the textbooks provide meaningful insight or discussion in how to assess:"

In 1986, at the request of the U.S. Bureau of Land Management, the U.S. Bureau of Mines studied the 5-acre Zwagg Island Wilderness Study Area (OR-12-14) in order to evaluate its identified mineral resources. The study area is located in Curry County, OR, immediately west of Brookings. There is no evidence of mining activity on or near Zwagg Island, and no mineral resources were identified. The steep-sided, flat-topped island is normally accessible from the mainland by way of a sandy beach. The island consists mainly of marine sandstone, mudstone, and conglomerate of the Jurassic Dothan Formation. Dothan Formation sandstone has been used locally as construction material. Rock along the coast has also been used for construction of jetties to improve harbor access; additional work has been proposed at the port of Brookings. Although suitable for other construction uses, stone from Zwagg Island would probably not meet physical requirements for jetty stone. Development of a stone resource on the island is unlikely.

This study was conducted using field interview methods to determine the data and information requirements of the underground coal mining industry for improving mine ground control procedures in existing and future mines. The establishment of a central geologic and engineering information resource center with computer searchable abstract files that can locate and supply descriptive and case-study literature addressing specific mining problems is recommended. Batches of red or brown iron ore concentrates, weighing 100 or 200 grams, were reduced with gaseous reductants in a laboratory-sized fluidized-bed reactor. Using a loss of weight technique, the rates of reduction were measured and the effects of temperature, gas consumption, and gas flow rates were determined. Below about 725° C the rate of reduction was controlled by the chemical reaction at the reduction interface having an activation energy of about 10 kcal/g mole. Above about 725° C the rate of reduction appeared to be controlled by the flow of the gaseous reductant and therefore to be directly proportional to the flow rate. Above 725° C, hydrogen reduced the iron ore much faster than carbon monoxide. In gas mixtures, carbon monoxide or nitrogen reduced the hydrogen partial pressure and lowered the reaction rate proportionally. When present in large concentrations, carbon monoxide had a serious inhibiting effect on the reduction rate, probably owing to carburization and sintering of the reduced iron.

New technology projects, particularly those involving first-of-a-kind metallurgical facilities, have a poor track record of achieving their overall financial targets, as they incorporate technological risks in addition to normal project risks. Traditional project management tools focus on risk from concept development through implementation, but typically fail to address the additional risks associated with a new technology project including slow start-up, slow ramp-up, and failure to achieve design performance (quality or throughput). These additional risks affect future cash flows produced by the project and hence its value, typically measured as net present value (NPV) or internal rate of return (IRR). Common accounting practice is to reduce the present value of future cash flows by applying a chosen discount rate, often reflecting the corporation?s cost of capital. This discount rate does not contain an allowance for technological risk, which is often underestimated or misunderstood by both the financial and engineering project managers. This paper will focus on the topic of project risk and new technology, and how these play a role in the probability of achieving the predicted future cash flows. By properly addressing technological risk in the form of risk-weighted returns, marginal projects can be prevented from proceeding, while sound projects can be given the additional time and resources required to achieve the optimal level of front-end-loading (the level which returns the maximum risk-weighted net present value). A simple set of mathematical tools should enable engineers and financial professionals to establish a common level of understanding, thereby leading to a more accurate assessment whether a new technology project is ready to pass through its next level of approvals.

Mining companies and contractors are looking at Wirtgen Surface Miners to provide flexibility at their operations. Wirtgen Surface Miners have been used in the surface mining industry for over fifteen years. The principle behind the Wirtgen Miner is a rotating drum equipped with tungsten carbide tipped cutting tools that excavates and sizes the material. The drum helix augurs the mined material to the center of the drum where it is loaded onto the primary conveyor belt. From there the material is conveyed to the discharge belt and into a truck bed. The cutting drum is located in the center of the machine, near the center of gravity. This permits the machine to cut hard materials as the entire machine weight is converted into cutting forces. Conventional mining practices are process, equipment, and labor intensive. Furthermore, strict environmental requirements pertaining to noise levels, dust pollution, and blasting, near buildings or urban areas, limit the application of conventional methods. Mining/ Construction contractors are using Wirtgen Miners in various limestone excavation projects, such as highway expansion projects, and new roads in housing developments. Mining companies in the industrial minerals and coal industry are using Wirtgen Miners to load directly onto trucks, eliminating the rehandling process and ultimately, primary crushers.

Competition from foreign coal producers could displace as much as 9 Mt/a (10 million stpy) of US coal bound for power plants located near waterways. But, according to a Maryland-based coal consultant, it could be worse. Theoretically, about 52 Mt (57 million st) of domestic coal could be vulnerable to imports by 1990, said Forrest Hill, of Hill & Associates of Annapolis. Hill was speaking to about 150 coal industry executives and experts at the third annual Coal Marketing Strategies Conference in November. Most were at the Denver, CO conference to pick up tips on how to sell coal in an oversupplied market. Imports threaten domestic markets Of the 200 utilities studied by Hill and his staff, 119 are situated near major waterways, he said. However, most of those waterways are not equipped to handle coal from the shipping lanes. Good news for US coal producers. But don&apos;t breathe too easily. Foreign coal still poses a major threat to domestic markets, Hill continued. The strength of the US dollar is an obvious factor. But easy access to US utilities located on waterways combined with low rates for ocean vessels give foreign producers another edge. And foreign mines are typically low cost producers. For example, Hill said Colombia&apos;s huge Cerrejon mine can deliver coal to an Illinois power plant for $47.87/t ($43.39 per st). That compares to $52.85/t ($47.95 per st) from a Kentucky coal mine. Also, Colombian coal producers can generate a cash flow even if the coal sells for $27.50/t ($25 per st). The main competition US producers face from abroad, as Hill sees it, comes from Australia, Canada, Colombia, Poland, South Africa, and, perhaps, from Venezuela. Domestic producers, however, do have some things going for them. Most US coal buyers, Hill said, "are not wild about the idea of buying foreign coal." A 10% price advantage or pressure from public utility commissions would be needed to force some of them to buy foreign coal, he said. Some of the utilities contacted said they would not commit to more than one-third of their stockpiles to a foreign source. Another advantage for US producers is that they have room in their long-term contracts to cut prices and still maintain a profit, Hill said. Railroads, too, have high profit margins written into their rate structures. They are also aware of the import threat. Therefore, they may reduce their rates accordingly to keep their markets. Marketing&apos;s role The energy price shocks of 1972 and 1976 caused major adjustments in world economics. Coal was no exception. High oil prices caused many utilities to look at less expensive coal. As the market improved, coal prices began to respond, said R. Gale Daniel, of Anaconda Minerals Co. But clean air legislation and the strengthening dollar caused havoc in the coal markets, he said. The price of oil crashed, taking with it predictions of record coal use. The result is an oversupply of coal that has led to price cutting. Now, coal marketing must be recognized in its "true leadership role," Daniel said. Keywords: Recycling, Consumer Battery, Fee rates On November 11, 1999, all types of spent consumer batteries are proclaimed to be collected in Taiwan. Under current recycling system, all producers and importers of consumer batteries have to pay a collection/recycling fee to the Recycling Management Fund (RMF) of EPA based upon the rate approved by EPA. Collectors and recycling plants receive subsidies from RMF according to the collected and treated amounts audited by an independent auditing organization. Collected spent consumer batteries only reached 923 tons in 2002 equivalent to 10% of the total quantity sold at the same year. How to increase the collection efficiency of used batteries becomes an important issue for EPA in Taiwan. The objective of this study is to explore current recycling system of spent consumer battery in increasing collection efficiency. Results provide some suggestions to Taiwan EPA regarding recycling route, recycling system, recycling rates, and recycling subsidies.

The critical importance of wire rope integrity to the mining industry has led to the creation of standards for the retirement of wire rope as well as to the development of electromagnetic devices and procedures that would allow wire ropes to be tested against these standards without destroying them, The U.S. Bureau of Mines (USBM) conducted research to evaluate these devices by measuring loss of metallic cross-sectional area and the number of wire breaks, or local faults. The intent was to define the extent to which the technique can identify degradation within the rope, e.g., broken wires, corrosion, and wear. Experimental results showed that instrument sensitivity depended on a number of factors, including rope diameter, placement of sensors, location and characteristics of flaws, rope speed, and tension on the rope. The experiments also demonstrated the importance of having an elongated recorder trace and a properly trained and experienced operator to interpret overall results. A design of a rope and test procedures for use in laboratory evaluation of instruments have been recommended by the American Society for Testing and Materials Committee E07.07.10, Wire Rope Applications. The USBM is working through this committee to develop standards for an electro-magnetic method of wire rope inspection.

Collectorless flotation of sulphide minerals from Lyon Lake ore (Mattabi Mines Ltd.) has been studied both in the laboratory and plant. Among the four major minerals chalcopyrite showed the greatest response to collectorless flotation, followed by galena. Flotation of sphalerite and pyrite was minimal. Collectorless flotation was influenced by pulp potential and pH. Experiments using different gas compositions showed collectorless flotation was promoted when the pulp potentials were greater than about 0 mV (SCE). The study of the effect of depressants showed that SO2 at pH < 8.0 depresses both galena and chalcopyrite but at pH 9.5 flotation of chalcopyrite recovers. Use of NaCN preferentially depressed chalcopyrite compared to galena. Preliminary results from plant tests at Mattabi Mines Ltd. confirm that collectorless flotation occurs and that it is preferential for chalcopyrite and to a lesser extent galena. Practical implications of collectorless flotation in plant practice are discussed. A critical factor will be silver recovery.

British Columbia (BC) is Canada’s most western province. The landscape in BC is amongst the most beautiful and pristine in the world. Along with its beauty come vast mountain ranges, coastal rain forests, swamplands, deep river valleys, and almost every type of landscape imaginable. These all have, or at one time had, logging roads constructed. Some of these areas should never have had roads constructed due to poor drainage and unstable slopes, but some of the roads could be considered engineering masterpieces. To navigate a vehicle the size of an off-road logging truck down the side of a mountain with an 80°h side slope or greater can be a hair-raising experience. Not to mention coastal rains and/or high elevation blizzards. These factors collectively make logging BC’s most hazardous occupation. People are seriously hurt, sometimes even killed everyday.

How much trouble can narrow vertical fissures in bedrock cause on a shallow bedrock site? Enough trouble so end-bearing H-piles range between 8 and 100 feet long in a single pile cap and tiebacks range from 40 to over 100 feet long within a 10 foot wall span. Sloped bedrock surfaces also complicated the tieback installation as the drill casing followed the sloped rock surface while drilling and caused unanticipated bending (and sometimes breaking) of the drill string. Warned via pre-construction rock probes and cores reaching over 80 feet deep at the Winooski, Vermont project site, the design and construction team expected anomalies in pile driving and tieback drilling due to fissures in the bedrock filled with soil and weathered-rock. The degree of abruptness in the variability in the rock surface, and its impact on pile driving and tieback drilling were less predictable. Close monitoring of pile and tieback installation, redesign of pile caps, and lengthened tiebacks overcame these anomalous fissures. This paper shares the lessons learned for overcoming these geologic challenges to benefit designers, contractors bidding the work, and ultimately, the owner, with lower contingencies.
