A Major Hazard Risk Assessment (MHRA) was developed in Australia after a series of mine disasters in the 1990’s. A MHRA is used to help prevent major hazards, i.e. fire, explosion, wind-blast, outbursts, spontaneous combustion, roof instability and chemical and hazardous substances, from injuring miners. A MHRA is a structured process that identifies the characteristics of major hazards, assesses and ranks the risk they present, and evaluates engineering and administrative controls to mitigate them. These controls typically consists of a broad spectrum of prevention, monitoring, first response, and emergency response techniques and helps to move an operation from a reactive to a proactive approach towards safety. This paper documents a MHRA performed at an underground mine where strata instabilities and fire hazards may threaten the condition of its escapeways. The objective of this MHRA is to 1) identify what hazards could affect the egress through the mine’s escapeways, 2) determine what unwanted events pose the greatest threat for the mine, and 3) recommend a plan to prevent or recover from the potential disruption of egress through the escapeway. The plan provides information on the key existing controls that should be monitored and audited, and makes recommendation of new potential controls to further reduce related risks. By documenting the use of MHRA to this specific ground control issue, this paper provides a framework for others to judge the merits of this approach and to help design and perform these activities.

The report provides a detailed analysis of 23 electrical accident countermeasures contemplated for use in the metal/non-metal mining industry. The costs and potential benefits of implementing groups of these countermeasures are estimated, and methods arc outlined by which the operator ran choose countermeasures in an effective manner. The analysis is based largely on an extensive electrical accident event tree which is developed in detail. For those countermeasures which are not currently technically feasible, several prototype devices were developed to help speed development. These included a trolley arc eliminator and a safer mine electricians multimeter.

The National Institute for Occupational Safety and Health (NIOSH) Pittsburgh Research Laboratory (PRL) participated in an experiment to facilitate the remote control of dozers used on coal stockpiles. The experiment consisted of supplementing the normal sensory cues an operator requires to manipulate the dozer from a remote location. NIOSH provided and tested a vision systems intended to give the operator the ability to operate the dozer safely and efficiently from a remote position. This paper highlights the genesis of the experiment, identifies the contributions of Consol Energy and Caterpillar, and details the NIOSH efforts and successful test results. German periodicals for 1917 were not procurable and references to articles in those and in other foreign journals that were not available have been credited to the sources from which they were procured.

Discussions are presented of the three phases of the noise reduction program on flight-type coal conveyors. The three phases are identification of noise sources, evaluation of potential noise reduction concepts, and implementation of noise reduction which is intended to reduce conveyor noise to less than 90 dB(A) in the underground mining environment. The major noise sources are impacts of the chain on the tail roller and on the top and bottom decks. A total noise reduction of 11 dB(A) was achieved by using techniques which are applicable to retrofit. These are a vibration isolated tail roller and an isolated wear strip on the return deck. A theoretical model of isolation provided by the isolated tail roller is developed. Appendices are presented which contain information on other potential concepts and additional data generated.

The Washington National Cathedral is one of the largest masonry structures in the USA, and like many of its European Gothic counterparts, it required nearly a century to construct. The design was altered during this period, resulting in greater loadings than were originally anticipated on the soil beneath the foundation. When signs of continuing differential settlement were observed in the early 1990's, the Cathedral approached the US Bureau of Mines (now the National Institute for Occupational Safety and Health (NIOSH)) for assistance in monitoring the movements of the massive towers and walls. An array of dual-axis tiltmeters was installed about 40m above ground level to measure wall and tower inclination, with the data sent over a dedicated phone line to the NIOSH Pittsburgh Research Laboratory for processing. Mechanical gages and string-potentiometers are also being used to measure motion across cracks and joints, and differential pier settlement is being monitored with optical-level surveying. Data have now been collected for almost 7 years, and they show a long-term settlement trend masked by dramatic diurnal and seasonal thermal movements. The paper discusses the major findings of the study, implications for the future of the Cathedral, and conclusions regarding the use of geotechnical monitoring at major national monuments.

"INTRODUCTION This report describes the results obtained from a diamond-drill exploration project performed by the Bureau of Mines on the nickel deposits of the Yakobi Island, Alaska. The Federal Geological Survey 6/ completed a detailed geologic study and mapped the area during the summer of 1940, and a Bureau of Mines examining engineer 7/ made a preliminary examination of the deposits in June 1941, at which time core drilling was suggested as the logical means of determining the extent and grade of the deposits. Equipment and supplies were unloaded on the beach at Yakobi Island on October 21, 1941. The drills were moved to Bohemia Basin, where actual drilling operations wore begun under the direction of a Bureau of Mines engineer 8/ on November 7, 1941. Two core-drill holes were completed on December 7, 1941. Upon the request of the drilling contractor, operations were recessed owing to severe winter conditions. Work on the project was resumed April 27, 1942, under the direction of another Bureau of Mines engineer 9/ and was completed August 15, 1942. During the 1942 season, 13 core-drill holes were completed, and 10 surface pits and a tunnel were sampled. A total of 5, 191 feet of core-drill hole was drilled in 15 holes during the entire project. The results of drilling and sampling form the basis of this report."

The Bureau of Mines conducted mineral investigations of the Peace Rock and Deadwood River roadless areas in Idaho, in 1992. Literature research was done by the Idaho Geological Survey. The study areas contain black sand placers (thorium, columbium, tantalum, titanium, uranium, and rare earth elements), gold, and molybdenum prospects. Except for unrecorded placer gold, there has been no mineral production from the areas. No mineral resources were delineated, but a multi-million tonne molybdenum resource is in the Big Pine Creek area and a 142 million m' black sand deposit is in the Stolle Meadows, Peace Valley, and Deadwood River areas. The Deadwood mine produced zinc, lead, and silver. There are six mining districts in and adjoining the areas; Warm Lake, Deadwood, Garden Valley/Lowman, Banner, Summit Flat, and Grimes Pass. Few claims, mostly placer, were active in the study area in 1992. The study areas are underlain by granitic rocks of the Idaho batholith. Most of the deposits are alluvial accumulations of heavy minerals weathered out of the Idaho batholith. Stockwork and narrow veins along shears account for the base and precious metal concentrations. Eleven prospects were examined. Workings consist of drill holes, pits and adits. Geothermal resources (hot springs) occur mainly along the Middle Fork Payette River; Vulcan Hot Springs is a designated Known Geothermal Resource Area. Sand and gravel resources are of local interest.

NO MORE important question has come before the coal-mining industry in many places, especially in the anthracite region of Pennsylvania, than prevention of stream pollution by mine drainage. Available information on acid mine drainage in the anthracite region of Pennsylvania indicates that a pollution problem must be solved in any program of anthracite mine drainage. The major problem concerns satisfactory disposal of the daily average mine-water discharge of 327,000 gallons per minute (g. p. m.), or 472 million gallons per day (g. p. d.), containing a free-acid load of 431 tons or a total acid load of 934 tons a clay as H2S04. This report indicates the scope of the problem and gives some suggestions concerning its solution.

This report presents the results of laboratory mineral evaluations conducted by the federal Bureau of Mines under the joint sponsorship of the U.S. Atomic Energy Commission (AEC) and the Bureau a! Ships, U. S. Department of the Navy. The major purpose of this investigation was to examine and evaluate zirconium minerals representing sizeable deposits in an effort to locate com¬mercial sources of high-hafnium ores. Although determination of the hafnium content was the principal objective, each of the zirconium minerals also was analyzed for combined zirconium-hafnium oxide, silica, uranium, thorium, yttrium, and cerium. The general laboratory procedure involved microscopic examination of the test samples, concentration of the hafnium-bearing zirconium mineral by standard laboratory techniques, and analysis of the derived concentrate by quantitative microscopic, chemical, and X-ray spectrographic methods. Of the several hundred samples tested under this program, 147 are discussed in this report, including 94 from domestic occurrences and 53 from widespread foreign sources.

This bulletin is a revision of Bulletin 63 and is published by the Bureau of Mines in order that purchasers of coal for Government, State, municipal, or private use may be informed regarding advances or refinements in sampling coal deliveries and preparing specifications for the purchase of coal. The purchase of coal by the Government under specifications depending on the heating value of the coal, its content of ash and of moisture, and other considerations, rather than upon the reputation or trade name of the coal, was based on the fuel investigations begun by the Technologic Branch of the United States Geological Survey in 1904. The plan was first adopted by the Treasury Department in 1906. Since then the plan, variously modified in form, but the same in principle, has been gradually adopted by other departments until, at present, of the coal used by the Government, the total value of which approximates $8,000,000 annually, more than half is purchased under specifications. The Government publications dealing with the adoption by the Government of the specification plan, the number of contracts awarded on that basis, and the quality of the coal delivered under such contracts in the several fiscal years covered by the reports are given in publioations a listed at the end of this bulletin. Under the authority of acts of Congress making appropriations for analyzing and testing fuels belonging to or for the use of the United States Government, a laboratory is maintained at the headquarters of the Bureau of Mines, Washington, D. C., where samples representing deliveries of coal purchased under specifications for Government use are analyzed and tested. At this laboratory more than 1,200 samples have been analyzed and tested in one month. With the complete equipment and the efficient force employed, the analytical work has been so perfected that the analyses and heating-value tests of the samples received are as accurate as may be reasonably expected in laboratory work of this nature. One of the serious drawbacks to the general adoption of the specification method for the purchase of coal is the difficulty of obtaining at reasonable cost samples of coal that can be considered fairly representative of the commercial product delivered in wagons, railroad cars, or ships. Therefore the method of taking and preparing samples for shipment to the laboratory has been given fully as much care as the making of the analyses and tests, and a general plan of collecting samples fairly representative of the delivered coal has been evolved through various modifications and improvements based on experience and increased knowledge of the physical and chemical characteristics of the various coals that are purchased by the Government. The method that is in general use by the Government is described in the following pages. In connection with studies of the coal deposits of the country, of the best methods of preventing waste in mining, and of increasing efficiency in the utilization of coal belonging to or for the use of the Government, geologists and engineers of the United States Geological Survey and of the Bureau of Mines have visited more than 2,500 coal mines scattered through all of the coal-producing States and Territories. From each mine 2 to 8 or more samples were taken, the number depending upon the size of the mine and its output. The analyses and descriptions of the samples collected up to the end of the fiscal year 1913 are published in Bureau of Mines bulletins 22 a and 85,b and those of samples collected in the fiscal years 1914 and 1915 are presented in a bulletin that will soon be published by the bureau. A study of the analyses of these samples and of the samples taken from cars shipped from a number of the same mines shows that the mine samples are as a rule of higher grade than the average commercial shipments, particularly with respect to containing a lower percentage of ash and having a correspondingly higher heating value. This difference is due to the fact that the Government inspector, proceeding on the basis of what can be done by a good miner, usually removes more of the partings of bone, slate, and other extraneous matter from his mine samples than the average miner does in ordinary practice, the aim of the miner being to get the maximum number of tons past the tipple inspection, as his earnings are based on his output. The average miner, therefore, does not always take as much care as he might in rejecting the impurities. "Methods and costs of rock-dusting bituminous coal mines have been discussed in various publications by the United States Bureau of Mines and others.3 This paper discusses the present methods and cost of dusting at the Swastika mine of the St. Louis, Rocky Mountain and Pacific Co., Raton, N. M.In the Swastika mine the recommendations of the United States Bureau of Mines with regard to rock-dusting are followed at least as well as in any mine in the States of Wyoming, Colorado, and New Mexico. This mine is located at Swastika, N. M., approximately 4 miles west of the town of Raton and is in the Raton coal seam. The mine is rated as gassy and produces approximately 1,000 tons of coal per day.A mining system known as the ""room-and-pillar with cut-offs"" is used which results in recovery of nearly 100 per cent of the coal. The mine employs approximately 250 men.Ventilation is maintained by a Sirocco fan so arranged that the air current may be reversed and exhausting approximately 130,000 cubic feet of air per minute at 3 inches water gauge. This fan is supplemented by two auxiliary fans for emergency use. Three main entries penetrate the hill in a straight line for approximately 8,000 feet. The haulage is through the center entry, which is also used as the intake in conjunction with a two-compartment concrete-lined shaft located at the end of the entries. The main return for 3,000 feet inby the fan is 12 feet wide and 7 feet high. For this distance the ribs have been reinforced by a wall made of coke-breeze blocks plastered over with cement. Solid concrete blocks are used for the top row of blocks of the wall. Steel 10-inch I-beams spaced 40 inches apart center to center are supported on these solid blocks and thus support the roof. The next 1,000 feet inby has the same area, but the cement blocks extend to the roof, as no crosspieces are needed. The airways are kept clean and well rock-dusted.A rather unique system is used to handle gas feeders along the intake and haulage entries. There these feeders occur in the roof or where gas is apt to collect in high points along the haulage, tin funnels as large as 3 feet in diameter are placed near the roof. From these a 2-inch or other suitable-sized pipe is run into the return through the wall of the nearest break-through. A valve on this pipe line controls the amount of air pulled. These devices keep the high places clear of any gas, even when strong feeders are encountered.Fifteen-ton electric trolley locomotives are used on the main haulage and approximately 30 roller-bearing oars, each having a capacity of approximately 3,000 pounds of coal, compose a trip. All loaded tripe carry steel drags on the last car, and automatic derails are located at advantageous points throughout the mine.All loaded trips are thoroughly sprinkled on the sectional partings and at the main parting. The track is sprinkled every idle day, or if needed, every day; from a large tank mounted on a truck hauled by a motor. Neither nozzle nor spray is used, as these were unsatisfactory. The water leaves the tank. through a 11/2-inch valve and is permitted to flow on the road through a horizontal 11/2-inch T. Water is obtained from filling stations located throughout the mine. It is stated by the officials that the use of this method of sprinkling permits the entire haulage roadbed to be wetted down in two hours."

The Coal Mine Roof Rating (CMRR) was developed 10 years ago to fill the gap between geologic characterization and engineering design. It combines many years of geologic studies in underground coal mines with world-wide experience with rock mass classification systems. Like other classification systems, the CMRR begins with the premise that the structural competence of mine roof rock is determined mainly by the discontinuities that weaken the rock fabric. However, the CMRR is specifically designed for bedded coal measure rock. Since its introduction, the CMRR has been incorporated into many aspects of mine planning, including longwall pillar design, roof support selection, feasibility studies, extended-cut evaluation, and others. It has also become truly inter-national, with involvement in mine designs and funded research projects in South Africa, Canada, and Australia. This paper discusses the sources used in developing the CMRR, describes the CMRR data collection and calculation procedures, and briefly presents a number of practical mining applications in which the CMRR has played a prominent role.

The National Institute for Occupational Safety and Health (NIOSH) has been conducting re-search to determine the influence of mining machine mounted water sprays on airflows and methane concentrations at the face when blowing ventilation systems are used. Tests were conducted in a full-scale ventilation gallery. Airflow speeds and directions were measured at several locations near the face with ultrasonic anemometers. Methane was released from the face and concentrations were measured in the entry at locations above the mining machine using fixed point methanometers. Changes in airflow speed, direction, and methane concentrations were correlated with water spray operations. The test results using different spray arrangements and water pressures showed that operation of the machine-mounted sprayers can improve face ventilation effectiveness by increasing the velocity of airflow moving toward and away from the face. The improved ventilation resulted in reduced methane levels near the face.

The Bureau of Mines conducted electric furnace smelting tests on sintered flotation concentrate and siliceous log-washed are. The flotation concentrate was derived from the Maggie Canyon deposit of the Artillery Mountains area, Mohave County, Ariz., and contained, before sintering, 35.6 percent Mil and 19.4 percent Si02 + A1203. The log-washed ore was from the McGregor deposit of the Artillery Mountains area, Mohave County, Ariz., and contained 31.2 to 33.6 percent Mil, 19.5 to 27.6 percent Si02 + A1203, and 2.5 percent Pb. This work was undertaken to determine the most effective smelting technique for producing standard products from such nonspecification ore and concentrates. The objective was to collect data, using a small furnace, which would be reasonably accurate and permit extrapolation to large-scale practice for comparing with results on specification-grade are.

Over the past century coal miner safety and health has seen tremendous improvements, the fatality and injury rates continue to decrease while productivity continues to increase. Many of the hazards that plagued miners in the past, such as coal bumps, methane and coal dust explosions, ground fall accidents and health issues have been significantly reduced. The contribution of NIOSH research includes products for prevention and survival of mine fires, methane control measures, design procedure for underground coal mines, methods for excavation surface controls, methods and procedures for blasting, laser usage in underground mines and prevention of electrocution from overhead power lines that have reduced accidents and injuries in underground coal mines. Health research has produced products such as the personal dust monitor, noise abating technologies and ergonomic solutions for equipment operators. Research priorities at NIOSH are set by considering surveillance statistics, stakeholder inputs and loss control principles. Future research in coal mining is directed towards respiratory diseases, noise induced hearing loss, repetitive musculoskeletal injuries, traumatic injuries, falls of ground and mine disasters. The recent spate of accidents in coal mines resulted in the Miner Act of 2006 which includes a specific role for NIOSH in future mine safety research and development. The mine safety achievements in the USA reflect the commitment of industry, labour, government and research organizations to improving the safety of the mine worker.

This information circular documents the status of a laser-based guidance system to track and control the tramming maneuvers of a continuous mining machine. The work is part of a multiple project effort at the U.S. Bureau of Mines to develop technology for computer-assisted mining systems. The system includes stationary offboard laser-scanning sensors that report the angular coordinates of retroreflective targets on the mining machine. A microcomputer system processes the angular coordinates, determines position and heading, handles requests for data, and controls tram maneuvers of a continuous mining machine. This report describes the hardware for the sensor and computer systems and the software algorithms for continuous miner position and heading determination, closed-loop tramming, and data request handling. Improvements made to the system since previous documentation include the rewriting of the software on a stand-alone real-time microcomputer system, the implementation of real-time programming techniques, the use of a new version of the laser sensor, and the design and development of explosion-proof enclosures for the lasers and corresponding power supply.

This Bureau of Mines report presents expected levels of air pollutants in the face areas of oil shale mines, based upon data collected by the authors and previous investigators. Ventilation requirements to maintain these pollutant levels below their threshold limit values and Federal and local mine air quality standards are discussed. Two practical face ventilation systems are discussed in terms of actual in-mine test experience.
