Investigation of the costs of industrial accidents dates back to the 1920s. The Bureau of Mines was one of the pioneers in accident cost investigation with numerous studies published between 1928 and 1933. The bulk of these early investigations dealt only with costs which appeared as payouts on company books; that is, the direct cost of insurance premiums and the costs of medical expenses which resulted from industrial accidents. These early investigations, especially the Bureau's study of accident costs in Pennsylvania and Indiana coal mines provided the first real evidence that promotion of industrial safety pays back in dollars as well as in human life and wellbeing. In the 30s and 40s, investigations began to accumulate evidence that industrial accidents impacted more than just the direct operating expenses of a company (i.e., higher insurance premiums). The work of Heinrich pioneered these investigations, and produced strong evidence that the un¬insured costs of accidents far outweighed those costs covered by insurance. Heinrich developed and published a ratio of insured to uninsured costs of 1.4. Despite his cautionary statements about blanket application of this ratio it is used even today to estimate total costs of accidents. Simonds, in the late 40s and early 50s, extended the definitions of uninsured or indirect accident costs and developed what is now regarded as the principal method for estimating the total economic impact of industrial accidents. Simonds' method served as the basis for development of the logic of the Accident Cost Indicator Model (ACIM). However, several departures from Simonds' method were made to satisfy certain requirements imposed by the Bureau of Mines, and because information gathered during the course of our investigation justified a departure. "GENERAL PROJECT DATAThe Sparvo Tunnel is part of the “Variante di Valico” project, a new 65km (40mi) highway link between Bologna and Florence, designed to renew the existing A1 route which connects Bologna and Florence. It is composed of 2 x 2,5km (1,55mi) twin tunnels excavated inside a complex geological context, characterized by the presence of a series of surface landslides and methane gas.The inclination is 2,6 % and the minimum radius is 1.400m (4.600ft). The two tunnel tubes were excavated by a 15,6m (51ft) diameter EPB-TBM from summer 2011 to summer 2013. The internal diameter of the concrete lining is 13,6m (44,5ft), made up of precast concrete segments 2m (6,5ft) long and 70cm (28”) thick, being the largest and heaviest segments ever produced. Excavation went smooth apart from initial problems concerning clogging problems and a main bearing damage of the TBM, which are discussed in a further paper during the congress. No structural damage was observed until the end of the TBM drives.SITE OBSERVATIONS DURING THE POST-EXCAVATION PHASEThe first evidence of the stress state of the segmental lining appeared on November 20, 2013 inside the northern tube (1st drive). Ring No. 884 showed obvious signs of overstress, resulting in spalling and crushing of the concrete, and the subsequent bending of the reinforcement, as visible in figure 1.At this time the tunnel invert was already partially backfilled, and construction traffic was running through the tunnel. First safety measures included closing of the tunnel, and initial measurements of the displacements."

Several static tests estimate the capacity of mine waste to produce and neutralize acid and are used as tools for predicting metal-mine waste drainage quality. Various methods and their accuracies are addressed. More accurate quantification of these capacities, as well as their availability and reactivity, may be necessary to predict drainage quality from some mine wastes. Chemical analyses, x-ray diffraction, optical microscopy, scanning electron microscopy, and electron beam analyses are among the techniques commercially available to more accurately quantify these capacities and provide insight into their availability and reactivity.

The western united States is not usually noted for the production of fluorspar; however, many significant fluorspar districts do occur in the Rocky Mountain region and constitute a resource of sizable dimension and major potential. This is particularly true within the area of the West served by the rail lines of the Union Pacific Railroad (see Figure 1). Fluorspar deposits near the Union Pacific have produced over 1.3 million tons of finished fluorspar products from 2.3 million tons of ore. Approximately 80% of this production was of acid-grade and 20% of metallurgical-grade fluorspar. Production of ceramic-grade was negligible. Through the years substantial production has come from eight mining districts. Five of these have produced metallurgical-grade and three acid-grade. The best-known fluorspar producing area in the West is the Jamestown Mining District. It was one of the first producers of fluorspar in the western United States and is estimated to have produced over 500,000 tons of finished fluorspar from over a million tons of ore. The district encompasses approximately 36 square miles in central Boulder County, Colorado, about 9 miles northwest of the City of Boulder. Characterization of ore and waste-rock material using automated quantitative micro-mineralogical techniques (e.g., QEMSCAN® and MLA) has the potential to complement traditional acid-base accounting and humidity cell techniques when predicting acid generation and metal release. These characterization techniques, which most commonly are used for metallurgical, mineral-processing, and geometallurgical applications, can be broadly applied throughout the mine-life cycle to include numerous environmental applications. Critical insights into mineral liberation, mineral associations, particle size, particle texture, and mineralogical residence phase(s) of environmentally important elements can be used to anticipate potential environmental challenges. Resources spent on initial characterization result in lower uncertainties of potential environmental impacts and possible cost savings associated with remediation and closure. Examples illustrate mineralogical and textural characterization of fluvial tailings material from the upper Arkansas River in Colorado.

Property sales and acquisition are prominent features of today's coal industry. Before selling or buying a site, a company must know the property's economic net worth. Any evaluation of economic net worth would be inadequate if determinations of a property's environmental liabilities and costs were omitted. Short-term and long-term costs can result from liabilities related to reclamation, water treatment, subsidence, and hazardous waste disposal. A structured, interdisciplinary approach can aid in the delineation, summation, and scheduling of these costs and liabilities.

The Nchanga Cu-Co ore deposit of the Zambian Copperbelt, is hosted in Neoproterozoic siliciclastic-carbonate sedimentary rocks, and it consists of two orebodies: the Lower Orebody (shale) and the Upper Orebody (arkose). Refractory Cu ores present in the Upper Orebody comprise 150 Mt with average 0.87% Cu, and are characterized by inadequate recovery of copper by leaching. This materials characterization study, conducted on refractory ore samples from the Upper Orebody using optical microscopy, QEMSCAN®, and microprobe, has shown that copper occurs mainly in biotite, phlogopite, malachite, pseudomalachite and goethite. QEMSCAN® has quantified the abundance of each phase and total Cu deportment in each phase.

The stockpiling and reclaiming of bulk materials can be a challenging task for engineers seeking the optimum solution for plant needs, reliability, and at lowest unit cost. The objective of this paper will be to expose advantages, disadvantages and limitations of many storage and reclaiming systems to the responsible engineer and management. The paper will emphasize the practical aspect of stockpiling and reclaiming methods. Many valuable and interesting papers are available dealing with the theoretical viewpoint and application of a particular manufacturer's machine. Several of these are mentioned in the bibliography. As many practical parameters as possible will be given. After twenty years in the field of designing and selling bulk materials handling systems, the needs of the user and the plant are recognized and reasonably understood. This paper cannot give solutions to all problems In this vast field but will stress practical applications and examples of various installations. The evolution of advancements in materials handling is based upon not only technological innovations, but learning from many experiences of others and utilizing them as stepping stones to improved installations.

Ten years ago, only 3% of Consol's underground coal production resulted from using the longwall mining method. Today, that figure has grown to 45% and has helped the Pittsburgh-based concern fuel an upswing in productivity. The company reported that productivity at its underground mines, excluding labor strike periods, increased 24% between 1978 and 1981. Last year, an additional 8% increase was achieved. Efforts such as improved safety, training, engineering, and maintenance programs as well as the closing of unproductive mines has contributed to these gains, according to Joseph M. Richards, Consol's executive vice president of operations. But, Richards adds, a great part of the credit goes to longwall mining. What is Longwall Mining? To fully appreciate the advantages of longwall, one needs an understanding of how mining is done by other methods. For instance, in both conventional and continuous mining, rooms or tunnels are dug in the coal seam. Pillars of coal are left standing to support the roof as mining moves forward in the seam. In conventional mining, a slot is cut in the base of the seam and holes for blasting are drilled. After blasting, the coal is loaded. A more efficient method - continuous mining - uses a machine that combines cutting and loading. Longwall mining, by contrast, works backwards from the end of a panel. Two parallel sets of tunnels (development panels) are cut into a seam with continuous mining machines, then connected with a set of crosscuts at the far end. Min¬ing then proceeds back toward the entrance. Longwall mining employs some very productive machines. Large drum shearers, for example, run on a track along the length of the coal face for some 150-180 m (500-600 ft). The shearers cut the coal which falls onto a conveyor system or pan that also runs the entire length of the face. The longwall crew works under a roof of heavy steel supports called shields. As the coal is removed, shearers, pans, and shields are moved forward by hydraulic rams. The roof is allowed to collapse behind the shields. In addition to increasing production, longwall mining is considered to be safer because the miners work under strong steel shields. New designs have made them more rugged and maintenance free, as well as more stable on soft mine floors. Early roof supports, called chocks, had gaps in the roof canopies. The new roof shields now cover the entire working area and are better able to prevent roof material from falling into the working area, thereby improving safety and productivity. The Hong Gai coal field in northeast Vietnam has been the main coal mining area of Vietnam for more than 150 years. Anthracite coal is produced from several open pit and underground mines situated close to the UNESCO world natural heritage site Ha Long Bay. The mining-induced environmental impacts such as dust emissions, acid mine drainage, and visual impairments due to waste dumps resulted in severe conflicts of interests regarding future land use and the protection of the world natural heritage site Ha Long Bay. Within the RAME project (Research Association Mining and Environment in Vietnam), Brenk Systemplanung is contributing to an improved environmental and sustainable planning in Vietnam's mining industry and the reduction of environmental impacts on Ha Long Bay by developing suitable and sustainable technology and planning measures (e.g. for dump construction/rehabilitation, predictive modelling, and/or monitoring) for Vietnam's leading coal producer.

The 26th international APCOM symposium, hosted by The Pennsylvania State University, was held Sept. 16 to 20, 1996 in University Park, PA. Conducted about every 18 months throughout the world, APCOM symposia focus on the Application of Computers and Operations Research in the Mineral Industry. The APCOM International Council is the governing body of the symposia and determines the symposium’s locations and sponsorships. This group is composed of the nine permanent (voting) members. They include the University of Arizona, Colorado School of Mines, Pennsylvania State University, Society for Mining, Metallurgy and Exploration, Inc., Canadian Institute of Mining, Metallurgy and Petroleum, South African Institute of Mining and Metallurgy, Technical University of Berlin, Institution of Mining and Metallurgy and Australasian Institute of Mining and Metallurgy. The three nonvoting members are Moscow Mining Institute, China Institute of Mining and Rudarski Mining Institute of Slovenia.

The Mineral & Metallurgical Processing Division (MPD) of the Society for Mining, Metallurgy & Exploration held the 12th MPD Student Poster Contest on March 2, 2022, during the MINEXCHANGE 2022 SME Annual Conference & Expo. The event showcased the work of 32 graduate and undergraduate students from U.S. and international institutions. The contest began in January when the students submitted one‐page abstracts for judging. In February, they prepared and presented posters that explained their research, and each student had five minutes to explain their research to a panel of judges during the conference. Recognizing the students’ efforts in working hard on their research topics and setting aside time to complete the necessary steps to enter the poster contest, the MPD provides monetary rewards to all participants. For graduate students, first place receives $1,000, second place $500, and third through seventh places $200 each, and for undergraduate students, first place receives $1,000 and second place $500. The main authors of nonplacing entries receive $125 each. Participation in the contest includes a free ticket to the MPD Luncheon, where the winners are announced.

A tower-like magnetic system comprising eight permanent magnets was installed at the center of a flotation column to separate chalcopyrite and pyrrhotite minerals. A cyclonic flotation was installed within the underpart of the column to separate the pyrrhotite from the gangues. A three-product magnetic flotation column was fabricated to create a magnetic field and a cyclonic zone. The products were Cu concentrates, pyrrhotite concentrates and tailings. The theoretical magnetic field strengths and the magnetic forces were calculated. The results of the flotation experiments showed that the Cu grade increased from 14.20% without the magnetic field to 26.40% in the presence of the magnetic field and that the S grade in the pyrrhotite concentrates increased in the presence of cyclonic flotation. The separation efficiency of this magnetic flotation column was evident. Therefore, this three-product magnetic flotation column may have a potential application, due to its high separation efficiency.

With the increasing need to exercise better control over mill circuits has come a requirement to develop rapid, accurate means of measuring circuit parameters. One of the most important parameters is the assay of the material in the circuit (or about to be introduced to the circuit). To date, most on-stream analysis has been performed using X-ray fluorescence equipment. Neutron activation offers an alternative method in certain circumstances, and one which has several interesting advantages. Recognizing the need for a simple, safe neutron activation unit suitable for on-stream analysis in the mineral industries, Atomic Energy of Canada Limited has designed, built and tested the SHERLOCK system. One prototype (SHERLOCK I) is located in Ottawa and is now used primarily in research. A second prototype (SHERLOCK III) is located in Vancouver at the laboratories of B.C. Research. This unit is being used for some development research and for evaluating the unit in specific applications. Most of the work completed to date has been aimed at proving the technique and in developing some of the parameters needed to design systems for a milling plant. Funding for the work has been through research grants and through sponsor¬ship by interested companies.

The shear strength properties of generic cement foam material and cement foam/concrete interfaces were examined at three testing scales. In laboratory direct shear tests on specimens with average shearing area of 0.006 m2 (10 in.2), the cement foam material had average peak and residual shear strengths of 0.88 MPa (128 psi) and 0.39 MPa (56 psi), respectively. Depending on the concrete strength, the cement foam/concrete interfaces had peak and residual shear strengths ranging from 0.71 to 0.81 MPa (103 to 117 psi) and from 0.27 to 0.29 MPa (39 to 42 psi), respectively. In laboratory direct double shear tests on specimens with average shearing area of 0.084 m2 (130 in.2), the cement foam/concrete interfaces had average peak and residual shear strengths of 0.17 to 0.30 MPa (25 to 44 psi) and of 0.11 to 0.23 MPa (16 to 33 psi), respectively. In large shear tests of simulated cement foam seals with the Mine Roof Simulator (MRS) test frame, cement foam/concrete interfaces with shearing area of 1.486 m2 (2,304 in.2) had average shear strengths of about 0.08 and 0.16 MPa (11 and 23 psi) along the roof and floor interfaces, respectively. The average shear strength through the cement foam material was 0.38 MPa (55.3 psi). Finite element analysis of the large seal tests and the back analysis of the shear strength of full-scale cement foam seals conducted at the NIOSH Lake Lynn Laboratory confirmed shear strength of about 0.14 MPa (20 psi) along the seal/rock interfaces. The study concluded that the structural analysis of cement foam seals should be based on their residual shear strength determined from small laboratoryscale tests of the cement foam/concrete or rock interface and not be based on their peak strength.

"Frac sand is industrial silica sand much like glass, foundry, filtration and blast sand. The sand must meet certain specifications set forth by the industry that consumes the product. The production and consumption of frac sand has experienced record growth in the last 10 years because of new oil and gas drilling, new horizontal well drilling, multi-stage fracturing and the stimulation of older wells that have little or no production. Frac sand is needed to “prop open” the fractures in the rock reservoirs to allow oil and gas to flow out more freely.According to the U.S. Geological Survey (USGS), the estimated production of frac sand in 2015 was 71 percent of the total production of industrial silica sand or 67.4 Mt (74.3 million st). This is a decrease of about 12 percent from 2014. However, the silica sand industry and other industry sources estimate only 50 Mt (55 million st) of frac sand were produced in 2015. In recent years both the USGS and industry studies have been very close in estimating the production of frac sand but with the recent downturn in frac sand production, perhaps, reporting to the USGS was not as accurate. These discrepancies may be that the USGS statistics are based on voluntary reporting by the industry and some companies report their approximate production, others may not. It is always difficult to obtain accurate production figures of any industrial mineral or construction aggregate operation as most companies tend to keep those figures somewhat confidential. It is certainly accurate to report that the production of frac sand had decreased since the last quarter of 2014 and through 2015. The oversupply of oil and natural gas has led to this decline in frac sand usage. Oversupply has idled drilling rigs and well completion activities. World Oil Magazine reports that 2015 was the worst year for rigs in a quarter century. The only wells being completed are those that have contract agreements to do so, others have been capped. Very few new wells are being drilled. Since about 98 percent of well completion is sand and water, the sand supply has caught up with the demand. There are some bright spots in the “oil patch” as the Permian Basin in Texas and the Woodford Shale in Oklahoma have kept pace somewhat with drilling and completion. It has been reported that the frac sand producers in the Brady-Voca, TX area are producing to nearly capacity. The northern frac sand producers like those in Wisconsin, Illinois and Minnesota are feeling the downturn because of the cost of shipping sand to these moderately active basins and plays. The “rule of thumb” is that freight is 2/3 and sand is 1/3 the cost."

This paper will review several projects completed during the period from 1976 through 1984 which have been carried out utilizing big hole drilling concepts. The projects discussed will be from various industries to demonstrate the extent that big hole drilling methods are being used along with the current state of development. There will be a discussion of projects planned for the future where big hole drilling methods are the only technology being considered. Also included in the discussions will be an overview of the most recently completed heavy oil recovery project. During the next decade, big hole drilling methods will be used more and more in all industries where underground access is required. The safety, speed and economics associated with shaft sinking using drilling methods are becoming more and more recognized and the technology is rapidly becoming an accepted shaft sinking method.

"Analysis of Longwall Pillar Stability (ALPS) and Analysis of Retreat Mine Pillar Stability (ARMPS) treat the pillar as a passive structure that is designed to carry overburden dead-weight. This dead-weight is calculated by simple rules based on the geometry of the mining, such as “tributary area theory,” “pressure arch theory,” and “abutment angle theory.” Although the pressure arch loading approach indirectly accounts for the generally stiffer overburden response of narrow and deep panels, it does not include the effect of specific geology of the overburden in load calculations. The relationship between successful pillar layouts and overburden geology can be incorporated into the load calculation by using the Ground Response Curve (GRC) approach.This paper introduces the GRC modeling methodology to investigate the effect of overburden geology and excavation geometry on load transfer mechanisms using seven field measurement case studies from four U.S. mines. It was also shown that the modeling methodology used to derive GRC in this study approximates mining-induced stresses and deformations within 5% of the values measured in the field.INTRODUCTIONFrith and Reed (2017) stated that current state-of-the-art pillar design methods ignore the overburden mechanics and use estimated dead-weight of overburden to compute pillar sizes. In the United States and around the world, overburden loading is typically estimated by simple geometric rules, generally with the tributary area theory (TAT). The specific overburden mechanics, structural competence of the overburden strata, geology, in situ stresses, and overburden/pillar interactions are typically ignored. However, these important mechanical responses affect the stability of the mine pillars and openings; therefore, they also affect the safety of the mine workers.Research conducted by the National Institute for Occupational Safety and Health (NIOSH) after the Crandall Canyon mine disaster (Esterhuizen, Mark, and Murphy, 2010) showed that small panel width-to-depth ratios and stiff-strong overburden result in a reduction of the observed pillar loads, much smaller than the TAT estimate. In fact, as a response to the Crandall Canyon disaster, Mark (2010) implemented a pressure arch loading approach into the latest ARMPS program to recognize the inherently greater stability of narrow panels at depth. The pressure arch approach is a first logical step to introducing non-tributary area loading into the practical ARMPS program. Although the pressure arch loading indirectly accounts for the generally stiffer overburden response of narrow and deep panels, it does not include the effect of specific geology and mechanical response of overburden in the load calculations."

The West Elk Mine is located near Somerset, Colorado approximately 70 miles southeast of Grand Junction, Colorado and 72 miles southwest of Glenwood Springs, Colorado on state highway 133 (Figure 1). The West Elk Mine was opened January 20, 1982 as the Mount Gunnison #1 Mine operated by the West Elk Coal Company, a division of ARCO Coal. In the early 1990?s ARCO changed the mine name to West Elk Mine and the operating company was renamed Mountain Coal Company. Arch Coal, Inc. purchased the West Elk Mine in 1998. West Elk Mine has produced coal from three different seams. Coal was first mined from the F Seam, the upper most seam in the stratigraphic sequence, from 1982 through 1991. Coal was produced from the B Seam, the lower most seam, starting with continuous miners in 1991 and transitioning to longwall mining in 1992and continued on through October 2008. The development of the E Seam, which is the middle seam in the stratigraphic column, started in 2004 with longwall production commencing in 2008. The ventilation conversion from exhausting to forcing for the E Seam portion of the West Elk Mine was implemented in late October 2009.
