This paper reports on the implementation of the critical control concept to manage material risks in the mining and metals industries. The critical control management (CCM) approach assumes it is better to identify weaknesses in risk controls before an incident happens as opposed to discovering weaknesses from an investigation after an incident occurs. The first is sometimes referred to as active monitoring and the second as reactive monitoring (Health and Safety Executive and Chemical Industries Association, 2006). Both are needed. However, whilst methods for both active and reactive monitoring of risk controls are well established in many high hazard industries, such as oil and gas, petroleum and chemical industries, until the publication in 2015 of the International Council on Mining and Metals (ICMM) guides, (Good Practice Guide and the subsequent Implementation Guide) (ICMM, 2015), there was no specific mining and metals industry guidance on critical control management. This paper reports on progress in implementing the ICMM guidance drawing on two main sources. First, a benchmarking survey of a representative sample of coal and metalliferous mines, both underground and open cut/pit), sponsored jointly by the Queensland Resources Council and the Queensland Commissioner for Resources Safety and Health in the context of a recommendation that the Queensland mining industry adopts high reliability organisation (HRO) principals (Queensland Resources Council and Commissioner for Resources Safety and Health, 2022). Second, it draws on Noetic’s experience of implementing critical control management in mining and other high hazard industries in Australia, Africa, China and North America (Wilkinson, 2018). The paper describes good practices found and where significant improvements can be made, providing specific and actionable examples of both. From this it also identifies how future guidance on this topic could be improved.

The oil crisis had repercussions throughout the world and most importantly on mining, where the drive to reduce the use of liquid fossil fuel led directly to the appearance and subsequent growth in the design of mobile crushers in late 1970s early 1980s, which heralded a new era in both quarrying and mining. Even though mobile crushing has now become common, even preferred in the quarrying industry, this has not been the case in mining. This paper looks at why, in spite of the initial optimism, this technology has yet to gain full acceptance in the mining industry. The paper examines some of the problems that have been encountered in the past and offers suggestions as to how they can be avoided by applying the appropriate technology. It then takes a look at the successful application of the semi-mobile in-pit crushing system used by Italian Thai Development Company (ITD), on the Mae Moh Phase 5 contract and examines how the techniques used successfully there could be applied to the industry at large.

The Vancouver Island Highway Project (VIHP) was a $CAD 1.2 billion expenditure by the Province of British Columbia to improve travel safety and relieve traffic congestion between Victoria and Campbell River on Vancouver Island. In the section of highway between the Tsolum River and Oyster River, on the Inland Island Highway portion of the VIHP, over half of the rock to be cut and filled was found to be net acid generating. The Tsolum River has been impacted by metal leaching and acid rock drainage (ML/ARD) for decades from an upstream closed minesite. Geochemical testwork indicated: over half of the rock along this section of highway was potentially net acid generating based on a site-specific SNPR (=NP/SAP) criterion of 1.5; some core samples were already acidic; pyrite occurred in disseminated framboidal and gel forms; the rate of acid generation was not high; measured neutralisation potential (NP) provided some lag time for the onset of net acidity; and non-carbonate neutralising minerals prevented low pH (These predictions led to ML/ARD controls involving: realignment of the highway; elevation control to minimise the volume of disturbed rock; encapsulation of blasted-rock fills within the highway, surrounded by low-permeability till and covered with asphalt; and diversion of water away from cut faces. Internal pipes drained any water reaching the fills into external ditches and ponds, where the water could be monitored and, if necessary, treated. Water from the cuts was also captured in ditches and carried to monitored ponds. Elevated aqueous sulfate concentrations in several ponds confirmed that sulfide minerals were oxidising, but the lack of both acidic pH and increasing trends in concentrations demonstrated that ML/ARD was controlled. Contingency plans for any pond that may become acidic include water treatment, which predictive testwork has shown would not require substantial amounts of limestone or lime.

Bureau of Medical Inspection, Broken Hill, New South Wales.INTRODUCTIONFor a considerable time it has be3n realised that the pathological changes produced in human lungs by the inhalation of free silica varied, not only according to the amount of silica inhaled, its particle size and the period during which it was inhaled, but also according to other mineral elements which might be admixed with the fine silica dust and inhaled along with it, or inhaled subsequently to the silica containing dust. It was observed that, in many instances, these admixed dusts tended to inhibit the action of the free silica in causing injury to the lungs, as well as the development of certain complications, whilst in other cases it was felt that certain substances could actually accelerate the tissue damage caused by the silica.A fresh impetus to the problem of the prevention of the development of silicosis was given when, in 1937 and 1939, Denny, Robson and Irwin published papers entitled "The Prevention of Silicosis by Metallic Aluminum." Working at the same time at the Saranac La.boratory, and quite independently, and using a colloidal aluminum hydroxide, Gardner arrived at the same conclusions as Denny, Robson and Irwin, namely, that in experimental animals the development of silicotic changes could be prevented if eithertogether with the silica, or subsequent to its inhalation...

The Mt Tom Price deposit is a world-class high-grade hematite deposit in the Hamersley Province of Western Australia with an original resource of 900 Mt of almost pure hematite, averaging 63.9 wt per cent Fe. Petrological and geochemical studies at both the Southern Ridge and the North deposit at Mt Tom Price have identified three hypogene alteration zones between unmineralised BIF and high-grade iron ore: distal magnetite-siderite-stilpnomelane; intermediate hematite-magnetite-ankerite-talc-chlorite; and proximal martite-microplaty hematite-magnetite-apatite alteration zones.Fluid inclusions trapped in siderite within the distal magnetite-siderite- stilpnomelane alteration zone at the North deposit revealed primary high salinity (25.5 eq wt per cent NaCl-CaCl2) inclusions that homogenised between 107¦ and 142¦C into liquid. Fluid inclusions trapped in ankerite within ankerite-microplaty hematite veins in the intermediate hematite-ankerite-magnetite-talc-chlorite alteration zone at the North deposit revealed mostly H2O-CaCl2 pseudosecondary and secondary inclusions with salinities of 22.4 - 25.4 eq wt per cent and 22.9 - 25.9 eq wt per cent CaCl2, respectively. Pseudosecondary inclusions homogenised between 153¦ and 449¦C and secondary inclusions homogenised between 103¦ and 157¦C. Fluid inclusions trapped in apatite within talc-microplaty hematite veins in the intermediate hematite-magnetite-talc-chlorite-apatite alteration zone at Southern Ridge revealed that primary, medium salinity (7.0 - 9.0 eq wt per cent NaCl) inclusions homogenised between 181¦ and 257¦C and primary high salinity (22.8 - 25.9 eq wt per cent CaCl2) fluid inclusions homogenised between 118¦ and 257¦C into liquid. Microthermometric analysis of quartz from quartz-hematite veins from the Southern Batter Fault, Southern Ridge show a complex fluid inclusion history. Primary fluid inclusions consist of: low- and high-salinity H2O-NaCI inclusions trapped at temperatures of approximately 140¦ to 230¦C; vapour-rich inclusions of unknown compositions; and æcomplexÆ salt-rich (Ca, Mg, K and Na inclusions).Secondary inclusions consist of medium-salinity fluid inclusions trapped at temperatures of about 140¦ to 280¦C.A two-stage hydrothermal model is proposed for the formation of both the Southern Ridge and North deposits. Early 1a hypogene alteration involved the release of hydrothermal NaCl-CaCl2-rich (25.5 eq wt per cent) basinal brines (110¦ - 150¦C) from the underlying Wittenoom Formation and directed upward along normal faults and focused within the silica-rich rocks of the Dales Gorge Member, by the shales of the underlying Mt McRae Shale Member and overlying Whaleback Formation. Within the Dales Gorge Member hydrothermal basinal brines migrated laterally within large-scale folds with permeability controlled by shale bands and the NW trending dolerite dyke sets. Fluid rock reactions transformed unmineralised BIF to magnetite-siderite-iron silicate BIF, with subsequent desilification of the chert bands. Stage 1b hypogene involved an increase in temperature of the hydrothermal, CaCl2-rich saline (24 eq wt per cent) basinal brines (250¦ - 300¦C) resulting in formation of hematite-ankerite-magnetite-talc-chlorite alteration and the crystallisation of microplaty hematite. Late stage 1c hypogene alteration involved the interaction of low-temperature (~120¦C) basinal brines with the hematite-ankerite-magnetite mineral assemblage. At the Southern Ridge deposit this process was very intense (ie high fluid flux), therefore resulting in the almost total removal of ankerite and resulting in the increased porosity of the ore. Stage 2 supergene enrichment in the Tertiary resulted in the removal of residual ankerite and apatite and the weathering of the shale bands to clay.

Following the preparation of tantalum oxide and carbide (1) an investigation of the details relating to the production of tantalum metal powder was planned. From the literature (2) it was evident that tantalum could be produced either by reduction of potassium tantalum fluoride with an alkali metal or by electrolysis of a fused salt mixture containing a tantalum compound. The latter method appeared to be the more suitable and, as there was considerable divergence of opinion in the literature as to the nature of the reactions involved, it was decided to study the electrolytic process. The ultimate aim in any case was to produce tantalum pOWder for use in an investigation of the problems relating to the preparation of consolidated tantalum.FACTORS GOVERNING ELECTROLYSISAfter a few preliminary exploratory electrolyses several factors which needed amplification were revealed. These will be mentioned before the details of the electrolysis experiments are given.COMPOSITION OF THE BATHWithout the knowledge gained in the preliminary experiments it would be thought that the best material for the bath would be that one which underwent electrolytic decomposition. Consider what at first appeared the most likely - namely - potassium tantalum fluoride. The melting point of K2TaF7 was determined to be 630°C with volatilization and decomposition taking place at that temperature so that it would not have been very satisfactory.

A great range of contacting equipment is used in solvent extraction. However, a tower filled with a regular packing offers many advantages in the direct extraction from slurries. A study was made of the flow of a dense slurry through a packed tower in which the continuous phase was kerosene. The grid type packing, which was preferentially wetted by the slurry, had slats inclined at an angle of 30¦, to the horizontal. On the sur- face of the slats there were uniformly spaced longitudinal indentat- ions. These were of a height slightly less than the film thickness and divided the slurry discharging from the bottom of the slats into a number of uniform streams. When slurry entered the tower it was distributed by means of a troughs and weirs type of distributor. Because of the relatively high slurry velocities that occur at the entry region, it was necess- ary to have the distributor and the first two grids above the solvent level. The interface at the bottom of the tower was located at the mid-point of the bottom grid, so that the slurry entered the interface as a continuous film across the full width of the slats. Using the above system, stable flow with low entrainment was achieved at slurry flow rates of 3.4 mis/cm/sec. This would be equivalent to a flow rate of 177 gallons per minute through a tower of cross-section 4 ft 4 in. by 4 ft 10 in., or 1200 tons per day of solids of specific gravity 2.6 when the pulp density was 65 per cent. Previous tests indicated that a total tower height of about 30 ft would be required for a recovery of better than 99 per cent.

Many open cut mines extend to depths well below the natural water table level with constant dewatering required to maintain operations. When mining activities cease the groundwater starts to fill the void and pit lakes form. The resultant pit lake is directly connected to the surrounding water table and understanding the hydrological and chemical evolution of these lakes is important for minimising environmental impacts. In Western Australia alone there are currently 1800 mine voids and more than 150 mines operating below the water table (Johnson and Wright, 2003). In conjunction with BHP Billiton Iron Ore, CSIRO has been undertaking research to measure and model rates of evaporation from pit lakes in the Pilbara region of Western Australia. Dr Wark said: I have not come to Broken Hill to tell you how to conduct flotation operations; indeed, during my short stay here I have already learned much concerning the process, of the many difficulties which have been overcome, and of those still awaiting solution. My purpose tonight is rather different. All who have been concerned with the operation of the flotation process must have wondered, like myself, why the process is possible at all. In particular, why can minerals which are heavier than water be made to l10at to its surface; why do certain minerals float while others do not; and what are the functions of the added chemicals?I have been asked to describe tonight some of the ways which have been adopted in the Melbourne University laboratory in an endeavour to find answers to these. questions. Knowing that you are just as interested in them as we are and, furthermore, appreciating that were it not for the co-operation of the Broken Hill companies our researches would not have been possible, I regard it as a high privilege that I should have been invited to tell you of our work. Before doing so, I would like to tell you the reasons why the work was started and for whom it has been carried out. Soon after the flotation process had been successfully exploited on a commercial scale a number of investigators sought to determine the theoretical basis upon which it rests, and our knowledge of the function of the added chemicals has been considerably broadened by the published accounts of many workers, particularly in recent years of those working in American universities.The Australian metallurgist played, as you know, a part second to none in the early development of the process, and Australia's mining fields had already reaped such rich rewards from it that, late in 1929, it was felt by some of the leaders of the industry that we should take a larger part in the efforts being made to extend the knowledge of the principles upon which the process is based.Accordingly, several companies - the Broken Hill South, the North Broken Hill, The Zinc Corporation, the Electrolytic Zinc and the Mount Lyell Companies - established a laboratory in the chemistry department of the University of Melbourne for the purpose of investigating the principles underlying flotation.Subsequently another British company, the Burma Corporation Ltd, expressed a desire to co-operate.

Hillgrove, situated on the New England plateau of New South Wales, is an area rich in history and resources. Key resources are predominantly antimony, gold and scheelite and the area surrounding the historical field is environmentally sensitive and topographically challenging. Since acquiring the Hillgrove leases, Straits Hillgrove Gold has undertaken a significant exploration program on these historical mining areas with the goal to return the field to production. This exploration program has enabled Straits to significantly increase the resource base and has subsequently enabled the planned redevelopment of Hillgrove to be realised. In October 2006, installation of mining infrastructure and the initial stripping of existing development had commenced. To successfully redevelop Hillgrove, Straits has had to adopt a highly mechanised, trackless, diesel mining operation which will see the production rate commence at an initial rate of 250 kt/a of ore from a narrow vein environment. This ore will be processed on site to produce antimony metal, gold bullion and a scheelite concentrate.

Drilling and shooting techniques at Moura open-cut have evolved over a period of 16 years and are the result of hard earned experience combined with established practices and consul- tation with professional services. The wide range of overburden types encoun- ted in the field requires varying methods to achieve the best results at all times. Safe working conditions for the employees and protection of mining equipment is of para- mount importance and close liaison is maintain- ed with the Department of Mines Queensland to keep abreast with changing legislation. Drill patterns, charging profiles, types of explosives and ultimate results are des- cribed in detail along with a resume of equip- ment. Costs of drilling, labour and explosives are also analysed with a total of approximately 20c per cubic metre of shot ground. Future developments and improvements to facilities are also outlined.

Mineralogy is the link between ore formation and ore extraction. It is the most fundamental component of geomet programs, and the most important aspect of a life-of-project approach to mineral resource projects. Understanding orebodies is achieved by understanding the mineralogy and texture of the materials, throughout the process, because minerals hold the information required to unlock the value they contain. Geomet mineralogy programs absolutely require the appropriate expertise and at least three steps of mineral characterisation prior to using semi-automated or other methods: field examination, thorough core logging, and optical microscopy. Economic geological inputs for orebody characterisation are necessary for orebody understanding, and are exemplified by current research in the Zambian Copperbelt, where revised sequence stratigraphy and understanding of alteration, metasomatism and metamorphism can be used to predict topical issues at mine sites. Environmental inputs for sustainability characterisation are demonstrated by recent work on tailings from the Leadville, Colorado, USA area, including linking mineralogy to water quality issues. Risk assessments need to take into account the technical uncertainties around geological variability and mineral extractability, and mineralogy is the only metric that can be used to make this risk contribution.CITATION:Olson Hoal, K, Woodhead, J and Smith, K S, 2013. In the process of mining the lower seam in a multi-seam operation, a lot of oblique fract- ures appeared in the upper seam which was in the equilibrium zone. These fractures closed soon after the lower face line passed. Despite the occurrance of some degree of undulations, and and step-like unevenness, the upper seam was subsequently extracted without difficulty. When attempting multi-seam extraction, from the height of the equilibrium zone, the minimum distance between seams in ascending mining should be predicted. By analysing roof con- vergences data and the geometrical configuration of subsidence profile, it was found that the minimum safe distance for ascending multi-seam mining depends on the mining height of the lower seam and the properties and structure of the inter-stratified rocks.

The paper describes developments since 1959 in major excavators to meet the demand for higher outputs of overburden and coal under less favourable batter conditions than used hitherto in the coal deposits of the Latrobe Valley. Particular reference is made to investigations of designs of bucket wheel head configurations for the digging of these flat batters on high faces, and examples are given of the wide use of models inthese studies. An explanation is given of how solution of these problems and surface conditions unfavourable for wheeled transport confirmed selection of the long boom rather than the compact type of bucket wheel excavator for the Latrobe Valley applications.The past decade has seen advances in the manufacture in Australia of components for excavators and similar machines. The history of Australian manufacture of components for major brown coal excavators is traced;and reference is made to study and application of Australian and overseas (mainly German) codes and practices in the design and construction of machines made partly here and partly in Germany.A section of the paper deals with advances in electrical engineering for large excavators in Australia and its interdependence with mechanical design and construction.Finally, as an example of the versatility of the bucket wheel excavator, a description is given of a recent application for the mining of mineral sands in northern N.S.W.

The Que River Mine is a small under-ground base and precious metals mine located in north-western Tasmania. Current production rate is 220,000 tonne/yr from reserves grading 0.37% copper, 7.1% lead, 12.2% zinc, 195 g/tonne silver and 3.4 g/tonne gold.Ore is mined using longhole open stoping techniques. Cemented fill is placed in primary stopes to permit safe and efficient recovery of pillars. The operation is trackless with both shaft and decline access from surface.Ore produced is trucked 40 km by road to be sold to the Electrolytic Zinc Company of Australia Ltd., for treatment at their Rosebery concentrator.

As filtering technology improves and there is an increased focus on environmental impacts, the concept of managing fine coal rejects by filtering, mixing with coarse rejects and dry stacking them is being considered more seriously. In Queensland, there appears to be a desire from regulators to move away from wet rejects facilities towards dry-stacked facilities, resulting in a change in the risk profile for the facility.A coalminer has a site with many constraints, including limited area that is not planned to be undermined and nearby rivers that are prone to flooding. The design of a dry-stacked facility that meets the operational requirements and environmental compliance criteria is challenging in subtropical climatic settings, where there is a need to consider wet seasons with high-volume, sustained rainfall events. The impact of the rainfall is multifaceted, requiring careful surface water management to separate clean and contact water, adequate foundation preparation to limit seepage to groundwater, management of stack stability given a potentially high phreatic surface and the avoidance of potential flood zones. These design constraints were compounded by a limited area available for rejects storage that would not be undermined.This paper presents a summary of the conceptual design of a dry rejects facility, including consideration of regulatory and client requirements, provision for expansion while also considering progressive rehabilitation, diversion of clean water as much as practicable during operations and siting constraints. This talk was delivered in Kalgoorlie in April this year, to the Kalgoorlie Branch of The Institution of Engineers, Australia. We believe it is of such interest to members of our Institute and readers of this publication we believe it important to extend the audience who have the opportunity to read it. An edited version follows:"Why should we be so concerned about Mining Related Education; after all is said and done, the mining industry is not a great employer of persons?"The mining industry is capital rather than labour intensive. It employs only 2.7 per cent of the national workforce. However; the minerals industry is Australia's major exporter accounting for over 40 per cent of the nation's total export of goods.In Western Australia the value of minerals production in 1985 was about $5,000 million and consequently mineral exports accounted for about 53 per cent of the total value of overseas exports from the State. The diversity of the mining industry is also worthy of mention and produces iron ore, alumina, gold, nickel and mineral sands. That apart, our potential to become a major producer of diamonds, rare earths and petroleum products is considerable, not to mention base metals (copper, lead and zinc) and uranium.Our mining industry is of national importance and contributes in excess of 30 per cent of the total value of Australian overseas trade. Although the mining ind ustry does not employ many people, the people it employs contribute very much more, per capita, to the wealth of the nation than do employees in any other sector of our economy.No-one would deny that during the 20th century Australians have depended on the export earnings from the sale of relatively unprocessed agricultural products and more recently the sale of relatively unprocessed minerals for their standard of living.Not surprisingly, therefore, our economy is highly vulnerable to unstable world commodity market prices.The slump in trade we are currently experiencing (in first quarter 1987-Edition) was brought about by the over supply of the raw materials we produce. However, the recent downturn in world market prices is more serious than previous slumps because in part it is due to technological advances 'in other countries which could have a permanent effect on the world trade of primary products.

During late 1953 and early 1954, tests were carried out in Sydney metropolitan brickpits in order to determine if some improvement might not be effected in the personal shock, fly rock and possible ground displacement accompanying daily blasting occurrences in the numerous quarrying activities of the metropolitan area. As a result of this preliminary work (Reid, 1954), the Department of Mines decided to further investigate the problem with a view to placing this type of blasting on a sound economical and safe basis for both operator and nearby residents alike. To assist in the work, a Cambridge Universal vibrograph was acquired.Human safety was the first consideration followed by property damage to homes and factories in the extractive areas. Although most of the actual field work has been confined to the Sydney metropolitan area and in particular to the large clay-shale quarries, many cities and country towns have blasting problems associated with the extractive industries, engineering projects both above and below ground level, and in building excavations to which the results may apply.The following figures illustrate how much activity of this type is located around cities and towns:Sydney Metropolitan Area:

The development of the Moura Coal Preparation Plant is described, commencing with the first'stage of the plant commissioned in 1963. The subsequent expansion stages are traced through to October, 1973. The plant has undergone frequent stages of expansion and modification and this process is continuing. The development programme for 1974 is described as proposed in late 1973. The characteristics of the Moura and Kianga field coal seams with respect to washability are summarised and some comment is made on the blending of these coals prior to washing, to ensure production of export grade coal within specification.
