Noise-induced hearing loss (NIHL) is the most common occupational illness in the United States, with 30 million workers exposed to excessive noise levels [NIOSH 1996] every day. Of particular concern is the mining industry; which has the highest prevalence of hazardous noise exposure of any major industry sector [Tak et al. 2009] and is second only to the railroad industry in prevalence of work-ers reporting hearing difficulty [Tak and Calvert 2008]. This document is for operators, safety personnel, and mechanics in the mining industry who are not specialists in noise control engineering or acoustics. Evaluations of successful and unsuccessful attempts at controlling noise on several large, underground metal mining machines are detailed to illustrate the basic principles of noise control. Once personnel understand the guidelines and principles of noise control, they will be able to ¦evaluate the extent of a noise problem; ¦determine the best approach to the problem; and ¦apply the most appropriate solution. Because of the insidious nature of NIHL, it can go unnoticed until a considerable loss of hearing has occurred. In some cases, diagnosis is delayed because an exposed individual claims to have become accustomed to the noise. In reality, that person may have already suffered irreversible hearing loss. Humans can hear sounds in the frequency range from about 20 to 20,000 Hertz (Hz). Within this range, NIHL usually begins in the frequency region around 4,000?6,000 Hz, the upper levels of the speech region. The first noticeable symp-toms include difficulty understanding higher pitched voices, such as the voices of females and children, and difficulty understanding certain consonant sounds, which are primarily high frequency in nature. The extent of NIHL varies depending on the level and duration of noise exposure and on an individual?s susceptibility; despite having similar noise exposure, individuals can experience differing degrees of hearing loss, or none at all. NIHL is almost always preventable. To reduce or eliminate the possibility of NIHL, an individual?s noise environment must be analyzed and appropriate action taken to reduce noise exposure.

The Emeishan large igneous province (ELIP) of SW China is host to magmatic Ni–Cu–(PGE) sulfide and Fe–Ti–V oxide deposits which contain economic to subeconomic concentrations of precious and base metals. The formation of a given deposit-type (ie sulfide versus oxide) is thought to be related to the type of parental magma (ie high-Ti versus low-Ti) and the subsequent processes (ie fractional crystallisation and sulfide segregation), which allowed for metal enrichment.We examined the PGE of Baima and Taihe oxide-bearing layered mafic intrusions from the ELIP (Shellnutt, Ma and Qi, in press), and compared them with the nearby Xinjie and Hongge intrusions as well as with the Emeishan flood basalts. The PGE results for the two intrusions are contrasting. The Baima gabbroic rocks are almost PGE-barren (SPGE < 4 ppb) whereas the Taihe gabbroic rocks are variable, but generally much higher in PGE concentrations (SPGE < 2 to ~300 ppb), although the gabbros from both intrusions are enriched in platinum-subgroup PGE (PPGE = Rh, Pt and Pd) relative to iridium-subgroup PGE (IPGE = Os, Ir and Ru). Cu/Zr, Ni/Ir, Y/Pt, Cu/Pd and Ni/Ir systematics suggest that the Baima rocks were derived from a relatively evolved, mafic, S-undersaturated parental magma that was affected by earlier sulfide segregation. In contrast, the Taihe rocks record evidence of both S-saturated and S-undersaturated conditions and that the parental magma was likely emplaced very close to S-saturation. Through comparison of the PGE contents in the Emeishan flood basalts and the Emeishan oxide-bearing intrusions, we provide a self-consistent model for ELIP that the PGE budget in a magma is not strictly controlled by magma series (high-Ti versus low-Ti), but very much by crustal contamination.CITATION:Ma, G, Shellnutt, J G and Qi, L, 2015. Due to the evolution in recent years, the Press-in Method is now applicable to 2.5m thick retaining walls, over 50m deep foundations, overcoming rock conditions and confined construction conditions.The Press-in PrinciplesThe basis of the Press-in Method is simple. The Silent Piler presses a pre-formed displacement pile into the ground by static jacking force. The reaction for the Press-in Force is generated by gripping previously installed piles. As a pile is pressed into the ground, “A Pressure Bulb” is formed around the pile toe. Accordingly, higher bearing capacity can be generated compared to driven piles without disturbing the ground and producing any vibration. A unique sluicing process for recovering tailing impounded in a valley was developed by Electrolytic Zinc Company of Australasia Ltd., West Coast Mines, Rosebery, Tasmania.Earthmoving equipment could not be used as most of the material to be recovered was semi-fluid. A conventional sluking method was also impracticable as there was no suitable working face from which the process could be commenced.Operations started from a flat surface, using special techniques for sinking through the tailing and establishing producing working faces as the sinking process continued.After about 2 years the bottom of the dam was reached and conventional sluicing methods could be used.

The recent seismic events have shown that earthquakes have an immense destructive power. Chile was hit in February 2010 with a seismic intensity of 8.8 on the Richter scale, and had loss of about 700 killed persons, while Haiti was hit in January 2010 with 7.0 killing about 220?000 people. The event in Haiti showed that the seismic events can lead to a disaster if no preventive measures are taken. On the other side, the seismic event in Chile has hit a well-prepared country with moderate consequences. The earthquake with 6.6 Richter in Japan in March 2011, have further shown how severe the consequences can be if critical equipment is involved. Fortunately, the equipment in a mine has not the potential hazard as a nuclear power plant. Anyway, we are diligent and it is obvious that the seismic risk may lead to a real disaster even if the probability is small. The GMD is an important component in the grinding circuit of a mine. This paper maps out earthquake-prone areas including expected earthquake magnitude calculated according to relevant standards, focusing mainly on the building code IBC 2009. An overview is provided on how this standard is applied to mining equipment to minimize damage and aid large mills in operation to successfully withstand seismic effects. The differences in stresses and forces on the GMD and foundation are shown based on a turning and stopped mill simulation during an earthquake. The results demonstrate the need to maintain active and operating earthquake detection systems. Aside from detecting an earthquake?s magnitude, the sensor information can also be used to reduce the harmful effects that an earthquake has on mining equipment. As follows the earthquake requirements are defined. The last part of this paper explains how to fullfill these requirements.

This Federal Bureau of Mines publication is a statistical analysis of all surface and underground coal mine fires reported in the United States from 1950 to 1977. Accounts of selected nonreportable fires and opinion data gathered via interviews with mine safety directors were separately analyzed. Both reported and nonreportable fires were analyzed by time trends, State, ignition source, burning substance, location in mine, equipment, detection, duration, injuries, fatalities, and successful extinguishing agents. It was found that the majority of all mine fires were electrical in origin. The equipment most frequently involved in underground fires was conveyor belts, and in surface fires, it was crushers and/or breakers. Underground fires occurred most often near the working face. Water and hand-portable extinguishers were the most common methods of extinguishment.

"The problem of recovering potash from the mineral polyhalite (K2SO4.MgSO4.2CaSO4.2H2O), occurring in large quantities in the salt beds of Texas and New Mexico, has been under investigation at the Nonmetallic Minerals Experiment Station of the U. S. Bureau of Mines continuously since 1928. Most of the results previously obtained have been published in technical journals and in a series of Bureau of Mines Reports of Investigations.The experiments described in the present report were performed to ascertain whether a continuous countercurrent extraction of calcined polyhalite by hot water would yield satisfactory recoveries and concentrations of sium sulphate in the top liquor. Previous investigation by Storch 4 had shown that the rate of solution of uncalcined polyhalite was too slow to be of value in an industrial process, and Storch and Clarke 5 had demonstrated that calcination of the polyhalite greatly, accelerated the rate of solution in hot water, allowing most of the potassium sulphate to be readily extracted."

Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are improving but more slowly and yet, serious accidents and fatality rates are not: in fact, these rates of more serious incidents have plateaued or deteriorating. In more dramatic cases, organizations that had ?exemplary? safety statistics, suddenly had a catastrophic or multi-fatality event occurring. Popular books such as In Search of Excellence of Peters and Waterman or Good to Great of Jim Collins created many delusional ?secrets? or recipes, to achieve great performance and yet, most of those ?great? or ?excellent? companies have now declined in their performance, some dramatically and some have fallen from grace, like Fanny Mae, a company Jim Collins identified as a ?great? one, that had to be bailed out in the latest financial crisis. These organizations share several ?delusions? about managing a business, and often, these delusions are created by academics, consultants and authors of popular books about (safety) management. More damaging however is that safety management is defined by a set of key practices or axioms: a focus on human error, compliance to rules and regulations, creating consistency of systems and actions, reacting to accidents and incidents and on prevention of those, measuring safety with performance metrics and rewarding employees on those measurements. All of these axioms contain the seeds of destruction, and they create conditions and dynamics that precede the disaster - and ironically, they are all 'good things' we do in safety management. For example, a mere focus on safety in the business, and the measurement of it, create a culture of repression of mistakes and incidents, leading to a situation where the organization knows incrementally less and less of risks and risktaking behaviour - until the day it all coincides destructively! This paper discusses seven such safety delusions and how they impact on the views and safety management approaches. They are: ??The delusion of (risk) control, that risks can actually be identified, analyzed and mitigated. ??The delusion of compliance, that mere compliance to rules will safeguard the business ??The delusion of consistency, that standard operating procedures will ensure safe operations ??The delusion of predictability, that accidents and trends tell us everything about the risks of the future ??The delusion of human error, that people are the weakest link and that it can actually be dealt with through discipline and enforcement. ??The delusion of statistical trends, that safety is measurable with statistics ??The delusion of invulnerability, that we have achieved high levels integrity and capability to deal wit risks. Organizations that suffered catastrophes all share some or all of these delusions and this paper traces how these played a role in many disasters, ranging from the Titanic, Challenger, Piper Alpha, Bhopal, BP Deepwater Horizon and many more.

The Huntington Beach oil field is on the Pacific Coast, southeast of the City of Los Angeles (Fig. 1). It lies both onshore and offshore. Most of the onshore section is operated by Standard Oil Co. of California, WOI. The Tar reservoir is of upper Pliocene age, about 200 ft thick, and consists principally of fine-grained, unconsolidated, somewhat shaly sand, with thin inter-bedded shale stringers throughout (Fig. 2). The reservoir is a broad, highly faulted, elongated anticline, with an areal extent of about 370 acres. Most of the faults bisecting the structure are normal, trend north-south, and are the main cause of reservoir discontinuity (Figs. 3 and 4). From 1964 to Dec., 1967, 259 steam stimulations were performed on a total of 125 Tar sand wells. Eighty-six new wells were drilled and steam stimulated. Of the total, 96 have been stimulated twice, 34 three times and 3 four times. During this same period, a two-injector steam drive pilot has been in progress. The response obtained from this project has been compared with the response to cyclic stimulation. This paper presents an analysis of the field observations for the two methods of thermal recovery. History of Development Drilling to the Tar pool commenced in 1924 with an initial well production of 40 to 50 B/D of 12o API gravity oil. Complete primary development of the pool has been spread over several years. Approxi- mately 40 wells were drilled during the first phase of development, which was completed in 1945. The subsequent primary development started in 1960 with initial well productions of 30 to 40 B/D oil. Thirty-seven wells were completed in the Tar pool during this phase. For the most part, slotted, gravel flow packed liners have been used in the wells in this pool. A cyclic steam stimulation pilot was initiated in March, 1964. Based on the very successful stimulation in the pilot area, the cyclic stimulation project was expanded to include all of the Tar pool wells producing at the end of 1965 (Fig. 5). Also, a development drilling program was begun at this time. To date, 86 new wells have been drilled under this program. The number of wells drilled each year in this secondary phase of development is also shown in Fig. 5. New Well Completion Practices Most of the new wells have been completed to produce from both the Upper and Lower Tar zones. Generally, a casing stub is cemented from the top of Lower Tar to the base of clean Upper Tar sands to exclude intermediate water-bearing sands. The water string is then cemented from the top of Upper Tar to about 1,500 ft with neat cement and 30 percent silica flour. The annulus from 1,500 ft to surface is then filled with a petroleum-based gel through a high-temperature port collar. Industrial engineering departments perform staff duties and are most effective when sincerely supported by management. In accordance with the needs and desires of the mining companies involved, various industrial engineering functions have been adopted widely at western metal mines during the period 1950-60. Newly organized departments usually conduct method and equipment studies, work-measurement programs, and establish and maintain production standards. Supervisor-training programs and courses are effective in showing the relationship between supervisory responsibility and the purpose for which industrial engineering is practiced. Industrial engineering staffs of western metal mines are composed predominantly of personnel from within the companies. Experienced industrial engineers, not necessarily experienced in mining, frequently are hired by a company to help establish the department. Management consultants frequently are called upon to advise management and instruct department personnel in industrial engineering practices. Some departments have developed satisfactory industrial engineering programs without benefit of formal industrial engineering training or advice.

The concept of using travelling magnetic fields created by linear motors was stimulated by Laithwaite who advanced ideas on motors with shaded poles (Laithwaite, 1968), rack and pinion motors (Laithwaite, 1970) as well as low speed, brushless motors and three dimensional motors (Laithwaite, 1973a and 1973b).Sudan (1963) and Sundberg (1969) had applied the concept to metallurgical processes. However, the use of travelling magnetic fields has not been used widely to produce conditions for the separation of magnetic from non-magnetic minera1s or even magnetic materials from non-magnetic materials, although some attempts were made by McCarthy (1922), Davis (1934), Dean, Gottschalk and Davis (1934) and Gottschalk (1934).Laithwaite (1973, a) while working with linear motors made suggestions for designing separators and in the work described here, the observations of Laithwaite and others are extended to determine the characteristics of the motor with a view to using it as a magnetic separator.

[ ] In November 2005, Essroc Cement, part of the Italcementi Group, commissioned a long overland conveyor at their Nazareth plant in Pennsylvania. This conveyor, tagged CV-105, was designed to transport 1000 tph of limestone from the quarry to the kiln, and 400 tph of hot clinker (at 300°F) at back from the kiln to a handling facility about halfway back. Conveyor CV-105 is 8969 ft (2733 m) long. It gains 152 ft (46 m) of elevation in the first 1570 ft (480 m), then is mostly flat for the remaining 7400 ft (2250 m) (Figure 1). After the limestone is discharged at the head pulley, hot clinker is loaded onto the return side of the belt. This clinker can be discharged at one of two locations.

Any discussion of outside support to Latin American mineral industries must concede at once the pre-eminent role of U. S. industry and business. American capital has developed the great copper resources of Chile and Peru, the lead-zinc of Peru and Mexico, the bauxite of Surinam and Jamaica, the petroleum and iron ore of Venezuela. American consumers have provided the market for their products. U. S. companies operating south of the border have trained Latin American geologists, engineers, chemists, and skilled labor—including metal, mineral, and fuel technicians—to new and greater competence. The industrial demands of World War II drastically altered world economics and concepts of U. S. foreign mineral requirements. In this country discovery of vital mineral resources fell far behind the alarming depletion rates, and there was new appreciation of the actual and potential importance of the world's less extensively explored areas. During the war and afterward, the task of supplying the nation's mineral requirements became greater than private industry could carry alone, and the Government assumed a position of continuing support. In Latin America, as elsewhere, this support applies in three categories: banking, procurement, and technical assistance. The first two are now generally accepted as necessary Government functions; the third has often been misunderstood and misinterpreted, although it has given rise to legitimate controversy. There have been major accomplishments in all three categories, as well as some errors of judgment. Since the beginning of World War 11, U. S. Government financing, procurement, and technical assistance have been applied on a large scale to a wide range of commodities and services throughout the free world. The present discussion—under the separate headings mentioned—will concern only the support related to a single industrial group in a single geographic area. During the War Federal banking was conducted on an emergency scale by temporary agencies created to meet the abnormal circumstances. Unfortunately, there are no readily available data on these activities that would permit evaluating loans and credits extended for mineral projects in Latin America. The only agency operating in this field since World War II is the Export-Import Bank, under its own authority and to some extent from credits authorized by the Office of Defense Mobilization. The Bank can offer more capital—on more favorable terms—than is available from most Latin American banks, and its staff of engineers evidently can appraise loan applications more favorably than commercial banks in this country are willing to do. The risks seem to be justified, as defaults have been well under 1 pct of total loans. From V-J day 1945 to the end of 1957, mineral loans to Latin America have been authorized for projects totaling $541.63 million in eight countries. In order of magnitude of credits, they have been made to Brazil, Peru, Chile, Mexico, Argentina, Venezuela, Guatemala, and Bolivia. In order of magnitude by industries. they are for development and expansion of steel mills, copper, manganese, nitrate plants, iron ore, zinc plants, sulfur plants, cement plants, tungsten, coal, and lead. Loan Distribution: By far the largest loans have been $307.9 million for iron and steel projects in Brazil, Chile, Mexico, Argentina, and Peru: $100.4 for developing the great low-grade copper deposits of southern Peru; and $55.3 million for developing manganese deposits in the Territory of Amapa, Brazil. Others range from $20.8 million for a zinc smelter in central Peru to $19 thousand for iron-mine trucking equipment in Chile. It has been noted that 59 pct of the loans have been to foreign companies, 14 pct to companies of mixed foreign and American capital, and 27 pct to American mining companies. This project had as its objective the experimental investigation of water capsule production utilizing the Southwest Research Institute's submerged nozzle encapsulation apparatus (U.S. Patent 3,389,194). Initial efforts on this project involved nozzle configuration and shell formulation optimization. The shell formulation developed on a prior program was modified during the course of the project to obtain the best balance of capsule breakage resistance and water retention. Subsequent effort centered on determining operating conditions which would produce the best quality capsules for each candidate shell formulation as determined by laboratory tests. Some preliminary work was done to increase the production capacity of the encapsulation equipment. The program was extended with major emphasis concerned with scale-up studies of capsule production capacity and the treatment of problems associated with grouting cartridge fabrication. During the course of the program, water-containing capsules were produced which have good long-term water retention, block resistance and adequate strength. Using a single-nozzle design, a production capacity of 15.9kg (35 pounds) per hour was demonstrated to give acceptable capsules based on initial results.

A survey is made of changes in the technical status of a number of western world copper extraction and refining operations over the past 20 years. Specific developments in pyro- and hydro-metallurgy and refining practices are identified together with factors in the changing environment of the industry which has influenced their adoption. Nickel extraction and refining is similarly discussed.The replacement of traditional technology by more attractive alternatives has occurred in a considerable number of operations. The develop-ment and status of new continuous smelting processes, advances in hydrometallurgy and refining innovations which emerged during the 1960s and early 1970s is discussed. Errors in ore reserve estimation can occur during drilling and sampling, sample preparation and analysis, geological logging and modelling, database construction, statistical or geostatistical evaluation, selection of economic parameters, selection of ore reserve method and assessment of mining constraints. Core sample recovery can be a major source of error, and it is proposed that the modelling of grade, metal accumulation and thickness should be accompanied by the modelling of core recovery (or core loss) so that for each ore block a weighted block recovery factor is obtained. This gives a good indicator of the reliability of a reserve estimate. It is suggested that a resource reliability rating (RRR) system be established to quantify what is meant by geological assurance and to combine this with an estimate of the precision of estimation as gained by geostatistical analysis. Proven and probable minable reserves would be those with an RRR better than 50% and a precision of estimation better than 20%
