A recent headline in the Wall Street Journal declared, "trendy management schemes very often fail." Many of the management techniques addressed in the accompanying article are associated with quality management including reengineering, benchmarking, total quality management, and employee empowerment. The article made the point that executives, employees, and customers who are involved in such programs rate the effectiveness of these programs very low, and the programs often fail. Yet, we all know of companies that have made significant advances in their operations using these same quality management practices. Why do a few succeed while many fail? History has shown that the companies that successfully carry out quality management programs have two things in common: 1) Strong leadership committed to the success of the program. 2) A comprehensive plan for carrying out and maintaining the program. Stated another way -the reason many companies fail is the lack of committed leaders and inadequate planning. It is ironic that the title of the Wall Street Journal article is "The Best Laid Plans."

The Rio Blanco Oil Shale Company completed a modified in situ oil shale retorting demonstration at its Tract C-a Lease in the Piceance Creek Basin of northwestern Colorado during the spring of 1982. This demonstration was the first MIS project with shafts rather than adits for access to the mining operation, and it involved several potential hazards which required that particular attention be paid to mine ventilation. Methane was first encountered during shaft sinking and ultimately resulted in the mine being classified gassy by MSHA. Secondly, we encountered hydrogen sulfide in association with the ground water, and this proved to be a more persistent problem than did the methane. Thirdly, in order to burn the underground retorts, it was necessary to provide for potential leakage of retort gases and to obtain a modification to the MSHA standard which forbids fires under- ground. This modification had a significant effect on the ultimate ventilation procedures at Tract C-a. An extensive under- ground air monitoring program was included in the ventilation system.

The southwestern United States has an abundance of natural lightweight aggregate (NLA) deposits of volcanic origin that are being used in the construction industry within the region. New Mexico, Arizona, Nevada and California utilize NLA in portland cement concrete and concrete masonry units for weight reduction and related economic reasons. The geological environment of the western United States is amenable to the occurrence of natural lightweight rocks because it contains Tertiary and Quaternary volcanic products that include low-density glass and vesicular material. There are two major classical types of natural lightweight materials of igneous origin; pumice and cinder. These account for the vast majority of volcanic rocks that are presently utilized as NLA. Pumice is an ejecta product generally found as part of tuffaceous units associated with nearby siliceous volcanic centers. Cinder, also an ejecta product, is associated with mafic volcanics and is most often found as a cinder cone. Both are highly vesicular. A third type of natural lightweight material, though also associated with siliceous volcanics, is less vesicular than pumice. It is best described as being a slightly pumiceous rhyolite or rhyolite-tuff breccia that occurs as a siliceous dome or tuft unit that is most likely to be glassy. Uses of these natural lightweight rocks center on the construction industry where they are utilized in lightweight portland cement concrete and lightweight concrete masonry units. The major reason for usage in portland cement concrete is cost saving effected by weight reduction in high-rise construction. The major usage in concrete masonry units, or "block", is for weight reduction that enables a mason to lay more lock.

"Each January, tunneling and underground professionals gather in New York City for the George A. Fox Conference. The one-day event offers attendees the opportunity to listen to their peers outline what’s new in the underground construction industry, get updates on major projects in North America and have a chance to catch up of old friends.The conference is put on by the Underground Construction Association of SME. This latest conference attracted about 370 specialists in the industry. The theme for this year’s event was “Water Tunnels, Past, Present and Future.”The technical session included talks focused on major tunneling projects, including the massive California Water Fix program and the Massachusetts Water Resources Authority (MWRA) Metropolitan tunnel redundance program in Boston. The conference also included an update on the Gateway Project from Phil Rice from WSP and Mahammed Nasim of Amtrak as well as a Tunnel Industry update.The following is a sample of a few of the presentations. Technical presentationsDEP’s Tunneling Program Evolution. James Mueller is the agency chief engineer of New York City’s Department of Environmental Protection (DEP). Its mission is to plan sustainable infrastructure investments for the future of clean water and the environment in the city and its watersheds, he said. The agency’s goals are to synthesize information across bureaus within DEP to allow data-driven decision-making. It is also to optimize internal agency processes and systems related to the implmenation of engineering programs within a structured assessment process. The agency also prioritizes projects and funding within a structured integrated planning process. And it standardizes project performance and delivery to meet the needs of the agency in alignment with industry standards and market capabilities."

A unique machine has been designed for mining narrow veins. The machine, called the NONN, walks on the walls of the stope and carries a boom-mounted hydraulic impact breaker. The concept of walking on the walls allows an existing muck handling system such as LHDs to operate beneath the NOMN. The NOMM has four sets of legs which can be manipulated independently or concurrently to cause the carrier to walk in any direction and to turn sharp corners. The mining rate using the breasting method is targeted to be 25 t/hr.

A metallurgical/flux is a substance that is added to combine with gangue (unwanted minerals) during ore smelting, with impurities in a molten metal, or with other additives in metal refining processes to form a slag that can be separated from the metal. Because slags are immiscible with the metallic melt and are of lower density, a separation of the slag and metal occurs if the viscosity and surface tension are of the proper values. The chemistries of slags are adjusted to provide the proper melting point, viscosity, surface tension, conductivity, specific heat, density, or chemical properties to effect the desired task. In addition to absorbing impurities from the metal, the purposes of the slag are to thermally insulate the metal bath, protect the molten metal from the atmosphere, and control the chemical potential of the system. Several excellent references on slags and metallurgical fluxes are available (Boynton, 1980, Lankford, Jr., et al., 1985, Turkdogan, 1983, Rosenqvist, 1974, Fine and Gaskell, 1984). The function of the slag might vary at different stages within a process prior to final melting. For example, the slag composition and behavior will change as materials descend in the iron blast furnace, or as they melt in the early stages of formation of steel- making slags. Selection of the chemistry for a slag might be influenced by factors outside the primary function as when blast furnace slags are used to make cement, rock wool insulation, or fertilizer supplements. Sometimes slags contain a sufficient amount of valuable recoverable elements to be sold as raw materials for other processes. Fluxes are often referred to as acid, basic, or neutral. Acid fluxes are those that generally form acids in water and bases are those that would generally form bases in water. Typical acid fluxes are silica, alumina, and phosphorus, although alumina can function as either an acid or base. Typical basic fluxes are lime and magnesia. Fluorspar, or calcium fluoride, is a neutral substance because it can be viewed as the reaction product of a base and an acid. The degree of acidity (ratio of acids to bases) or basicity (ratio of bases to acids) is often specified to characterize the slag chemistry for a particular system. The system might also be referred to as acid or basic depending on the choice of slag, for example, basic steelmaking uses slags with more bases (lime and magnesia) than acids (silica and alumina). Many forms of minerals and compounds have been used as fluxes depending on the process requirements, availability, costs, requirements for recycling of intermediate products, and environmental concerns. Because slags are most often mixtures of oxides and silicates, fluxes are usually oxides, carbonates (which decompose to oxides) and silicates. Slags comprising phosphates, borates, sulfides, carbides, or halides have also been used (Rosenqvist, 1974, Moore, 1981, Szekely et al., 1989). In ironmaking and steelmaking, some acidic components are sometimes added. In nonferrous processes slags and fluxes are acidic with silica as the primary component, and although the use of basic flux such as limestone or lime is not extensive, some amount is used to modify slag properties and some refining slags are lime based. The production of iron is accomplished by smelting of ores, pellets, and sinter in blast furnaces with subsequent refining of molten iron and scrap in oxygen-blown processes. The electric furnace is used to make steel, stainless steels, ferroalloys, and special alloys, whereas ferromanganese is often made in blast furnaces. Fluxes are used in all of these processes, and either limestone or lime is the major flux component with some dolomite, dolomitic lime, silica, alumina, and fluorspar being used. In ironmaking, the flux is added either by direct charging or through sinter and fluxed pellets. In steel refining by the basic oxygen process, flux is added as lime and dolomitic lime that are either charged as lumps or injected as fines. Some small amount of limestone is sometimes used. Fluorspar is often added as lumps or mixed with other fine materials in briquettes. Alumina is sometimes added to blast furnaces either by direct charging of sized lumps or through sinter. Typical acid fluxes are sand, gravel, quartz rock, used silica brick, or raw siliceous ore (Lankford, Jr., et al., 1985). Olivine, which is a magnesium-silicate mineral, has been added to iron blast furnaces to enhance removal of alkalis (Lankford, Jr., et al., 1985). Alumina in the form of bauxite, aluminiferous clay, and recycled alumina brick has been used as flux. Alumina can function amphoterically as either an acid or base, for example it can form aluminum silicate in high silica slags or calcium aluminate in lime-bearing slags (Lankford, Jr., et al., 1985). Limestone (calcium carbonate) and dolomite (magnesium-calcium carbonate), or fluxstones as they are sometimes called (Boynton, 1980), and their calcined forms, lime and dolomitic lime are the major basic fluxes. In the latter cases, the carbonates are de- composed in a kiln-type process to drive off carbon dioxide that might otherwise interfere with the subsequent smelting or refining process or require expensive forms of energy to effect the decomposition (e.g., by combustion of coke in the iron blast furnace). Limestone is considerably less expensive than lime because it is not calcined; however, lime is far more difficult to handle and reacts readily with water. Dolomite and dolomitic lime are used for their magnesia content. Fluorspar is a neutral flux often used as an additive in steel- making slags to improve fluidity, but it has also been used in combination with lime as a primary slag in electroslag refining (Duckworth and Hoyle, 1969). Fluorspar is available in several grades, with the lowest grade often used for steelmaking. Theoretical considerations, experimental results and plant experience have lead to a new design concept called the unit reactor approach. It has been successfully implemented to the largest flotation cell on the market, the 4000 cu ft OK TankCell. The work has also lead to a new operating idea called size specific flotation. The paper discusses these concepts in the view of full scale field experiments. In large scale porphyry copper flotation experiments conducted in Chile the new large TankCell with its new low energy input Free Flow mechanism out-performed the more conventional designs. The recovery was substantially better, especially in the coarse fractions. In some experiments the difference in total recovery was up to 10%-units and the savings in total energy consumption at the same time over 40%.

The Archaean Yilgam craton has been a major gold province since 1888 with production and reserves of about 4700 tonnes. Production in 1994 was about 135 tonnes. The largest deposit, the famous Kalgoorlie Golden Mile, contains about 2000 tonnes of gold. About 40 deposits containing more than 35 tonnes (1Moz) of gold has been discovered so far in the Yilgarn. Most of the craton has deep weathering profiles inherited from previous tropical climatic conditions, modified by subsequent arid conditions. A mantle of transported material covers much of the variably stripped weathering profiles. Resolving the geology of the volcanic-sedimentary units (greenstones) and the structures that host the gold mineralization relies heavily on aeromagnetics and shallow drilling.

How many times have you said or heard someone say, 'What are you doing right now?" And how many times have you heard the reply, "Breathing air!" Probably never, or at most only a few times in a facetious vein. Breathing air is absolutely necessary to sustain human life, and yet most of us take it completely for granted. Beginning ten to fifteen years ago there was growing concern on the part of the general public that pollution of the air is a threat to man's health and welfare. Concurrently much thought and effort was devoted to the task of defining and resolving the problem of air pollution. In the course of this effort it has become quite clear that our National policy in this regard should be founded upon three basic axioms: (1) Action should not be postponed until emergency conditions make it compulsory; (2) The air cannot be purified after it is polluted, and (3) Air pollution control cannot be accomplished without close cooperation among all levels of government, industry, and the public.

Application of an SEM-based methodology suggests a reason for past contradictory results on the relationship between quartz and coal workers' pneumoconiosis (CWP). Results indicate that coal seams with the same bulk analysis of quartz have different amounts of surface-available quartz. Our research shows that the amounts of pure and surface-available quartz in a respirable dust vary directly with the rank of a coal seam, except in the case where rock materials are mined. The estimated amount of surface- available quartz therefore provides a measure of the quartz-related fibrogenic potential of a respirable dust.

The Getchell Mine is located in Humboldt County, Nevada, approximately 70 km northeast of Winnemucca. Slope management has been a critical factor in the feasibility of mining operations since they began in the early 1930's. Although underground entries began in 1937, underground production was limited because hanging walls consisting of clay gouge and crushed rock tended to collapse into the workings. Controlling these collapses required extensive timbering. Due to these poor ground conditions, surface open-pit mining was the primary mode of production from 1938 to 1967 with only minor contributions from underground operations. Mining was suspended in 1967 due to economic pressures. From 1960 to 1967, the previous operator mined the property in three open pits. Initially, these pits were constructed with footwall benches, but these failed. These failures were aggravated by construction of 60° slopes in footwalls with shale partings dipping into the pit at 43° to 60°, resulting in large planar failures and wedge failures. In response, the previous operator adopted a slope parallel to the bedding planes.

The testing of different kinds of process flow configurations and cell volumes in industrial scale flotation processes is in general difficult to accomplish and even expensive to perform.1 Also the lack of suitable measuring and analyzing devices often prevents effective comparison of results. Nowadays, the existing flotation processes contain, almost with exception, several return loads which result in difficult control of the process. There exist two good reasons for testing different kinds of flotation circuits at the Outokumpu Oy Kotalahti mine. Firstly it is easy to change the process flow configuration without any additional installation work and secondly, the concentrator is highly automated with magnetic flow meters, on-line analyzer and process computers. In this presentation, open circuit flotation is compared with the classical closed circuit flotation which contains return loads. In additional, the effectiveness of computer control in both cases is estimated and also the effect of cell volume on flotation results is examined.

Asia is experiencing a growth market for industrial kaolins for use in ceramics, paper, plastics and other market sectors. Export and import data for the Asian countries reflects this growth with the Peoples Republic of China as the major exporter of clays and Japan the major importer. The growth in tonnage and product sophistication of kaolins produced on Belitung in Indonesia is typical of developments that are taking place in many locations in Asia. This region therefore presents opportunities for mineral processors and a challenge for the exploration geologist, the mineral processing engineer, the mineral economist and the marketing specialist.

Sublevel stoping generally is a large-scale open stoping method. It sometimes is referred to as long-hole or blasthole stoping. This method usually is applied to strong ore bodies that require minimal support and are surrounded by strong country rock. The ore body should be fairly regular in shape and well-defined. The dip of the footwall normally should be sufficient to allow broken ore to gravitate freely, although the method has been adapted successfully to mine some of the flatter ore bodies. Sublevel stoping is not dependent on the width of the ore body. Widths of less than 6 m (20 ft), however, make the utilization of long-hole drilling techniques more difficult. The main criteria for sublevel open stoping are competent ore and stable host rock, regular ore boundaries, and a footwall dip that exceeds the angle of repose of broken ore. Basically, the method entails providing access to the ore body at various subintervals between the main haul- age levels in order to drill and blast the intervening ore. Stope drilling is carried out from drilling drifts on the sublevels, and the ore is blasted in slices towards an open face, which generally is vertical on the downholes and may be inclined towards the open face for the up holes. The blasted ore gravitates to the bottom of the stope and is collqcted through drawpoints. Fig. 1 illustrates a typical sublevel stoping mine. The method requires extensive ore body development with relatively high capital expenditures. How- ever, much of the development is in ore, and production costs are comparatively low. Productivity rates are in the 13.6 to 27.2 t (15 to 30 st)/manshift range. The drilling, blasting, and loading operations are performed independently, and equipment utilization is high. Large outputs can be obtained with few units and limited personnel. Dilution with waste rock may occur if ore boundaries are irregular or if caving occurs, but 100% of the ore within the stope usually is recovered. Pillar recovery sometimes is a problem. It is at this point that most of the waste dilution occurs. At the time data was gathered for this report (1973), the sublevel open stoping method was used by more than 21% of the metallic ore mines in the US (9 out of 42 mines producing 1088 + t/day (1200 + stpd) and provided more than 12% of the total metallic ore production from these mines. DEVELOPMENT Access to a sublevel open stope mine is gained by a slope or shaft that is normally sunk in the footwall of the ore zone, away from any possible effects from blasting or other production operations in the stoping process. Main level interval is selected and usually varies from 45.7 to 121.9 m (150 to 400 ft), depending on the vertical extent of the ore body, the number of drawpoints required to maintain output, and the eventual stope height. The main haulage drifts are located at the bottom of the stope horizon, either directly in ore or in the footwall, with access crosscuts at intervals. Raises are driven up through the ore to connect to the level above and provide access and ventilation to the sublevel drilling drifts, which are driven horizontally in the ore for the length of the stope. Depending on the drawpoint system to be used, the stope is either undercut by a raise-and-cone system, from a scram or slusher drift, or simply provided with a drilling drift from which access is made at intervals to the main haulage level. A raise driven either at the end or in the middle of the stope (if stope access is from both ends) provides a start for opening up a vertical slot across the stope to establish a free or open face for long-hole drilling and blasting. Sublevel stoping has proved to be a relatively safe and economical system.

The New Celebration Gold Mine near Kalgoorlie in Western Australia was commissioned in late 1986 and the CIL treatment plant currently processes 1.7 million tonnes of gold ore annually. The 1988 geological resource was 13.2 million tonnes grading 2.37 g/t Au in several open pittable deposits with the bulk of gold mineralisation occurring as a stockwork in Archaean quartz feldspar porphyry intrusives along a major regional shear zone. Mine planning, scheduling and operating techniques have focused on minimising open pit dilution whilst maintaining production requirements. This paper describes the successful achievement of this aim in extremely variable ground conditions and orebody widths.

Attempts were made to improve the recovery of coarse phosphate particles by adding selected frothers or precoated fine particles. A number of frothers for floating, specifically coarse coal and metallic minerals, were examined. Two of the frothers increased the flotation recovery of coarse phosphate by 3% on bench scale. Plant evaluation yielded improvements in overall phosphate recovery in the presence of these frothers. Addition of fine particles was also found to enhance coarse flotation under given conditions. Size, amount, and hydrophobicity of the fines added was determined to have a major influence on the coarse flotation efficiency. The grade of the product with and without frothers and particulate additives remained the same. Change in froth stability and viscosity are believed to be the primary reasons for enhanced recovery of coarse phosphate particles.

Artesian pressures of unusual magnitude occur in the deeper aquifer units beneath a considerable part of southeastern North Carolina and extend into South Carolina. The high pressures were first observed in 1968 in wells near Wilmington that were drilled to "basement" rock and which were screened between depths of about 260 to 300 m (meters). The pressure was recognized by Peek and Heath1 as an anomalous geo-pressure and unrelated to recharge conditions in the region. Since 1973, much new information has been obtained on hydro-geologic conditions in the area and a preliminary report on the deep artesian pressures was published in 1975.2 Most of the basic information has been obtained from groundwater research stations constructed by the Groundwater Section of the North Carolina Department of Natural and Economic Resources as a part of Ground-water studies in the Coastal Plain Region. These stations consist of an exploratory well drilled to basement rock with wells completed in each principal aquifer unit to provide data on aquifer characterestics, water levels and water quality.

A column reactor was applied for leaching with simultaneous flotation a complex sulfide ore in ferric sulfate-solution under oxygen pressure. Elemental sulfur and the flotable, nonreacted sulfides were collected in the froth and separated from the insoluble, nonflotable gangue minerals and solid particles formed during pressure leaching. The two-stage laboratory unit, built from Corning-glass elements is described. Results of the continuous leaching/flotation experiments conducted under 20-40 psig of oxygen pressure are presented, as are supplementary batch leaching experiments with iron precipitation under 150 psig oxygen pressure. High zinc extraction to solution and near total accumulation of lead sulfate in the nonflotable fraction are easily attainable. Relatively poor flotation of pyrite is probably caused by the partial oxidation of its sulfidic sulfur to sulfate.

A review of the evolution of the North American sulfur market's structure serves to dramatize the kinds of changes that the market is undergoing at the present time. In 1970, the U.S. accounted for 23 percent of world sulfur production. This share has not declined dramatically--to 21 percent in 1993--but the U.S. developed into a substantial net importer of sulfur in the past 25 years. U.S. output was based on large deposits of native sulfur in the western part of the Gulf Coast region with production by the Frasch mining process. Producers of Frasch, or on-purpose sulfur, have historically absorbed cyclical fluctuations in sulfur demand. Two major changes in the North American market from that time to now have been the introduction of increasingly large quantities of by-product sulfur from petroleum refining and natural gas processing operations in the U.S. and Canada, and that Canadian producers have become the marginal supply source for U.S. sulfur consumers.

During the last 30 years, a dramatic increase in the supply of byproduct sulfur substantially reduced the share of the market held by Frasch-mined (non-by-product) sulfur in North America. Even though Canadian sulfur production is not discretionary, the geographic remoteness from the large US markets and the high transportation costs associated with delivering sulfur to those markets has meant that Canadian producers act like discretionary producers in many important ways. They have effectively replaced the US Frasch producers as the cost-marginal sulfur source to large US markets. Implications of this change in market structure include more volatile sulfur prices and longer sulfur price recessions.
