A challenge on many industrial flotation circuits is controlling concentrate mineral grades using measurements that are provided infrequently. The mineral grades in concentrate streams are a critical performance indicator in any industrial flotation circuit. Failure to achieve concentrate mineral grade targets has significant financial consequences on a flotation operation. A violation of a gangue mineral grade limits by the concentrator may also result in significant penalties. Online grade analysers which can provide reliable real-time grade measurements are essential for effective grade optimisation. The Blue Cube MQi analyser (by Blue Cube Systems) has recently come to prominence for its ability to rapidly determine mineral concentrate grades online and in-stream. This has provided excellent insight into the actual process dynamics in the flotation circuit, and hence enabled real-time control and optimisation of concentrate mineral grades. MintekÆs Measurement and Control division, specialising in advanced control solutions for mineral and metallurgical industries, has had the opportunity to link the FloatStar Grade-Recovery Optimiser to the measurements provided by the Blue Cube system at a nickel concentrator plant. The project initially focused on stabilising the final concentrate grade without directly accounting for nickel recovery. The site later procured additional Blue Cube MQi analysers, resulting in the expansion of the FloatStar Grade-Recovery Optimiser to incorporate recovery component in the performance. This site already had the FloatStar Grade-Recovery Optimiser for several years, but the original grade measurement device was unreliable and ultimately failed. Fatality prevention and zero harm are key focus areas for resource companies. However, they remain an ongoing challenge for high hazard industries, including the mining sector. As one example the latest International Council on Mining and Metals (ICMM) Safety Performance Report shows that, while there is generally a declining fatality trend since the organisation commenced publishing data in 2012, there were still 33 fatalities across member companies in 2022 (ICMM, 2023b). Every fatal incident brings significant pain and suffering to the colleagues, friends and family of those involved, however the failures and causal factors leading to each incident are frequently very familiar to people working in the industry, particularly those tasked with managing risk and safety. In the words of Dame Judith Hackitt, Grenfell Tower Fire Investigator, ‘there are no new accidents – just different people making the same mistakes because they haven’t learned’ (Hackitt, 2022). The recurring themes in terms of the nature of fatal hazards and their associated causes across the mining sector show that, despite improvements evidenced by the favourable declining fatality trends over the long-term (in general and outside of catastrophic multiple fatality events), changes are required to achieve fatality elimination and the aspiration of zero harm. This extended abstract and the accompanying presentation apply a mining frame of reference to the idea that there are no new accidents, examining how this applies within the industry and why the lessons from past failures are not always well understood or applied to prevent the reoccurrence of material unwanted events. Many contemporary mining and resources organisations have extensive internal incident history data and a wide variety of information on fatality incidents is publicly available, including the annual ICMM Safety Performance Report. This extended abstract and the accompanying presentation explore how this existing data can be used effectively to learn from the past and proposes practical ideas regarding strategies for successful knowledge transfer to the workforce as a key component of preventing history repeating and improving safety performance across the industry. As noted by ICMM, ‘sharing lessons from failure is vital to improving health and safety, but it’s not enough’ (ICMM, 2023a).

A major Ventilation Upgrade Project commenced at the Fosterville Gold Mine (FGM) in 2017 to service the ongoing ventilation requirements of the underground mining operation. The project consists of three large diameter ventilation raises totalling 1100 metres, and new axial primary ventilation fans to upgrade the mine ventilation system. With a mining depth of 1200 metres and a thermal gradient of approximately 2.7 °C per 100 metres of depth, the completion of the Ventilation Upgrade Project is key to the ongoing success at FGM. The Ventilation Upgrade Project will allow continued mining, including high-grade gold deposits, lower in the mine where heat is increasing. Figure 1 below illustrates the underground workings at FGM, and provides a general view of the ventilation upgrade works to establish the new exhaust system from the lower workings to surface. Whilst raisebore pilot drilling the H4760 Return Air Rise (RAR) from surface to 395 metres, the raisebore drill string became stuck in the raisebore pilot hole at approximately 300 metres down hole, presenting a significant technical problem and delays to the project. This Case Study provides an overview of the recovery process undertaken at FGM including the recovery of the stuck drill string. The value of the stuck in-hole equipment, the investment in the shaft site preparation, the collar stabilisation, the underground development to the base of the ventilation raise, the surface footprint prepared for the Primary Fans construction and the limited available space on the site were all motivations to recover the raisebore drill sting and ultimately recover the ventilation raise location. More importantly, the long lead-time required should a new shaft location be the final solution, and the delay this would incur to the ventilation upgrade was unacceptable. Recovery of the drill string was not possible using the capability of the Contractor’s Raise Drill. An alternative method for freeing the drill string was required. A significant amount of engagement with industry was undertaken to seek out potential solutions and draw from the experience of others. All methodologies available were considered and assessed. Ultimately, after several attempts at freeing the raisebore drill string using various methodologies, Adams Drillers and Clear Cut Interventions were engaged to provide a turnkey recovery solution.

Northern Star Resources (NSR) operates several operations utilising narrow vein stoping operations in the Kundana area. Stope dilution performance is variable across these operations, with unplanned dilution reducing individual stope economic viability, increasing risk to personnel and machinery operating in and around the stope and delaying the overall stoping sequence. Technical investigations were undertaken to objectively understand the variables which had the largest impact on stope performance and then use the findings to help develop mining strategies to reduce dilution. A stope reconciliation process was developed to compile key stope performance information such as stope design parameters, geotechnical and drill and blast factors. These were compared against the measured dilution. Production targets necessitated a high turnover of stopes in the operations, and hence a requirement for an expeditious process and mine design software with automated tools. The output of these results, together with numerical variables from geological, geotechnical and drill and blast data were analysed via multivariate regression. The analysis identified the factors with the greatest influence over stope dilution, and a forecast model was consequently created. Confidence was gained in the model when the subsequent stopes performed in line with prediction. Different drill and blast designs were subsequently trialled and found to reduce dilution. The model was then updated with the new practices, facilitating further regression analysis to identify revised key controlling factors. The subsequent production results however, displayed a poor correlation with the prediction from the revised model. Inconsistencies in drill and blast designs and execution were identified as key factors in the dilution performance. This was an important finding as the correct implementation of the design is critical and this has become a key focus for future stoping. Overall the technique presents an objective process to identify causal factors of stope dilution resulting in improved economic metrics for stope production. The process will be applied to other similar Northern Star operations.

Traditionally occupational health and safety (OH&S) has been managed for the direct benefits of reducing injury and illness to workers on mine sites. In the current skill shortage climate it is important to recognise the value that OH&S management can have in assisting mines in minimising their exposure to this issue. In times of skills shortage turnover is high as mines poach personnel from each other. This leads to loss of corporate knowledge and loss in productivity through the need to train new personnel in mine-specific operating procedures. In addition absenteeism can significantly disrupt production as there may be no ready replacements available for key jobs. Pressure will be placed upon workers to remain at work even though they are not completely fit for work, leading to presenteeism and consequently loss in productivity. In addition to the threats to routine operation, the loss of management and professional staff can significantly increase catastrophic risk through loss of experience and corporate knowledge. There is increased pressure to focus on events that occur regularly in the here and the now and underestimate the rare, or unlikely, events.The theme of this paper is that by properly addressing OH&S management at mine sites, companies can minimise their exposure to these problems. Put simplistically, a safe and healthy mine is a sustainable and productive mine. Case studies and relevant literature will be used to support this premise.CITATION:Cliff, D and Bailey, K, 2012. "This paper presents a case study demonstrating active geotechnical pit slope management at the now completed Carina Iron Ore Project operated by Mineral Resources Ltd (MRL). The site is located 105 km north-east of Southern Cross, Western Australia. The paper is primarily focused on the slope stability failure modes and the remedial measures employed to maintain mining.Between a quarter and a third of the excavated slopes were impacted by some form of instability that was beyond expectations of the intended slope design criteria and common practice. Ongoing mining was supported by intensive geotechnical pit slope management including close cooperation between geotechnical, mine planning and operations staff to achieve timely and practical mining solutions.Mining encountered varied slope failure mechanisms which required a combination of remedial solutions comprising: horizontal depressurisation holes, deferral of excavation, unloading, buttressing and catch berms.The root cause of the challenging slope performance was a mismatch between the predicted and encountered geotechnical model, resulting from a failure to recognise the critical mechanisms that control slope stability.Despite the extensive zones of instabilities at completion of mining, full ore recovery was achieved without major delays and in a controlled and safe manner.Citation:Fowler, M, Strang, D and Jaine, R, 2016. In addition to volatile commodity prices influencing decision making, in recent years, social and environmental performance have become important decision drivers within the mining industry. There are increased demands from stakeholders, communities, and governments to ensure that raw materials are mined, processed and refined responsibly. The industry also faces the challenge of better managing its waste, which has led to severe consequences when not done properly. The high commodity prices of the late 90s and early 2000s encouraged larger economies of scale and drove mining companies to extract large volumes of rock for separation and concentration at the downstream processes. Over time, as ore grades dropped, the cost of mining escalated and tailings volumes grew. This arduous process also caused strains on recovery cost and performance. Scarcity of water and energy resources coupled with increased production demands and higher operating costs are putting pressure on companies to find innovative ways to increase mining intensities. Hatch’s value chain optimisation methodologies, specifically Grade Engineering® and early waste rejection of the mine, have been of interest to intensify operational performance and minimise waste generation. Enabling preconcentration of the plant throughput, the methodology has significant potential to improve overall metal production and reduce water and energy intensities, whilst minimising wet tailings. Using representative data from real world mines, this paper reports on a case study that evaluates the impact of preconcentration using sensor-based ore sorting on downstream process improvements as well as energy, water and tailings reduction. Financial and economic viability of this technique is presented for both brownfield and greenfield implementation cases. The results show that business case for preconcentration is best when: the distribution of metal in the run of mine ore is negatively skewed and left tailed; mining costs are low; energy, water and tailings management costs are high; and low cost methods of recovering pay metal from low grade material are available.

"In today’s mining industry, the pressure of reducing costs and increasing production while maintaining high safety standards is at the forefront of operational endeavours. A combination of specific mining knowledge and technological advances has helped enable this notion. Fleet management systems have been developed to enable sites to better measure and manage production to improve operational efficiency.The integral part to managing production is having accurate, readily accessible data. This empowers key stakeholders in the management team to drive production reporting, materials tracking and budget forecasting.If a site does not run a fleet management system, manual input is heavily relied upon for production data, which introduces increased risk of human input error. Errors can drive outcomes that don’t achieve increased production at a reduced cost. Alternatively, a fleet management system can have improved accuracy but involves a high monetary investment. The Broken Hill deposit is located in Australia in the far western region of New South Wales. The deposit was found in 1883 and has been mined continuously since its discovery. It is considered the largest lead, zinc, silver deposit that has been discovered in the world and originally contained around 340 Mt of ore at a grade of around 12 per cent lead, ten per cent zinc and 150 g/t silver. The material remaining in the deposit represents only a fraction (perhaps ten per cent) of the original resource. Several of the world’s largest mining companies can trace their origins to the Broken Hill Field. The remaining resources on the CML7 consist of two large lower grade resource lodes that have not been subject to mining, some scattered areas of zinc lode material and the pillars and remnants of the main lead lodes.The deposit occurs in metamorphic rocks of the Broken Hill Group a division of the Willyama Super Group contained within the Curnamona Craton. The Broken Hill Group rocks have undergone multiple phases of deformation. Metamorphism is granulite grade with major shears cutting through the deposit having been retrogressed to mica schists. The deposit lies in an antiform associated with the second phase of deformation.The resultant proposed mine is situated on Consolidated Mining Lease 7 (CML 7) which occupies a strike length of 3.6 km in the central part of the 7.8 km long footprint of the orebody. Underground mining on CML7 ceased in 1975 with surface mining of silver chlorides in the weathered zone continuing until 1991. CBH Resources has been continually working to define and evaluate the remaining resource since 2002. This work has included four surface drilling programs, two underground drilling programs, an exploration decline, enormous amounts of compilation of historical data and detailed three- dimensional (3D) modelling of historical mining and the resource lodes. It is currently planned to commence production at the Rasp Mine in late 2011. The operations will produce from previously unmined lodes and from remnants and pillars.This paper primarily aims to encapsulate the processes involved from information acquisition through to resource estimation and also to highlight some of the challenges encountered and the specific techniques and practices used to overcome them. Many of the experiences at Broken Hill are applicable to all brown field sites.

The Ernest Henry Iron Oxide Copper Gold deposit is hosted in a large dilation between two moderately dipping shear zones found in the footwall and hanging wall of the deposit. These shears are controlled by a large diorite intrusion to the north-east and south-west of the deposit, formed ~1660 Ma. A shear zone dubbed the ‘Interlens’ uncovered through underground workings truncates the main orebody into two lenses, and is hypothesized to have formed in conjunction with the Hanging Wall Shear Zone and Footwall Shear Zone. All three shears dip moderately to the SSE (50–140) from open cut exposures through to the current mining horizon approximately 800 m below the surface. Routine geological work and a capital drilling campaign has identified a significant flexure in these sub parallel ore bounding shears with increasing depth. A shift in shear orientation in the Interlens over approximately 100 m vertically from ~50° to the SSE to a steep, easterly trend (~85° to the east) is observed in both drill core and exposures in the underground workings. It is hypothesized the diorite intrusive to the south-west of the deposit controls the shear zone geometries at depth. The geometry of the Interlens mirrors the surface expression of the Proterozoic package of the orebounding shears to the south-west of the deposit, suggesting the surface geometries of the shear zones are consistent with depth. Additional drilling to explore the validity of this hypothesis is planned for the near future to better understand the structural controls on the Ernest Henry mineralisation with increasing depth.

Evaluation of the construction method used for the pedestrian underpass construction at Ipswich Road, Brisbane, Australia is presented in this paper. The pedestrian underpass was constructed through the existing railway embankment between Harrogate and O’Keefe Streets on the eastern side of Ipswich Road. This underpass construction is part of the CLEM7 tunnel project (formerly known as the North-South Bypass Tunnel) which is the longest toll road in Australia at a total length of 6.8 km. The jacked box construction technique was initially identified as a preferred option during the tender and concept design phases. However at the final design stage, the cut and cover technique was selected as this method can reduce the construction time and cost significantly. The design and construction teams effectively managed the challenges, complexities and risks associated with undertaking cut and cover tunnel through the existing heavily utilised rail embankment. The on-site construction time for the underpass using the cut and cover technique was reduced to less than 42 hours.

The increasing availability of marker data in sublevel caving operations is changing the way draw can be managed to achieve positive recovery outcomes in these mines. This paper describes how marker data has been incorporated into flow simulations to help calibrate full mine ore recovery simulations for the forecasting and optimisation of recovery.Some flow modelling techniques do not use marker data, relying instead on the outcomes of physical and numerical experiments to guide assumptions about an idealised recovery flow shape. Other mines have access to marker trial data, but find it difficult to specifically link this marker trial information to their recovery models. This can make it difficult to create a link between the marker trial information and recovery forecasting and limits the potential value of the marker trials at the mine.In the simulations shown here, the marker rings are fully incorporated into the flow model, and can be tracked in the model. The model is compared against the real underground marker recovery information to give the user an indication of how accurately the flow settings in the model are reflecting the conditions in the experimental marker rings. BHP Billiton Iron Ore is one of the world’s premier suppliers of iron ore. It currently has operations in Western Australia and Brazil and also has some iron ore interests in West Africa.BHP Billiton’s Optimisation R&D team has developed a proprietary mine planning optimisation tool that uses a mixed integer linear programming (MILP). This tool is called Blasor and it is used to rapidly evaluate the resource development potential (RDP) scenarios for the company’s business development strategy. In addition it generates life of asset (LOA) mine plans for the company’s operations and growth projects. The Blasor optimiser maximises the discounted operating cash fl ow (DCF) as the objective function, subject to mining, processing and transportation capacity constraints and blended product quality constraints. The paper will outline how Blasor has been applied to the resource development potential (RDP) strategy scenario studies, growth project evaluations and LOA mine planning within BHP Billiton Iron Ore.The practical application of Blasor to resource planning optimisation within BHP Billiton Iron Ore has demonstrated the value of such a tool for optimising strategic directions through the evaluation of a number of carefully proposed scenarios and maximisation of operational decisions, such as cut-off grade, open pit development sequence, etc. Particularly for the company’s business development direction studies, Blasor plays the role of a fair valuation tool in that it discovers the most valuable resource development scenario, thus establishing a sound basis for comparing the economic merits of each scenario.

Jetcrete Australia Pty Ltd, shotcrete contractor took the initiative in 2008 to discontinue the use of ‘road registerable’ concrete trucks for all underground works in Australia and internationally.The rationale for this decision was basic, safety. Historically these vehicles have been and still are involved in far too many accidents; this obvious trend needs to be addressed by engineering out the root cause of the problem which in this case is suitability for purpose.Road registerable concrete trucks have been widely used for the delivery of concrete in Australian mines for many years prior to and after the inception of wet shotcrete as ‘in cycle’ ground support in underground excavations around 1992. With this inception came a wide interest in the shotcrete process and an influx of new shotcrete contractors, all using road registerable concrete trucks to deliver the large volumes of Shotcrete required for these services and in turn largely increasing the use of these vehicles underground. The road registerable concrete trucks were difficult to control and each trip underground required the operators to maintain good management of the inherent risks. We virtually had no reasonable alternative for the industry and could only encourage the operators to drive with soft controls such as, reducing overall speed to a maximum of 15 km per hour when declining, regardless of the road conditions, coming to a complete halt prior to turning into any sharp corners, remaining in the same gear both inclining and declining.

The empirical McCracken and Stacey stability and risk assessment method has been in use for more than 20 years to assess geotechnical stability issues for raise bored shafts. The methodology has been applied with various levels of success. Increasingly, trends in mining and civil tunnelling have been towards larger diameter, single pass, raise bored shafts to rapidly provide means of ventilation, material movement (ore passes), or emergency egress. Identifying and assessing potentially problematic zones before raise boring commences can allow for appropriate risk-based decisions on construction, or to investigate alternative solutions. The implementation of a proactive solution to raising through an identified weak zone is preferred to the application of an engineered solution to an area subjected to substantial instability or failure. Methods for investigating potentially problematic areas are discussed in this paper based on AMC’s benchmarking data. Proactive investigation programmes resulting from these investigations are also discussed. Previous publications discussing the benchmarking data have concentrated on Australian case studies. Increased efforts and recent updates have been made to expand the database to include international case studies. This has provided further refinement of lower bound ‘raise bore rock quality index’ (QR) values, and various rock mass parameters for stability assessments. Grade control is a process that requires the interaction between many components of the mining operation and successful grade control is only realised through good data, robust systems, and a well-trained, motivated and disciplined workforce.The Mt Magnet Gold Project Pty Ltd (MMGP), a wholly owned subsidiary of Ramelius Resources Limited, started mining at Mt Magnet in February 2012 with high expectations for success. Positive cash flow would fund an aggressive exploration and mine development program in the area to increase mine life. Disappointment followed when after start-up the operation had issues achieving budget grade and realising the predicted grade of the mined ore. In January 2013, things started to improve. During the next six months, gold production steadily increased with successive months achieving record production. What had changed?Improvements started following the realisation that controlling grade was not the sole responsibility of the mining geologist and that everyone on-site has a role to play. Over the 40 years that the author has been engaged in the Australian underground coal mining industry, there has been a continuous improvement in safety performance as the sector has grown in size and productivity. The underground environment hosts many hazards with significant risk profiles that are kept in check by effective health and safety management systems that have evolved largely following major underground disasters. Observations are made on the changes observed for specific aspects of the underground coal sector, with particular reference to ventilation issues, and predications are made as to the needs of the future.CITATION:Robertson, B, 2013. Record iron ore prices and a renaissance of infrastructure in the Pilbara have Flinders Mines poised to move from explorer/developer to an iron ore producer. The potential of the company’s Pilbara Iron Ore Project was first highlighted by the announcement of Rio Tinto’s Caliwingina North deposit to the north and the Fortescue Metals group’s Serenity deposit in the southeast. The success of Flinders Mines’ exploration has been based on incorporating landscape studies into an existing geological framework, and using this to understand the iron-rich materials that were emplaced during the Tertiary. The development of the Delta and Eagle deposit resource models highlights this process. There are three main mineralisation styles at the project; detrital iron, channel iron and bedded iron deposits. They have textural and mineralogical similarities with comparative deposits in the region. Gaining an insight into the geological processes unique to detrital iron deposits and more distal channel iron deposits has been important to understanding the geometallurgy of the mineralisation. The model sits comfortably with other regional oregenesis models and allows a contextual framework to be developed around the timing of deposition of the Fe-rich detritus and pisolitic nodules that dominate the deposits. The exploration model also allows Flinders to explore more effectively for bedded iron mineralisation developed prior to, and since the development of the Tertiary weathering landscape.

Fortescue Metals Group flagship operation, Cloudbreak, is located in the Pilbara region of Western Australia. The mine is capable of delivering an annualised production rate of 40 Mt/a. Positioned 50 km to the east of Cloudbreak is Fortescue’s Christmas Creek operation, which has a combined processing capacity in excess of 50 Mt/a. Following on from these successes, Fortescue advanced development of its western Pilbara mines at the 60 Mt/a Solomon Hub. These processing installations place Fortescue in a strong position to deliver on a guidance of between 155–160 Mt/a for financial year 2014/2015.As Fortescue enters a phase of asset consolidation and debt repayment over expansion, the business is commanding stronger performance through enhanced plant operating practices and mine site cost reduction initiatives. To this end, the strategic placement of instrumentation is an area being given greater consideration. Access to real-time information pertaining to conveyed materials is of major interest to operators for making timely process control decisions. Fortescue has recognised the importance of accurate and up-to-date process information about their ores and hence, has adopted the use of Scantech’s Geoscan mineral analysers at two of their mines.There are two Geoscans at Cloudbreak; one on the ROM feed conveyor and another on the final product conveyor. These allow the in-feed material grade characteristics to be assessed. Moreover, the final product stream can be monitored to ensure product quality is maintained within acceptable quality tolerances. A third Geoscan is installed at Christmas Creek mine on a conveyor that transfers final products to Stockyards.This paper discusses the use of the Geoscans in these installations, including benefits to Fortescue’s operations, as well as functional considerations such as the impact of maintenance and calibration. A discussion of recent advances by Scantech is provided including enhancements to the calibration process and minimising plant downtime, along with a summary of results obtained.CITATION:Balzan, L, Beven, B J and Harris, A, 2015.
