Each year, companies spend millions of dollars on safety training and on safe equipment, money that is generally well spent. Despite that, we still make mistakes. What happens if the mistakes are made not because of inadequate training, but because we are hard-wired to act in a certain way? If you read any of the ?Because of Human Error? text boxes in this document, situations like these may look familiar in your own workplace. ?The issues of risk, health, safety and the environment are getting more attention now than ever before, at all levels of the organization,? said Calvin Price, vice president of Global Health and Safety at LNC-Lavalin Inc., a company with 21,000 workers worldwide. Most would agree with Price that attention to safety is increasing because responsible companies care about their workers and the workers? colleagues, families and communities. However, cynics might say that the safety spotlight is powered by the corporate bottom line. Direct costs created by workplace injuries total $51.8 billion annually and include workers? compensation payments, medical expenses, civil liability damages and litigation expenses. This sum represents a staggering one quarter of each dollar of pretax corporate profits. Indirect costs may run as much as 20 times the direct costs of on-the-job accidents and illnesses. Examples of indirect costs include training replacement employees, investigating the accident and implementing corrective measures, lost productivity, repairing damaged equipment and property, and costs associated with lower employee morale and absenteeism. What?s interesting is that up to 90 percent of incidents are due to human error, not faulty equipment or other factors.

The analytical approaches used to solve a specific problem are dependent upon the information required, coupled with limitations imposed by available facilities, costs, and time. The organic matter associated with ore deposits has the potential to yield information for two problems. Information can be obtained to provide guidelines for prospecting, and, secondly, to solve genetic problems. A major potential of organic matter in providing prospecting guides lies in the high thermal sensitivity of organic matter in sediments which are metamorphosed at grades lower than greenschist facies. The organic matter can then be used to map thermal anomalies potentially associated with mineralization. The analytical approach is to determine the thermal maturity of the total organic matter, or of specific fractions, using techniques which are relatively quick and inexpensive. Consideration of genetic models (for example, reduction, complexing, involvement of oil field brines) generally requires more specific information than the overall thermal maturity of the organic matter, necessitating infor¬mation on molecular composition and structure. The facilities, time, and cost of this information are greater than for mapping thermal anomalies. The analytical techniques discussed include elemental analyses, spectroscopy (infra-red, visible and ultra-violet), Fourier Transform infra-red, nuclear magnetic resonance, extraction, pyrolysis techniques, chromatography and mass spectrometry.

"A carefully planned laboratory test program is the foundation for the successful development of a sulfide flotation process. In this paper, the development of a copper-molybdenum flotation process is described. However, the concepts discussed can be applied to most sulfide flotation systems and to many industrial minerals projects as well. Each step is examined, beginning with sample procurement and covering sample preparation, flotation test preparation grinding, scoping flotation tests for reagent scheme selection, rougher flotation parameter evaluation and cleaner circuit evaluation, through to locked-cycle testing and pilot plant design and operation. Ore variability and modeling to determine the response of various ore types to the optimum conditions established is also examined.IntroductionA carefully planned laboratory test program is essential for the successful development of any sulfide flotation process. These programs usually begin with a series of batch tests on several ore type composites to evaluate appropriate reagent schemes and delineate optimum rougher flotation conditions, such as primary grind size, flotation pulp density, flotation pulp pH and retention time. Cleaner parameters such as regrind size requirements, cleaner pH, pulp density and number of stages are then defined. Semicontinuous locked-cycle tests are performed using the parameters established in the batch tests to determine the effect of middling streams recirculation on flotation performance. The data generated and the experience gained in the locked-cycle tests may be used to design and operate a continuous pilot plant if relatively large amounts of concentrate are required for further process development, such as optimization of byproduct molybdenum production from bulk copper-molybdenum concentrate.A comprehensive ore variability program is undertaken to determine the response of various ore types to the optimum conditions established. These ore variability programs often test hundreds of samples in standardized batch rougher and cleaner tests to ensure that the orebody is well understood.This article describes the development of a copper-molybdenum flotation process, but the concepts discussed can be applied to most sulfide flotation systems and many industrial minerals projects. "Underground structures such as square/arch steel sets, king beam structure, etc. are widely used for the roof support in underground mines. In designing and calculating the required carrying capacities of these underground structures, how to identify the potential rock loads on the structures over long term is a challenging question for ground control engineers. Based on design practices of both surface and underground structures, the authors have designed and applied appropriate load factors in underground structural design projects to accommodate different rock types, entry/intersection dimensions, roof geologic conditions, availability of roof bolts and water, etc. Classification of load (reduction) factors for large-span underground intersections are presented for reference. Success of designed underground structures in effectively controlling roof stability over years indicates the applicability of the load factor concept for underground structural design, and its same significance as of the load factor specifications in ASCE/SEI 7-10, ANSI/AWC NDS (2015), and AASHTO LRFD Bridge Design Specifications (2012). The load factor concept will facilitate ground control engineers to appropriately utilize popular structural design software, such as RISA-3D or -2D to efficiently design and analyze underground structures.LOAD FACTORS IN STRUCTURE DESIGN APPLICATIONSIn structures, loads cause stresses, deformations, and displacements. Assessment of their effects is carried out by the methods of structural analysis with the help of load factors. Loads can be increased or decreased by load factors to meet the requirement that design strength be higher than maximum loads. For example, in designing a staircase, a dead load factor may be 1.2 times the weight of the structure, and a live load factor may be 1.6 times the maximum expected live load based on the Load and Resistance Factor Design (LRFD) method. The paper reviews current mining subsidence research at The Department of Mining Engineering, University of Notting- ham. Special attention is being devoted to prediction of ground movements between the mining horizon and the surface using physical and computer-based models. The paper presented discusses linear and non- linear finite element methods and compares results with those from physical models and field observations. Special attention is focussed on the determination of the effect of caved zones on overlying and underlying geological structures. The paper presents data from comparisons with U.K. case histories.

"While the American Exploration & Mining Association (AEMA) was preparing to close one chapter and begin another during its annual conference in Spokane, WA Dec. 3-7, word of a significant victory on an issue that has been important to the association for many years came from the halls of Washington, D.C.During a keynote address on Dec. 6, Joe Balash, assistant secretary of Land and Minerals Management for the U.S. Department of the Interior (DOI), told a sold-out lunch crowd that the DOI had released final drafts of proposed revisions to the Obama-era greater sage grouse conservation plans. The revisions suggest removing hundreds of thousands of acres of federally protected habitat in Utah, and easing restrictions on energy development and other activities, including mining, in Colorado, Idaho and Wyoming.The protections of the sage grouse were put in place by the Obama administration in 2015. The sage grouse, a prairie fowl, is considered by conservationists to be a key indicator species for the health of America’s sagebrush ecosystem. The previous plan to protect the ground-dwelling bird imposed restrictions to development in sage grouse habitat, but did not place the birds on the endangered species list — a move that many believed would have imposed far more rigid rules.AEMA, and its executive director Laura Skaer, led the way in the fight against the Obama-era greater sage grouse plan, arguing that the plan unnecessarily hurt economic development.“We were the first organization to point out that the greater sage grouse was being used to stop mining, oil and gas production and grazing on public lands in the interior mountain west,” said Skaer who is retiring from AEMA with a significant legal win on this front. “Early on, we cautioned that the proposed Land Use Plan Amendments would be worse than a listing (of the sage grouse) under the Endangered Species Act. Very few believed us until they saw the plans with all of the restrictions and the recommendation for a 4-million-ha (10-million-acre) withdrawal from mineral entry. We were the only organization that filed comments on all 88 Land Use Plan Amendments in 11 states covering more than 24.2 million ha (60 million acres).”"

The kinetics of gold dissolution in iodide solutions with oxygen in an autoclave was investigated. The leaching process was described by a gas-liquid-solid heterogeneous reaction. The effects of stirring speed, temperature, oxygen partial pressure and lixiviant, concentrations on the overall leaching rate was investigated. It was found that the dissolution of gold could be represented by a heterogeneous chemical reaction. Under the conditions studied, the apparent activation energy ranged from 34.7 to 54.3 kJ/mol. Experimental findings were in good agreement with the kinetic model proposed.

Intensive efforts to develop potash use in agricultural production were begun in the US 50 years ago. Early growth was rapid and continued well into the 1960s. Many soils were inherently low in K, so crop responses to fertilization were often dramatic. Deficiency symptoms were common. Per-acre use has continued to increase in recent years, but annual growth rates have slowed. Soil buildup has occurred in some areas. Poor economics in the 1980s have been difficult to overcome. Efforts are now well under way to develop potential markets around the world.

The Sales Comparison Approach to estimating the value of a mineral property must always be attempted1 . In addition to other parameters, the approach depends heavily on the availability of transaction data. The consideration for the mineral property must be in form of cash, cash equivalent or other consideration. In other words, “What dollar amount did the property sell for?” Mineral properties are generally real estate, albeit frequently buried deeply under the overlying surface real estate, unless the property may be a quarry. In some states, transactions must be recorded in county courthouses, and the sale consideration must be included. In such cases, the appraiser’s job is made easy. In other locations, the sale considerations are withheld in the courthouse data; but occasionally a local transfer tax is collected and recorded. Again, the appraiser’s job is made easy by back calculation with knowledge of the legislated transfer tax percentage. The worst problem occurs when the consideration for a mineral real estate sale is totally withheld. That may occur in so-called nondisclosure states. The adsorption of gold onto activated carbon in iodide solutions was studied. The adsorption rates were determined as a function of temperature, salt concentration and pH, as well as activated carbon concentration. A kinetic model was developed for the adsorption. The adsorption rate was believed to be controlled by a combined mechanism of film diffusion and pore diffusion with the film diffusion having the most significant effect. Experimental findings were in good agreement with the model developed. The apparent activation energy was found to be 14.9 kJ/mol.

In flotation practices, as particles become finer, such as smaller than 20 μm, process efficiency declines markedly. Selective agglomeration of fine particles is a technique to enhance the recovery of fine valuable minerals in flotation circuits. Different agglomerating methods have been introduced and tested. One method that has recently received much attention is to make use of magnetic force to aggregate fine particles. A selective aggregation of fine paramagnetic particles, such as sulfide minerals, can be achieved in the high-intensity magnetic field prior to or during flotation. The method is simple, selective and effective for fine particles and has low operational cost, though a minimum magnetic susceptibility for fine grains is required to promote the process. In this paper, the implementation of the ProFlote magnetic conditioning device in Boliden’s Garpenberg old concentrator in Sweden, where a complex massive sulfide ore is treated, is presented and discussed based on Boliden’s Garpenberg plant. The results showed an increase in the flotation recovery of fine valuable minerals. Zinc (Zn) production was increased by between 1,100 and 1,500 t/a, while salable yearly silver (Ag) production was increased by 1,500 kg. The copper (Cu) grade in the final concentrate was increased by 1 percent, while reductions in its Zn and lead (Pb) grades were obtained. Moreover, considerable reduction in Zn deportment to the final tail was observed

Reaction kinetics for gold dissolution in acid thiourea solutions were investigated using formamidine disulfide as an oxidant. For the conditions studied it appears that a surface reaction limits the rate of gold dissolution. The rate was found to be first order with respect to thiourea concentration and half order with respect to the concentration of formamidine disulfide; the apparent activation energy for the reaction was 10.40 kcal/mol (43.40 kJ/mol); and the reaction rate was independent of pH for the pH range 1.0 to 3.0.

Four different types of fire suppression systems were evaluated in large-scale fire experiments to study the effect of air velocity on their effectiveness in extinguishing conveyor belt fires. The four different types of suppression system were water sprinkler, deluge-type water spray and two different dry chemical fire suppression systems. The large-scale fire tests were conducted with 1.8-m- (72-in.)-wide fire-resistant rubber belt that met the Mine Safety and Health Administration flame resistant requirement specified in Title 30, Code of Federal Regulations, Part 18, section 18.65 (also known as the 2G Flame Test). Each fire suppression system was tested at air velocities of 152 - 168 m (500–550 ft) per minute (mpm) and 411 - 457 mpm (1,350 - 1,500 fpm). Two tests were conducted at both air velocities for each fire suppression system. Both of the water-based fire suppression systems were able to suppress the fire to the point that a miner could extinguish it with a fire extinguisher. However, the amount of water needed to suppress the fire to the point where a miner could walk up to and extinguish any smoldering belting was greater than the current MSHA regulations required. MSHA regulations only require 10 minutes of water supply for a suppression system. In the test setup, neither of these systems would have suppressed the fire had the water been turned off to the system after 10 minutes. The dry chemical suppression systems produced mixed results. Dry chemical suppression system A did not suppress the conveyor belt fire at either air velocity. Dry chemical system B suppressed the fire at 152 mpm (500 fpm) air velocity, but produced mixed results at the higher air velocity. Details are presented on the large-scale fire test setup, arrangement of the fire suppression systems and conclusions regarding the effect of air velocity on fire suppression system design and performance.

Gold-bearing outcrops were discovered in 1862 by Union soldiers garrisoned at Needles, CA, near the northwest corner of the future State of Arizona. Sporadic mining began several years later, but engineered shaft sinking and mill construction were not adopted at Oatman and the neighboring camp of Goldroad until about 1900. Geological interpretations in 1915 enabled major production during the next decade. In the late-1930s, USSR&M built a new mine and mill in Gold Road and the District flourished until closure by Federal Order L-208 in October 1942. For about a decade, Oatman’s mines were among the largest gold producers in the American West.

In many mining operations, particularly northern open-pit mines, large seasonal variations in grinding slurry temperature can occur. It is shown in this paper that this temperature shift can cause a winter-time loss in autogenous grinding efficiency of as much as 20Y. if it is not controlled. Laboratory experimentation indicates that much of this effect is due to increases in slurry viscosity as the temperature is decreased. An additional effect may result from changes in rock strength upon freezing, for which possible mechanisms are discussed. However, no effect clearly resulting from rock strength changes has been definitely confirmed.

"The DC Clean Rivers (DCCR) Project is a 25 year, $2.6 Billion court ordered project to reduce combined sewer overflows (CSOs) discharging into the Potomac and Anacostia Rivers and Rock Creek, and also reduce flooding in the northern areas of the District of Columbia. Like all massive infrastructure programs, DCCR faced technical challenges, management challenges, risks of cost and schedule overruns, procurement challenges, and financing challenges. While DC Water is no stranger to large capital improvement projects (CIP), this CSO Program represents a once in a lifetime event for the Owner requiring the assembly of a single purpose team to implement. Some of the DCCR Project team’s most successful accomplishments to date are attributable to its introduction of early contractor involvement (ECI) to thoroughly vet the work from the perspective of the future constructors, development of functional baselines to share risks that are difficult to price and reduce reliance on geotechnical baseline interpretation during tunnel excavation, and attainment of financing through Municipal Century Green Bonds based on using design criteria of a minimum of 100 year life for the structures to establish intergenerational equity and fairness. This paper will discuss these innovations in detail and provide some sets of recommendations for the underground industry on when and how to use different approaches to better manage projects of this magnitude. INTRODUCTION The District of Columbia Water and Sewer Authority (DC Water) is an independent multi-jurisdictional authority that was established in 1996. Its primary mission is to provide water distribution services to more than 640,000 residents in the District of Columbia (District), and sewer treatment and disposal to 1.6 million residents of the District, Montgomery and Prince George’s counties in Maryland and Fairfax and Loudoun counties in Virginia."

When a company like Lonmin, the third largest platinum producer in the world, is mining one of the most precious metals in the world, time is money. Every piece of equipment is expected to be in top shape because every minute of downtime can add up to lost profit. Two different types of platinum ore are mined and processed by Lonmin, namely Merensky and UG2. Merensky has a grade of 3.5 g/t (0.1 oz/st) and UG2 has a grade of 4.5 g/t (0.13 oz/st). The mine also produces platinum group metals (PGM), which consist of platinum, osmium, palladium, rhodium, iridium and ruthenium. Although South Africa has more than 80 percent of the world?s PGM reserves, these minerals are very scarce. Lonmin is considered a low-cost producer of these minerals, generating 21.5 t (694,000 oz) of platinum and 18.6 t (600,000 oz) of PGMs in 2012. So when the company?s Karee K3 concentrator plant in Rustenburg, North West Province, South Africa, was experiencing high operating costs on the transfer points, a solution was needed.

Field data combined with theoretical derivations were used to design a surface mining operation using explosive casting. Calculated casting results varied from 44 to 65% depending on bench height. This model was then applied to field tests at an eastern, contour, coal stripping operation. Results from five experimental casting shots are summarized and compared to conventional mining methods. The overburden material removed by casting ranged from 35 to 65% with an average of 50%. Fragmentation was improved and it was demonstrated that this procedure is economically attractive with a resultant cost savings of $.59/m3 or $5.93/t of clean coal.

Generally speaking, erosion, as a process which entails degradation of surface layers of earth under the influence of the mechanical energy of surface waters, and from the point of view of biosphere sustainability, is a harmful process. This is absolutely true when agricultural and forest areas are concerned. However, if erosion is viewed selectively, this general definition about its harm raises serious questions. In other words, the erosion which takes place in the terrains where there is no vegetation because of the geological composition of the soil, does no harm. On the contrary, if the mineralogical composition of such terrains is characterized by a presence of precious and rare metals, erosion can be very useful. It contributes to the creation of new alluvial deposits of these metals in the waters in the plain land. It is here that torrents from hills come down and cause erosion. And finally, if there were no erosion, there would be no renewable deposits of gravel and sand.

The reliability of a set of flotation results is key to minimizing project risk, either at the commissioning of a concentrator or at the point of implementing an improvement to an existing concentrator operation. High-confidence flotation testing (HCFT) was developed for this purpose and is part of the list of technologies at various scales used by Xstrata Process Support (XPS) in addressing these projects. The methodology is based on two principles: one, to ensure that the ore sample is representative and has been well-blended and subsampled; two, to perform the flotation tests in sufficient numbers of replicates with appropriate quality controls, so as to improve the reproducibility of the test data. Across ten years of research on this methodology at Xstrata Process Support, Sudbury, several improvements to the HCFT method and complementary connections between sampling and quantitative mineralogy have been developed. Two case studies are reviewed. These show the capability of this system in both optimizing an existing concentrator operation and in serving a new mine scenario. Further work to improve the efficiency of this procedure has been identified. Key words: Flotation testing, Flotation
