El presente trabajo es un análisis del sistema de organización del sistema de seguridad a través del modelo Reason de causalidad o de "queso suizo", donde se identifica qué aspectos y decisiones se convierten en un factor condicionante para un accidente y también cómo la organización puede aprender de un accidente perfeccionando sus defensas en un ciclo de mejora continua. De forma que este modelo explica que un accidente es la consecuencia final de una superposición de fallas que va desde la última línea de defensa, pasando por el acto inseguro, la condición insegura, la falla de la supervisión, la falla de la asesoría (seguridad) hasta la falla de la organización. Para solucionar dicho problema, se realiza una gestión del error que incluye una serie de medidas preventivas como descubrir, evaluar y eliminar los factores que producen el error dentro de la fuerza laboral y así desarrollar la realidad intrínseca de la organización contra los errores humanos.

Anyone who has spent any time in and around the drilling and blasting profession knows that probably the most critical part is the drilling portion. A blasthole is nothing more than a cylindrical vehicle designed and strategically situated to hold and contain an explosive charge so that it can be detonated in the most efficient and optimum manner possible. It matters very little if the latest “State-of-the-Art” explosive or detonation system Is employed if the hole is poorly placed; i.e. excessive burden and spacing, sized incorrectly for desired results, insufficient in depth etc.. This is not to mention that the drilling phase is the most expensive, requiring sizeable initial outlays and upkeep. It’s also true that improper drilling can cause costs to skyrocket throughout the remainder of the work cycles such as excavating and hauling, crushing, screening and so on.

Sustainable development is currently an extremely relevant concept worldwide, especially in developing countries like South Africa. Mining and metals companies are also directly affected by the sustainable development agenda. The Mining Charter?s requirements as well as new legislation (Mineral and Petroleum Resources Development Act) expect, among others, for mines to support infrastructure and poverty eradication projects in the communities surrounding the mines. The aim of this paper is to, against the background of a qualitative study of mining communities in the North-West Province of South Africa, put forward some critical factors for the sustainability of social development projects in the mining sector.

Solvent extraction has recently been introduced into Australian hydrometallurgical practice for the production of a high grade uranium concentrate from El Sherana ore by a sulphuric acid leach and solvent extraction process. The solvent is a 5 per cent solution of tri(iso-octyl)amine in kerosene modified with 4 per cent of a long chain alcohol. The stripping reagent is a molar solution of sodium chloride.Preliminary testing for the development of the process showed that the leach liquor was quite amenable to the solvent extraction process and that the equilibrium and rate considerations of both the extraction and stripping steps were very favourable.The industrial plant contains four stages of extraction, three stages of chloride stripping and one stage of secondary carbonate stripping. An internal type of mixer-settler is used for the stagewise contact, the design being based on tests with model equipment.The performance and control of the industrial plant is satisfactory; 99·7 per cent of the uranium is recovered for a loss of 0·3 gallons of kerosene and 0· 4 pounds of amine per 1,000 gallons of liquor treated.INTRODUCTIONThe processing of El Sherana uranium ore from the South Alligator River valley commenced in April, 1959, and thereby introduced into Australian hydrometallurgical practice the unit operation of solvent extraction on an industrial scale. Although there are now some twelve mineral processing plants in the United States and Canada using solvent extraction, its application to the field of primary hydrometallurgy is quite new. Prior to 1956 none of these twelve plants was operating and hydrometallurgical application waslimited to the refining of comparatively high grade uranium compounds and fission products from nitrate and nitric acid solutions.

Libby, Montana is the site of a large vermiculite deposit that was mined between 1920 and 1990 to extract vermiculite for commercial applications such as insulation, gardening products, and construction materials. The Libby vermiculite deposit also contains amphibole minerals including tremolite, actinolite, richterite, and winchite. Historically, Libby mine workers experienced high exposures to amphibole structures, and, as a group, have experienced the health consequences of those occupational exposures. It has been suggested that Libby residents also have been and continue to be exposed to amphibole structures released during the vermiculite mining operations and therefore are at increased risk for disease. The Agency for Toxic Substance and Disease Registry (ATSDR) conducted two epidemiological-type studies of residents living in Libby and the surrounding areas to assess these risks. The Environmental Protection Agency (EPA) collected and analyzed exposure data in Libby and used those data to project risks of asbestos-associated disease for Libby residents. The EPA has placed the Libby Asbestos Site, which includes the mine and the town of Libby, on its National Priority List of hazardous waste sites in need of clean up. This article presents a review of the exposure studies conducted in Libby and an analysis of health risks based on the data collected in those studies. Libby mine workers have experienced elevated levels of asbestos-associated disease as a consequence of their occupational exposures to amphibole structures. Libby residents? exposures typically are substantially lower than mine workers? historical exposures, and the health risk projections for residents are, accordingly, substantially lower. © 2007 Published by Elsevier Inc. Flash Smelting Furnace (FSF) hot dust-laden is cooled in a waste heat boiler (WHB) and then dedusted in electrostatic precipitators (ESP’s) before being treated, together with the Peirce Smith Converters off-gases, in the sulfuric acid plants for SO2 removal. FSF dust is sulfated by SO2 and O2 in the WHB. Typically, 70% of the dust from the FSF drops in the WHB while the remainder is caught in the ESPs. The collected dust is recycled to the FSF for Cu recovery. Some smelters have developed hydrometallurgical treatments to recover Cu from the dust; impurities, such as As and Bi, are bled as leach plant residues. This paper presents a study (laboratory scale), of leaching the FSF dust, generated in Atlantic Copper Smelter using the weak acid bleed from the gas scrubbing/cooling section (GSCS) of sulfuric acid plants. Leaching was performed with 0.5 mol L-1 sulfuric acid concentration, 60ºC, and a solid/liquid ratio 1/3. After leaching, H2O2 was used for arsenic and iron oxidation. pH was adjusted for precipitation of arsenic and iron as scorodite or as scorodite and arsenical jarosite. The precipitation was carried out at 95ºC during 3-6 h. Liquids after precipitation are suitable for subsequent treatments of electrolysis. INTRODUCTION The copper smelter dusts are usually recirculated totally or partially to the flash furnace. This practice increases the amount of impurities feeding the furnace. Some authors have developed processes for treatment of these materials for copper and zinc recovery and for arsenic, antimony and bismuth inertization. The processes for the treatment of copper smelter dusts can be pyrometallurgical (Yu, 1987; Carter, Vance, Aldridge, Zaw, & Khoe, 1994; Fu, Jiang &Wang, 2000), hydrometallurgical, or mixed processes (Yin et al, 1992; Mulale, Mwema, & Mashala, 1999; Gorai, Jana, & Khan, 2002). Hydrometallurgical processes have been extensively studied and, in some cases, applied to an industrial scale. The most important chemical reagent proposed for leaching the dusts has been sulfuric acid (Roman-Moguel, Plascencia, Pérez, & Garcia, 1995; Gabb, & Evans, 1997; Larouche, 2001; Morales, Cruells, Roca, & Bergó, 2010; Cáceres, Cruells, Morales, & Roca, 2012; Ecometales, 2013; Yang, Fu, Liu, Chen, & Zhang, 2017). Processes developed in alkaline media, and in some cases with the presence of sulfide, have also been carried out (Gabler, & Jones, 1988; Morales, Hevia, & Cifuentes, 2009; Alguacil, Garcia-Diaz, Lopez, Rodriguez, & Alonso, 2013), as well as by using a bioleaching process (Massinaie, Oliazadeh, & Seyed Bagheri, 2006; Bakhtiari, Atashi, Zivdar, Seyedbagheri, & Fazaelipoor, 2011).

The present U.S. production of bromine is from inland brines located in Arkansas and Michigan. The most concentrated domestic brines (up to 5,000 ppm bromide)are situated in Arkansas. Less concentrated brines(about 2,500 ppm bromide) are located in Michigan. The amount of bromine sold or used in the United States came from three companies operating in those two states. Its value was $191 million. In 2004, the estimated U.S. bromine production was 222 kt (244,000 st). That compared with 216 kt (238,000st) produced in 2003.

A generalization of Gy?s model for the fundamental sampling error introduced the new ?parameter for the dependent selection of particles?, denoted as Cij. This allows for modeling deviations from the ideal situation where the selection of a pair of particles is composed of two independent selections. The generalized model potentially leads to more accurate variance estimates in the case of clustering of particles, differences in densities or sizes of the particles or repulsive inter-particle forces. A straightforward and practically applicable method is needed for the determination of this parameter for miscellaneous mixtures in industrial settings. In this contribution, the feasibility of using digital image analysis to determine this parameter Cij, will be demonstrated. Line transect sampling of a digital image was used to construct a transition probability matrix. A new algorithm to derive quantitative estimates for Cij will be presented and discussed. The applicability was verified by a photograph of zirconium silicate particles of sizes typical for industries dealing with pharmaceutical, food/feed, and environmental applications. Conditions affecting the practical applicability are identified and potential pitfalls will be discussed, including e.g. how a potential unrepresentative surface can affect the quality of the estimate of Cij.

To convert the railway-bridge across the Oude Maas between Dordrecht and Zwijndrecht into a vertical lifting bridge, a drastic adaptation of the existing piers is necessary. Extra piles have to be placed quite close to the present piers to bear the extra weight of lift-towers and bridge-sections. This article describes the special technology applied to drive steel tubular piles l800 mm. in diameter and 36 m. in length vibration-free into position. At the same time a method of pile-point-reinforcement has been applied to minimize the settlements.

Jointed rock slopes are generally stable, as there is no freedom of movement for blocks of rock. In an open- pit mining environment, blasting opens up space into which blocks of rock can fail, resulting in the common occurrence of slope failure. Prior to the blasting of a trim pattern, the immediate highwall is scaled, where loose rock material is removed by means of a mechanical scaler. As mining of the pit progresses, the height of the scaled highwall from the pit floor progressively increases. These highwalls are never scaled again for the remainder of the pit life, and time-dependant deterioration contributes to these highwalls becoming subject to rockfall and, sometimes, slope failure. This paper documents the development of a South African innovation, a remote-controlled mechanical highwall scaler, a world first for the mining industry. The scaler will enable open-pit highwalls to be scaled from crest to toe whenever the need arises. It will reduce the risk associated with highwalls and deliver on the Anglo American promise of producing safe, profitable platinum.

The recent 20 years witnessed a boom and bust in the nickel laterite industry. Most notoriously is the story of the first three Western Australian nickel laterite projects which all suffered major ramp-up issues. Many hard lessons were learned by the industry and it has been widely reported that the major contributing factors included insufficient testing as well as a limited understanding of the materials handling characteristics at the front-end stage of the flowsheet. This paper compares the current "race to production" in the REE industry to the laterite industry 20 years ago and addresses the fundamental question: ‘Have mining and exploration companies with rare earth projects learned from past lessons and are they doing sufficient and appropriate testwork to prevent failure’? The authors will provide the perspective of a commercial metallurgical laboratory that has gone through the laterite cycle and is active in the present REE cycle.

"The Island Copper plant was designed and built by Fluor Utah Ltd, in conjunction with Utah design staff, Originally designed to treat 33,000 tons/day (t/d) of ore, the current average milling capability is 38,000 t/d (Nov'74). The first of six semi-autogenous grinding lines was brought on line in October 1971, startup of the remainder being governed by equipment delivery schedules.The summer of 1973 saw the start-up of an additional three ball mills, and attainment of the present production level. The current process flowsheet is shown in simplified form.CRUSHINGThe crushing circuit is relatively simple, comprising single stage crushing of the ore followed by screening to fine and coarse fractions.The crusher is a 54 x 74in. Allis Chalmers Superior Gyratory with a setting of 9-lOin., driven by a 500-hp motor. Trucks dump on three sides of a rectangular dump pocket. An 84in. x 20ft. link belt apron feeder draws crushed ore from a 250-ton surge bin below the crusher, discharging onto a 72in. wide inclined steel cord conveyor belt. A fixed splitter box at the conveyor discharge separates feed to two parallel Jeffreys 6 x 20ft. vibrating grizzlies for screening into +4in. and -4in. fractions. The fractions are collected on short reversible conveyors which feed the coarse and fine ore stacker conveyors. Trippers distribute ore to outside stockpiles adjacent to, and above, the concentrator grinding 'floor."

Two of the most important issues mines have to deal with are tracking the ore mined from pit to plant and to test the efficiency of beneficiation equipment. Kumba Resources and Saco Systems are working together at moment to develop a RFID tracer system to help address these issues. This paper will focus on the tracking of ore from pit to plant. The main advantages for ore tracking is to ensure plant feed blending and dilution according to plan. The RFID tracer system works on the basis of a tracer that physically simulates the ore properties and radio frequency technology that facilitates communication with the tracer and allowing information to be stored on the tracer. This paper tends to indicate the progress on the application of RFID tracer technology in the minerals processing environment as tested at Kumba Resources so far. It is concluded that the system has great potential, but some important problems remain

Several deep Swedish cut-and-fill mines experience severe ground control problems, including roof collapse and wall sloughing, as a result of variable rock quality and high in situ stresses. A project to examine the problem included two main areas. The first was prediction of mining conditions at depth and the adaptation of cut-and-fill mining to such conditions; the second was the assessment and improvement of support methods at present and future depths. Simple, elastic, mine-scale numerical models were used to optimise stope sequencing and more detailed, stope-scale models to simulate the failure processes around a stope and the interaction of rock mass and support. Kristineberg was chosen for testing and calibration of the predictive methods because of its long history of severe ground conditions and the large base of observational and monitoring data available for comparison. Horizontal cut and fill was found technically suitable to depths beyond 1 000 m and economically viable if ore value covers support costs. Stability can be achieved with the present support technology down to 1 000 m but the 7-strand 15.2 mm cables have poor deformation properties (5% failure strain) compared with rebar rock bolts (17%). Reducing the intervals between levels from 100 m to 50 m will cut support costs and improve production reliability and capacity. Simulation of sill-pillar mining indicated that conditions would be most severe when pillar height was reduced to 15-20 m; mining of a sill pillar has been carried out successfully to the last 3 m, whose extraction is planned for the near future

Longwall management, regular shield maintenance, and evaluation of ground control problems can be performed using real-time shield monitoring program. Based on real-time shield pressure data, evaluation of intensity and interval of periodic weighting, estimation of time-dependent pressure increase and the effects of geologic anomalies can be obtained. In this study, full-face shield pressure data of over 457 m (1,500 feet) of face advancement is analyzed to evaluate shield response in the presence of surface lineaments, floor undulations, and multiple main roofs. Shield pressure increase due to web cutting is also correlated with setting pressure during periodic weighting and normal time. In addition, time-dependent pressure increase rate on Sundays and holidays is also correlated with time average pressure and the location of previous periodic weighting.

So good are things that from April 23 to 29 more than 500,000 visitors involved in those two industries attended the bauma 2007 Trade Show in Munich, Germany. Bauma is an international trade fair for construction and mining equipment that took place at the New Munich Trade Fair Center. It is hosted by Munich once every three years. This year was the most successful bauma ever and further strengthened the shows position as the world leader in the field.

Surface subsidence induced by underground coal mining activities, especially longwall mining operations, can cause various problems to interstate highways ranging from structural integrity, drivability to safety. Accurate subsidence prediction, correct influence assessments, and effective mitigation measures are the keys to ensure the safety of highway traffic. The surface subsidence prediction program, called comprehensive and integrated subsidence prediction model (CISPM), was developed based on the principle of influence function method and has been validated with a large number of subsidence measurements. In this paper, the techniques to use the predicted final surface movements and deformations for assessing their influence on the integrity and functionality of highway structures (e.g., road surface pavements, concrete slabs, transverse joints, etc.), vehicle dynamics, comfort level, and drivability are presented. A number of mitigation measures to protect the highway structures and to ensure traffic safety are proposed along with an application case.
