This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. The National Institute for Occupational Safety and Health (NIOSH) Pittsburgh Research Laboratory (PRL), RAG Pennsylvania and Strata Control Technologies of Australia collaborated in an intensive study of ground behavior, reinforcement performance, and stress redistribution at the Emerald Mine in Southwestern Pennsylvania. The study site was a longwall tailgate subjected to a severe horizontal stress concentration. Field measurements indicated that the stresses applied to the study site nearly doubled during longwall mining, resulting in roof deformations extending to a height of 4.8m (16 ft) above the entry. A computer simulation of the field site was conducted using FLAC-2D, incorporating a broad range of rock behaviors and failure mechanisms. Comparison between the measurements and the simulation showed that the model was able to capture the most significant aspects of the roof and support system behavior, particularly, the extensive slip along bedding that created a partially destressed "softened" zone in the immediate roof. The model also showed that supplementing the normal roof bolt support pattern with cable bolts would allow the entry to survive a further 20-25% increase in the applied horizontal stress. Such information could have very practical application to the design of roof support systems for coal mines.

"The Schlumberger configuration used in geoelectrical sounding is slightly adapted here to obtain reliable data for both shallow and deep penetration depths from the same survey. In this configuration, two kinds of current electrode half-spacings are used: the first enables construction of vertical electrical resistivity sounding (VES) curves for shallow depths (<50 m), and the second for depths =250 m. For a given VES location, two field curves are measured and interpreted using two standard approaches. Practical characteristics of such a modified configuration are illustrated with two field applications in Syria. The first aims to characterize the structure of Quaternary and recent deposits in the Al-Ghab depression region, and the second deals with exploration for phosphatic sedimentary units in the Al-Sharquieh mine. La configuration de Schlumberger normalement employée dans les levés géoélectriques a été légèrement modifiée afin d’obtenir des données fiables à faible et forte profondeur durant le même levé. Dans cette nouvelle configuration, deux types de demi-espacements des électrodes sont utilisés : le premier permet le développement de courbes de sondage de résistivité électrique verticales (REV) pour les faibles profondeurs (<50 m), et le second atteint des profondeurs =250 m. The backfilling of underground stopes is a common practice throughout the mining industry. The stresses imposed by the backfill on the surrounding rock mass need to be assessed because of their influence on ground stability and on the hydro-geochemical response of the fill material. There is, however, a complex interaction between the relatively soft backfill and the much stiffer adjacent rock mass. In recent years, the authors have presented results from investigations conducted on stress distributions (obtained numerically and analytically) in and around backfilled stopes with (sub)vertical walls. In this paper, the influence of stope inclination and of backfill properties is evaluated. Results indicate that, in general, the vertical and horizontal stresses in inclined stopes are close to, but somewhat smaller than the stresses in vertical stopes. Nonetheless, significant differences can be observed in some cases, especially for the vertical stress along the inclined central line and along the hanging wall. It is further shown that neglecting deformability in limit equilibrium analyses is an acceptable assumption. However, Poisson?s ratio and the dilatancy angle, usually neglected in limit equilibrium analyses, may have significant effect on the stress distribution. Results also confirm that the backfill strength parameters (cohesion and friction angle) are critical factors required to properly evaluate the stress distribution, at least within a certain range of parameter values.

The need to hoist increasing loads from great depths is leading to the more general adoption of winders of the Koepe and Blair types. The trend has introduced a number of new problems in winding ropes, more particularly in the Koepe system where regular destructive tests during the life of a rope are not possible. Consequently, the importance of introducing non-destructive tests is much greater, especially so in the case of Koepe ropes used for deep winding where there are three major destructive fatiguing influences. These are rotational effects when a vertical rope is changed end-for- end each trip, the much greater range of stress to which the rope is subjected each trip as compared with drum winding, and the destructive effect of a high coefficient of friction between wheel and rope. A plea for a lower coefficient in the interest of better rope and tread life, substantiated by analysis, is made for Koepe winders operating at depth.Co-ordination of rope testing with general hoisting problems is stressed and the active participation of the Wire Rope Committee of the Australian Mineral Industries Research Association Ltd. in the promotion ofthe project is noted.The general introduction of a rope testing programme in Australia, personnel requirements and some local basic research in the subject to cope with future demands are considered.

Heat and humidity buildup within refuge alternatives (RAs) may expose occupants to physiological hazards such as heat stress. The Mine Safety and Health Administration (MSHA) regulations require RAs in underground coal mines to provide a life-sustaining environment for miners trapped underground when escape is impossible. RAs are required to sustain life for 96 h while maintaining an apparent temperature (AT) below 95 °F (35 °C). The National Institute for Occupational Safety and Health (NIOSH) tested a 10-person tent-type RA, a 23-person tent-type RA, and a 6-person metal-type RA in its underground coal mine facilities to investigate the thermal environment over a 96-h period. The test results showed that mine air and mine strata temperatures surrounding an RA occupied by simulated miners (SMs) increased over the 96-h test period. The test results suggest that RA manufacturers should consider this increase in temperatures when calculating and evaluating RA components during surface and laboratory tests. The findings can equip stakeholders with additional considerations for calculating the interior heat and humidity temperature profiles for occupied RAs not tested in situ

Recruiting and retaining students in the explosives industry is becoming increasingly important each year. With no undergraduate degree in explosives engineering currently available in the United States, it is from other industries and degree disciplines we must recruit. The 2014 annual ISEE conference saw the first anonymous online survey distributed to members of SEE student chapters of the International Society of Explosives Engineers. The survey contained questions about future job and career plans, ideal job placement, expectations of salary and current academic degrees. The purpose of the survey was to determine what these students expect or require from a career in the explosives industry, and their current path to pursue this. This information can help steer recruitment activities and job offers for graduating students. The success for both student response rates and interest from the industry has allowed space for a yearly update of this survey. A second survey was distributed in 2015 and included international students. This paper provides the data from a third online survey distributed to all student members of the International Society of Explosives Engineers, globally. Understanding the main incentives for a student to accept a graduate position as an explosives engineer can help us, as an industry; position ourselves to remain competitive in recruiting against other major degree disciplines.

The Cliffs and Associates Limited (CAL) 500,000 tpy HBI plant located in Trinidad was commissioned after modifications in early 2001. This plant employs the novel Lurgi Circored® process to reduce iron ore fines using a combination of circulating fluidized bed and conventional fluidized bed reactors in a hydrogen based. This paper will review the advantages offered by working in the Caribbean nation of Trinidad and Tobago, the Circored® processes, and plant start-up experience.

DOMESTIC OUTPUT of white arsenic, As2O3, was derived entirely as a byproduct of smelting arsenic-containing copper ores by The Anaconda Company at Anaconda, Mont., and American Smelting and Refining Company at Tacoma, Wash. Arsenic metal was not produced. Consumption, and Uses.-Most of the white arsenic produced in 1964 was consumed in manufacturing lead and calcium arsenate insecticides. In the 12 year period since 1951 the production of lead arsenate insecticide has varied between 7,800 tons and 4,900 tons, indicating that lead arsenate has maintained its position in competition with organic insecticides.

The geotechnical construction industry generally considers soil mixing to be defined as an in situ ground improvement or treatment method by which a dry or wet reagent is introduced into the ground and is blended with the soil by mechanical or rotary mixing tools. This results in relatively homogenous mixtures whose engineering properties are improved and are greater than the soil properties before mixing. Reagents usually include cement, but may also include processed clay such as bentonite, when a reduction in hydraulic conductivity is desired, or other proprietary chemicals when chemical stabilization is the objective. Mixing may be either shallow or deep. Shallow Mixing is the term arbitrarily used for treatment of soils within the upper 30 feet; Deep Mixing denotes treatment below 30 feet to the depth limits of the equipment. Mixing equipment may consist of single or multiple-shaft tools having cutting and mixing blades of many different configurations. To avoid using proprietary nomenclature, reference to Shallow Mixing Methods (SMM) and Deep Mixing Methods (DMM) are the preferred terms. Although originally developed and patented in the United States in 1956, in situ construction using various soil mixing methods is more prevalent in Japan and the Scandinavian countries. Reintroduced into the United States from Japan in 1986 for ground improvement and liquefaction mitigation at Jackson Lake Dam, Wyoming, acceptance of soil mixing methods has been gradual but slow. "Circulating fluidized-bed (CFB) combustion technology has been around for over 40 years, but over the last 6 years it has been commercially demonstrated at the 500 MWe scale at the Lagisza plant located in Bedzin, Poland. The CFB at the Lagisza plant has unique first-of-a-kind design features, such as vertical-tube supercritical steam technology and a lowtemperature flue-gas heat extraction that allows the plant to achieve high plant efficiencies of over 43% (net lower heating value). Another unusual feature for a coal power plant is that this plant meets all its atmospheric emission permit levels without any post-combustion de-NOx or de-SOx equipment such as selective catalytic reduction (SCR) or flue-gas desulphurization (FGD).CFB clean coal power technology is entering the utility power sector just in time to help deal with declining quality in internationally traded coals and to promote the large-scale use of economic, low-quality domestic fuels. Owing to the very attractive price discounts, growing supplies of low-quality Indonesian coals are outpacing the supply of high-quality Australian, Russian, and US coals. In Germany and Turkey, the use of domestic lignites for power production provides a secure and economic energy solution while creating domestic jobs.Conventional pulverized coal (PC) boilers will have trouble accepting these off-specification coals because of their narrow fuel specifications; they typically call for heating values above 5500 kcal/kg. This limitation is not an issue for CFB technology because of its ability to burn the worst and best of coals with heating values ranging from 3900 to 8000 kcal/kg.This paper provides an outlook for future coal supply, quality, and price, as well as a review of the technical and economic benefits of CFB technology firing low-quality fuels for utility power generation."

Material handling in open pit mining accounts for about 50% of production costs. The selection and deployment of efficient, safe, and economic loading and haulage systems is thus critical to the production process. The problems of truck and shovel selection and sizing include determination of the optimal number and capacities of haulage and loading units, as well as their allocation and operational strategies. Critical survey and analysis of the literature has shown that deterministic, stochastic, and experimental approaches to these problems result in considerably different outputs. This paper presents a comprehensive simulation framework for the problem of truck and shovel selection and sizing based on the random processes underlying the network-continuous-discrete event nature of the mining operation. The framework builds on previous research in this field and attempts to address limitations of available methodologies in the form of a comprehensive algorithm. To test the validity of the framework a large open pit mine was evaluated. The stochastic processes governing the uncertainties underlying the material loading and haulage input variables were defined and built into the stochastic model. Discrete event simulation was used to simulate the stochastic model. The proposed model resulted in several modifications to the case study.

Esso-BHP have installed a production supervisory control system that is presently controlling the offshore oil and gas facilities on the Barracouta and Marlin platforms from an onshore master station. This control is achieved with a computerized master station onshore which uses a UHF radio system for telemetry and control.The system is designed to control, monitor, and gather data from a large number of oil and gas production platforms and has flexibility to handle future variations in operating conditions.The system will be extended to the Halibut platform late in 1970 and to the Kirlgfish platforms during 1971.

Preliminary laboratory experiments have been conducted by the Bureau of Mines to define the influence of bit geometry on methane ignitions as a step toward establishing safer mining-machine operating parameters. The experimental system was designed to simulate the operation of a continuous mining machine, using a drum-type cutterhead of normal diameter with a narrow cross section, permitting full-diameter drum cutting tests to be run on material representative of sandstone lenses (or inclusions) in coal seams. Two conical bit types were tested at shallow depths of cut with three drum speeds. Bits that rotated in the mounting block during testing did not produce ignitions. The ignition frequency for used bits was three to four times greater than for new bits at the same kerf depth.

Libby, Montana is the site of a large vermiculite deposit that was mined between 1920 and 1990 to extract vermiculite for commercial applications such as insulation, gardening products, and construction materials. The Libby vermiculite deposit also contains amphibole minerals including tremolite, actinolite, richterite, and winchite. Historically, Libby mine workers experienced high exposures to amphibole structures, and, as a group, have experienced the health consequences of those occupational exposures. It has been suggested that Libby residents also have been and continue to be exposed to amphibole structures released during the vermiculite mining operations and therefore are at increased risk for disease. The Agency for Toxic Substance and Disease Registry (ATSDR) conducted two epidemiological-type studies of residents living in Libby and the surrounding areas to assess these risks. The Environmental Protection Agency (EPA) collected and analyzed exposure data in Libby and used those data to project risks of asbestos-associated disease for Libby residents. The EPA has placed the Libby Asbestos Site, which includes the mine and the town of Libby, on its National Priority List of hazardous waste sites in need of clean up. This article presents a review of the exposure studies conducted in Libby and an analysis of health risks based on the data collected in those studies. Libby mine workers have experienced elevated levels of asbestos-associated disease as a consequence of their occupational exposures to amphibole structures. Libby residents? exposures typically are substantially lower than mine workers? historical exposures, and the health risk projections for residents are, accordingly, substantially lower. © 2007 Published by Elsevier Inc.
