THE ANTHRACITE region of Pennsylvania contains over nine-tenths of the total anthracite deposits of North America and produces all but an insignificant proportion of the total output of anthracite in the Nation. The whole economy of the northeastern section of the United States depends, to a large extent, on the mining of Pennsylvania anthracite, which is shipped to New Jersey, New York, and New England and is a principal source of energy and heat for that entire region. The responsibility of the Federal Government for developing an engineering method that will prevent inundation of the Pennsylvania anthracite mines and keep them in operation has long been recognized. To obtain a reliable picture of the anthracite-mine-water problem and to solve that problem, it is necessary to have factual data on the volume and the quality and character of the mine water handled by the pumping plants in the anthracite region, as well as the kinds of pumps utilized and problems concerning their operation.

HOLDING the annual dinner-dance of the Institute at the Waldorf-Astoria had become such a tradition that there was widespread regret when it became known that the demolition of the building to make way for a huge office structure would perforce bring to an end the long and enjoyable series of gatherings our Institute had held there. This year the . Commodore was the place at which it was held and the commuters on the New York Central and the New Haven railroad systems, at least, found it a more convenient place for them. Except for the famous "Hoover" dinner, which will probably set a mark for all time, it was the largest dinner the Institute held, 1000 being present, and it taxed the facilities of the hotel to the utmost.

"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

WHAT is written here features some of the things that I would say if I were to de- liver a Presidential address during the Annual Meeting to be held this month in New York. I am aware that custom favors written rather than vocal delivery of such a discourse; and, as this accords exactly with my own preference, I assure readers that they will never be called upon to hear these remarks spoken- anyhow, not by me. My year as President is drawing to a close. As I look back I like to classify and analyze the impressions that have been left with me. It has been a delightful and fascinating experience to me personally; I have appreciated the courtesies and aids, and have been deeply gratified at the honors accorded me. Perhaps the source of greatest satisfaction is the opportunity I have had to sense more about the provinces of this Institute and about the useful functions of the profession that it represents. For 38 years I have been a member. I served two terms as Director-one in 1914-'16 and again in 1927-'29. I have been honored by selection as a Saunders Medalist. I thought I was reasonably familiar with the functions, the ideals, and the activities of the Institute. But the things that I have discerned during the past year have dispelled that conceit. I still think that, a year ago, I was as well informed as most members; but since then I have had the rare privilege of executive service and insight. That has given me a perspective that I feel can be obtained in no other way-as I think the illustrious engineers who have preceded me in office will agree. I wish that I had the power to translate to every member somewhat of the feelings of pride in our organization that a year as President cannot help but instill.

Application of integrally bladed disks (blisks) is of increasing interest in compressors of aero engines. Blisks are preferred due to their potential for size and weight reduction, for increased performance and for reduced manufacturing costs compared to conventionally bladed disks. Erosion of the blisk aerofoils in service reduces engine performance, and can result in short shop visit cycles. This significantly increases the life cycle cost of these components, particular when considering that eroded aerofoils can't be exchanged easily for blisks during refurbishment. Shop visit cycle times can be extended and maintenance costs can be reduced through the application of an effective protective coating. The ongoing development of an erosion resistant coating and deposition process suitable for coating complex blisk geometries is described here. The coating is a multilayered W /TiB2 system exhibiting superior erosion resistance compared to known single- and other multilayer concepts. A modified sputter process is used to enable coating even of blisk geometries with shadowed surface to competitive recurring cost.

Reclamation costs were estimated for 20 coal surface mining operations in the Appalachian and Midwestern Coal Supply Districts (these two districts were divided into three multi-State regions). The cost data were obtained through onsite observations, interviews with mining company personnel, and the authors' cost estimating procedures. The cost data are presented in terms of dollars per acre, dollars per ton of coal produced, and dollars per cubic yard of overburden moved during backfilling. The reclamation process is divided-into three cost categories: Premining planning, backfilling, and revegetation. The average total reclamation costs for the three regions are $2.77 per ton of coal produced and $8,168 per acre. As the result of comparisons of reclamation costs by region, by mining method, by slope, and by size of operation, the most important reclamation cost difference is between area mining methods ($1.73 per ton) and contour mining methods ($3.81 per ton). The Federal Bureau of Mines conducted this study to determine as accurately as possible the actual costs involved with the reclamation of coal surface mined land done in conjunction with active mining operations.

Cooperation between the United States Bureau of Mines and the Safety in Mines Research Board of Great Britain, continuous, since 1924, s made possible this and other papers on safety subjects. Grateful acknowledgment is made to representatives of the board for their assistance in arranging visits to several mine-safety stations, and to F. R. Wynne, Deputy Chief Inspector of Mines, for arranging visits to mines in Great Britain and certain other countries in Europe. During the summer of 1927 the writer had the privilege of visiting the mine-safety testing stations An Great Britain, Belgium, Germany, and France, in the order named. All of these countries have large coal mines, many of which are rated as gassy. Therefore, a the installation of electrical equipment is contemplated each of these countries is confronted with the same safety problem as is the United States that is, the development of electrical equipment that will not, ignite gassy atmospheres should they through neglect or accident surround the equipment. The means employed by these countries in safeguarding gassy mines should therefore be of general interest to safety engineers of American coal mines, and a brief survey is given for each of the four countries mentioned. The fourth or last of these surveys covers conditions in France.

The Bureau of Mines conducted a study of current trends in methods employed by the coal industry to dispose of acid mine drainage (AMD) treatment sludge. Thirty-three AMD treatment plants, operated by 11 Pennsylvania coal companies, were randomly surveyed. These plants provided data for calculating sludge volumes, as well as information on sludge settling, transportation, and disposal. Onsite visits allowed observation of problems associated with the various methods of sludge disposal: (1) disposal into a deep mine, (2) permanent retention in ponds, (3) disposal at an active coal refuse area, and (4) onsite burial. Method 1 is the most frequently used and the most environmentally sound. It is apparent, based on this representative sampling of AMD treatment plants, that the magnitude of sludge production is such that an aggressive and directed effort in reduction of sludge volume is seriously needed. The areas of major concern are (1) the amount of surface land required for sludge storage and disposal, (2) reclamation of these disturbed surface areas, and (3) effective and efficient means of removing, transporting, and disposing of AMD sludge. In this report, an engineering approach to the practical problems associated with the existing disposal methods is presented, together with recommendations for future research.

AS every school boy knows, the Andes mountain range forms the backbone of South America, extending the full length of the continent along its western edge and fairly close to the Pacific coast. But in Colombia these mountains are divided into three distinct ranges known as the Western, Central and Eastern ranges of the Andes. United at the southern end, they spread out like three fingers in a northerly direction across the Republic of Colombia. These great natural barriers have only in recent years been tarried by railroads or wagon roads, and they have had their effect upon the history of the country, both political and ethnological. In 2006, McNally International Inc., in a joint venture partner-ship with Aecon Constructors, was the successful bidder on two York Region Tunnel Projects: the Bathurst Collector/Langstaff Trunk Sewers and the York Durham Sewer System (YDSS) Interceptor Sewer 19th Avenue, both in the Toronto area. These projects comprised a total of 13 km (8 miles) of 2.74 m (9 ft) finished diameter tunnels and are both part of the YDSS. Both contracts required the use of earth pressure balance (EPB) tunnel boring machines (TBM) with a precast concrete segmental liner.

Bupyung Silver Mines daily treats average 14 tons of concentrates containing 9,000 g Ag per ton. Bupyung Silver Refinery uses Merrill Crowe process to recover the silver from the leach solution containing 1.0 g/l Ag; Series of experiments were carried out to recover the silver from the solution by means of direct electrowinning in place of Merrill Crowe process. The recovery of the silver was 99% when the cell voltage and the retention time were 1.9 v and 3.0 hours, respectively. Power consumption was 3.2 Kwh/kg of Ag. It was found that impurity buildup was not significant during the recycling. The direct operating cost for the electrowinning was evaluated to be $ 0.35/kg of Ag.

What is your assessment of the status of the mining industry as we see some evidence of an economic recovery? I am optimistic about the short and midterm prospects for the industry and, hence, for the professionals in the industry that constitute the majority of the SME membership. All who will read this interview recognize that the quality of life we enjoy today is largely a result of our achievements in the mining and minerals processing sectors. I believe that the sustainability of that quality of life and the aspirations of other sectors of the global community to attain that same lifestyle will significantly depend on the continued successes of these industries. Looking at some of the traditional economic indicators, we see predictions of continued increases in gold prices during the next 12 months as a new round of quantitative easing keeps real interest rates low and drives excess capital flows from countries with trade surpluses - who have traditionally parked their money in U.S. government bonds - into other investable assets, such as gold. On the fuel side of the industry, we have during the last decade seen a steady rise in the cost of coal, and some of the recent spot prices have seen dramatic increases. Some suggest that coal prices may rise sooner than anyone expects, as global reserves are revised downward. This is based on the hypothesis that some of the main coal-producing countries are overestimating supplies and that coal prices could start rising far sooner than anyone thinks - with obvious serious implications for energy policy. Those forecasters note that, for the past couple of decades, global supply has been dropping at a faster rate than consumption.

The beneficiation of heavy metal oxides, such as iron ore, is usually performed with heavy medium separation in cyclones, static baths and lately with Larcodems. Atomized ferrosilicon is most suitable as a heavy medium at densities above 3600 kg/m3 whereas a mixture of milled and atomized ferrosilicon is suitable at densities below 3600 kg/m3. When operating a beneficiation plant at densities higher than 3600 kg/m3 the quality of the ferrosilicon particles is an important factor that influences the control of the beneficiation process. Therefore, it is important to know how the rheology would be affected by the ferrosilicon quality as it also influences the control of the process at these elevated densities. Background The Kumba Resources Sishen Iron Ore Mine in South Africa produces 26.5 Mt iron ore per annum from its beneficiation plant. The ore body consists mainly of laminated and massive type hematite ore that is crushed down to ?90mm before being beneficiated by means of a combination of Wemco drums, Larcodem and dense medium cyclone separators. The need for higher densities When the percentage of high-density gangue2 is higher than normal in the run-of-mine (ROM), a higher separation density is necessary to produce a product to specification. Certain ore types of this quality in the Sishen reserves require a separating density higher than 4000 kg/m3, whereas densities would normally average 3600 kg/m3 when beneficiating ores of normal high-density gangue content. Sink and float tests conducted on the ROM material at Sishen revealed some interesting characteristics. In the waste fraction, (density < 3600 kg/m3) it was found that the %Fe was <21% (Fig. 1). Similarly, for the product fraction (density >3000 kg/m3), the percentage Fe varied between 35% and 67.2%. The bulk of the material (88%) actually had a density greater than 4000 kg/m3 and a Fe content that varied between 63% and 67.2%. At a density interval of 3600 to 4000 kg/m3, the Fe content varied from 37% to 60% with an average of 45%.

Flotation of rare earths is complicated by the number of mineral classes that carry rare earth elements, including carbonates, phosphates, and silicates that often co-occur within the same deposit. Gangue constituents often belong to the same mineral classes as the rare earth minerals themselves. Chemical regimes developed for specific separations employ customized combinations of collectors, modifiers, and depressants that are not directly translatable to other operations. In this rougher flotation case study, a reagent suite including phosphate ester and sulfosuccinamate, originally developed for the recovery of bastnasite and monazite from silicate hosted ore, could not be applied to a synchysite ore hosted in carbonate gangue because the suite was aimed at silicate gangue rejection only. The separation of a fluorocarbonate mineral from carbonate gangue required a different chemical regime involving the use of hydroxamate and sodium silicate. Maximizing rare earth recovery was complicated by the presence of slimes. The effect of desliming and the increase in grind size was explored.

The Mississippi embayment east of the Mississippi River is part of a vast geologic and hydrologic province. The abundance of water available is largely responsible for the agricultural history of the area and is perhaps the key factor in the rapidly accelerating trend toward industrialization. The multiple artesian aquifer systems that underlie the region and the alluvial aquifer along the Mississippi River are virtually undeveloped sources of fresh water in that they are yielding only a fraction of the water available. The potential yield of the 12 aquifer systems is estimated to be about 25,000 mgd (million gpd) of which only about 600 mgd is presently being withdrawn. The amount of water in storage in these systems is about 1600 trillion gal. Ground water having a dissolved-solids content of less than 500 ppm is generally available at depths of less than 1000 ft, and water having a dissolved-solids content of less than 1000 ppm is available in some places to depths of more than 3000 ft. Iron is the most common troublesome chemical constituent in the ground water. Water in varying amounts is also available from streams within the region. The amount of water which originates within the region and which leaves it as streamflow during a year averages about 50 million acre-ft (about 45,000 mgd). An additional 375 million acre-ft (about 340,000 mgd), which originated outside the region, leaves the region as streamflow during an average year. Some streams have considerable flow during periods of no precipitation and the water during these periods generally has a low dissolved-solids content. Large quantities of water from streams and waterbearing sands (aquifers) are available in the Mississippi embayment east of the Mississippi River. Most of the region is underlain by aquifer systems that will yield large quantities of water to wells, making ground water the most readily available source of fresh water. About 90% of the water used in the region is obtained from wells. Streams have been used primarily to dispose of, and dilute, wastes that have been dumped into them. The Mississippi, Ohio and Tennessee rivers, and the lower parts of the Tombigee and Yazoo rivers also serve as a means of transportation. This nominally water-rich region receives from 48 to 56 in. of precipitation in average years and is crossed by several large rivers. The largest, the Mississippi River, is the western boundary of the region discussed in this paper. The economy of the region is basically agricultural, but industry has become increasingly important since World War 11. The diversification and mechanization of agriculture have resulted in a surplus of manpower, most of which is being absorbed by the expansion of industry. Further industrial expansion probably will occur because of the available manpower, natural resources and economical transportation. Increasing demands on available water supplies will be made and the future economic growth of the region will be largely dependent upon wise utilization and management of the water resources.

One of the major problems in the exploitation of deep copper ore deposits in the Legnicko-Glogowski Copper Field is ground control and, in particular, roof control of the headings. The depth of copper deposits ranges from 600 to 1200 m. The parameters of ore-bearing rocks and the surrounding rocks show considerable variance of strength properties along the mining thickness (Fig. 1). The roof of the openings exploitation headings consists of dolomite-calciferous rocks. The roof strata consist of layers, the thickness of which vary from several centimeters to 0.6 m (sometimes to 1.0 m). These strata differ in strength and are characterized by igneous and sedimentary discontinuities. On the basis of studies in structure, properties and geological mining conditions of the roof strata in Polish copper mines, the method of roof classification has been developed. (1) This method uses the stability index "L" of the roof, calculated from [ ] The value of "t" ranges from 0, l to 0,8. Based on the value of "L", four classes of the roof (Table 1) corresponding to different degrees of fall hazard have been developed. The mining of copper ore deposit is carried out by a single lift room-and-pillar mining system or a two-stage room-and-pillar system with roof caving. (3, 4). The single lift room-and-pillar mining system is most often used. It consists in working of the deposit of 2.5 to 4.5 m mining thickness, 150-350 m long mining line, consisting of parallel room faces 6 m wide with pillars 5.7 or 16 m wide. As the mining line advances, cross-cuts joining individual rooms are made, thus forming support pillars 3x5, 7x7, 5x16, and 7x16 m (Fig. 2). The two-stage system (Fig. 3) mines the deposit with rooms 6 m wide into blocks 25 x 35 x 25 x 45 m. During the second step these pillars are worked with rooms and cross-cuts into pillars about 3 x 4 to 4 x 5 m (retreat mining). The pillars are crushed just before the fall of the roof. The maximum opening of workings in the two-step system is about 10 m, i.e. one belt entry and one row of bearing pillars. In single lift room-and-pillar systems it is about 35 m., i.e. three belt entries and three rows of bearing pillars. The support of the headings consists of expansion bolts 1.6-1.8 m installed in a bolting pattern 1.1 x 1.5 m. In both systems the fall step is about 10 m, i.e. the width of a belt and a bearing pillar. The Indications of Rock Mass Pressure Individual Dog Heading Individual dog headings are set wide apart from each other in the unmined ore deposit area where there is no interaction of headings. The indication of rock mass pressure are, in such cases, limited to the deformations and damage of side walls. Radiometric studies of rock mass loosening showed that the maximum range of lifts in side-walls immediately after web was 2 m (4,5). In the course of time the cracked side rocks fall off, thus widening the heading. The morphology of side-walls is influenced by the location of the heading in relation to clump layer/tarry. "Rare earth resources usually contain minor amounts of radioactive elements. The most widely recognised sources of radioactivity are associated with thorium and uranium. However, also present in the resources are isotopes of rare earth elements (REE) that are themselves radioactive, including lanthanum (La-138), lutetium (Lu-176) and samarium (Sm-147). These radioisotopes, as well as remnants of the natural thorium (Th-232) and uranium (U-238, U-235) decay series, can contribute to significant levels of radioactivity in mixed REE concentrates and in separated products. Levels such as these can present challenges for transport, transboundary movement, product storage, workers’ health and safety, as well as waste management issues at the facilities that separate out the individual REE’s. These are discussed in this paper. A particularly challenging radioactive isotope is Ac-227 (a decay product in the U-235 decay series). Actinium behaves chemically similar to lanthanides and hence normally concentrates with REE multi-element composites. Reliable analysis of Ac-227 is not widely available and the methodology required to deal with this challenge is briefly described. A principle reason for minimization of radioactive content throughout REE processing is restrictive specifications by concentrate purchasers in order to reduce waste management challenges. Approaches to the minimization of radioisotope content in REE concentrates and separated products are outlined in this paper."
